Bothersome gastrointestinal (GI) signs/symptoms, including abdominal pain, distension, nausea, and flatulence, are common in children. A diet low in fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAPs) is frequently recommended for children with GI symptoms. Currently, there are no studies on the effect of FODMAPs in healthy schoolchildren. In this cross-sectional study, schoolchildren reported an association between FODMAPs and GI symptoms through a standardized questionnaire and images of 20 common staples known to be rich in FODMAPs. A total of 208 schoolchildren aged 8–18 years old participated. A proportion of 38.0% of children reported GI symptoms, with abdominal pain (33%) being the most common complaint followed by abdominal distension (24%) and nausea (23%). The majority of children who reported intolerances to FODMAP-containing foods were intolerant to less than two food groups (76%). While vegetables and legumes (26%), particularly black beans (11%) and onions (7%), emerged as the most common group of triggers, milk (12%) stood out as the single food most frequently associated with GI symptoms. In conclusion, there was a high prevalence of FODMAPs intolerance among schoolchildren. Larger studies are recommended to confirm these findings and to inform possible dietary interventions to reduce the effect of FODMAPs on schoolchildren. [ABSTRACT FROM AUTHOR]

JAMA Netw Open. 2022;5(2): e220978. https://doi.org/10.1001/jamanetworkopen.2022.0978]..This study aims to analyze characteristics and to estimate relative frequencies of gender incongruence in a population of children and adolescents receiving gender-affirming care at a high-complexity university hospital located in the third largest city in Colombia. Methods: This was a retrospective descriptive study of patients under 18 with gender incongruence that received gender-affirming care between January 2018 and June 2022 at Fundacion Valle del Lili in Cali, Colombia. Sociodemographic and clinical characteristics of 43 patients were assessed, as well as the relative frequencies of gender incongruence. Data analysis was performed with the statistical package STATA®. To determine significant differences between the characteristics of the patients who participated in the study, the Mann‒Whitney U test was performed for numerical variables with non-parametric distribution, while either Pearson's Chi-2 test or Fisher's exact test was performed for categorical variables. Results: For every ten individuals assigned female at birth, who manifested gender incongruence, there were eight assigned male at birth. The median age of onset of gender incongruence was ten years (IQR: 5–13 years), and the median time elapsed between the reported onset of gender incongruence and the first consultation with a multidisciplinary gender-affirming team was three years (IQR: 1–10 years). The frequency of transgender identity was notable in participants with ages between 15 and 17 years. Depressive symptoms, anxiety, and psychotropic drug use were significantly higher in individuals assigned female at birth. Among 25 individuals assigned female at birth who participated in this study, 60% self-recognized as transgender men.18 individuals assigned male at birth, 67% self-recognized as transgender women. The most frequent treatment was a referral to mental health services (46.51%). Conclusion: Based on the cohort of our study, we can conclude that patients consult for gender-affirming treatment 3 years after the onset of gender incongruence. Anxiety and depression were higher in individuals assigned female at birth. Additionally, they presented at a later stage of sexual maturation, reducing the possibility of using puberty blockers. This study aimed to examine the association between breastfeeding and childhood obesity. A multinational cross‐sectional study of 4,740 children aged 9–11 years was conducted from 12 countries. Infant breastfeeding was recalled by parents or legal guardians. Height, weight, waist circumference, and body fat were obtained using standardized methods. The overall prevalence of obesity, central obesity, and high body fat were 12.3%, 9.9%, and 8.1%, respectively. After adjustment for maternal age at delivery, body mass index (BMI), highest maternal education, history of gestational diabetes, gestational age, and child's age, sex, birth weight, unhealthy diet pattern scores, moderate‐to‐vigorous physical activity, sleeping, and sedentary time, exclusive breastfeeding was associated with lower odds of obesity (odds ratio [OR] 0.76, 95% confidence interval, CI [0.57, 1.00]) and high body fat (OR 0.60, 95% CI [0.43, 0.84]) compared with exclusive formula feeding. The multivariable‐adjusted ORs based on different breastfeeding durations (none, 1–6, 6–12, and > 12 months) were 1.00, 0.74, 0.70, and 0.60 for obesity (Ptrend =.020) and 1.00, 0.64, 047, and 0.64 for high body fat (Ptrend =.012), respectively. These associations were no longer significant after adjustment for maternal BMI. Breastfeeding may be a protective factor for obesity and high body fat in 9‐ to 11‐year‐old children from 12 countries. [ABSTRACT FROM AUTHOR]

The article discusses a study which discusses the health risk associated with socioeconomic inequalities among Colombian children. Topics discussed include the association of low socioeconomic status (SES) with higher risk of cardiometabolic diseases in developed societies, the positive association of SES with obesity, and how SES is linked with abdominal adiposity.

Background: Almost all nutrition policies in Colombia currently focus on either undernutrition or obesity, with the predominant emphasis on undernutrition. It is crucial to assess the prevalence of the dual burden of malnutrition in Colombia to better target programs and policies. Objective: The aim was to estimate the national prevalence of the dual burden of malnutrition in Colombia at the individual and household levels in children aged <5 y, school-age children, adolescents, and adults. Design: This was a cross-sectional analysis from the 2010 Colombian Demographic and Health Survey and the National Nutritional Survey that included 17,696 children aged <5 y, 25,508 school-aged children, 28,328 adolescents, 89,164 adults, and 10,487 households with mothers and children aged <5 y. The dual burden of malnutrition was defined as the coexistence of overweight and stunting or anemia in the same person or household. Results: In Colombia, low to high prevalences of overweight and obesity (3.4-51.2%) coexist with moderate to high prevalences of anemia (8.1-27.5%) and stunting (13.2%). The observed prevalence of the dual burden was lower than expected. Approximately 5% of households had at least one stunted child aged <5 y and an overweight mother compared with an expected prevalence of 6.9% (P < 0.001). Among school-aged children, 0.1% were classified as stunted and obese and 1.4% were both anemic and overweight compared with expected prevalences of 0.5% (P < 0.001) and 1.5% (P = 0.037), respectively. Among 13- to 49-y-old women, 3.4% had anemia and were overweight compared with an expected prevalence of 3.5% (P = 0.038). Conclusions: National estimates of the dual burden of malnutrition in Colombia are lower than expected. Despite the independence of the occurrence of these conditions, the fact that the dual burden coexists at the national, household, and intraindividual levels suggests that public policies should address both conditions through multiple strategies. It is imperative to evaluate the current nutrition policies to inform malnutrition prevention efforts in Colombia and to share lessons with other countries at a similar stage of nutritional transition. Introduction: Roy, in his adaptation model, suggests that a contextual stimulus as a nursing intervention generates in the human system an adaptive response that can be effective or ineffective. Objective: To describe and analyze the adaptive response in the physiological mode in infants 2-23 months of age with low cardiac output in critical condition, when performing daily nursing interventions in the pediatric intensive care unit. Method: Observational-descriptive quantitative approach and repeated measures design. The proposed indicators of positive physiological adaptation measured were: systolic blood pressure, heart rate and capillary refill. The sample (n = 75) was made up of children and infants. It was divided into three groups according to the severity of low cardiac output (n = 25). Statistical analysis: nonparametric analysis in SPSS 15 Wilcoxon, Kruskal-Wallis and chi-square. Boxplot graph analysis. Result: The nursing intervention (change of position) has a long and significant negative effect (p <0.05) in the physiological adaptive capacity of infants in critical condition; It depends on the severity of low cardiac output: the greater severity of low cardiac output, the greater the presence of ineffective adaptation. Conclusions: It is possible to quantify the physiological adaptability of a human system through sensitive and specific indicators of positive physiological adaptation. The technique, duration and frequency of changes in position of the infant with low cardiac output in critically condition should be a nursing decision based on hemodynamic response and care needs of each patient. Background: Iron deficiency and poor linear growth are common in infants from deprived socioeconomic backgrounds and may be associated with inadequate complementary feeding (CF) practices. Objective: We tested the hypothesis that new CF guidelines emphasizing meat as a source of iron and zinc would improve linear growth, iron, and zinc status in infants living in poor socioeconomic circumstances in Bogota, Colombia. Design: A total of 85 term infants who were exclusively breastfed for ≥4 mo were randomly assigned at 6 mo of age to a control group [CG (n = 43); current advice] or intervention group (new guidelines group [NGG (n = 42); with counseling to 1) continue breastfeeding, 2) offer red meat ≥3 d/wk, and 3) offer fruit and vegetables daily]). Main outcomes were 1) linear growth from 6 to 12 mo of age; 2) hemoglobin, hematocrit, iron [serum ferritin (SF)], and zinc status at 12 mo of age; and 3) meat intake at 12 mo of age (by using a food-frequency questionnaire). Results: A total of 38 infants/group provided data at 12 mo of age. NGG infants had significantly higher red meat intake [mean 6 SD: 5.4 ± 1.8 compared with 3.5 6 1.7 d/wk at 12 mo of age; P, 0.001), higher hemoglobin and hematocrit at 12 mo of age, and a significantly greater increase in hemoglobin (mean ± SD change: 0.41 ± 0.8 compared with -0.13 ± 1.0; P = 0.01) and hematocrit (1.04 ± 2.2 compared with -0.15 ±2.4; P = 0.03) from 6 to 12 mo of age than those in CG infants. There were no significant differences in linear growth from 6 to 12 mo of age or in SF or zinc. Conclusions: The new guidelines showed efficacy with higher red meat intake and positive effects on hemoglobin and hematocrit. The intervention was acceptable and affordable for most mothers. These preliminary results suggest that the intervention merits investigation in a larger cohort with longer-term follow-up. Introduction: Hearing loss is a frequent problem in childhood with an incidence of about one case per 1000 births. Control of deafness should be aimed at prevention and early diagnosis in efforts to provide appropriate treatment and stimulate adequate communication in children affected. The objective of this study was to determine the prevalence of different etiologies among deaf children with a diagnosis of prelingual sensorineural hearing loss referred to the Fundación CINDA in Bogotá, Colombia, between 1997 and 2008. Materials and methods: The medical records were selected from those with prelingual hearing loss. Information was gathered in a format containing variables related to the risk factors suggested by the Joint Committee of Infant Hearing. Results: We studied 254 children; boys and girls were equally distributed. The most common etiological diagnosis was «unknown cause», followed by genetic causes (31 cases), and 38 cases from TORCH infections (toxoplasmosis, others - syphilis, rubella, cytomegalovirus, herpes), with rubella as the most common cause. Conclusions: Review of prenatal, perinatal, and postnatal history often reveals the cause of the deafness in children; therefore, appropriate evaluation of pregnant mothers could result in decreased frequency of deafness in children in our country.
