The construction of sub‐daily pressure series is described for the cities of London (GB) and Paris (FR). The series extend back 1692 and 1748, respectively, and as such they represent two of the longest sub‐daily series of barometric pressure available. These series are updated from the previously documented London and Paris daily series and offer more homogeneous series, and in the case of the London series a more temporally complete sequence of data. A pairwise homogenization procedure has been applied to the two series alongside the long series of pressure that exists for De Bilt (NL). The De Bilt series has been available for some time in the International Surface Pressure Dataset (ISPD), but further quality control and homogeneity‐checking procedures have been applied to the data in this paper and therefore the three series are released together in this dataset. The series are of immediate interest for understanding changes to storm activity across the English Channel and North Atlantic region over an extended timeframe but may also be assimilated into reanalysis datasets such as the 20th‐century reanalysis. This paper is based on research commissioned by the Wellcome Trust in 2015 and catalogues current initiatives and trends in the systems and processes surrounding peer review. It considers issues such as open and interactive reviews, post-publication comments and ratings, and the platforms provided by both publishers and other organisations to support such activity; third-party peer review platforms; and measures from publishers and others to provide more recognition and rewards for peer reviewers. Adaptations to flood‐proof individual properties (referred to here as property‐level adaptation, PLA) provide a potential means of reducing flood risk at isolated properties, whilst conventional community‐scale flood protection (CSFP) is usually more economical in protecting high‐density urban spaces. This paper develops a risk‐based framework to identify the tipping point when PLA measures become more cost‐beneficial when compared to CSFP in different urban densities. The framework was demonstrated using a hypothetical case study based on a residential area in Teddington, London. Sensitivity analysis was performed by varying the building densities in the urban space. Results show that PLA can have a role to supplement CSFP even in dense urban areas if the CSFP standard of protection is low. However, adding some element of CSFP to PLA can be more cost‐effective than implementing a single higher protection standard of PLA. Given the unique flood risk condition of most urban spaces, and the sensitivity of cost‐effectiveness of flood adaptation measures such as those demonstrated in this work, this approach can provide additional information to assist decisions in finding a sensible portfolio of measures that match that risk condition. Introduction: The proliferation of providers and practitioners of cosmetic botulinum toxin and dermal filler has profound public health implications. The Advertising Standards Authority (ASA) regulates the use of advertising materials in the United Kingdom and prohibits the promotion of prescription‐only medicines. Aims: We aim to perform a cross‐sectional analysis of the practitioners in London, UK to evaluate the distribution of clinics within Greater London, prices advertised for interventions, and compliance with the ASA code. We also aim to identify whether there are any differences in cost of botulinum toxin or dermal filler between the boroughs. Methods: Between December 2021 and January 2022, we performed a systematic search using the internet search engine Google. Five searches were performed (1) [london] botox, (2) [london] botulinum toxin, (3) [london] anti wrinkle injection, (4) [london] filler, (5) [london] dermal filler. One hundred websites per search string were systematically reviewed and those which met the inclusion/exclusion criteria of each search string were included and analyzed. Each clinic's product/service range compliance with the ASA/CAP code was assessed. Any reference to Botulinum Toxin or anti‐wrinkle injections was noted and analyzed. Further analysis would look to calculate price per milliliter (mL) of botulinum toxin and dermal filler per borough and to calculate whether there were any statistical differences between the 32 different London boroughs. Results: A total of 500 websites were visited and evaluated. After removal of duplicates, a total of 233 independent clinics was identified. A total of 206 out of the 233 clinics sampled (88%) were in direct infringement of the enforcement notice through advertising a prescription medicine. The overall average cost per mL of dermal filler was £330.89 and there was a statistically significant variance across London boroughs (p < 0.05). The overall average cost per mL of Botulinum Toxin was £284.45 and the variance across London boroughs was close to significant (p = 0.058). Conclusion: This paper demonstrates poor compliance with the ASA/CAP guidelines and further provides an insight into the industry mechanics associated with aesthetic injectables in a major UK city, identifying regional variance in price and clinic density. The advertising of prescription‐only medication may pose a potential risk to patients and will be an important consideration in proposed legislation to introduce licensing to the industry. Pharmacogenomics (PGx)‐based personalized medicine (PM) is increasingly utilized to guide treatment decisions for many drug‐disease combinations. Notably, London Health Sciences Centre (LHSC) has pioneered a PGx program that has become a staple for London‐based specialists. Although implementational studies have been conducted in other jurisdictions, the Canadian healthcare system is understudied. Herein, the multistakeholder perspectives on implementational drivers and barriers are elucidated. Using a mixed‐method qualitative model, key stakeholders, and patients from LHSC's PGx‐based PM clinic were interviewed and surveyed, respectively. Interview transcripts were thematically analyzed in a stepwise process of customer profiling, value mapping, and business model canvasing. Value for LHSC located specialist users of PGx was driven by the quick turnaround time, independence of the PGx clinic, and the quality of information. Engagement of external specialists was only limited by access and awareness, whereas other healthcare nonusers were limited by education and applicability. The major determinant of successful adoption at novel sites were institutional champions. Patients valued and approved of the service, expressed a general willingness to pay, but often traveled far to receive genotyping. This paper discusses the critical pillars of education, awareness, advocacy, and efficiency required to address implementation barriers to healthcare service innovation in Canada. Further adoption of PGx practices into Canadian hospitals is an important factor for advancing system‐level changes in care delivery, patient experiences, and outcomes. The findings in this paper can help inform efforts to advance clinical PGx practices, but also the potential adoption and implementation of other innovative healthcare service solutions. Aim: To determine the predictors of magnitude of change in response to a participation‐focused leisure‐time physical activity intervention in children with cerebral palsy (CP) using the ParticiPAte CP protocol. Method: We included 33 children (16 males, 17 females) aged 8 to 12 years (mean age=10y, SD=1y 6mo) with CP with pre/postintervention data from a wait‐list randomized trial. The hypothesized linear predictors of change in primary outcomes (Canadian Occupational Performance Measure [COPM]‐performance and COPM‐satisfaction, Belief in Goal Self‐Competence Scale (BiGSS), and minutes per day moderate‐to‐vigorous physical activity [MVPA]) were: age; Gross Motor Function Classification System level; comorbid autism spectrum disorder (ASD); Goal Attainment Scaling T score; Problems in Schools Questionnaire; Physical Activity Climate Questionnaire; Motives for Physical Activities Measure‐Revised; and stage of behaviour change. Multivariable models were selected using the Bayesian information criterion. Results: Overcoming barriers to participation, age, and comorbid ASD explained 49% of the variance in change in COPM‐performance. Being motivated by interest and/or enjoyment and age explained 32% of the variance in change in COPM‐satisfaction. Being motivated by physical activity competence or appearance (extrinsic motivation) explained 24% of the variance in change in BiGSS. Parental autonomy supportiveness, overcoming barriers to participation, appearance motivation, and baseline MVPA explained 59% of the variance in change in MVPA. Interpretation: These findings support a behaviour paradigm for conceptualizing physical activity in children with CP. Outbreaks of infectious diseases bring behavior and policy responses into sharp focus since societies face acute constraints and uncertainties. This paper compares two infectious disease outbreaks: the Covid‐19 pandemic and the 1665 London plague outbreak described by Daniel Defoe in A Journal of the Year of the Plague published in 1722. We compare three aspects: individual behavior, social behavior, and governance and find striking similarities in behavior in spite of these events being separated by 350 years. We contend that the same models of behavior can be used to explain human responses during such outbreaks regardless of when they occur. [ABSTRACT FROM AUTHOR]

Due to the cell size decrements and limited sojourn time on a high speed train mobility, unnecessary handovers (HOs) occur, which can lead to higher network communication costs, and affect passengers quality of service (QoS). This paper proposes a novel blockchain‐enabled privacy preserving HO skipping framework by using train mobility dataset from the city of London. Using a complex dataset parameters, passenger traffic flows are modelled by averaging various train lines and station's footfall numbers utilising blockchain to maintain privacy. The framework stores pseudonym addresses in order to track the path of users. The proposed framework allows for a better trade‐off in terms of 2% (approx.) gain in average throughput, over 100% gain in the last‐hop signal quality, and a 50% reduction in HO costs, while also addressing the needs for resources to operate the blockchain. Aim: To investigate the utility of the Insight Inventory (a structured clinical inventory completed by caregivers) for assessment of children with cerebral visual impairment; and to investigate effectiveness of tailored habilitational strategies derived from the responses to the Insight Inventory.Method: Fifty-one eligible children (26 males, 25 females; mean age 9y 5mo, SD 3y, range 5-16y) were recruited from Great Ormond Street Hospital, London. They underwent baseline assessment including neuro-ophthalmological and neuropsychological evaluations, and parent- and child-reported ratings on a questionnaire-based measure of quality of life. Parents also completed the Insight Inventory. On the basis of responses to the Inventory, families received individualized habilitational strategies. Follow-up assessments 6 months later included repeating the Insight Inventory and quality of life questionnaires.Results: Correlations were found between the Insight Inventory and the Wechsler Intelligence Scale for Children, Fourth Edition, the Beery-Buktenica Test of Visual-Motor Integration, and the Benton Facial Recognition Test, suggesting that the Insight Inventory is an effective tool to estimate visual-perceptual difficulties. At 6 months follow-up, caregiver reports indicated significant improvements in the quality of life of children below the age of 12 years.Interpretation: The Insight Inventory is a simple questionnaire which covers practical aspects of cognitive visual function in everyday life. It provides in-depth information about the aspects that children struggle with. It can also guide programmes of individualized habilitation strategies, which may enhance the quality of life of younger children.What This Paper Adds: Questionnaire scores demonstrate biologically plausible correlations with formal neuropsychological tests of visual function. There is a longstanding literature on the unequal geographical distribution of welfare. In this paper we argue that increasingly geography is becoming the basis for rationing access to some forms of welfare. Focusing on access to secondary schools in East London, England, where the demand for places at the more popular schools generally far exceeds the number of places available, we show how distance from school has now become the primary means of allocating places. Rather than educational resources attempting to compensate for geographical disadvantage, geography (in the form of distance from school) has become the rationale by which those living in advantaged areas continue to have privileged access to educational resources. Whereas previously the role of the state was to compensate for the unfairness of such geographical inequalities, geography (via distance to school) is now used to justify the unequal allocation of scarce school places. The paper demonstrates that not only does the near universal adoption of distance-based allocation policies in East London lead to the reproduction of social advantage and disadvantage, but also it is creating new hierarchies of school popularity and more important unpopularity which are not always clearly related to issues of school attainment. This paper considers the relations between practice, knowledge and context in design consultancies. It uses a case study of design consultancies in London based on in-depth interviews with designers working in design agencies in the city. The paper argues that the relations between design knowledge and context have been conceived in two ways. The first account emphasises the sharing of tacit knowledge in a design community marked by relatively strong and durable social ties. The second approach argues instead that design is a creative collectivity with much weaker social ties and a broader range of types of knowledge. It is argued that while both of these approaches illuminate parts of situated learning and context in design consultancies, these are actually more complex and mixed than either account recognises. Both accounts overlook the crucial importance of what are termed medium-strength ties between designers and their clients that combine market contracts with personal regard and friendship. This paper explores the emergence of a new ‘migrant division of labour’ in London. In contrast to a vision of ‘professionalization’, it shows that London's labour market has been characterized by processes of occupational polarization and that a disproportionate number of London's low-paid jobs are now filled by foreign-born workers. Drawing on original survey data, the paper explores the pay and conditions of London's low-paid migrant workers and develops a framework for understanding the emergence of a new migrant division of labour in London. In particular, the paper stresses the role of the British state in shaping this divide. The paper concludes that the emergence of such a divide in London necessitates a re-conceptualization of the place of migrant workers in the ‘global city’ and of the processes shaping global city labour markets, and outlines what this new division of labour might mean for politics and policy in London. The covered interest parity theorem states that the covered interest differential between two identical assets denominated in different currencies should be zero. Profitable deviations from the parity represent riskless arbitrage opportunities and so indicate market inefficiency. Previous empirical work on this topic has used data that have not been contemporaneously sampled, and does not therefore constitute a proper test of covered interest parity (since those prices never appeared in the market simultaneously). In this paper the covered interest parity condition is tested using high-frequency, contemporaneously sampled data gathered in the London foreign exchange market. Using London Stock Exchange data, we test the central implication of the canonical model of Ho and Stoll (1983) that relative inventory differences determine dealer behavior. We find that relative inventories explain which dealers obtain large trades and show that movements between best ask, best bid, and straddle are highly correlated with both standardized and relative inventory changes. We show that the mean reversion in inventories is highly nonlinear and increasing in inventory levels. We show that a key determinant of variations in interdealer trading is inventories and that interdealer trading plays an important role in managing large inventory positions. [ABSTRACT FROM AUTHOR]

Compared with the attention paid to written texts, geographers and others have neglected the spoken word in its many forms, particularly in investigations of the power relations of colonialism and imperialism. This paper argues that considering orality as a series of embodied, situated enunciations, declarations and conversations can provide a basis for historical geographies of the spoken word that engage with representation as practice. Using evidence from the domain of law within the context of Britain's plantation colonies in the Caribbean - particularly Barbados and Jamaica - this paper argues for the significance of the oral culture of empire. This was evident in the ways in which the power of speech - through the rules on oath-taking and evidence-giving - was part of the making of imperial and colonial identities and relationships dividing white and non-white, free and unfree, both within the spaces of plantation societies and in the broader British Atlantic world. The fragility of the identities and relationships made through the spoken word is also demonstrated through a series of moments when changes in the regulation of speech in courts of law were suggested and contested. There has been a plethora of literature in the last few years attempting to conceptualise how the (international) firm operates in the notion of what has been termed ‘buzz’. In this paper, we aim to highlight how the use of social network analysis (SNA) can provide a nuanced view of ‘buzz’, through a focus on London's advertising industry. In this case study, we use the data on interlocking board members of the advertising companies in London, and visualise their network maps through sociograms. This method of analysis, under-utilised in the economic geography literature, highlights the intensity of connections between companies and particular individuals. It shows the paths of knowledge flow within the industry, and can highlight the key ‘gatekeepers’ within what is already known to be a highly networked and socialised industry. This is a specific conceptualisation of interaction and provides a quantitative conception of what has hitherto been largely evaluated through qualitative means. [ABSTRACT FROM AUTHOR]

This paper examines how embodied acts of building home(s) for others shape particular constructions of cultural difference and otherness. It presents a largely unexplored perspective of home – one that is uninhabited by its builders. This perspective becomes relevant through a study of Polish home-builders who have legally entered the building sector in London since EU expansion in 2004, who put together homes for others, temporarily inhabiting them during building, and moving out as soon as the work is completed. Using participant photo-diaries and semi-structured interviews, this paper suggests that the building act – putting together of homes for others as part of a political economy of labour and materials in different localised contexts, produces particular distinctions between the ‘socio-technical’ nature of homes across national territories. In doing so, it extends different perspectives on homes from cultural geography, anthropology and architecture to suggest a more nuanced material geography of home – as an assemblage of building elements under different contexts which, through the building acts of migrant men, allow scrutiny of cultural difference and otherness. Participants’ own homes left behind in Poland, homes they build for others in London and the future homes that they aspire to, suggest how differences in the ways that they are put together are ascribed cultural meanings of dwelling that are inherently corporal. Building homes is a deeply masculine act – a sexualised and embodied politics of home, which validates particular versions of gendered ethno-national identities as Polish men, and locates the future home within a Polish home/land. Although inhabiting the future home is uncertain, building this home is seen as an extension of their identity as Polish men who ensure the continuity of the Polish home and family. Drawing from debates in economic geography on relational and organisational proximity as a substitute for geographical proximity, the paper explores characteristics of knowledge transfer in London's recorded music industry through an examination of organisational connections on local and global scales. The paper demonstrates that knowledge transfer within the industry occurs simultaneously across multiple geographical scales, with certain organisational connections facilitating the transfer of tacit knowledge across organisational boundaries. However, the paper argues that these connections do not offer the same scope for trust as is afforded by frequent face-to-face contact and therefore offer only a partial substitute for geographical proximity. This paper examines the building that presently houses the Imperial War Museum, investigating the transformation of the archetypal ‘mad space’ of the Bethlem Royal Hospital into what has been described as the ‘biggest boy’s bedroom in London’. Following recent concerns in human geography with Imperial cities, it highlights the differing ways in which this transformation embodies a number of themes of degeneration and regeneration in early twentieth-century Britain. This paper presents estimates of an equilibrium-based dynamic adjustment model of the office market, using supply and demand relationships to link construction, absorption, vacancies and rents to employment growth and real interest rates. The model is estimated using data from the City of London office market over 1977-1996. The model tracks the market dynamically, and the severe 1985-1996 cycle is shown to be related to the cycle in employment growth and the movement of real interest rates. Although a number of studies published in the economic history literature discuss the development, organization and function of the London Stock Exchange, there is little quantitative evidence on how well this market functioned. This is a significant omission, since the finance literature provides a well developed set of criteria against which the performance of capital markets may be judged. In particular, the finance literature emphasizes the importance of the efficient impounding of information into market prices. So-called 'weak-form efficiency' is the least demanding version and is therefore the logical starting point. This paper reports the results of weak-form efficiency tests of the London market for 3 per cent Consols from 1821 to 1860. The tests are based on over 10,000 daily price changes, The evidence presented here is similar to that found in contemporary markets, suggesting that the market for 3 per cent Consols was efficient in the weak-form sense. From a finance perspective, this market is of interest, given the institutional differences between it and contemporary markets. Purpose: To present the development of an in‐house coded solution for treatment planning of tangential breast radiotherapy that creates single click plans by emulating the iterative optimization process of human dosimetrists. Method: One hundred clinical breast cancer patients were retrospectively planned with an automated planning (AP) code incorporating the hybrid intensity‐modulated radiotherapy (IMRT) approach. The code automates all planning processes including plan generation, beam generation, gantry and collimator angle determination, open segments and dynamic IMRT fluence and calculations. Thirty‐nine dose volume histogram (DVH) metrics taken from three international recommendations were compared between the automated and clinical plans (CP), along with median interquartile analysis of the DVH distributions. Total planning time and delivery QA were also compared between the plan sets. Results: Of the 39 planning metrics analyzed 23 showed no significant difference between clinical and automated planning techniques. Of the 16 metrics with statistically significant variations, 2 were improved in the clinical plans in comparison to 14 improved in the AP plans. Automated plans produced a greater number of ideal plans against international guidelines as per EviQ (AP:77%, CP:68%), RTOG 1005 (AP:80%, CP:71%), and London Cancer references (AP:80%, CP:75%). Delivery QA results for both techniques were equivalent. Automated planning techniques resulted in an average reduction in planning time from 23 to 5 minutes. Conclusion: We have introduced an automated planning code with iterative optimization that produces equivalent quality plans to manual clinical planning. The article offers information on the mandatory Public Access Policies introduced by the U.S. National Institutes of Health (NIH) and the British agency Wellcome Trust for all research papers. The NIH policy requires all investigators funded by the National Library of Medicine's PubMed Central (PMC) to submit an electronic version of their final manuscripts. The Wellcome Trust policy mandates electronic copies of any research papers accepted for publication to be made available through PMC.

London’s skyline is changing significantly with a new generation of iconic buildings, of which the Swiss-Re Tower is the most well known. Despite the fact that many of these buildings are located in the City (London’s financial heart), little attention has been paid to the relationship between the transformation of London’s skyline and the recent institutional reconfiguration of the Corporation of London, the authority that runs the City. Focusing empirically on the City’s iconic architecture, and foregrounding a period of institutional crisis for the Corporation (1970–1990), the paper: first, departs from the standard analysis of iconic buildings as signifiers of economic success, and sketches a framework for examining the role of iconic architecture during moments of crisis and, second, offers a new approach to understanding the City’s iconic commissions: not as signifiers of London’s international economic power, but as symptoms of changes in the institutions and élites that promote the City’s new urbanity. The article details how the internationalisation of London’s economy after the 1970s challenged the Corporation’s insular character. The Corporation’s resistance to the ‘invasion’ of foreign companies, people and architectural styles in the City in the midst of a rapid expansion of London’s economy and growing inter-urban competition, led to open threats from the government for the abolition of the Corporation. Responding to these threats, the Corporation reinvented itself with an institutional reform and re-branded its identity in the early 2000s as an outward-looking institution, open to London’s new transnational élites. The 2002 Unitary Development Plan that introduced a new architectural language in the City corresponds to the same need to construct a new imaginary identity for a re-branded Corporation. Towering over the City’s traditional signifiers, the City’s new buildings constitute an ode to the Corporation’s new identity and a visual coup d’état against its time-old heritage-oriented planning. In this paper we explore the impacts of the training programmes offered to lone mothers with young children on the Government's ‘New Deal for Lone Parents’ in one local labour market: West London. Our research suggests that regulatory workfare policies are (re)producing and reinforcing gendered inequalities in the labour market by encouraging lone mothers to undertake training in feminised occupational areas such as childcare. We will argue that in a local economy such as West London where more childcare workers are desperately needed to enable other more highly skilled workers to take up employment opportunities, such training programmes may be doing little more than exacerbating the already gendered and class-based polarisation of the labour market – embedding low-skilled, poorly qualified lone mothers into low-paid jobs. The relationship between gentrification and globalisation has recently become a significant concern for gentrification scholars. This has involved developing an understanding of how gentrification has become a place-based strategy of class (re)formation during an era in which globalisation has changed sociological structures and challenged previously established indicators of social distinction. This paper offers an alternative reading of the relationship between gentrification and globalisation through examining the results of a mixed method research project which looked at new-build gentrification along the River Thames, London, UK. This research finds gentrification not to be distinguished by the gentrifer-performed practice of habitus within a ‘global context’. Rather, the responsibility for gentrification, and the relationship between globalisation and gentrification, is found to originate with capital actors working within the context of a neoliberal global city. In order to critically conceptualise this form of gentrification, and understand the role of globalisation within the process, the urban theory of Lefebvre is drawn upon. [ABSTRACT FROM AUTHOR]

In this paper we argue that a process of super-gentrification, similar to that first identified by Lees (2003 Urban Studies 40 2487–509) in Brooklyn Heights, New York City, is occurring in the already gentrified, inner London neighbourhood of Barnsbury. A new group of super wealthy professionals working in the City of London is slowly imposing its mark on this inner London housing market in a way that differentiates it and them both from traditional gentrifiers and from the traditional urban upper classes. We suggest that there is a close interaction between work in the newly globalizing industries of the financial services economy, elite forms of education, particularly Oxbridge, and residence in Barnsbury which is very different from other areas of gentrified inner London. In this article we develop a model of moving and location decisions, which demonstrates how cumulative processes of growth, decline and segregation occur that conflict with the aims of policies aimed at improving social mix. Abstract In recent years, a sharp divergence of London Stock Exchange equity prices from dividends has been noted. In this paper, we examine whether this divergence can be explained by reference to the existence of a speculative bubble. Three different empirical methodologies are used: variance bounds tests, bubble specification tests, and cointegration tests based on both ex post and ex ante data. We find that, stock prices diverged significantly from their fundamental values during the late 1990's, and that this divergence has all the characteristics of a bubble. [ABSTRACT FROM AUTHOR]

The international prominence of Young British art (YBa) in the 1990s gave London a contemporary art movement to match its role as one of the world's key centres of art exchange. Examining the rise of YBa in retrospect, this paper is concerned with the difference place makes in helping to shape the (hi)story of art. It is argued that London's established role as an international art centre was crucial in providing the density of networks, associations and facilities necessary to sustain an international art movement. At the same time, YBa's success can be linked to London's changing status as a cultural capital, as well as profound changes in the business of contemporary art. Public access to environmental information received much attention in the last decade. Recent developments promote access to this information through public telecommunication networks. This paper describes a Web-based survey that explored the requirements and needs of likely users of public environmental information systems for London: educated middle-class members of the public, with high interest in environmental issues and computer literacy. This paper presents the results from exploratory research which set out to investigate London children's perceptions and uses of river environments. Mainly qualitative, multi-method research was carried out in four primary schools with children aged 9-11, focusing on two rivers near to the schools. Rivers were found to be marginal to the children's everyday lives and outdoor play, being perceived as polluted, neglected, and initially dangerous places. However, when experienced on visits observed as part of the research, some of these perceptions changed and rivers were also found to afford many special activities and experiences for the children. The visits were generally seen to have a positive effect on the children's perceptions of rivers, at least in the short term. This suggests that, if managed appropriately, rivers could be potentially rewarding play areas for children. Moreover, children could provide valuable input for river managers in suggesting ways of improving suitable local rivers for increased recreational uses. [ABSTRACT FROM AUTHOR]

Introduction: Gastrointestinal dysfunction, particularly constipation, is among the most common non‐motor manifestations in Parkinson's Disease (PD). We aimed to identify high‐resolution anorectal manometry (HR‐ARM) abnormalities in patients with PD using the London Classification. Methods: We conducted a retrospective review of all PD patients at our institution who underwent HR‐ARM and balloon expulsion test (BET) for evaluation of constipation between 2015 and 2021. Using age and sex‐specific normal values, HR‐ARM recordings were re‐analyzed and abnormalities were reported using the London Classification. A combination of Wilcoxon rank sum and Fisher's exact test were used. Results: 36 patients (19 women) with median age 71 (interquartile range [IQR]: 69–74) years, were included. Using the London Classification, 7 (19%) patients had anal hypotension, 17 (47%) had anal hypocontractility, and 3 women had combined hypotension and hypocontractility. Anal hypocontractility was significantly more common in women compared to men. Abnormal BET and dyssynergia were noted in 22 (61%) patients, while abnormal BET and poor propulsion were only seen in 2 (5%). Men had significantly more paradoxical anal contraction and higher residual anal pressures during simulated defecation, resulting in more negative recto‐anal pressure gradients. Rectal hyposensitivity was seen in nearly one third of PD patients and comparable among men and women. Conclusion: Our data affirms the high prevalence of anorectal disorders in PD. Using the London Classification, abnormal expulsion and dyssynergia and anal hypocontractility were the most common findings in PD. Whether the high prevalence of anal hypocontractility in females is directly related to PD or other confounding factors will require further research. [ABSTRACT FROM AUTHOR]

Objective The measurement of health-related quality of life (HRQL) has become increasingly common in health services research. Whilst useful, its focus on behaviour, capacities and activities means that it remains relatively specific. This paper explores the possibility of extending the evaluation of health by considering the concept of patients’ expectations. Design In-depth and semi-structured interviews were used to explore the concept of expectations from the patients’ perspective. Patients’ expectations were then used in the construction of a two-part questionnaire. Settings and participants Expectations were explored with a group of 33 cardiac patients. The resulting questionnaire was given to 400 cardiac patients in a large teaching hospital in London. Results Patients identified a range of expectations which related to their health and seemed to represent the desired results of their hospital stay. Comparison of the content of patient expectations with a commonly used generic measure of HRQL, the Short-Form 36 (SF-36), found some overlap but indicated that patients seemed to adopt a broader approach to their health. Expectations that patients identified were used to construct two scales to measure expectations and their evaluation. The internal consistency of these scales was 0.82 and 0.88, respectively. Conclusion The study indicates the potential complexity of the concept of expectations and the need for further exploration. It also demonstrates the feasibility of constructing standardized scales to measure patient expectations. Whilst conceptually different from HRQL such standardized expectations scales could provide a useful adjunct to HRQL measurement and provide a meaningful context for the interpretation of HRQL data. In the second of two papers (Paper I, Geogrl J. 157(1)) the motive forces driving current development of the Central London economy are set against possible constraints on its ability to grow in the future. It is concluded that, while growth in finance and business service activity which has been the prime dynamic force within the local economy over the recent past is likely to continue, the rate of this growth depends crucially on London maintaining its position as an international financial centre in the face of competition from other European cities. Labour shortages arising from the demographic structure of its population are identified as a source of difficulty but one that can be moderated by the smooth timing of growth and an effective rail system. It is clear, however, that investment in new rail facilities will be hampered if their financing is related too closely to current revenues which are exposed to short-run variations in property values rather than to the pursuit of a longer term strategy for the development of Central London. It is argued that such a long-term strategy might include the encouragement of less essential employment to move away from Central London in order to allow greater investment in road, rail and air connections outside the central area to improve London's international accessibility. Introduction: Non‐invasive fetal electrocardiography (NIFECG) has potential benefits over the computerized cardiotocography (cCTG) that may permit its development in remote fetal heart‐rate monitoring. Our study aims to compare signal quality and heart‐rate detection from a novel self‐applicable NIFECG monitor against the cCTG, and evaluate the impact of maternal and fetal characteristics on both devices. Material and methods: This prospective observational study took place in a university hospital in London. Women with a singleton pregnancy from 28 + 0 weeks' gestation presenting for cCTG were eligible. Concurrent monitoring with both NIFECG and cCTG were performed for up to 60 minutes. Post‐processing of NIFECG produced signal loss, computed in both 0.25 (E240)‐ and 3.75 (E16)‐second epochs, and fetal heart‐rate and maternal heart‐rate values. cCTG signal loss was calculated in 3.75‐second epochs. Accuracy and precision analysis of 0.25‐second epochal fetal heart‐rate and maternal heart‐rate were compared between the two devices. Multiple regression analyses were performed to assess the impact of maternal and fetal characteristics on signal loss. ClinicalTrials.gov Identifier: NCT04941534. Results: 285 women underwent concurrent monitoring. For fetal heart‐rate, mean bias, precision and 95% limits of agreement were 0.1 beats per minute (bpm), 4.5 bpm and −8.7 bpm to 8.8 bpm, respectively. For maternal heart‐rate, these results were −0.4 bpm, 3.3 bpm and −7.0 to 6.2 bpm, respectively. Median NIFECG E240 and E16 signal loss was 32.0% (interquartile range [IQR] 6.5%–68.5%) and 17.3% (IQR 1.8%–49.0%), respectively. E16 cCTG signal loss was 1.0% (IQR 0.0%–3.0%). For NIFECG, gestational age was negatively associated with signal loss (beta = −2.91, 95% CI −3.69 to −2.12, P < 0.001). Increased body mass index, fetal movements and lower gestational age were all associated with cCTG signal loss (beta = 0.30, 95% CI 0.17–0.43, P < 0.001; beta = 0.03, 95% CI 0.01–0.05, P = 0.014; and beta = −0.28, 95% CI −0.51 to −0.05, P = 0.017, respectively). Conclusions: Although NIFECG is complicated by higher signal loss, it does not appear to be influenced by increased body mass index or fetal movement. NIFECG signal loss varies according to method of computation, and standards of signal acceptability need to be defined according to the ability of the device to produce clinically reliable physiological indices. Background: Final year podiatry students volunteer annually as part of the wider interprofessional medical team at both the Brighton and London Marathon race events, supervised by qualified podiatrists, allied health professionals and physicians. Volunteering has been reported to be a positive experience for all participants and a way of developing a range of professional, transferable, and where appropriate, clinical skills. We sought to explore the lived experience of 25 students who volunteered at one of these events and aimed to: i) examine the experiential learning reported by students while volunteering in a dynamic and demanding clinical field environment; ii) determine whether there were elements of learning that could be translated to the traditional teaching environment in a pre-registration podiatry course. Methods: A qualitative design framework informed by the principles of interpretative phenomenological analysis, was adopted to explore this topic. We used IPA principles to enable analysis of four focus groups over a two-year period to generate findings. Focus group conversations were led by an external researcher, recorded, independently transcribed verbatim and anonymised prior to analysis by two different researchers. To enhance credibility, data analysis was followed by independent verification of themes, in addition to respondent validation. Results: In total, five themes were identified: i) a new inter-professional working environment, ii) identification of unexpected psychosocial challenges, iii) the rigors of a non-clinical environment, iv) clinical skill development, and v) learning in an interprofessional team. Summary: Throughout the focus group conversations, a range of positive and negative experiences were reported by the students. This volunteering opportunity fills a gap in learning as perceived by students, particularly around developing clinical skills and interprofessional working. However, the sometimes-frantic nature of a Marathon race event can both facilitate and impede learning. To maximize learning opportunities, particularly in the interprofessional environment, preparing students for new or different clinical settings remains a considerable challenge. Buildings are responsible for one‐third of the UK's greenhouse gas (GHG) emissions. The move to reduce emissions has resulted in recent stringent building regulations primarily aimed at reducing heating and associated energy use, often by improving the building fabric's airtightness. However, internal gains dominated, highly–insulated and airtight non‐domestic buildings will likely overheat in the projected warming climate, requiring energy‐intensive cooling, thus diminishing the effectiveness of heating efficiency focused regulations. This research investigated the effects of the warming climate on overheating and energy use and resulting emissions in representative urban office spaces in London in the present‐day and projected future climates using hourly dynamic thermal simulations. Findings suggest that airtight and highly–insulated office buildings in the temperate UK will overheat in the 2050s. Heating demand reduces by at least 36% in the 2050s but electricity consumption and summertime space conditioning will increase by at least 13% and 55% respectively when hybrid cooling is adopted to ameliorate overheating. Despite the increase, a mixed‐mode ventilation strategy is one of the ways of achieving overall energy efficiency while meeting benchmark overheating and emissions targets. Current heating‐focused legislation needs to be re‐evaluated to account for the effects of the warming climate and overheating risks. [ABSTRACT FROM AUTHOR]

This study aims to estimate the affect of urbanisation on daily maximum and minimum temperatures in the United Kingdom. Urban fractions were calculated for 10 km × 10 km areas surrounding meteorological weather stations. Using robust regression a linear relationship between urban fraction and temperature difference between station measurements and ERA‐Interim reanalysis temperatures was estimated. For an urban fraction of 1.0, the daily minimum 2‐m temperature was estimated to increase by 1.90 ± 0.88 K while the daily maximum temperature was not significantly affected by urbanisation. This result was then applied to the whole United Kingdom with a maximum Tmin urban heat island intensity (UHII) of about 1.7K in London and with many UK cities having Tmin UHIIs above one degree. This paper finds through the method of observation minus reanalysis that urbanisation has significantly increased the daily minimum 2‐m temperature in the United Kingdom by up to 1.70 K. British planning is among the most restrictive in the world and has been restrictive for longer than elsewhere. Restrictive regulation substantially increases the costs of office space and, especially in London, increases the house prices. Partly in response to the crisis in housing supply an automatic right to convert offices to residential use was introduced in 2013. Some major office locations in central London and Manchester were excluded from this relaxation. We exploit the resulting boundary discontinuities to estimate the impact on prices of the new right to convert offices to housing. Using a panel data set of some 2,000 office transactions between 2009 and 2016, we find a significant increase in the price of offices eligible for this automatic conversion: our central estimate is a premium of 50%. This result demonstrates that London's restrictions on the supply of housing were substantially more severe than those on offices–already estimated to have been tighter than anywhere else in Europe. This article contributes to the small literature analyzing the restrictive effect of regulation on offices and is, as far as we are aware, the first analyzing regulatory restriction on offices relative to housing.
