Purpose: To evaluate the difference in ganglion cell layer (GCL) thickness on optical coherence tomography (OCT) and waveform on pattern electroretinography (PERG) among glaucoma suspects and healthy controls. Methods: An analytical, cross-sectional study was done on 100 eyes of 50 subjects, equally divided in the glaucoma suspect and control group. Subjects with an asymmetrically increased vertical cup-to-disc ratio and without any perimetric change were taken as suspects, and subjects with normal findings were taken as controls. GCL thickness was evaluated on spectral domain OCT at 3 mm, 3.45 mm, and 6 mm centering the foveola, and P50, N95 wave forms were recorded according to the standard PERG protocol. Results: In this study, we observed that the average GCL thickness is significantly less in glaucoma suspects at 3.45 mm (p = 0.045) and at 6 mm (p < 0.001) circle zone. On PERG, P50 amplitude was significantly low in glaucoma suspects in comparison to controls (p = 0.007). There was significantly increased implicit time for both P50 and N95 in glaucoma suspects (p < 0.001). For N95 amplitude, a non-significant (p = 0.127) difference was observed among the two groups. At 3.45 mm, average GCL thickness showed a weak negative correlation with N95 implicit time (r = − 0.286, p = 0.044), a weak positive correlation with P50 amplitude (r = 0.349, p = 0.013), and at 6 mm, a weak positive correlation with P50 amplitude (r = 0.311, p = 0.028) in glaucoma suspects. Conclusions: Analysing GCL thickness can help in the structural assessment of preperimetric glaucoma. Abstract Background To report a case of tuberculum meningioma with recovery of glaucoma-like visual field defects after chiasmal decompression. Case presentation A 39-year-old woman presenting with headache was found to have bilateral arcuate retinal nerve fiber layer (RNFL) thinning on optical coherence tomography (OCT) with a corresponding arcuate scotomas consistent with glaucomatous change. However a suprasellar tumor compressing the anterior chiasm from below was found on magnetic resonance imaging of the brain. After resection of the mass, which was diagnosed as meningothelial meningioma by the pathological examination, the glaucoma-like visual field defects resolved despite the RNFL thinning on the OCT showing no improvement. Conclusions Chiasmal compression may mimic glaucoma and produce arcuate scotoma rather than temporal visual field loss. There is a possibility that the development of chiasmal compression somehow converted preperimetric glaucoma into a more advanced form accompanied by visual field defects and that the glaucoma reverted to the preperimetric state after chiasmal decompression.

POAG, or primary open angle glaucoma, is the most common type. The pattern electroretinogram (PERG) has become a promising technique for detecting glaucoma early-on. The goal of this study was to assess the ability of PERG to diagnose POAG, especially in early, difficult-to-diagnose cases in comparison with other already established diagnostic methods.Methods: 150 participants (300 eyes) were enrolled in a cross-sectional study at ophthalmology department at Menoufia University Hospital in August 2022 to February 2023. All recruited participants underwent comprehensive ophthalmological and PERG exams. The studied eyes were divided into three groups as 100 normal eyes (Group I), 100 eyes with preperimetric glaucoma (Group II), and 100 eyes with established perimetric glaucoma (Group III).Results: OCT-RNFL average thickness had a significant positive correlation with P50 latency (r=0.289, p=0.041) or P50 amplitude (r=0.302, p=0.018) and N95 amplitude (r=0.640, p=0.001) among group (II). Also, RNFL thickness had negative correlation with P50 amplitude (r= − 0.268, p=0.043) among group (III). RNFL thickness and P50 and N95 amplitude showed highest AUC values in detecting preperimetric glaucomatous eyes vs normal eyes (AUC=0.927, 0.952, 904), and for detecting established perimetric glaucomatous eyes vs normal eyes (AUC=1.00, 0.957, 0.983 respectively) compared with VF MD which showed AUC (0.458 and 0.901 respectively).Conclusion: Glaucoma patients exhibit PERG alterations (comparable to RNFL thickness changes) so, it could be used as an accurate diagnostic method in POAG. Because PERG alterations occur before visual field abnormalities, it could be relied on as an early diagnostic tool in preperimetric glaucoma. We can use both RNFL thickness assessment by OCT with PERG parameters as complementary tests for accurate diagnosis of preperimetric glaucoma which represents the most difficult diagnostic challenge in glaucoma diagnosis.Keywords: pattern electroretinogram, preperimetric glaucoma, primary open angle glaucoma, RNFL thickness

PurposeWe evaluated the clinical outcomes after implantation of a new monofocal intraocular lens (IOL) with enhanced intermediate function in patients with preperimetric glaucoma and compared those with patients without retinal nerve fiber layer (RNFL) defects.MethodsAll patients were implanted bilaterally a new monofocal IOL with enhanced intermediate function. Patients with bilateral RNFL defects and no glaucomatous visual field defects were classified as the preperimetric glaucoma group. A total of 60 eyes of 30 patients with preperimetric glaucoma were compared with 60 eyes of 30 patients without RNFL defects. Uncorrected distance visual acuity (UDVA), uncorrected intermediate visual acuity (UIVA), uncorrected near visual acuity (UNVA), defocus curve, contrast sensitivity, and questionnaire were evaluated 1 month and 3 months after surgery.ResultsNo difference in binocular UDVA, UIVA, and UNVA was evident between the two groups at 1 and 3 months postoperatively. Additionally, there were no significant differences between the two groups regarding the proportion of severe or very severe photic phenomena, such as glare and halos, or the overall satisfaction.ConclusionBilateral implantation of a new monofocal IOL with enhanced intermediate function in patients with preperimetric glaucoma demonstrated commensurate clinical outcomes and could be considered a feasible alternative.

Background: Spectral-domain optical coherence tomography (OCT) can identify the presence of localized glaucomatous structural damage. The peripapillary retinal nerve fiber layer (RNFL) thickness is a popular OCT parameter used for glaucoma diagnosis and monitoring progression. Aim: To measure the peripapillary RNFL thickness, the pattern of RNFL defect, and association of RNFL with the severity of glaucoma using Spectralis Spectral Domain – OCT in patients with primary open-angle glaucoma (POAG) at a glaucoma clinic in a tertiary care teaching hospital in south India. Methods: Cross-sectional study design. Peripapillary RNFL thickness of 109 glaucomatous eyes of 56 patients aged 40–75 years having POAG, ocular hypertension, or suspected to have glaucoma but without visual field (VF) defects were included in the study. The cross-sectional thickness of RNFL using a 3.46 mm circle centered around the disc in 6 segments were measured and defects in the RNFL were identified by comparing measurements from each patient with the normative database. Results: The study included 109 eyes with 51 (46.79%) preperimetric eyes and 58 (53.21%) POAG eyes (with field defects). The average global thickness in the study population was 91.31 ± 12.81 μm for preperimetric eyes and 67.76 ± 17.33 μm for eyes with glaucoma. There was significant thinning in all segments between eyes with POAG and normal eyes with the mean difference highest in the inferotemporal segment (mean difference of 73.05) and the superotemporal segment (mean difference of 55.85). The global RNFL thickness had a coefficient of − 0.015 (standard error [SE] 0.003, 95% confidence interval [CI]: −0.02–−0.009, R2 = 0.29) for mild glaucoma, −0.010 (SE 0.004, 95% CI: −0.02–−0.003, R2 = 0.12) for moderate and − 0.18 (SE: 0.001, 95% CI: −0.022–−0.014, R2 = 0.60) for severe glaucoma compared to preperimetric eyes in a linear regression model that adjusted for age and gender. Global and sector-wise RNFL thickness also had a significant area under Receiver Operator Characteristic (0.84, 95% CI: 0.78. 0.91) that showed the ability to discriminate between preperimetric and glaucomatous eyes. Possible “floor effect” of RNFL thickness was shown by 7 eyes with advanced glaucoma cases. Conclusion: RNFL thinning can be used as a parameter to assess glaucomatous structural loss that can aid in early diagnosis and monitoring of disease progression, to grade the severity of the disease in primary open-angle glaucoma, and when VFs cannot be done.

Purpose: To investigate macular thickness (MT), ganglion cell complex (GCC), and retinal nerve fiber layer (RNFL) thickness changes in preperimetric glaucoma patients. Materials and Methods: In the present study 105 eyes of 75 patients were investigated. Of these patients, 56 eyes of 45 patients who were diagnosed as preperimetric glaucoma are taken as a study group and 49 eyes of 30 cases without any ocular pathology are taken as the control group. The mean values of central MT, parafoveal MT, perifoveal MT, parafoveal GCC thickness, and perifoveal GCC thickness evaluations using optical coherence tomography (OCT) were compared between the two groups. Peripapillary RNFL measurements; the mean values of the total, superior hemi-central, inferior hemi-central, superior, inferior, nasal, and temporal quadrants, and twelve sectors were compared between the two groups. Results: In the study group, perifoveal GCC thickness was significantly thinner in the superonasal, superotemporal, inferonasal, and inferotemporal quadrants compared to the control group. Peripapillary RNFL thickness was significantly thinner in the study group in total, superior hemi-central, inferior hemi-central, superior, inferior, nasal, temporal, and hour 1,2,3,4,5,7,8,9 quadrants. No significant difference was detected between the two groups in other measurements (p>0.05). Conclusion: However both macular and peripapillary cellular losses have been initiated in cases with preperimetric glaucoma, this loss can not be detected by standard automated perimetry. As a result, perifoveal GCC and peripapillary RNFL measurements by OCT in patients with preperimetric glaucoma are very helpful for diagnosing suspicious cases. [ABSTRACT FROM AUTHOR]

Purpose: To investigate the vessel density (VD) of the radial peripapillary capillary (RPC) plexus in patients affected by preperimetric glaucoma (PPG), amnestic mild cognitive impairment (aMCI) and in a healthy control group using optical coherence tomography angiography (OCTA) in order to clarify the pathogenetic mechanisms of these neurodegenerative diseases. Methods: In this prospective study, we studied 54 eyes of 54 patients with PPG, 54 eyes of 54 patients with aMCI and 54 healthy controls. All subjects underwent structural spectral domain optical coherence tomography (SD)-OCT to assess the ganglion cell complex (GCC) and the retinal nerve fibre layer (RNFL). OCTA was used to evaluate the VD of the RPC in different regions (whole image, inside disc and peripapillary). Results: The PPG and aMCI groups showed a statistically significant reduction in SD-OCT and parameters with respect to controls (p < 0.001). No statistically significant difference was found in GCC and RNFL parameters between the two study groups (p > 0.05). At OCTA examination, PPG and aMCI patients exhibited a statistically significant reduction in the VD of the RPC in whole image, inside and peripapillary regions compared to healthy controls (p < 0.001). When comparing the two study groups, the OCTA parameters were significantly impaired in PPG with respect to aMCI patients. Significant correlations were found between structural OCT and OCTA parameters in PPG and aMCI groups (p < 0.05). Conclusions: RPC vessel density could represent a helpful and sensible biomarker to identify early retinal microvascular changes in PPG and MCI in order to better understand the vascular pathophysiological mechanisms involved in these neurodegenerative diseases. Purpose: To evaluate diagnostic performance of ganglion cell inner plexiform layer (GCIPL) and retinal nerve fiber layer (RNFL) parameters measured with Cirrus high-definition optical coherence tomography (OCT) in patients with preperimetric glaucoma., Methods: In this multicenter cross-sectional study, 150 eyes of 83 patients with preperimetric glaucoma were compared with 200 eyes of age and sex matched healthy subjects. All patients had visual field testing and OCT scanning of GCIPL and RNFL in all quadrants. The independent Samples t -test was used to determine if a difference exists between the means of two independent groups on a continuous dependent variable. The area under the receiver operating characteristic (ROC) curve (AUC) of each parameter was calculated for discriminatory ability between normal controls and preperimetric glaucoma. The sensitivity and specificity were estimated by point coordinates on ROC curve., Results: The best parameters for distinguishing preperimetric glaucoma from healthy eyes were the combined average GCIPL + average RNFL, followed by average RNFL + GCIPL (inferotemporal), and average RNFL + GCIPL (minimum). The GCIPL parameters with the highest to lowest AUC (in decreasing order) were inferotemporal, followed by average, minimum, superior, inferior, superonasal, inferonasal, superotemporal, and quadrants. The RNFL parameters with the highest to lowest AUC (in decreasing order) were average, followed by nasal, temporal, superior, and inferior quadrants. The sensitivity of combined GCIPL + RNFL parameters ranged 85%-88% and the specificity ranged 76%-88%. • The vascular involvement in choriocapillary region play a crucial role in the etiopathogenesis of preperimetric glaucoma (PPG) and perimetric glaucoma (PG). • Optical coherence tomography angiography (OCTA) represents a non-invasive imaging technique that could help to clarify, in detail, the role of choriocapillary in the glaucoma ethiopatogenesis. • In our results the vessel density of choriocapillary turned to be significantly reduced in PPG and PG patients respect to healthy subjects. • The quantitative analysis with OCT-A could represent a useful biomarker to highlight the vascular role in the etiopathogenesis of these diseases. To investigate the vessel density (VD) of the choriocapillary (CC) plexus in patients affected by preperimetric glaucoma (PPG), advanced glaucoma (AG) and in a healthy control group using Optical Coherence Tomography Angiography (OCTA) in order to clarify the pathogenetic mechanisms of this disease. In this prospective observational study, 19 eyes from 19 patients affected by preperimetric glaucoma (PPG) and 18 eyes from 18 patients affected by advanced glaucoma were studied from January 2022 to May 2022 at the University of Naples "Federico II". These patients had been compared with 20 eyes of 20 healthy subjects that represented the control group. All subjects underwent EDI-OCT to assess the subfoveal choroidal thickness (SFCT). OCTA was used to evaluate the vessel density (VD) of the CC in whole image in the studies groups. The PPG and AG groups showed a statistically significant reduction in CC vessel density parameters with respect to controls (p < 0.001). Regarding EDI OCT results AG patients exhibited a statistically significant increase in the SFCT compared to healthy controls (p < 0.001). Whereas,no statistically significant difference was between the PPG groups and to healthy controls (p 0.851). CC vessel density could represent a helpful and sensible biomarker to identify early choroidal microvascular changes in PPG and MCI in order to better understand the vascular pathophysiological mechanisms involved in glaucoma diseases.
