Dissertação para obtenção do grau de Mestre no Instituto Universitário Egas Moniz Objetivo: A presente revisão sistemática teve como principais objetivos avaliar o outcome do reimplante intencional, face à evolução dos materiais e técnicas endo-cirúrgicas, assim como identificar os possíveis fatores prognósticos associados. Materiais e Métodos: A pesquisa sistemática foi efetuada em três bases de dados digitais (PubMed, Scopus e Web of Science), a fim de eleger toda a literatura disponibilizada na sua versão completa, publicada nas línguas portuguesa e inglesa, durante os últimos 10 anos (até 29 de janeiro de 2022). A qualidade metodológica dos estudos incluídos, foi avaliada através da medição do risco de viés associado, de acordo com as ferramentas desenvolvidas pela National Heart, Lung, and Blood Institute. A taxa de sucesso do reimplante intencional foi estimada, de forma indireta, a partir do cálculo da taxa de insucesso por mês. Posteriormente, os resultados foram ajustados através de modelos meta-analíticos randomizados. Resultados: O processo de seleção de dados permitiu a inclusão de 13 estudos para revisão. Dos estudos incluídos, 9 foram submetidos a meta-análise. A análise global dos 9 estudos permitiu estimar uma taxa de insucesso média de, aproximadamente, 0.73%/mês. Dentro das limitações da metodologia utilizada, foi possível extrapolar os resultados obtidos para taxas de sucesso de 91.2% e 82.4%, ao fim de 1 e 2 anos, respetivamente. A partir da análise dos modelos meta-analíticos observou-se que a taxa de insucesso foi significativamente inferior para o grupo que utilizou as curvas de Kaplan-Meier no cálculo do outcome (p < 0.001) e para as amostras que incluíram um maior número de dentes pertencentes ao setor anterior (p = 0.002). Conclusões: Face à evidência atual e às taxas de sucesso e de sobrevivência reportadas, o reimplante intencional pode ser considerado, em casos devidamente selecionados, uma abordagem terapêutica alternativa com previsibilidade na preservação da dentição natural. Objective: The main objective of this systematic review was to assess the outcome of intentional replantation, given the evolution of materials and endodontic surgical techniques, as well as to identify the possible associated prognostic factors. Materials and Methods: A systematic search was carried out in three digital databases (PubMed, Scopus, and Web of Science), in order to select all the literature available in its full version, published in Portuguese and English, during the last 10 years (until January 29, 2022). The methodological quality of the included studies was assessed by measuring the associated risk of bias, according to tools developed by the National Heart, Lung, and Blood Institute. The success rate of intentional replantation was estimated, indirectly, from the calculation of the failure rate per month. Subsequently, the results were adjusted using randomized meta-analytic models. Results: The data selection process allowed the inclusion of 13 studies for review. Of the included studies, 9 underwent meta-analysis. The global analysis of the 9 studies allowed to estimate an average failure rate of approximately 0.73%/month. Within the limitations of the methodology used, it was possible to extrapolate the results obtained to success rates of 91.2% and 82.4%, after 1 and 2 years, respectively. From the analysis of the meta-analytic models, it was observed that the failure rate was significantly lower for the group that used the Kaplan-Meier curves to calculate the outcome (p < 0.001) and for the samples that included a greater number of teeth belonging to the anterior sector (p = 0.002). Conclusions: Given the current evidence and the reported success and survival rates, intentional replantation can be considered, in duly selected cases, an alternative therapeutic approach with predictability in preserving the natural dentition.
