Surgical phase recognition is critical for assisting surgeons in understanding surgical videos. Existing studies focused more on online surgical phase recognition, by leveraging preceding frames to predict the current frame. Despite great progress, they formulated the task as a series of frame-wise classification, which resulted in a lack of global context of the entire procedure and incoherent predictions. Moreover, besides online analysis, accurate offline surgical phase recognition is also in significant clinical need for retrospective analysis, and existing online algorithms do not fully analyze the entire video, thereby limiting accuracy in offline analysis. To overcome these challenges and enhance both online and offline inference capabilities, we propose a universal Surgical Phase Localization Network, named SurgPLAN++, with the principle of temporal detection. To ensure a global understanding of the surgical procedure, we devise a phase localization strategy for SurgPLAN++ to predict phase segments across the entire video through phase proposals. For online analysis, to generate high-quality phase proposals, SurgPLAN++ incorporates a data augmentation strategy to extend the streaming video into a pseudo-complete video through mirroring, center-duplication, and down-sampling. For offline analysis, SurgPLAN++ capitalizes on its global phase prediction framework to continuously refine preceding predictions during each online inference step, thereby significantly improving the accuracy of phase recognition. We perform extensive experiments to validate the effectiveness, and our SurgPLAN++ achieves remarkable performance in both online and offline modes, which outperforms state-of-the-art methods. Accurate classification of port wine stains (PWS, vascular malformations present at birth), is critical for subsequent treatment planning. However, the current method of classifying PWS based on the external skin appearance rarely reflects the underlying angiopathological heterogeneity of PWS lesions, resulting in inconsistent outcomes with the common vascular-targeted photodynamic therapy (V-PDT) treatments. Conversely, optical coherence tomography angiography (OCTA) is an ideal tool for visualizing the vascular malformations of PWS. Previous studies have shown no significant correlation between OCTA quantitative metrics and the PWS subtypes determined by the current classification approach. This study proposes a new classification approach for PWS using both OCT and OCTA. By examining the hypodermic histopathology and vascular structure of PWS, we have devised a fine-grained classification method that subdivides PWS into five distinct types. To assess the angiopathological differences of various PWS subtypes, we have analyzed six metrics related to vascular morphology and depth information of PWS lesions. The five PWS types present significant differences across all metrics compared to the conventional subtypes. Our findings suggest that an angiopathology-based classification accurately reflects the heterogeneity in PWS lesions. On-chip micro-ring resonators (MRRs) have been proposed for constructing delay reservoir computing (RC) systems, offering a highly scalable, high-density computational architecture that is easy to manufacture. However, most proposed RC schemes have utilized passive integrated optical components based on silicon-on-insulator (SOI), and RC systems based on lithium niobate on insulator (LNOI) have not yet been reported. The nonlinear optical effects exhibited by lithium niobate microphotonic devices introduce new possibilities for RC design. In this work, we design an RC scheme based on a series-coupled MRR array, leveraging the unique interplay between thermo-optic nonlinearity and photorefractive effects in lithium niobate. We first demonstrate the existence of three regions defined by wavelength detuning between the primary LNOI micro-ring resonator and the coupled micro-ring array, where one region achieves an optimal balance between nonlinearity and high memory capacity at extremely low input energy, leading to superior computational performance. We then discuss in detail the impact of each ring's nonlinearity and the system's symbol duration on performance. Medical visual question answering (VQA) bridges the gap between visual information and clinical decision-making, enabling doctors to extract understanding from clinical images and videos. In particular, surgical VQA can enhance the interpretation of surgical data, aiding in accurate diagnoses, effective education, and clinical interventions. However, the inability of VQA models to visually indicate the regions of interest corresponding to the given questions results in incomplete comprehension of the surgical scene. To tackle this, we propose the surgical visual question localized-answering (VQLA) for precise and context-aware responses to specific queries regarding surgical images. Furthermore, to address the strong demand for safety in surgical scenarios and potential corruptions in image acquisition and transmission, we propose a novel approach called Calibrated Co-Attention Gated Vision-Language (C$^2$G-ViL) embedding to integrate and align multimodal information effectively. Additionally, we leverage the adversarial sample-based contrastive learning strategy to boost our performance and robustness. We also extend our EndoVis-18-VQLA and EndoVis-17-VQLA datasets to broaden the scope and application of our data. Extensive experiments on the aforementioned datasets demonstrate the remarkable performance and robustness of our solution. Our solution can effectively combat real-world image corruption. The recent Segment Anything Model (SAM) 2 has demonstrated remarkable foundational competence in semantic segmentation, with its memory mechanism and mask decoder further addressing challenges in video tracking and object occlusion, thereby achieving superior results in interactive segmentation for both images and videos. Building upon our previous empirical studies, we further explore the zero-shot segmentation performance of SAM 2 in robot-assisted surgery based on prompts, alongside its robustness against real-world corruption. For static images, we employ two forms of prompts: 1-point and bounding box, while for video sequences, the 1-point prompt is applied to the initial frame. Through extensive experimentation on the MICCAI EndoVis 2017 and EndoVis 2018 benchmarks, SAM 2, when utilizing bounding box prompts, outperforms state-of-the-art (SOTA) methods in comparative evaluations. The results with point prompts also exhibit a substantial enhancement over SAM's capabilities, nearing or even surpassing existing unprompted SOTA methodologies. Besides, SAM 2 demonstrates improved inference speed and less performance degradation against various image corruption. As a crucial and intricate task in robotic minimally invasive surgery, reconstructing surgical scenes using stereo or monocular endoscopic video holds immense potential for clinical applications. NeRF-based techniques have recently garnered attention for the ability to reconstruct scenes implicitly. On the other hand, Gaussian splatting-based 3D-GS represents scenes explicitly using 3D Gaussians and projects them onto a 2D plane as a replacement for the complex volume rendering in NeRF. However, these methods face challenges regarding surgical scene reconstruction, such as slow inference, dynamic scenes, and surgical tool occlusion. This work explores and reviews state-of-the-art (SOTA) approaches, discussing their innovations and implementation principles. Furthermore, we replicate the models and conduct testing and evaluation on two datasets. Continuum robots can be miniaturized to just a few millimeters in diameter. Among these, notched tubular continuum robots (NTCR) show great potential in many delicate applications. Existing works in robotic modeling focus on kinematics and dynamics but still face challenges in reproducing the robot's morphology -- a significant factor that can expand the research landscape of continuum robots, especially for those with asymmetric continuum structures. This paper proposes a dual stereo vision-based method for the three-dimensional morphological reconstruction of millimeter-scale NTCRs. The method employs two oppositely located stationary binocular cameras to capture the point cloud of the NTCR, then utilizes predefined geometry as a reference for the KD tree method to relocate the capture point clouds, resulting in a morphologically correct NTCR despite the low-quality raw point cloud collection. The method has been proved feasible for an NTCR with a 3.5 mm diameter, capturing 14 out of 16 notch features, with the measurements generally centered around the standard of 1.5 mm, demonstrating the capability of revealing morphological details. The recent advance in neural rendering has enabled the ability to reconstruct high-quality 4D scenes using neural networks. Although 4D neural reconstruction is popular, registration for such representations remains a challenging task, especially for dynamic scene registration in surgical planning and simulation. In this paper, we propose a novel strategy for dynamic surgical neural scene registration. We first utilize 4D Gaussian Splatting to represent the surgical scene and capture both static and dynamic scenes effectively. Then, a spatial aware feature aggregation method, Spatially Weight Cluttering (SWC) is proposed to accurately align the feature between surgical scenes, enabling precise and realistic surgical simulations. Lastly, we present a novel strategy of deformable scene registration to register two dynamic scenes. By incorporating both spatial and temporal information for correspondence matching, our approach achieves superior performance compared to existing registration methods for implicit neural representation. The proposed method has the potential to improve surgical planning and training, ultimately leading to better patient outcomes.

Surgical instrument segmentation is crucial in surgical scene understanding, thereby facilitating surgical safety. Existing algorithms directly detected all instruments of pre-defined categories in the input image, lacking the capability to segment specific instruments according to the surgeon's intention. During different stages of surgery, surgeons exhibit varying preferences and focus toward different surgical instruments. Therefore, an instrument segmentation algorithm that adheres to the surgeon's intention can minimize distractions from irrelevant instruments and assist surgeons to a great extent. The recent Segment Anything Model (SAM) reveals the capability to segment objects following prompts, but the manual annotations for prompts are impractical during the surgery. To address these limitations in operating rooms, we propose an audio-driven surgical instrument segmentation framework, named ASI-Seg, to accurately segment the required surgical instruments by parsing the audio commands of surgeons. Specifically, we propose an intention-oriented multimodal fusion to interpret the segmentation intention from audio commands and retrieve relevant instrument details to facilitate segmentation. Moreover, to guide our ASI-Seg segment of the required surgical instruments, we devise a contrastive learning prompt encoder to effectively distinguish the required instruments from the irrelevant ones. Therefore, our ASI-Seg promotes the workflow in the operating rooms, thereby providing targeted support and reducing the cognitive load on surgeons. Extensive experiments are performed to validate the ASI-Seg framework, which reveals remarkable advantages over classical state-of-the-art and medical SAMs in both semantic segmentation and intention-oriented segmentation. Sampling-based kinodynamic motion planners (SKMPs) are powerful in finding collision-free trajectories for high-dimensional systems under differential constraints. Time-informed set (TIS) can provide the heuristic search domain to accelerate their convergence to the time-optimal solution. However, existing TIS approximation methods suffer from the curse of dimensionality, computational burden, and limited system applicable scope, e.g., linear and polynomial nonlinear systems. To overcome these problems, we propose a method by leveraging deep learning technology, Koopman operator theory, and random set theory. Specifically, we propose a Deep Invertible Koopman operator with control U model named DIKU to predict states forward and backward over a long horizon by modifying the auxiliary network with an invertible neural network. A sampling-based approach, ASKU, performing reachability analysis for the DIKU is developed to approximate the TIS of nonlinear control systems online. Furthermore, we design an online time-informed SKMP using a direct sampling technique to draw uniform random samples in the TIS. Simulation experiment results demonstrate that our method outperforms other existing works, approximating TIS in near real-time and achieving superior planning performance in several time-optimal kinodynamic motion planning problems.

Wireless Capsule Endoscopy (WCE) is highly valued for its non-invasive and painless approach, though its effectiveness is compromised by uneven illumination from hardware constraints and complex internal dynamics, leading to overexposed or underexposed images. While researchers have discussed the challenges of low-light enhancement in WCE, the issue of correcting for different exposure levels remains underexplored. To tackle this, we introduce EndoUIC, a WCE unified illumination correction solution using an end-to-end promptable diffusion transformer (DiT) model. In our work, the illumination prompt module shall navigate the model to adapt to different exposure levels and perform targeted image enhancement, in which the Adaptive Prompt Integration (API) and Global Prompt Scanner (GPS) modules shall further boost the concurrent representation learning between the prompt parameters and features. Besides, the U-shaped restoration DiT model shall capture the long-range dependencies and contextual information for unified illumination restoration. Moreover, we present a novel Capsule-endoscopy Exposure Correction (CEC) dataset, including ground-truth and corrupted image pairs annotated by expert photographers. Extensive experiments against a variety of state-of-the-art (SOTA) methods on four datasets showcase the effectiveness of our proposed method and components in WCE illumination restoration, and the additional downstream experiments further demonstrate its utility for clinical diagnosis and surgical assistance., Comment: To appear in MICCAI 2024. As the significance of simulation in medical care and intervention continues to grow, it is anticipated that a simplified and low-cost platform can be set up to execute personalized diagnoses and treatments. 3D Slicer can not only perform medical image analysis and visualization but can also provide surgical navigation and surgical planning functions. In this paper, we have chosen 3D Slicer as our base platform and monocular cameras are used as sensors. Then, We used the neural radiance fields (NeRF) algorithm to complete the 3D model reconstruction of the human head. We compared the accuracy of the NeRF algorithm in generating 3D human head scenes and utilized the MarchingCube algorithm to generate corresponding 3D mesh models. The individual's head pose, obtained through single-camera vision, is transmitted in real-time to the scene created within 3D Slicer. The demonstrations presented in this paper include real-time synchronization of transformations between the human head model in the 3D Slicer scene and the detected head posture. Additionally, we tested a scene where a tool, marked with an ArUco Maker tracked by a single camera, synchronously points to the real-time transformation of the head posture. Foundation models have become prominent in computer vision, achieving notable success in various tasks. However, their effectiveness largely depends on pre-training with extensive datasets. Applying foundation models directly to small datasets of capsule endoscopy images from scratch is challenging. Pre-training on broad, general vision datasets is crucial for successfully fine-tuning our model for specific tasks. In this work, we introduce a simplified approach called Adapt foundation models with a low-rank adaptation (LoRA) technique for easier customization. Our method, inspired by the DINOv2 foundation model, applies low-rank adaptation learning to tailor foundation models for capsule endoscopy diagnosis effectively. Unlike traditional fine-tuning methods, our strategy includes LoRA layers designed to absorb specific surgical domain knowledge. During the training process, we keep the main model (the backbone encoder) fixed and focus on optimizing the LoRA layers and the disease classification component. We tested our method on two publicly available datasets for capsule endoscopy disease classification. The results were impressive, with our model achieving 97.75% accuracy on the Kvasir-Capsule dataset and 98.81% on the Kvasirv2 dataset. Advances in endoscopy use in surgeries face challenges like inadequate lighting. Deep learning, notably the Denoising Diffusion Probabilistic Model (DDPM), holds promise for low-light image enhancement in the medical field. However, DDPMs are computationally demanding and slow, limiting their practical medical applications. To bridge this gap, we propose a lightweight DDPM, dubbed LighTDiff. It adopts a T-shape model architecture to capture global structural information using low-resolution images and gradually recover the details in subsequent denoising steps. We further prone the model to significantly reduce the model size while retaining performance. While discarding certain downsampling operations to save parameters leads to instability and low efficiency in convergence during the training, we introduce a Temporal Light Unit (TLU), a plug-and-play module, for more stable training and better performance. TLU associates time steps with denoised image features, establishing temporal dependencies of the denoising steps and improving denoising outcomes. Moreover, while recovering images using the diffusion model, potential spectral shifts were noted. We further introduce a Chroma Balancer (CB) to mitigate this issue. Our LighTDiff outperforms many competitive LLIE methods with exceptional computational efficiency.

Depth estimation plays a crucial role in various tasks within endoscopic surgery, including navigation, surface reconstruction, and augmented reality visualization. Despite the significant achievements of foundation models in vision tasks, including depth estimation, their direct application to the medical domain often results in suboptimal performance. This highlights the need for efficient adaptation methods to adapt these models to endoscopic depth estimation. We propose Endoscopic Depth Any Camera (EndoDAC) which is an efficient self-supervised depth estimation framework that adapts foundation models to endoscopic scenes. Specifically, we develop the Dynamic Vector-Based Low-Rank Adaptation (DV-LoRA) and employ Convolutional Neck blocks to tailor the foundational model to the surgical domain, utilizing remarkably few trainable parameters. Given that camera information is not always accessible, we also introduce a self-supervised adaptation strategy that estimates camera intrinsics using the pose encoder. Our framework is capable of being trained solely on monocular surgical videos from any camera, ensuring minimal training costs. Experiments demonstrate that our approach obtains superior performance even with fewer training epochs and unaware of the ground truth camera intrinsics. Capsule endoscopes, predominantly serving diagnostic functions, provide lucid internal imagery but are devoid of surgical or therapeutic capabilities. Consequently, despite lesion detection, physicians frequently resort to traditional endoscopic or open surgical procedures for treatment, resulting in more complex, potentially risky interventions. To surmount these limitations, this study introduces a chained flexible capsule endoscope (FCE) design concept, specifically conceived to navigate the inherent volume constraints of capsule endoscopes whilst augmenting their therapeutic functionalities. The FCE's distinctive flexibility originates from a conventional rotating joint design and the incision pattern in the flexible material. In vitro experiments validated the passive navigation ability of the FCE in rugged intestinal tracts. Further, the FCE demonstrates consistent reptile-like peristalsis under the influence of an external magnetic field, and possesses the capability for film expansion and disintegration under high-frequency electromagnetic stimulation. These findings illuminate a promising path toward amplifying the therapeutic capacities of capsule endoscopes without necessitating a size compromise.

Helicobacter pylori, a pervasive bacterial infection associated with gastrointestinal disorders such as gastritis, peptic ulcer disease, and gastric cancer, impacts approximately 50% of the global population. The efficacy of standard clinical eradication therapies is diminishing due to the rise of antibiotic-resistant strains, necessitating alternative treatment strategies. Photodynamic therapy (PDT) emerges as a promising prospect in this context. This study presents the development and implementation of a magnetically-guided origami robot, incorporating flexible printed circuit units for sustained and stable phototherapy of Helicobacter pylori. Each integrated unit is equipped with wireless charging capabilities, producing an optimal power output that can concurrently illuminate up to 15 LEDs at their maximum intensity. Crucially, these units can be remotely manipulated via a magnetic field, facilitating both translational and rotational movements. We propose an open-loop manual control sequence that allows the formation of a stable, compliant triangular structure through the interaction of internal magnets. This adaptable configuration is uniquely designed to withstand the dynamic squeezing environment prevalent in real-world gastric applications. The precise tracking and segmentation of surgical instruments have led to a remarkable enhancement in the efficiency of surgical procedures. However, the challenge lies in achieving accurate segmentation of surgical instruments while minimizing the need for manual annotation and reducing the time required for the segmentation process. To tackle this, we propose a novel framework for surgical instrument segmentation and tracking. Specifically, with a tiny subset of frames for segmentation, we ensure accurate segmentation across the entire surgical video. Our method adopts a two-stage approach to efficiently segment videos. Initially, we utilize the Segment-Anything (SAM) model, which has been fine-tuned using the Low-Rank Adaptation (LoRA) on the EndoVis17 Dataset. The fine-tuned SAM model is applied to segment the initial frames of the video accurately. Subsequently, we deploy the XMem++ tracking algorithm to follow the annotated frames, thereby facilitating the segmentation of the entire video sequence. This workflow enables us to precisely segment and track objects within the video. Through extensive evaluation of the in-distribution dataset (EndoVis17) and the out-of-distribution datasets (EndoVis18 & the endoscopic submucosal dissection surgery (ESD) dataset), our framework demonstrates exceptional accuracy and robustness, thus showcasing its potential to advance the automated robotic-assisted surgery., Comment: IEEE ICRA 2024 C4SR+ Workshop

Recent advancements in Surgical Visual Question Answering (Surgical-VQA) and related region grounding have shown great promise for robotic and medical applications, addressing the critical need for automated methods in personalized surgical mentorship. However, existing models primarily provide simple structured answers and struggle with complex scenarios due to their limited capability in recognizing long-range dependencies and aligning multimodal information. In this paper, we introduce Surgical-LVLM, a novel personalized large vision-language model tailored for complex surgical scenarios. Leveraging the pre-trained large vision-language model and specialized Visual Perception LoRA (VP-LoRA) blocks, our model excels in understanding complex visual-language tasks within surgical contexts. In addressing the visual grounding task, we propose the Token-Interaction (TIT) module, which strengthens the interaction between the grounding module and the language responses of the Large Visual Language Model (LVLM) after projecting them into the latent space. We demonstrate the effectiveness of Surgical-LVLM on several benchmarks, including EndoVis-17-VQLA, EndoVis-18-VQLA, and a newly introduced EndoVis Conversations dataset, which sets new performance standards. Our work contributes to advancing the field of automated surgical mentorship by providing a context-aware solution.

The precise segmentation of ore images is critical to the successful execution of the beneficiation process. Due to the homogeneous appearance of the ores, which leads to low contrast and unclear boundaries, accurate segmentation becomes challenging, and recognition becomes problematic. This paper proposes a lightweight framework based on Multi-Layer Perceptron (MLP), which focuses on solving the problem of edge burring. Specifically, we introduce a lightweight backbone better suited for efficiently extracting low-level features. Besides, we design a feature pyramid network consisting of two MLP structures that balance local and global information thus enhancing detection accuracy. Furthermore, we propose a novel loss function that guides the prediction points to match the instance edge points to achieve clear object boundaries. We have conducted extensive experiments to validate the efficacy of our proposed method. Our approach achieves a remarkable processing speed of over 27 frames per second (FPS) with a model size of only 73 MB. Moreover, our method delivers a consistently high level of accuracy, with impressive performance scores of 60.4 and 48.9 in~$AP_{50}^{box}$ and~$AP_{50}^{mask}$ respectively, as compared to the currently available state-of-the-art techniques, when tested on the ore image dataset. Wireless capsule endoscopy (WCE) is a non-invasive diagnostic procedure that enables visualization of the gastrointestinal (GI) tract. Deep learning-based methods have shown effectiveness in disease screening using WCE data, alleviating the burden on healthcare professionals. However, existing capsule endoscopy classification methods mostly rely on pre-defined categories, making it challenging to identify and classify out-of-distribution (OOD) data, such as undefined categories or anatomical landmarks. To address this issue, we propose the Endoscopy Out-of-Distribution (EndoOOD) framework, which aims to effectively handle the OOD detection challenge in WCE diagnosis. The proposed framework focuses on improving the robustness and reliability of WCE diagnostic capabilities by incorporating uncertainty-aware mixup training and long-tailed in-distribution (ID) data calibration techniques. Additionally, virtual-logit matching is employed to accurately distinguish between OOD and ID data while minimizing information loss. To assess the performance of our proposed solution, we conduct evaluations and comparisons with 12 state-of-the-art (SOTA) methods using two publicly available datasets. In the realm of automated robotic surgery and computer-assisted interventions, understanding robotic surgical activities stands paramount. Existing algorithms dedicated to surgical activity recognition predominantly cater to pre-defined closed-set paradigms, ignoring the challenges of real-world open-set scenarios. Such algorithms often falter in the presence of test samples originating from classes unseen during training phases. To tackle this problem, we introduce an innovative Open-Set Surgical Activity Recognition (OSSAR) framework. Our solution leverages the hyperspherical reciprocal point strategy to enhance the distinction between known and unknown classes in the feature space. Additionally, we address the issue of over-confidence in the closed set by refining model calibration, avoiding misclassification of unknown classes as known ones. To support our assertions, we establish an open-set surgical activity benchmark utilizing the public JIGSAWS dataset. Besides, we also collect a novel dataset on endoscopic submucosal dissection for surgical activity tasks. Deep Neural Networks (DNNs) based semantic segmentation of the robotic instruments and tissues can enhance the precision of surgical activities in robot-assisted surgery. However, in biological learning, DNNs cannot learn incremental tasks over time and exhibit catastrophic forgetting, which refers to the sharp decline in performance on previously learned tasks after learning a new one. Specifically, when data scarcity is the issue, the model shows a rapid drop in performance on previously learned instruments after learning new data with new instruments. The problem becomes worse when it limits releasing the dataset of the old instruments for the old model due to privacy concerns and the unavailability of the data for the new or updated version of the instruments for the continual learning model. For this purpose, we develop a privacy-preserving synthetic continual semantic segmentation framework by blending and harmonizing (i) open-source old instruments foreground to the synthesized background without revealing real patient data in public and (ii) new instruments foreground to extensively augmented real background. To boost the balanced logit distillation from the old model to the continual learning model, we design overlapping class-aware temperature normalization (CAT) by controlling model learning utility. We also introduce multi-scale shifted-feature distillation (SD) to maintain long and short-range spatial relationships among the semantic objects where conventional short-range spatial features with limited information reduce the power of feature distillation. We demonstrate the effectiveness of our framework on the EndoVis 2017 and 2018 instrument segmentation dataset with a generalized continual learning setting. In the realm of robot-assisted minimally invasive surgery, dynamic scene reconstruction can significantly enhance downstream tasks and improve surgical outcomes. Neural Radiance Fields (NeRF)-based methods have recently risen to prominence for their exceptional ability to reconstruct scenes but are hampered by slow inference speed, prolonged training, and inconsistent depth estimation. Some previous work utilizes ground truth depth for optimization but is hard to acquire in the surgical domain. To overcome these obstacles, we present Endo-4DGS, a real-time endoscopic dynamic reconstruction approach that utilizes 3D Gaussian Splatting (GS) for 3D representation. Specifically, we propose lightweight MLPs to capture temporal dynamics with Gaussian deformation fields. To obtain a satisfactory Gaussian Initialization, we exploit a powerful depth estimation foundation model, Depth-Anything, to generate pseudo-depth maps as a geometry prior. We additionally propose confidence-guided learning to tackle the ill-pose problems in monocular depth estimation and enhance the depth-guided reconstruction with surface normal constraints and depth regularization. Our approach has been validated on two surgical datasets, where it can effectively render in real-time, compute efficiently, and reconstruct with remarkable accuracy.

Purpose: Depth estimation in robotic surgery is vital in 3D reconstruction, surgical navigation and augmented reality visualization. Although the foundation model exhibits outstanding performance in many vision tasks, including depth estimation (e.g., DINOv2), recent works observed its limitations in medical and surgical domain-specific applications. This work presents a low-ranked adaptation (LoRA) of the foundation model for surgical depth estimation. Methods: We design a foundation model-based depth estimation method, referred to as Surgical-DINO, a low-rank adaptation of the DINOv2 for depth estimation in endoscopic surgery. We build LoRA layers and integrate them into DINO to adapt with surgery-specific domain knowledge instead of conventional fine-tuning. During training, we freeze the DINO image encoder, which shows excellent visual representation capacity, and only optimize the LoRA layers and depth decoder to integrate features from the surgical scene. Results: Our model is extensively validated on a MICCAI challenge dataset of SCARED, which is collected from da Vinci Xi endoscope surgery. We empirically show that Surgical-DINO significantly outperforms all the state-of-the-art models in endoscopic depth estimation tasks. The analysis with ablation studies has shown evidence of the remarkable effect of our LoRA layers and adaptation. Conclusion: Surgical-DINO shed some light on the successful adaptation of the foundation models into the surgical domain for depth estimation. There is clear evidence in the results that zero-shot prediction on pre-trained weights in computer vision datasets or naive fine-tuning is not sufficient to use the foundation model in the surgical domain directly. Surgical tool segmentation and action recognition are fundamental building blocks in many computer-assisted intervention applications, ranging from surgical skills assessment to decision support systems. Nowadays, learning-based action recognition and segmentation approaches outperform classical methods, relying, however, on large, annotated datasets. Furthermore, action recognition and tool segmentation algorithms are often trained and make predictions in isolation from each other, without exploiting potential cross-task relationships. With the EndoVis 2022 SAR-RARP50 challenge, we release the first multimodal, publicly available, in-vivo, dataset for surgical action recognition and semantic instrumentation segmentation, containing 50 suturing video segments of Robotic Assisted Radical Prostatectomy (RARP). The aim of the challenge is twofold. First, to enable researchers to leverage the scale of the provided dataset and develop robust and highly accurate single-task action recognition and tool segmentation approaches in the surgical domain. Second, to further explore the potential of multitask-based learning approaches and determine their comparative advantage against their single-task counterparts. A total of 12 teams participated in the challenge, contributing 7 action recognition methods, 9 instrument segmentation techniques, and 4 multitask approaches that integrated both action recognition and instrument segmentation. Despite the successful application of convolutional neural networks (CNNs) in object detection tasks, their efficiency in detecting faults from freight train images remains inadequate for implementation in real-world engineering scenarios. Existing modeling shortcomings of spatial invariance and pooling layers in conventional CNNs often ignore the neglect of crucial global information, resulting in error localization for fault objection tasks of freight trains. To solve these problems, we design a spatial-wise dynamic distillation framework based on multi-layer perceptron (MLP) for visual fault detection of freight trains. We initially present the axial shift strategy, which allows the MLP-like architecture to overcome the challenge of spatial invariance and effectively incorporate both local and global cues. We propose a dynamic distillation method without a pre-training teacher, including a dynamic teacher mechanism that can effectively eliminate the semantic discrepancy with the student model. Such an approach mines more abundant details from lower-level feature appearances and higher-level label semantics as the extra supervision signal, which utilizes efficient instance embedding to model the global spatial and semantic information. In addition, the proposed dynamic teacher can jointly train with students to further enhance the distillation efficiency. Extensive experiments executed on six typical fault datasets reveal that our approach outperforms the current state-of-the-art detectors and achieves the highest accuracy with real-time detection at a lower computational cost. Considering that Coupled Dictionary Learning (CDL) method can obtain a reasonable linear mathematical relationship between resource images, we propose a novel CDL-based Synthetic Aperture Radar (SAR) and multispectral pseudo-color fusion method. Firstly, the traditional Brovey transform is employed as a pre-processing method on the paired SAR and multispectral images. Then, CDL is used to capture the correlation between the pre-processed image pairs based on the dictionaries generated from the source images via enforced joint sparse coding. Afterward, the joint sparse representation in the pair of dictionaries is utilized to construct an image mask via calculating the reconstruction errors, and therefore generate the final fusion image. The experimental verification results of the SAR images from the Sentinel-1 satellite and the multispectral images from the Landsat-8 satellite show that the proposed method can achieve superior visual effects, and excellent quantitative performance in terms of spectral distortion, correlation coefficient, MSE, NIQE, BRISQUE, and PIQE., Comment: To appear in IEEE Sensors Journal

Surgical robotics is a rapidly evolving field that is transforming the landscape of surgeries. Surgical robots have been shown to enhance precision, minimize invasiveness, and alleviate surgeon fatigue. One promising area of research in surgical robotics is the use of reinforcement learning to enhance the automation level. Reinforcement learning is a type of machine learning that involves training an agent to make decisions based on rewards and punishments. This literature review aims to comprehensively analyze existing research on reinforcement learning in surgical robotics. The review identified various applications of reinforcement learning in surgical robotics, including pre-operative, intra-body, and percutaneous procedures, listed the typical studies, and compared their methodologies and results. The findings show that reinforcement learning has great potential to improve the autonomy of surgical robots. Reinforcement learning can teach robots to perform complex surgical tasks, such as suturing and tissue manipulation. It can also improve the accuracy and precision of surgical robots, making them more effective at performing surgeries.

On-chip microring resonators (MRRs) have been proposed to construct the time-delayed reservoir computing (RC), which offers promising configurations available for computation with high scalability, high-density computing, and easy fabrication. A single MRR, however, is inadequate to supply enough memory for the computational task with diverse memory requirements. Large memory needs are met by the MRR with optical feedback waveguide, but at the expense of its large footprint. In the structure, the ultra-long optical feedback waveguide substantially limits the scalable photonic RC integrated designs. In this paper, a time-delayed RC is proposed by utilizing a silicon-based nonlinear MRR in conjunction with an array of linear MRRs. These linear MRRs possess a high quality factor, providing sufficient memory capacity for the entire system. We quantitatively analyze and assess the proposed RC structure's performance on three classical tasks with diverse memory requirements, i.e., the Narma 10, Mackey-Glass, and Santa Fe chaotic timeseries prediction tasks. The proposed system exhibits comparable performance to the MRR with an ultra-long optical feedback waveguide-based system when it comes to handling the Narma 10 task, which requires a significant memory capacity. Nevertheless, the overall length of these linear MRRs is significantly smaller, by three orders of magnitude, compared to the ultra-long feedback waveguide in the MRR with optical feedback waveguide-based system. The compactness of this structure has significant implications for the scalability and seamless integration of photonic RC.

Endoscopy is a widely used technique for the early detection of diseases or robotic-assisted minimally invasive surgery (RMIS). Numerous deep learning (DL)-based research works have been developed for automated diagnosis or processing of endoscopic view. However, existing DL models may suffer from catastrophic forgetting. When new target classes are introduced over time or cross institutions, the performance of old classes may suffer severe degradation. More seriously, data privacy and storage issues may lead to the unavailability of old data when updating the model. Therefore, it is necessary to develop a continual learning (CL) methodology to solve the problem of catastrophic forgetting in endoscopic image segmentation. To tackle this, we propose a Endoscopy Continual Semantic Segmentation (EndoCSS) framework that does not involve the storage and privacy issues of exemplar data. The framework includes a mini-batch pseudo-replay (MB-PR) mechanism and a self-adaptive noisy cross-entropy (SAN-CE) loss. The MB-PR strategy circumvents privacy and storage issues by generating pseudo-replay images through a generative model. Meanwhile, the MB-PR strategy can also correct the model deviation to the replay data and current training data, which is aroused by the significant difference in the amount of current and replay images. Therefore, the model can perform effective representation learning on both new and old tasks. SAN-CE loss can help model fitting by adjusting the model's output logits, and also improve the robustness of training. Extensive continual semantic segmentation (CSS) experiments on public datasets demonstrate that our method can robustly and effectively address the catastrophic forgetting brought by class increment in endoscopy scenes. The Segment Anything Model (SAM) serves as a fundamental model for semantic segmentation and demonstrates remarkable generalization capabilities across a wide range of downstream scenarios. In this empirical study, we examine SAM's robustness and zero-shot generalizability in the field of robotic surgery. We comprehensively explore different scenarios, including prompted and unprompted situations, bounding box and points-based prompt approaches, as well as the ability to generalize under corruptions and perturbations at five severity levels. Additionally, we compare the performance of SAM with state-of-the-art supervised models. We conduct all the experiments with two well-known robotic instrument segmentation datasets from MICCAI EndoVis 2017 and 2018 challenges. Our extensive evaluation results reveal that although SAM shows remarkable zero-shot generalization ability with bounding box prompts, it struggles to segment the whole instrument with point-based prompts and unprompted settings. Furthermore, our qualitative figures demonstrate that the model either failed to predict certain parts of the instrument mask (e.g., jaws, wrist) or predicted parts of the instrument as wrong classes in the scenario of overlapping instruments within the same bounding box or with the point-based prompt. In fact, SAM struggles to identify instruments in complex surgical scenarios characterized by the presence of blood, reflection, blur, and shade. Additionally, SAM is insufficiently robust to maintain high performance when subjected to various forms of data corruption. We also attempt to fine-tune SAM using Low-rank Adaptation (LoRA) and propose SurgicalSAM, which shows the capability in class-wise mask prediction without prompt. In the realm of modern diagnostic technology, video capsule endoscopy (VCE) is a standout for its high efficacy and non-invasive nature in diagnosing various gastrointestinal (GI) conditions, including obscure bleeding. Importantly, for the successful diagnosis and treatment of these conditions, accurate recognition of bleeding regions in VCE images is crucial. While deep learning-based methods have emerged as powerful tools for the automated analysis of VCE images, they often demand large training datasets with comprehensive annotations. Acquiring these labeled datasets tends to be time-consuming, costly, and requires significant domain expertise. To mitigate this issue, we have embraced a semi-supervised learning (SSL) approach for the bleeding regions segmentation within VCE. By adopting the `Mean Teacher' method, we construct a student U-Net equipped with an scSE attention block, alongside a teacher model of the same architecture. These models' parameters are alternately updated throughout the training process. We use the Kvasir-Capsule dataset for our experiments, which encompasses various GI bleeding conditions. Notably, we develop the segmentation annotations for this dataset ourselves. Robot-assisted airway intubation application needs high accuracy in locating targets and organs. Two vital landmarks, nostrils and glottis, can be detected during the intubation to accommodate the stages of nasal intubation. Automated landmark detection can provide accurate localization and quantitative evaluation. The Detection Transformer (DeTR) leads object detectors to a new paradigm with long-range dependence. However, current DeTR requires long iterations to converge, and does not perform well in detecting small objects. This paper proposes a transformer-based landmark detection solution with deformable DeTR and the semantic-aligned-matching module for detecting landmarks in robot-assisted intubation. The semantics aligner can effectively align the semantics of object queries and image features in the same embedding space using the most discriminative features. To evaluate the performance of our solution, we utilize a publicly accessible glottis dataset and automatically annotate a nostril detection dataset. The visual-question localized-answering (VQLA) system can serve as a knowledgeable assistant in surgical education. Except for providing text-based answers, the VQLA system can highlight the interested region for better surgical scene understanding. However, deep neural networks (DNNs) suffer from catastrophic forgetting when learning new knowledge. Specifically, when DNNs learn on incremental classes or tasks, their performance on old tasks drops dramatically. Furthermore, due to medical data privacy and licensing issues, it is often difficult to access old data when updating continual learning (CL) models. Therefore, we develop a non-exemplar continual surgical VQLA framework, to explore and balance the rigidity-plasticity trade-off of DNNs in a sequential learning paradigm. We revisit the distillation loss in CL tasks, and propose rigidity-plasticity-aware distillation (RP-Dist) and self-calibrated heterogeneous distillation (SH-Dist) to preserve the old knowledge. The weight aligning (WA) technique is also integrated to adjust the weight bias between old and new tasks. We further establish a CL framework on three public surgical datasets in the context of surgical settings that consist of overlapping classes between old and new surgical VQLA tasks. With extensive experiments, we demonstrate that our proposed method excellently reconciles learning and forgetting on the continual surgical VQLA over conventional CL methods. Colorectal cancer is one of the most common cancers in the world. While colonoscopy is an effective screening technique, navigating an endoscope through the colon to detect polyps is challenging. A 3D map of the observed surfaces could enhance the identification of unscreened colon tissue and serve as a training platform. However, reconstructing the colon from video footage remains difficult. Learning-based approaches hold promise as robust alternatives, but necessitate extensive datasets. Establishing a benchmark dataset, the 2022 EndoVis sub-challenge SimCol3D aimed to facilitate data-driven depth and pose prediction during colonoscopy. The challenge was hosted as part of MICCAI 2022 in Singapore. Six teams from around the world and representatives from academia and industry participated in the three sub-challenges: synthetic depth prediction, synthetic pose prediction, and real pose prediction. This paper describes the challenge, the submitted methods, and their results. We show that depth prediction from synthetic colonoscopy images is robustly solvable, while pose estimation remains an open research question.

Noisy label problems are inevitably in existence within medical image segmentation causing severe performance degradation. Previous segmentation methods for noisy label problems only utilize a single image while the potential of leveraging the correlation between images has been overlooked. Especially for video segmentation, adjacent frames contain rich contextual information beneficial in cognizing noisy labels. Based on two insights, we propose a Multi-Scale Temporal Feature Affinity Learning (MS-TFAL) framework to resolve noisy-labeled medical video segmentation issues. First, we argue the sequential prior of videos is an effective reference, i.e., pixel-level features from adjacent frames are close in distance for the same class and far in distance otherwise. Therefore, Temporal Feature Affinity Learning (TFAL) is devised to indicate possible noisy labels by evaluating the affinity between pixels in two adjacent frames. We also notice that the noise distribution exhibits considerable variations across video, image, and pixel levels. In this way, we introduce Multi-Scale Supervision (MSS) to supervise the network from three different perspectives by re-weighting and refining the samples. This design enables the network to concentrate on clean samples in a coarse-to-fine manner. Experiments with both synthetic and real-world label noise demonstrate that our method outperforms recent state-of-the-art robust segmentation approaches. Medical students and junior surgeons often rely on senior surgeons and specialists to answer their questions when learning surgery. However, experts are often busy with clinical and academic work, and have little time to give guidance. Meanwhile, existing deep learning (DL)-based surgical Visual Question Answering (VQA) systems can only provide simple answers without the location of the answers. In addition, vision-language (ViL) embedding is still a less explored research in these kinds of tasks. Therefore, a surgical Visual Question Localized-Answering (VQLA) system would be helpful for medical students and junior surgeons to learn and understand from recorded surgical videos. We propose an end-to-end Transformer with the Co-Attention gaTed Vision-Language (CAT-ViL) embedding for VQLA in surgical scenarios, which does not require feature extraction through detection models. The CAT-ViL embedding module is designed to fuse multimodal features from visual and textual sources. The fused embedding will feed a standard Data-Efficient Image Transformer (DeiT) module, before the parallel classifier and detector for joint prediction. We conduct the experimental validation on public surgical videos from MICCAI EndoVis Challenge 2017 and 2018. The experimental results highlight the superior performance and robustness of our proposed model compared to the state-of-the-art approaches. Ablation studies further prove the outstanding performance of all the proposed components. The proposed method provides a promising solution for surgical scene understanding, and opens up a primary step in the Artificial Intelligence (AI)-based VQLA system for surgical training. Wireless capsule endoscopy (WCE) is a painless and non-invasive diagnostic tool for gastrointestinal (GI) diseases. However, due to GI anatomical constraints and hardware manufacturing limitations, WCE vision signals may suffer from insufficient illumination, leading to a complicated screening and examination procedure. Deep learning-based low-light image enhancement (LLIE) in the medical field gradually attracts researchers. Given the exuberant development of the denoising diffusion probabilistic model (DDPM) in computer vision, we introduce a WCE LLIE framework based on the multi-scale convolutional neural network (CNN) and reverse diffusion process. The multi-scale design allows models to preserve high-resolution representation and context information from low-resolution, while the curved wavelet attention (CWA) block is proposed for high-frequency and local feature learning. Furthermore, we combine the reverse diffusion procedure to further optimize the shallow output and generate the most realistic image. The proposed method is compared with ten state-of-the-art (SOTA) LLIE methods and significantly outperforms quantitatively and qualitatively. Despite their impressive performance in various surgical scene understanding tasks, deep learning-based methods are frequently hindered from deploying to real-world surgical applications for various causes. Particularly, data collection, annotation, and domain shift in-between sites and patients are the most common obstacles. In this work, we mitigate data-related issues by efficiently leveraging minimal source images to generate synthetic surgical instrument segmentation datasets and achieve outstanding generalization performance on unseen real domains. Specifically, in our framework, only one background tissue image and at most three images of each foreground instrument are taken as the seed images. These source images are extensively transformed and employed to build up the foreground and background image pools, from which randomly sampled tissue and instrument images are composed with multiple blending techniques to generate new surgical scene images. Besides, we introduce hybrid training-time augmentations to diversify the training data further. Extensive evaluation on three real-world datasets, i.e., Endo2017, Endo2018, and RoboTool, demonstrates that our one-to-many synthetic surgical instruments datasets generation and segmentation framework can achieve encouraging performance compared with training with real data. Notably, on the RoboTool dataset, where a more significant domain gap exists, our framework shows its superiority of generalization by a considerable margin. Accurate and robust medical image segmentation is fundamental and crucial for enhancing the autonomy of computer-aided diagnosis and intervention systems. Medical data collection normally involves different scanners, protocols, and populations, making domain adaptation (DA) a highly demanding research field to alleviate model degradation in the deployment site. To preserve the model performance across multiple testing domains, this work proposes the Curriculum-based Augmented Fourier Domain Adaptation (Curri-AFDA) for robust medical image segmentation. In particular, our curriculum learning strategy is based on the causal relationship of a model under different levels of data shift in the deployment phase, where the higher the shift is, the harder to recognize the variance. Considering this, we progressively introduce more amplitude information from the target domain to the source domain in the frequency space during the curriculum-style training to smoothly schedule the semantic knowledge transfer in an easier-to-harder manner. Besides, we incorporate the training-time chained augmentation mixing to help expand the data distributions while preserving the domain-invariant semantics, which is beneficial for the acquired model to be more robust and generalize better to unseen domains. Extensive experiments on two segmentation tasks of Retina and Nuclei collected from multiple sites and scanners suggest that our proposed method yields superior adaptation and generalization performance. Meanwhile, our approach proves to be more robust under various corruption types and increasing severity levels. In addition, we show our method is also beneficial in the domain-adaptive classification task with skin lesion datasets. Fully-supervised polyp segmentation has accomplished significant triumphs over the years in advancing the early diagnosis of colorectal cancer. However, label-efficient solutions from weak supervision like scribbles are rarely explored yet primarily meaningful and demanding in medical practice due to the expensiveness and scarcity of densely-annotated polyp data. Besides, various deployment issues, including data shifts and corruption, put forward further requests for model generalization and robustness. To address these concerns, we design a framework of Spatial-Spectral Dual-branch Mutual Teaching and Entropy-guided Pseudo Label Ensemble Learning (S$^2$ME). Concretely, for the first time in weakly-supervised medical image segmentation, we promote the dual-branch co-teaching framework by leveraging the intrinsic complementarity of features extracted from the spatial and spectral domains and encouraging cross-space consistency through collaborative optimization. Furthermore, to produce reliable mixed pseudo labels, which enhance the effectiveness of ensemble learning, we introduce a novel adaptive pixel-wise fusion technique based on the entropy guidance from the spatial and spectral branches. Our strategy efficiently mitigates the deleterious effects of uncertainty and noise present in pseudo labels and surpasses previous alternatives in terms of efficacy. Ultimately, we formulate a holistic optimization objective to learn from the hybrid supervision of scribbles and pseudo labels. Extensive experiments and evaluation on four public datasets demonstrate the superiority of our method regarding in-distribution accuracy, out-of-distribution generalization, and robustness, highlighting its promising clinical significance. Despite the availability of computer-aided simulators and recorded videos of surgical procedures, junior residents still heavily rely on experts to answer their queries. However, expert surgeons are often overloaded with clinical and academic workloads and limit their time in answering. For this purpose, we develop a surgical question-answering system to facilitate robot-assisted surgical scene and activity understanding from recorded videos. Most of the existing VQA methods require an object detector and regions based feature extractor to extract visual features and fuse them with the embedded text of the question for answer generation. However, (1) surgical object detection model is scarce due to smaller datasets and lack of bounding box annotation; (2) current fusion strategy of heterogeneous modalities like text and image is naive; (3) the localized answering is missing, which is crucial in complex surgical scenarios. In this paper, we propose Visual Question Localized-Answering in Robotic Surgery (Surgical-VQLA) to localize the specific surgical area during the answer prediction. To deal with the fusion of the heterogeneous modalities, we design gated vision-language embedding (GVLE) to build input patches for the Language Vision Transformer (LViT) to predict the answer. To get localization, we add the detection head in parallel with the prediction head of the LViT. We also integrate GIoU loss to boost localization performance by preserving the accuracy of the question-answering model. We annotate two datasets of VQLA by utilizing publicly available surgical videos from MICCAI challenges EndoVis-17 and 18. Our validation results suggest that Surgical-VQLA can better understand the surgical scene and localize the specific area related to the question-answering. Robotic-assisted tracheal intubation requires the robot to distinguish anatomical features like an experienced physician using deep-learning techniques. However, real datasets of oropharyngeal organs are limited due to patient privacy issues, making it challenging to train deep-learning models for accurate image segmentation. We hereby consider generating a new data modality through a virtual environment to assist the training process. Specifically, this work introduces a virtual dataset generated by the Simulation Open Framework Architecture (SOFA) framework to overcome the limited availability of actual endoscopic images. We also propose a domain adaptive Sim-to-Real method for oropharyngeal organ image segmentation, which employs an image blending strategy called IoU-Ranking Blend (IRB) and style-transfer techniques to address discrepancies between datasets. Experimental results demonstrate the superior performance of the proposed approach with domain adaptive models, improving segmentation accuracy and training stability. Due to its non-invasive and painless characteristics, wireless capsule endoscopy has become the new gold standard for assessing gastrointestinal disorders. Omissions, however, could occur throughout the examination since controlling capsule endoscope can be challenging. In this work, we control the magnetic capsule endoscope for the coverage scanning task in the stomach based on reinforcement learning so that the capsule can comprehensively scan every corner of the stomach. We apply a well-made virtual platform named VR-Caps to simulate the process of stomach coverage scanning with a capsule endoscope model. We utilize and compare two deep reinforcement learning algorithms, the Proximal Policy Optimization (PPO) and Soft Actor-Critic (SAC) algorithms, to train the permanent magnetic agent, which actuates the capsule endoscope directly via magnetic fields and then optimizes the scanning efficiency of stomach coverage. We analyze the pros and cons of the two algorithms with different hyperparameters and achieve a coverage rate of 98.04% of the stomach area within 150.37 seconds., Comment: IEEE ROBIO 2022

Video-assisted transoral tracheal intubation (TI) necessitates using an endoscope that helps the physician insert a tracheal tube into the glottis instead of the esophagus. The growing trend of robotic-assisted TI would require a medical robot to distinguish anatomical features like an experienced physician which can be imitated by utilizing supervised deep-learning techniques. However, the real datasets of oropharyngeal organs are often inaccessible due to limited open-source data and patient privacy. In this work, we propose a domain adaptive Sim-to-Real framework called IoU-Ranking Blend-ArtFlow (IRB-AF) for image segmentation of oropharyngeal organs. The framework includes an image blending strategy called IoU-Ranking Blend (IRB) and style-transfer method ArtFlow. Here, IRB alleviates the problem of poor segmentation performance caused by significant datasets domain differences; while ArtFlow is introduced to reduce the discrepancies between datasets further. A virtual oropharynx image dataset generated by the SOFA framework is used as the learning subject for semantic segmentation to deal with the limited availability of actual endoscopic images. We adapted IRB-AF with the state-of-the-art domain adaptive segmentation models. The ability to automatically detect and track surgical instruments in endoscopic videos can enable transformational interventions. Assessing surgical performance and efficiency, identifying skilled tool use and choreography, and planning operational and logistical aspects of OR resources are just a few of the applications that could benefit. Unfortunately, obtaining the annotations needed to train machine learning models to identify and localize surgical tools is a difficult task. Annotating bounding boxes frame-by-frame is tedious and time-consuming, yet large amounts of data with a wide variety of surgical tools and surgeries must be captured for robust training. Moreover, ongoing annotator training is needed to stay up to date with surgical instrument innovation. In robotic-assisted surgery, however, potentially informative data like timestamps of instrument installation and removal can be programmatically harvested. The ability to rely on tool installation data alone would significantly reduce the workload to train robust tool-tracking models. With this motivation in mind we invited the surgical data science community to participate in the challenge, SurgToolLoc 2022. The goal was to leverage tool presence data as weak labels for machine learning models trained to detect tools and localize them in video frames with bounding boxes. We present the results of this challenge along with many of the team's efforts. We conclude by discussing these results in the broader context of machine learning and surgical data science. Segment Anything Model (SAM) is a foundation model for semantic segmentation and shows excellent generalization capability with the prompts. In this empirical study, we investigate the robustness and zero-shot generalizability of the SAM in the domain of robotic surgery in various settings of (i) prompted vs. unprompted; (ii) bounding box vs. points-based prompt; (iii) generalization under corruptions and perturbations with five severity levels; and (iv) state-of-the-art supervised model vs. SAM. We conduct all the observations with two well-known robotic instrument segmentation datasets of MICCAI EndoVis 2017 and 2018 challenges. Our extensive evaluation results reveal that although SAM shows remarkable zero-shot generalization ability with bounding box prompts, it struggles to segment the whole instrument with point-based prompts and unprompted settings. Furthermore, our qualitative figures demonstrate that the model either failed to predict the parts of the instrument mask (e.g., jaws, wrist) or predicted parts of the instrument as different classes in the scenario of overlapping instruments within the same bounding box or with the point-based prompt. In fact, it is unable to identify instruments in some complex surgical scenarios of blood, reflection, blur, and shade. Additionally, SAM is insufficiently robust to maintain high performance when subjected to various forms of data corruption. Advances in GPT-based large language models (LLMs) are revolutionizing natural language processing, exponentially increasing its use across various domains. Incorporating uni-directional attention, these autoregressive LLMs can generate long and coherent paragraphs. However, for visual question answering (VQA) tasks that require both vision and language processing, models with bi-directional attention or models employing fusion techniques are often employed to capture the context of multiple modalities all at once. As GPT does not natively process vision tokens, to exploit the advancements in GPT models for VQA in robotic surgery, we design an end-to-end trainable Language-Vision GPT (LV-GPT) model that expands the GPT2 model to include vision input (image). The proposed LV-GPT incorporates a feature extractor (vision tokenizer) and vision token embedding (token type and pose). Given the limitations of unidirectional attention in GPT models and their ability to generate coherent long paragraphs, we carefully sequence the word tokens before vision tokens, mimicking the human thought process of understanding the question to infer an answer from an image. Quantitatively, we prove that the LV-GPT model outperforms other state-of-the-art VQA models on two publically available surgical-VQA datasets (based on endoscopic vision challenge robotic scene segmentation 2018 and CholecTriplet2021) and on our newly annotated dataset (based on the holistic surgical scene dataset). We further annotate all three datasets to include question-type annotations to allow sub-type analysis.
