3. A scoping review of the benefits and challenges of mHealth for clinical decision-making in neuro-physiotherapy.

- Abstract
OBJECTIVE: Mobile health (mHealth) technologies have emerged as promising tools in the field of neuro-physiotherapy, offering innovative solutions for enhancing clinical decision-making processes. This scoping review explores the existing literature on the use of mHealth applications in neuro-physiotherapy with a specific focus on their impact on clinical decision-making. MATERIALS AND METHODS: The PubMed, Google Scholar, Cochrane Library, ScienceDirect, and Scopus databases were comprehensively searched for both qualitative and quantitative peer-reviewed articles written in the English language and published till 2023 that focus on mHealth applications in neuro-physiotherapy and clinical decision-making. RESULTS: The key findings from the 14 included studies highlighted the diverse array of mHealth applications employed in neuro-physiotherapy, ranging from wearable sensors and mobile apps to virtual reality platforms. Synthesis of the evidence from these studies demonstrated the potential of these technologies in clinical decision-making and improving patient outcomes, patients' and therapists' perspectives of these applications, their clinical clues, and the challenges with their use. CONCLUSIONS: The findings from the review underscore the need for continued exploration of these technologies to optimize their effectiveness in rehabilitation settings and ultimately improve clinical decision-making and patient care in neuro-physiotherapy. Thrombocytopenia is a common problem in NICU. there are so many causes of thrombocytopenias, in this study we are evaluating clinical and etilogies of thrombocytopenias. The incidence of thrombocytopenia in this study was 43%.In our study, out of the 28 cases of early onset thrombocytopenia, the leading cause was culture proven sepsis seen in 43.9 % of cases followed by 31.8 % cases having birth asphyxia, 15.15% of cases were born to mothers having preeclampsia and 9 % of cases were IUGR neonates. Out of the 15 cases of late onset thrombocytopenia, 62.2% of cases were having c sepsis and 26.6 %of cases having meconium aspiration syndrome, 8% cases having birth asphyxia and 0% cases having Rh incompatibility. Objective. This study was aimed at evaluating the knowledge and behavior toward venous thromboembolism (VTE) prophylaxis among medical interns. Methods. This is a questionnaire-based cross-sectional observational cohort study of medical interns that used a validated questionnaire. The questionnaire comprised of items that assessed behavior, knowledge, and self-assessment of VTE risk factors, diagnosis, and prophylaxis. The study was conducted in Riyadh, Saudi Arabia, from October 2020 till September 2021. Results. The respondents were 246 medical interns. The overall rate of correct responses to behavior items was 41.82%. The overall rate of correct responses to knowledge items was 47.35%. A total of 61.8% responded negatively to the use of VTE risk assessment guidelines ( p < 0.0001 ). For the self-assessment of knowledge of VTE, more than 70% believed they did not have appropriate knowledge, were not prepared to establish the risk of VTE, and were not prepared to provide adequate prophylaxis for VTE ( p < 0.0001 ). A high proportion of medical interns (83.3%, p < 0.0001 ) believed they needed further training on this topic. Conclusion. Participants in this study showed poor knowledge and negative behavior regarding the assessment of risk factors, diagnosis, and prophylaxis of VTE. The majority of participants reported they needed training on this topic. These findings underscore the need for educational programs during undergraduate training and orientation of medical interns for VTE risk assessment, diagnosis, and prophylaxis at the beginning of their internship.

- Abstract
The rapid rise in global oil prices, the scarcity of petroleum sources, and environmental concerns have all created severe issues. As a result of the country's rapid expansion and financial affluence, Malaysia's energy consumption has skyrocketed. Biodiesel and solar power are currently two of the most popular alternatives to fossil fuels in Malaysia. These two types of renewable energy sources appear to be viable options because of their abundant availability together with environmental and performance competence to highly polluting and fast depleting fossil fuels. The purpose of adopting renewable technology is to expand the nation's accessibility to a reliable and secure power supply. The current review article investigates nonconventional energy sources added with nanosized metal particles called as nanomaterials including biodiesel and solar, as well as readily available renewable energy options. Concerning the nation's energy policy agenda, the sources of energy demand are also investigated. The article evaluates Malaysia's existing position in renewable energy industries, such as biodiesel and solar, as well as the impact of nanomaterials. This review article discusses biodiesel production, applications, and government policies in Malaysia, as well as biodiesel consumption and recent developments in the bioenergy sector, such as biodiesel property modifications utilizing nanoparticle additions. In addition, the current review study examines the scope of solar energy, different photovoltaic concentrators, types of solar energy harvesting systems, photovoltaic electricity potential in Malaysia, and the experimental setup of solar flat plate collectors (FPC) with nanotechnology.

Valence bond theory and electron transfer theory are both widely applied throughout Chemical Science, usually to different applications. Nevertheless, electron transfer theory is just an extensive development of one type of valence bond model. In this article, we demonstrate how electron-transfer theory can be more broadly applied to other valence-bond scenarios. This involves (1) rederivation for Morse oscillators of many key analytical expressions from electron-transfer transfer theory pertinent to harmonic oscillators, (2) renormalization of the key parameters from electron transfer theory to facilitate broad applications, and (3) development of 3 Morse-oscillator applications.

26. Variations of compound warm, dry, wet, and cold climate extremes in India during 1951 to 2014.

- Abstract
The simultaneous occurrence of climate extremes significantly impacts ecosystems, increasing the vulnerability to physical risks. Despite extensive research on hot extremes and droughts globally, there remains a significant gap in comprehending the occurrence, magnitude, spatial extent, and associated risks of compound extremes, encompassing scenarios like warm/dry, cold/dry, warm/wet, and cold/wet. This study investigates various compound extreme scenarios by examining combinations of maximum temperature (Tx) and the Standardized Precipitation Evapotranspiration Index (SPEI) using monthly data from 1951 to 2014 acquired from the India Meteorological Department (IMD) for Indian landmass. From the results, the spatial extent of warm/dry events has increased at 1.8 % per decade, while cold/wet events decreased by 1.1 % over India. The warm/wet events have shown an increased trend of about 0.3 %, and cold/dry events at modest rise of 0.7 % per decade. Furthermore, compound warm/dry and cold/wet extremes over India exhibit extreme frequency and shorter return periods, posing greater risk. Conversely, compound cold/dry and warm/wet extremes occur less often, indicating longer return periods and lower risk. Across much of the country, the frequency of warm/dry, cold/dry, warm/wet, and cold/wet extremes ranges from 30 to 45, 15-30, 20-30, and 25-45 months, respectively. Notably, warm/dry conditions exhibit increased frequency in the recent period (1983-2014) with 31 years compared to the base period (1951-1982) which had approximately 24 years for a spatial extent exceeding 5 %. 27. MoSe 2 -Layered Nanosheet Decorated SnO 2 Hollow Nanofiber-Based Highly Sensitive and Selective Room Temperature H 2 S Gas Sensor.

- Abstract
In this work, we successfully demonstrated a MoSe
2 @SnO2 nanocomposite-based room temperature H2 S gas sensor. A sensing mechanism was proposed based on experimental results and density functional theory calculations. The FESEM micrographs of the heterostructure formed by hydrothermally grown MoSe2 -layered nanosheets and SnO2 -hollow nanofiber result in a high surface area for H2 S gas adsorption. On exposure to calcination, the electro-spun PVP/SnO2 nanofiber undergoes the Kirkendall phenomenon, resulting in 94.6 nm thick hollow nanofibers. The combination of TMD@SMO shows an abundance of charge transfer, resulting in an excellent response toward H2 S gas. The MoSe2 @SnO2 detects a low concentration of 500 ppb with a relative response of ∼19.9% at room temperature (RT). The simulation, using density functional theory (DFT), discloses that the adsorption energies ranged from -0.3645 to -0.5193 eV, indicating reduced bond lengths and significant H2 S interactions. The sensor proves an excellent sensitivity toward H2 S gas, ranging from 100 ppm to 500 ppb, with a LoD of ∼15 ppb at RT. As the sensor worked at RT with accuracy and reliability, consistent performance was observed upon exposure to various humidity levels, making it suitable for exhaled breath gas sensors. The sensor, as developed, also exhibited a good selectivity toward H2 S gas in contrast to other gases as well as stability and longevity over time.- Published

28. Incidental Finding of a Persistent Left Superior Vena Cava During Permanent Dual-Chamber Pacemaker Implantation: A Case Report.

- Abstract
Persistent left superior vena cava (PLSVC) is a rare congenital venous anomaly. It is often asymptomatic and has atypical venous drainage that can complicate central venous catheterisation, pacemaker implantation, and cardiac surgeries. In most cases, the PLSVC drains into the right atrium via the coronary sinus, but in a minority of cases, it drains into the left atrium, leading to a right-to-left shunt, which can cause mild hypoxia or paradoxical embolism. Due to its abnormal anatomy, PLSVC can complicate lead placement during permanent pacemaker (PPM) insertion. Lead navigation becomes more complex, sometimes necessitating alternative lead placement techniques or imaging guidance to ensure proper functionality. In most cases, the PLSVC is identified incidentally during the initial venogram. We present the case of a 64-year-old male with sarcoidosis, hypercholesterolemia, hypertension, hepatitis, and recent atrial fibrillation (AF) who presented for elective direct current cardioversion (DCCV). Pre-DCCV, the ECG showed AF with a slow ventricular response, and following a 200-joule synchronised shock as per local protocol, sinus rhythm was restored. Post-DCCV ECG showed a first-degree AV block, which progressed to an intermittent 2:1 block, leading to a decision to implant a dual-chamber PPM. An echocardiogram revealed normal left ventricular function, a dilated left atrium and normal right ventricle, mild tricuspid regurgitation, and a possible patent foramen ovale (PFO). A venogram performed during PPM implantation revealed a PLSVC, which posed challenges in lead placement. Despite initial success, a post-procedure chest X-ray revealed displacement of the atrial lead, prompting a successful repositioning. The patient remained stable and asymptomatic; outpatient follow-ups showed satisfactory PPM function. PLSVC is a congenital anomaly arising from incomplete regression of the left anterior cardinal vein during embryonic development. Though it is often discovered incidentally, the anomaly becomes clinically significant during procedures such as pacemaker implantation due to its impact on venous anatomy and lead placement. This case also underscores the need for specialised techniques when managing patients with PLSVC during device implantation. Given the abnormal venous pathway, alternative strategies such as utilising the coronary sinus or imaging guidance, like fluoroscopy, may be necessary to ensure proper lead placement and avoid complications such as lead displacement or venous thrombosis. The literature supports using advanced imaging modalities and tailored surgical approaches to improve outcomes in patients with PLSVC. 29. A Gravitational-like Relationship of Dispersion Interactions is Exhibited by 40 Pairs of Molecules and Noble Gas Atoms.

- Abstract
We present computational results of many-body dispersion (MBD) interactions for 40 pairs of molecular and atomic species: hydrocarbons, silanes, corresponding fluorinated derivatives, pairs which have multiple H---H contacts between the molecules, as well as pairs having π-π interactions, and pairs of noble gases. The calculations reveal that the MBD stabilization energy ( E
DISP,MBD ) obeys a global relationship, which is gravitational-like . It is proportional to the product of the masses of the two molecules ( M1 M2 ) and inversely proportional to the corresponding distances between the molecular centers-of-mass ( RCOM-COM ) or the H---H distances of the atoms mediating the interactions of the two molecules ( RH-H ). This relationship reflects the interactions of instantaneous dipoles, which are formed by the ensemble of bonds/atoms in the interacting molecules. Using the D4-corrected dispersion energy ( EDISP,D4 ), which accounts for three-body interactions, we find that the EDISP,MBD and EDISP,D4 data sets are strongly correlated. Based on valence-bond modeling, the dispersion interactions occur primarily due to the increased contributions of the oscillating-ionic VB structures which maintain favorable electrostatic interactions; the [Sub─C+ :H-+ H:C- ─Sub] and [Sub─C:-+ H- H:C+ ─Sub] structures; Sub symbolizes general residues. This augmented contribution is complemented by simultaneously diminished-weights of the destabilizing pair of structures, [Sub─C+ :H-- H:C+ ─Sub] and [Sub─:C- H++ H:C- ─Sub]. The local charges are propagated to the entire ensemble of bonds/atoms by partially charging the Sub residues, thus bringing about the "gravitational-like" dependence of dispersion.- Published

30. Molecular Mechanisms of Neurofilament Alterations and its Application in Assessing Neurodegenerative Disorders.

- Abstract
Neurofilaments are intermediate filaments present in neurons. These provide structural support and maintain the size and shape of the neurons. Dysregulation, mutation, and aggregation of neurofilaments raise the levels of these proteins in the blood and cerebrospinal fluid, which are characteristic features of axonal damage and certain rare neurological diseases, such as Giant axonal neuropathy and Charcot-Mare-Tooth disease. Understanding the structure, dynamics, and function of neurofilaments has been greatly enhanced by a diverse range of biochemical and preclinical investigations conducted over more than four decades. Recently, there has been a resurgence of interest in post-translational modifications of neurofilaments, such as phosphorylation, aggregation, mutation, oxidation, etc. Over the past twenty years, several rare disorders have been studied from structural alterations of neurofilaments. These disorders are monitored by fluid biomarkers such as neurofilament light chains. Currently, there are many tools, such as Enzyme-Linked Immunosorbent Assay, Electrochemiluminescence Assay, Single-Molecule Array, Western/immunoblotting, etc., in use to assess the neurofilament proteins in Blood and CSF. However, all these techniques utilize expensive, non-specific, or antibody-based methods, which make them unsuitable for routine screening of neurodegenerative disorders. This provides room to search for newer sensitive, cost-effective, point-of-care tools for rapid screening of the disease. For a long time, the molecular mechanisms of neurofilaments have been poorly understood due to insufficient research attempts, and a deeper understanding of them remains elusive. Therefore, this review aims to highlight the available literature on molecular mechanisms of neurofilaments and the function of neurofilaments in axonal transport, axonal conduction, axonal growth, and neurofilament aggregation, respectively. 31. Impact of Karnofsky performance status on outcomes of patients with severe alcohol-associated hepatitis: a propensity-matched analysis.

- Abstract
Background and Aims: Severity scores, including the model for end-stage liver disease (MELD) and discriminant function score, guide the treatment of patients with severe alcohol-associated hepatitis (AH). We aimed to investigate the impact of functional status on outcomes of patients with AH., Methods: Medically managed patients (n = 133) with AH from 1 January 2019 to 31 December 2022 were included in this prospective study. The objectives were to compare the long-term survival, recompensation rates, corticosteroid response, incidence of infections, hepatic encephalopathy (HE) and acute kidney injury (AKI) among propensity score-matched patients with good Karnofsky performance status (KPS) (score ≥50) and poor KPS (score <50) using Kaplan-Meier analysis., Results: Twenty-five patients with good KPS were matched with 25 patients with poor KPS and followed up for a median duration of 10 (0.5-33) months. Survival was 76% (19/25; 95% confidence interval (CI), 54.9-90.6) in patients with good KPS compared to 42.3% (11/25; 95% CI, 23.4-63.1) patients with poor KPS (P = 0.001) at 10 months. The recompensation rate was higher in the good KPS group than in the poor KPS group (68% vs 44%; P = 0.04). A higher proportion of patients in the good KPS group (78.9%) than in the poor KPS group (42.8%; P = 0.03) responded to corticosteroids. Survival was lower among non-responders in the poor KPS group (0% vs 75%; P = 0.01). The proportion of patients who developed infection (36% vs 28%; P = 0.051), HE (36% vs 12%; P = 0.01) and AKI (60% vs 16%; P < 0.001) was higher in patients with poor KPS than in good KPS., Conclusions: KPS is an important determinant of outcomes in patients with AH, including survival, recompensation, response to corticosteroids and complications., (© 2024 Royal Australasian College of Physicians.)

Background: Wound healing pivots on a finely orchestrated inflammatory cascade, critical for tissue repair. Chronic wounds, compounded by persistent inflammation and susceptibility to infection, pose formidable clinical challenges. Nanofiber dressings offer promising avenues for wound care, yet their interaction with inflammation and infection remains elusive. We aim to delineate the inflammatory cascade preceding wound closure and assess Cu@Bbc nanofibers' therapeutic efficacy in mitigating inflammation and combating infection. Their unique attributes suggest promise in modulating inflammation, fostering tissue regeneration, and preventing microbial colonization. Investigating the intricate interplay between nanofiber scaffolds, inflammation, and infection may unveil mechanisms of enhanced wound healing. Our findings could stimulate the development of tailored dressings, urgently needed for effective wound management amidst immune dysregulation, infection, and inflammation., Methods: In this investigation, we synthesized Cu@Bbc nanofibers, incorporating curcumin and berberine chloride, for wound healing applications. We evaluated their individual and combined antibacterial, anti-biofilm, and antioxidant activities, alongside binding affinity with pro-inflammatory cytokines through molecular docking. Morphological characterization was conducted via SEM, FTIR assessed functional groups, and wettability contact angle measured hydrophobic properties. The physical properties, including tensile strength, swelling behavior, and thermal stability, were evaluated using tensile testing, saline immersion method and thermogravimetric analysis. Biodegradability of the nanofibers was assessed through a soil burial test. Biocompatibility was determined via MTT assay, while wound healing efficacy was assessed with in vitro scratch assays. Controlled drug release and antibacterial activity against MRSA were examined, with in vivo assessment in a zebrafish model elucidating inflammatory responses and tissue remodeling., Results: In this study, the synergistic action of curcumin and berberine chloride exhibited potent antibacterial efficacy against MRSA, with significant anti-mature biofilm disruption. Additionally, the combination demonstrated heightened antioxidant potential. Molecular docking studies revealed strong binding affinity with pro-inflammatory cytokines, suggesting a role in expediting the inflammatory response crucial for wound healing. Morphological analysis confirmed nanofiber quality, with drug presence verified via FTIR spectroscopy. Cu@Bbc demonstrated higher tensile strength, optimal swelling behavior, and robust thermal stability as evaluated through tensile testing and thermogravimetric analysis. Additionally, the Cu@Bbc nanofiber showed enhanced biodegradability, as confirmed by the soil burial test. Biocompatibility assessments showed favorable compatibility, while in vitro studies demonstrated potent antibacterial activity. In vivo zebrafish experiments revealed accelerated wound closure, re-epithelialization, and heightened immune response, indicative of enhanced wound healing., Conclusion: In summary, our investigation highlights the efficacy of Cu@Bbc nanofibers, laden with curcumin and berberine chloride, in displaying robust antibacterial and antioxidant attributes while also modulating immune responses and inflammatory cascades essential for wound healing. Despite advancements in detection and treatment, tuberculosis (TB), an infectious illness caused by the Mycobacterium TB bacteria, continues to pose a serious threat to world health. The TB diagnosis phase includes a patient's medical history, physical examination, chest X-rays, and laboratory procedures, such as molecular testing and sputum culture. In artificial intelligence (AI), machine learning (ML) is an advanced study of statistical algorithms that can learn from historical data and generalize the results to unseen data. There are not many studies done on the ML algorithm that enables the prediction of treatment success for patients with pulmonary TB (PTB). The objective of this study is to identify an effective and predictive ML algorithm to evaluate the detection of treatment success in PTB patients and to compare the predictive performance of the ML models. In this retrospective study, a total of 1236 PTB patients who were given treatment under a randomized controlled clinical trial at the ICMR-National Institute for Research in Tuberculosis, Chennai, India were considered for data analysis. The multiple ML models were developed and tested to identify the best algorithm to predict the sputum culture conversion of TB patients during the treatment period. In this study, decision tree (DT), random forest (RF), support vector machine (SVM) and naïve bayes (NB) models were validated with high performance by achieving an area under the curve (AUC) of receiver operating characteristic (ROC) greater than 80%. The salient finding of the study is that the DT model was produced as a better algorithm with the highest accuracy (92.72%), an AUC (0.909), precision (95.90%), recall (95.60%) and F1-score (95.75%) among the ML models. 34. An integrated method for detecting lung cancer via CT scanning via optimization, deep learning, and IoT data transmission.

With its increasing global prevalence, lung cancer remains a critical health concern. Despite the advancement of screening programs, patient selection and risk stratification pose significant challenges. This study addresses the pressing need for early detection through a novel diagnostic approach that leverages innovative image processing techniques. The urgency of early lung cancer detection is emphasized by its alarming growth worldwide. While computed tomography (CT) surpasses traditional X-ray methods, a comprehensive diagnosis requires a combination of imaging techniques. This research introduces an advanced diagnostic tool implemented through image processing methodologies. The methodology commences with histogram equalization, a crucial step in artifact removal from CT images sourced from a medical database. Accurate lung CT image segmentation, which is vital for cancer diagnosis, follows. The Otsu thresholding method and optimization, employing Colliding Bodies Optimization (CBO), enhance the precision of the segmentation process. A local binary pattern (LBP) is deployed for feature extraction, enabling the identification of nodule sizes and precise locations. The resulting image underwent classification using the densely connected CNN (DenseNet) deep learning algorithm, which effectively distinguished between benign and malignant tumors. The proposed CBO+DenseNet CNN exhibits remarkable performance improvements over traditional methods. Notable enhancements in accuracy (98.17%), specificity (97.32%), precision (97.46%), and recall (97.89%) are observed, as evidenced by the results from the fractional randomized voting model (FRVM). These findings highlight the potential of the proposed model as an advanced diagnostic tool. Its improved metrics promise heightened accuracy in tumor classification and localization. Target protein degradation (TPD) is an emerging approach to mitigate disease-causing proteins. TPD contains several strategies, and one of the strategies that gained immersive importance in recent times is Proteolysis Targeting Chimeras (PROTACs); the PROTACs recruit small molecules to induce the poly-ubiquitination of disease-causing protein by hijacking the ubiquitin-proteasome system (UPS) by bringing the E3 ligase and protein of interest (POI) into appropriate proximity. The steps involved in designing and evaluating the PROTACs remain critical in optimising the PROTACs to degrade the POI. It is observed that using in-silico and biochemical methods to study the ternary complexes (TCs) of the POI-PROTAC-E3 ligase is essential to understanding the structural activity, cooperativity, and stability of formed TCs. A better understanding of the above-mentioned leads to an appropriate rationale for designing the PROTACs targeting the disease-causing proteins. The recent global energy crisis scenario echoes the past, and the vulnerability of traditional fuels remains the proximate cause of environmental degradation. The fundamental changes in mobilising finance resources are predominant in an energy transition. So, acknowledging finance and energy transition via financial and environmental technology is necessary to meet sustainable development goals. Accordingly, this study employs panel econometric models to investigate how financial resources can ensure the production of renewable energy and environmental protection in 38 OECD member nations over a time period from 2000 to 2021. The empirical finding shows that foreign direct investment and financial technology ameliorate environmental performance by alleviating CO
2 emissions. Environmental technologies positively impact environmental protection by eliminating CO2 emissions. The financial resources ensure renewable energy production and environmental protection by reducing the environmental externalities and encouraging renewable energy consumption. Similarly, the consumption of renewable energy catalyses environmental improvement. Economic globalisation spurs environmental externalities. The D-H causality test also shows a reciprocal movement from environmental performance to financial technology, environmental technology and renewable energy consumption. The study's outcomes offer a new model of insight for governments as well as financial and energy policymakers to protect the environment. Ganoderma sp., the fungal agent causing basal stem rot (BSR), poses a severe threat to global oil palm production. Alarming increases in BSR occurrences within oil palm growing zones are attributed to varying effectiveness in its current management strategies. Asymptomatic progression of the disease and the continuous monoculture of oil palm pose challenges for prompt and effective management. Therefore, the development of precise, early, and timely detection techniques is crucial for successful BSR management. Conventional methods such as visual assessments, culture-based assays, and biochemical and physiological approaches prove time-consuming and lack specificity. Serological-based diagnostic methods, unsuitable for fungal diagnostics due to low sensitivity, assay affinity, cross-contamination which further underscores the need for improved techniques. Molecular PCR-based assays, utilizing universal, genus-specific, and species-specific primers, along with functional primers, can overcome the limitations of conventional and serological methods in fungal diagnostics. Recent advancements, including real-time PCR, biosensors, and isothermal amplification methods, facilitate accurate, specific, and sensitive Ganoderma detection. Comparative whole genomic analysis enables high-resolution discrimination of Ganoderma at the strain level. Additionally, omics tools such as transcriptomics, proteomics, and metabolomics can identify potential biomarkers for early detection of Ganoderma infection. Innovative on-field diagnostic techniques, including remote methods like volatile organic compounds profiling, tomography, hyperspectral and multispectral imaging, terrestrial laser scanning, and Red-Green-Blue cameras, contribute to a comprehensive diagnostic approach. This paper provides a new, less complex multilevel inverter topology that can be used for industrial loads and renewable energy sources. The arrangement consists of eight switches to produce an 11-level output voltage with two uneven input sources. Moreover, it can be connected in a cascade to increase the output voltage and number of levels even further. Due to its control mechanism, which is based on the Rotating Trapezoidal Sinusoidal Pulse Width Modulation control method for creating a high-quality output voltage, the inverter's key characteristic is its very low harmonic distortion of 3.45% at the output voltage when compared to the advanced triangular carrier-based PWM techniques of phase disposition sinusoidal pulse width modulation (PD-SPWM) it produces harmonic distortion of 11.38% at the output voltage, phase opposition disposition sinusoidal pulse width modulation (POD-SPWM) it produces harmonic distortion of 6.43% at the output voltage, alternative phase opposition disposition sinusoidal pulse width modulation (APOD-SPWM) it produces harmonic distortion of 6.69% at the output voltage, IN-Shift sinusoidal pulse width modulation (IN-shift SPWM) it produces harmonic distortion of 3.93% at the output voltage. This inverter is commonly used in wind turbines and solar cells. Breast cancer is a major challenge in the field of oncology, with around 2.3 million cases and around 670,000 deaths globally based on the GLOBOCAN 2022 data. Despite having advanced technologies, breast cancer remains the major type of cancer among women. This review highlights various collagen signatures and the role of different collagen types in breast tumor development, progression, and metastasis, along with the use of photoacoustic spectroscopy to offer insights into future cancer diagnostic applications without the need for surgery or other invasive techniques. Through mapping of the tumor microenvironment and spotlighting key components and their absorption wavelengths, we emphasize the need for extensive preclinical and clinical investigations.

- Abstract
The discovery of effective multitarget-directed ligands (MTDLs) against multifactorial Alzheimer's disease (AD) remnants has been focused in an incessant drug discovery pursuit. In this perception, the current study explores the rational design, synthesis, and evaluation of 26 quinazolinone-hydrazine cyanoacetamide hybrids 7(a-j) , 8(a-j) , and 9(a-f) as MTDLs against AD. These new compounds were synthesized in four-step processes using simple phthalimide as the starting material without any major workup procedures and were characterized by different spectroscopic techniques. In Ellman's assay, the most potent analogues 7i , 8j , and 9d were identified as selective and mixed-type inhibitors of hAChE. Furthermore, biophysical and computational assessments revealed that the analogues 7i , 8j , and 9d were bound to both the catalytic active site and peripheral anionic site of hAChE with high affinity. The molecular dynamics simulation analysis highlighted the conformational changes of hAChE upon binding of 7i , 8j , and 9d and also the stability of resulting biomolecular systems all over 100 ns simulations. In addition to antioxidant activity, the most active congeners were found to protect substantially SK-N-SH cells from oxidative damage. Decisively, the most active analogues 7i , 8j , and 9d were assessed as potent Aβ
1-42 fibril modulators and protective agents against Aβ1-42 -induced toxicity in SH-SY5Y cells. Additionally, glioblastoma C6 cell-based assays also demonstrated the use of the most active congeners 7i , 8j , and 9d as protective agents against Aβ1-42 -induced toxicity. Overall, this multifunctional capacity of quinazolinone-hydrazine cyanoacetamide hybrids demonstrated the noteworthy potential of these hybrids to develop as effectual MTDLs against AD. However, further pharmacokinetics, toxicology, and behavioral studies are warranted.- Published

- Abstract
At present, the availability of nonrenewable sources and their usage for electric vehicle technology is reducing gradually because of their disadvantages are more environmental pollution, direct effect on human health, less reliability, and taking more time to start functioning. So, in this article, the proton exchange membrane fuel cell (PEMFC) is considered for the automotive application because of its advantages quick startup, more power density, more safety to handle, high efficiency, and capability of operating at very low operational temperature conditions. However, the drawback of PEMFC is very difficult to identify the accurate MPP position of the fuel system. Here, the improved variable step genetic algorithm is added with the adaptive neuro-fuzzy inference system for tracking the operational point of the proposed system with high efficiency. These hybrid MPPT controller features are easy to understand, more accurate, have a better dynamic response, and have low design complexity. The evaluated proposed MPPT controller operational efficiency, and settling time of the converter voltage at different fuel stack temperature conditions are 98.7402%, and 0.01607 s respectively. Finally, the boost converter is used in this work to enhance the voltage supply capability of the entire system. The proposed system is investigated by applying the MATLAB tool., (© 2024. - Abstract
Posttransplant cyclophosphamide (PtCy) has been shown to decrease post-hematopoietic stem cell transplant acute and chronic graft-versus-host disease (GVHD). In this study, PtCy was used in 44 patients along with mycophenolate and tacrolimus with HLA matched (29) and mismatched (15) unrelated donors to determine the impact of graft content on outcome; thus, all patients had flow cytometric analysis of their graft content including the number of B cells, NK cells, and various T cell subsets. Higher γδ T cell dose was associated with the development of acute GVHD (p = .0038). For PtCy, further studies of the cell product along with further graft manipulation, such as selective γδ T cell depletion, could potentially improve outcomes., (© 2024 The Authors. Epithelioid inflammatory myofibroblastic sarcoma (EIMS), a variant of inflammatory myofibroblastic tumor (IMT), is a rare malignant tumor commonly associated with anaplastic lymphoma kinase (ALK) gene fusions and is aggressive in nature with local recurrence. Here, we report a case of a 23-year-old female who presented with a cough and, upon investigations, was found to have a mass in the left upper lobe of the lung detected by chest computed tomography (CT). Biopsy revealed EIMS with ALK and desmin protein expression. The patient underwent a lobectomy via video-assisted thoracoscopic surgery (VATS). This study aims to analyze the heat transfer behavior of the magnetohydrodynamic blood-based Casson hybrid nanofluid in the occurrence of a non-Fourier heat flux model and linear thermal radiation over a horizontal porous stretching cylinder with potential applications in biomedical engineering. The present investigation utilised titanium dioxide and silver nanoparticles, which exhibit considerable potential in the realm of cancer therapy. Thus, there is a growing interest among biomedical engineers and clinicians in the study of entropy production as a means of quantifying energy dissipation in biological systems. Suitable self-similarity variables are employed to transform the nonlinear mathematical equations such as velocity, temperature, skin friction coefficient, and heat transfer rate, which are computed via homotopy perturbation method (HPM). HPM computations have been executed to solve the influences of various parameters such as porosity parameter ( K = 0.0 , 1.0 , 2.0 ) , Curvature parameter ( α = 0.0 , 1.0 , 3.0 , 5.0 ) , Casson parameter ( β = 0.0 , 0.5 , 1.5 ) , inertia coefficient ( Fr = 0.5 , 1.5 , 2.5 ) , thermal relaxation parameter ( δ ∗ = 0.0 , 0.5 , 1.0 ) , radiation ( Rd = 0.0 , 0.5 , 1.0 ) , Eckert number ( Ec = 0.0 , 0.1 , 0.2 ) , Brinkman number ( Br = 0.5 , 1.0 , 1.5 ) and temperature difference parameter ( α 1 = 0.0 , 0.5 , 1.0 ) . The comparison using the homotopy perturbation technique produces a more accurate and reliable consequence than the numerical method (Runge-Kutta method). The higher values of the Casson and Curvature parameters decrease the velocity profile. The temperature profile of M = 1 and M = 0 increases with improving values of the thermal relaxation parameter. Entropy generation rises to enhance Brinkman number values, whereas Bejan number exhibits the reverse influence. Improving the value of the heat source parameter declines the Nusselt number.

45. Neuropsychiatric Systemic Lupus Erythematosus (NPSLE): A Case Report and an Overview of the Diagnosis, Treatment Modalities, and Prognosis.

- Abstract
Systemic lupus erythematosus (SLE) is a chronic inflammatory, multisystem autoimmune disease with a broad spectrum of clinical presentations. Neuropsychiatric systemic lupus erythematosus (NPSLE) refers to neurological and psychiatric symptoms involving the central and peripheral nervous systems. A 23-year-old African American female with a history of undifferentiated connective tissue disease on hydroxychloroquine and poor medication adherence presented to the emergency department with an altered mental status and generalized headache. In addition, she had a fever, associated tachycardia (104 BPM), and hypotension (90/63 mmHg). She was given fluids and started on broad-spectrum antibiotics and antivirals, suspecting bacterial or viral meningitis. However, a broad infectious workup, including cerebral spinal fluid (CSF) culture, was unrevealing. Given the lack of improvement of antibiotics, an immunological workup for SLE was initiated, which showed low CH50, C3, and C4; anti-nucleic acid antibody (ANA) was 1:1280, anti-double-stranded (anti-DS) DNA antibody not detected, and fluorescent ANA was positive. For severe NPSLE, rituximab is the most commonly utilized immunosuppressant; it was not utilized in this case due to the patient's insurance. The patient was placed on methylprednisolone and cyclophosphamide (CYC) infusion per ACR guidelines. Due to the toxic effects of CYC on the gonads, we offered ovarian preservation; however, the patient opted to refuse. The patient's mental status started to improve after three days of pulse steroids. The patient was advised to follow up with rheumatology for CYC therapy and a gradual taper of her steroids. NPSLE is a diagnosis of exclusion primarily based on expert opinion due to the absence of a gold standard diagnostic procedure. Disease-specific therapy, symptomatic therapy, nonpharmacological approaches, and correction of aggravating variables are all used to treat individuals with NPSLE. This paper aims to contribute to the existing literature on NPSLE, with the intention to educate and strive for early detection and treatment. We hereby present an interesting case of SLE in a 23-year-old female who would not have responded to one treatment. 46. Experimental and simulation study of American saffron seed oil blended with diesel.

- Abstract
In a variety of industries, including transportation, agriculture, and manufacturing, diesel engines are often employed. Due of rising prices and environmental concerns, researchers examined whether biodiesels might replace diesel. The current study looks into American Saffron Oil's feasibility as a feedstock for biodiesel production. The transesterification technique is used to extract American saffron oil methyl este(ASOME), which is then examined for its physical and chemical properties in accordance with ASTM standards. Diesel fuel and American Saffron Oil methyl ester are mixed on a volume basis to create a variety of fuel blends, including B20, B40, and B60. The test results showed that the 20 % ASOME mix had better performance and reduced emissions. Also, utilizing DIESEL-RK simulation software, diesel engine tests are conducted for the B20, B40, and B60 under identical operating circumstances. Moreover, diesel engine testing for the B20, B40, and B60 are carried out using DIESEL-RK simulation software under comparable operating circumstances. Results of simulation software tests show improved engine performance and reduced pollutants. When experimental data is compared to DIESEL-RK modeling software, it is found that brake thermal efficiency increased by 5.7 % and emissions of hydrocarbon and carbon monoxide decreased by 2.5 % and 14.3 %, respectively., Competing Interests: The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (© 2024 The Authors. - Abstract
Nonenzymatic glycation (NEG) unfolds and crosslinks proteins, resulting in aggregation. Label-free evaluation of such structural changes, without disturbing molecular integrity, would be beneficial for understanding the fundamental mechanisms of protein aggregation. The current study demonstrates the assessment of NEG-induced protein aggregation by combining autofluorescence (AF) spectroscopy and imaging. The methylglyoxal (MG) induced protein unfolding and the formation of cross-linking advanced glycation end-products (AGEs) leading to aggregation were evaluated using deep-UV-induced-autofluorescence (dUV-AF) spectroscopy in proteins with distinct structural characteristics. Since the AGEs formed on proteins are fluorescent, the study demonstrated the possibility of autofluorescence imaging of NEG-induced protein aggregates. Autofluorescence spectroscopy can potentially reveal molecular alterations such as protein unfolding and cross-linking. In contrast, AGE-based autofluorescence imaging offers a means to visually explore the structural arrangement of aggregates, regardless of whether they are amyloid or non-amyloid in nature.

- Abstract
The influence of boundary layer flow of heat transfer analysis on hybrid nanofluid across an extended cylinder is the main focus of the current research. In addition, the impressions of magnetohydrodynamic, porous medium and thermal radiation are part of this investigation. Arrogate similarity variables are employed to transform the governing modelled partial differential equations into a couple of highly nonlinear ordinary differential equations. A numerical approach based on the BVP Midrich scheme in MAPLE solver is employed for solution of the set of resulting ordinary differential equations and obtained results are compared with existing literature. The effect of active important physical parameters like Magnetic Field, Porosity parameter, Eckert number, Prandtl number and thermal radiation parameters on dimensionless velocity and energy fields are employed via graphs and tables. The velocity profile decreased by about 65% when the magnetic field parameter values increases from 0.5 to 1.5. On the other hand increased by 70% on energy profile. The energy profile enhanced by about 62% when the Radiation parameter values increases from 1.0 < Rd < 3.0. The current model may be applicable in real life practical implications of employing Engine oil-SWCNTs-MWCNTs-TiO
2 nanofluids on cylinders encompass enhanced heat transfer efficiency, and extended component lifespan, energy savings, and environmental benefits. Background: Tracheal necrosis post endotracheal intubation is a rare life-threatening disease that can compromise airway patency. We demonstrated a novel usage of upper airway ultrasonography (USG) to diagnose tracheal necrosis., Case Presentation: A middle-aged smoking male presented with productive cough, noisy breathing and exertional dyspnea for 2 weeks. He was intubated one month prior due to a traumatic brain injury. Upper airway USG findings showed irregular air-mucosal interface (AMI) and comet tail artefacts over the 1st and 2nd tracheal ring. A direct laryngoscopy in the operating room showed thick mucopus inferior to the vocal cords, with necrotic tracheal cartilages and debris obstructing the airway. He was successfully treated with parenteral antibiotics, wound debridement and tracheostomy., Conclusion: Our case highlights the first documented USG findings of tracheal necrosis. Upper airway USG serves as a potential diagnostic modality in managing the condition., (© 2024. The Author(s).)

- Abstract
Renal cell carcinoma is a highly vascular tumor associated with vascular endothelial growth factor (VEGF) expression. The Vascular Endothelial Growth Factor -2 (VEGF-2) and its receptor was identified as a potential anti-cancer target, and it plays a crucial role in physiology as well as pathology. Inhibition of angiogenesis via blocking the signaling pathway is considered an attractive target. In the present study, 150 FDA-approved drugs have been screened using the concept of drug repurposing against VEGFR-2 by employing the molecular docking, molecular dynamics, grouping data with Machine Learning algorithms, and density functional theory (DFT) approaches. The identified compounds such as Pazopanib, Atogepant, Drosperinone, Revefenacin and Zanubrutinib shown the binding energy - 7.0 to - 9.5 kcal/mol against VEGF receptor in the molecular docking studies and have been observed as stable in the molecular dynamic simulations performed for the period of 500 ns. The MM/GBSA analysis shows that the value ranging from - 44.816 to - 82.582 kcal/mol. Harnessing the machine learning approaches revealed that clustering with K = 10 exhibits the relevance through high binding energy and satisfactory logP values, setting them apart from compounds in distinct clusters. Therefore, the identified compounds are found to be potential to inhibit the VEGFR-2 and the present study will be a benchmark to validate the compounds experimentally., (© 2024.
