In this paper, we investigate devolution in the Northern Irish context. The role of institutional geography is highlighted. Both contemporary and historical (c. 1921–1972) devolution models are discussed. It is argued that institutional factors provide explanations for economic weaknesses found under both devolution models. The evidence presented suggests that the institutional geography has both changed and exhibited continuities and we discuss these features in relation to contemporary issues. In particular, it is argued that institutional geography considerations have shaped the creation of the current review of tax devolution as well as the creation of an independent fiscal council. Across the world acute hospitals are under unprecedented pressures due to shrinking budgets and increasing demand, against this backdrop they are also experiencing record levels of activity in Accident & Emergency and delayed transfers of care. Reducing pressure on hospitals by avoiding unnecessary admissions and delayed discharges has risen up the global policy agenda. However, reviews of strategies and policies have rarely involved discussions about the role that hospital social workers play in achieving timely hospital discharge. Yet discharge planning has become a, if not the, central function of these professionals. This paper presents the results of a small-scale exploratory study of hospital social work in an acute hospital in Northern Ireland. The findings reveal that the work of hospital social workers is characterised by increased bureaucracy, an emphasis on targets and a decrease in the time afforded to forming relationships with older people. Hospital social workers highlight concerns that the emphasis on discharge planning and pressures associated with the austerity agenda limits their capacity to provide other more traditional roles such as advocacy and counselling. It is argued that hospital social work should not be narrowly defined as 'simply' co-ordinating discharge plans. This paper sets out the socio-spatial practices of PS2 and Street Society in territorialised, post conflict Northern Ireland – whose work combines pedagogical and improvisation theory to prototype distinctive social models. These practices were formed by a continual process of practical involvement and reflection (through writing) and evaluation (funding permitting), alongside the author-practitioners' search for theoretical concepts that would bring clarity and an alternative perspective to the processes and outcomes. Concepts and approaches taken from pedagogical theory and, uniquely in the case of architecture, improvisational theory, will be used to explore critical aspects of the specific practices, and more generally assert that such socio-spatial 'street pedagogy' offers a space of rehearsal and ultimately, a site for the development of socially-just aesthetics. The paper thus proposes a theoretical scaffolding for practices which increase diversity and diminish resource consumption in the contexts of climate emergency and a politics of precarity. In this paper, a unique dataset of improvised explosive device attacks during “The Troubles” in Northern Ireland (NI) is analysed via a Hawkes process model. It is found that this past dependent model is a good fit to improvised explosive device attacks yielding key insights about the nature of terrorism in NI. We also present a novel approach to quantitatively investigate some of the sociological theory surrounding the Provisional Irish Republican Army which challenges previously held assumptions concerning changes seen in the organisation. Finally, we extend our use of the Hawkes process model by considering a multidimensional version which permits both self and mutual-excitations. This allows us to test how the Provisional Irish Republican Army responded to past improvised explosive device attacks on different geographical scales from which we find evidence for the autonomy of the organisation over the six counties of NI and Belfast. By incorporating a second dataset concerning British Security Force (BSF) interventions, the multidimensional model allows us to test counter-terrorism (CT) operations in NI where we find subsequent increases in violence. Developing the equality agenda has been a major preoccupation of policy intervention in Northern Ireland since Direct Rule from London was instituted in 1972. This paper examines how policy has developed and its effectiveness. The paper highlights new developments since the Belfast Agreement of 1998 and examines in particular new attempts to mainstream equality in the policy process. 'Reconciliation' generally means the development of good relations where they have never truly existed before. This paper refers principally to the example of Northern Ireland and the Irish peace process. Psychiatrists should examine what really contributes to 'large group' reconciliation, as the absence of the psychiatric perspective would be a serious loss in the search for post-conflict well-being at the communal level. Northern Ireland has been dubbed by the media as the ‘race hate capital of Europe’ and attracted recent international criticism after one hundred Roma families were forced to flee their homes following racist attacks. This paper examines the problem of racism in Northern Ireland from a number of perspectives. First, it considers the effectiveness of the Government's response to racism against its Racial Equality Strategy 2005–10 using performance criteria designed to track the implementation of the strategy. Second, it considers and empirically tests the assertion in the literature that sectarianism shapes the way in which racism is reproduced and experienced. Third, it explores racism at the level of the individual – which factors influence people in Northern Ireland to exhibit racist behaviour. Finally, the paper considers the likely policy implications of the research findings in the context of devolved government where addressing racism is part of a wider political imbroglio which has gridlocked decision-making within the power-sharing Executive of Northern Ireland. Archive-based regional studies can contribute much that is new to the economic, political and social history of the Second World War. This paper considers the process of industrial mobilization in Northern Ireland, a politically divided region which was part of the United Kingdom but which had its own government. It examines the changing administrative framework of war production, the debate on military and industrial conscription, the role of women and the economic implications of geographical remoteness from London. This paper treats of the peculiarity of the Irish case. Professionalization of history came late to Ireland, and when it did happen, it was with a view to overcoming the inter-denominational and inter-communal point scoring that had energized most previous writing of Ireland's history. In tracing the further development of the history profession in Ireland, the paper alludes to the extent to which the posing of new questions and the employment of new methods were motivated by historical developments elsewhere in the western academic world. The outbreak of civil conflict in Northern Ireland inspired a new phase of introspective writing about Irish identity, sometimes given the semblance of universality through the invocation of post-colonial theory. This writing was usually presented in historical format, was composed mostly by academics employed by literature and social science departments, and was severely critical of what they described as the historical revisionism in which most professional historians in Ireland were believed to have engaged. It concludes with a consideration of how historians responded both to the challenge to their integrity and to various pressures to become more judgemental in writing about Ireland's past. This paper examines the extent of mental ill-health and probable depression among older people in long-term care. It presents selected findings from a study in Greater Belfast, Northern Ireland, that compared the quality of life, autonomy and mental health of older people living in nursing and residential homes with those of older people living in private households who were receiving domiciliary care. Structured interviews were conducted with 214 residents in institutions and 44 older people receiving domiciliary care. The study found that those in private households were more severely physically-impaired and had a higher level of mental ill-health than the residents of institutional homes. It is suggested, however, that the mental ill-health effects were associated less with physical impairments than with the restrictions placed on the older person's decisional autonomy, and that long-term care environments that constrain the older person's autonomy contribute to the development of depression. Although the UK National Service Framework for Older People specified that those with depression should be given treatment and support, priority should also be given to preventing the depression associated with living in long-term care settings. We investigate economic resilience of UK regions before, during and after the 2007/8 global financial crisis. We date business cycle turning points in real output, employment and productivity to assess the resilience dimensions of resistance, recovery and renewal and rank the economic resilience of regions in a resilience scorecard. Our empirical results reveal that the business cycle in productivity has not returned to its pre-recession peak level for Yorkshire and the Humber and the employment level has not recovered in Scotland. The resilience scorecard ranks the South East as the most resilient region with Northern Ireland the least resilient. [ABSTRACT FROM AUTHOR]

Objective: The school food environment (SFE) is an ideal setting for encouraging healthy dietary behaviour. We aimed to develop an instrument to assess whole-SFE, test the instrument in the school setting and demonstrate its use to make food environment recommendations. Design: SFE literature and UK school food guidance were searched to inform instrument items. The instrument consisted of (i) an observation proforma capturing canteen areas systems, food presentation and monitoring of food intake and (ii) a questionnaire assessing food policies, provision and activities. The instrument was tested in schools and used to develop SFE recommendations. Descriptive analyses enabled narrative discussion. Setting: Primary schools. Participants: An observation was undertaken at schools in urban and rural geographical regions of Northern Ireland of varying socio-economic status (n 18). School senior management completed the questionnaire with input from school caterers (n 16). Results: The instrument captured desired detail and potential instrument modifications were identified. SFE varied. Differences existed between food policies and how policies were implemented and monitored. At many schools, there was scope to enhance physical eating environments (n 12, 67 %) and food presentation (n 15, 83 %); emphasise healthy eating through food activities (n 7, 78 %) and increase parental engagement in school food (n 9, 56 %). Conclusions: The developed instrument can measure whole-SFE in primary schools and also enabled identification of recommendations to enhance SFE. Further assessment and adaptation of the instrument are required to enable future use as a research tool or for self-assessment use by schools. [ABSTRACT FROM AUTHOR]

Deliberative minipublics are becoming increasingly popular, with both scholars and practitioners highlighting their potential to bolster public approval of political decision-making. Yet, it remains unclear whether minipublics are able to do so in contexts where the public itself is deeply divided – a concern which becomes only more relevant as levels of polarization are said to rise across the globe. In this study, we argue that polarized citizens may perceive minipublics and their outcomes as less legitimate than more moderate citizens. We use original survey data from Northern Ireland (n = 932), a highly polarized society where a minipublic was organized on the contentious issue of the region's constitutional future. We find that higher levels of ideological polarization and, to an extent, affective polarization are associated with lower levels of perceived minipublic legitimacy among the wider public, although effects are small. The history of sonic arts is charged with transgressive practices that seek to expose the social, aural and cultural thresholds across various listening experiences, posing new questions in terms of the dialogue between listener and place. Recent work in sonic art exposes the need for an experiential understanding of listening that foregrounds the use of new personal technologies, environmental philosophy and the subject–object relationship. This paper aims to create a vocabulary that better contextualises recent installations and performances produced within the context of everyday life, by researchers and artists at the Sonic Arts Research Centre at Queen's University Belfast. This paper builds on a developing field of enquiry regarding the potency of film as an intervention into normative patterns in popular culture which are recognisable in a legal pluralist sense, and that relate to more standard legal analyses of constitutional development. The argument is developed that Steve McQueen in Hunger has made a film which marries a particular filmic formal device of lingering on highly aestheticised details of human behaviour with an overall filmic sensibility of refusing 'politics' in favour of 'humanity'. The film demonstrates a resounding success, both in critical terms and in having the film accepted on its own terms of 'humanity'. The suggestion in this paper is that the ongoing processes of 'dealing with the past' in Northern Ireland and other postconflict societies may have something to learn from the marriage of idea and aesthetic form in Hunger. This paper addresses, from a comparative perspective, the legal position of the lay employees of religious institutions such as schools and hospitals. The legal regimes governing ‘ethos’ in the Republic of Ireland, Northern Ireland and Britain are compared. The tension which exists between the right of religious schools and hospitals to uphold their ethos in a secular society on the one hand, and the rights of their employees to privacy and personal autonomy on the other, is highlighted. It will be argued that legislative reform is required to remove uncertainty from the lives of lay employees of religious institutions who work conscientiously but fear dismissal or discrimination because an aspect of their lifestyle – usually their sexuality – is considered unorthodox by their employer. The need for a change in judicial attitudes and a lessening of deference towards the actions of religious denominations is also a theme throughout. [ABSTRACT FROM AUTHOR]

Against a background of limited previous research, this paper examines the access to health and social care among older people in the farming communities of County Down, Northern Ireland. In-depth interviews were conducted with 45 people aged 60 or more years living on family farms to collect information about health care needs and service use and adequacy. In addition, interviews with service providers provided information on their perceptions of the farming communities' needs. The findings indicate that there are specific rural dimensions of access to services and that among the respondents there was substantial unmet need. For many farming families, using services is determined by much more than being able to reach them physically. The lack of reliable information, the culture of stoicism and the absence of appropriate services impeded obtaining effective support. Recent health care policies and strategies have stressed the importance of developing local services that are responsive to need in consultation with service users, but there is worryingly little evidence that this has occurred. It is concluded that if effective outcomes are to be achieved, policies must recognise the specific characteristics of rural populations and be sensitive to the needs, attitudes and expectations of farming families. The current lack of understanding about the distinct needs of these communities at present exacerbates the isolation and marginalisation of already vulnerable older people. [ABSTRACT FROM AUTHOR]

Two decades ago allegations of religious discrimination and the onset of 'the troubles' led the British Government to institute a programme of administrative and legislative reform in the province. These reforms culminated in the Fair Employment Act (1976). More recently, the Government began a review of the efficacy of the existing legislation and this has now resulted in a new Fair Employment Bill for Northern Ireland. The new Bill should be seen as a serious attempt to grapple with the chronic problem of religious discrimination in the province. The realisation of equality of opportunity in Northern Ireland, however, continues to be as much a test of political will as of the ability of those who frame 'fair employment' legislation. Data from a survey of 1699 currently (once) married males are used in examining ideal family size in Northern Ireland, an area where fertility remains quite high by west European standards. Although there are several problems concerning the measurement and use of ideal family size, an analysis of the Northern Irish data indicates that the responses appear to be valid. For example, most respondents can formulate an ideal family size in numerical terms, there is a considerable degree of variation in the responses and these are not necessarily a function of actual family size. Northern Irish family size ideals (mean 3·6) are considerably greater than those of many west European countries and these ideals also vary within Northern Ireland. Religious grouping proves to be the major factor influencing these variations, with Roman Catholics generally expressing an ideal (mean 4·4) for large families. However, other factors are also associated with variations in ideal family size and the paper considers the roles played by education, age, occupation, region of residence and religiosity. Also, ideal family size does not always correspond with actual family size, indicating that while some married men possess relatively large family size ideals but have smaller families, others have large families but favour more moderately sized ideals. It is evident that attitudes to fertility in Northern Ireland are in a state of change. The value of ideal family size data in understanding fertility patterns is emphasized. The number of Northern Ireland women obtaining abortions in England has risen sharply from 36 for April–December 1968 to 775 in 1972. The figure for 1973 when it becomes available will probably be about 1000.The sample on which this paper is based consisted of 522 women who between January 1971 and August 1973 sought an abortion through the Ulster Pregnancy Advisory Association. The sample amounted to about one-quarter of all residents of Northern Ireland obtaining an abortion in England over the same period and was therefore not a random one. It was, however, fully representative for age and marital status and was thought to be generally reliable.The 522 women included 108 Roman Catholics, who were therefore under-represented in the sample compared with the general population at risk.The 108 Roman Catholics included 72.2% of single women, compared with 65.1% of single Protestants; they were also younger, 42.6% coming with the age group 20–24 years compared with 33.4% of the Protestants, and a higher proportion, 46.3% compared with 30.9%, had stayed at school to the age of 17 or 18 years. Only 15.7% of the Catholics had been told of the UPAA by their own doctors, compared with 45.7% of the Protestants. [ABSTRACT FROM PUBLISHER]

Later years have seen the growth of a vibrant theoretical discussion on agonistic peace and the importance of creating space for contestation, plurality, and dissensus post-accord. However, there has been very few attempts at embedding agonistic theory in empirical analyses of peace agreements. This study attends to that lacuna by investigating how agonistic principles can be integrated and investigated in peace agreements. We suggest a threefold set of indicators for assessing the degree to which peace agreements are invested with agonistic dynamics: (1) what types of spaces for interaction are offered post-accord; (2) what forms of inclusion are stipulated; and (3) how is the peace agreement framed in terms of conflict termination and consensus/dissensus? We illustrate how the various indicators could be put into motion in concrete analyses applying them to examples from the Oslo Accords, the Belfast Agreement, and the Colombian Peace Agreement. Finally, we discuss four dilemmas and problematiques of integrating agonistic ideas in peace agreements; the issue of power, the mixing of agonistic and liberal ideals in peace agreements, the principle of 'nothing is agreed until everything is agreed' and the related double-edged potential of constructive ambiguity and finally the challenges of implementing peace agreements. Within the United Kingdom (UK), many of the arguments driving devolution and Brexit focused on equality. This article assesses how notions of equality have been shaped over the past two decades. Using a chronology of theoretical, political and public interpretations of equality between 1998 and 2018, the article highlights the shifting positions of Northern Ireland (NI) and the rest of the UK. NI once led the way in relation to equality legislation, and equality was the cornerstone of the Good Friday/Belfast peace agreement. However, the Equality Act 2010 in Great Britain meant that NI was left behind. The nature of future UK/EU relationships and how these might influence the direction and extent of the equality debate in the UK is unclear. Food records or diaries, dietary recalls and FFQ are methods traditionally used to measure dietary intake; however, advancing technologies and growing awareness in personalised health have heightened interest in the application of new technologies to assess dietary intake. Dietary intake data can be used in epidemiology, dietary interventions and in the delivery of personalised nutrition advice. Compared with traditional dietary assessment methods, new technologies have many advantages, including their ability to automatically process data and provide personalised dietary feedback advice. This review examines the new technologies presently under development for the assessment of dietary intakes, and their utilisation and efficacy for personalising dietary advice. New technology-based methods of dietary assessment can broadly be categorised into three key areas: online (web-based) methods, mobile methods and sensor technologies. Several studies have demonstrated that utilising new technologies to provide tailored advice can result in positive dietary changes and have a significant impact on selected nutrient and food group intakes. However, comparison across studies indicates that the magnitude of change is variable and may be influenced by several factors, including the frequency and type of feedback provided. Future work should establish the most effective combinations of these factors in facilitating dietary changes across different population groups. [ABSTRACT FROM PUBLISHER]

This article describes the social care funding and delivery arrangements of a varied selection of developed countries, focusing on long-term care of older people. International evidence and latest reforms can inform the debate as countries struggle economically. Some have opted for mandatory social insurance that provides universal coverage. A premium is paid and if the insured individual or relatives require support, they are entitled to it. Others opted for a similar universal system but with earmarked taxation, while others fund their social care entirely from general taxation. Many chose a safety-net system in which benefits are means-tested leaving wealthier individuals to secure private arrangements of care. Within the UK, the level of support varies as Scotland provides personal care free of charge, being more generous than England, Wales and Northern Ireland. Restorative justice principles often feature prominently in peace agreements and initiatives to foster reconciliation and peace-building. As part of its own transitional process, Northern Ireland has undertaken a wide-ranging programme of criminal justice reform, whereby restorative practices have become a central response to juvenile offending. Drawing on a major evaluation of the Northern Ireland Youth Conferencing Scheme, this paper suggests that restorative conferencing holds the potential not only to promote reconciliation between victims and offenders, but it may even bolster the legitimacy deficit suffered by criminal justice institutions. Whilst is vital that such schemes continue to foster their engagement with civil society and the wider community, the broader potential of restorative processes to contribute to post-conflict peace-building is considerable, especially in relation to fostering a sense of legitimacy necessary for the operation of society and the institutions of the state. Mount Sandel has long been an iconic site for the Irish Mesolithic, having produced evidence for a sequence of occupation huts and pits and the earliest radiocarbon dates for the Mesolithic on the island. This paper presents details of a recent programme of redating whereby the application of Bayesian modelling has confirmed the early date for the site but also helped to refine its internal chronology. The major phase of hut building at Mount Sandel took place within a much shorter period of time than had previously been thought, perhaps only a generation or two. The dating of pits of differing sizes suggests that many of them were created during other slightly later visits to the area. The implications of the dating programme for the place of Mount Sandel in the Irish Mesolithic, and for the chronology of the period and its relations with that on the British mainland, are discussed. The article focuses on the formative years of the Giant's Causeway in north of Ireland as a field site or "philosophical landscape" in the 17th century. It states that the phenomenon of the Giant's Causeway in Ireland has drew much attention for more than five centuries already. It is mentioned that the view of the Giant's Causeway as a self-contained unit in which science is practiced is depicted problematic. It is also seen as part of a network of intersecting venues within which scientists and science disseminates.

This article uses recently released archival material to examine the role of the Irish government in the negotiation of the Sunningdale communiqu? of 1973, which marked, among other things, an agreement to establish a Council of Ireland and was therefore a key part of the first attempt to establish a power-sharing devolved executive in Northern Ireland. The article will problematise the distinctions which have been made between various strains of political thought held by leading intellectuals and politicians on the national question and show how the discourse of ?revisionist nationalism? and reconciliation which sponsored the key institution of the Sunningdale communiqu?, the Council of Ireland, was in contradiction to the meaning attached to the functions of the Council, which was in fact closer to traditional nationalist aims. Let Down (2018) is a practice-as-research (PaR) dance performance that communicates women's experiences of breastfeeding in Northern Ireland, a jurisdiction with one of the lowest breastfeeding rates in the world, due, in part, to the social stigma attached to breastfeeding in public. Choreographed in collaboration with a composer and social scientists, Let Down is a duet for two lactating women who dance alongside a digitally transposed and augmented soundscape of sonic aspects of maternal experience, and improvise to the live sounds produced by infants in the audience. The work responds to a 'quietening' of maternal corporeality in some Western societies through a feminist dramaturgy of sonic disruption that refigures intermedial relations between sound and movement in performance to make unheard experience sensible. Attending to the complex sociopolitical and affective terrain that informed the work's creation, I discuss how a methodology of 'quietening' developed during the choreographic process generated space for a dialogue between private and public spheres of experience. I propose that the methodological concept of quietening offers both an alternative approach to choreographies of affect, and a critical framework for questioning representations of socially 'quietened' corporealities. Objective: The aim of the study was to identify components of the COM-B (capability, opportunity, motivation and behaviour) model that influences behaviour to modify dietary patterns in 40-55-year-olds living in the UK, in order to influence the risk of cognitive decline in later life.Design: This is a qualitative study using the COM-B model and theoretical domains framework (TDF) to explore beliefs to adopting the Mediterranean-DASH Intervention for Neurodegenerative delay (MIND) diet.Setting: Northern Ireland.Participants: Twenty-five participants were recruited onto the study to take part in either a focus group or an interview. Participants were men and women aged between 40 and 55 years. Participants were recruited via email, Facebook and face to face.Results: Content analysis revealed that the main perceived barriers to the adoption of the MIND diet were time, work environment, taste preference and convenience. The main perceived facilitators reported were improved health, memory, planning and organisation, and access to good quality food.Conclusions: This study provides insight into the personal, social and environmental factors that participants report as barriers and facilitators to the adoption of the MIND diet among middle-aged adults living in the UK. More barriers to healthy dietary change were found than facilitators. Future interventions that increase capability, opportunity and motivation may be beneficial. In his well-researched biography, Mike Chinoy chronicles Kevin Boyle's life and career as a scholar, activist and lawyer, bringing to light his under-appreciated role in the civil rights movement in Northern Ireland and the efforts to find a peaceful solution to the conflict, as well as his contributions to human rights movements in the United Kingdom, Europe and the world. Are You With Me? is an important contribution to the literature on the actors who have shaped the norms, institutions and operations of human rights. In these respects Are You With Me? also provides valuable insights for socio-legal scholars, especially those focusing on legal mobilisation. In 2018 Pearson et al. published a new sequence of annual radiocarbon (14C) data derived from oak (Quercus sp.) trees from Northern Ireland and bristlecone pine (Pinus longaeva) from North America across the period 1700–1500 BC. The study indicated that the more highly resolved shape of an annually based calibration dataset could improve the accuracy of 14C calibration during this period. This finding had implications for the controversial dating of the eruption of Thera in the Eastern Mediterranean. To test for interlaboratory variation and improve the robustness of the annual dataset for calibration purposes, we have generated a replicate sequence from the same Irish oaks at ETH Zürich. These data are compatible with the Irish oak 14C dataset previously produced at the University of Arizona and are used (along with additional data) to examine inter-tree and interlaboratory variation in multiyear annual 14C time-series. The results raise questions about regional 14C offsets at different scales and demonstrate the potential of annually resolved 14C for refining subdecadal and larger scale features for calibration, solar reconstruction, and multiproxy synchronization. This article explores popular-musical invocations of the Northern Ireland conflict (1968–1998), focussing specifically on the period between the IRA hunger strike of 1981 and the British Government's Broadcasting Act in 1988. Whilst most songs addressed to the 'Troubles' were marked by (lyrical) abstraction and (political) non-alignment, this period witnessed a series of efforts that issued upfront and partisan views. The article explores two such instances – by That Petrol Emotion and Easterhouse – addressing each band's respective views as well as the specific performance strategies that they deployed in staging their interventions. Drawing on original interviews that the author has conducted with the musicians – alongside extensive archival research of print and audio/visual media – the article explores the bands' songs in conjunction with salient ancillary media (such as record sleeves, videos and interviews), yielding a more nuanced account of popular music's engagement with the 'Troubles' than has been offered in existing work (which often assumes the form of broad surveys). Although sectarian violence characterized life in Belfast for hundreds of years, 1864 marked a shift in how violence played out in the city. Unlike previous conflicts that occurred in open spaces and reflected long-held rural rituals, the riots of August 1864 took place in the city's rapidly developing urban streets. The violence broke out in response to celebrations around the foundation laying for a new statue of Daniel O'Connell, the late Catholic politician, in Dublin. Thousands of Belfast Catholics traveled to Dublin for the celebration. Upon their return to Belfast, ten thousand Protestant loyalists greeted them by burning an effigy of O'Connell on Boyne Bridge and staging a mock funeral and procession that attempted to enter a Catholic burial ground. The resulting violence and rioting continued for ten days on the city streets, where homes and businesses faced destruction on a scale previously unseen. Expelling residents of opposing views, rioters reinforced older ideas of "communal conflict" expressed through "disagreements over each group's place—literally and imaginatively—in the city" and strengthened notions of neighborhood geography based on religious beliefs. As historian Mark Doyle argues, the shifting patterns of violence resulted from "[t]he steady advance of working-class alienation from the state, the growing hegemony of violent extremists in working-class neighbourhoods, the sectarian alliance between Protestant workers and elites, the insecurity of the Catholics and, above all, the polarising effects of earlier outbreaks of violence." Background. To assess the prevalence of autism traits in individuals accessing gender affirming treatments, we conducted a cross-sectional survey in the regional specialist gender services in Northern Ireland. Methods. One hundred and twenty-three individuals (38 adolescents and 69 adults) currently attending or who previously attended specialist gender services in Northern Ireland were recruited. Fifty-six individuals assigned male at birth (AMAB) and 66 individuals assigned female at birth (AFAB) took part in the study. Main outcome measures: Autism Quotient (AQ), Cambridge Behavior Scale (EQ), and RAADS-14. Results. Autism trait prevalence rates of 19.5% (AQ); 25.4% (RAADS-14); and 35.8% (poor empathy traits). A combined measure comprising all three provided a prevalence of 17.2%. There were no mean differences in the scores between AMAB (assigned male at birth) individuals and AFAB (assigned female at birth) individuals. Conclusions. Autism traits present additional challenges during the assessment and treatment of individuals with gender dysphoria. Vitamin D deficiency is a common occurrence globally, and particularly so in pregnancy. There is conflicting evidence regarding the role of vitamin D during pregnancy in non-skeletal health outcomes for both the mother and the neonate. The aim of this study was to investigate the associations of maternal total 25-hydroxy vitamin D (25OHD) with neonatal anthropometrics and markers of neonatal glycaemia in the Belfast centre of the Hyperglycemia and Adverse Pregnancy Outcome (HAPO) study. Serological samples (n 1585) were obtained from pregnant women in the Royal Jubilee Maternity Hospital, Belfast, Northern Ireland, between 24 and 32 weeks’ gestation as part of the HAPO study. 25OHD concentrations were measured by liquid chromatography tandem-MS. Cord blood and neonatal anthropometric measurements were obtained within 72 h of birth. Statistical analysis was performed. After adjustment for confounders, birth weight standard deviation scores (SDS) and birth length SDS were significantly associated with maternal total 25OHD. A doubling of maternal 25OHD at 28 weeks’ gestation was associated with mean birth weight SDS and mean birth length SDS higher by 0·05 and 0·07, respectively (both, P=0·03). There were no significant associations with maternal 25OHD and other measures of neonatal anthropometrics or markers of neonatal glycaemia. In conclusion, maternal total 25OHD during pregnancy was independently associated with several neonatal anthropometric measurements; however, this association was relatively weak. Background: Although substance misuse is a key risk factor in suicide, relatively little is known about the relationship between lifetime misuse and misuse at the time of suicide.Aims: To examine the relationship between substance misuse and subsequent suicide.Method: Linkage of coroners' reports to primary care records for 403 suicides occurring over 2 years.Results: With alcohol misuse, 67% of the cohort had previously sought help for alcohol problems and 39% were intoxicated at the time of suicide. Regarding misuse of other substances, 54% of the cohort was tested. Almost one in four (38%) tested positive, defined as an excess of drugs over the prescribed therapeutic dosage and/or detection of illicit substances. Those tested were more likely to be young and have a history of drug misuse.Conclusions: A deeper understanding of the relationship between substance misuse and suicide could contribute to prevention initiatives. Furthermore, standardised toxicology screening processes would avoid diminishing the importance of psychosocial factors involved in suicide as a 'cause of death'. [ABSTRACT FROM AUTHOR]

Over a decade since the completion of the human genome sequence, the promise of personalised nutrition available to all has yet to become a reality. While the definition was originally very gene-focused, in recent years, a model of personalised nutrition has emerged with the incorporation of dietary, phenotypic and genotypic information at various levels. Developing on from the idea of personalised nutrition, the concept of targeted nutrition has evolved which refers to the delivery of tailored dietary advice at a group level rather than at an individual level. Central to this concept is metabotyping or metabolic phenotyping, which is the ability to group similar individuals together based on their metabolic or phenotypic profiles. Applications of the metabotyping concept extend from the nutrition to the medical literature. While there are many examples of the metabotype approach, there is a dearth in the literature with regard to the development of tailored interventions for groups of individuals. This review will first explore the effectiveness of personalised nutrition in motivating behaviour change and secondly, examine potential novel ways for the delivery of personalised advice at a population level through a metabotyping approach. Based on recent findings from our work, we will demonstrate a novel strategy for the delivery of tailored dietary advice at a group level using this concept. In general, there is a strong emerging evidence to support the effectiveness of personalised nutrition; future work should ascertain if targeted nutrition can motivate behaviour change in a similar manner. BackgroundAgeing is an important factor in the development of mental health problems and their treatment. We assessed age trajectories of common mental disorders (CMDs) and psychotherapy utilization from adolescence to old age, and examined whether these trajectories were modified by time period or birth cohort effects.MethodBritish Household Panel Survey (BHPS) with an 18-year follow-up between 1991 and 2009 (n = 30 224 participants, aged 15–100 years, with an average 7.3 person-observations per person). CMDs were assessed with the 12-item version of the General Health Questionnaire (GHQ). Psychotherapy treatment utilization during the past year was self-reported by the participants. The modifying influences of time period and cohort effects were assessed in a cohort-sequential longitudinal setting.ResultsFollowing a moderate decrease after age 50, the prevalence of GHQ caseness increased steeply from age 75. This increase was more marked in the 2000s (GHQ prevalence increasing from 24% to 43%) than in the 1990s (from 22% to 34%). Psychotherapy utilization decreased after age 55, with no time period or cohort effects modifying the age trajectory. These ageing patterns were replicated in within-individual longitudinal analysis.ConclusionsOld age is associated with higher risk of CMDs, and this association has become more marked during the past two decades. Ageing is also associated with an increasing discrepancy between prevalence of mental disorders and provision of treatment, as indicated by lower use of psychotherapy in older individuals.
