Orbital Compartment Syndrome (OCS) represents a critical medical emergency characterized by heightened orbital pressure, jeopardizing blood flow and posing a risk to the optic nerve. Often stemming from surgical procedures such as craniotomies for arachnoid cyst resection, OCS necessitates swift diagnosis and intervention. We report the case of a 77-year-old female in São Paulo, Brazil, who underwent left pterional craniotomy for arachnoid cyst resection. Postoperatively, she manifested reduced visual acuity in the left eye, restricted ocular movement, and peripalpebral hematoma indicative of orbital compression syndrome. Computed tomography scans confirmed pneumocephalus and pneumo-orbital complications, prompting an immediate diagnosis of OCS. Immediate interventions, including lateral canthotomy and cantholysis, were employed, resulting in successful decompression and substantial symptom improvement within a 10-day period. This case underscores the significance of preoperative imaging, prompt diagnostic measures, and collaborative postoperative care in managing orbital complications following neurosurgery.

Background: The relationship between superinfection by multidrug-resistant Gram-negative bacteria and mortality among SARS-CoV-2 hospitalized patients is still unclear. Carbapenem-resistant Acinetobacter baumannii and carbapenemase-producing Enterobacterales are among the most frequently isolated species when it comes to hospital-acquired superinfections among SARS-CoV-2 patients. Methods: Herein, a retrospective study was carried out using data from adult patients hospitalized for COVID-19. The interaction between in-hospital mortality and rectal carriage and superinfection by carbapenemase-producing Enterobacterales and/or carbapenem-resistant Acinetobacter baumannii was assessed. Results: The incidence of KPC-producing Klebsiella pneumoniae and/or carbapenem-resistant Acinetobacter baumannii rectal carriage was 30%. Bloodstream infection and/or pneumonia due to KPC-producing Klebsiella pneumoniae and/or carbapenem-resistant Acinetobacter baumannii occurred in 20% of patients. A higher Charlson comorbidity index (OR 1.41, 95% CI 1.24–1.59), being submitted to invasive mechanical ventilation/ECMO ≥ 96 h (OR 6.34, 95% CI 3.18–12.62), being treated with systemic corticosteroids (OR 4.67, 95% CI 2.43–9.05) and having lymphopenia at the time of admission (OR 0.54, 95% CI 0.40–0.72) were the features most strongly associated with in-hospital mortality. Conclusions: Although KPC-producing Klebsiella pneumoniae and/or carbapenem-resistant Acinetobacter baumannii rectal carriage, and/or bloodstream infection/pneumonia were diagnosed in a remarkable percentage of COVID-19 patients, their impact on in-hospital mortality was not significant. Further studies are needed to assess the burden of antimicrobial resistance as a legacy of COVID-19 in order to identify future prevention opportunities.

Background: Rheumatic heart disease (RHD) is the most serious manifestation of rheumatic fever, which may also affect the brain. The current study assessed the prevalence of neuropsychiatric manifestations in patients with RHD, including clinical features associated with basal ganglia motor dysfunction (BGMD). Methods: We conducted neurologic and psychiatric assessments in consecutive patients with RHD referred to a tertiary center for heart valve diseases. Echocardiography was performed to assess the pattern of valvular involvement and RHD severity. Validated questionnaires for the evaluation of cognition, depression, anxiety, and obsessive-compulsive symptoms (OCS) were applied. BGMD was clinically defined by the presence of hyperkinetic movement disorders. Results: Fifty patients with age of 43.2 ± 10.8 years, 84% female, were included. Mitral valve was affected in 47 patients (94%), and 21 of them (42%) also had aortic valve involvement. Chorea (22%), chronic tics (18%), OCS (48%), major depression (34%), generalized anxiety disorder (54%), cognitive complaints (66%), migraine (52%) and seizures (18%) were frequently reported. The factors associated with BGMD were age (p = 0.018), major depression (p = 0.013), and Yale-Brown Obsessive Compulsive (Y-BOCS) score (p = 0.011). The severity of heart disease was not associated with BGMD. Conclusions: Neuropsychiatric manifestations are frequent in RHD patients, which may persist up to three decades after acute rheumatic fever. Age, major depression and severity of OCS were independently associated with BGMD. These manifestations deserve a close attention of clinicians and researchers dealing with adult patients with RHD.

marginal zone b-cell lymphomas ,orbital lymphoma ,proptosis ,Ophthalmology ,RE1-994 - Abstract
Orbital lymphomas are rare conditions, and most non-Hodgkin's lymphomas of the orbit are extranodal lymphomas of B cells of the marginal zone of the mucosa-associated lymphoid tissue type (MALT). In this report, we describe a case of MALT-type marginal zone B lymphoma in the orbital region. A female patient, 45 years old, searched for ophthalmological care, complaining, for 15 years, of low visual acuity (VA) associated with nonpulsatile bilateral proptosis, VA of 20/30 in the right eye and 20/40 in the left eye, without other changes. Magnetic resonance imaging of the brain and orbits showed solid expansive lesions in both orbital spaces, with biopsy and immunohistochemical study of the left orbit, showing lymphoproliferative syndrome, favoring B lymphoma of the MALT-type marginal zone. Thus, in view of the nonspecific initial presentation of MALT lymphoma, the use of imaging tests, as well as microsurgical biopsy, is essential for early diagnosis and surgical planning and constitutes the gold standard for adequate diagnostic elucidation.

Market liberalization has been carried out by many developing countries in the hopes of stimulating trade, investment and technology transfer. In order to analyze the impacts of liberalization on a specific industry sector, this paper compares the experiences of Brazil and Mexico in liberalizing the computer industry in the 1990s. The authors conclude that liberalization leads to lower prices and more rapid diffusion of computer use throughout the economy, but at a cost to domestic computer firms who were harmed by foreign competition. Both countries saw an increase in computer production, but Mexico's production was mainly for export to the United States, while Brazil was producing for the domestic market. The differences between outcomes in the two countries have been determined more by environmental factors than by the nature and pace of liberalization. © 2001 Elsevier Science Ltd.

- Abstract
Using unique features of a tariff schedule for natural gas in Buenos Aires, we find that a price increase causes a prompt and significant decline in residential consumption. The results also indicate that consumers respond more to past bills-actual cost paid-than to expected prices. Policymakers and regulators need to move beyond the assumption of perfect awareness of complex price schedules by consumers.

Os descritores utilizados para busca foram: avaliação AND dor AND idosos AND pacientes geriátricos, demência AND comprometimento cognitivo.

This article examines the goals, instruments and characteristics of Information and Communication Technolgy (ICT) enabled innovations in the Brazilian software industry emphasizing technologies, transmitting channels, learning processes, and organizational changes required by advanced digital tools. The methodology steams from the adaptation of an existing taxonomy for our specific purposes and the development of a field research in Brazil, with the help of the software Atlas.ti. We found that ICT enabled innovations are not isolated events, since they affect existing processes, the relationship with clients and suppliers, and require greater integration of organizational functions. Important differences were observed due to the firms’ size, internationalization level, technical capabilities and access to proprietary technology.

The generation of free radicals is a physiological event resulting mainly from the cellular respiration process and the overactivation of the NOX leads to an excess production of ROS that is associated with oxidative stress. Chrysobalanus icaco, a medicinal plant that belongs to the Chrysobalanaceae family, possesses a high number of polyphenols, including phenolic acids and flavonoids. Due to its phytochemical composition, this study aimed to evaluate the antioxidant potential of the hydroalcoholic extract from the leaves of Chrysobalanus icaco (HECi) and the inhibitory potential of its main flavonoids against NOX. The in silico predictions of absorption, distribution, metabolism, excretion, and toxicity (ADMET), drug-likeness properties, and molecular docking with the enzyme NOX (PDB code 2CDU) were also performed to support the experimental results. The phytochemical screening of the HECi showed the presence of phenols and flavonoids. HECi performed an excellent antioxidant activity (IC50 = 8.1 μg/mL), probably due to its rich phenolic (220.11 ± 0.4 mg GAE/g) and flavonoid (110.98 ± 0.37 mg QE/g) constitution. The ADMET prediction indicated that myricetin and quercetin had the best pharmacokinetic parameters. The molecular docking results showed that myricetin and especially quercetin had strong docking score on NOX (ΔG = –8.1 kcal/mol and ΔG = –8.3 kcal/mol, respectively). Frontier Orbital’s analyzes (HOMO and LUMO) suggested that quercetin has better antioxidant properties than myricetin. Our results demonstrate for the first time the in silico action of quercetin against NOX, as well as reiterate the antioxidant potential of C. icaco.

Software and information services (SIS) have become a field of increasing opportunities for international trade due to the worldwide diffusion of a combination of technological and organizational innovations. In several regions, the software industry is organized in clusters, usually referred to as "knowledge cities" because of the growing importance of knowledge-intensive services in their economy. This paper has two primary objectives. First, it raises three major questions related to the attractiveness of different cities in Argentina and Brazil for hosting software companies and to their impact on local development. Second, a new taxonomy is proposed for grouping clusters according to their dominant business segment, ownership pattern and scope of operations. The purpose of this taxonomy is to encourage further studies and provide an exploratory analytical tool for analyzing software clusters.

- Abstract
How much do schools differ in their effectiveness? Recent studies that seek to answer this question account for student sorting using random assignment generated by central allocation mechanisms or oversubscribed schools. However, the resulting estimates, while causal, may also reflect peer effects due to differences in peer quality of non-randomized students. We exploit universal random assignment of students to high schools in certain areas of South Korea to provide estimates of school effects that may better reflect the effects of school practices. We find significant effects of schools on scores in high-stakes college entrance exams: a 1 standard deviation increase in school quality leads to 0.06-0.08 standard deviations higher average academic achievement in Korean and English languages. Analogous estimates from areas of South Korea that do not use random assignment, and therefore include the effects of student sorting and peer effects, are substantially higher.

Intellectual property rights aim at providing incentives for innovation, but can also constitute a burden for technological diffusion. This article discusses the conflicts arising from software patent protection, an area in which the assets are intangible and replicable at almost no cost. Current practices of intellectual property (IP) protection in Latin America are discussed based on their efficiency in stimulating information technologies innovation and diffusion. The methodology adopted includes the analysis of regional software industry development, firms' strategies to protect IP, legislation and the examination of software patents submission at the Brazilian IP Office (INPI). It concludes that harmonizing conflicting interests among existing and new business models and international agreements is a major task that goes beyond pure technical considerations.

This paper reviews the legal framework for intellectual property protection for software as a product in the USA, European Union and Brazil. Rapid technological change in the world software industry poses new challenges for existing intellectual property regimes. The USA has responded with a flexible interpretation of property rights which in fact favors the software industry. In the European Union, the larger heterogeneity of actors and interests resulted in a fail in the attempt to approve a unified directive. Brazil follows the rules agreed in the TRIPS but also faces difficulties for harmonizing incentives for innovation with conducts which head to social and economic development.

The structure of the world industry is constantly affected by technological innovations and by an institutional dynamics which destroy and create firms and markets. Historically, the increasing speed of these changes has demanded a continuous reformulation of the economic theories dealing with the firm. This article examines such theories under the analytical framework of technological change in three paradigms: (i) the British industrial revolution which dominated the world economy during the XIX century and was a starting point to the development of the neoclassical microeconomic theory; (ii) the fordist paradigm which gave rise to the industrial economics; and (iii) the information technology paradigm, in which theories are still under development, based mainly on evolutionist and neo-institutionalist frameworks. The analysis of the evolution of the theory of the firm and its relationships with different organizational paradigms shows that theories are based by different methodological affiliations, focus on different aspects (production or transactions) and are based on a diversity of institutional, sectorial and historical contexts.

Sydenham's chorea is an autoimmune chorea emerging after a group A beta-hemolytic streptococcal (GABHS) infection, i.e. a rheumatic chorea with or without the presence of carditis or arthritis. The disorder, defined by the presence of chorea, is also associated with cognitive and behavioral symptoms, including emotional lability, anxiety, depressive and obsessive-compulsive symptoms. The authors review the pathophysiology, clinical characteristics, and available evidence on therapeutic strategies, the latter including the secondary prevention of GABHS infections, reduction of chorea, and immune modulation. Sydenham's chorea has been regarded as a model for pediatric autoimmune neuropsychiatric disorders, however, the field is marked by conflicting results and controversies. Regarding therapeutics, there are limited high-quality interventional studies and the selection of treatment strategy often relies on the clinician's experience. A serial treatment algorithm is presented based upon the severity of clinical presentation and response to symptomatic pharmacotherapy.

The rapid diffusion of information technologies and the Internet gave birth to radical interpretations about their worldwide economic impacts. Three hypotheses widely discussed in the specialized literature are particularly interesting for their relevance for economic analysis and will be reviewed in this article based on empirical research on electronic commerce (EC) conducted by the Globalization and Electronic Commerce project at the University of California, Irvine. The first hypothesis states that EC diffusion is both a driving force and a consequence of the globalization process. The second is that new IT is giving birth to a "new economy" based on new information-based firms. Finally we analyze the hypothesis that EC is reducing the competitive disadvantages of small firms face their larger counterparts, thanks to informational and geographical impacts. Based on a survey with 200 firms, the article concludes that, at least in the case of Brazil, the findings do not support those hypotheses and that despite the undisputed importance of EC for business development, the technology itself determines neither industry structure or firms globalization strategies. Rather, EC helps firms and countries to further explore their existing economic trajectories.

This paper uses municipal-level data from South Africa for the period 1996–2011 to estimate the medium to long-run effects of trade liberalization on local labor markets. It finds that local labor markets that were more exposed to tariff cuts tended to experience slower growth in employment and income per capita than less exposed regions. The longer-term effects of trade liberalization on regional earnings are stronger than the medium-term effects, and tend to be more pronounced among municipalities that included the former homelands.

This paper examines the welfare effects of international trade on workers in a new dynamic general equilibrium discrete choice model of labor mobility, where the workers’ choice set of jobs is endogenous. The analysis exploits differential exposure of sectors and regions to destination-specific demand shocks to estimate the impacts of exports on wages, employment, and labor mobility, using employer-employee panel data for Brazil. It then employs the same empirical strategy to estimate structural parameters and the different components of changes in model-implied worker welfare. Counterfactual simulations show that the endogenous number of job options significantly magnifies the welfare effects of trade shocks.

Protein functional annotation consists in associating proteins with textual descriptors elucidating their biological roles. The bulk of annotation is done via automated procedures that ultimately rely on annotation transfer. Despite a large number of existing protein annotation procedures the ever growing protein space is never completely annotated. One of the facets of annotation incompleteness derives from annotation uncertainty. Often when protein function cannot be predicted with enough specificity it is instead conservatively annotated with more generic terms. In a scenario of protein families or functionally related (or even dissimilar) sets this leads to a more difficult task of using annotations to compare the extent of functional relatedness among all family or set members. However, we postulate that identifying sub-sets of functionally coherent proteins annotated at a very specific level, can help the annotation extension of other incompletely annotated proteins within the same family or functionally related set. As an example we analyze the status of annotation of a set of CAZy families belonging to the Polysaccharide Lyase class. We show that through the use of visualization methods and semantic similarity based metrics it is possible to identify families and respective annotation terms within them that are suitable for possible annotation extension. Based on our analysis we then propose a semi-automatic methodology leading to the extension of single annotation terms within these partially annotated protein sets or families.

- Abstract
Context: Epilepsy affects more than 50 million people worldwide, which is an important cause of morbidity and functional limitations. Cortical development malformations represent about 8% of epilepsy cases, and are associated with cognitive deficits, that are frequently diagnosed in childhood. Case report: L.G.M, female, 35 years old, was attended in an emergency department with psychotic symptoms, aggressiveness and lowering of the sensorium. Computed Tomography (CT) scan of the brain evidenced hypodensity and loss of cortico-medullary differentation in the left temporo-parietal region. The neuroimaging findings motivated the referral to our service for investigation. Upon admission, has been shown a history of frequent focal motor and non-motor seizures associated with cognitive deficit since the age of 12. During hospitalization, a Magnetic Resonance Imaging (MRI) of the brain was performed, which showed subependymal heterotopy of gray matter (Figures 1 and 2). Since then, the monotherapy treatment with carbamazepine aiming at seizure control was chosen. Discussion and conclusion: Cortical development malformations can be classified into three groups of abnormalities, such as: 1) neuronal and glial proliferation and apoptosis; 2) neuronal migration; 3) cortical organization. A heterotopy of the gray matter is related to the migration disorder of the germinal matrix neurons on the wall of ventricle lateral to the cortex. It is the most frequent anomaly of cortical development. The perception of cognitive deficit associated with epileptic seizures should always awaken to the need for early investigation by image examination, in particular brain MRI, in order to diagnose possible malformations of cortical development.

- Abstract
Context: COVID19 has better known respiratory impacts than cardiovascular1 and high D-dimer as the most significant coagulation parameter2 . Otherwise, paradoxical embolism due to Patent Foramen Ovale (PFO) and Ischaemic Cerebral Vascular Accident (iCVA) mechanisms associated are poorly documented3, 4 . We aimed to report an associated case of iCVA and PFO and highlight COVID19 hypercoagulability triggering thromboembolisms. Case report: CTT, 76y, female, hypertensive, former-smoker was hospitalized with right-hemiplegia, transcortical aphasia, dysarthria by iCVA and evolved with headache, odynophagia, fever, chills. RT-PCR-SARS-Cov2: positive; chestCT: 25% bilateral pulmonary involvement, ground-glass opacities. 8days later, was transferred to ICU-COVID-HU-UFJF; D-dimer=827. In 30days, went to ward with O2-dependent pulmonary sequelae; CT-angiography excluded Pulmonary Thromboembolism. Searched iCVA mechanism, ECO detected PFO=2.8mm and Interatrial Septum aneurysm. Prescribed anticoagulants. Subsequently, presented right-clonus, further previous RankinScale=5. Requested brain-MRI pointed lobar hemorrhage with mass effect in recent iCVA’s territory (left-Middle Cerebral Artery). After 10days, a head-CT evidenced partial resorption and reduced mass effect. Patient was discharged taking rivaroxabana. Conclusions: Retrospectively, we suspected that COVID19 hypercoagulability triggered Deep Vein Thrombosis and the consequent PFO paradoxical embolism, which caused iCVA. Therefore, vascular pathologies in COVID19 deserve further studies. Treatment for secondary prevention in iCVA by PFO is uncertain3 .

- Abstract
Background: Alzheimer’s disease (AD) represents one of the main causes of cognitive and functional decline in the world. Concomitant with pharmacological treatment, the practice of aerobic exercises (AE) can help in the symptomatic control of the disease. Objectives: To evaluate the effects of AE on activities of daily living and cognition in patients with AD. Methods: A systematic review was undertaken. EMBASE, Pubmed and BVS databases were searched using the terms “Alzheimer disease”, “Alzheimer syndrome” and “Alzheimer dementia”; “aerobic” and “exercise”. The inclusion criteria were: randomized controlled trials from 2016 to 2021, English language studies and human studies. Among 854 studies found, six were included in the review. Results: The potential benefits of AE training in AD patients are: improvement of functioning, quality of life and cognitive performance; better control of neuropsychiatric symptoms and possible reduction of systemic inflammation. Conclusions: AEs are associated with cognitive and functional performance gain in AD, probably related to synaptic plasticity optimization and improvement of the feeling of well-being. Although AEs may improve cognitive and neuropsychiatric symptoms, the response to treatment is individual. Future longitudinal studies with larger cohorts and functional neuroimaging studies are required for a better understanding of the real benefit of AE in AD.

- Abstract
Background: Headache is the most frequent neurological complaint in the population and the group of tension-type headaches (TTH) is the most prevalent subtype. Nevertheless, more information about the clinical features of headaches in patients attended at specialized centers are demanded. Methods: Cross-sectional, descriptive study. Data from patients referred to an outpatient specialized headache center from 2018 to 2019 were analyzed and clinical and epidemiological information was collected. This study was authorized by the research ethics committee of the HUUFJF (CAAE 03530818.9.0000.5133). Results: Data from 153 patients were assessed. The mean age of patients was 45,6 years and most cases were women (80,4%). The most frequent diagnosis were migraine (49,7%), TTH (22,8%) and temporomandibular disorders (8,5%). The prevalence of chronic headaches was 46,6%. Analgesic abuse was identified in 32% of participants, with a higher prevalence in women (Fischer’s exact test, P=0,05). Prophylactic treatment was used by 84,3% of the subjects. Pain was self-reported mild in 21,6% of cases, moderate in 30,1% and severe in 47,1%. The pain severity was inversely proportional to age (P=0,012). The most frequent associated symptoms were photophobia (57,5%), phonophobia (56,9%), nausea/ vomiting (47,1%). Conclusions: The findings show important differences in the prevalence of headache cases in specialized centers compared to the general population. Given the high prevalence of analgesic abuse reported, the development of effective educational programs for patients and healthcare providers at primary and secondary health services, might reduce the social burden of chronic headaches and decrease the demand for consultations on specialized headache clinics.
