系统性红斑狼疮（SLE）是一种系统性自身免疫疾病, 临床异质性强, 以多器官不同程度受累、病情反复复 发与缓解、体内存在大量自身抗体为主要特点。寻找安全有效的治疗药物和治疗方法来延缓疾病进展, 降低其发病 率和病死率是当前临床关注的重点。羟氯喹是氯喹的羟基化类似物, 治疗 SLE 效果较为显著。HCQ 可通过抑制 Toll 样受体信号传导、阻断抗原呈递、抑制细胞自噬、抑制钙动员等多种途径发挥对 SLE 的治疗作用。HCQ 作为 SLE 的基础用药, 常与糖皮质激素、免疫抑制剂、生物制剂联合应用, 对改善临床症状、延缓疾病进展、改善 SLE 相关 并发症具有显著效果。探讨 HCQ 治疗 SLE 的作用机制及疗效, 能够为临床治疗 SLE 提供更充分的理论依据, 并扩 大 HCQ 的临床应用。 [ABSTRACT FROM AUTHOR]

目的: 探讨超声引导腹横肌平面阻滞 (transversus abdominis plane block, TAPB) 对下腹部切口疝修补术术后疼痛的 影响。方法: 选择择期全身麻醉行腹部切口疝无张力修补术患者60例, 采用随机法分为对照组 (C组) 和TAPB组, 每组30例. C组行常规全麻; TAPB组全麻后行超声引导下TAPB。以视觉模拟评分法 (visual analogue scale, VAS) 评估术后2 h, 4 h, 6 h, 24 h, 90 d的疼痛评分; 记录术前, 术中, 术后患者的平均动脉压 (mean arterial pressure, MAP) 和心率 (heart rate, HR), 术中芬太尼和 瑞芬太尼的使用量, 术后恶心, 呕吐的发生率, 抽血检测术前和术后24 h血清皮质醇 (cortisol, Cor), 去甲肾上腺素 (norepineph⁃ rine, NE), 白介素 (interleukin, IL) ⁃6水平。结果: TAPB组患者术后2 h, 6 h, 24 h, 90 d的VAS评分, 术中, 术后MAP, 术中芬太尼 使用量及术后24 h血清Cor, NE, IL⁃6水平均显著低于C组 (P < 0.05) 。结论: TAPB可为下腹部切口疝修补术患者提供良好的 术后镇痛, 减轻炎症和应激反应.
