Abstract: An analysis of solar neutrino data from the fourth phase of Super-Kamiokande~(SK-IV) from October 2008 to May 2018 is performed and the results are presented. The observation time of the data set of SK-IV corresponds to $2970$~days and the total live time for all four phases is $5805$~days. For more precise solar neutrino measurements, several improvements are applied in this analysis: lowering the data acquisition threshold in May 2015, further reduction of the spallation background using neutron clustering events, precise energy reconstruction considering the time variation of the PMT gain. The observed number of solar neutrino events in $3.49$--$19.49$ MeV electron kinetic energy region during SK-IV is $65,443^{+390}_{-388}\,(\mathrm{stat.})\pm 925\,(\mathrm{syst.})$ events. Corresponding $\mathrm{^{8}B}$ solar neutrino flux is $(2.314 \pm 0.014\, \rm{(stat.)} \pm 0.040 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$, assuming a pure electron-neutrino flavor component without neutrino oscillations. The flux combined with all SK phases up to SK-IV is $(2.336 \pm 0.011\, \rm{(stat.)} \pm 0.043 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$. Based on the neutrino oscillation analysis from all solar experiments, including the SK $5805$~days data set, the best-fit neutrino oscillation parameters are $\rm{sin^{2} \theta_{12,\,solar}} = 0.306 \pm 0.013 $ and $\Delta m^{2}_{21,\,\mathrm{solar}} = (6.10^{+ 0.95}_{-0.81}) \times 10^{-5}~\rm{eV}^{2}$, with a deviation of about 1.5$\sigma$ from the $\Delta m^{2}_{21}$ parameter obtained by KamLAND. The best-fit neutrino oscillation parameters obtained from all solar experiments and KamLAND are $\sin^{2} \theta_{12,\,\mathrm{global}} = 0.307 \pm 0.012 $ and $\Delta m^{2}_{21,\,\mathrm{global}} = (7.50^{+ 0.19}_{-0.18}) \times 10^{-5}~\rm{eV}^{2}$., Comment: 47 pages, 61 figures

Abstract: Neutrinos associated with solar flares (solar-flare neutrinos) provide information on particle acceleration mechanisms during the impulsive phase of solar flares. We searched using the Super-Kamiokande detector for neutrinos from solar flares that occurred during solar cycles $23$ and $24$, including the largest solar flare (X28.0) on November 4th, 2003. In order to minimize the background rate we searched for neutrino interactions within narrow time windows coincident with $\gamma$-rays and soft X-rays recorded by satellites. In addition, we performed the first attempt to search for solar-flare neutrinos from solar flares on the invisible side of the Sun by using the emission time of coronal mass ejections (CMEs). By selecting twenty powerful solar flares above X5.0 on the visible side and eight CMEs whose emission speed exceeds $2000$ $\mathrm{km \, s^{-1}}$ on the invisible side from 1996 to 2018, we found two (six) neutrino events coincident with solar flares occurring on the visible (invisible) side of the Sun, with a typical background rate of $0.10$ ($0.62$) events per flare in the MeV-GeV energy range. No significant solar-flare neutrino signal above the estimated background rate was observed. As a result we set the following upper limit on neutrino fluence at the Earth $\mathit{\Phi}<1.1\times10^{6}$ $\mathrm{cm^{-2}}$ at the $90\%$ confidence level for the largest solar flare. The resulting fluence limits allow us to constrain some of the theoretical models for solar-flare neutrino emission., Comment: 36 pages, 18 figures, 9 tables (Figure 12 was replaced because it was incorrect in version 1.)

Abstract: We present the development of neutron-tagging techniques in Super-Kamiokande IV using a neural network analysis. The detection efficiency of neutron capture on hydrogen is estimated to be 26%, with a mis-tag rate of 0.016 per neutrino event. The uncertainty of the tagging efficiency is estimated to be 9.0%. Measurement of the tagging efficiency with data from an Americium-Beryllium calibration agrees with this value within 10%. The tagging procedure was performed on 3,244.4 days of SK-IV atmospheric neutrino data, identifying 18,091 neutrons in 26,473 neutrino events. The fitted neutron capture lifetime was measured as 218 \pm 9 \mu s.

Abstract: Super-Kamiokande has been searching for neutrino bursts characteristic of core-collapse supernovae continuously, in real time, since the start of operations in 1996. The present work focuses on detecting more distant supernovae whose event rate may be too small to trigger in real time, but may be identified using an offline approach. The analysis of data collected from 2008 to 2018 found no evidence of distant supernovae bursts. This establishes an upper limit of 0.29 year$^{-1}$ on the rate of core-collapse supernovae out to 100 kpc at 90% C.L.. For supernovae that fail to explode and collapse directly to black holes the limit reaches to 300 kpc.

Abstract: In 2020, the Super-Kamiokande (SK) experiment moved to a new stage (SK-Gd) in which gadolinium (Gd) sulfate octahydrate was added to the water in the detector, enhancing the efficiency to detect thermal neutrons and consequently improving the sensitivity to low energy electron anti-neutrinos from inverse beta decay (IBD) interactions. SK-Gd has the potential to provide early alerts of incipient core-collapse supernovae through detection of electron anti-neutrinos from thermal and nuclear processes responsible for the cooling of massive stars before the gravitational collapse of their cores. These pre-supernova neutrinos emitted during the silicon burning phase can exceed the energy threshold for IBD reactions. We present the sensitivity of SK-Gd to pre-supernova stars and the techniques used for the development of a pre-supernova alarm based on the detection of these neutrinos in SK, as well as prospects for future SK-Gd phases with higher concentrations of Gd. Abstract: Non-Standard Interactions (NSI) between neutrinos and matter affect the neutrino flavor oscillations. Due to the high matter density in the core of the Sun, solar neutrinos are suited to probe these interactions. Using the $277$ kton-yr exposure of Super-Kamiokande to $^{8}$B solar neutrinos, we search for the presence of NSI. Our data favors the presence of NSI with down quarks at 1.8$\sigma$, and with up quarks at 1.6$\sigma$, with the best fit NSI parameters being ($\epsilon_{11}^{d},\epsilon_{12}^{d}$) = (-3.3, -3.1) for $d$-quarks and ($\epsilon_{11}^{u},\epsilon_{12}^{u}$) = (-2.5, -3.1) for $u$-quarks. Abstract: Radioactivity induced by cosmic muon spallation is a dominant source of backgrounds for $\mathcal{O}(10)~$MeV neutrino interactions in water Cherenkov detectors. In particular, it is crucial to reduce backgrounds to measure the solar neutrino spectrum and find neutrino interactions from distant supernovae. In this paper we introduce new techniques to locate muon-induced hadronic showers and efficiently reject spallation backgrounds. Applying these techniques to the solar neutrino analysis with an exposure of $2790\times22.5$~kton.day increases the signal efficiency by $12.6\%$, approximately corresponding to an additional year of detector running. Furthermore, we present the first spallation simulation at SK, where we model hadronic interactions using FLUKA. The agreement between the isotope yields and shower pattern in this simulation and in the data gives confidence in the accuracy of this simulation, and thus opens the door to use it to optimize muon spallation removal in new data with gadolinium-enhanced neutron capture detection.

Abstract: A new search for the diffuse supernova neutrino background (DSNB) flux has been conducted at Super-Kamiokande (SK), with a $22.5\times2970$-kton$\cdot$day exposure from its fourth operational phase IV. The new analysis improves on the existing background reduction techniques and systematic uncertainties and takes advantage of an improved neutron tagging algorithm to lower the energy threshold compared to the previous phases of SK. This allows for setting the world's most stringent upper limit on the extraterrestrial $\bar{\nu}_e$ flux, for neutrino energies below 31.3 MeV. The SK-IV results are combined with the ones from the first three phases of SK to perform a joint analysis using $22.5\times5823$ kton$\cdot$days of data. This analysis has the world's best sensitivity to the DSNB $\bar{\nu}_e$ flux, comparable to the predictions from various models. For neutrino energies larger than 17.3 MeV, the new combined $90\%$ C.L. upper limits on the DSNB $\bar{\nu}_e$ flux lie around $2.7$ cm$^{-2}$$\cdot$$\text{sec}^{-1}$, strongly disfavoring the most optimistic predictions. Abstract: In order to improve Super-Kamiokande's neutron detection efficiency and to thereby increase its sensitivity to the diffuse supernova neutrino background flux, 13 tons of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ (gadolinium sulfate octahydrate) was dissolved into the detector's otherwise ultrapure water from July 14 to August 17, 2020, marking the start of the SK-Gd phase of operations. During the loading, water was continuously recirculated at a rate of 60 m$^3$/h, extracting water from the top of the detector and mixing it with concentrated $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ solution to create a 0.02% solution of the Gd compound before injecting it into the bottom of the detector. A clear boundary between the Gd-loaded and pure water was maintained through the loading, enabling monitoring of the loading itself and the spatial uniformity of the Gd concentration over the 35 days it took to reach the top of the detector. During the subsequent commissioning the recirculation rate was increased to 120 m$^3$/h, resulting in a constant and uniform distribution of Gd throughout the detector and water transparency equivalent to that of previous pure-water operation periods. Using an Am-Be neutron calibration source the mean neutron capture time was measured to be $115\pm1$ $\mu$s, which corresponds to a Gd concentration of $111\pm2$ ppm, as expected for this level of Gd loading. Abstract: In this study, resistivity measurements are made during continuous heating and cooling on four different Ni-based superalloys of different grain structures and with different phases (i.e., γ′ and carbide). The results are directly compared with differential scanning calorimetry (DSC) profiles to identify the material’s resistivity response. The resistivity measurements have been performed using an electro-thermal mechanical testing (ETMT) system having a capability of heating and cooling a sample at a rate of up to 100 K/s by Joule heating, which is not possible with standard heating methods used in previous in-situ microstructure analysis approaches. By comparing different precipitate variations and thermal histories, γ′ volume fraction and precipitate number density are found to be the most important factors determining the resistivity of the materials. In-situ resistivity measurement was applied to several case studies to show that it can provide microstructural information in complex high temperature experiments.

Abstract: In 2020, the Super-Kamiokande (SK) experiment moved to a new stage (SK-Gd) in which gadolinium (Gd) sulfate octahydrate was added to the water in the detector, enhancing the efficiency to detect thermal neutrons and consequently improving the sensitivity to low energy electron anti-neutrinos from inverse beta decay (IBD) interactions. SK-Gd has the potential to provide early alerts of incipient core-collapse supernovae through detection of electron anti-neutrinos from thermal and nuclear processes responsible for the cooling of massive stars before the gravitational collapse of their cores. These pre-supernova neutrinos emitted during the silicon burning phase can exceed the energy threshold for IBD reactions. We present the sensitivity of SK-Gd to pre-supernova stars and the techniques used for the development of a pre-supernova alarm based on the detection of these neutrinos in SK, as well as prospects for future SK-Gd phases with higher concentrations of Gd. For the current SK-Gd phase, high-confidence alerts for Betelgeuse could be issued up to nine hours in advance of the core-collapse itself., Comment: 20 pages

Abstract: Due to a very low production rate of electron anti-neutrinos ($\bar{\nu}_e$) via nuclear fusion in the Sun, a flux of solar $\bar{\nu}_e$ is unexpected. An appearance of $\bar{\nu}_e$ in solar neutrino flux opens a new window for the new physics beyond the standard model. In particular, a spin-flavor precession process is expected to convert an electron neutrino into an electron anti-neutrino (${\nu_e\to\bar{\nu}_e}$) when neutrino has a finite magnetic moment. In this work, we have searched for solar $\bar{\nu}_e$ in the Super-Kamiokande experiment, using neutron tagging to identify their inverse beta decay signature. We identified 78 $\bar{\nu}_e$ candidates for neutrino energies of 9.3 to 17.3 MeV in 2970.1 live days with a fiducial volume of 22.5 kiloton water (183.0 kton$\cdot$year exposure). The energy spectrum has been consistent with background predictions and we thus derived a 90% confidence level upper limit of ${4.7\times10^{-4}}$ on the $\nu_e\to\bar{\nu}_e$ conversion probability in the Sun. Abstract: Scintillators are key components in high-energy x-ray detectors for x-ray computed tomography, which are applied in medical diagnosis, nondestructive testing, and security inspection. Scintillators in x-ray imaging are required to have both high spatial resolution and sensitivity; however, a trade-off between spatial resolution and sensitivity exists. To improve the sensitivity of a scintillator, its thickness must be increased; however, spatial resolution deteriorates with increasing thickness. Here, we developed a well-aligned Tb-doped GdAlO3-αAl2O3 phase-separated scintillator fiber (PSSF) with a diameter of 1.25-μm by a micro-pulling-down method. The luminescence, scintillation, and imaging performances of a grown PSSF sample were evaluated. X-ray-induced radioluminescence measurements of the sample showed Tb3+ 4f–4f emissions in the wavelength range of 470–700 nm, and the maximum emission peak was at 550 nm. The contrast transfer function at 10 lp/mm was 82% for both 270 and 500 μm-thick PSSF samples. We calculated the luminescence index, which is the product of the scintillator's light yield and sensitivity, of the samples and compared their values with commercially available conventional CsI:Tl columnar scintillators. The Tb-doped GdAlO3-αAl2O3 PSSF exhibits higher contrast transfer function and luminescence index values at above 60 keV x-ray region compared with the tested conventional scintillators. Abstract: Super-Kamiokande has been searching for neutrino bursts characteristic of core-collapse supernovae continuously, in real time, since the start of operations in 1996. The present work focuses on detecting more distant supernovae whose event rate may be too small to trigger in real time, but may be identified using an offline approach. The analysis of data collected from 2008 to 2018 found no evidence of distant supernovae bursts. This establishes an upper limit of 0.29 yr−1 on the rate of core-collapse supernovae out to 100 kpc at 90% C.L. For supernovae that fail to explode and collapse directly to black holes the limit reaches to 300 kpc.

Abstract: In order to improve Super-Kamiokande's neutron detection efficiency and to thereby increase its sensitivity to the diffuse supernova neutrino background flux, 13 tons of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ (gadolinium sulfate octahydrate) was dissolved into the detector's otherwise ultrapure water from July 14 to August 17, 2020, marking the start of the SK-Gd phase of operations. During the loading, water was continuously recirculated at a rate of 60 m$^3$/h, extracting water from the top of the detector and mixing it with concentrated $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ solution to create a 0.02% solution of the Gd compound before injecting it into the bottom of the detector. A clear boundary between the Gd-loaded and pure water was maintained through the loading, enabling monitoring of the loading itself and the spatial uniformity of the Gd concentration over the 35 days it took to reach the top of the detector. During the subsequent commissioning the recirculation rate was increased to 120 m$^3$/h, resulting in a constant and uniform distribution of Gd throughout the detector and water transparency equivalent to that of previous pure-water operation periods. Using an Am-Be neutron calibration source the mean neutron capture time was measured to be $115\pm1$ $\mu$s, which corresponds to a Gd concentration of $111\pm2$ ppm, as expected for this level of Gd loading. Abstract: We present the development of neutron-tagging techniques in Super-Kamiokande IV using a neural network analysis. The detection efficiency of neutron capture on hydrogen is estimated to be 26%, with a mis-tag rate of 0.016 per neutrino event. The uncertainty of the tagging efficiency is estimated to be 9.0%. Measurement of the tagging efficiency with data from an Americium-Beryllium calibration agrees with this value within 10%. The tagging procedure was performed on 3,244.4 days of SK-IV atmospheric neutrino data, identifying 18,091 neutrons in 26,473 neutrino events. Abstract: The Super-Kamiokande detector can be used to search for neutrinos in time coincidence with gravitational waves detected by the LIGO-Virgo Collaboration (LVC). Both low-energy ($7-100$ MeV) and high-energy ($0.1-10^5$ GeV) samples were analyzed in order to cover a very wide neutrino spectrum. Follow-ups of 36 (out of 39) gravitational waves reported in the GWTC-2 catalog were examined; no significant excess above the background was observed, with 10 (24) observed neutrinos compared with 4.8 (25.0) expected events in the high-energy (low-energy) samples. A statistical approach was used to compute the significance of potential coincidences. For each observation, p-values were estimated using neutrino direction and LVC sky map ; the most significant event (GW190602_175927) is associated with a post-trial p-value of $7.8\%$ ($1.4\sigma$). Additionally, flux limits were computed independently for each sample and by combining the samples. Abstract: Digital pathology has enabled the noninvasive quantification of pathological parameters. In addition, the combination of digital pathology and artificial intelligence has enabled the analysis of a vast amount of information, leading to the sharing of much information and the elimination of knowledge gaps. Fibrosis, which reflects chronic inflammation, is the most important pathological parameter in chronic liver diseases, such as viral hepatitis and metabolic dysfunction-associated steatotic liver disease. It has been reported that the quantitative evaluation of various fibrotic parameters by digital pathology can predict the prognosis of liver disease and hepatocarcinogenesis. Liver fibrosis evaluation methods include 1 fiber quantification, 2 elastin and collagen quantification, 3 s harmonic generation/two photon excitation fluorescence (SHG/TPE) microscopy, and 4 Fibronest™. . 　In this review, we provide an overview of role of digital pathology on the evaluation of fibrosis in liver disease and the characteristics of recent methods to assess liver fibrosis., (© 2024. 40. Application of on-line sample preconcentration by large-volume dual preconcentration by isotachophoresis and stacking (LDIS) on straight-channel microchips.

Abstract: In this study, large-volume dual preconcentration by isotachophoresis and stacking (LDIS) which is an on-line sample preconcentration technique coupling large-volume sample stacking with an electroosmotic flow pump (LVSEP) with transient isotachophoresis (tITP) was applied to microchip electrophoresis (MCE) for improving both detection sensitivities and peak shapes. To realize LDIS in MCE, we investigated experimental procedures for injecting a short plug of a leading electrolyte (LE) solution into a straight microchannel without any sophisticated injector apparatus. We found that a short LE plug could be injected into a sample-filled straight-channel only by making the liquid level of the LE solution in an outlet reservoir higher than that in an inlet one. By applying a reversed-polarity voltage to the microchip, anionic analytes injected throughout the microchannel were first enriched by LVSEP, followed by tITP. Through the second preconcentration effect by tITP in LDIS, sensitivity enhancement factor (SEF) and asymmetry factor for a standard dye were improved from 878 and 0.62 to 1330 and 1.14, respectively, relative to those in conventional LVSEP. It should be noted that more viscous running buffer containing sieving polymers could be employed to the LDIS analysis, which was effective for improving the SEF and the separation efficiencies, especially for bio-polymeric compounds. Finally, LDIS was applied to the oligosaccharide and protein analyses in MCE, resulting in the SEFs of 1410 and ca. 50 for maltotriose and bovine milk casein, respectively., (© 2024. 41. Transcranial motor-evoked potentials disappear with pons transection.

Abstract: Motor-evoked potential (MEP) monitoring by transcranial electrical stimulation (TES) is important for intraoperative motor function assessment in neurosurgery; however, false-negative results sometimes occur, and these findings should be interpreted with caution. Herein, we report an interesting MEP change resulting from a pons transection. The patient was a boy aged 5 years and 2 months. He underwent multiple craniotomies for cerebellar anaplastic ependymoma and was already paralyzed in the right upper and lower limbs. Therefore, we decided to remove the recurrent lesion from the left anterior pons. MEPs were recorded on both the right and left sides after the start of surgery but disappeared 1 h 30 min after the start of surgery in the TES on the operative side, even when the stimulation intensity was increased. The contralateral TES consistently recorded stable MEPs throughout the surgery. The tumor was completely resected on imaging. Immediately postoperatively, the patient experienced flaccid paralysis on the right side of the body, which recovered to preoperative levels over time. A transcranial MEP cannot be derived if the corticospinal tract is transected at the pons. Transcranial MEP findings may accurately reflect the corticospinal tract function if the injury is caudal to the pons., (© 2024. 42. Is orthotic treatment beneficial for fresh osteoporotic vertebral fractures? A propensity score matching study.

Abstract: Background Context: Orthotic treatment is a common option for the conservative treatment of osteoporotic vertebral fractures (OVF). However, there is insufficient evidence of its clinical benefit., Purpose: To investigate the effectiveness of orthotic treatment for OVF., Study Design/setting: Retrospective cohort study with data from two prospective studies., Patient Sample: This study included 160 patients with fresh OVF enrolled in 2012 and 2020 prospective cohort studies., Outcome Measures: The visual analog scale (VAS) score for low back pain was used for clinical outcomes, and radiographic parameters included the percent height of the vertebra and angular change of the vertebral body. Moreover, the occurrence of secondary vertebral fractures was followed-up over time., Methods: The patients were divided into brace and no-brace groups and were matched according to propensity score for age, sex, anterior percent height at the initial examination, and presence of old OVFs. Hazard ratio for the cumulative incidence of secondary vertebral fractures with and without bracing were calculated and analyzed using the generalized Wilcoxon test. In addition, the brace group was divided into soft and rigid brace groups and compared with the no-brace group., Results: Each group had 61 cases after propensity score matching. There were no significant differences in the VAS improvement for low back pain and the change in percent height of the anterior and posterior walls from initial examination to 6 months after injury (p=.87, p=.39 and p=.14, respectively, mixed-effect models). Meanwhile, the mean angular change of fractured vertebrae was 4.3° / 3.2° initially and 1.2° / 2.5° at 6 months (the brace group / no-brace group, respectively; p=.007, mixed-effect models). A significant difference was also observed between the rigid brace group and the no-brace group (p=.008, mixed effect models). The incidence of secondary vertebral fractures was 1.6% / 11.4% at 1 month, indicating a significant difference (the brace group / no-brace group, respectively; p = .028). The hazard ratio for the cumulative incidence of secondary fractures due to orthotic treatment was 0.47 (95% confidence interval 0.20-1.09, p=.054)., Conclusions: Although orthotic treatment for fresh OVF did not relieve pain, it might contribute to the stabilization of the fractured vertebra, especially using a rigid brace. 44. Double-chambered left ventricle as a novel cause of aneurysm formation at the apex of accessory chamber.

Abstract: This case report shows unique aneurysm at the left ventricular apex associated with a double-chambered left ventricle (DCLV) in asymptomatic 69-year-old female. The aneurysm was located at the apex of an accessory chamber, which was formed by hypertrophied muscle band and the interventricular septum, along with a pronounced jet flow directed towards the apex aneurysm. These finding suggests that the obstruction at the base of the accessory chamber caused a pressure overload at the apex, ultimately leading to the development of the aneurysm. Abstract: Background/aim: Neskeep ® , an absorbable polyglycolic acid spacer, has been developed as the optimal material for spacer placement surgery. However, preventing its severe adhesion is a crucial concern. Therefore, we aimed to identify an effective anti-adhesion agent for Neskeep ® using rat models., Materials and Methods: Animal experiments were performed using 60 rats, which underwent Neskeep ® placement on the abdominal wall. Three types of anti-adhesion agents were employed, establishing four subgroups: Seprafilm ® , INTERCEED ® , AdSpray ® , and only Neskeep ® (control) groups. Rats were sacrificed on postoperative days 7, 14, and 28 to assess adhesion levels around the Neskeep ® Macroscopic visual assessment with the Lauder score and histopathological evaluation were performed to assess the degree of adhesion., Results: There were no significant differences in the proportion of Lauder scores on days 7 and 14 between the four groups. Histological evaluation revealed no significant differences between groups at any observation time. However, the mean Lauder scores at day 28 were 5.0, 1.6, 4.0, and 4.8 in the Neskeep ® , Seprafilm ® , INTERCEED ® , and AdSpray ® groups, respectively. Abstract: In panic disorder (PD), functional disturbance of the hypothalamus-pituitary-adrenal (HPA) axis has been considered. However, in neuroimaging studies of PD, the hypothalamus and pituitary gland are poorly studied.We investigated the volume of PD patients' hypothalamus and pituitary gland, enrolling 38 PD patients and 38 healthy controls. Severity of PD was mild to moderate according to the Panic Disorder Severity Scale, and the illness duration was relatively short (median = 2.8 years). The hypothalamus' gray matter was automatically extracted and segmented, whereas the pituitary gland was manually traced. Regarding the hypothalamus, the paraventricular nucleus (PVH), which produces the corticotropin-releasing hormone, was of interest.The volumes of the pituitary and the bilateral anterior-superior hypothalamic subunits, where the PVH would be located, were compared by the multiple regression analyses controlling for age and intracranial content volume. To compensate for limitation in the abovementioned segmentation and analyses, the voxel-based morphometry with small volume correction (VBM-SVC) targeting the whole hypothalamus was also performed.The multiple regression analyses did not find significant effect of PD diagnosis on the volumes. However, in the VBM-SVC analysis, volume reduction of the PVH was suggested in PD even when patients who experienced PD for ≥ 3 years were excluded [peak coordinate (x, y, z = -2, 3, -8), FWE-corrected P = .022 (cluster-level) and 0.003 (peak-level), voxel size = 63]. Our results suggested structural alteration of the PVH in PD patients for the first time, indicating importance of the HPA-axis in PD pathology., (© 2024. Abstract: Aim: Several studies have shown the efficacy and safety of low-molecular-weight heparin use in coronavirus disease 2019 (COVID-19), but that of unfractionated heparin (UFH) has not been investigated. We investigated the prevalence of bleeding complications during UFH administration, its impact on mortality, and the risk factors of bleeding outcomes associated with UFH., Methods: This retrospective cohort study was conducted at a single-center tertiary care hospital, including hospitalized patients with COVID-19. The primary outcomes were measured as the prevalence of bleeding complications during hospitalization, and the secondary outcomes were thromboembolic events and 60-day mortality rates. Logistic regression analysis and propensity score matching were used to assess risk factors for bleeding complications and their impact on mortality., Results: Among 1035 included patients, 516 patients were treated with UFH. Twelve (2.3%) patients in the UFH group experienced major bleeding. The prevalence of major bleeding in patients treated with therapeutic-dose UFH was 9.2%. Logistic regression analysis showed that age ≥ 60 years (adjusted odds ratio [aOR], 3.89; 95% confidence interval [CI], 1.01-15.0; P＜.05) and COVID-19 severity (aOR, 35.9; 95% CI, 4.57-282; P＜.05) were associated with major bleeding complications. After propensity score matching, 11 major and 11 non-major bleeding cases (including minor bleeding) were matched. The 60-day cumulative mortality rate between the two groups did not differ significantly (P=.13, log-rank test)., Conclusions: The incidence of major bleeding in COVID-19 patients using therapeutic-dose UFH was relatively high. Critical COVID-19 and older age were risk factors for bleeding complications.

49. Relationship between cement penetration and incidence of a radiolucent line around the tibia 2 years after total knee arthroplasty: A retrospective study.

Abstract: Objectives: Cement penetration (CP) plays a key role in implant stability for cemented total knee arthroplasty (TKA), and the radiolucent line (RLL) >2 mm is a preliminary sign for loosening of components. However, the direct relationship between CP and the frequency of RLL >2 mm is unclear, and the best cut-off value for CP to prevent RLL >2 mm also remains unclear. This study aimed to investigate this relationship between CP and RLL and to determine the clinical cut-off value for tibial CP in patients 2 years after TKA., Methods: This retrospective study investigated 157 knees from 123 patients with osteoarthritis who underwent cemented TKA. The CP and RLL immediately after TKA and the RLL 2 years after TKA were measured for the medial, lateral, anterior, and posterior tibial baseplate zones. Receiver operating characteristic (ROC) curves were constructed to determine the best cut-off values for CP., Results: RLL >2 mm was not observed just after TKA. An RLL >2 mm was observed in any tibial baseplate zone in 22 knees from 20 patients (RLL+ group) and was not observed in the remaining (RLL- group) 2 years after TKA. The mean CP for all zones was significantly higher in the RLL- group (2.5 ± 1.1 mm) than in the RLL+ group (1.7 ± 0.6 mm; P < 0.001). An RLL >2 mm was seen in 21 knees in the medial zone, 9 knees in the lateral zone, 8 knees in the anterior zone, and 3 knees in the posterior zone. CP values with RLL >2 mm were significantly lower than those without the RLL at the medial, anterior, and posterior tibial baseplate zones. The best cut-off values from the ROC curve of CP in each zone were between 1.1 mm and 2.1 mm., Conclusions: The depth of the CP directly affects the incidence of an RLL >2 mm. Abstract: Self-amplifying RNAs (saRNAs) are versatile vaccine platforms that take advantage of a viral RNA-dependent RNA polymerase (RdRp) to amplify the messenger RNA (mRNA) of an antigen of interest encoded within the backbone of the viral genome once inside the target cell. In recent years, more saRNA vaccines have been clinically tested with the hope of reducing the vaccination dose compared to the conventional mRNA approach. The use of N1-methyl-pseudouridine (1mΨ), which enhances RNA stability and reduces the innate immune response triggered by RNAs, is among the improvements included in the current mRNA vaccines. In the present study, we evaluated the effects of this modified nucleoside on various saRNA platforms based on different viruses. The results showed that different stages of the replication process were affected depending on the backbone virus. For TNCL, an insect virus of the Alphanodavirus genus, replication was impaired by poor recognition of viral RNA by RdRp. In contrast, the translation step was severely abrogated in coxsackievirus B3 (CVB3), a member of the Picornaviridae family. Finally, the effects of 1mΨ on Semliki forest virus (SFV), were not detrimental in in vitro studies, but no advantages were observed when immunogenicity was tested in vivo., (© 2024. The Author(s).)
