Background: Spinal manipulation (SM) has been hypothesized to influence the autonomic nervous system (ANS). Further, it has been proposed that the effects may vary depending on the segment manipulated. The aim of this systematic review was to synthesize the current level of evidence for SM in influencing the ANS in healthy and/or symptomatic population. Methods: Various databases (n = 8) were searched (inception till May 2023) and 14 trials (n = 618 participants) were included in the review. Two authors independently screened, extracted and assessed the risk of bias in included studies. The data were synthesized using standard mean differences and meta-analysis for the primary outcome measures. The Grading of Recommendations, Assessment, Development and Evaluation (GRADE) was used for assessing the quality of the body of evidence for each outcome of interest. Results: Overall, there was low quality evidence that SM did not influence any measure of ANS including heart rate variability (HRV), oxy-hemoglobin, blood pressure, epinephrine and nor-epinephrine. However, there was low quality evidence that cervical spine manipulation may influence high frequency parameter of HRV, indicating its influence on the parasympathetic nervous system. Conclusion: When compared with control or sham interventions, SM did not alter the ANS. Due to invalid methodologies and the low quality of included studies, findings must be interpreted with great caution. Objective: To investigate whether sensitivity to movement‐evoked pain (SMEP), central sensitivity symptom burden, and quantitative sensory testing (QST) outcomes differ between healthy controls and people with chronic shoulder pain. Methods: People with chronic shoulder pain (n = 39) and healthy controls (n = 26) completed validated questionnaires measuring demographic, pain characteristics, psychological factors, social support, sleep quality, central sensitivity inventory (CSI), and physical activity levels. A blinded assessor administered QST measuring pressure pain threshold, temporal summation, conditioned pain modulation, and cold hyperalgesia. All participants performed repeated lifting of weighted canisters and reported severity of pain over successive lifts of the weighted canisters. Between‐group differences in the QST, SMEP and CSI scores were investigated. Demographic and psychosocial variables were adjusted in the analyses. Results: Dynamic mechanical allodynia, mechanical temporal summation, movement‐evoked pain scores, SMEP index, and CSI scores were significantly (p ≤ 0.05) higher in the chronic shoulder pain group than in healthy controls. A significant proportion of people with chronic shoulder pain presented with pro‐nociceptive profiles and experienced higher pain severity, interference, and disability. Conclusions: People with chronic shoulder pain displayed symptoms and signs of central sensitization. 10-15% of individuals who sustain a concussion develop persistent post-concussion symptoms (PPCS). Presently, there is no consensus regarding the pathophysiology of PPCS. Preliminary evidence suggests that individuals with PPCS have either a dysfunctional autonomic nervous system (ANS) or hypothalamic-pituitary-adrenal axis (HPA-axis). The ANS and HPA-axis collectively make up the two arms of the peripheral stress response. In the presence of a stressor, the two arms work interchangeably to return the body back to homeostasis. With concussion as the stressor, it is plausible that the development and maintenance of PPCS are due to a dysfunctional stress response. Only one study has explored the relationship between PPCS and the two arms of the stress response (ANS and HPA-axis) in combination, which presents a clear gap in the literature. The proposed protocol aims to determine the extent of evidence for dysfunction of the stress response in individuals with PPCS. This review will individually map the literature surrounding quantifiable changes in the (1) ANS and (2) HPA-axis in individuals with PPCS. The review will also highlight any (3) knowledge gaps in this emerging field of research. A scoping review study design has been chosen to facilitate a thorough literature search on this broad exploratory topic. The scoping review proposed will follow the recommended methodological guidelines. Understanding the potential role of a dysfunctional stress response in those with PPCS will create more effective treatment pathways and reduce the chronicity of this condition. Shoulder pain is a common musculoskeletal complaint associated with a significant impact on health and socioeconomic outcomes. Current evidence indicates a poor understanding of the mechanisms driving shoulder pain experience. The heightened nociceptive processing (pro-nociceptive pain modulation profile) within the central nervous system could explain variability in the shoulder pain experience. This topical review and hypothesis-generating paper discuss the current evidence and gaps regarding abnormal nociceptive processing in patients with shoulder pain. This paper also proposes hypotheses to test the presence of a pro-nociceptive pain modulation profile by measuring the pain responses to conditioned pain modulation, mechanical temporal summation and movement-evoked pain procedures. Moreover, this paper proposes whether a pro-nociceptive pain modulation profile can explain the variability in the pain experience in people with shoulder pain. Gaining insights into the abnormal nociceptive processing in people with acute and chronic shoulder pain will guide future research and clinical practice to improve shoulder pain outcomes. Cervical spine manual therapy has been shown to differentially modulate the stress response in healthy populations. No study has investigated whether cervical spine mobilizations can differentially modulate the stress response in individuals with persistent post-concussion symptoms (PPCS), a population characterized by a dysfunctional stress response.A randomized, controlled, parallel design trial was performed to investigate whether upper or lower cervical spine mobilization can differentially modulate components of the stress response in individuals with PPCS. The outcomes were salivary cortisol (sCOR) concentration (primary) and the HRV metric, rMSSD, measured with a smartphone application (secondary). Nineteen males diagnosed with PPCS, aged 19–35, were included. Participants were randomly assigned into either intervention group, upper (n = 10) or lower (n = 9) cervical spine mobilization. Each outcome was collected at different time points, pre- and post-intervention. Statistical analyses were performed using the Friedman’s Two-Way ANOVA, Mann-Whitney U test, and Wilcoxon Signed Rank Test.There was a statistically significant within-group reduction in sCOR concentration 30 minutes following lower cervical spine mobilizations and statistically significant within-group increase in rMSSD 30 minutes following upper cervical spine mobilizations.The results of this trial provide preliminary evidence for cervical spine mobilizations to differentially modulate components of the stress response at specific time points. Understanding the mechanisms of the effect of cervical spine mobilizations on the stress response provides a novel rationale for selecting cervical spine mobilizations to rehabilitate individuals with PPCS.
