1. Effects of salinity stress on the seed germination and seedling characters of carrots (Daucus carota L.)

Abstract

Carrot is an important root vegetable and salinity has strong effect on its seed germination and seedling characters. An experiment was conducted at the Postgraduate Laboratory of the Department of Horticulture, Bangladesh Agricultural University, Mymensingh during the period from July to August, 2023 to study the effect of salinity stress on the seed germination and seedling characters of four carrot varieties. The two-factor experiment consisted of four carrot varieties viz. Orange king, Orange star, Sangal and Red lady, and five salinity levels viz. control (tap water), 25 mM, 50 mM, 75 mM and 100 mM NaCl. The experiment was laid out in completely randomized design with three replications. Results revealed that salinity and carrot varieties significantly influenced all the parameters studied. The highest seed germination (44.44%) was obtained from Orange king while the lowest (32.89%) was obtained from Red lady. The highest seed germination (41.11%) was obtained from 100 mM NaCl solution and the lowest (34.44%) was found from 75 mM NaCl. Among the treatment combinations, the highest seed germination (60.00%) was obtained from Orange king along with 50 mM NaCl. The maximum radicle (15.46 mm) and plumule length (12.36 mm), leaf size (5.50 mm), number of leaves (2.73) and seedling height (27.82 mm) was obtained from Orange king. Maximum plumule length (13.27 mm), leaf size (5.98 mm) and seedling height (23.27mm) was obtained from control treatment and maximum number of leaves (2.08) from 25 mM NaCl solution. Among the treatment combinations, highest radicle length (18.27 mm), plumule length (17.40 mm), leaf size (7.60 mm), seedling height (35.67 mm) was obtained from Orange king along with 25 mM NaCl solution. Therefore, Orange king along with 50 mM NaCl solution and 25 mM NaCl solution was found to be better in respect of seed germination and seedling characters of carrots compared to other treatments respectively. Abstract

Abstract Myotonic dystrophy type 2 (DM2) is a tetranucleotide CCTG repeat expansion disease associated with an increased prevalence of autoimmunity. Here, we identified an elevated type I interferon (IFN) signature in peripheral blood mononuclear cells and primary fibroblasts of DM2 patients as a trigger of chronic immune stimulation. Although RNA-repeat accumulation was prevalent in the cytosol of DM2-patient fibroblasts, type-I IFN release did not depend on innate RNA immune sensors but rather the DNA sensor cGAS and the prevalence of mitochondrial DNA (mtDNA) in the cytoplasm. Sublethal mtDNA release was promoted by a chronic activation of the ATF6 branch of the unfolded protein response (UPR) in reaction to RNA-repeat accumulation and non-AUG translated tetrapeptide expansion proteins. ATF6-dependent mtDNA release and resulting cGAS/STING activation could also be recapitulated in human THP-1 monocytes exposed to chronic endoplasmic reticulum (ER) stress. Altogether, our study demonstrates a novel mechanism by which large repeat expansions cause chronic endoplasmic reticulum stress and associated mtDNA leakage. This mtDNA is, in turn, sensed by the cGAS/STING pathway and induces a type-I IFN response predisposing to autoimmunity. Elucidating this pathway reveals new potential therapeutic targets for autoimmune disorders associated with repeat expansion diseases.

Abstract

Weaning patients from mechanical ventilation (MV) is a critical and resource intensive process in the Intensive Care Unit (ICU) that impacts patient outcomes and healthcare expenses. Weaning methods vary widely among providers. Prolonged MV is associated with adverse events and higher healthcare expenses. Predicting weaning readiness is a non-trivial process in which the positive end-expiratory pressure (PEEP), a crucial component of MV, has potential to be indicative but has not yet been used as the target. We aimed to predict successful weaning from mechanical ventilation by targeting changes in the PEEP-level using a supervised machine learning model. This retrospective study included 12,153 mechanically ventilated patients from Medical Information Mart for Intensive Care (MIMIC-IV) and eICU collaborative research database (eICU-CRD). Two machine learning models (Extreme Gradient Boosting and Logistic Regression) were developed using a continuous PEEP reduction as target. The data is splitted into 80% as training set and 20% as test set. The model's predictive performance was reported using 95% confidence interval (CI), based on evaluation metrics such as area under the receiver operating characteristic (AUROC), area under the precision-recall curve (AUPRC), F1-Score, Recall, positive predictive value (PPV), and negative predictive value (NPV). The model's descriptive performance was reported as the variable ranking using SHAP (SHapley Additive exPlanations) algorithm. The best model achieved an AUROC of 0.84 (95% CI 0.83-0.85) and an AUPRC of 0.69 (95% CI 0.67-0.70) in predicting successful weaning based on the PEEP reduction. The model demonstrated a Recall of 0.85 (95% CI 0.84-0.86), F1-score of 0.86 (95% CI 0.85-0.87), PPV of 0.87 (95% CI 0.86-0.88), and NPV of 0.64 (95% CI 0.63-0.66). Most of the variables that SHAP algorithm ranked to be important correspond with clinical intuition, such as duration of MV, oxygen saturation (SaO2), PEEP, and Glasgow Coma Score (GCS) components. This study demonstrates the potential application of machine learning in predicting successful weaning from MV based on continuous PEEP reduction. The model's high PPV and moderate NPV suggest that it could be a useful tool to assist clinicians in making decisions regarding ventilator management.

Abstract

Occlusion of the left coronary circumflex artery (LCX) during surgical procedures of the mitral valve is an infrequent but potentially life-threatening complication (1–3). Due to its close anatomical relationship to the posterior mitral valve annulus, there is a relevant risk of causing a stenosis or an occlusion of the left circumflex artery, especially by surgical annular sutures. The perioperative clinical course is heterogeneous, ranging from—initially—asymptomatic or solely electrocardiographic abnormalities to cardiogenic shock. Both severely impaired ventricular contractility or malignant arrhythmia may potentially lead to a weaning failure from cardiopulmonary bypass (CPB) and eventually result in chronic heart failure with persistently reduced ejection fraction. Possible therapeutic strategies include the immediate reopening of causal sutures, aortocoronary bypass grafting or percutaneous coronary intervention (PCI), yet PCI seems to be the preferred method at present.

Abstract

Drought is one of the major environmental challenges constraining the production of agricultural crops, including carrot. Seed germination is the initial and most critical stage of crop establishment, and it is very sensitive to drought stress because water scarcity affects the enzymatic solubilization of stored metabolites in seeds that provide energy for the growth of germinating embryo. Few studies evaluating the effect of drought stress on carrot seed germination of more than a few cultivars grown under stress have been published. Therefore, the present study was designed to define the appropriate osmotic potential for evaluating drought tolerance of carrot, evaluate the response of diverse carrot germplasm to drought stress during seed germination to identify drought-tolerant accessions that may be used by plant breeders, and evaluate the relation between amylase activity and germination rate of drought-tolerant and drought-sensitive accessions. To identify an appropriate screening osmotic potential, two commercial cultivars and two United States Department of Agriculture inbreeds were evaluated at six osmotic potentials (00, −0.30, −0.51, −0.58, −0.80, and −1.05 MPa); −0.58 MPa was identified as the optimal osmotic potential for screening the drought tolerance of carrot seed. Cultivated and wild carrot plant introductions (PIs) (n = 200 and n = 50, respectively) from the National Plant Germplasm System were evaluated for drought tolerance under nonstress and simulated drought stress conditions (00 MPa and −0.58 MPa, respectively) by calculating the absolute decrease (AD) in percent germination, inhibition index (II), relative drought tolerance (RDT), and drought tolerance index (DTI). All measurements of drought tolerance identified significant differences among accessions; the AD in seed germination ranged from 0.0% to 69.3%, II ranged from 0.0% to 80.2%, RDT ranged from 0.2 to 1.0, and DTI ranged from 0.13 to 1.47. All wild carrot accessions displayed low levels of drought tolerance, but PI 652387 and PI 177381 (both from Turkey) and PI 274297 (Pakistan) were most drought-sensitive, whereas cultivated accessions PI 643114 (United States), PI 652208 (China), and PI 502347 (Uzbekistan) were most drought-tolerant. Tolerant accessions displayed much higher α-amylase activity under nonstress conditions than sensitive accessions, and α-amylase activity of tolerant accessions was also reduced less with seed germination under increasing osmotic potential (range, 0.0 to −1.05 MPa) than sensitive accessions over 24, 48, and 72 hours of seed germination. This is the first evaluation of drought stress tolerance during seed germination and the enzymatic response of diverse carrot germplasms under simulated drought stress.

6. A multifaceted educational intervention improved anti-infectious measures but had no effect on mortality in patients with severe sepsis

Abstract Sepsis is a major reason for preventable hospital deaths. A cluster-randomized controlled trial on an educational intervention did not show improvements of sepsis management or outcome. We now aimed to test an improved implementation strategy in a second intervention phase in which new intervention hospitals (former controls) received a multifaceted educational intervention, while controls (former intervention hospitals) only received feedback of quality indicators. Changes in outcomes from the first to the second intervention phase were compared between groups using hierarchical generalized linear models controlling for possible confounders. During the two phases, 19 control hospitals included 4050 patients with sepsis and 21 intervention hospitals included 2526 patients. 28-day mortality did not show significant changes between study phases in both groups. The proportion of patients receiving antimicrobial therapy within one hour increased in intervention hospitals, but not in control hospitals. Taking at least two sets of blood cultures increased significantly in both groups. During phase 2, intervention hospitals showed higher proportion of adequate initial antimicrobial therapy and de-escalation within 5 days. A survey among involved clinicians indicated lacking resources for quality improvement. Therefore, quality improvement programs should include all elements of sepsis guidelines and provide hospitals with sufficient resources for quality improvement. 7. Adverse effects of delayed antimicrobial treatment and surgical source control in adults with sepsis: results of a planned secondary analysis of a cluster-randomized controlled trial

Abstract

Abstract Background Timely antimicrobial treatment and source control are strongly recommended by sepsis guidelines, however, their impact on clinical outcomes is uncertain. Methods We performed a planned secondary analysis of a cluster-randomized trial conducted from July 2011 to May 2015 including forty German hospitals. All adult patients with sepsis treated in the participating ICUs were included. Primary exposures were timing of antimicrobial therapy and delay of surgical source control during the first 48 h after sepsis onset. Primary endpoint was 28-day mortality. Mixed models were used to investigate the effects of timing while adjusting for confounders. The linearity of the effect was investigated by fractional polynomials and by categorizing of timing. Results Analyses were based on 4792 patients receiving antimicrobial treatment and 1595 patients undergoing surgical source control. Fractional polynomial analysis identified a linear effect of timing of antimicrobials on 28-day mortality, which increased by 0.42% per hour delay (OR with 95% CI 1.019 [1.01, 1.028], p ≤ 0.001). This effect was significant in patients with and without shock (OR = 1.018 [1.008, 1.029] and 1.026 [1.01, 1.043], respectively). Using a categorized timing variable, there were no significant differences comparing treatment within 1 h versus 1–3 h, or 1 h versus 3–6 h. Delays of more than 6 h significantly increased mortality (OR = 1.41 [1.17, 1.69]). Delay in antimicrobials also increased risk of progression from severe sepsis to septic shock (OR per hour: 1.051 [1.022, 1.081], p ≤ 0.001). Time to surgical source control was significantly associated with decreased odds of successful source control (OR = 0.982 [0.971, 0.994], p = 0.003) and increased odds of death (OR = 1.011 [1.001, 1.021]; p = 0.03) in unadjusted analysis, but not when adjusted for confounders (OR = 0.991 [0.978, 1.005] and OR = 1.008 [0.997, 1.02], respectively). Only, among patients with septic shock delay of source control was significantly related to risk-of death (adjusted OR = 1.013 [1.001, 1.026], p = 0.04). Conclusions Our findings suggest that management of sepsis is time critical both for antimicrobial therapy and source control. Also patients, who are not yet in septic shock, profit from early anti-infective treatment since it can prevent further deterioration. Trial registration ClinicalTrials.gov ( NCT01187134 ). Abstract

Introduction Previous studies demonstrated that the implementation of the Kidney Disease Improving Global Outcomes (KDIGO) guideline-based bundle, consisting of different supportive measures in patients at high risk for acute kidney injury (AKI), might reduce rate and severity of AKI after surgery. However, the effects of the care bundle in broader population of patients undergoing surgery require confirmation.Methods and analysis The BigpAK-2 trial is an international, randomised, controlled, multicentre trial. The trial aims to enrol 1302 patients undergoing major surgery who are subsequently admitted to the intensive care or high dependency unit and are at high-risk for postoperative AKI as identified by urinary biomarkers (tissue inhibitor of metalloproteinases 2*insulin like growth factor binding protein 7 (TIMP-2)*IGFBP7)). Eligible patients will be randomised to receive either standard of care (control) or a KDIGO-based AKI care bundle (intervention). The primary endpoint is the incidence of moderate or severe AKI (stage 2 or 3) within 72 hours after surgery, according to the KDIGO 2012 criteria. Secondary endpoints include adherence to the KDIGO care bundle, occurrence and severity of any stage of AKI, change in biomarker values during 12 hours after initial measurement of (TIMP-2)*(IGFBP7), number of free days of mechanical ventilation and vasopressors, need for renal replacement therapy (RRT), duration of RRT, renal recovery, 30-day and 60-day mortality, intensive care unit length-of-stay and hospital length-of-stay and major adverse kidney events. An add-on study will investigate blood and urine samples from recruited patients for immunological functions and kidney damage.Ethics and dissemination The BigpAK-2 trial was approved by the Ethics Committee of the Medical Faculty of the University of Münster and subsequently by the corresponding Ethics Committee of the participating sites. A study amendment was approved subsequently. In the UK, the trial was adopted as an NIHR portfolio study. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and will guide patient care and further research.Trial registration number NCT04647396.

Abstract

Immunization by vaccination is one of the most important tools for fighting the COVID-19 pandemic. Yet in many countries, immunization campaigns have been hampered by vaccine hesitancy within the population. Building on the idea that vaccination decisions are embedded in the broader societal context, we study the role of generalized trust-the belief that most people can generally be trusted-in vaccine acceptance. Immunization campaigns face an inherent collective action problem: As all individuals benefit collectively from high immunization rates regardless of individual contribution, especially those with a low risk of severe COVID infection have an incentive to decide against the (perceived) costs and risks of vaccination. We argue that generalized trust may help to overcome this problem by encouraging the belief that cooperation for the common good is achievable and that those who cooperate are unlikely to be exploited by others. We further argue that the positive effect of generalized trust on vaccination decisions is weaker among individuals who are at higher risk of severe outcomes from the disease, as the collective action problem is less pronounced in this group. To test our predictions, we used data from the SOEP-CoV survey, which queried a representative probability sample of Germany's population between January and February 2021 on topics connected to the pandemic. Using multiple logistic regression models, and in line with expectations, we found a positive and robust link between generalized trust and the willingness to accept vaccination as soon as offered. However, overall, our examination of heterogeneous effects does not unequivocally support the idea that the role of generalized trust varies according to individual COVID risk.

Abstract

Comparisons are crucial in shaping evaluations of one's own position. Following this notion, we investigated the role of historical, financial, partner, occupational, and regional pay referents in predicting the just gross hourly earnings in a representative sample of German workers. Looking at this broad range of pay referents, we find that higher reference earnings were generally associated with higher just earnings. In particular, controlling for actual gross hourly earnings, higher occupational, regional, and partners' hourly earnings were associated with higher just gross hourly earnings. One's own pay history, in form of average hourly earnings over the past 10 years, did not shape just earnings. Financial needs on the household level, however, mapped on to higher just earnings and this association was particularly pronounced among men. Overall, occupational and financial referents showed the strongest association with just earnings followed by partner earnings. These findings underscore that, while referents closely related to one's job (e.g. occupational referents) are important, referents that extend beyond the sphere of work (e.g. regional, financial, and partner referents) are also relevant in shaping ideas of the just reward. 21. Proenkephalin A and bioactive adrenomedullin are useful for risk prognostication in cardiac surgery

Abstract

IntroductionVarious clinical scores have been developed to predict organ dysfunction and mortality in patients undergoing cardiac surgery, but outcome prediction may be inaccurate for some patient groups. Proenkephalin A (penKid) and bioactive adrenomedullin (bio-ADM) have emerged as promising biomarkers correlating with shock and organ dysfunction. This imposes the question of whether they can be used as prognostic biomarkers for risk stratification in the perioperative setting of cardiac surgery.MethodsPatients undergoing cardiac surgery were prospectively enrolled in this observational study. PenKid and bio-ADM plasma levels, as well as markers evaluating inflammation and organ dysfunction, were measured at five perioperative time points from before the induction of anesthesia to up to 48 h postoperatively. Clinical data regarding organ dysfunction and patient outcomes were recorded during the intensive care unit (ICU)-stay with a special focus on acute kidney injury (AKI).ResultsIn 136 patients undergoing cardiac surgery, the bio-ADM levels increased and the penKid levels decreased significantly over time. PenKid was associated with chronic kidney disease (CKD), the incidence of AKI, and renal replacement therapy (RRT). Bio-ADM was associated with lactate and the need for vasopressors. PenKid was useful to predict an ICU-length of stay (LOS)>1 day and added prognostic value to the European System for Cardiac Operative Risk Evaluation Score (EuroSCORE) II when measured after the end of cardiopulmonary bypass and 24 h after cardiac surgery. For bio-ADM, the same was true when measured 24 h after surgery. PenKid also added prognostic value to the EuroSCORE II for the combined outcome “ICU length of stay >1 day and in-hospital mortality.”ConclusionThe combination of preoperative EuroSCORE II and intraoperative measurement of penKid may be more useful to predict a prolonged ICU LOS and increased mortality than EuroSCORE II alone. Bio-ADM correlates with markers of shock. More research is encouraged for early risk stratification and validation of penKid and bio-ADM as a tool involved in clinical decisions, which may enable the early initiation of organ protective strategies.

Betrachtet man die Gegenwart der Architektur im Bezug auf ihren Umgang mit der Klimakrise, so scheint auch die Architektur in einer Krise zu stecken: Gebäude, die zur Lösung der Klimakrise beitragen sollen, verstecken sich hinter gut gemeinten Vorsätzen, wohlwollenden Versprechungen und oberflächlichen Lösungsansätzen. Während sie damit die Situation weiter verschlechtern, werden sie dennoch zu immer höheren Preisen verkauft und täuschen Veränderung und Fortschritt vor, ohne tatsächlich dazu beizutragen.Um die schlimmsten Auswirkungen der Klimakrise zu verhindern, müssen sich die Art und Weise auf die wir Architektur schaffen dringend verändern. Angesichts dieses Umstandes und der ihm inhärenten Fragestellungen, werden in dieser Arbeit die Gegenwart der Architektur, sowie die ihr zugrundeliegenden historischen, gesellschaftlichen und ökonomischen Hintergründe analysiert, um anhand der Erkenntnisse potenzielle Lösungen für die Zukunft zu finden. Die enge Beziehung zwischen sozialen Themen und Problemen, zu Fragen des Klimas in der Architektur und folglich die Komplexität gesellschaftlicher, politischer und ökonomischer Systeme und ihrer Interdependenzen stehen dabei besonders im Fokus. Auf Basis einer Literaturanalyse werden drei Handlungsfelder – Boden, Raum und Ressourcen – erarbeitet und Zusammenhänge, Missstände und Potentiale identifiziert, um so relevante Handlungsmöglichkeiten hervorzubringen. Diese Analyse stellt die Basis eines Manifests dar, welches die gewonnenen Erkenntnisse mit Forderungen vereint um im zweiten Teil der Arbeit als Grundlage eines Entwurfskonzepts experimentell prototypisch auf seine Anwendbarkeit geprüft zu werden., Considering present architecture in terms of how it is dealing with the climate crisis, architecture too seems to be stuck in a crisis: buildings that are supposed to contribute to solving the climate crisis, are hiding behind well-intentioned resolutions, benevolent promises and superficial solutions. While making things worse, they are sold at ever rising prices, simulating change and progress without actually contributing to it.To prevent the worst effects of the climate crisis, the way we create architecture urgently needs to change. In view of this very circumstance and the questions inherent in it, this thesis analyzes the present of architecture and its underlying historical, social and economic background in order to find potential solutions for the future based on the findings. The close relationship between social questions, questions of climate in architecture and consequently the complexity of social, political and economic systems and their interdependencies are of particular interest to this thesis.On the basis of a literature analysis, three fields of action – ground, space and resources – as well as their connections, deficits and potentials are being identified in order to produce relevant options for action. This analysis lays the groundwork of a manifesto that combines the knowledge gained. In the second part of the work the manifesto is applied to an experimental, prototypical design.

Abstract

Background and objectives: A quantitative evaluation of the PK of meropenem, a broad-spectrum ��-lactam antibiotic, in plasma and interstitial space fluid (ISF) of subcutaneous adipose tissue of obese patients is lacking as of date. The objective of this study was the characterisation of meropenem population pharmacokinetics in plasma and ISF in obese and non-obese patients for identification of adequate dosing regimens via Monte-Carlo simulations. Methods: We obtained plasma and microdialysate concentrations after administration of meropenem 1000 mg to 15 obese and 15 non-obese surgery patients from a prospective clinical trial. After characterizing plasma- and microdialysis-derived ISF pharmacokinetics via population pharmacokinetic analysis, we simulated thrice-daily (TID) meropenem short-term (0.5 h), prolonged (3.0 h), and continuous infusions. Adequacy of therapy was assessed by the probability of pharmacokinetic/pharmacodynamic (PK/PD) target attainment (PTA) analysis based on time unbound concentrations exceeded minimum inhibitory concentrations (MIC) on treatment day 1 (%fT > MIC) and the sum of PTA weighted by relative frequency of MIC values for infections by pathogens commonly treated with meropenem. To avoid interstitial tissue fluid concentrations below MIC for the entire dosing interval during continuous infusions, a more conservative PK/PD index was selected (%fT > 4 �� MIC). Results: Adjusted body weight (ABW) and calculated creatinine clearance (CLCRCG_ABW) of all patients (body mass index [BMI] = 20.5-81.5 kg/m2) explained a considerable proportion of the between-patient pharmacokinetic variability (15.1-31.0% relative reduction). The ISF:plasma ratio of %fT > MIC was relatively similar for MIC ��� 2 mg/L but decreased for MIC = 8 mg/L over ABW = 60-120 kg (0.50-0.20). Steady-state concentrations were 2.68 times (95% confidence interval [CI] = 2.11-3.37) higher in plasma than in ISF, supporting PK/PD targets related to four times the MIC during continuous infusions to avoid suspected ISF concentrations constantly below the MIC. A 3000 mg/24 h continuous infusion was sufficient at MIC = 2 mg/L for patients with CLCRCG_ABW ��� 100 mL/min and ABW < 90 kg, whereas 2000 mg TID prolonged infusions were adequate for those with CLCRCG_ABW ��� 100 mL/min and ABW > 90 kg. For MIC = 2 mg/L and %fT> MIC = 95, PTA was adequate in patients over the entire investigated range of body mass and renal function using a 6000 mg continuous infusion. A prolonged infusion of meropenem 2000 mg TID was sufficient for MIC ��� 8 mg/L and all investigated ABW and CLCRCG_ABW when employing the PK/PD target %fT > MIC = 40. Short-term infusions of 1000 mg TID were sufficient for CLCRCG_ABW ��� 130 mL/min and distributions of MIC values for Escherichia coli, Citrobacter freundii, and Klebsiella pneumoniae but not for Pseudomonas aeruginosa. Conclusions: This analysis indicated a need for higher doses (��� 2000 mg) and prolonged infusions (��� 3 h) for obese and non-obese patients at MIC ��� 2 mg/L. Higher PTA was achieved with prolonged infusions in obese patients and with continuous infusions in non-obese patients.

29. [Safe position check of gastric tubes: an often underestimated topic to prevent possible severe complications]

Abstract

The insertion of a gastric tube in the operating room (OR) or in an intensive care unit (ICU) is a routine procedure. Although the tube is often inserted by nursing staff, the indications and selection of the procedure for position control remain the physician's responsibility. For immediate position control, the injection of air through the inserted tube with simultaneous auscultation of the epigastrium is probably the most simple and common clinical method. A typical "bubbling" is often regarded as a sufficiently reliable sign of correct gastric tube position. This procedure can be described as a routine standard both in the OR and the ICU; however, numerous examples from clinical practice and quality reviews show gastric tube mispositioning in esophageal, bronchial, pulmonary or even pleural positions in individual cases. Since auscultation findings are misinterpreted as tracheal, bronchial, or pleural secretions, mispositioning may remain undetected. In addition, adequate documentation of the procedure is lacking. In the worst case, hyperosmolar tube feeding occurs via the malpositioned tube into the lungs or pleural space causing subsequent severe pneumonia or pleuritis, which is often fatal for the patient outcome. In contrast to many other similar procedures in intensive care medicine, such as the installation of central venous catheters or endotracheal intubation, to our knowledge there is no uniform standard for forensic verification of the correct position after gastric tube positioning. The present work provides an overview of existing practices and the scientific background for confirmation of gastric tube positioning without ionizing radiation.Die Anlage einer Magensonde im OP oder auf einer Intensivstation (ITS) stellt eine alltäglich durchgeführte Prozedur dar. Obwohl die Sonde häufig durch Pflegekräfte eingebracht wird, verbleiben die Indikationsstellung und Auswahl des Verfahrens zur Lagekontrolle im ärztlichen Verantwortungsbereich. Zur unmittelbaren Lagekontrolle wird klinisch wohl am gebräuchlichsten die Injektion von Luft durch die gelegte Sonde bei gleichzeitiger Auskultation des Epigastriums durchgeführt. Ein typisches „Blubbern“ wird oft als hinreichend sicheres Zeichen für eine korrekte Lage der Sonde im Magen angesehen. Dieses Verfahren darf sowohl im OP als auch auf der ITS als praktizierter Standard bezeichnet werden. Zahlreiche Beispiele aus der klinischen und gutachterlichen Praxis zeigen allerdings immer wieder, dass es in Einzelfällen ösophageale, bronchiale, pulmonale oder sogar pleurale Fehllagen geben kann, bei denen der Auskultationsbefund durch tracheales, bronchiales oder pleurales Sekret irreführend fehlgedeutet wird. Gleichzeitig wird regelhaft nicht dokumentiert, wie und durch wen die Sondenlage kontrolliert wurde. Kommt es im schlimmsten Falle zu einer Applikation von hyperosmolarer Sondenkost über die fehlliegende Sonde, wird die nachfolgende schwere Pneumonie oder Pleuritis, abhängig von den Begleiterkrankungen des Patienten, oft nicht überlebt. Im Gegensatz zu vielen anderen vergleichbaren Verfahren der Intensivmedizin, wie beispielsweise der Anlage zentraler Venenkatheter oder der endotrachealen Intubation, besteht unseres Wissens kein einheitlicher Standard für die forensische Sicherung eines Nachweises der korrekten Lage nach Anlage von Magensonden. Die vorliegende Arbeit gibt eine Übersicht über bestehende Praktiken und den wissenschaftlichen Hintergrund zur nichtinvasiven Lagekontrolle von Magensonden ohne ionisierende Strahlung.

Abstract

BACKGROUND: High intraoperative PEEP with recruitment manoeuvres may improve perioperative outcomes. We re-examined this question by conducting a patient-level meta-analysis of three clinical trials in adult patients at increased risk for postoperative pulmonary complications who underwent non-cardiothoracic and non-neurological surgery. METHODS: The three trials enrolled patients at 128 hospitals in 24 countries from February 2011 to February 2018. All patients received volume-controlled ventilation with low tidal volume. Analyses were performed using one-stage, two-level, mixed modelling (site as a random effect; trial as a fixed effect). The primary outcome was a composite of postoperative pulmonary complications within the first week, analysed using mixed-effect logistic regression. Pre-specified subgroup analyses of nine patient characteristics and seven procedure and care-delivery characteristics were also performed. RESULTS: Complete datasets were available for 1913 participants ventilated with high PEEP and recruitment manoeuvres, compared with 1924 participants who received low PEEP. The primary outcome occurred in 562/1913 (29.4%) participants randomised to high PEEP, compared with 620/1924 (32.2%) participants randomised to low PEEP (unadjusted odds ratio [OR]=0.87; 95% confidence interval [95% CI], 0.75-1.01; P=0.06). Higher PEEP resulted in 87/1913 (4.5%) participants requiring interventions for desaturation, compared with 216/1924 (11.2%) participants randomised to low PEEP (OR=0.34; 95% CI, 0.26-0.45). Intraoperative hypotension was associated more frequently (784/1913 [41.0%]) with high PEEP, compared with low PEEP (579/1924 [30.1%]; OR=1.87; 95% CI, 1.60-2.17). CONCLUSIONS: High PEEP combined with recruitment manoeuvres during low tidal volume ventilation in patients undergoing major surgery did not reduce postoperative pulmonary complications. Abstract

In der Dissertation wird retrospektiv ein 103 Patienten umfassendes selektives Patientenkontingent der Universitätsklinik Köln in dem Zeitraum von 2006 bis 2019 mit einem mittleren Alter von 64,7 Jahren analysiert. Grundlegende neurochirurgische, onkologische bzw. therapeutische und prognostische Scores dienen als Grundlage dieser Arbeit. Das Ziel dieser Arbeit ist die Darstellung einer Korrelation klinischen Auftretens der Patienten, Allgemeinzustand, Schmerzsymptomatik und Stabilität der Wirbelsäule im Hinblick auf durchgeführte operative Therapien bei neoplastischen Grunderkrankungen, alle als Faktoren des „spinal neoplastic instability score“ (SINS), sowie den Klassifikationen nach Tokuhashi et al. und nach Tomita et al. Die Therapieempfehlung eine Stabilisierung durchzuführen, hängt nach den erarbeiteten Ergebnissen nicht allein von dem Gesamtscore der Klassifikation, sondern signifikant von dem Allgemeinzustand des Patienten (p<0,001), von dem Ausmaß der Wirbelköperfrakturen (p=0,038), sowie der präoperativen Schmerzsymptomatik (p=0,045) ab. Den SINS isoliert betrachtet, zeigt sich zudem ein signifikanter Unterschied der Therapie, zwischen Patienten mit einem Gesamtscore von 10 und 11 Punkten. Trotz für beide Gruppen gleicher Therapieempfehlung, sind im direkten Vergleich mehr Patienten in Gruppe 11 stabilisiert worden (p= 0,035). Diese Beobachtung ließ sich mit Hilfe eines systematischen Reviews mit zehn vergleichbaren Arbeiten für die dargestellten Faktoren weiter bestätigen. Dennoch ist die Indikation zur operativen Therapie weiterhin individuell an den Patienten anzupassen und jedes Mal neu zu prüfen und zu bewerten.

Abstract

Background and objective: Tigecycline, a broad-spectrum glycylcycline antibiotic, is approved for use at a fixed dose irrespective of body weight. However, its pharmacokinetics may be altered in obesity, which would impact on the antibiotic's effectiveness. The objective of this study was to investigate the plasma and subcutaneous tissue concentrations of tigecycline in obese patients compared with those in a non-obese control group. Methods: Fifteen obese patients (one class II and 14 class III) undergoing bariatric surgery and 15 non-obese patients undergoing intra-abdominal surgery (mainly tumour resection) received a single dose of 50 or 100 mg tigecycline as an intravenous short infusion. Tigecycline concentrations were measured up to 8 h after dosing in plasma (total concentration), in ultrafiltrate of plasma (free concentration), and in microdialysate from subcutaneous tissue, respectively. Results: In obese patients, total peak plasma concentration (1.31 ± 0.50 vs 2.27 ± 1.40 mg/L) and the area under the concentration-time curve from 0 to 8 h (AUC8h,plasma: 2.15 ± 0.42 vs 2.74 ± 0.73 h⋅mg/L), as normalized to a 100 mg dose, were significantly lower compared with those of non-obese patients. No significant differences were observed regarding the free plasma concentration, as determined by ultrafiltration, or the corresponding AUC8h (fAUC8h,plasma). Concentrations in interstitial fluid (ISF) of subcutaneous tissue were lower than the free plasma concentrations in both groups, and they were lower in obese compared to non-obese patients: the AUC8h in ISF (AUC8h,ISF) was 0.51 ± 0.22 h⋅mg/L in obese and 0.79 ± 0.23 h⋅mg/L in non-obese patients, resulting in a relative tissue drug exposure (AUC8h,ISF/fAUC8h,plasma) of 0.38 ± 0.19 and 0.63 ± 0.24, respectively. Conclusion: Following a single dose of tigecycline, concentrations in the ISF of subcutaneous adipose tissue are decreased in heavily obese subjects, calling for an increased loading dose. 37. Magnetism of Nano- to Micrometer-Sized Anisotropic Materials

Abstract

Magnetic materials have a large variety of everyday uses. From audio speakers to magnetic memory or logic devices, many branches or modern technology at least partially rely on magnets. Magnetometry of individual nano- to micron-sized magnetic particles using conventional, commercially available measurement techniques is complicated due to the inherently small generated signals and hence the necessity to investigate large ensembles of particles. This typically leads to random orientation of, and considerable interactions between the particles. Consequently, information on magnetic states or anisotropies is averaged out. Dynamic Cantilever Magnetometry (DCM) has been established as a sensitive method to investigate very small, individual magnets without the need for electrical contacts or large sample volumes. This thesis offers a contribution to the research on the magnetism of nano- to micrometersized magnetic particles using Dynamic Cantilever Magnetometry. We analyze the data using analytical models and micromagnetic simulations. We demonstrate the manipulation of the remanent state of spherical Janus Particles with a magnetic cap, using exchange bias as a means to provide an artificial, unidirectional anisotropy. We further analyze the anisotropy and blocking temperature of superparamagnetic maghemite mesocrystals. Both types of magnetic systems have previously not been investigated on the individual level. We also highlight the contribution to the investigation of phase transitions and domain wall magnetism in the multiferroic polar magnet GaV4Se8.

Abstract

Aims: The most suitable method for predicting the glomerular filtration rate (GFR) in obesity is currently debated. Therefore, multiple GFR/creatinine clearance prediction methods were applied to (morbidly) obese and nonobese patients ranging from moderate renal impairment to glomerular hyperfiltration and their predictions were rated based on observed fosfomycin pharmacokinetics, as this model drug is exclusively eliminated via glomerular filtration. Methods: The GFR/creatinine clearance predictions via the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), Modification of Diet in Renal Disease (MDRD; indexed and de-indexed by body surface area) and creatinine clearance via the Cockcroft–Gault formula (CLCRCG) using different body size descriptors were compared to the fosfomycin clearance (CLFOF) from 30 surgical patients (body mass index = 20.1–52.0 kg m−2), receiving 8000 mg as intravenous infusion. Results: The concordance between CLFOF and creatinine clearance predictions was highest for CLCRCG employing either ideal body weight or adjusted body weight (if body mass >1.3 ideal body weight; CLCRCG_ABW-Schwartz, concordance-correlation coefficient [95% confidence interval] = 0.474 [0.156; 0.703], CCC) and GFR predictions via the de-indexed MDRD equation (concordance-correlation coefficient = 0.452 [0.137; 0.685]). The proportion of predicted GFR values within ±30% of the observed CLFOF (P30 = 72.3–76.7%) was only marginally lower than the reported P30 in the original CKD-EPI and MDRD publications (P30 = 84.1–90.0%). Conclusion: This analysis represents a successful proof-of-concept for evaluating GFR/creatinine clearance prediction methods: Across all body mass index classes CLCRCG_ABW-Schwartz or the de-indexed MDRD were most suitable for predicting creatinine clearance/GFR also in (morbidly) obese, CKD stage

39. Methods for Determination of Individual PEEP for Intraoperative Mechanical Ventilation Using a Decremental PEEP Trial

Abstract

(1) Background: Individual PEEP settings (PEEPIND) may improve intraoperative oxygenation and optimize lung mechanics. However, there is uncertainty concerning the optimal procedure to determine PEEPIND. In this secondary analysis of a randomized controlled clinical trial, we compared different methods for PEEPIND determination. (2) Methods: Offline analysis of decremental PEEP trials was performed and PEEPIND was retrospectively determined according to five different methods (EIT-based: RVDI method, Global Inhomogeneity Index [GI], distribution of tidal ventilation [EIT VT]; global dynamic and quasi-static compliance). (3) Results: In the 45 obese and non-obese patients included, PEEPIND using the RVDI method (PEEPRVD) was 16.3 ± 4.5 cm H2O. Determination of PEEPIND using the GI and EIT VT resulted in a mean difference of −2.4 cm H2O (95%CI: −1.2;−3.6 cm H2O, p = 0.01) and −2.3 cm H2O (95% CI: −0.9;3.7 cm H2O, p = 0.01) to PEEPRVD, respectively. PEEPIND selection according to quasi-static compliance showed the highest agreement with PEEPRVD (p = 0.67), with deviations > 4 cm H2O in 3/42 patients. PEEPRVD and PEEPIND according to dynamic compliance also showed a high level of agreement, with deviations > 4 cm H2O in 5/42 patients (p = 0.57). (4) Conclusions: High agreement of PEEPIND determined by the RVDI method and compliance-based methods suggests that, for routine clinical practice, PEEP selection based on best quasi-static or dynamic compliance is favorable.

Abstract

Der deutsche Zivilprozess ist als ein Verfahren zwischen zwei Parteien zur Durchsetzung ihrer Einzelinteressen konzipiert. Demgegenüber führen die Musterfeststellungsklage und die Abhilfeklage zu einer Zunahme des kollektiven Rechtsschutzes. Beide Verfahrensarten sind in den weiterhin am Individualinteresse orientierten Zivilprozess zu integrieren, wodurch ein Potential für Friktionen mit dem tradierten Zivilprozess entsteht. Hier setzt die Arbeit an und analysiert die Integration dieser, vom materiellen Rechtsinhaber wegführenden, Klagearten in eine auf ein Zwei-Personen-Verhältnis angelegte Verfahrensordnung. Zu diesem Zweck wird die Rechtsnatur dieser prozessualen Instrumente herausgearbeitet und darauf aufbauend verschiedene Bereiche des Erkenntnisverfahrens unter dem Gesichtspunkt der Parteirollenverschiebung untersucht. Ferner werden die konsensuale und die streitige Verfahrensbeendigung sowie die hierbei auftretenden Auswirkungen einer Klage durch einen Rechtsfremden untersucht.

41. Non-migrants' and migrants' interethnic relationships: the third party role of cohabiting partners.

Abstract

Considering both non-migrant and migrant couples, this paper studies the effect of cohabiting life partners' attitudes, resources, and social network compositions on their spouse's interethnic friendships and acquaintances. Thus, partners are conceptualized as important "third parties" for interethnic relationship formation. Analysing representative German household panel data, I find that partner characteristics play an important role for a person's interethnic relationships. In particular, a strong positive connection between cohabiting life partners' interethnic relationships indicates that couples' social networks merge over time. Partner's preferences for interethnic contacts mainly exert an indirect influence that is mediated by the partner's own interethnic relationships and – to some extent – by a person's preferences. Abstract

Abstract Sepsis, marked by organ dysfunction, necessitates reliable biomarkers. Ribonuclease inhibitor 1 (RNH1), a ribonuclease (RNase) inhibitor, emerged as a potential biomarker for acute kidney injury and mortality in thoracoabdominal aortic aneurysm patients. Our study investigates RNH1 dynamics in sepsis, its links to mortality and organ dysfunction, and the interplay with RNase 1 and RNase 5. Furthermore, we explore RNH1 as a therapeutic target in sepsis-related processes like inflammation, non-canonical inflammasome activation, and iron homeostasis. We showed that RNH1 levels are significantly higher in deceased patients compared to sepsis survivors and correlate with creatine kinase, aspartate and alanine transaminase, bilirubin, serum creatinine and RNase 5, but not RNase 1. RNH1 mitigated LPS-induced TNFα and RNase 5 secretion, and relative mRNA expression of ferroptosis-associated genes HMOX1, FTH1 and HAMP in PBMCs. Monocytes were identified as the predominant type of LPS-positive PBMCs. Exogenous RNH1 attenuated LPS-induced CASP5 expression, while increasing IL-1β secretion in PBMCs and THP-1 macrophages. As RNH1 has contradictory effects on inflammation and non-canonical inflammasome activation, its use as a therapeutic agent is limited. However, RNH1 levels may play a central role in iron homeostasis during sepsis, supporting our clinical observations. Hence, RNH1 shows promise as biomarkers for renal and hepatic dysfunction and hepatocyte injury, and may be useful in predicting the outcome of septic patients.

43. Standardized pharmacological management of delirium after on-pump cardiac surgery reduces ICU stay and ventilation in a retrospective pre-post study

Abstract

Abstract Cardiac surgery patients not only undergo a highly invasive procedure but are at risk for a diversity of postoperative complications. Up to 53% of these patients suffer from postoperative delirium (POD). This severe and common adverse event increases mortality and prolonged mechanical ventilation and extends the intensive care unit stay. The objective of this study was to test the hypothesis that standardized pharmacological management of delirium (SPMD) may reduce the length of stay in the intensive care unit (ICU), duration of postoperative mechanical ventilation, and the incidence of postoperative complications such as pneumonia or bloodstream infections in on-pump cardiac surgery ICU patients. In this retrospective, single-center observational cohort study, 247 patients were examined between May 2018 to June 2020, who underwent on-pump cardiac surgery, suffered from POD, and received pharmacological POD treatment. 125 were treated before and 122 after SPMD implementation in the ICU. The primary endpoint was a composite outcome, including the length of ICU stay, postoperative mechanical ventilation time, and ICU survival rate. The secondary endpoints were complications including postoperative pneumonia and bloodstream infections. Although the ICU survival rate was not significantly different between both groups, the length of ICU stay (control group: 23 ± 27 days; SPMD group: 16 ± 16 days; p = 0.024) and the duration of mechanical ventilation were significantly reduced in the SPMD-cohort (control group: 230 ± 395 h; SPMD group: 128 ± 268 h; p = 0.022). Concordantly, the pneumonic risk was reduced after SPMD introduction (control group: 44.0%; SPMD group: 27.9%; p = 0.012) as well as the incidence for bloodstream infections (control group: 19.2%; SPMD group: 6.6%; p = 0.004). Standardized pharmacological management of postoperative delirium in on-pump cardiac surgery ICU patients reduced the length of ICU stay and duration of mechanical ventilation significantly, leading to a decrease in pneumonic complications and bloodstream infections.

Abstract

Abstract Little is known about the impact of multiple trauma (MT)-related systemic hypoxia on osseous protein concentration of the hypoxia transcriptome. To shed light on this issue, we investigated erythropoietin (Epo), erythropoietin receptor (EpoR), and Y-box binding protein 1 (YB-1) concentrations in the fracture zone in a porcine MT + traumatic hemorrhage (TH) model. Sixteen male domestic pigs were randomized into two groups: an MT + TH group and a sham group. A tibia fracture, lung contusion, and TH were induced in the MT + TH group. The total observation period was 72 h. YB-1 concentrations in bone marrow (BM) were significantly lower in the fracture zone of the MT + TH animals than in the sham animals. Significant downregulation of BM-localized EpoR concentration in both unfractured and fractured bones was observed in the MT + TH animals relative to the sham animals. In BM, Epo concentrations were higher in the fracture zone of the MT + TH animals compared with that in the sham animals. Significantly higher Epo concentrations were detected in the BM of fractured bone compared to that in cortical bone. Our results provide the first evidence that MT + TH alters hypoxia-related protein concentrations. The impacts of both the fracture and concomitant injuries on protein concentrations need to be studied in more detail to shed light on the hypoxia transcriptome in fractured and healthy bones after MT + TH.
