Background: External ventricular drainage (EVD) is an emergency process intended to reduce intracranial hypertension resulting from the obstruction of cerebrospinal fluid (CSF) flow. This creates a temporary situation to extract CSF that cannot pass through normally. Knowing the surface anatomy for EVD implantation is important to prevent its inadvertent complications. The external landmarks have been designed in this anatomic study to review the classical landmarks and come up with new landmarks to improve this simple but lifesaving procedure. Methods: From November 1998 to October 2012, we implanted 439 EVDs. Results: In the first years, we employed usual landmarks to implant 97 EVDs. Since 2002, we used modified anatomical landmarks to implant 342 EVDs directly in the third ventricle. Conclusion: Using effective landmarks for EVD implementation allows the catheter to be inserted in the third ventricle. In addition, it permits more precise accuracy to ensure a safer procedure with fewer complications. [ABSTRACT FROM AUTHOR]

The Bacille Calmette-Guérin (BCG)-induced osteomyelitis is an extremely rare systemic adverse reaction after BCG vaccination in immunocompetent children and the correct diagnosis is frequently missed. We present 4 clinical cases of BCG-induced osteomyelitis reported over a 10-year period in a high-TB incidence country Lithuania. - Abstract
Purpose: Diabetic retinopathy (DR) is characterized by dopaminergic neuron loss in the retina of the eyes. [
18 F]fluoropropyl-(+)-dihydrotetrabenazine ([18 F]FP-(+)-DTBZ) positron emission tomography (PET) has been shown to detect dopaminergic neuron loss. The study is to investigate the feasibility of PET imaging with [18 F]FP-(+)-DTBZ for early diagnosis of diabetic retinopathy (DR) in diabetes mellitus (DM) rat models., Methods: The DM rat model was established by a single intraperitoneal injection of streptozotocin (STZ) (65 mg/kg). After 4 weeks, 8 weeks, and 12 weeks of STZ injection, the retinas of the rats were evaluated by electroretinogram (ERG), color fundus photography (CFP), fundus fluorescein angiography (FFA), and small animal PET scan with [18 F]FP-(+)-DTBZ by targeting vesicular monoamine transporter 2 (VMAT2). [18 F]FP-(+)-DTBZ uptake in retina was quantified as standardized uptake value (SUV). Immunofluorescence staining and Western blot were also performed to confirm the expression level of VMAT2 in retina., Results: ERG dysfunction was observed at 8 weeks in STZ-diabetic rats, evidenced by smaller amplitudes of oscillatory potentials (OPs) when compared with OPs in normal rats. CFP and FFA showed no significant difference in vascular leakage and neovascularization between STZ-diabetic retinas and normal ones until 8 weeks. PET imaging revealed that the SUV of [18 F]FP-(+)-DTBZ was significantly lower in the STZ-diabetic retinas compared with the normal ones as early as of week 4. The results from immunofluorescence staining and Western blots confirmed the early findings in PET imaging studies., Conclusions: Early DR can be non-invasively detected with PET imaging using [18 F]FP-(+)-DTBZ targeting VMAT2. The expression level of VMAT2 in retina may act as a new biomarker for early DR diagnosis.- Published

Introduction: Intrapleural fibrinolytic enzyme therapy is a potentially surgery-sparing treatment for poorly resolving parapneumonic effusion and empyema. It is safe in the majority of patients, however the most significant risk associated with this treatment is severe bleeding secondary to pleural hemorrhage. Contraindications for intrapleural enzyme therapy are not widely agreed upon and little is known about how to treat this difficult and potentially lethal hemorrhagic complication.Case Presentation: An independent 82-year-old Caucasian man presented to hospital with an empyema complicating community-acquired pneumonia and coincidental pulmonary embolus. He was initially commenced on intravenous antibiotics, pleural drainage and anticoagulation, however failed to improve significantly and was commenced on intrapleural fibrinolytic enzyme therapy. Shortly after, he suffered severe pleural hemorrhage that was uncontrollable despite emergency thoracotomy and washout. Subsequent hemostasis was achieved after re-exploration and application of topical fibrin-thrombin sealant spray. The patient survived and was discharged home.Conclusions: Intrapleural enzyme therapy can be effective in loculated parapneumonic effusion and empyema, but massive pleural hemorrhage can complicate its use. Pleural hemorrhage appears to be associated with anticoagulation or coagulopathy, and can be difficult to manage. This case adds to the body of data on bleeding complications following intrapleural enzyme therapy, and to the best of our knowledge is the first report of fibrin-thrombin sealant use in this setting. Objectives: No study on the characteristics of injury of neural tracts following external ventricular drainage (EVD) in a large number of consecutive patients following EVD has been reported. In this study, using diffusion tensor tractography (DTT), we attempted to investigate the characteristics of injury of neural tracts associated with EVD using a frontal approach in stroke patients.Methods: We recruited 43 consecutive hemorrhagic stroke patients with a history of EVD using a frontal approach. Five neural tracts were reconstructed [corticospinal-tract (CST), corticoreticular-pathway (CRP), arcuate-fasciculus (AF), cingulum, and superior-longitudinal-fasciculus (SLF)].Results: Among five neural tracts, neural injury by EVD was observed on only two neural tracts (the CRP and the cingulum): CRP—seven (16.3%, five patients—partial tearing and two patients—complete discontinuation) of 43 patients and cingulum—eight (18.6%, eight patients—complete discontinuation of the anterior portion of the cingulum) of 43 patients.Conclusions: It appears that neural injury occurred in a considerable number of patients who underwent EVD; therefore, conduct of further studies on measures to prevent or minimize neural injury by EVD should be encouraged. Background: Empyema thoracis in children causes significant morbidity. Standard treatment of Empyema thoracis includes tube drainage and antibiotics. But the tube drainage often fails. Intrapleural Streptokinase has been used in empyema thoracis with good success rate. Objectives: We evaluated the efficacy of intra-pleural Streptokinase in management of empyema thoracis even in advanced stages. Patients and Methods: A total of 28 patients with empyema thoracis requiring intercostal tube drainage aged zero to twelve years were included in the study who were admitted in Pediatric intensive care unit. 15,000 units/kg of Streptokinase was instilled into the pleural cavity. Response was assessed by clinical outcome, after unclamping and subsequent chest radiography and serial chest ultrasounds. Results: Streptokinase enhanced drainage in all patients with complete resolution of empyema thoracis in 26 patients. Two patients were referred for surgery. Only 7.2% required surgery. Streptokinase was equally effective if started before or after seven days. Conclusions: Intrapleural Streptokinase is the preferred treatment for treating pediatric empyema thoracis even in advanced stages and can avoid surgery. [ABSTRACT FROM AUTHOR]

- Abstract
Liposarcomas (LPS) are malignant tumours of mesodermal origin that can develop at any site where fatty tissue is present. Liposarcomas of the spermatic cord have been classified as part of the 'paratesticular' tumours which mostly originate in the spermatic cord and are found anywhere along its course. They are rare, but well-described neoplasms which present as painless swellings, most often in the groin or scrotal areas. They mimic common surgical conditions, such as hydroceles, hernias or testicular tumours, thus attracting surgical and urologic interest as they are occasionally the culprit of misdiagnoses. We present the case of a liposarcoma of the spermatic cord mimicking inguinal hernia along with a brief review of the literature. [ABSTRACT FROM AUTHOR]

Purpose: There are considerable variations in the number of adverse reaction reports related to vaccine from different countries. The aim of this study was to review the development of adverse reactions to bacille Calmette-GuΘrin (BCG) vaccination among hospitalized patients in an Iranian referral hospital. Materials and Methods: We identified hospitalized patients with BCG complications in Pediatric Infectious Disease Research Center, Tehran University of Medical Sciences, Tehran, Iran during January 2007-April 2009. Data on demographics, clinical features, laboratory findings, personal history (including vaccination history), family history, and outcomes were retrieved from medical records. Results: There were 46 cases with BCG complication during the 2 years period. All of the children received vaccination at birth. Twenty-eight patients (61%) were male. The mean age of the patients was 13.5 ▒11.3 months (range, 1 to 52 months; median, 10 months). The majority of children (57%) with BCG complication were less than 1 year old. Among hospitalized patients due to BCG complications, suppurative lymphadenitis was occurred in 28 children (61%) and lymphadenopathy was seen in 9 children (20%). Disseminated BCG was detected in 8 patients (17%) and only 1 child (2%) was presented with abscess. In 7% (n = 3) of children, the family history of BCG complications were positive. Conclusion: The most common side effect of the BCG vaccine in our study was suppurative lymphadenitis. Disseminated BCG infection in complications leading to hospitalization in our study was 17%. With regard to the difficulty in implementing such a guideline in settings where BCG is given to all newborns, registration of Iranian primary immunodeficiency disorders (PID) patients would be helpful to increase the awareness of medical community of Iran to investigate underlying disease. In addition, BCG vaccination should postpone in each newborn with a family history of PID until the definite condition has been ruled out. [ABSTRACT FROM AUTHOR]

61. Improvised external ventricular drain in neurosurgery: A Nigerian tertiary hospital experience.

Background: The most common type of hydrocephalus in developing countries is post infective hydrocephalus. Infected cerebrospinal fluid (CSF) however cannot be shunted for the reason that it will block the chamber of the ventriculo-peritoneal (VP) shunt due to its high protein content. In centers where standard external ventricular drain (EVD) sets are not available, improvised feeding tube can be used. Aim: The main focus of this study is to encourage the use of improvised feeding tube catheters for EVD when standard sets are not available to improve patients' survival. Methodology: This was a prospective study. Consecutive patients with hydrocephalus that cannot be shunted immediately for high chances of shunt failure or signs of increasing intracranial pressure were recruited into the study. Other inclusion criteria were preoperative brain tumor with possibility of blocked CSF pathway and massive intraventricular hemorrhage necessitating ventricular drainage as a salvage procedure. Standard EVD set is not readily available and too expensive for most of the parents to afford. Improvised feeding tube is used to drain/divert CSF using the standard documented procedure for EVD insertion. Outcome is measured and recorded. Results: A total of 28 patients were recruited into the study over a time frame of 2 years. There were 19 (67.9%) male and 9 (32.1%) females with a ratio of about 2:1. Age ranges varied from as low as 7 days to 66 years. The median age of the study sample was 6.5 months while the mean was 173.8 months. Duration of EVD varied from 2 days to 11 days with a median of 7 while the average was 6 days. Eventual outcome following the procedure of EVD showed that 19 (67.9%) survived and were discharged either to go home or to have VP shunt afterwards while 8 (28.6%) of the patients died. Conclusions: External ventricular drain can and should be done when it is necessary. Potential mortalities could be reduced by the improvised drainage using a standard feeding tube as described. 62. BCG vaccination-induced suppurative lymphadenitis: four signs to pay attention to.

ABSTRACT Suppurative lymphadenitis is one of the severe complication after BCG vaccination, but its diagnostic criteria and treatment guidelines have not yet been established. In this article, we describe a case of suppurative lymphadenitis caused by BCG vaccination and propose diagnostic criteria and treatment guidelines of the disease. The lymphadenitis was presented as skin involving mass and was completely extirpated. Pathological evaluation revealed a necrotising lymphadenitis, consistent with the diagnosis of BCG lymphadenitis. The patient was administered adjuvant medical treatment with anti- TB medications (Isoniazid and Rifampicin) for 3 months. At 6 months follow-up, the disease was in complete remission without complications. We recommend focus on the following four signs when diagnosing BCG lymphadenitis: (i) previous history of vaccination on the ipsilateral side of the lesion, (ii) absence of any other infection signs, (iii) absence of fever and (iv) isolated axillary or supraclavicular/cervical lymph node enlargement proven by ultrasonography or computed tomography scan. BCG vaccination-induced suppurative lymphadenitis can easily be overlooked, but prompt, accurate diagnosis followed by appropriate surgical resection should result in complete healing as in this case. 63. Preventative Effects of Ginkgo biloba Extract (EGb761) on High Glucose-Cultured Opacity of Rat Lens.

Diabetic cataract is one of the earliest secondary complications of diabetes, and it is characterized by opacification of the eye lens. In this study, we examined the protective effects of Ginkgo biloba extract (EGb761) on rat lenses cultured in high-glucose conditions. The cultured rat lenses were divided into six groups: normal group, high-glucose group, high glucose plus low, medium, and high concentrations of EGb761 groups, and a high glucose plus bendazac lysine group. The activities of antioxidases, aldose reductase, advanced glycosylation end products, transforming growth factor-β2, Smad2/3, E-cadherin, and α-smooth muscle actin were assessed by different methods. Compared with the levels in the high glucose group, EGb761 decreased the intensity of oxidative stress, decreased aldose reductase activation and the level of advanced glycosylation end products, and suppress the transforming growth factor-β2 or Smad pathway activation, further increase the expression of E-cadherin and decrease α-smooth muscle actin, and therefore, prevents the pathological changes of high glucose-induced lens epithelial cells and ameliorated lens opacity. These results suggest that EGb761 has protective effects on several pharmacological targets in the progress of diabetic cataract and is a potential drug for the prevention of diabetic cataract. 64. The Protective Effect of Vanadium Against Diabetic Cataracts in Diabetic Rat Model.

The present study was designed to investigate the effect of vanadium in alloxan-induced diabetes and cataract in rats. Different doses of vanadium was administered once daily for 8 weeks to alloxan-induced diabetic rats. To know the mechanism of action of vanadium, lens malondialdehyde (MDA), protein carbonyl content, activity of superoxide dismutase (SOD), activities of aldose reductase (AR), and sorbitol levels were assayed, respectively. Supplementation of vanadium to alloxan-induced diabetic rats decreased the blood glucose levels due to hyperglycemia, inhibited the AR activity, and delayed cataract progression in a dose-dependent manner. The observed beneficial effects may be attributed to polyol pathway activation but not decreased oxidative stress. Overall, the results of this study demonstrate that vanadium could effectively reduce the alloxan-induced hyperglycemia and diabetic cataracts in rats. [ABSTRACT FROM AUTHOR]

Introduction: One of the preventive methods of pulmonary tuberculosis is bacillus Calmette–Guérin (BCG) vaccination and one of the common complications of vaccine is lymphadenitis. We aimed to evaluate clinical course of BCG lymphadenitis in children of Kerman Province, Iran.Method: 314 children with BCG lymphadenitis were enrolled in study. Patients were followed up monthly regarding clinical improvement and complications.Results: BCG lymphadenitis was improved in many patients during follow up; during 6 months 23.7% of patients had spontaneous regression. Only the primary size of lymphadenitis had significant effect on clinical course of lymphadenitis and developing complication (p = 0.0540).Conclusion: Many patients improved without any intervention. The primary size of lymphadenitis may affect clinical course of lymphadenitis and dispose person to abscess and fistula. To confirm this result and evaluate efficiency of preventive surgery instead of observation further study is recommended. [ABSTRACT FROM AUTHOR]

Hypoxia-induced retinal ganglion cell (RGC) death has been proposed to be the critical event in the pathophysiology of glaucoma. Therefore, delaying or halting RGC degeneration, known as neuroprotection, is a novel and promising approach with potential clinical applications for treating glaucoma. In this study, we investigate hypoxia-induced cell death of RGCs and the underlying mechanisms of N-acetylcysteine (NAC) as a neuroprotectant. To establish a model for chemical hypoxia-induced cell death, RGC-5 cells were treated with the hypoxia mimetic cobalt chloride (CoCl). Following CoCl exposure, significant levels of apoptotic and autophagic cell death were observed in RGC-5 cells, evidenced by lysosome dysfunction and autophagosome formation. Pretreating RGC-5 cells with NAC significantly counteracted the autophagic cell death. NAC-mediated neuroprotection was attributed to the direct scavenging of reactive oxygen species and was mediated by targeting the hypoxia-inducible factor-1α pathway via the BNIP3 and PI3K/Akt/mTOR pathways. These results provide insights into the degeneration of RGCs and present a potential clinical application for NAC as a neuroprotectant. [ABSTRACT FROM AUTHOR]

Glutathione depletion has been postulated to be the prime reason for galactose cataract. The current research seeks the prospect of targeting erythrocytes to pursue the lens metabolism by studying the glutathione system. Aims: To study the activity of the glutathione-linked scavenger enzyme system in the erythrocyte and lens of rats with cataract. Materials and Methods: Experiments were conducted in 36 male albino rats weighing 80 ± 20 g of 28 days of age. The rats were divided into two major groups, viz. experimental and control. Six rats in each group were sacrificed every 10 days, for 30 days. Cataract was induced in the experimental group by feeding the rats 30% galactose (w/w). The involvement of reduced glutathione (GSH) and the linked enzymes was studied in the erythrocytes and lens of cataractous as well as control rats. Statistical Analysis: Parametric tests like one-way ANOVA and Student’s ‘t’ test were used for comparison. Correlation linear plot was used to compare the erythrocyte and lens metabolism. Results: The concentration of GSH and the activity of linked enzymes were found decreased with the progression of cataract, and also in comparison to the control. The same linear fashion was also observed in the erythrocytes. Conclusion: Depletion of GSH was the prime factor for initiating galactose cataract in the rat model. This depletion may in turn result in enzyme inactivation leading to cross-linking of protein and glycation. The correlation analysis specifies that the biochemical mechanism in the erythrocytes and lens is similar in the rat model. [ABSTRACT FROM AUTHOR]

The patient was investigated for the suspicion of Mendel susceptibility (MSMD) to mycobacterial diseases, and it was later found that the patient was the homozygous carrier of the mutation leading to IFNgR1 deficiency. Background Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials. gov, number NCT02662231. Findings Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58.5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31.2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (1.2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12.3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9.4%] of 7339 patients), middle (549 [14.0%] of 3918 patients), and low (298 [23.2%] of 1282) HDI (p, DFID-MRC-Wellcome Trust Joint Global Health Trial Development Grant, National Institute of Health Research Global Health Research Unit Grant.

The administration of intrapleural streptokinase (IPSK) is widely practiced in the management of loculated empyema thoracis. To our knowledge, there have been only 4 cases of hemorrhagic complications attributed to the administration of IPSK reported in the literature. In this article, we report a case of a 17-year-old girl who received IPSK and developed shock, anemia, coagulopathy and massive hemothorax. Our discussion focuses on the hemorrhagic complication of chest tube insertion and the role of IPSK in blood clot lysis and inhibition of local hemostasis. [ABSTRACT FROM AUTHOR]

Glutathione (GSH) is a ubiquitous tripeptide (γ-L-glutamyl-L-cysteinyl-glycin) playing an important role in cellular redox homeostasis, defense against electrophilic reactive species, and control of several cellular events, among others. Most of these cellular functions are related to its thiol (SH) function of the cysteine moiety. The aim of this 7-chapter, single-author textbook entitled,'Pathology of Skin Infections,'is to provide an overview of and illustrate pathological features of skin infections caused by bacteria, fungi, viruses, protozoa, helminth and arthropods. Histological features based on hematoxylin and eosin staining, and gross appearance comprise the major part of the illustrations. Immunostaining using the indirect immunoperoxidase or amino acid polymer method and non-isotopic in situ hybridization technique were employed when necessary. The indirect immunoperoxidase method using 1:500 to 1:1,000 diluted patients'serum as a primary antibody was also applied to localize pathogens within the lesion fixed in 10-20% formalin and embedded in paraffin. Papanicolaou- or Giemsa-stained cytology evaluation, as well as electron microscopic analysis and polymerase chain reaction (PCR), were also performed upon request. The prompt and precise diagnosis of emerging and re-emerging infectious diseases, nosocomial infections and sexually transmitted infections has an important socio-public impact for the prevention of unnecessary transmission of microorganisms. The skin may function as a mirror of internal organs. The author has tried to include information and unique episodes that have occurred in Japan and are picked up as notes for the readers'curiosity and intelligence. He, as a diagnostic pathologist, sincerely hopes that this textbook helps pathologists, dermatologists and other clinicians make an appropriate diagnosis of skin infections. He strongly believes that proper diagnosis may lead patients to proper treatment and cure, and may contribute to human society by preventing infectious disorders.
