Abstract

We provide a comprehensive theoretical study of the radio-frequency (rf) spectra of a two-component Fermi gas with balanced populations in the normal region of the temperature-vs-coupling phase diagram. In particular, rf spectra are analyzed in terms of two characteristic peaks, which can be either distinct or overlapping. On the BEC side of the crossover, these two contributions are associated with a fermionic quasi-particle peak and a bosonic-like contribution due to pairing. On the BCS side of the crossover, the two peaks are instead associated with interactions between particles occurring, respectively, at high or low relative momenta. Through this two-peak analysis, we show how and to what extent the correlation between the widths of the rf spectra and the pair size, previously identified in the superfluid phase at low temperature, can be extended to the normal phase, as well as how the temperature-vs-coupling phase diagram of the BCS-BEC crossover can be partitioned in a number of distinct physical sectors. 4. Investigation of non-ideal effects in compressible boundary layers of dense vapors through direct numerical simulations

Abstract

In this work, we present an investigation about the sources of dissipation in adiabatic boundary layers of non-ideal compressible fluid flows. Direct numerical simulations (DNS) of transitional, zero-pressure gradient boundary layer flows are performed for two fluids characterized by different complexity of the fluid molecules, namely, “air” and siloxane MM. Different sets of thermodynamic free-stream boundary conditions are selected to evaluate the influence of the fluid state on both the frictional loss and the dissipation mechanisms. The thermophysical properties of siloxane MM are calculated with a state-of-the-art equation of state. Results show that the dissipation due to both time-mean strain field, irreversible heat transfer, and turbulent dissipation differs significantly depending on both the molecular complexity of the fluid and its thermodynamic state. The dissipation coefficient calculated from the DNS results is then compared against the one obtained using a reduced-order model (ROM), which solves the two-dimensional boundary layer flow equations for an arbitrary fluid [M. Abstract

The gas dynamics of single-phase nonreacting fluids whose thermodynamic states are close to vapor-liquid saturation, close to the vapor-liquid critical point, or in supercritical conditions differs quantitatively and qualitatively from the textbook gas dynamics of dilute, ideal gases. Due to nonideal fluid thermodynamic properties, unconventional gas dynamic effects are possible, including nonclassical rarefaction shock waves and the nonmonotonic variation of the Mach number along steady isentropic expansions. This review provides a comprehensive theoretical framework of the fundamentals of nonideal compressible fluid dynamics (NICFD). The relation between nonideal gas dynamics and the complexity of the fluid molecules is clarified. The theoretical, numerical, and experimental tools currently employed to investigate NICFD flows and related applications are reviewed, followed by an overview of industrial processes involving NICFD, ranging from organic Rankine and supercritical CO2 cycle power systems to supercritical processes. 6. The effect of working fluid and compressibility on the optimal solidity of axial turbine cascades

Abstract

The blade solidity, namely the blade chordtopitch ratio, largely affects the fluiddynamic performance of turbomachinery. For turbomachines operating with air or steam, the optimal value of the solidity which maximizes the efficiency is estimated with empirical correlations such as the ones proposed by Zweifel (1945) and Traupel (1966). However, if the turbomachine operates with unconventional fluids, the accuracy of these correlations becomes questionable. Examples of such working fluids are the nonideal (dense) vapors of organic compounds (e.g., hydrocarbons, siloxanes) used to operate organic Rankine cycle (ORC) power systems. This study investigates the effect of both the working fluid and the flow compressibility on the optimum pitchtochord ratio of turbine stages. A first principle model for the profile losses is developed for this purpose. Charts providing the optimal pitchtochord ratio for unconventional turbine stages are then provided. Numerical simulations of the flow over a turbine stator cascade have been conducted to validate the model results and evaluate the influence of both working fluid, flow compressibility, and solidity value on the loss breakdown. The results show that the optimal solidity of turbine cascades value significantly increases with the flow compressibility. 9. Validation of the SU2 Fluid Dynamic Solver for Isentropic Non-Ideal Compressible Flows

Abstract

This work assessed the accuracy of the SU2 flow solver in predicting the isentropic expansion of Siloxane MM through the converging-diverging nozzle test section of the Organic Rankine Cycle Hybrid Integrated Device (ORCHID) [9]. The expansion is modeled using compressible Euler equations, and assuming adiabatic flow, while the fluid thermodynamic properties are estimated using the Peng-Robinson equation of state. The boundary conditions for the experiment and simulations correspond to a stagnation temperature and pressure of T¯0=253.7∘C and P¯0=18.36bar. At these inlet conditions the compressibility factor of the fluid is Z0= 0.58. The back pressure was equal to P¯b=2.21bar. The Mach number along the centreline, and static pressure along the nozzle surface were used as the system response quantities for the validation exercise. The studied SU2 model provides valid predictions for Mach number and static pressure. The largest deviation observed in the Mach number comparison between the simulation and experiment is in the uniform flow region of the nozzle and is equal to EMach= 0.045. Regarding the pressure trend, the largest discrepancy occurs in the kernel region and is equal to Epressure= 9 kPa. At the same time, the simulated Mach number and static pressure reach a maximum absolute uncertainty of ± 0.015 and of ± 20 kPa, respectively. For both quantities, these values are reached in the region close to the throat. All the uncertainties calculated for the simulated pressure profile were larger than those of the experiments. The static pressure is particularly sensitive to the geometrical uncertainties of the nozzle profile, especially inside the kernel region. A proper characterisation of the nozzle geometry was therefore required to perform a meaningful validation of the fluid dynamic solver. Abstract

We present a diffuse interface method for a pressure-based Baer-Nunziato type model for compressible two-phase flows, which allows the use of generic equations of state to describe each phase. The model is made dimensionless by means of a special pressure scaling that recovers the correct scaling of the discrete governing equations in the zero Mach limit, and overcomes the difficulties related to the lack of a clear notion of reference speed of sound in non-equilibrium two-phase flows. The model is equipped with pressure and velocity relaxation terms to impose the mechanical equilibrium between phases after their independent evolution. Two different finite volume schemes are presented. First, a 1D semi-implicit staggered scheme is introduced to show the capability of the model to work with the Peng-Robinson EOS when each phase evolves close to the saturation curve. Then, a preliminary 2D explicit scheme, which does not include the relaxation terms, is presented as a first step toward the development of an unstructured 2D scheme for compressible two-phase flows at all Mach numbers. 12. The Influence of Environmental Factors on the Spread of COVID-19 in Italy

Abstract

The aim of this work is to investigate possible relationships between air quality and the spread of the pandemic. We evaluate the performance of machine learning techniques in predicting new cases. Specifically, we describe a cross-correlation analysis on daily COVID-19 cases and environmental factors, such as temperature, relative humidity, and atmospheric pollutants. Our analysis confirms a significant association of some environmental parameters with the spread of the virus. This suggests that machine learning models trained using environmental parameters might provide accurate predictions about the number of infected cases. Our empirical evaluation shows that temperature and ozone are negatively correlated with confirmed cases (therefore, the higher the values of these parameters, the lower the number of infected cases), whereas atmospheric particulate matter and nitrogen dioxide are positively correlated. We developed and compared three different predictive models to test whether these technologies can be useful to estimate the evolution of the pandemic.

14. NiceProp: An interactive Python-based educational tool for non-ideal compressible fluid dynamics

Abstract

Non-ideal compressible flows exhibit physical behaviors that are quantitatively and qualitatively different than those of a perfect or ideal gas. As such, the classical gas dynamic relationships that can be found on fluid-mechanics textbooks cannot be directly applied to characterize this type of flows. NiceProp is a tool for interactively learning the fundamentals of non-ideal compressible fluid dynamics and the design implications for fluid machinery components. The software is written in Python and features a highly modular structure to ease code readability and to facilitate its further development. 15. Multirow Adjoint-Based Optimization of NICFD Turbomachinery Using a Computer-Aided Design-Based Parametrization

Abstract

Currently, most of the adjoint-based design systems documented in the open literature assume that the fluid behaves as an ideal gas, are restricted to the optimization of a single row of blades, or are not suited to impose geometric constraints. In response to these limitations, this paper presents a gradient-based shape optimization framework for the aerodynamic design of turbomachinery blades operating under nonideal thermodynamic conditions. The proposed design system supports the optimization of multiple blade rows, and it integrates a computer-aided design (CAD)-based parametrization with a Reynolds-averaged Navier–Stokes (RANS) flow solver and its discrete adjoint counterpart. The capabilities of the method were demonstrated by performing the design optimization of a single-stage axial turbine that employs isobutane (R600a) as working fluid. Notably, the aerodynamic optimization respected the minimum thickness constraint at the trailing edge of the stator and rotor blades and reduced the entropy generation within the turbine by 36%, relative to the baseline, which corresponds to a total-to-total isentropic efficiency increase of about 4 percentage points. 16. A Code for the Preliminary Design of Cooled Supercritical CO2 Turbines and Application to the Allam Cycle

Abstract

This paper documents a thermo-fluid-dynamic mean-line model for the preliminary design of multistage axial turbines with blade cooling applicable to supercritical CO2 turbines. Given the working fluid and coolant inlet thermodynamic conditions, blade geometry, number of stages and load criterion, the model computes the stage-by-stage design along with the cooling requirement and ultimately provides an estimate of turbine efficiency via a semi-empirical loss model. Different cooling modes are available and can be selected by the user (stand-alone or combination): convective cooling, film cooling, and thermal barrier coating. The model is applied to attain the preliminary aero-thermal design of the 600 MW cooled axial supercritical CO2 turbine of the Allam cycle. Results show that a load coefficient varying from 3 to 1 throughout the machine, and a reaction degree ranging from 0.1 to 0.5 lead to the maximum total-to-static turbine efficiency of about 85%. Consequently, as opposed to uncooled CO2 turbines, a repeated stage configuration is an unsuited design choice for cooled sCO2 machines. 17. Design optimization of a high-speed twin-stage compressor for next-gen aircraft environmental control system

Abstract

The environmental control system (ECS) is the largest auxiliary power consumer, i.e, around 75% of non-propulsive power, among the aircraft subsystems. The adoption of a novel ECS architecture, based on an electrically-driven vapor compression cycle system, can enable a twofold increase of coefficient of performance (COP), as compared to the conventional air cycle machine (ACM). The core of this technology is a high-speed, miniature centrifugal compressor, consisting of two impellers mounted in back-to-back configuration, and running on gas bearings operating with refrigerant. The fluid dynamic design optimization of the twin-stage compressor, to be installed in the vapor compression cycle test rig under realization at Delft University of Technology, is documented in this paper. First, the scaling analysis for centrifugal compressor is extended to provide guidelines for the design of twin-stage machines. Then, a multi-objective conceptual design optimization is performed by resorting to an in-house reduced-order model (ROM), coupled to a genetic algorithm. The fluid dynamic performance and the structural integrity of the optimal design are assessed by means of a hybrid framework, encompassing CFD and ROMs, and by FEA. First, the development and validation of a reduced-order model based on scaling principles is documented. Then, the validated compressor model is used to generate design maps for stages operating with arbitrary fluid molecules, and characterized by different size. The results show that compressors operating with fluids made by heavy and complex molecules provide lower efficiency over the entire design space, if compared to their simple-molecule counterparts. However, compressors for complex-molecule fluids require lower rotational speed, and generate lower axial thrust on bearings, thus making them particularly suitable for small-scale applications. Furthermore, a decreasing value of the size parameter has a detrimental effect on the stage efficiency, as a result of manufacturing constraints. The results computed by the compressor model suggest that the efficiency penalty is more sensitive to variations of clearance gap than to surface finishing. Lastly, the reduced-order model has been used to perform a design exercise, i.e., the multi-objective optimization of the first compressor stage of the heat pump test rig being realized at Delft University of Technology. The key characteristics of the optimal compressor design has been compared to those derived from the design maps, to corroborate their validity. 19. Flow deviation and critical choking in transonic turbine cascades operating with non-ideal compressible flows

Abstract

In this work we examine the flow deviation and its relationship to critical choking, i.e., choking of the meridional component of velocity, in transonic turbine cascades operating with non-ideal compressible flows. To this purpose, a generalized expression of the corrected flow per unit area as a function of both the thermodynamic state and the molecular complexity of the working fluid, the Mach number, and the amount of swirl is derived. The trends of the corrected flow with respect to these quantities are used to infer physical insights on the flow deviation and on the operability of transonic turbine cascades in off-design conditions. Moreover, reduced-order models for the estimation of the flow deviation and the preliminary assessment of the losses have been developed and validated against the results of CFD simulations performed on a representative transonic turbine stator. Results suggest that flows of dense organic vapors exhibit larger deviations than those pertaining to compounds made of simple molecules, e.g., air. Abstract

Despite the large impact of digital technology on the lives and future of all people on the planet, many people, especially from the Global South, are not included in the debates about the future of the digital society. This inequality is a systemic problem which has roots in the real world. We refer to this problem as “digital coloniality.” We argue that to achieve a more equitable and inclusive global digital society, active involvement of stakeholders from poor regions of the world as co-researchers, co-creators, and co-designers of technology is required. We briefly discuss a few collaborative, community-oriented technology development projects as examples of transdisciplinary knowledge production and action research for a more inclusive digital society.

Abstract

L’épidémiologie de l’hypertension pulmonaire associée à une maladie pulmonaire et/ou une hypoxie (HTP-MPC), incluant l’HTP associée à une pneumopathie interstitielle diffuse (HTP-PID), est peu connue en France. Cette étude longitudinale rétrospective a pour objectif de caractériser la prévalence et l’incidence de ces deux situations à partir de la base de données des demandes de remboursement des hôpitaux français (Programme de médicalisation des systèmes d’information, PMSI).

Abstract

Background: Delirium is thought to be common across various settings of care; however, still little research has been conducted in rehabilitation. Aim: We investigated the prevalence of delirium, its features and motor subtypes in older patients admitted to rehabilitation facilities during the three editions of the “Delirium Day project”. Methods: We conducted a cross-sectional study in which 1237 older patients (age ≥ 65 years old) admitted to 50 Italian rehabilitation wards during the three editions of the “Delirium Day project” (2015 to 2017) were included. Delirium was evaluated through the 4AT and its motor subtype with the Delirium Motor Subtype Scale. Results: Delirium was detected in 226 patients (18%), and the most recurrent motor subtype was mixed (37%), followed by hypoactive (26%), hyperactive (21%) and non-motor one (16%). In a multivariate Poisson regression model with robust variance, factors associated with delirium were: disability in basic (PR 1.48, 95%CI: 1.17–1.9, p value 0.001) and instrumental activities of daily living (PR 1.58, 95%CI: 1.08–2.32, p value 0.018), dementia (PR 2.10, 95%CI: 1.62–2.73, p value < 0.0001), typical antipsychotics (PR 1.47, 95%CI: 1.10–1.95, p value 0.008), antidepressants other than selective serotonin reuptake inhibitors (PR 1.3, 95%CI: 1.02–1.66, p value 0.035), and physical restraints (PR 2.37, 95%CI: 1.68–3.36, p value < 0.0001). Conclusion: This multicenter study reports that 2 out 10 patients admitted to rehabilitations had delirium on the index day. Mixed delirium was the most prevalent subtype. Delirium was associated with unmodifiable (dementia, disability) and modifiable (physical restraints, medications) factors. Identification of these factors should prompt specific interventions aimed to prevent or mitigate delirium.

Abstract

Introduction Delirium and sarcopenia are common, although underdiagnosed, geriatric syndromes. Several pathological mechanisms can link delirium and low skeletal muscle mass, but few studies have investigated their association. We aimed to investigate (1) the association between delirium and low skeletal muscle mass and (2) the possible role of calf circumference mass in finding cases with delirium. Methods The analyses were conducted employing the cross-sectional “Delirium Day” initiative, on patient 65 years and older admitted to acute hospital medical wards, emergency departments, rehabilitation wards, nursing homes and hospices in Italy in 2017. Delirium was diagnosed as a 4 + score at the 4-AT scale. Low skeletal muscle mass was operationally defined as calf circumference ≤ 34 cm in males and ≤ 33 cm in females. Logistic regression models were used to investigate the association between low skeletal muscle mass and delirium. The discriminative ability of calf circumference was evaluated using non-parametric ROC analyses. Results A sample of 1675 patients was analyzed. In total, 73.6% of participants had low skeletal muscle mass and 24.1% exhibited delirium. Low skeletal muscle mass and delirium showed an independent association (OR: 1.50; 95% CI 1.09–2.08). In the subsample of patients without a diagnosis of dementia, the inclusion of calf circumference in a model based on age and sex significantly improved its discriminative accuracy [area under the curve (AUC) 0.69 vs 0.57, p Discussion and conclusion Low muscle mass is independently associated with delirium. In patients without a previous diagnosis of dementia, calf circumference may help to better identify those who develop delirium.

Abstract

Aim: Obstructive sleep apnea (OSA) is a growing health problem affecting nearly 1 billion people worldwide. The landmark feature of OSA is chronic intermittent hypoxia (CIH), accounting for multiple organ damage, including heart disease. CIH profoundly alters both visceral white adipose tissue (WAT) and heart structure and function, but little is known regarding inter-organ interaction in the context of CIH. We recently showed that visceral WAT senescence drives myocardial alterations in aged mice without CIH. Here, we aimed at investigating whether CIH induces a premature visceral WAT senescent phenotype, triggering subsequent cardiac remodeling., Methods: In a first experiment, 10-week-old C57bl6J male mice (n = 10/group) were exposed to 14 days of CIH (8 h daily, 5%-21% cyclic inspired oxygen fraction, 60 s per cycle). In a second series, mice were submitted to either epididymal WAT surgical lipectomy or sham surgery before CIH exposure. Finally, we used p53 deficient mice or Wild-type (WT) littermates, also exposed to the same CIH protocol. Epididymal WAT was assessed for fibrosis, DNA damages, oxidative stress, markers of senescence (p16, p21, and p53), and inflammation by RT-qPCR and histology, and myocardium was assessed for fibrosis and cardiomyocyte hypertrophy., Results: CIH-induced epididymal WAT remodeling characterized by increased fibrosis, oxidative stress, DNA damage response, inflammation, and increased expression of senescent markers. CIH-induced epididymal WAT remodeling was associated with subtle and early myocardial interstitial fibrosis. Both epididymal WAT surgical lipectomy and p53 deletion prevented CIH-induced myocardial fibrosis., Conclusion: Short-term exposure to CIH induces epididymal WAT senescent remodeling and cardiac interstitial fibrosis, the latter being prevented by lipectomy. Abstract

Isolated attosecond pulse (IAP) generation usually involves the use of short-medium gas cells operated at high pressures. In contrast, long-medium schemes at low pressures are commonly perceived as inherently unsuitable for IAP generation due to the nonlinear phenomena that challenge favourable phase-matching conditions. Here we provide clear experimental evidence on the generation of isolated extreme-ultraviolet attosecond pulses in a semi-infinite gas cell, demonstrating the use of extended-medium geometries for effective production of IAPs. To gain a deeper understanding we develop a simulation method for high-order harmonic generation (HHG), which combines nonlinear propagation with macroscopic HHG solving the 3D time-dependent Schrödinger equation at the single-atom level. Our simulations reveal that the nonlinear spatio-temporal reshaping of the driving field, observed in the experiment as a bright plasma channel, acts as a self-regulating mechanism boosting the phase-matching conditions for the generation of IAPs., (© 2024. 27. Health and Economic Impact of Different Long-Term Oxygen Therapeutic Strategies in Patients with Chronic Respiratory Failure: A French Nationwide Health Claims Database (SNDS) Study.

Abstract

Introduction: Long-term oxygen therapy (LTOT) is reported to improve survival in patients with chronic respiratory failure. We aimed to describe effectiveness, burden, and cost of illness of patients treated with portable oxygen concentrators (POC) compared to other LTOT options., Methods: This retrospective comparative analysis included adult patients with chronic respiratory insufficiency and failure (CRF) upon a first delivery of LTOT between 2014 and 2019 and followed until December 2020, based on the French national healthcare database SNDS. Patients using POC, alone or in combination, were compared with patients using stationary concentrators alone (aSC), or compressed tanks (CTC) or liquid oxygen (LO2), matched on the basis of age, gender, comorbidities, and stationary concentrator use., Results: Among 244,719 LTOT patients (mean age 75 ± 12, 48% women) included, 38% used aSC, 46% mobile oxygen in the form of LO2 (29%) and POC (18%), whereas 9% used CTC. The risk of death over the 72-month follow-up was estimated to be 13%, 15%, and 12% lower for patients in the POC group compared to aSC, CTC, and LO2, respectively. In the POC group yearly mean total costs per patient were 5% higher and 4% lower compared to aSC and CTC groups, respectively, and comparable in the LO2 group. The incremental cost-effectiveness ratio (ICER) of POC was €8895, €6288, and €13,152 per year of life gained compared to aSC, CTC, and LO2, respectively., Conclusion: Within the POC group, we detected an association between higher mobility (POCs autonomy higher than 5 h), improved survival, lower costs, and ICER - €6 238, compared to lower mobility POCs users., (© 2024. Abstract

We report on the complete temporal characterization of ultrashort pulses, generated by resonant dispersive wave emission in gas-filled hollow-capillary fibers, with energy in the microjoule range and continuously tunable from the deep-ultraviolet to the ultraviolet. Temporal characterization of such ultrabroad pulses, particularly challenging in this spectral region, was performed using an all-in-vacuum setup for self-diffraction frequency resolved optical gating (SD-FROG). Sub-3-fs pulses were measured, tunable from 250 nm to 350 nm, with a minimum pulse duration of 2.4 ± 0.1 fs.

31. Osteopontin promotes age-related adipose tissue remodeling through senescence-associated macrophage dysfunction.

Abstract

Adipose tissue macrophages (ATMs) play an important role in obesity and inflammation, and they accumulate in adipose tissue (AT) with aging. Furthermore, increased ATM senescence has been shown in obesity-related AT remodeling and dysfunction. However, ATM senescence and its role are unclear in age-related AT dysfunction. Here, we show that ATMs (a) acquire a senescence-like phenotype during chronological aging; (b) display a global decline of basic macrophage functions such as efferocytosis, an essential process to preserve AT homeostasis by clearing dysfunctional or apoptotic cells; and (c) promote AT remodeling and dysfunction. Importantly, we uncover a major role for the age-associated accumulation of osteopontin (OPN) in these processes in visceral AT. Consistently, loss or pharmacologic inhibition of OPN and bone marrow transplantation of OPN-/- mice attenuate the ATM senescence-like phenotype, preserve efferocytosis, and finally restore healthy AT homeostasis in the context of aging. Collectively, our findings implicate pharmacologic OPN inhibition as a viable treatment modality to counter ATM senescence-mediated AT remodeling and dysfunction during aging.

33. NanoModeler CG: A Tool for Modeling and Engineering Functional Nanoparticles at a Coarse-Grained Resolution.

Abstract

Functionalized metal nanoparticles (NPs) are macromolecular assemblies with a tunable physicochemical profile that makes them interesting for biotechnology, materials science, and energy conversion. In this regard, molecular simulations offer a way to scrutinize the structural and dynamical features of monolayer-protected NPs and their interactions with relevant matrices. Previously, we developed NanoModeler, a webserver that automates the preparation of functionalized gold NPs for atomistic molecular dynamics (MD) simulations. Here, we present NanoModeler CG (www.nanomodeler.it), a new release of NanoModeler that now also allows the building and parametrizing of monolayer-protected metal NPs at a coarse-grained (CG) resolution. This new version extends our original methodology to NPs of eight different core shapes, conformed by up to 800,000 beads and coated by eight different monolayer morphologies. The resulting topologies are compatible with the Martini force field but are easily extendable to any other set of parameters parsed by the user. Finally, we demonstrate NanoModeler CG's capabilities by reproducing experimental structural features of alkylthiolated NPs and rationalizing the brush-to-mushroom phase transition of PEGylated anionic NPs. By automating the construction and parametrization of functionalized NPs, the NanoModeler series offers a standardized way to computationally model monolayer-protected nanosized systems.

34. Application of Phage Therapy in a Case of a Chronic Hip-Prosthetic Joint Infection due to Pseudomonas aeruginosa : An Italian Real-Life Experience and In Vitro Analysis.

Abstract

Background: Prosthetic joint infection (PJI) caused by Pseudomonas aeruginosa represents a severe complication in orthopedic surgery. We report the case of a patient with chronic PJI from P. aeruginosa successfully treated with personalized phage therapy (PT) in combination with meropenem., Methods: A 62-year-old woman was affected by a chronic right hip prosthesis infection caused by P. aeruginosa since 2016 . The patient was treated with phage Pa53 (I day 10 mL q8h, then 5 mL q8h via joint drainage for 2 weeks) in association with meropenem (2gr q12h iv) after a surgical procedure. A 2-year clinical follow up was performed. An in vitro bactericidal assay of the phage alone and in combination with meropenem against a 24-hour-old biofilm of bacterial isolate was also carried out., Results: No severe adverse events were observed during PT. Two years after suspension, there were no clinical signs of infection relapse, and a marked leukocyte scan showed no pathological uptake areas. In vitro studies showed that the minimum biofilm eradicating concentration of meropenem was 8 µg/mL. No biofilm eradication was observed at 24 hours incubation with phages alone (10 8  plaque-forming units [PFU]/mL). However, the addition of meropenem at suberadicating concentration (1 µg/mL) to phages at lower titer (10 3  PFU/mL) resulted in a synergistic eradication after 24 hours of incubation., Conclusions: Personalized PT, in combination with meropenem, was found to be safe and effective in eradicating P. aeruginosa infection. These data encourage the development of personalized clinical studies aimed at evaluating the efficacy of PT as an adjunct to antibiotic therapy for chronic persistent infections., Competing Interests: Potential conflicts of interest. Abstract

Obesity triggers skeletal muscle physio-pathological alterations. However, the crosstalk between adipose tissue and myogenic cells remains poorly understood during obesity. We identified NID-1 among the adipose tissue secreted factors impairing myogenic potential of human myoblasts and murine muscle stem cells in vitro. Mice under High Fat Diet (HFD) displayed increased NID-1 expression in the skeletal muscle endomysium associated with intramuscular fat adipose tissue expansion and compromised muscle stem cell function. We show that NID-1 is highly secreted by skeletal muscle fibro-adipogenic/mesenchymal progenitors (FAPs) during obesity. We demonstrate that increased muscle NID-1 impairs muscle stem cells proliferation and primes the fibrogenic differentiation of FAPs, giving rise to an excessive deposition of extracellular matrix. Finally, we propose a model in which obesity leads to skeletal muscle extracellular matrix remodeling by FAPs, mediating the alteration of myogenic function by adipose tissue and highlighting the key role of NID-1 in the crosstalk between adipose tissue and skeletal muscle., Competing Interests: Declaration of Interests The authors declare no competing interests., (Copyright © 2022. 36. The Medical Home Initiative in Italy: an Analysis of Changes in Healthcare Utilization.

Abstract

Background: Seventeen medical homes (MHs) were established in the Local Health Authority (LHA) of Parma (about 450,000 residents), Emilia Romagna, Italy, between 2011 and 2016., Objective: To estimate the effects of MH implementation on healthcare utilization., Design: We conducted a longitudinal cohort study (01/2011-12/2017) using the Parma LHA administrative healthcare database., Participants: Residents for ≥1 year and older than 14 years of age with a documented primary care physician (PCP) in Parma LHA., Intervention: MH exposure status was classified for each resident as either receiving care from a PCP that (1) eventually practices in an MH (pre-MH), (2) is currently in an MH (post-MH), or (3) does not join an MH (non-MH)., Main Outcome Measures: Risks of ordinary inpatient hospital admissions, day hospital admissions, admissions for ambulatory care sensitive conditions (ACSCs), all-cause emergency department (ED) visits, and deferrable ED visits were compared using Cox proportional hazards regression and risks of all-cause 30- and 90-day readmissions for congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD) were compared using logistic regression., Key Results: Prior to MH implementation, the risk of all-cause ED visits for pre-MH residents was 0.93 (95% CI: 0.92-0.94) that of non-MH residents. After MH implementation, the relative risk for post-MH versus non-MH was 0.86 (95% CI: 0.85-0.87) and, over time, post-MH versus pre-MH was 0.93 (95% CI: 0.92-0.94). Hospitalization risks were generally lower among the pre-MH and post-MH, compared to non-MH. However, hospitalizations and HF or COPD readmissions were not generally lower post-MH compared to pre-MH., Conclusions: This MH initiative was associated with a 7% reduction in risk of ED visits. More research is necessary to understand if ED visit risk will continue to improve and how other aspects of healthcare utilization might change as more MHs open and the length of exposure to MHs increases., (© 2021.
