Beim diabetischen Fuß ist die Zahl der Amputationen noch immer erschreckend hoch, die Dauer der Behandlung für chronische Wunden ist außerordentlich lang. Beim angiopathischen Fuß (aF) handelt es sich um Durchblutungsstörungen. Das Krankheitsbild ist bekannt. Die Behandlung besteht in der Wiederherstellung der Perfusion durch Gefäßmediziner, deren Prinzipien sowie die wünschenswerten Verbesserungen in den Kapiteln 2 - 4 ausgeführt werden.Der neuropathische Fuß (nF) ist weniger bekannt, was insbesondere auf die bedrohlichen Spätstadien zutrifft. Im Gegensatz zum aF existieren keine wertigen Klassifikationen und keine strukturierten Behandlungskonzepte. Die Folge sind lange Behandlungszeiten für infizierte Wunden und hohe Amputationszahlen.Im Buch werden auf der Grundlage von ausgereiften Klassifikationen fachübergreifende Behandlungskonzepte erörtert und das breite Spektrum der Indikationen und Verfahren dargestellt. The prevalence of diabetes continues to increase worldwide. Traditionally, diabetes in its adult form has not been considered a serious life-threatening disease. This attitude needs to be changed because the complications asso­ ciated with the adult form of diabetes affect almost every organ system. The high morbidity and mortality of non-insulin-dependent diabetes mellitus (NIDDM) suggest that current treatment strategies are unsatisfactory. We therefore face an urgent need for new therapeutic approaches. When the first Handbook 0/ Oral Antidiabetics was edited by H. Maske in 1971, the risks and benefits associated with the use of oral antidiabetics were still under discussion. Nowadays, oral antidiabetics hold a strong posi­ tion in the long-term treatment of diabetes. Roughly 30% -50% of the patients with diabetes in Europe and the United States are treated with oral antidiabetics, chiefly sulfonylureas. While acknowledging the value of the ß­ cytotropic sulfonylureas, we also need to recognize important limitations of their use, e.g., in the treatment of obese diabetic patients.

This book is intended to provide up-to-date and emerging information in the field of diabetes mellitus with a focus on preventive, predictive and personalized medicine.

Investigation of the pharmacokinetics and metabolism of human proteins has escalated over the last two decades because of the use of recombinant human proteins as therapeutic agents. In addition, the development and improvement of analytical techniques enabling the detection of minute quantities of proteins in biological matrices have aided this process. In assembling this volume, we sought to provide a state-of-the-art as­ sessment of the pharmacokinetics and metabolism of protein therapeutics through complete reviews of selected examples. A comprehensive review of all protein therapeutics was not attempted; the majority -of the therapeutic protein classes and crucial scientific issues have been addressed, however. Therefore, we are confident that this volume will provide a useful reference for scientists in this field. The volume has been divided into two general parts. Advances in Abdominal Surgery 2002 contains papers from a group of Italian academic surgeons who contribute to a national research program entitled `Progress in Abdominal Surgery'. The research is supported by the Italian Ministry of the University. The primary objective of the group is to combine experimental and clinical approaches in the study of several surgical abdominal diseases. The authors of the individual chapters were requested to present an update in their own fields of research. This volume summarizes several important updates in abdominal surgery and pathophysiology. The contributions are grouped into six sections, namely Surgical Infection, Hepatobiliary and Pancreatic and Splenic Surgery, Colorectal Surgery, Fecal Incontinence, Minimally-Invasive Surgery, and Transplantation.

In Zurich at the 7th International Annual Meeting of the Society of Nuclear Hedicine in Europe, held in 1969, a group of young scientists from eleven countries dedicated some papers to the memory of Georg von Hevesy. On the occasion of the Second International World Congress of Nuclear Medicine and Biology held in 1978 in Washington D.C., under the presidency of Henry N.Wagner,Jr., a group of young scientists again dedicated important papers from the Congress to the memory of Georg von Hevesy. This book consists of these papers, which present new results in the field of Nuclear Medicine reported by physicians, physicists, chemists, engineers, and computer scientists. The author of this Foreword has recently retired after spending 25 years in academia and 15 years in the pharmaceutical industry. Most of this time has been spent following and, hopefully in some instances, contributing to advancement of the discipline of pharmacokinetics. During the last 40 years, pharmacokinetics has grown from a fledgling in the 1950s to an adult in the 1990s. The late development of the discipline of pharmacokinetics, relative to other disciplines such as chemistry, bio­ chemistry, and pharmacology, probably stems both from general ignorance of the importance of the time course of concentration-effect relationships in drug therapy and from our technical inability to do anything about it had we been more enlightened. Just as the end of the historical dark ages had to await the beginning of the Carolingian revival, so the end of the pharma­ co kinetic dark age had to await the discovery of adequate analytical methods and also an intellectual leap of faith to accept that drug action is in some way dependent on receptor site occupancy, and therefore on drug con­ centration. The recent evolution of pharmacokinetics has occurred in three phases which may be identified as those of discovery, stabilization, and rationaliz­ ation. The discovery phase, which occurred in the 1950s and 1960s, esta­ blished the mathematics and concepts of'modern'pharmacokinetics and sought areas of application, ranging from model-independent methods, through compartment approaches, to complex physiological models.
