14. Developing Systems of Care for Stroke in Resource-limited Settings.

Abstract

Although stroke prevention and treatment strategies have significantly advanced in recent years, implementation of these care elements in resource-limited settings can be challenging, since the burden of stroke is higher and access to stroke care is lower. Barriers to stroke care in resource-limited settings include insufficient prevention, reduced awareness of stroke symptoms, limited prehospital care and lack of triage systems, limited access to comprehensive stroke centers, inadequate personnel education, lack of staff and resources, as well as limited access to neuroimaging, thrombolytics, mechanical thrombectomy, neurosurgical care, and rehabilitation. Here, we suggest strategies to improve stroke care in these settings, including public health campaigns, protocols for prehospital notification, organized flow to specialized stroke centers, development of dedicated stroke units, and utilization of telemedicine and telerehabilitation. We also highlight the role of international organizations and governments in reducing the global burden of stroke. [ABSTRACT FROM AUTHOR]

16. To extract or not to extract? Influence of chemical extraction treatment of wood samples on element concentrations in tree-rings measured by X-ray fluorescence

Abstract

In micro-densitometry of wood it is standard procedure to extract resin and other soluble compounds before X-ray analysis to eliminate the influence of these extractives on wood-density. Dendrochemical studies using X-ray fluorescence analysis on the other hand are commonly conducted without previous extraction. However, it is well known that translocation processes of elements during heartwood formation in trees or (temporal) differences in sap content of wood samples can influence dendrochemical element profiles. This might bias environmental signals stored in time series of element concentrations in wood proxies. We hypothesize that metals tightly bound to cell walls show a more robust proxy potential for environmental conditions than easily translocated ones. To eliminate the noise of these soluble substances in wood elemental time series, their extraction prior to analysis might be necessary. In our study we tested the effect of different solvents (water, alcohol, and acetone) and different extraction times on elemental time series of three tree species with differing wood structure (Pinus sylvestris; Quercus robur and Populus tremula). Micro-XRF analysis was conducted on nine replicates per species using an ITRAX-Multiscanner. A set of elements commonly detected in wood (S, Cl, K, Ca, Ti, Mn, Fe, and Ni) was analysed at high resolution before and after several extraction runs. Besides lowering their levels, extraction did not significantly change the temporal trends for most elements. However, for some elements, e.g., Potassium, Chlorine or Manganese, especially the water extraction led to significant decreases in concentrations and altered temporal trends. Apparently the dipole effect of water produced the strongest extraction power of all three solvents. 23. Exploring Perceived Gender Disparities in Latin America's Vascular Neurology Workforce: Insights from a Survey-Based Study.

Abstract

Introduction: Limited research exists on women's challenges as specialized healthcare professionals in Latin America's stroke field. This survey-based study addresses the potential gender disparities in these professionals' work environments., Methods: This exploratory study used an online survey to investigate the work environment of women healthcare professionals in stroke across several Latin American countries. Conducted between September and November 2023, it included demographics, relationship status, reproductive history, and gender roles related to healthcare work and/or academic life. Women responders were invited through professional networks and local stroke care organizations. Descriptive analyses were performed, and sub-group comparisons were made using statistical tests such as Chi-square, FisherExact, or Kruskal-Wallis., Results: A total of 291 responses were gathered from 16 Latin American countries. The average age was 40.01±9.61 years, 34% reported holding leadership positions, with 49.5% having women as supervisors. Furthermore, 41% were married, and 52.9% reported having children. Among those, 29.2% perceived adverse effects of childcare on their academic trajectories, with 71.43% being unable to participate in academic conferences. Only 16.1% held leadership roles in scientific organizations, although 52% were involved in educational endeavors within university settings., Conclusions: Our survey reveals perceived significant hurdles women healthcare professionals encounter in stroke, notably concerning the influence of maternity on job performance and career development. Furthermore, these results highlight inequalities in leadership roles and career pathways. By shedding light on these obstacles, we aim to increase awareness and advocate for implementing fair policies to create a supportive work environment., (S. Karger AG, Basel.)
