Objective: To investigate the effectiveness of the Melanoma Care Programme when implemented into routine clinical practice coupled with fear of cancer recurrence (FCR) screening and a stepped‐care model of intervention delivery. Methods: Using a Type‐I hybrid effectiveness‐implementation design, individuals with stage 0‐II melanoma and a Fear of Cancer Recurrence Inventory FCR severity score of ≥ 13 were offered the Melanoma Care Programme. The programme included a psychoeducational booklet and 3 to 5 psychotherapeutic telehealth sessions with a clinical psychologist, timed around routine dermatological appointments. Multivariable linear mixed modelling was used to analyse the effect of the intervention at 1‐week post‐intervention on patient‐reported outcomes, including FCR severity (primary outcome), symptoms of depression, anxiety, and stress, melanoma‐related knowledge, and health‐related quality of life. Results: One hundred and twelve participants completed the intervention from 146 participants screened for FCR. Adjusted multivariable linear mixed modelling demonstrated that participants who received the intervention reported a reduced FCR severity at 1‐week post‐intervention (mean change: −3.81 [95% CI: −4.67, −2.95], p < 0.001) compared to baseline. Participants also reported improvements in melanoma‐related knowledge (mean change: 0.64 [95% CI: 0.13, 1.15], p = 0.014), depressive symptoms (mean change: −1.41 [95% CI: −1.92, −0.90], p < 0.001), anxiety (mean change: −1.05 [95% CI: −1.48, −0.61], p < 0.001), stress (mean change: −1.58 [95% CI: −2.22, −0.93], p < 0.001), and health‐related quality of life (mean change: 4.05 [95% CI: 2.84, 5.26], p < 0.001). Conclusions: The Melanoma Care Programme maintained effectiveness when implemented into routine clinical practice with the addition of FCR screening and a stepped care model of delivery. Population‐wide skin cancer screening is not currently recommended in most countries. Instead, most clinical guidelines incorporate risk‐based recommendations for skin checks, despite limited evidence around implementation and adherence to recommendations in practice. We aimed to determine adherence to personal risk‐tailored melanoma skin check schedules and explore reasons influencing adherence. Patients (with/without a previous melanoma) attending tertiary dermatology clinics at the Melanoma Institute Australia, Sydney, Australia, were invited to complete a melanoma risk assessment questionnaire via iPad and provided with personal risk information alongside a risk‐tailored skin check schedule. Data were collected from the risk tool, clinician‐recorded data on schedule deviations, and appointment booking system. Post‐consultation, we conducted semi‐structured interviews with patients and clinic staff. We used a convergent segregated mixed methods approach for analysis. Interviews were audio recorded, transcribed and data were analysed thematically. Participant data were analysed from clinic records (n = 247) and interviews (n = 29 patients, 11 staff). Overall, there was 62% adherence to risk‐tailored skin check schedules. In cases of non‐adherence, skin checks tended to occur more frequently than recommended. Decisions to deviate were similarly influenced by patients (44%) and clinicians (56%). Themes driving non‐adherence among patients included anxiety and wanting autonomy around decision‐making, and among clinicians included concerns around specific lesions and risk estimate accuracy. There was moderate adherence to a clinical service program of personal risk‐tailored skin check recommendations. Further adherence may be gained by incorporating strategies to identify and assist patients with high levels of anxiety and supporting clinicians to communicate risk‐based recommendations with patients. Background: We investigated differences in cumulative incidence of first distant recurrence (DR) following non-metastatic breast cancer over a time period when new adjuvant therapies became available in Australia. Methods: We conducted a health record linkage study of females with localized (T1-3N0) or regional (T4 or N+) breast cancer in the New South Wales Cancer Registry in 2001 to 2002 and 2006 to 2007. We linked cancer registry records with administrative records from hospitals, dispensed medicines, radiotherapy services, and death registrations to estimate the 9-year cumulative incidence of DR and describe use of adjuvant treatment. Results: The study included 13,170 women (2001-2002 n = 6,338, 2006-2007 n = 6,832). The 9-year cumulative incidence of DR was 3.6% [95% confidence interval (CI), 2.3%-4.9%] lower for 2006-2007 diagnoses (15.0%) than 2001-2002 (18.6%). Differences in the annual hazard of DR between cohorts were largest in year two. DR incidence declined for localized and regional disease. Decline was largest for ages <40 years (absolute difference, 14.4%; 95% CI, 8.3%-20.6%), whereas their use of adjuvant chemotherapy (2001-2002 49%, 2006-2007 75%) and HER2-targeted therapy (2001-2002 0%, 2006-2007 16%) increased. DR did not decline for ages =70 years (absolute difference, 0.9%; 95% CI, -3.6%-1.8%) who had low use of adjuvant chemotherapy and HER2-targeted therapy. Conclusions: This whole-of-population study suggests that DR incidence declined over time. Decline was largest for younger ages, coinciding with changes to adjuvant breast cancer therapy. Skin cancer is a growing global problem and a significant health and economic burden. Despite the practical necessity for skin cancer to be managed in primary care settings, little is known about how quality of care is or should be measured in this setting. This scoping review aimed to capture the breadth and range of contemporary evidence related to the measurement of quality in skin cancer management in primary care settings. Six databases were searched for relevant texts reporting on quality measurement in primary care skin cancer management. Data from 46 texts published since 2011 were extracted, and quality measures were catalogued according to the three domains of the Donabedian model of healthcare quality (structure, process and outcome). Quality measures within each domain were inductively analysed into 13 key emergent groups. These represented what were deemed to be the most relevant components of skin cancer management as related to structure, process or outcomes measurement. Four groups related to the structural elements of care provision (e.g. diagnostic tools and equipment), five related to the process of care delivery (e.g. diagnostic processes) and four related to the outcomes of care (e.g. poor treatment outcomes). A broad range of quality measures have been documented, based predominantly on articles using retrospective cohort designs; systematic reviews and randomised controlled trials were limited. This article unpacks the intricacies of cultural identity, language, resistance and ancestral connection in the works of Caribbean-Canadian poet Marlene Nourbese Philip. Applying postcolonial and diaspora theoretical frameworks, the analysis reveals Philip's linguistic experimentation as a process of "writing back" against colonial hierarchies. Her innovative use of creolized syntax, vernacular idioms, and fragmented grammar enacts new anti-colonial meanings. Equally, Philip's thematic interrogation of fractured histories and spiritual loss critically engages the psychic implications of displacement. The article examines selected poems demonstrating how Philip contests Orientalist paradigms that exoticize diasporic cultures as static or inferior. Overall, Philip's poems are shown to navigate complex identity hybridities while underscoring continuities with African origins and epistemologies counter to ruptures expected with cultural mixture. This article develops the concept of 'entangled migration states' as a means of highlighting the centrality of migration governance to state-building processes, the constitution of state sovereignty, and interstate relations. Drawing on the example of France and Algeria, it demonstrates the key role that migration played in three phases of state-building: the colonial period of Algérie française; the Algerian nationalist independence movement; and postcolonial state-building. In the case of France and Algeria, at least three actors constructed and instrumentalised migration and mobility regimes as sources of power and control – the French imperial state; the non-state Algerian independence movement; and the newly sovereign postcolonial Algerian state. A focus on the entangled nature of migration management allows for a deeper historicisation of contemporary migration regimes and draws attention to the central role played by migration diplomacy and transnational governmentality in contemporary migration management regimes, thus calling into question some of the spatial assumptions undergirding the 'migration state' concept. The article traces the evolution of mobility patterns and control in the Franco-Algerian case; their relationship to state-building processes; and the implications for rethinking the 'migration state'. At present, the forensic DNA workflow is not capable of providing information about the contributor status (single source vs. multiple contributors) of evidentiary samples prior to end-point analysis. This exacerbates the challenges inherent to mixtures and low-template DNA samples. If additional sample information could be provided earlier in the workflow, protocols could be implemented to mitigate these challenges. An integrated Quantiplex®- high resolution melt (HRM) assay was shown to be effective in distinguishing between single source and mixture DNA samples; however, integration of the HRM assay into a more commonly used chemistry would be beneficial to the practitioner community. Thus, the assay was redesigned as an integrated Quantifiler™ Trio-HRM assay, which included the identification of a new DNA-binding dye, an increased reaction volume, and the establishment of new data analysis and standard curve metrics for all targets. This redesigned assay produced quantification values and qualitative values that were comparable to those produced when the same samples were tested using the standard Quantifiler™ Trio chemistry and settings. Further, STR profiles generated with quantification values produced from the integrated Quantifiler™ Trio-HRM assay and standard Quantifiler™ Trio chemistry were complete and fully concordant. Most importantly, the integrated Quantifiler™ Trio-HRM assay was able to accurately predict whether a sample was single source or a mixture 79.2% of the time, demonstrating the potential of this approach. With the incorporation of an expanded training set for prediction modeling, and completion of critical developmental validation studies, this assay could prove useful to the forensic DNA practitioner community. This article is about Israel's West Bank settler-colonial project from the standpoint of settlers who are of Mizrahi origin (i.e. Jews of African or Middle Eastern descent). While historically predominantly Ashkenazi (i.e. Jews of European descent), with time many Mizrahim have moved to the West Bank and joined the settlement project. And yet, there is more or less scholarly silence regarding the existence of Mizrahi West Bank settlers, and when they are addressed, their motivations are reduced to simplified "economic" considerations; in a way that devoids them of political consciousness compared to their Ashkenazi counterparts. While there has been an immense body of work dealing with ethnic tension between settlers and the indigenous population, by centering the experience of a particular group of Mizrahi settlers, this article examines how internal ethnic tensions within the society of settlers play into and are transformed in the settler-colonial process. Cutaneous VCL::ALK fusion spindle (ovoid) cell tumor is unique. Recently emerged RAS::MAP tyrosine kinase fusion sarcomas more commonly involve subcutis, skeletal muscle and even bone. We share our experience with a novel cutaneous VCL::ALK spindle cell tumor. An 11‐year‐old male presented with a back pedunculated pink–red papule thought to be a pyogenic granuloma. Biopsy histopathology revealed an epithelial collarette with pedunculated tumor extending to deep dermis/subcutis interface. The combination of spindled and epithelioid cells, an ovoid myopericytoid appearance within myxoid to collagenous stroma, low to moderate MIB1 and focal S100 protein without SOX10 immunostaining, were suggestive of a novel RAS::MAPK tyrosine kinase fusion sarcoma that is well described. ALK immunostain being positive, a next‐generation sequencing comprehensive fusion panel was performed to reveal a VCL::ALK fusion. While epithelioid fibrous histiocytoma shares this fusion and similar dermal location and collarette pedunculation, this and other entities were excluded by older patient age, deeper dermal involvement, ovoid‐to‐spindled morphology, central pericytoid vasculature, myxoid stroma, moderate cellularity with low to moderate MIB1 expression, superficial ulceration, and focal S100 protein expression. Complete excision was performed with favorable follow‐up to date. This novel VCL::ALK fusion spindle (ovoid) cell tumor of the dermis is best considered as part of the recently emerged RAS::MAP tyrosine kinase fusion sarcomas. [ABSTRACT FROM AUTHOR]

Infinite time horizon spatially distributed optimal control problems may show so–called optimal diffusion induced instabilities, which may lead to patterned optimal steady states, although the problem itself is completely homogeneous. Here we show that this can be considered as a generic phenomenon, in problems with scalar distributed states, by computing optimal spatial patterns and their canonical paths in three examples: optimal feeding, optimal fishing, and optimal pollution. The (numerical) analysis uses the continuation and bifurcation package p d e 2 p a t h to first compute bifurcation diagrams of canonical steady states, and then time–dependent optimal controls to control the systems from some initial states to a target steady state as t → ∞. We consider two setups: The case of discrete patches in space, which allows to gain intuition and to compute domains of attraction of canonical steady states, and the spatially continuous (PDE) case. In the Guyanese Amazon, Waiwai memory is made through the substances and associated sentiments that accrue in the body. Remembering and forgetting are morally weighted actions that transform the dispositions of others, enabling desired feelings of contentment or anger and ill will. Waiwai perspectives on the stakes of being or not being in the thoughts of others matter for their relations with the state: Waiwai people say that, in the past, their households were split between Guyana and Brazil because the Guyanese government forgot about them, while in the present, Waiwai people work to make Guyanese officials remember them, and this constitutes a form of political practice. Whereas anthropological accounts of the politics of memory emphasize how the past is operationalized in the present, Waiwai discussions of being forgotten and being remembered demonstrate the significance of affective, rather than representational, forms of memory for the political practices of indigenous‐state relations. In this article, Turing instability and the formations of spatial patterns for a general two-component reaction-diffusion system defined on 2D bounded domain, are investigated. By analyzing characteristic equation at positive constant steady states and further selecting diffusion rate $ d $ and diffusion ratio $ \varepsilon $ as bifurcation parameters, sufficient and necessary conditions for the occurrence of Turing instability are established, which is called the first Turing bifurcation curve. Furthermore, parameter regions in which single-mode Turing patterns arise and multiple-mode (or superposition) Turing patterns coexist when bifurcations parameters are chosen, are described. Especially, the boundary of parameter region for the emergence of single-mode Turing patterns, consists of the first and the second Turing bifurcation curves which are given in explicit formulas. Finally, by taking diffusive Schnakenberg system as an example, parameter regions for the emergence of various kinds of spatially inhomogeneous patterns with different spatial frequencies and superposition Turing patterns, are estimated theoretically and shown numerically.
