Abstract

Bordetella pertussis es un patógeno exclusivo de humanos que causa la tos ferina, enfermedad respiratoria aguda que afecta principalmente a la población pediátrica. Existen dos tipos de vacunas comercializadas contra este patógeno: celulares y acelulares. Las vacunas celulares han sido extensamente utilizadas y siguen teniendo gran relevancia. El presente trabajo tuvo como objetivo la estandarización de un ELISA para la cuantificación de anticuerpos IgG contra células enteras de Bordetella pertussis. Para ello se determinó la concentración de recubrimiento, el rango lineal de la curva, los parámetros de precisión intra e interensayo, la especificidad, el valor de corte y el límite de detección. Se determinó como concentración de recubrimiento 0,5 UO/mL de células enteras. La curva estándar utilizando un suero de referencia internacional presentó un buen ajuste a una función polinómica en un intervalo entre las diluciones 1/100 y 1/24.300 con un coeficiente de correlación R2≥0,98. Los coeficientes de variación en los ensayos de precisión intra e interensayo estuvieron en los intervalos establecidos para cada uno (≤10%, ≤20% respectivamente). Los resultados obtenidos avalan el empleo de este ELISA cuantitativo para la evaluación de la respuesta a células enteras de Bordetella pertussis en ensayos clínicos.

Abstract

Introduction: Pertussis is a respiratory infection caused by bacteria of the genus Bordetella, mainly pertussis and parapertussis species. Despite the high vaccination coverage in developed countries, it is considered a re-emerging disease that is also underreported and underdiagnosed, especially in patients who do not require hospital referral. Material and methods: Descriptive, prospective and multicentre study of pertussis diagnosis and contact investigation in 17 primary care paediatric clinics through collection of samples for polymerase chain reaction (PCR) testing over a period of 4 years and after the implementation of routine vaccination against pertussis during pregnancy. Results: Pertussis was diagnosed in a total of 50 patients; the estimated incidence in these years was higher compared to previous rates in the paediatric age group. Fourteen percent of the cases occurred in children aged less than 1 year. The mean age was 6.7 years. Cough was present in 100% of cases, followed in frequency by vomiting and rhinorrhoea. Only 1 patient required hospital admission, and none died or developed complications. B. pertussis was the predominant causative agent. Only 40% knew the source of infection. In 26% of the cases, pertussis was confirmed in contacts of the patient by PCR, and in 46% it was suspected based on the clinical presentation but without microbiological confirmation. Conclusions: Access to diagnostic tests (PCR) for pertussis in primary care allows us to optimise its diagnosis and treatment, to break the chain of transmission, to know the real incidence rates and to assess the impact of routine vaccination of pregnant women on this disease. Resumen: Introducción: La tos ferina es una infección respiratoria causada por bacterias del género Bordetella, principalmente por las especies pertussis y parapertussis. A pesar de las altas coberturas vacunales en países desarrollados, está considerada como una enfermedad reemergente existiendo, además, una infranotificación y un infradiagnóstico especialmente en los pacientes que no precisan derivación hospitalaria. Material y métodos: Estudio descriptivo, prospectivo y multicéntrico de diagnóstico de casos de tos ferina, así como el estudio de sus contactos en 17 consultas de pediatría de atención primaria mediante la toma de muestras para realización de técnicas de PCR (reacción en cadena de la polimerasa) a lo largo de cuatro años y tras la implantación de la vacunación sistemática de la tos ferina en el embarazo. Resultados: Se diagnostican un total de 50 pacientes; la tasa de incidencia estimada en estos años fue superior a las publicadas en edad pediátrica. Un 14% de los casos sucedieron en menores de 1 año. La media de edad fue de 6,7 años. La tos estuvo presente en el 100% de los casos, seguida de los vómitos y rinorrea como síntomas más frecuentes. Sólo 1 paciente precisó ingreso y ninguno falleció ni presentó complicaciones. B. pertussis fue el agente causal predominante. Sólo un 40% conocía la fuente de contagio. En un 26% de los casos se comprobó mediante PCR tos ferina en sus contactos y en un 46% se sospechó clínicamente, aunque sin confirmación microbiológica. Conclusiones: El acceso a pruebas diagnósticas (PCR) para tos ferina en Atención Primaria permite optimizar su diagnóstico y tratamiento, cortar la cadena de transmisión, conocer las tasas de incidencia reales y valorar el impacto de la vacunación sistemática de las embarazadas en esta enfermedad.

Abstract

Objetivo. Desarrollar y evaluar un método de bajo costo basado en celulosa para la purificación rápida y amplificación directa de ADN de Bordetella pertussis de hisopados nasofaríngeos. Materiales y métodos. Se prepararon discos de celulosa y se evaluaron diferentes parámetros (buffers de lisis/lavado, número de discos y elución de ADN). El método se acopló a una amplificación directa por PCR en tiempo real (qPCR) y se es­timó el rendimiento utilizando hisopados nasofaríngeos que fueron positivos (n=100) y negativos (n=50) para ADN B. pertussis por qPCR, comparado con el método basado en columnas de sílice. Se calculó el grado de concordancia, sensibilidad, especificidad, valor predictivo positivo (VPP) y valor predictivo negativo (VPN). Se evaluó la factibilidad del método rápido para ser acoplado a un ensayo colorimétrico de amplificación isotérmica mediada por lazo (LAMP). Resultados. El método rápido con un disco de celulosa y buffer de lisis y lavado conteniendo PVP-40 y Tween 20, respectivamente, mostró una mayor capacidad para purificar ADN amplificable de B. pertussis. El método tuvo una sensibilidad de 89,0% (IC95%, 80,2%-94,9%) y una especificidad de 98,5% (IC95%, 92,1%-100,0%), con un buen grado de concordancia (Kappa=0,867; IC95% 0,788 – 0,946), respecto al método referencial. Los VPP y VPN fueron 98,6% (IC95%, 92,7,2%-100,0%) y 88,2% (IC95%, 78,7%-94,4%), respectivamente. Se evidenció una amplificación exitosa por LAMP, y se obtuvieron resultados comparables con el método por columnas de sílice. Conclusión. El método desarrollado es simple, de bajo costo y libre de equipos para la obtención rápida (60 segundos) de ADN en el punto de atención, y puede ser implementado en diversas técnicas moleculares orientados al diagnóstico oportuno y al estudio epidemiológico de tos ferina

Abstract

Background Monitoring effectiveness of pertussis vaccines is necessary to adapt vaccination strategies. PERTINENT, Pertussis in Infants European Network, is an active sentinel surveillance system implemented in 35 hospitals across six EU/EEA countries. We aim to measure pertussis vaccines effectiveness (VE) by dose against hospitalisation in infants aged <1 year. Methods From December 2015 to December 2019, participating hospitals recruited all infants with pertussis-like symptoms. Cases were vaccine-eligible infants testing positive for Bordetella pertussis by PCR or culture; controls were those testing negative to all Bordetella spp. For each vaccine dose, we defined an infant as vaccinated if she/he received the corresponding dose >14 days before symptoms. Unvaccinated were those who did not receive any dose. We calculated (one-stage model) pooled VE as 100*(1-odds ratio of vaccination) adjusted for country, onset date (in 3-month categories) and age-group (when sample allowed it). Results Of 1,393 infants eligible for vaccination, we included 259 cases and 746 controls. Median age was 16 weeks for cases and 19 weeks for controls (p < 0.001). Median birth weight and gestational age were 3,235 g and week 39 for cases, 3,113 g and week 39 for controls. Among cases, 119 (46 %) were vaccinated: 74 with one dose, 37 two doses, 8 three doses. Among controls, 469 (63 %) were vaccinated: 233 with one dose, 206 two doses, 30 three doses. Adjusted VE after at least one dose was 59 % (95 %CI: 36–73). Adjusted VE was 48 % (95 %CI: 5–71) for dose one (416 eligible infants) and 76 % (95 %CI: 43–90) for dose two (258 eligible infants). Only 42 infants were eligible for the third dose. Conclusions Our results suggest moderate one-dose and two-dose VE in infants. O objetivo deste estudo foi investigar a reemergência da coqueluche, notificação e casos confirmados da doença na mesorregião do campo das vertentes, microrregião de São João del-Rei e o estado de Minas Gerais, além de correlacionar o número de casos nessas regiões. Materiais e Métodos: Investigou-se a incidência no estado de Minas Gerais e no município de São João del-Rei, através da coleta de dados no TABNET, DATASUS, SINAN e boletins da Secretaria Municipal de Saúde de São João del-Rei (SMS-SJDR), durante o período compreendido entre 2008 a 2018, considerando os casos suspeitos e os confirmados da doença. Resultados: Os dados permitiram informar a comunidade dos municípios pesquisados fornecendo informações que poderão alertar e prevenir a reemergência dessa doença. Conclusão: O diagnóstico precoce permite sucesso no tratamento e diminuição das complicações da doença. The objective of this study was to investigate the reemergence of pertussis, notification and confirmed cases of the disease in Campos das Vertentes mesoregion, São João del-Rei microregion, state of Minas Gerais, in addition to correlating the number of cases in these regions. Materiales y Métodos: Se investigó la incidencia en el estado de Minas Gerais y en la ciudad de São João del-Rei, a través de la recolección de datos en TABNET, DATASUS, SINAN y boletines de la Secretaría de Salud Municipal de São João del-Rei (SMS) -SJDR), de 2008 a 2018, considerando los casos sospechosos y confirmados de la enfermedad. Resultados: Los datos permitieron informar a la comunidad de los municipios encuestados brindando información que pudiera alertar y prevenir el rebrote de esta enfermedad. Conclusión: El diagnóstico precoz permite el éxito del tratamiento y la reducción de las complicaciones de la enfermedad.

Abstract

[ES] En la última década la tos ferina ha experimentado un resurgimiento en muchas zonas del mundo, incluidos los países con altas coberturas de vacunación. Entre las razones que explican esta reemergencia están la evanescencia de la protección que confiere las vacunas de tos ferina, la sustitución de las vacunas de células enteras por las vacunas acelulares, la mejora en la disponibilidad y sensibilidad de las pruebas diagnósticas y la aparición de cepas que escapan de la inmunidad producida por las vacunas acelulares. La tos ferina mantiene su presentación cíclica y Bordetella pertussis circula en todos los grupos de edad. En España desde el año 2010 la tos ferina se encuentra en epidemia sostenida que alcanzó el pico máximo en la onda 2014-2019. La tos ferina afecta a todas las edades, pero es típicamente una enfermedad infantil con las formas de presentación más graves en niños en los primeros meses de vida. En España la vacunación de tos ferina en las gestantes, que está implantada en todo el país desde 2016, está contribuyendo a la reducción de la enfermedad grave en los lactantes, particularmente en los tres primeros meses de vida, cuando todavía no se ha completado la pauta de primovacunación. Manteniendo altas coberturas con la administración sin demora de las dosis del calendario, (primovacunación a los 2, 4 meses, primer recuerdo a los 11 meses y segundo recuerdo a los 6 años) y promoviendo la vacunación de las gestantes, se reduce la intensidad de las epidemias y sobre todo el riesgo de enfermedad grave entre los más vulnerables. El objetivo general de este estudio es el análisis de los casos, hospitalizaciones y muertes por tos ferina en España en el periodo 2005-2020, evaluando el impacto de la vacunación de las embarazadas sobre la epidemiología de la enfermedad. Se analiza la calidad de la notificación de casos de tos ferina a la RENAVE. [EN] Whooping cough is considered a re-emerging disease in many areas of the world. In recent years, an increase in cases of whooping cough has been observed, both in Spain and in other countries with similar vaccination policies. The evanescence of the protection conferred by the pertussis vaccine and the replacement of whole cell vaccines by acellular vaccines could be contributing to the increase in cases worldwide. In addition, the improvement in the availability of simple and rapid diagnostic tests, such as PCR, facilitates the diagnosis and notification of pertussis cases. Pertussis maintains a cyclical presentation and B. pertussis circulates in all age groups. In Spain, pertussis has been in a sustained epidemic since 2010, peaking in the 2014-2019 wave. Pertussis affects all age groups, but is typically a childhood disease with the most severe forms of presentation in infants in the first months of life. In Spain, pertussis vaccination in pregnant women, implemented nationwide since 2016, is contributing to the reduction of severe disease in infants, particularly in the first three months of life, when the primary vaccination schedule has not yet been completed. Maintaining high coverage with prompt administration of schedule doses (primary vaccination at 2, 4, months, first booster at 11 months and second booster at 6 years) and promoting vaccination of pregnant women reduce the intensity of epidemics and especially the risk of severe disease among the most vulnerable. New pertussis vaccines providing more effective and longer-lasting protection against disease and reducing the circulation of B. pertussis in the population, are needed. The objective of this study is to analyse pertussis cases, hospitalisations and deaths in Spain in the period 2005-2020, assessing the impact of vaccination of pregnant women on the epidemiology of the disease. Background PERTINENT is an active hospital-based surveillance system for pertussis in infants. In 2019, four of the six participating European countries recommended pertussis vaccination in pregnancy. Among infants aged <2 months, we measured the vaccine effectiveness (VE) in pregnancy; among infants aged 2–11 months, VE of vaccination in pregnancy and of primary vaccination (PV). Methods From December 2015 to 2019, we included all infants aged <1 year presenting with pertussis-like symptoms. Using a test-negative-design, cases were infants testing positive for Bordetella pertussis by PCR or culture. Controls were those testing negative for all Bordetella species. Vaccinated mothers were those who received vaccine in pregnancy. Vaccinated infants were those who received =1 dose of PV > 14 days before symptom onset. We excluded infants with unknown maternal or PV status or with mothers vaccinated =14 days before delivery. We calculated pooled VE as 100 * (1-odds ratio of vaccination) adjusted for study site, onset date in quarters and infants’ age group. Results Of 829 infants presenting with pertussis-like symptoms, 336 (41%) were too young for PV. For the VE in pregnancy analysis, we included 75 cases and 201 controls. Vaccination in pregnancy was recorded for 9 cases (12%) and 92 controls (46%), adjusted VE was between 75% [95%CI: 35–91%] and 88% [95%CI: 57–96%]. Of 493 infants eligible for PV, we included 123 cases and 253 controls. Thirty-one cases and 98 controls recorded both PV with = 1 dose and vaccination in pregnancy, adjusted VE was between 74% [95%CI: 33–90] and 95% [95%CI: 69–99]; 27 cases and 53 controls recorded PV only, adjusted VE was between 68% [95%CI: 27–86] and 94% [95%CI: 59–99]. Conclusion Our findings suggest that vaccination in pregnancy reduces pertussis incidence in infants too young for PV. Introducción: la tos ferina es una enfermedad respiratoria, prevenible por vacuna y causada principalmente por la Bordetella pertussis. Fue una de las mayores causas de morbilidad y mortalidad infantil antes del inicio de la vacunación en 1940, posteriormente se registró una disminución de la mortalidad y letalidad. Metodología: estudio descriptivo de corte transversal de casos probables de tos ferina notificados a Sivigila entre 2007 y 2019. La definición de caso confirmado y descartado fue tomada del protocolo de vigilancia en salud pública del Instituto Nacional de Salud. Se depuraron y consolidaron las bases de datos, se calcularon medidas de frecuencia con intervalos de confianza al 95 %, referenciación de casos según las localidades de Bogotá y un modelo de regresión lineal Resultados: entre 2007 y 2019 en Bogotá se notificaron 24.892 casos de tos ferina, el 12,53 % fue confirmado, el 47,75 % contaba con antecedente vacunal, el 66,59 % fue en menores entre 2 meses y 5 años, el 49,28 % consultó en la fase catarral de la enfermedad. Los menores de dos meses (OR: 1,25; IC: 1,13-1,38; p=0.000) y mayores de cinco años (OR: 1,32; IC: 1,14-1,52; p=0.000) en comparación con los niños entre dos meses y cinco años tienes un riesgo mayor de contraer tos ferina; como factor protector se reportó el antecedente vacunal (OR: 0,86; IC: 0,78-0,95; p=0.002). Conclusiones: El aumento de casos de tos ferina en Bogotá tiene relación con coberturas de vacunación por debajo de la meta optima. Se recomienda continuar identificando factores de riesgo para redirigir actividades de promoción y prevención, y continuar con la vacunación en menores de cinco años y gestantes. It was one of the major causes of infant morbidity and mortality before the start of vaccination in 1940, later there was a decrease in mortality and lethality. The objective of the study was to estimate the association of pertussis and social, demographic and clinical variables in a population from Bogotá, cohort 2007-2019. Methodology: descriptive cross-sectional study of probable cases of whooping cough notified to Sivigila between 2007 and 2019. The definition of confirmed and discarded case takes from the public health surveillance protocol of the National Institute of Health. The databases were purged and consolidated, frequency measures calculate with 95% confidence intervals, referencing of cases according to the localities of Bogotá and a linear regression model. Results: between 2007 and 2019 in Bogotá, 24,892 cases of whooping cough reports, 12.53% confirm, 47.75% had a history of vaccination, and 66.59% were in children between 2 months and 5 years, the 49.28% consulted in the catarrhal phase of the disease. Those younger than two months (OR: 1.25; CI: 1.13-1.38; p=0.000) and older than five years (OR: 1.32; CI: 1.14-1.52; p= 0.000) compared to children between two months and five years you have a higher risk of contracting whooping cough; Vaccination history was reported as a protective factor (OR: 0.86; CI: 0.78-0.95; p=0.002). Conclusions: The increase in cases of whooping cough in Bogotá relate to vaccination coverage below the optimal goal. It recommend continuing identifying risk factors to redirect promotion and prevention activities, and continue with vaccination in children under five years of age and pregnant women.

Abstract

Purpose The purpose of this study was to characterize the nasopharyngeal microbiota of infants with possible and confirmed pertussis compared to healthy controls. Methods This prospective study included all infants Results Twelve confirmed cases, 21 possible cases and 9 healthy controls were included. Confirmed whooping cough was primarily driven by detection of Bordetella with no other major changes on nasopharyngeal microbiota. Possible cases had limited abundance or absence of Bordetella and a distinctive microbiota with lower bacterial richness and diversity and higher rates of viral co-infection than both confirmed cases and healthy controls. Bordetella reads determined by 16S rRNA gene sequencing were found in all 12 confirmed cases (100%), 3 out of the 21 possible cases (14.3%) but in any healthy control. Conclusion This study supports the usefulness of 16S rRNA gene sequencing for improved sensitivity on pertussis diagnosis compared to real-time PCR and to understand other microbial changes occurring in the nasopharynx in children

Abstract

Introduction: Pertussis is a respiratory infection caused by bacteria of the genus Bordetella, mainly pertussis and parapertussis species. Despite the high vaccination coverage in developed countries, it is considered a re-emerging disease that is also underreported and underdiagnosed, especially in patients who do not require hospital referral., Material and Methods: Descriptive, prospective and multicentre study of pertussis diagnosis and contact investigation in 17 primary care paediatric clinics through collection of samples for polymerase chain reaction (PCR) testing over a period of 4 years and after the implementation of routine vaccination against pertussis during pregnancy., Results: Pertussis was diagnosed in a total of 50 patients; the estimated incidence in these years was higher compared to previous rates in the paediatric age group. Fourteen percent of the cases occurred in children aged less than 1 year. The mean age was 6.7 years. Cough was present in 100% of cases, followed in frequency by vomiting and rhinorrhoea. Only 1 patient required hospital admission, and none died or developed complications. B. pertussis was the predominant causative agent. Only 40% knew the source of infection. In 26% of the cases, pertussis was confirmed in contacts of the patient by PCR, and in 46% it was suspected based on the clinical presentation but without microbiological confirmation., Conclusions: Access to diagnostic tests (PCR) for pertussis in primary care allows us to optimise its diagnosis and treatment, to break the chain of transmission, to know the real incidence rates and to assess the impact of routine vaccination of pregnant women on this disease., (Copyright © 2022 Asociación Española de Pediatría. Abstract

Pertussis is a highly contagious disease caused by Bordetella pertussis, which may be preventable by vaccination. There are two types of vaccines: whole-cell vaccines and acellular vaccines. Since pertussis control worldwide is heterogeneous, re-emergence of whooping cough has been observed in some countries. This re-emergence has been related to several factors: increased susceptibility to infection, better detection of disease, problems in obtaining adequate vaccination coverage, increase in susceptible subjects (mainly under 6 months of age), loss of immunity in adolescents and young adults, and likely genetic and adaptive B. pertussis changes. This paper discusses whole-cell and acellular vaccines' characteristics, advantages, and disadvantages. International recommendations are presented, and the participants' position is offered regarding the influence of the use of acellular vaccines and the potential disadvantages of reintroducing whole-cell vaccines, mainly due to their reactogenicity. Finally, strategies to achieve better control of pertussis in Mexico are discussed.
