Monica McGoldrick stands as a towering figure in the field of family therapy. Initially earning a Masters in Russian Studies, she then pivoted to social work and systems thinking. McGoldrick's illustrious career has been marked by significant contributions to family therapy, particularly through her work with family genograms, mapping family relationships and histories to identify patterns across generations. She expanded the use of genograms to incorporate cultural, ethnic and gender contexts, revolutionising the field. Her work emphasises understanding individuals within their historical, social and cultural contexts. She has authored multiple seminal texts, including ‘Ethnicity and family therapy’, and Genograms: Assessment and treatment, which are essential reading in family therapy training. McGoldrick has also been a trailblazer in addressing issues related to gender and power in family therapy. In this candid interview, she contextualises and frames her experience in a historical scaffold at a pivotal time in the evolution of family therapy. Monica details her involvement in the development of important endeavours such as the Multicultural Family Institute and the Women of Stonehenge where she emphasises the vital role of women in our community and the profound contribution of women in our profession, often dismissed and undervalued. Youth in the legal system (YILS) report high rates of substance use (SU), complex family/social relationships, and chronic trauma. The current study tested the feasibility of a prevention intervention, Trust-based Relational Intervention® (TBRI®), that leverages family systems by strengthening connection and providing emotional and instrumental guidance and support. TBRI includes the primary TBRI Intervention, comprised of Caregiver Training, Youth Training, and joint youth-caregiver Nurture Groups, and TBRI Family Coaching. With a sample of eight youth-caregiver dyads, the study adopted a mixed-methods design with a multi-informant approach to fulfill two goals: (1) testing TBRI as a prevention intervention for opioid use (OU), other SU, and related issues, and (2) testing the feasibility and acceptability of the TBRI Intervention by virtual delivery. Session attendance and completion rates demonstrated feasibility of recruiting and retaining participants and intervention fidelity. Preliminary results were reported on intervention outcomes, including OU and other SU, illegal activities, and educational attainment. Pre- and post-intervention comparisons showed decreases in youth negative urgency, conduct problems, and hyperactivity. Caregiver and staff participants responded favorably to TBRI and its virtual delivery; youth were more capable of expressing their needs and acknowledged the importance of families in preventing problems after discharge from secure facilities. While acknowledging sufficiency of intervention content, caregivers expressed the desire for more sessions. Results demonstrate the feasibility and acceptability of a trauma-informed, attachment-based prevention intervention for youth and families in contact with the legal system. This article aims to apply structural family therapy to working with family and intimate partner violence in after-violence context, where risks and safety issues have been addressed. This practice research study uses case analyses based on the work done with a Chinese couple and a Chinese family in Singapore, respectively. It identifies the salient therapeutic focus and healing processes in after-violence context with Chinese families. Reflections and feedback of social workers and clients during and after the conjoint sessions were included systematically. Panic disorder (PD) affects family members and can be influenced by familial factors. Adverse family events may contribute to anxiety and PD. We examined a family therapy case of a client with PD in South Korea. We used a thematic analysis to analyze therapy transcripts and video recordings and displayed our findings using a network. We identified five stages of the psychological process in the development of PD: (1) formation of anxiety, (2) sheltered life, (3) crisis, (4) loss of haven, and (5) inner breakdown. External situations, family dynamics, and psychological experiences at each stage are examined. Family dynamics that occurred as the family coped with external situations contributed to the client's psychological experiences, which triggered anxiety that led to PD. This study suggests family dynamics can be associated with one's internal experiences that could lead to PD, highlighting the importance of functional family dynamics and coping when facing crises. Neurodevelopmental disorders affect the lifespan of diagnosed individuals and their families. COVID-19 challenged these families with daily routine unpredictability requiring rapid adaptations. Moreover, associations and schools were closed, leaving these families without regular social support. Here, we investigate which individual and family factors can predict the caregiver's depressive state and overall burden. An online study took place between 2021 and 2022. A total of 32 caregivers (30 women; 48 ± 8.22 years old; range 26 to 63 years old) reported having a family member with a neurodevelopmental disorder, the majority diagnosed with autism spectrum disorder. Caregivers responded to a protocol to assess the burden, resilience, depressive, anxious, and stress symptomatology, as well as the behavior of the diagnosed individual. Hierarchical multiple regressions were performed to identify protective and risk factors for the caregivers' well-being. Caregivers' depressive statewas explained by 29.3% of the variance of the family cohesion factor, indicating that high levels of balanced family cohesion represent a crucial protective factor for reducing the caregiver's depressive state. Additionally, overall caregiver burden was explained by 17.8% of the variance due to self-perception and 26.4% due to family cohesion, with the caregiver's self-perception playing an important protective role in the overall perception of burden. The proportion of male and female respondents seems to corroborate the significant role of women in caregiving. These results emphasize the importance of considering both individual and family factors of caregivers during interventions, which have implications for family therapy with families of members diagnosed with neurodevelopmental disorders, specifically with autism. Growing evidence indicates that family-based mind-body therapy programs (FMBTs) act as an important alternative for attention-deficit/hyperactivity disorder (ADHD) treatment in the youth with minimal side-effects. Moreover, FMBTs contribute to improving the family functioning of those affected by ADHD. To summarize and analyze the benefits and mechanisms, an extensive literature search of the EBSCOhost, PubMed, Web of Science, and Scopus databases was conducted to identify eligible studies. Fifteen studies published between 2010–2023 were reviewed. The included FMBTs were implemented with 850 children/adolescents and 996 parents/caregivers, of whom 201 parent couples attended the programs together with their children. All included FMBTs adopted mindfulness/meditation as the core practice to enhance the participants' mental focus and attention, and were designed for long-term engagement, with six including parent–child joint session(s). Positive changes were observed in both children and adolescents, including improvements in ADHD symptoms, behavioral problems, executive function, and school performance. Additionally, parents experienced positive changes in ADHD traits/symptoms, mental health, and parenting behaviors. Furthermore, FMBTs were found to enhance family functioning by delivering mindful strategies to family, restoring psychological capacity and interpersonal skills in family members, and improving parent–child relationships. The reviewed FMBTs demonstrated high levels of feasibility and participant satisfaction. The factors and issues potentially influencing the effectiveness and feasibility of FMBTs are also discussed. The findings indicate that FMBT holds promise as an ADHD treatment option in home settings. The context of this paper is family situations where young people are experiencing significant behavioural and emotional problems. Based on a qualitative study, it offers an empirical and theoretical contribution to the understanding of family‐related factors that promote or hinder behaviour change. Data were collected through face‐to‐face, semi‐structured group interviews with nine families (28 participants), all of them mandatory recipients of child welfare services in Croatia. Thematic analysis comprised primary coding designed to elicit information about individual family members' capacity for change and secondary coding of family‐level phenomena. The results indicated 10 phenomena that were judged to represent factors that helped or hindered change. The discussion section suggests three broad categories of capacity to change at family level (Goals and Priorities; Problem solving dynamics; and Development of role relationship). Family‐centred practice (FCP) has been suggested as a best practice for treating youth with emotional and behavioural difficulties in residential care. In this preregistered systematic review, we examined how FCP is operationalized and measured in residential youth care and which family outcomes are associated with FCP. Our systematic search in six databases identified 5784 articles. We analysed included articles by coding the operationalizations of FCP in the interventions and study measures. Further, we descriptively summarized relations between FCP and family outcomes. Thirteen articles met our inclusion criteria. In these articles, FCP was operationalized by five types of youth worker actions in interventions and was measured as five types of family involvement. Ten articles examined pre–post differences in family‐centred interventions. Only four articles analysed relations between FCP and a family outcome. Studies found some support for a positive association between FCP and whole‐family functioning, but overall, the association between FCP and family outcomes remains unclear. Our findings demonstrate that more research is needed that specifically examines different aspects of FCP and how they relate to family outcomes in residential youth care. There is a paradoxical paradigm operating in neurodiverse families with autistic children, indicating increased stress alongside enrichment of family life. An emerging body of research has shown that the use of family therapy can improve family functioning and relationships in neurodiverse families. The current study uses constructivist grounded theory to explore clinicians' experiences and perceptions of utilising family therapy in the context of autism. Study findings suggest that clinicians integrate therapeutic techniques from diverse family therapy approaches to improve family relations by bridging different ways of learning and communicating as well as facilitating multiple perspectives. Integrating therapeutic techniques across diverse approaches further encompasses working with the systems around the individual with autism. To adapt family therapy in the context of autism, it is suggested that clinicians examine their own perspectives and beliefs about autism and keep a curious stance during the process. Women who have experienced conflict‐related sexual violence report significant long‐term effects, including posttraumatic stress disorder (PTSD), depression and relationship difficulties. Research has demonstrated that maternal trauma is associated with children's behavioural difficulties and challenges in family functioning, such as impaired communication and harsh parenting. This pilot study is aimed at evaluating the preliminary effectiveness of family therapy for Kosovar mothers who experienced conflict‐related sexual violence in 1998–1999 and later developed PTSD and their children in improving family functioning and reducing behavioural difficulties in postwar times. Sixty‐four mothers were randomised to an intervention group or a waitlist control group. Data was collected during a screening phase, at baseline before intervention initiation, after the intervention group completed family therapy and once the waitlist control group received the intervention. Generalised linear mixed models were used to analyse group differences in family functioning and children's behaviours over time. At follow‐up, mothers in the intervention group reported improved family functioning. However, mothers in the waitlist control group reported significantly fewer behavioural difficulties than mothers in the intervention group before the control group had started family therapy. There was no significant interaction between group condition and time for child‐rated family functioning. Overall, this pilot study suggests that family therapy could be effective in reducing the effects of intergenerational trauma related to PTSD and conflict‐related sexual violence. Families must restructure and renegotiate rules, roles, and boundaries when a member is incarcerated. Nonincarcerated partners may act as gatekeepers and primary caretakers, however, a strong coparenting alliance can facilitate better family outcomes such as greater relationship satisfaction and cohesion. Structural family therapy (SFT) can help families conceptualize a healthy structure when experiencing the incarceration of a member. In this study, an actor-partner interdependence model (APIM) was estimated using structural equation modeling (SEM) to analyze 864 heterosexual romantic couples where the man was incarcerated, using data from the Multi-site Family Study on Incarceration, Parenting and Partnering (MFS-IP). We examined the relationship between 4-point scales measuring the perceived reliability of parenting responsibilities and internally consistent 8-item Dyadic Adjustment Scale (DAS) scores that capture relationship satisfaction and cohesion. Both actor and partner effects are significant for each dyad member; being able to rely on one's partner to follow through with their parenting responsibilities and having a partner who can rely on them are important predictors of greater relationship satisfaction and cohesion. Results emphasize the need for therapeutic services to strengthen romantic and coparenting relationships where one partner is incarcerated. SFT allows for the theoretical understanding of structure, rules, and boundaries for families experiencing incarceration. Videogaming rates among adolescents in the U.S. are very high. In addition, gaming disorder was recently added to the ICD-11 and Internet gaming disorder (IGD) is a part of the DSM-5-TR's Conditions for Further Study, suggesting increasing import in the clinical realm. This review is an up-to-date resource for clinicians treating adolescent IGD, highlighting important areas of knowledge, awareness, and skills/behaviors to consider. Given evidence from multiple systemic reviews on the important role family dynamics play related to IGD, Attachment-Based Family Therapy is presented as a model for intervention, utilizing a case study to demonstrate how it may be applied. Family violence is becoming increasingly visible in Australia, with many state and federal governments taking on more responsibility to address family violence and its impacts on those affected. Current efforts are focused on practice frameworks, identifying and responding to risk factors, and social justice frameworks including legislating against family violence in a range of jurisdictions and addressing more broadly the structural and cultural forces that perpetuate violence and further oppress those victimised. Family violence is not unique to Australia, with prevalence rates internationally suggesting family violence occurs irrespective of country, race, age, culture, effectiveness of social policies or socioeconomic status. This paper explores relevant foundational concepts of Bowen family systems theory (BFST) as it relates to family violence and explores opportunities to evolve practice in this area. BFST offers a unique contribution towards addressing family violence, particularly in relation to the public health challenge of addressing family polyvictimisation. The paper also explores how BFST conceptualises family violence and the role of the professional in working with the family as a single emotional unit. The unique focus of BFST on observing and defining self within the emotional family unit through observation of the underlying multigenerational emotional patterns of functioning provides unique opportunities to evolve and develop both the capacity of clinicians and responses to families who are navigating the serious and challenging impacts of family violence. Dr Murray Bowen, developer of Bowen family systems theory (BFST), had this to say to clients about working in family systems: if you get bogged down in one area, move into another (Bowen & Kerr, 1985). This statement, along with the knowledge of BFST, offered an inspiration for thinking about a method of therapy. This article highlights a method of working with an individual through a systemic lens. Two ideas are integral to this focus. One is that a client can move more easily into observing self within the system when they are not just observing self in one context or relationship but rather looking at how they function in different contexts or relationships. And second, by moving into different contexts of the system, the therapist is better able to manage the tendency towards symptom focus and stay centred on the work of differentiation. A therapist can truly have a stadium view of the system when speaking with the client about how they function in different areas of life. This method defined happens in three phases: in phase 1, the client observes themselves in their system in different contexts and looks for patterns in their functioning; in phase 2, the client takes this new self‐recognition and experiments with different ways of being in one context; and in phase 3, the insights gained from experiments in one context are applied in other contexts. Both client and clinician will benefit from less symptom focus and increased ability to observe patterns in relationships, both of which are core tenets of the work of differentiation in BFST. The concept of differentiation of self and its relationship to the natural capacity of humans and other social animals to adapt underpins clinician and researcher Victoria Harrison's long‐term investigation of the biology of reactivity in relationships. Victoria's early research related to infertility and reproduction demonstrated how reactivity in at least three prior generations can contribute to chronic conditions, such as endometriosis, that are less amenable to intervention. Her most recent project, the 'Observations of Change in Physiology and Family' research, highlights how work on differentiation of self improves functional outcomes in physical and mental health for the family. While an individual may not experience the 'calm mind' that is often the goal of conventional therapeutic intervention, a significant reduction in symptoms and an improvement in overall functioning and relatedness are outcomes. This occurred when participants focused on increasing person‐to‐person contact with extended family members, reducing their emotional reactivity to significant others in the important triangles of their life and family and maintaining intellectual activity, often described as curiosity, even while reacting. TIAP is an observational procedure to assess family functioning detecting simultaneously the role of each participant and the interdependence of relational behaviors. In particular, the procedure requires family members to play according to different interactive configurations (parent1-children; parent2-children, all together, children and parents as separate units) and therefore different microtransitions from one configuration to another. As such, the procedure allows to study how family members coordinate to maintain stability, promote change, and encourage members to explore different interactive configurations within the family system. TIAP has been validated through several studies conducted with different non-clinical groups of families that have highlighted the salient aspects of family functioning, and significant correlations with variables external to the family system, such as children's social-emotional competence in the educational context. This paper focuses on the use of TIAP in the contexts of assessing parental competence. Specifically, the article aims to describe, through the reference to a clinical case, the results emerged from a study conducted with 33 families involved in a parenting assessment process. The study is part of a broader collaborative project between the Child and Adolescent Neuropsychiatry Clinic of the Italian National Health Service in Parma, the University of Parma, and the Bologna Family Therapy Center. TIAP was administered to all the families involved as a complement to other tools routinely used for all cases handled by the professionals of the clinic. The coding system includes different indices. Some analyze the interactive family modes: family coordination (mutual attention and responsiveness), the responses to potentials for change (disregard, absorption, amplification), and intra-familiar exploration. Other indices concern the quality of the interactions: the relational triadic dynamic of microtransition (detaching-entrusting-welcoming-joining) and the consistency/inconsistency of the communication channels. The results highlighted how TIAP makes it possible to identify the specific interactive modalities of the different members and their interdependence and reciprocity, favoring the identification of both family weaknesses and family resources, including the children's contribution. Adopted children are often at a risk of experiencing several neurobiological and psychosocial difficulties. Adoptive parents must support these difficulties whilst managing their own idiosyncratic challenges. Family-based psychotherapeutic interventions which promote adoptive family functioning, environments and relationships can mediate these difficulties for adopted families. This review synthesises evidence exploring family-based psychological interventions for adoptive families, appraises the literature's strengths and weaknesses, and reports characteristics of promising interventions. Included studies recruited domestically adoptive families receiving psychotherapeutic interventions delivered to at least one adoptive parent and child. The authors searched seven electronic information databases, four grey literature databases, two journals and five relevant websites up to 12.12.2022. The quantitative Risk of Bias in Non-Randomised Studies of Interventions tool and the qualitative Critical Skills Appraisal Programme checklist assessed risk of bias. The narrative synthesis presents 20 papers detailing 18 studies involving at least 729 adopted children and 829 adoptive parents. Findings provide preliminary support for integrative interventions which include aspects of sensory activities, attachment-based play, Dyadic Developmental Psychotherapy and Eye Movement Desensitisation and Reprocessing with Therapeutic Input provided to adopted children and adoptive parents separately, alongside the adoptive family. However, risk of bias was high, limiting the conclusions. There is increasing evidence that highlights the benefits of Family-oriented Therapeutic Conversations (FAM-TC) for the patient and the family; however, studies show variability regarding the content and the way these interventions are offered. This may hamper its further development in clinical practice. This review systematically maps the available literature on nurse-led FAM-TC and offers a solid synthesis of the characteristic, effectiveness, and feasibility of these interventions. A systematic search in PubMed, CINAHL, Cochrane, Web of Science, PsycINFO, Trip (Turning Research Into Practice), BASE (Bielefeld Academic Search Engine), OATD (Open Access Theses and Dissertations), and ProQuest databases identified 37 studies. The interventions varied in interventionist nurses' profile, the intervention content, or the duration of the sessions offered. Most of the interventions showed beneficial effects on perceived family support and family functioning. This review offers suggestions for future studies, such as the inclusion of specific theoretical frameworks for intervention design, targeting both the patient and the family and offered by nurses with family nursing competency. Although structural family theory proposes that family boundaries play a significant role in marital relationships, limited empirical evidence supports this theoretical model. The present study tested structural family theory by examining the complex associations between intergenerational boundaries and adult children's marital satisfaction, as well as the indirect effect of family adaptability and cohesion. A nonclinical convenience sample of individuals (333 participants, 168 females) married for at least one year was recruited. Participants provided self-report data on demographic variables, family boundaries, family cohesion and adaptability, and marital satisfaction. The internal intrusion score (intrusion by families of origin on the nuclear family) of intergenerational family boundaries was negatively associated with marital satisfaction, while the external intrusion score (intrusion by the nuclear family on families of origin) was positively associated with marital satisfaction. The effect of intergenerational family boundaries on marital satisfaction was exerted indirectly through family cohesion and adaptability. There was a significant association between intergenerational family boundaries and adult children's marital satisfaction. This study contributes to the conceptual framework of structural family theory, enabling family therapy practitioners to better understand the pathways through which intergenerational family boundaries impact adult children's marital satisfaction. The internal intrusion of intergenerational family boundaries was negatively associated with marital satisfaction, while the external intrusion demonstrated a positive association. This conceptual paper explores the surprising impact exerted by third elements on the escalation and de‐escalation of conflicts in business families. To this end, we draw on core concepts of Batesonian cybernetic theory to perform a reconceptualisation of pertinent literature on changing conflict dynamics gathered in family business research and consultancy. Based on this reconceptualisation, we identify explanations and key mechanisms for the impact of third elements on conflict dynamics. There is a dearth of research examining the relationship perspectives of Latina young people, a population that reports disproportionate rates of teen dating violence (TDV). This strengths-based, qualitative study utilized critical narrative inquiry to explore Latinas' relationship perspectives through in-depth interviews. Rigorous analysis revealed participants' desire for freedom from violence/abuse, open communication, independence, and egalitarian values in relationships, while reporting relationship models and media representation as their main influences on relationship perspectives. Our findings demonstrate that although patriarchal gender norms influence romantic relationships with Latinx cultures, many participants rejected traditional gendered power dynamics that can lead to violence, providing important information for practitioners seeking to promote healthy relationships among this population. The need to expand knowledge of family-based intervention for people with ASD has been observed. However, there is a lack of study in Chinese context such as Hong Kong exploring the use of multifamily therapy (MFT) for adults with ASD. The primary aim of the present study is to assess the feasibility of a process-oriented MFT in promoting family relationships for young adults with high functioning-ASD (HF-ASD) and their parents. We also sought to explore the therapeutic process factors of MFT that were deemed helpful by the family participants in our study. Two types of MFT—one online, the other face-to-face—were conducted from March to August 2021 for families with adults with HF-ASD in Hong Kong. In total, 13 families participated in our multi-method study. Among the results, cross-family therapeutic alliance and treatment engagement were positively associated with enhanced family relationships among participants. Significant differences emerged in changes in the cross-family therapeutic alliance and family relationships between the online and face-to-face MFTs. Two themes regarding the helpful aspects of MFT emerged: first, having a cross-family interactional context and, second, the authentic engagement of people with HF-ASD. Overall, our results imply that MFT, in either online or face-to-face mode, could be a feasible family group psychotherapy for adults with HF-ASD and their parents. [ABSTRACT FROM AUTHOR]

This participatory research project highlighted the views and experiences of young people who are the brothers and sisters of children and young people who have experienced child sexual abuse. By working with young people who had experienced sexual abuse and their siblings, this practitioner research sought to give these young people a voice. The research involved a group process with young people who have experienced sexual abuse and have siblings, and one‐to‐one online consultations with the brothers and sisters of young people who have experienced abuse. All the young people spoke of their sibling and family relationships as unique and requiring unique responses to these relationships by services and health‐care professionals. Siblings' needs in the aftermath of a child sexual abuse discovery are often overlooked. This research describes how just as children who have experienced sexual abuse may feel anger, fear, guilt, shame, anxiety, and confusion, so do their siblings. Brothers and sisters of young people who have experienced sexual abuse describe significant relational trauma with intense emotional experiences of isolation, confusion, sadness, and anger. There is a real need to support young people and families to find words and develop capacities to talk about child sexual abuse ‐‐ a need to find supportive ways to speak about what has occurred and understand its impact on all. This paper describes a multi‐family therapy (MFT) group for adult outpatients with obsessive‐compulsive disorder and their family members. Since family accommodation (FA) is known to maintain OCD symptoms by preventing exposure and response prevention (ERP), reduction of FA is a major focus of the treatment. Furthermore, psychoeducation, conjoint ERP and (re)discovering resources and strengths of families are also important therapeutic foci of the family group. The organisation of MFT is explained and involvement of family is illustrated with clinical vignettes. Finally, some reflections about MFT are provided. Objective: Parent-only cognitive-behavioural therapy (CBT) interventions have promise for youth with anxiety disorders. Fear-Less Triple P (FLTP) is one such intervention that has been found comparable to child-focused CBT. Although traditionally administered in six sessions, a one-day workshop format of FLTP was developed to improve accessibility. The current study compared the effectiveness of the six-session and one-day workshop formats. Method: Seventy-three youth (mean age, 8.4 years; 74% male) were randomized to traditional FLTP (6-week group) or the one-day workshop format. Anxiety diagnostic status, self- and parent-reported anxiety symptoms scores, independent evaluator-rated improvement, treatment satisfaction, and measures of family functioning were included to assess treatment outcome. Data were collected prior to treatment, and 1-week, 6-months, and 12-months following treatment. Results: Both conditions resulted in significant improvement in child anxiety symptom scores per parent report (on both questionnaire and diagnostic interview measures). Furthermore, significant decreases in sibling anxiety were observed in both treatment conditions. There were no statistically significant differences between conditions on any outcome measure. Conclusions: Results of this study add to the growing evidence that brief, low-intensity, parent-only interventions can effectively target child psychopathology. These brief interventions are ideal for families for whom the resources and time required to commit to a standard multi-week intervention are prohibitive. Binge-spectrum eating disorders (EDs; bulimia nervosa, binge eating disorder) often develop during adolescence and are associated with serious psychological and physical consequences. Current treatments for adolescents are highly behavioral in nature and while efficacious, many patients do not reach remission indicating that current treatments fail to target a key maintenance factor for EDs. One potential maintenance factor is poor family functioning (FF). In particular, high family conflict (e.g., arguing, critical comments) and low family cohesion (e.g., warmth, support) are known to maintain ED behaviors. Poor FF can (1) cause or exacerbate an adolescent's use of ED behaviors to cope with life stress and/or (2) inhibit parents from being a resource to adolescents during ED treatment. Attachment-Based Family Therapy (ABFT) is specifically designed to improve FF, and thus may be a promising adjunct to behavioral ED intervention strategies. ABFT, however, has not been tested in adolescents with binge-spectrum EDs. Thus, the current study is the first to evaluate a 16-week adapted ABFT treatment for adolescents with EDs (N = 8, Mage = 16.00, 71.43% female, 71.43% White) fusing together behavioral treatment for EDs with ABFT for highest possible impact. Eight families were treated in an open pilot trial to examine treatment feasibility, acceptability, and preliminary efficacy on FF and eating pathology. Overall, findings were promising. ABFT + B treatment was feasible and acceptable and showed preliminary evidence that it could improve FF and ED behaviors. Given the growing global concern for mental health, the general population has begun to approach psychotherapy to treat symptoms and disorders and improve their quality of life; it is by this concern and interest that the motivation arises to generate an intervention program that joins efforts to work not only with the patient who presents the symptoms or who receives the diagnosis but also with the family, commonly seen as external to the patient's life and who sometimes comes to complement, accompany or supervise the process. Family, however, is rarely included as part of the process, except when the conflict affects the family directly or in a support group. The present work is a case study from the "circular investigation" proposed by González (1997), in which depressive disorder involves its dysthymic modality from psychodynamic therapy. This approach treats the family as a whole, respecting their processes, giving importance, and listening to each member, not just the applicant. Through this intervention, we examined the psycho-emotional impact experienced by relatives of patients diagnosed with depression. Subsequently, a psychodynamic family intervention was designed and applied through four phases: diagnosis, intervention, consolidation of objectives, and closure. This study presents a tiered conceptualization of family partnership developed by the Family-Run Executive Director Leadership Association (FREDLA) with examples of strategies from the literature. This sub-study was part of an overarching systematic review project that aimed to review the literature on family partnership in relation to youth outcomes. The tiers of family partnership include family involvement (i.e. family's inclusion in their child's care); family engagement (i.e. collaboration between TRC and families); family-driven (i.e. families as full partners). This review included thirty studies (n = 23 family involvement, n = 7 family engagement, n = 0 family-driven). The most common family involvement methods were family therapy and family visits to the program, primarily, delivered face-to-face. The most common family engagement method was activities, therapies, and skill building occurring at the home with family present. Methods of measuring family partnership primarily included the use of administrative data. Implications for research and practice include the provision of research that evaluates the effects of family partnership on outcomes important in the TRC setting and the development of research-practice and family-research collaborations to increase the uptake of effective family partnering methods. Most models that analyse risk and protective factors during adolescence include the family system and family functionality as key elements. For this reason, Family Counselling Services play an important role in addressing adolescent problems. These services require quick and easy-to-apply assessment tools that integrate different dimensions of family functioning. To develop and validate a brief scale for use in Family Counselling Services, exploring and confirming its factorial structure and testing its psychometric properties. Confirmatory Factor Analysis using structural equation modeling. 441 family members in seven units. Results: A 5-factor structure was confirmed, with a good fit. All the Average Variance Extracted values were higher than the Maximum Shared Squared Variance values. The results confirm the structure of the Family Functioning Scale-Madrid. In addition, the scale demonstrates adequate psychometric properties and stable structure during two evaluation periods with different samples. The instrument is considered reliable and consistent for of assessing family intervention carried out in Family Counselling Services. Highlights: What is known so far? Family system and functionality has a relevant role as a risk and/or protective factor with respect to maladaptive behaviours. One common demand of Family Counselling Services is to be able to count on assessment protocols and instruments adapted to their target population. This cross-sectional survey evaluated well-being and family dynamics before and during the COVID-19 pandemic in 1287 (16 + years of age, 68.3% female) Chinese participants. Structural equation modeling was used to test the association of well-being and systemic family dynamics, and related moderating factors. Results indicated some subscales of well-being and systemic family dynamics significantly worsened during the pandemic. A modified model fits well for both data before and during the pandemic in which well-being was significantly associated with systemic family dynamic and by family income. Age also positively related systemic family dynamics. The relationship between family income and well-being and the relationship between systemic family dynamics and well-being were moderated by the pandemic. The results suggest that well-being and systemic family dynamics and their associations are impacted by the COVID-19 pandemic. Background: This study explores family functioning and its associations with adolescent major depressive disorder (MDD), comparing its dynamics with healthy counterparts. Family functioning (cohesion, flexibility, communication, and satisfaction), maternal depressive symptoms, postpartum depression history, parental divorce, parental alcohol abuse, and the adolescents' cognitive flexibility, are examined. The research incorporates the perspectives of both adolescents and mothers. Methods: The sample includes 63 mother-teenager dyads in the clinical group and 43 in the control group. Instruments encompass the Family Adaptability and Cohesion Evaluation Scales (FACES IV), Children's Depression Inventory (CDI-2), Beck Depression Inventory (BDI-II), The Brixton Spatial Anticipation Test, and structured interviews. Results: Families of adolescents with MDD exhibit lower flexibility, cohesion, communication, and overall satisfaction. Depressed adolescents display reduced cognitive flexibility. Discrepancies were observed between adolescents' and mothers' perspectives as associated with adolescents' MDD. Teenagers emphasized the severity of maternal depressive symptoms, while mothers highlighted the importance of family cohesion and flexibility. Conclusions: This study emphasizes a holistic strategy in addressing adolescent depression, including family-based assessment and therapy. (1) Background: Family factors play an important role in the management of diabetes, establishing a relationship between conflicts and non-adherence to therapy. High values of HbA1c are involved in specific complications of the disease (retinopathy, nephropathy, neuropathy, ketoacidosis). This study aimed to determine the role of systemic family psychotherapeutic interventions in increasing the quality of parent–child/adolescent relationships and in optimizing the child's glycemic control. (2) Methods: In this prospective observational study, 64 parents of children and adolescents with type 1 diabetes were evaluated regarding their relationship with their children, using the Child–Parent Relationship Scale-Short Form (CPRS-short form). The children were divided into three groups: one participated for 6 months in systemic family psychotherapy with children and their parents (FT), the second group participated in individual psychotherapy (IT), and the control group (CG) received no intervention. HbA1c values were recorded before and after the interventions. (3) Results: HbA1c means decreased significantly after the family psychotherapy program. The scores on closeness in the family therapy group increased significantly, and the scores on conflict decreased significantly after the intervention, compared with IT and CG. Infertility remains one of the most salient forms of reproductive trauma that individuals experience. Indeed, infertility engenders feelings of shame and social isolation. However, infertility sufferers do not experience infertility as a one-time traumatic event; rather, implications exist for family members of those who are directly experiencing infertility, especially parents of adult children. Thus, infertility affects family social support for this critical population through disenfranchised grief. Clinically, we have found that family therapy remains a powerful experience for both individuals who are experiencing infertility, as well as their family circles. Indeed, infertility engenders egregious stress on families, not just individuals and couples. To our knowledge, no family therapy interventions have addressed this area of family stress within the academic literature. We propose that Emotionally Focused Family Therapy (EFFT) is an excellent theoretical fit for treating families experiencing infertility. Within this article, we will outline a review of how infertility not only impacts individuals and couples, but also families, and how family therapy provides a salient form of healing for all involved. We will also address how specific EFFT interventions may be applied to treating families navigating infertility through our three-stage family therapy process. For families who may be reluctant to attend multiple sessions, we also propose a brief EFFT psychoeducational coaching model which still addresses concerns specifically experienced by families navigating infertility. Healthy family functioning is important for successful adolescent development and serves as a protective factor against adolescent behavior problems. When adolescent behavior problems exist, family therapy can help promote warmth and cohesiveness in the family, which results in healthier family functioning. Furthermore, family therapy is the gold standard for treating adolescent behavior problems. However, most of the research on family therapy for adolescents are with manualized models that have difficulty being implemented in usual care. The purpose of this study was to compare the effectiveness of family therapy in improving family functioning as compared to individually-based treatments, all of which were offered in usual care settings. Participants were 205 adolescents and their caregivers living in a large, metropolitan area. Data were collected at four time points (baseline, 3-, 6-, and 12-month follow-up) and analyzed using latent growth curve modeling. Family functioning was assessed by separate caregiver and adolescent reports of cohesion and conflict subscales on the Family Environment Scale and caregiver-reported parent-adolescent domain of the Stress Index for Parents of Adolescents. Across treatments, caregivers reported improvement in family cohesion and decreases in family conflict and parental stress. Similarly, adolescents across treatments reported a decrease in family conflict but no concomitant increase in family cohesion. Overall, there was no between-treatment differences in overall change with both conditions showing improvement in family functioning. Results indicate that both family therapy and non-family treatment in usual care for adolescent behavior problems are effective for improving family functioning, suggesting that existing treatment services are viable options for adolescent behavioral health when offered under monitored conditions. This position paper from a core group of infant mental health academics and clinicians addresses the conspicuous underrepresentation of the infant in mainstream family therapy. Despite infants' social capacities and clear contributions to family dynamics, they remain largely overlooked within this therapeutic context. We suggest that family therapists have moral and professional responsibilities to support the participation, protection, and well‐being of all family members, including the infant. Here, we emphasise the importance of including the infant in the family therapy setting. By highlighting their frequent omission, we aim to amplify infants' often unheard 'voice,' role, and contributions to family development, especially recovery from distress. A shift towards infant inclusion as the rule rather than the exception represents a new frontier of integration. We first highlight the relational nature of infant development with a focus on the infants' psychosocial capacities and vulnerabilities. We then consider reasons why the infant may be overlooked in family and systemic therapies and offer a rationale for inviting the infant into these settings, illustrated through the use of a clinical case vignette. To facilitate infant inclusion, we propose a series of guidelines to meaningfully incorporate infants into family therapy practices. We conclude by encouraging shifts in family therapy research, training, and practice to better incorporate and understand the unique contributions of the infant to family life. Developmentalists have demonstrated that the quality of relationships established by infants with their proximal social environment is crucial for lifecourse development. However, studies of parent–infant relationships have mostly centred on the mother–child dyad. Stemming from family systems theory that considers interactions within the whole family as critical for an individual's personal development, a group of family therapists and researchers in Lausanne (Switzerland) tried to bridge the gap between systemic and developmental thinking by stressing the need to establish the mother–father–infant triad as a collective unit of study. In response, they created the Lausanne Trilogue Play (LTP), a method to systematically assess the quality of mother–father–infant interactions. The LTP is an observational situation during which parents are asked to play with their infant in four parts: (i) one parent plays with the infant, while the other parent is 'simply present'; (ii) the parents switch roles; (iii) all three play together; (iv) the parents discuss in front of the infant. The theoretical model underpinning the LTP is the family alliance model, which postulates that the quality of the coordination demonstrated by the triad to achieve this task can be assessed mainly through careful observation of non‐verbal behaviours as indicators of the achievement of four interactive functions (i.e., participation, organisation, focalisation, affect sharing); fulfilment of these functions determines the quality of relational functioning within the system. This article introduces the clinical, theoretical, and empirical foundations of using the LTP method with the family alliance model; its use in clinical and research contexts; and the most recent advances in the field of research on mother–father–infant interactions based on the LTP situation. The Biobehavioral Family Model (BBFM) was developed and evolved as a heuristic research model to support the investigation of pathways by which family relational function impacts individual family member wellbeing and disorder. Recently, the BBFM and its related assessment approach, the Family Relational Process Assessment Protocol (FRAP), have emerged as tools for clinical practice and training. The BBFM model will be presented, along with definitions of the dimensions constructed in the model, and research evidence in support of the model. To illustrate how the BBFM and FRAP are used in training, instructions for conducting the FRAP will be presented. Then, transcripts from two contrasting families participating in one of the interaction tasks will illustrate how, in training, the FRAP is interpreted through the BBFM lens to illuminate how these patterns of family relationship impact the identified patient. Finally, three applications of this training approach will exemplify the use of the BBFM and FRAP in the context of a Child and Adolescent Psychiatry Fellowship, a Family Medicine Fellowship, and a Family Therapy Training Program in Istanbul, Turkey. Limitations and future directions for the application of the BBFM in the exploration of multicultural aspects of family function for clinical and training purposes will be discussed. Families are core to human well-being. Therapeutic intervention may be needed in the context of family disruptions. Attachment theory conceptualizes parents as the secure base and safe haven that support children's optimal development. Parents who have experienced their own attachment difficulties or traumas may not provide quality caregiving necessary for balanced secure parent-child attachment relationships. Following Bowlby's original thinking (1988), an attachment approach to family intervention views the therapist as a secure base that enables families to explore individual and system problems to restore equilibrium. Attachment informed therapy uses attachment theory to understand family functioning. However, the unavailability of valid economical assessment for examining attachment representations has constricted the practical utility of attachment theory in family therapy beyond applications of general concepts. This chapter describes the Adult Attachment Projective Pictures System (AAP) and explores its use as an efficient manner for assessing attachment representations within families that allows therapists to understand problematic interactions, disabling defensive processes, make predictions concerning negative patterns, and create targets for change and restorative intervention. Consolidating three decades of attachment and caregiving system research, we describe how distinct patterns of AAP responses for each adult attachment group map onto expected parenting and family system expectations and behaviors to provide a concise and informative framework. In addition to the traditional adult attachment patterns (Secure, Dismissing, Preoccupied, Unresolved), we describe for the first time expectations for two additional forms of incomplete pathological mourning (Failed Mourning and Preoccupied with Personal Suffering) that have been overlooked in the field. While family work is acknowledged as relevant in the care of eating disorders (EDs), not much literature has explored it in the period of transition from adolescence to young adulthood (16-18 to 30 y.o.). Yet this period is of significant importance in the prognosis and evolution of EDs; but its particular stakes require specific therapeutic settings-especially for inpatient EDs. In this paper, we start from the paradoxical observation that some families refuse this type of work in its usual form, with a family-dedicated therapist, and require to only exchange with the psychiatrist in charge of the treatment plan. We use a psychosomatic-informed psychoanalytic approach to shed light on this refusal as a latent denial of the contribution of family dynamics to the current symptom, and an unconscious tendency to stick to a dependency-laden family scheme. We then explain the conception of a specifically dedicated therapeutic setting, designed to address this specific type of resistance, offered to families as a therapeutic compromise designed to give them a specific position in the care of their child. In our joint therapeutic consultations, family dynamics are addressed on the basis of exchanges regarding treatment and in particular feeding. While such exchanges start from medical considerations, the therapeutic couple (psychiatrist-psychologist) uses them to address the parent and patient expectations underlying the symptom. We propose to call this act "inscription"; it enables a separation from the underlying dependency-oriented family scheme, while stressing the importance to care for associated parental anxieties. Purpose: The study aimed to assess families' resource use and to explore parents' experiences of the Sofia model, a family-based intervention for parents with substance use disorder (SUD). Methods: Interviews were conducted with 14 parents. Data were analyzed with qualitative content analysis. The resource use assessment was estimated based on data for 40 families referred to the Sofia model and 30 reference families. Results: Parents described difficulties accessing family support and differences in received services in the Sofia model. The intervention lacked focus on the children. Families' communication regarding substance use did not increase. Resource use estimations indicated larger use of services by families in the Sofie model, but little co-parent and child-specific support. Conclusions: The intervention failed its aim to focus on the children, and enhance communication about SUD. Background: Family‐systems interventions have been proposed as one way of supporting families of people with an intellectual disability (ID) or who are autistic. This systematic review aimed to summarise what family‐systems interventions have been studied with this population, what evidence there is for their effectiveness and families' experiences of the interventions. Methods: The review was preregistered on PROSPERO (CRD42022297516). We searched five electronic databases, identified 6908 records and screened 72 full texts. Study quality was evaluated using the Mixed Methods Appraisal Tool, and a narrative synthesis was used. Results: We identified 13 eligible articles with 292 participating families. Most studies reported positive effects of the interventions on wellbeing and family relationships, and families reported positive experiences. However, research quality was poor and there are no any sufficiently powered randomised controlled trials demonstrating family‐systems interventions' effectiveness for this population. Conclusions: There is a need for higher‐quality research to establish whether family‐systems interventions are beneficial for families of people who have an ID or who are autistic. Background: There is a global need for interventions that support the wellbeing of people living with dementia and their family care partners. Studies show that shared musical activities may achieve this. Our systematic review aimed to synthesise existing research exploring dyads' experiences of shared musical activities across a range of contexts. Method: From 31 October 2020 we searched PubMed, PsycInfo, CINAHL Complete, EMBASE, RILM, Web of Science Core Collection, Google Scholar and ProQuest Dissertations & Theses for studies published up to 14 April 2021, and hand searched five music therapy journals plus citation lists. Thirteen qualitative studies reporting on dyads' experiences and perspectives of shared musical activities across a range of settings were included. Studies with mixed populations or mixed modality interventions were excluded. We analysed the final studies using thematic synthesis, engaging in reflective discussions and reflexivity throughout. The quality of included studies was assessed using the CASP qualitative checklist. This study is registered on PROSPERO: CRD42020169360. Results: Six themes were identified from 13 studies: 1) shared musical activities support wellbeing for people living with dementia, 2) music groups become ecological systems, 3) shared musical activities are experienced differently over time, 4) shared musical activities are experienced by me and as we, 5) music is a supportive structure, and 6) the thread of connection (an overarching theme). A GRADE-CERQual assessment found moderate to high confidence in these findings. Findings informed the development of the Contextual Connection Model of Health Musicking. Conclusion: Shared musical activities foster experiences of connection for people living with dementia and their family care partners. Experiences of connection are supported through professional facilitation and the structural aspects of music, and are influenced by the setting and changes over time. These experiences of connection play a central role in supporting dyadic and individual wellbeing. These findings are largely relevant to a western cultural context; future research should seek to include more diverse cultural experiences. [ABSTRACT FROM AUTHOR]

Although international research has defined best-practice intervention for children from vulnerable families as integrated and comprehensive, limited implementation and longitudinal evaluation of this approach has been conducted. The Spilstead Model (SM) of early years milieu intervention provides a uniquely integrated one stop shop model of care incorporating a comprehensive range of best-practice programs within a trauma-informed approach. Results from an initial evaluation involving 23 families (mean child age 3.7 years) indicated large effect size improvements 12 months post entry in family functioning as well as child development and emotional wellbeing (ES 0.8 -1.46, p < 0.001). This study aimed to evaluate the sustainability of these outcomes for both children and families via follow-up of the initial study co-hort 10 years post the initial evaluation. The study targeted families who participated in the original evaluation. Clinician and parent-rated adolescent measures paralleled the original assessments of parent, child and family functioning. Qualitative evaluation was also conducted via a semi-structured interview with parents. 83% of the original sample participated. Mean youth age was 13.2 years. Results indicated sustained improvements in parent-child relationship, child-wellbeing and reduction of parent stress with large effect size (1.14 – 1.92 p < 0.001). On average 73% of the adolescents scored within the normal range on each measure of functioning. Few had repeated school grades or been suspended. None had been arrested. Emerging themes from the qualitative evaluation confirmed the value of the integrated model. This study examines the impact of social media‐based drama therapy and family counselling in reducing symptoms of postpartum depression (PPD) in women aged 18–34. The researchers used a quasi‐experimental design and randomly assigned the participants (n = 303) to the control, drama therapy, and family counselling groups. The key findings of the study are as follows. First, at baseline, women not only showed more symptoms of PPD than their male counterparts, but men also reached the PPD threshold of 12/13. Second, the difference between women and men regarding their PPD scores achieved statistical significance (p = 0.004). However, during the post‐intervention assessment, men who received drama therapy intervention reported a significant drop in their PPD below the threshold of 12/13. Only women whose spouses received family counselling on spousal support reported a significant drop in their PPD. However, those whose partners did not receive family counselling on spousal support still reported PPD scores within the threshold. In both instances, women in the control group still had high PPD scores. Finally, during the 6‐month follow‐up assessment, even men in the control group did not have significant symptoms of PPD as their scores dropped below the threshold. On the contrary, women in the control group still had high PPD, indicating that PPD lasted more in women than in men. Also, women whose husbands received family counselling on spousal support reported PPD scores below the threshold. Child focus is a central construct within Bowen family systems theory (Bowen theory). A clinical implication is that mental health treatment focusing on a child may unwittingly reinforce the operation of child‐focused processes, which undermine rather than enhance child well‐being. The concept of child focus in Bowen theory presents significant implications for professionals working in school settings and in fields such as children's mental health, which are inherently child‐focused. Bowen theory is the guiding theoretical framework for School‐Based Filial Therapy (SBFT). SBFT is a play therapy intervention that was initially established in remote and outer‐regional New South Wales, Australia in response to the low availability of children's mental health services and the significant barriers associated with caregiver engagement in children's mental health treatment. It involves trained school personnel facilitating therapeutic play sessions with children experiencing emotional–behavioural problems. The intervention occurs on school grounds, during school hours, and children's family members do not participate in the intervention. This mixed‐methods study examines the impact of children's participation in SBFT upon family functioning. Interviews with caregivers (n = 10) of children who participated in 10 SBFT sessions were analysed using content analysis. Quantitative data were collected using the Differentiation of Self Inventory – Short Form and Visual Analogue Scale – Family Functioning. A Wilcoxon signed rank test was used to analyse the pre‐ and post‐data. Qualitative outcomes indicated changes in the categories of child functioning, caregiver functioning, and extended family functioning, whilst child‐focused processes remained dominant but changed in intensity and valence following SBFT. Multifamily therapy (MFT) for adolescent eating disorders (ED) is anchored in systemic theory and family therapy, but two conceptual and practical paths have emerged from this common framework. The first one, called ED‐focused MFT, is centered on behavioral change and weight gain in the early stages of treatment, while the second, known as relationship‐focused MFT, is less directly focused on symptom improvement and more on family changes. Compared to ED‐focused MFT, validation of more relationship‐focused MFT models has been lagging behind although they are frequently implemented and practiced in Europe. The purpose of this article is to give more visibility to existing relationship‐focused MFT models by presenting an integrative, yet predominantly family‐oriented MFT program developed for adolescent anorexia nervosa (AN) (12 to 18 years) on the ED unit of a large pediatric hospital in France. After presenting the history and development of this relationship‐focused MFT program, including the challenges it encountered and its evaluation, we describe its rationale and objectives, then outline its course and content, giving illustrations of techniques and activities for each of the five phases of the program. Finally, we review the current status of this model, its advantages and limitations, and provide a critical appraisal of existing evidence and recommended future research directions. Sibling bereavement is a life-changing event with implications for the individual and family. In a study guided by Gadamer's philosophical hermeneutics focused on sibling bereavement when the brother or sister dies for a drug-related reason, a critical realisation became apparent; it is possible to understand one person's story from multiple standpoints. This is directly relevant to social work practice as different viewpoints make it possible to illuminate other aspects of the person's experience keeping us open to hearing more, reflecting and willing to learn something new. A summary of the literature on the three subject areas is presented, as are the identified gaps that helped refine the research questions. A synopsis of what it means to conduct research underpinned by Gadamer's philosophy is outlined and then elucidated through application to one participant's interview. The article serves a dual purpose of presenting information on subject areas relating to drug and alcohol, thanatology and family therapy theory, promoting understanding in these horizons. The primary aim is to focus on one of the participants, Karen, from the larger study to show how different horizons bring different understandings to the fore. The effectiveness of intense specialised multi‐family therapy (ISMFT) for 111 multi‐stressed families, and the therapeutic alliance as a possible predictor of outcome, were examined. A repeated measures design was used, where changes in all ISMFT phases (preparation, multi‐family therapy and follow‐up) were assessed and compared for both mothers and fathers. Evidence was found for improved family functioning after the therapy period, which was maintained at 3 months follow‐up, although the multi‐stressed families still functioned in the problematic range. The therapy did however not decrease parenting stress, or did so only temporarily. Observations of the therapeutic alliance with the System for Observing Family Therapy Alliances (SOFTA) scales indicated that high therapist engagement was related to positive therapy outcomes, both at the start of therapy and later. High family engagement also predicted therapy effectiveness, but only at the start of therapy. The present study shows that solution‐focused multi‐family therapy at least seems to provide the first step in alleviating problems in multi‐stressed families. [ABSTRACT FROM AUTHOR]
