Abstract

Background The haemodynamic effects of a functioning haemodialysis arteriovenous fistula (AVF) can cause or exacerbate heart failure (HF). We investigated whether the presence of an AVF at the time of kidney transplant (KT) is associated with de novo HF. Methods This was an observational cohort study including adult patients who received a KT in the West of Scotland between 2010 and 2020. We evaluated the risk and associations of pretransplant factors with de novo HF, alone and as a composite cardiovascular (CV) outcome (including non-fatal myocardial infarction, non-fatal stroke, de novo HF and CV death). Multivariable proportional hazards regression and sensitivity analyses were used to identify independent correlates of the outcomes. Results Among 1330 included patients, the incident rate of de novo HF after transplantation was 58/1000 person-years [95% confidence interval (CI) 50–67] in AVF patients (n  = 716) compared with 33/1000 person-years (95% CI 27–41) in non-AVF patients (n  = 614). De novo HF was associated with the presence of an AVF [adjusted hazard ratio (aHR) 2.14 (95% CI 1.40–3.26)], duration of dialysis [aHR 1.03/year increase (95% CI 1.01–1.04)], age at transplant [aHR 1.03/year increase (95% CI 1.02–1.05)], female sex [aHR 1.93 (95% CI 1.40–2.65)] and pretransplant diabetes [aHR 2.43 (95% CI 1.48–4.01)]. The presence of an AVF was also associated with the composite CV outcome [aHR 1.91 (95% CI 1.31–2.78)]. Abstract

Introduction:\ud \ud Treatment of anaemia in dialysis patients has been associated with increased risk of vascular access thrombosis (VAT). PIVOTAL was a clinical trial of proactive compared to reactive intravenous iron therapy in patients requiring haemodialysis. We analysed the trial data to determine whether randomised treatment arm, alongside other clinical and laboratory variables, independently associated with VAT.\ud \ud Methods:\ud \ud In PIVOTAL, 2141 adult patients were randomised. The type of vascular access (AVF, AVG or CVC) was recorded at baseline and every month after randomization. The associations between clinical and laboratory data and first VAT were examined in a multivariate analysis.\ud \ud Results:\ud \ud 480 (22.4%) participants experienced VAT over a median 2.1 years follow up. 29. Effect of Coxiella burnetii infection on milk constituents and cow behaviour

Abstract

Context. The role of Coxiella burnetii in intramammary infection (IMI) in dairy cows is not fully understood. Aims. The objective of this study was to investigate changes in milk constituents and behaviour such as daily activity (arbitrary unit/day) and daily rumination (min/day) in cows exposed to C. burnetii. Methods. In total, 1029 quarter milk samples were manually collected from 48 cows before and after alveolar milk ejection in the automatic milking rotary at the University of Sydney’s dairy farm. Each milk sample was analysed for the following parameters: immunoglobulin G (cIgG) against C. burnetii via ELISA, somatic cell count (SCC), total immunoglobulin G (tIgG), lactate dehydrogenase (LDH), serum albumin (SA), milk protein%, milk fat%, and subjected to microbiological culture. The daily activity and daily rumination changes were recorded using heat-and rumination long-distance tags across 21 days before detection of IMI (n = 42 cows). Linear and logistic mixed models were used, with ‘cow’ and ‘quarter nested within cow’ as random effects. Results. The presence of cIgG was quarter-specific; the cIgG+ quarters (n = 64) had significantly greater tIgG (P < 0.001), LDH (P < 0.001), SA (P < 0.001) and milk protein% (P = 0.002) than did cIgG− quarters (n = 279). The cIgG+ quarters had significantly greater SCC, tIgG, LDH and SA responses than did controls (P < 0.05), but lower responses than did Gram-negative coliform IMI (P < 0.05). Gram-positive IMI caused by coagulase positive/negative Staphylococcus, Streptococcus uberis, Streptococcus dysgalactiae, Corynebacterium spp. in cIgG+ quarters resulted in greater tIgG, LDH and SA responses than in control quarters (P < 0.05). Coagulase-positive Staphylococcus IMI was associated with the presence of cIgG as assessed by Fisher’s exact test (P < 0.05). The cIgG+ group had a significant (P < 0.05) reduction in daily rumination compared with the cIgG− group in the study period. Conclusions and implications. The cIgG antibody responses are quarter specific with greater tIgG, LDH, SA and milk protein in the affected quarters, as well as behavioural changes in the cow, and therefore might be useful for detection of C. burnetii IMI.

Abstract

Background Lymphocyte ratios reflect inflammation and have been associated with adverse outcomes in a range of diseases. We sought to determine any association between neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) and mortality in a haemodialysis cohort, including a coronavirus disease 2019 (COVID-19) infection subpopulation. Methods A retrospective analysis was performed of adults commencing hospital haemodialysis in the West of Scotland during 2010–21. NLR and PLR were calculated from routine samples around haemodialysis initiation. Kaplan–Meier and Cox proportional hazards analyses were used to assess mortality associations. Results In 1720 haemodialysis patients over a median of 21.9 (interquartile range 9.1–42.9) months, there were 840 all-cause deaths. NLR but not PLR was associated with all-cause mortality after multivariable adjustment [adjusted hazard ratio (aHR) for in participants with baseline NLR in quartile 4 (NLR ≥8.23) versus quartile 1 (NLR <3.12) 1.63, 95% confidence interval (CI) 1.32–2.00]. The association was stronger for cardiovascular death (NLR quartile 4 versus 1 aHR 3.06, 95% CI 1.53–6.09) than for non-cardiovascular death (NLR quartile 4 versus 1 aHR 1.85, 95% CI 1.34–2.56). In the COVID-19 subpopulation, both NLR and PLR at haemodialysis initiation were associated with risk of COVID-19-related death after adjustment for age and sex (NLR: aHR 4.69, 95% CI 1.48–14.92 and PLR: aHR 3.40, 95% CI 1.02–11.36; for highest vs lowest quartiles). Conclusions NLR is strongly associated with mortality in haemodialysis patients while the association between PLR and adverse outcomes is weaker. 38. HER2/neu Levels, Premature Termination of Pregnancy and Breast Cancer Risk: A Hospital-Based Study from Lahore, Pakistan.

Abstract

Breast cancer is a leading cause of death in women. The objective of the present study was to evaluate diagnostic utility of HER2/neu test and the association of premature termination of pregnancy with breast cancer. Two hundred breast cancer patients from INMOL hospital, Lahore (Pakistan) were interviewed on reproductive factors. HER2/neu levels were determined by ELISA from 52 patients and 37 age-matched control women. The results of survey study showed that “premature termination of pregnancy” emerges as a potential risk factor of breast cancer. Number of pregnancies has significant association with premature termination of pregnancies (P = 6 × 10–11). The regression coefficient for number of pregnancies is 0.220 ± 0.029= 1.25× i.e. a 25% increase for each additional pregnancy. There is a significant positive association between premature termination of pregnancies and number of children (P = 0.024): the regression coefficient is 0.108±0.048=1.11× i.e. 11% increase for each additional child. The analysis of Her2/neu showed that its levels were higher in younger patients (≤ 42 years) than in older patients (0.884 vs 0.614 units, P= 0.049). The ROC curve indicated that the optimal test (maximum Se + Sp) occurred at HER-2 Neu values of = 0.615, where Se= 0.73 and Sp= 0.49. The AUC value for this test was 0.57, with 1 being a perfect test, and 0.5 being equivalent to random allocation. In brief, premature termination of pregnancy/ miscarriage is a potential risk factor and HER2/neu cannot be used as a suitable diagnostic test. [ABSTRACT FROM AUTHOR]

Abstract

Mesophyll conductance (g m) is an important factor limiting photosynthesis. However, g m response to long-term growth in variable [CO2] is not well understood, particularly in crop plants. Here, we grew two cultivars of wheat (Halberd and Cranbrook), known to differ in g m under current environmental conditions, in four [CO2] treatments: glacial (206 μmol mol−1), pre-industrial (344 μmol mol−1), current ambient (489 μmol mol−1), and super-elevated (1085 μmol mol−1), and two water treatments (well-watered and moderate water limitation), to develop an evolutionary and future climate perspective on g m control of photosynthesis and water-use efficiency (WUE). In the two wheat genotypes, g m increased with rising [CO2] from glacial to ambient [CO2], but declined at super-elevated [CO2]. The responses of g m to different growth [CO2] also depend on water stress; however, the specific mechanism of g m response to [CO2] remains unclear. Although g m and g m/ g sc (mesophyll conductance/stomatal conductance) were strongly associated with the variability of photosynthetic rates (A) and WUE, we found that plants with higher g m may increase A without increasing g sc, which increased WUE. 40. Early-cannulation arteriovenous grafts: Multidisciplinary learning is essential to optimize outcomes

Abstract

Background: It is likely that there will be an increasing role for early-cannulation arteriovenous grafts (ecAVG) with a wider recognition of the need to tailor vascular access to avoid futile procedures and unnecessary TCVC. However, experience of these products is not common and limited to early surgical adopters, with little information on the systemic changes and multi-disciplinary care needed to optimize outcomes. The aim of this study was to report the impact of a multi-disciplinary approach on quantifiable outcomes.Methods: A retrospective analysis of a prospectively maintained database of 295 ecAVG implanted over an 8-year time-period was performed. Indicative outcomes were chosen to reflect nephrology (patient selection), nursing care (cannulation complications of infection and pseudoaneurysm) and radiology (thrombosis) on cumulative impact (functional patency) over three distinct time periods.Results: The incidence of ecAVG increased 10-fold over the three time periods. The use of ecAVG changed significantly from salvage tertiary access to TCVC avoidance and salvage of existing AVF. Nursing complications reduced markedly with significantly fewer over-cannulation episodes and pseudo-aneurysms. With an improved pro-active surveillance programme, the time to first thrombosis doubled and the risk of thrombosis halved. Ultimately this resulted in significantly improved functional patency with a risk of ecAVG loss less than one-third by the last time-period.Conclusions: All aspects of ecAVG use require scrutiny and critical appraisal. Failure or success is not simply achieved by performing good technical surgery with an efficacious product, but by the care taken across a wide range of elements spanning case selection, implantation, use and maintenance.
