Gaps exist in the knowledge of return-to-driving guidelines, and education is needed for Australian health professionals working in the field of stroke. In this paper, authors evaluate the implementation of an e-learning module. A pre- and post-module knowledge test and survey were used to assess the e-module's success in achieving its learning objectives. Initially, 204 learners completed the module, with 68% of learners scoring 100% in the post-module knowledge test. Only 12 learners completed the survey. The authors recommend that health professionals other than occupational therapists access this e-module, and a review on e-module structure is required to improve results of knowledge tests. Background: There is increasing evidence for music-based interventions in neurorehabilitation, improving mood and functional outcomes. In response, there is growing interest from health-care providers in setting up Neurologic Music Therapy (NMT) services. This paper presents some preliminary data on the feasibility and acceptability of NMT in the acute stroke, multidisciplinary team setting, about which little is known. Objectives: To assess the feasibility and acceptability of a two-day per-week NMT service over 24 months. Methods: Data were collected on the number of referrals received, sessions attended, sessions declined and reasons why. Staff completed questionnaires, and collected them from patients and their relatives, rating interventions: 1. Not helpful, 2. Quite helpful, 3. Helpful, 4. Very helpful. Patients completed the Visual Analogue Mood Scale (VAMS) pre-/post- a single session. Results: Of 201 patients referred, 177 received treatment and 675 sessions were delivered. Twenty-four patients were discharged before sessions were scheduled and 28 sessions were declined, predominantly due to fatigue. Mean scores (SD) from questionnaire data were: patients (n = 99) 3.34 (0.825), relatives (n = 13) 3.83 (0.372), staff (n = 27) 3.85 (0.388). Mean, post-session VAMS data (n = 52) showed a non-significant reduction in 'Sad' (7.5, p =.007, CI = 2.1, 12.9) and an increase in 'Happy' (+ 6.2, p =.013, CI = −11.0, −1.4). Conclusions: Data suggest the service was feasible and helpful, particularly for patient mood, possibly improving engagement in rehabilitation. To the present day, the first and most widespread diagnostic approach in the assessment of acute stroke remains CT scan. Its sensitivity is very high (nearly 100%) in detecting intracerebral hemorrhage in the acute period, but its capability of revealing ischemic injury in the very first hours from symptom onset is relatively poor. Since the efficacy of thrombolytic treatment in acute stroke has been suggested by the ECASS and NINDS rt-PA trials, functional neuroimaging able to distinguish potentially salvageable tissue from irreversibly injured areas has acquired primary importance. The possibility to correctly identify the tissue of the ischemic penumbra within the first hours from symptom onset is essential for correct patient selection for thrombolitic treatment. Different imaging strategies are available for the definition of perfusion deficits within the acute time window; among these are positron emission tomography (PET), single photon emission computed tomography (SPECT), Xenon CT (XeCT), dynamic CT perfusion imaging (CTP), diffusion weighted magnetic resonance imaging (DW-MRI), and perfusion weighted magnetic resonance imaging (PW-MRI). Though each technique has its advantages and limitations to present day functional MRI remains the most widespread imaging technique in the assessment of acute stroke being more accessible than both SPECT and PET, and capable of giving information on both perfusion and tissue functional status in a single imaging session. Acute brain ischemia is accompanied by a disruption of low-molecular-weight aminothiols (LMWTs) homeostasis, such as homocysteine (Hcy), cysteine (Cys), and glutathione (GSH). We investigated the redox balance of LMWTs in blood plasma and its influence on ischemic stroke severity and the functional outcome in patients with an acute period. A total of 177 patients were examined. Total and reduced forms of LMWTs were determined in the first 10–24 h. Stroke severity and functional state were estimated using the National Institutes of Health Stroke Scale (NIHSS) and the modified Rankin Scale (mRs) at admission and after 21 days. Patients with high levels of total Hcy (> 19 μM) showed significantly reduced redox statuses of all LMWTs. Patients with low total GSH levels (≤ 1.07 μM) were at an increased risk of higher stroke severity (NIHSS > 10) compared to patients with a total GSH level > 2.64 μM (age/gender-adjusted odds ratio: 4.69, 95% CI: 1.43–15.4). Background: Early recognition of post stroke aphasia is thought to be crucial to plan effective rehabilitation. The Aachener Aphasie Bedside Test (AABT) is a widely-used tool for aphasia assessment in acute phase. The AABT was translated into Italian (I-AABT) in 2011, but its psychometric properties had not yet been studied. Aims: The aims of the study were to assess the I-AABT 1) short-term test-retest and inter-rater reliability, 2) concurrent and construct validity, 3) responsiveness and 4) to develop preliminary Italian normative scores to stage aphasia severity. Methods & Procedures: Participants were recruited from three Italian hospitals and divided into four groups: 1) patients with acute aphasia (PwAA; n=116), 2) patients with post-acute aphasia (PwPA; n=54), 3) patients with right-hemisphere damage (PwRHD; n=48) and 4) patients without neurological disorders (PwND; n=30). The I-AABT was administered to all participants. The Aachner Aphasie Test (AAT) was administered to assess concurrent validity. Spearman's correlations and Intraclass Correlation Coefficient (ICC) were used to calculate reliability. Concurrent and construct validity were assessed through Spearman's correlations between I-AABT and AAT subscales and Mann-Whitney test, respectively. Wilcoxon signed test was used to assess responsiveness. Both normalized z and T scores were calculated to produce Italian normative scores. Finally, ROC curves were drawn to determine the diagnostic accuracy of the I-AABT Comprehension and Production subtests. Outcomes & Results. Test-retest and inter-rater ICCs were highly significant and strong (ICC >.837, ICC >.698, p<.001, respectively). Correlations between I-AABT and AAT Comprehension and Production subtests were significant and strong (rs>.611, p <.001). Mann-Whitney test confirmed statistically significant difference between PwAA, and both PwRHD and PwND for I-AABT Spontaneous Language, Comprehension and Production subtests. A significant improvement in the I-AABT items Semantic, Phonemic and Syntactic Spontaneous Language, Oral praxis, Object identification, Automatic Language and Naming (p <.05) was detected after intensive language therapy. Preliminary conversion tables were devised to classify comprehension and production impairment levels. I-AABT's AUC-ROC values for Comprehension and Production parts were significant to detect patients with aphasia with a cut value of 139.5 (sensitivity = 72.9%, specificity = 79.5%9) and 94.5 (sensitivity = 75.2%, specificity = 74.4%.), respectively. Conclusions: The I-AABT proved to be a reliable, valid and responsive tool for the assessment of aphasia in acute stroke patients; its use is recommended in everyday clinical practice. Prehospital prediction models to estimate the likelihood of several types of stroke (large vessel occlusion [LVO], intracranial hemorrhage [ICH], and subarachnoid hemorrhage [SAH], and other types of stroke) should be useful to transfer those with suspected stroke to appropriate facilities. We recently reported Japan Urgent Stroke Triage (JUST) score with 21 items had excellent predictive abilities, and we further tried to simplify the score with parsimonious items and comparable predictive abilities. We conducted historical and prospective multicenter cohort studies at 8 centers from June 2015 to March 2018. We developed the prediction rules with select variables from JUST score for LVO, ICH, SAH and other types of stroke in 2236 patients with suspected stroke in historical derivation cohort. We validated the developed prediction rules in 964 patients in prospective validation cohort. There were 1150 stroke, including 235 LVO, 352 ICH, 107 SAH and 456 other types of stroke in the derivation cohort. We developed the scores with 7 items (high blood pressure, arrhythmia, conjugate deviation, headache, dysarthria, disturbance of consciousness, paralysis of upper limbs) and the developed scores had area under the receiver-operating curve (AUC) of 0.84 for any type of stroke, 0.89 for LVO, 0.79 for ICH, and 0.90 for SAH in the derivation cohort. There were 490 stroke, including 102 LVO, 138 ICH, 28 SAH and 222 other types of stroke in the validation cohort. The scores well discriminated these strokes in the validation cohort (AUC of 0.76 for any type of stroke; 0.81 for LVO, 0.73 for ICH, and 0.85 for SAH). The simplified 7-item JUST (JUST-7) score had good predictive ability and can help healthcare providers to estimate the likelihood of different types of stroke and decide the referral hospital. Background: The Aphasia Rapid Test (ART) is a screening test developed for fast speech/language assessment of people in the acute stroke period. This test has been developed for French and English and was recently adapted for Portuguese and Italian. Nowadays, such a standardised screening test is in a great need at clinics with Russian-speaking patients. To fill this gap, the ART was adapted for Russian. Aims: The current study investigated whether the Russian ART meets all the psychometric standards, and whether it is suitable for detecting speech/language disorders and estimating their severity, as well as for the evaluation of improvement in the acute post-stroke period. Methods & Procedure: First, we evaluated the validity, sensitivity, specificity, accuracy, test-retest reliability, inter-item consistency and inter-rater reliability of the test in a group of people with chronic speech/language disorders (N = 55) and in an age-matched control group of non-brain-damaged individuals (N = 50). Participants performed the Russian ART, and their linguistic status was confirmed by the Russian e-version of the Token Test. Second, to test the appropriateness of the Russian ART in the acute post-stroke period, a clinical group of such individuals (N = 43) performed the ART and the Token Test, as well as the Vasserman's scale which is widely used in Russian clinics. Finally, 16 people in the acute stroke period performed the Russian ART twice to prove that the test can detect early changes in an acute patient's linguistic status. Outcomes & Results: The results showed that the Russian ART can be considered as a valid, sensitive, specific, and accurate screening tool with the high test-retest reliability, inter-item consistency, and inter-rater reliability. In the acute post-stroke group, the correlation between the ART and the Token Test was high and significant; a moderate correlation and no significant correlation were found between the Vasserman's scale and the Russian ART and the Token Test correspondingly. The Russian ART also allowed us to detect the improvement in speech/language status in the acute post-stroke period. Conclusion: The study confirmed that the Russian ART meets all required standards to be suggested for usage in a Russian-speaking clinical population. Background: Discourse analysis allows the examination of functional and ecological language impairment in post-stroke aphasia. Given its complexity, various methods of analysis have been developed to measure the multiple components of discourse. Clinical assessment usually includes discourse analysis, but how clinicians should assess recovery of discourse, particularly in acute care settings, is still a matter of debate. Aims: This study aimed to measure improvements in discourse production in early post-stroke aphasia recovery. Methods & Procedure: Twenty-three persons with aphasia following a first left middle cerebral artery stroke were recruited in the stroke unit of Hôpital du Sacré-Coeur de Montréal (May 2015-July 2018). Patients treated with thrombolysis (n = 10) and untreated patients underwent two aphasia assessments (0 to 72 hours, 7 to 14 days post-onset). Discourse assessment consisted of the picture description task from the Western Aphasia Battery. Changes in microlinguistic and thematic informativeness measures between the two assessment periods were analyzed. Outcomes & Results: In-depth microlinguistic analyses showed no significant difference between the initial and follow-up assessments. Conversely, some thematic informativeness variables improved significantly during the same time period. Patients treated with thrombolysis produced more thematic units than untreated patients at both assessments, but the change between the two groups was not significant. Conclusions & Implications: This study suggests that thematic informativeness variables are sensitive to language improvement in early post-stroke aphasia recovery whereas no microlinguistic variables improved significantly in the same period. In contrast to previous evidence, the difference between patients treated with thrombolysis and untreated patients was not evident over time. The results suggest that thematic informativeness constitutes an interesting path to explore as a routine clinical assessment in acute-care settings because it is time-efficient, simple to conduct and reliable in assessing early changes in the discourse production of individuals with aphasia in the acute post-stroke phase. Purpose: Early rehabilitation is widely recommended for acute-stroke patients. We tested the hypothesis that the functional prognosis of stroke patients receiving daily early rehabilitation in a clinical practice setting is generally better than that of patients receiving rehabilitation only on weekdays. Materials and methods: We analyzed hospitalized patients who experienced either cerebral infarctions or cerebral hemorrhages and subsequently underwent rehabilitation at our hospital between October 2010 and September 2014. We examined the association between training frequency and activities of daily living improvements, as indicated by the Barthel Index (BI) effectiveness. Results: In total, 661 patients with cerebral infarctions and 245 with intracerebral hemorrhages (ICHs) were analyzed. The BI effectiveness was highest for patients receiving high-frequency therapy following cerebral infarction. In addition, multiple linear regression analysis indicated that BI effectiveness was significantly and positively correlated with high-frequency therapy (coefficient, 0.072; 95% confidence interval, 0.019-0.126; p < 0.01) in patients with cerebral infarctions. There was no significant difference in BI effectiveness between therapeutic protocols for patients with ICHs. Conclusion: This retrospective cohort study demonstrated that extensive therapy can result in functional recovery in patients with cerebral infarctions. Implications for Rehabilitation: Early intervention with intensive rehabilitation therapy is important for improving the functional recovery of patients during acute-care hospitalization. Few acute hospitals provide more than 2 h of daily rehabilitation for patients with acute stroke. In a daily clinical practice setting, this clinical study demonstrates a direct relationship between early intervention with intensive rehabilitation therapy and good functional recovery of stroke patients in an acute ward. Purpose/Aim: Data from chronic stroke studies have reported reduced blood flow and vascular endothelial function in the stroke-affected limb. It is unclear whether these differences are present early after stroke. First, we investigated whether vascular endothelial function in the stroke-affected limb would be different from healthy adults. Second, we examined whether between-limb differences in vascular endothelial function existed in the stroke-affected arm compared to the non-affected arm. Last, we tested whether reduced vascular endothelial function was related to pro-inflammatory markers that are present early after stroke. Materials and Methods: Vascular endothelial function was assessed by flow-mediated dilation (FMD) in the brachial artery within 72 h post-stroke. All participants withheld medications from midnight until after the procedure. Ultrasound scans and blood draws for pro-inflammatory markers occurred on the same day between 7:30 am and 9:00 am. Results: People with acute stroke had significantly lower FMD (4.2% ± 4.6%) than control participants (8.5% ± 5.2%, p = 0.037). Stroke participants had between-limb differences in FMD (4.2% ± 4.6% stroke-affected vs. 5.3% ± 4.4% non-affected, p = 0.02), whereas, the control participants did not. Of the pro-inflammatory markers, only vascular cell adhesion molecule-1(VCAM-1) had a significant relationship to FMD (stroke-affected limb, r = −0.62, p = 0.03; non-affected limb, r = −0.75, p = 0.005), but not tumor necrosis factor alpha nor interleukin-6. The aim of this study was to evaluate the safety and efficacy of multimodal reperfusion therapy (MMRT) for tandem internal carotid artery and middle cerebral arterial (TIM) occlusions. Cases of TIM occlusion were collected and retrospectively reviewed. The analyzed objects included etiology, sites of tandem occlusion, collateral flow, location and size of infarcts. Combined with mechanical recanalization techniques and its complications, the National Institute of Health Stroke Scale (NIHSS) score and imaging data that was derived pre- and post-procedure were further contrasted. The study enrolled six patients with TIM occlusions. The mean NIHSS score on admission was 17 (range 13–20) and the median time from puncture to recanalization was 141 min (range 60–230). The substantial recanalization rate (Thrombolysis in Cerebral Infarction 2b or 3) was 83.3% and no symptomatic intracerebral hemorrhage was observed. The mean NIHSS score after three days was 14 (range 10–19) and 9 (range 3–17) following discharge. However, one patient died of pulmonary infection one month after discharge. For the five patients who survived, the modified Rankin Scale was evaluated at three months, with scores of 3, 1, 3, 5 and 3, respectively. One of the biggest conundrums in acute treatment of childhood arterial ischemic stroke is administration of intravenous recombinant tissue plasminogen activator. Although the benefit of this treatment is well-established in adults, the same effectiveness in children has not been demonstrated. Diversity of underlying causes of ischemic stroke in children and delay and uncertainty in diagnosis are some of the complexities that make intravenous thrombolysis elusive in this population. Physiological, pharmacological and developmental factors may also play roles in variable effect of intravenous recombinant tissue plasminogen activator in children. Objective: We have evaluated the factors of unsuccessful re-canalisation in a large series of patients with hemispheric cerebral infarction treated with thrombolysis. Patients and methods: All patients aged 18-80 years with an acute hemispheric infarction, admitted within the first few hours of symptoms onset, were immediately submitted to Magnetic Resonance both Imaging (MRI) and Angiography (MRA). MRI and MRA were repeated at 24 h of stroke. Re-canalisation was attributed if grade 2 or 3 of Thrombolysis in Myocardial Infarction (TIMI) criteria for the myocardial infarction. Outcome was rated at three months of stroke. Re-canalisation was matched with ageing and with the common risk factors for stroke. Results: One hundred and twenty-one patients, 70 men and 51 women, with a median age of 67 years, were included. Re-canalisation was seen in 62 patients (51%). Twenty-three patients (19%) died by 90 days of stroke. Re-canalisation was associated to survival (1 death vs. 22, p < 0.0001). Regression analysis retained advanced age (Odds ratio 0.37, 95% Confidence interval 0.13-0.98), baseline National Institute of Health Stroke Scale (NIHSS) (Odds ratio 0.94, 95% Confidence interval 0.89-0.98) and diabetes mellitus (Odds ratio 0.28, 95% Confidence interval 0.09-0.84) as factors contrasting re-canalisation. Conclusions: Our study indicates that in patients with proven occlusion of the terminal segment of the internal carotid artery and/or of the mainstem of the Middle Cerebral Artery, re-canalisation at 24 h of the acute ischemic stroke is dramatically associated with survival, and halted by advanced age and diabetes mellitus. Circadian rhythm interactions of hemostatic factors can modify tissue plasminogen activator (tPA) effects. We assess the relationship of the time frame of intravenous tPA administration with the outcome of patients with acute ischemic stroke (AIS). We studied 135 consecutive patients with AIS and transcranial duplex documented middle cerebral artery (MCA) occlusion treated with intravenous tPA. Complete recanalization was defined as total improvement on thrombolysis in brain ischemia (TIBI) grades 2 h after tPA infusion. Clinical response was evaluated by the modified Rankin scale at 90 days. We determined plasminogen activator inhibitor-1 (PAI-1) levels in 33 patients with available plasma samples before treatment. Our results are follows: 92 (68.1%) patients were treated in the diurnal (9:00-21:00) and 43 (31.8%) in the nocturnal period (21:00-9:00). Complete recanalization was recorded in 52/135 (38.5%) patients. Both the rate of complete recanalization (45.6% vs. 23.2%; p = .01) and good clinical outcome (64.1% vs. 44.2%; p = .02) were significantly higher in the group of diurnal tPA administration compared with those treated in the nocturnal period. The adjusted odds ratio (OR) of diurnal tPA treatment for complete MCA recanalization was 2.37 (95% confidence interval [CI], 1.02-5.52; p = .045). Diurnal tPA infusion significantly improved the overall distribution of scores on the modified Rankin scale, as compared with nocturnal treatment (OR, 2.07; 95% CI, 1.16-4.64 by ordinal regression analysis). Low PAI-1 levels were associated with complete recanalization but did not significantly differ between the two time frames. Objective. Chronic kidney disease, characterized by a decline in renal function, is a public health challenge, affecting around 10% of the population worldwide. The aim of this study was to investigate the prevalence of decline in renal function in patients admitted for acute first-ever stroke from a local centre in China, and its impact on mortality and neurological impairment during a short follow-up period. Material and methods. In total, 640 patients hospitalized due to acute stroke were enrolled in this prospective study. Clinical data were obtained and a 3-month follow-up was executed by a team of trained investigators. The data indicated that the prevalence of estimated glomerular filtration rate (eGFR) decline (≤60 ml/min/1.73 m2) was 18.5% in this specific population. Results. Hyperuricaemia, elevated homocysteine and smoking history were identified as three major risk factors associated with eGFR decline. An eGFR ≤60 ml/min/1.73 m2 was detected in 45.1% of haemorrhagic stroke patients, while 14.7% of ischaemic stroke patients had an accompanying eGFR decline. During the follow-up period, the mortality rate was 18.1% in patients with eGFR decline and 1.5% in patients with eGFR >60 ml/min/1.73 m2. Conclusions. This study clearly shows that the prevalence of renal impairment is high in acute stroke patients in China, and renal impairment may confer a significant risk for worse outcomes such as impaired neurological function and death, at least in the short term. The Challenge-Stroke study was conducted in Japanese patients initiated on candesartan cilexetil therapy within 3 months of suffering a stroke to investigate the clinical use of candesartan and its efficacy/safety in this therapeutic setting. A total of 869 patients formed the safety analysis set. In total, 79.6% of patients with brain hemorrhage (BH) and 60.2% with brain infarction (BI) began candesartan before post-stroke day 3 and 7, respectively. Baseline average blood pressure (BP) was 152.0/83.2 mmHg in the BH group and 165.2/89.8 mmHg in the BI group; this was reduced to 125.8/75.4 mmHg and 136.3/78.1 mmHg, respectively, at 1 year. The incidence of adverse drug reactions was 6.7 and 8.0%, respectively. There were 12 recurrent strokes in the BH group and 11 in the BI group after 1 year. The risk of recurrent stroke was significantly higher for BH patients with a final systolic BP ≥150 mmHg than for those with a final systolic BP <130 mmHg (hazard ratio: 6.807; p = 0.004). Aggressive antihypertensive therapy is currently employed in Japanese patients with acute stroke. Candesartan was safe and effective for BP control in acute stroke patients. Evaluation of: Sandset EC, Bath PMW, Boysen G et al. The angiotensin-receptor blocker candesartan for treatment of acute stroke (SCAST): a randomized, placebo-controlled, double-blind trial. Lancet 377(9767), 741-750 (2011). Elevated blood pressure during the acute stroke period is associated with poor neurologic outcomes; however, treating blood pressure in this setting remains controversial. Interest in modulating the renin-angiotensin system in this setting has gained momentum because of neurohormonal properties, which may provide benefits beyond blood pressure control. The Scandinavian Candesartan Acute Stroke Trial (SCAST) was recently published describing the effects of candesartan in patients with hypertension during the acute stroke period. This study shows that lowering blood pressure with an ARB for 7 days poststroke does not improve 6-month neurologic outcomes. Currently, imaging is predominantly used to exclude patients for thrombolysis, rather than identify patients most likely to benefit. This means that patients are being selected for treatment without reference to tissue pathophysiology. Imaging of specific stroke pathophysiology may be the key to selecting patients most likely to benefit from thrombolysis, and could revolutionize acute stroke assessment and treatment. The technology is available to identify the acute infarct core and possibly the penumbra, via magnetic resonance diffusion-weighted imaging, and both magnetic resonance- and computed tomography-perfusion imaging techniques. Hypertension is commonly seen in the setting of an acute stroke. Although hypertension is the most important modifiable risk factor for both ischemic and hemorrhagic stroke, the immediate management of elevated blood pressure in this setting is controversial. Questions remain to be definitively answered, such as when to start lowering blood pressure, by how much to lower pressure, which pharmaceutical agents to use and whether to continue or stop previous antihypertensive medications. Background. A prospective observational study was aimed at assessing the role of blood pressure (BP) during the first 24 h from stroke onset on the outcome of acute ischaemic stroke. Methods. Subjects admitted within the first 3 h from stroke onset were included. Stroke severity was evaluated with the Canadian Stroke Scale (CSS). Functional recovery was defined as a modified Rankin Scale score ≤2. Results. One hundred subjects were included. In a logistic regression model, the independent predictors of poor functional recovery at discharge were: age (OR = 1.12; 95% CI 1.04-1.21; p = 0.0033), non-lacunar stroke subtype (OR = 4.31; 95% CI 1.07-17.31; p = 0.0395), diabetes mellitus (OR = 8.38; 95% CI 1.67-41.95; p = 0.0097), a CSS score at admission ≤8 (OR = 28.64; 95% CI 5.59-146.68; p<0.0001), an average systolic BP during the first 6 h ≥180 mmHg (OR = 13.34; 95% CI 1.34-133.10; p = 0.0272) and a lower diastolic BP average from 6 to 24 h (OR for 5 mmHg increase: 0.57; CI 95% 0.36-0.88; p = 0.0115). Similar results were observed after 3 months of follow-up. Conclusion. In ischaemic stroke patients, systolic BP over 180 mmHg in the first 6 h and a decrease of diastolic BP during the 6-24 h from stroke onset were independent predictors of a poor functional recovery. A dose-finding study with an adaptive design generates three computational problems: fitting the dose-response curve given the current data, identifying the dose to be given to the next patient that is optimal for learning about the dose-response curve, and pretrial simulation in order to establish operating characteristics of alternative designs. Identifying the 'optimal' dose is the rate-limiting step since conventional methods, estimating the full posterior predictive distribution of some utility function under each of the possible doses, are very slow. We explore a simpler strategy based on importance sampling, whereby the posterior mean of the utility at each candidate dose is estimated by taking its average across an empirical distribution for the model parameters from the current Markov chain Monte Carlo (MCMC) run, weighted according to the likelihood of one or more predicted observations. We identify appropriate settings for this algorithm and illustrate its application in the context of a normal dynamic linear model used in a dose-finding clinical trial of a neutrophil inhibitory factor in acute ischaemic stroke.
