Abstract

Background Both aspiration and stent retriever thrombectomy are safe and effective in patients with acute ischemic stroke due to large vessel occlusion in the anterior circulation. Little is known on the outcomes of these techniques in patients with basilar artery occlusion. This study aimed to compare clinical, technical, and safety outcomes of aspiration and stent retriever thrombectomy as first‐line treatment for basilar artery occlusion in the BASICS (Basilar artery International Cooperation Study) trial. Methods For this post hoc analysis of the BASICS trial, all patients with a basilar artery occlusion who received endovascular treatment with either direct aspiration or stent retriever thrombectomy as first‐line approach were included. When both techniques were registered as first choice, patients were considered to have been treated with stent retriever. The primary outcome was favorable functional outcome, defined as a modified Rankin scale score of 0–3 at 90 days follow‐up, and analyzed using binary logistic regression analysis. Secondary outcomes included the modified Rankin scale score at 90 days (ranging from 0 to 6), procedure duration, mortality at 90 days, and symptomatic intracranial hemorrhage. Secondary outcomes were analyzed using binary, linear, or ordinal regression analyses. All analyses were adjusted for predefined variables. Results Among 158 BASICS patients treated with endovascular treatment,127 were treated with either stent retriever (N=67, 53%), or aspiration (N=60, 47%) as the first‐line treatment modality. We observed no significant difference in favorable functional outcome between patients treated with aspiration and stent retriever thrombectomy as first modality (adjusted odds ratio, 1.80; [95% CI, 0.68–4.76]). Also modified Rankin scale score at 90 days (adjusted common odds ratio, 0.62; [95% CI, 0.30–1.27]) and incidence of symptomatic intracranial hemorrhage (adjusted odds ratio, 0.61; [95% CI, 0.08–4.76]) showed no significant differences between both techniques. Procedure time was shorter with a median of 32 versus 47 minutes (26%; 95% CI, −42 to −6) and mortality rates at 90 days were lower (adjusted odds ratio, 0.36; [95% CI: 0.13–1.00]) in the direct aspiration group. Conclusions This study shows no difference in favorable functional outcome in patients with a basilar artery occlusion treated with direct aspiration compared with patients treated with stent retriever thrombectomy within the BASICS trial, despite a shorter procedure time and lower mortality rate at 90 days.

5. Ethical considerations about the collection of biological samples for genetic analysis in clinical trials

Abstract

Progress in precision medicine is being achieved through the design of clinical trials that use genetic biomarkers to guide stratification of patients and assignation to treatment or control groups. Genetic analysis of biomarkers is, therefore, essential to complete their objectives, and this involves the study of biological samples from donor patients that have been recruited according to criteria previously established in the design of the clinical trial. Nevertheless, it is becoming very common that, in the solicitation of biological samples, purposes that are beyond the objectives of the stated therapeutic trial research are introduced, like the development of ill-explained exploratory studies or the use in unspecified future research. In the digital era, the sequencing of patients’ DNA needs to be considered as a serious security matter, not only for the patients, but also for their relatives. Genetic information may be easily stored, even forever, in digital files. This engenders a permanent risk of being stolen or misused in many ways. Furthermore, re-identification of sample donors is technically feasible through their genetic data. For these reasons, genetic analysis of samples collected in clinical trials should be restricted to the accomplishment of their main objectives or well justified goals.

Abstract

Summary: Background: Intensive blood pressure lowering may adversely affect evolving cerebral ischaemia. We aimed to determine whether intensive blood pressure lowering altered the size of cerebral infarction in the 2196 patients who participated in the Enhanced Control of Hypertension and Thrombolysis Stroke Study, an international randomised controlled trial of intensive (systolic target 130–140 mm Hg within 1 h; maintained for 72 h) or guideline-recommended (systolic target 150 mm Hg) after thrombolysis treatment for acute ischaemic stroke between March 3, 2012 and April 30, 2018. Methods: All available brain imaging were analysed centrally by expert readers. Log-linear regression was used to determine the effects of intensive blood pressure lowering on the size of cerebral infarction, with adjustment for potential confounders. The primary analysis pertained to follow-up computerised tomography (CT) scans done between 24 and 36 h. Sensitivity analysis were undertaken in patients with only a follow-up magnetic resonance imaging (MRI) and either MRI or CT at 24–36 h, and in patients with any brain imaging done at any time during follow-up. This trial is registered with ClinicalTrials.gov, number NCT01422616. Findings: There were 1477 (67.3%) patients (mean age 67.7 [12.1] y; male 60%, Asian 65%) with available follow-up brain imaging for analysis, including 635 patients with a CT done at 24–36 h. Mean achieved systolic blood pressures over 1–24 h were 141 mm Hg and 149 mm Hg in the intensive group and guideline group, respectively. There was no effect of intensive blood pressure lowering on the median size (ml) of cerebral infarction on follow-up CT at 24–36 h (0.3 [IQR 0.0–16.6] in the intensive group and 0.9 [0.0–12.5] in the guideline group; log Δmean −0.17, 95% CI −0.78 to 0.43). This supplementary dataset is supportive of the randomized sham-controlled, double-blind, crossover clinical trial investigating polarity- and intensity-dependent effects of high-definition transcranial electrical stimulation (HD-tDCS) applied over the right temporo-parietal junction on mean middle cerebral artery blood flow velocity (MCA-BFv) bilaterally. Data of eleven healthy right-handed adults (6 women, 5 men; mean age 31 ± 5.6 years old) were analyzed for MCA-BFv, assessed using transcranial doppler ultrasound on the stimulated and the contralateral hemisphere concomitantly, during and after 3 blocks of 2 min HD-tDCS at 1, 2, and 3 mA. Participants received three electrical stimulation conditions (anode center, cathode center, and sham) randomly ordered across different days. The collected data is publicly available at Mendeley Data. This article and the data will inform future related investigations and safety analysis of transcranial non-invasive brain stimulation.

Abstract

BackgroundPeak width of skeletonized mean diffusivity (PSMD) is a promising diffusion tensor imaging (DTI) marker that shows consistent and strong cognitive associations in the context of different cerebral small vessel diseases (cSVD).PurposeInvestigate whether PSMD (1) is higher in patients with Cerebral Amyloid Angiopathy (CAA) than those with arteriolosclerosis; (2) can capture the anteroposterior distribution of CAA-related abnormalities; (3) shows similar neuroimaging and cognitive associations in comparison to other classical DTI markers, such as average mean diffusivity (MD) and fractional anisotropy (FA).Materials and methodsWe analyzed cross-sectional neuroimaging and neuropsychological data from 90 non-demented memory-clinic subjects from a single center. Based on MRI findings, we classified them into probable-CAA (those that fulfilled the modified Boston criteria), subjects with MRI markers of cSVD not attributable to CAA (presumed arteriolosclerosis; cSVD), and subjects without evidence of cSVD on MRI (non-cSVD). We compared total and lobe-specific (frontal and occipital) DTI metrics values across the groups. We used linear regression models to investigate how PSMD, MD, and FA correlate with conventional neuroimaging markers of cSVD and cognitive scores in CAA.ResultsPSMD was comparable in probable-CAA (median 4.06 × 10–4 mm2/s) and cSVD (4.07 × 10–4 mm2/s) patients, but higher than in non-cSVD (3.30 × 10–4 mm2/s; p < 0.001) subjects. Occipital-frontal PSMD gradients were higher in probable-CAA patients, and we observed a significant interaction between diagnosis and region on PSMD values [F(2, 87) = 3.887, p = 0.024]. PSMD was mainly associated with white matter hyperintensity volume, whereas MD and FA were also associated with other markers, especially with the burden of perivascular spaces. PSMD correlated with worse executive function (β = −0.581, p < 0.001) and processing speed (β = −0.463, p = 0.003), explaining more variance than other MRI markers. MD and FA were not associated with performance in any cognitive domain.ConclusionPSMD is a promising biomarker of cognitive impairment in CAA that outperforms other conventional and DTI-based neuroimaging markers. Although global PSMD is similarly increased in different forms of cSVD, PSMD’s spatial variations could potentially provide insights into the predominant type of underlying microvascular pathology.

9. Quantifying the Impact of the Dust Torque on the Migration of Low-mass Planets

Abstract

Disk solids are critical in many planet formation processes; however, their effect on planet migration remains largely unexplored. Here we assess this important issue for the first time by building on the systematic measurements of dust torques on an embedded planet by Benitez-Llambay & Pessah. Adopting standard models for the gaseous disk and its solid content, we quantify the impact of the dust torque for a wide range of conditions describing the disk/planet system. We show that the total torque can be positive and reverse inward planet migration for planetary cores with M _p ≲ 10 M _⊕ . We compute formation tracks for low-mass embryos for conditions usually invoked when modeling planet formation processes. Our most important conclusion is that dust torques can have a significant impact on the migration and formation history of planetary embryos. The most important implications of our findings are as follows. (i) For nominal dust-to-gas mass ratios ϵ ≃ 0.01, low-mass planets migrate outwards beyond the water ice-line if most of the mass in the solids is in particles with Stokes numbers St ≃0.1. (ii) For ϵ ≳ 0.02–0.05, solids with small Stokes numbers, St ≃ 0.01, can play a dominant role if most of the mass is in those particles. (iii) Dust torques have the potential to enable low-mass planetary cores formed in the inner disk to migrate outwards and act as the seed for massive planets at distances of tens of au.

10. A fading radius valley towards M-dwarfs, a persistent density valley across stellar types

Abstract

The radius valley separating super-Earths from mini-Neptunes is a fundamental benchmark for theories of planet formation and evolution. Observations show that the location of the radius valley decreases with decreasing stellar mass and with increasing orbital period. Here, we build from our previous pebble-based formation model, which, combined with photoevaporation after disc dispersal, unveiled the radius valley as a separator between rocky- and water-worlds. We expand our models for a range of stellar masses spanning from 0.1 to 1.5 $M_\odot$. We find that the location of the radius valley is well described by a power-law in stellar mass as $R_{\rm valley} = 1.8197 \, M_{\star}^{\!0.14({+0.02}/{-0.01})}$, which is in excellent agreement with observations. We also find very good agreement with the dependence of the radius valley on orbital period, both for FGK- and M-dwarfs. Additionally, we note that the radius valley gets filled towards low stellar masses, particularly at 0.1-0.4 $M_\odot$, yielding a rather flat slope in $R_{\rm valley} - P_{\rm orb}$. This is the result of orbital migration occurring at lower planet mass for less massive stars, which allows for low-mass water-worlds to reach the inner regions of the system, blurring the separation in mass (and size) between rocky- and water-worlds. Furthermore, we find that for planetary equilibrium temperatures above 400 K, the water in the volatile layer exists fully in the form of steam, puffing the planet radius up compared to condensed-water worlds. This produces an increase in planet radii of $\sim 30\%$ at 1 $M_\oplus$, and of $\sim 15\%$ at 5 $M_\oplus$, compared to condensed-water-worlds. As with Sun-like stars, pebble accretion leaves its imprint on the overall exoplanet population as a depletion of planets with intermediate compositions, carving a valley in planet density for all spectral types (abridged)., Comment: Accepted for publication in A&A Letters. The study of protoplanetary disc evolution and planet formation has mainly concentrated on solar (and low) mass stars since they host the majority of the confirmed exoplanets. Nevertheless, the numerous planets found orbiting stars up to $\sim3M_\odot$ has sparked interest in understanding how they form and how their hosting discs evolve. Our goal is to improve our knowledge on the gas disc evolution around intermediate mass stars for future planet formation studies. We study the long-term evolution of protoplanetary discs affected by viscous accretion, X-ray and FUV photoevaporation from the central star around stars between $1 - 3M_\odot$ considering the effects of stellar evolution. We explore different values of the viscosity parameter and the initial mass of the disc. We find that the evolutionary pathway of disc dispersal depends on the stellar mass. Our simulations reveal four distinct evolutionary pathways for the gas disc not reported before that are a consequence of stellar evolution, and which will likely impact dust evolution and planet formation. As the stellar mass grows from 1 to $\sim2M_\odot$, the disc evolution changes from the conventional inside-out clearing to a homogeneous disc evolution scenario where both inner and outer discs, formed after photoevaporation opened a gap, vanish over a similar timescale. As the stellar mass continues to increase, reaching $\sim 3M_\odot$, we have identified a distinct pathway that we refer to as revenant disc evolution, where the inner and outer discs reconnect after the gap opened. For the largest masses, we observe outside-in disc dispersal, in which the outer disc dissipates first due to the strong FUV photoevaporation. Revenant disc evolution stands out as it is capable of extending the disc lifespan. 12. Constraining mass, radius and tidal deformability of compact stars with axial wI modes: new universal relations including slow stable hybrid stars

Abstract

We revisit asteroseismology with quadrupolar wI modes and present universal relationships for its fundamental and first overtone. In contrast to relationships proposed in the literature, our universal relationships are capable of including slow stable hybrid stars that appear when considering slow sharp hadron-quark phase transitions. We show that, if the frequency and damping time of the fundamental mode of a given pulsating object are measured, its mass, radius, and dimensionless tidal deformability can be inferred. Moreover, we show that the errors of such estimates are smaller than a few percent for the mass and radius. For the dimensionless tidal deformability, the errors are -- for compact objects with $M\gtrsim 1.4\,M_\odot$ -- in general smaller than ~100%. Comparison with previous universal relationships shows that the ones proposed in this work produce better estimates of the mass and radius of totally stable compact objects., Comment: Accepted to be published in the MNRAS

Abstract

Multi-messenger astronomy with gravitational waves is a blooming area whose limits are not clear. After the first detection of binary black hole merger and the famous event GW170817 and its electromagnetic counterpart, the compact-object astrophysical community is starting to grasp the physical implications of such event and trying to improve numerical models to compare with future observations. Moreover, recent detections made by the NICER collaboration increased the tension between several theoretical models used to describe matter in the inner core of compact objects. In this paper, we focus on quadrupolar purely spacetime wI-modes of oscillating compact objects described using a wide range of hybrid equations of state able to include several theoretical possibilities of exotic matter in the inner core of such stars. We study the case in which a sharp first order hadron-quark phase transition occurs and explore the scenarios of rapid and slow phase conversions at the interface. We put special attention on the validity of universal relationships for the oscillation frequency and damping time that might help unravel the mysteries hidden at the inner cores of compact objects. We show that, within the slow conversion regime where extended branches of hybrid stars appear, universal relationships for wI-modes proposed in the literature do not hold., Comment: 17 pages, 11 figures. Abstract

The early evolution of sex chromosomes has remained obscure for more than a century. The Vandiemenella viatica species group of morabine grasshoppers is highly suited for studying the early stages of sex chromosome divergence and degeneration of the Y chromosome. This stems from the fact that neo‐XY sex chromosomes have independently evolved multiple times by X‐autosome fusions with different autosomes. Here, we generated new chromosome‐level assemblies for two chromosomal races representing karyotypes with and without neo‐sex chromosomes (P24XY and P24X0), and sequence data of a third chromosomal race with a different neo‐XY chromosome system (P25XY). Interestingly, these two neo‐XY chromosomal races are formed by different X‐autosome fusions (involving chr1 and chrB, respectively), and we found that both neo‐Y chromosomes have partly ceased to recombine with their neo‐X counterpart. We show that the neo‐XY chromosomes have diverged through accumulation of SNPs and structural mutations, and that many neo‐Y‐linked genes have degenerated since recombination ceased. However, the non‐recombining regions of neo‐Y chromosomes host non‐degenerated genes crucial for sex determination, such as sex‐lethal and transformer, alongside genes associated with spermatogenesis, fertility, and reproduction, illustrating their integrative role as a masculinizing supergene. Contrary to expectations, the neo‐Y chromosomes showed (slightly) lower density of transposable elements (TEs) compared to other genomic regions. The study reveals the unique dynamics of young sex chromosomes, with evolution of recombination suppression and pronounced decay of (some) neo‐sex chromosome genes, and provides a compelling case illustrating how chromosomal fusions and post‐fusion mutational processes contribute to the evolution of supergenes. Abstract

Neutrophil and lymphocyte ratio (NLR) has emerged as a complementary marker in intensive care. This study aimed to associate high NLR values with mortality as the primary outcome, and length of stay and need for invasive mechanical ventilation as secondary outcomes, in critically ill patients with COVID-19. A cross-sectional study encompassing 189 critically ill patients with COVID-19 was performed. The receiver operating characteristic curve was used to identify the best NLR cutoff value for ICU mortality (≥ 10.6). An NLR ≥ 10.6, compared with an NLR < 10.6, was associated with higher odds of ICU mortality (odds ratio [OR], 2.77; 95% confidence interval [CI], 1.24–6.18), ICU length of stay ≥ 14 days (OR, 3.56; 95% CI, 1.01–12.5), and need for invasive mechanical ventilation (OR, 5.39; 95% CI, 1.96–14.81) in the fully adjusted model (age, sex, kidney dysfunction, diabetes, obesity, hypertension, deep vein thrombosis, antibiotics, anticoagulants, antivirals, corticoids, neuromuscular blockers, and vasoactive drugs). Abstract

Introduction: Poststroke apathy (PSA) is a common neuropsychiatric disorder that may affect up to 30% of stroke patients. Despite the difficulties of investigating this condition (overlapping with depression, heterogeneity of diagnostic criteria, a small number of studies), some recent diffusion tensor imaging studies have suggested that widespread microstructural white matter (WM) disruption plays a key role in the development of PSA. Therefore, we intended to investigate this hypothesis by evaluating the relationship between WM hyperintensities (WMH) and apathy in patients with cerebrovascular disease. Methods: We studied patients with apathy (n = 7), depression (n = 13), comorbid apathy and depression (n = 13), and controls (n = 20), and we investigated the variables associated with the volume of WMH measured by an automated brain MRI segmentation software. Results: The overall prevalence of PSA was 37.7% (pure and comorbid). Patients with apathy presented a higher volume of WMH in comparison to controls. Mini-Mental State Examination (MMSE), NPI-A, and the number of cerebral microbleeds were the only variables associated with WMH. Conversely, NPI-D did not correlate to WMH. Discussion/Conclusion: This is an exploratory study that supports the view of PSA as a distinct syndrome from PSD mediated mainly by diffuse white matter hyperintensities, which suggests that WM disruption is an important pathway to the development of apathy in stroke patients.

Abstract

Introduction The benefits of mechanical thrombectomy in the treatment of patients with acute stroke due to large vessel occlusions (LVOs) have been extensively demonstrated by randomized trials and registries in developed countries. However, data on thrombectomy outside controlled trials are scarce in developing countries. The aim of this study was to assess the safety and efficacy, and to investigate the predictors for good and poor outcomes of thrombectomy for treatment of AIS due to anterior circulation LVOs in Brazil. Materials and Methods This was a single center registry of thrombectomy in the treatment of stroke caused by anterior circulation LVOs. Between 2011 and 2019, a total of 565 patients were included. Results the mean baseline NIHSS score on admission was 17.2. The average baseline ASPECTS was 8, and 91.0% of patients scored ≥6. Half of the patients received intravenous thrombolysis. The mean time from symptom onset to arterial puncture was 296.4 minutes. The mean procedure time was 61.4 minutes. The rates of the main outcomes were recanalization (TICI 2b-3) 85.6%, symptomatic intracranial hemorrhage (sICH) 8,1%, good clinical outcome (mRS=0-2) 43,5%, and mortality 22.1% at three months. Conclusions This study demonstrates the efficacy and safety of mechanical thrombectomy for treatment of patients with AIS of the anterior circulation in real-life conditions under limited facilities and resources. The results of the present study were relatively similar to those of large trials and population registers of developed countries.

42. Subtle white matter intensity changes on FLAIR imaging in patients with ischemic stroke

Abstract

BackgroundThere is increasing evidence from pathology and neuroimaging suggesting that the structural abnormalities which characterize leukoaraiosis are present within regions of normal-appearing white matter. In this study, we aimed to verify whether signal intensity on FLAIR imaging in normal-appearing white matter is related to the leukoaraiosis burden.MethodsWe performed a cross-sectional study of adult patients admitted with a diagnosis of acute ischemic stroke or transient ischemic attack. Leukoaraiosis was segmented using a semi-automated method involving manual outlining and signal thresholding. White matter regions were segmented based on the probabilistic tissue maps from the International Consortium for Brain Mapping 152 atlas. Also, white matter was further segmented based on voxel-distance from leukoaraiosis borders. Normalized mean FLAIR signal intensity on normal-appearing white matter (NAWMM) was used as a dependent variable in univariate and multivariate statistical analysis, and leukoaraiosis volume quartiles (LKAV) and clinical data as independent variables.ResultsOne-hundred consecutive patients were selected for analysis (53% female, mean age 68 years). NAWMMwas higher in the vicinity of leukoaraiosis and progressively lower at increasing distances from leukoaraiosis. NAWMMwas independently associated with leukoaraiosis volume. In voxels in the vicinity of leukoaraiosis borders, there was a linear association between LKAVand NAWMM(B=0.03;PMamong patients in the second and third quartiles, and lower in the first and fourth quartiles (B=-0.21 for the second order term;PM.ConclusionsOur results show that normal-appearing white matter exhibits subtle signal intensity changes that are related to leukoaraiosis burden. The neuroimaging signature of these subtle changes suggests that they might not be a milder form of leukoaraiosis, but rather rather reflect widespread etiopathogenic processes underlying small-vessel disease and thus, leukoaraiosis development.

Abstract

Grasshoppers in the genus Ronderosia (divergence time < 5 Mya) provide a special opportunity to examine the impact of chromosome rearrangements (CRs) in evolution and speciation, because they exhibit extensively rearranged karyotypes involving autosome-autosome and autosome-X chromosomes, the latter leading to the formation of a neo-sex chromosome system (neo-SCS). Despite the potential role of CRs in speciation, the general patterns of karyotype changes resulting from CRs in Ronderosia are still unclear. Here we investigated karyotype evolution in eight Ronderosia species using cytogenetic and phylogenetic models of ancestral character reconstruction. The overall findings suggest a central role for CRs in generating variability in chromosome number and neo-SCS across Ronderosia grasshoppers. It also suggests that incipient species with little morphological change, such as those observed in Ronderosia species, could accumulate marked differences in their karyotypes contributing to post-zygotic reproductive isolation. Besides the evolution of chromosome number in the phylogeny of the genus, chromosome change due to centric fusion is coupled with the divergence pattern observed in the group. While a cladogenetic event could represent a case where karyotypic differences result in speciation, further in-depth genomic studies are needed to better understand how these dramatic restructurings of karyotypes may have occurred.

Abstract

BACKGROUND: The optimal anesthetic management for endovascular therapy (EVT) in patients with posterior circulation stroke remains unclear. Our objective was to investigate the impact of early intubation in patients enrolled in the BASICS trial (Basilar Artery International Cooperation Study). METHODS: BASICS was a multicenter, randomized, controlled trial that compared the efficacy of EVT compared with the best medical care alone in patients with basilar artery occlusion. In this post hoc analysis, early intubation within the first 24 hours of the estimated time of basilar artery occlusion was examined as an additional covariate using regression modeling. We estimated the adjusted relative risks (RRs) for favorable outcomes, defined as modified Rankin Scale scores of 0 to 3 at 90 days. An adjusted common odds ratio was estimated for a shift in the distribution of modified Rankin Scale scores at 90 days. RESULTS: Of 300 patients in BASICS, 289 patients were eligible for analysis (151 in the EVT group and 138 in the best medical care group). compared with medical care alone, EVT was related to a higher risk of early intubation (RR, 1.29 [95% CI, 1.09–1.53]; P<0.01), and early intubation was negatively associated with favorable outcome (RR, 0.61 [95% CI, 0.45–0.84]; P=0.002). Whereas there was no overall treatment effect of EVT on favorable outcome (RR, 1.22 [95% CI, 0.95–1.55]; P=0.121), EVT was associated with favorable outcome (RR, 1.34 [95% CI, 1.05–1.71]; P=0.018) and a shift toward lower modified Rankin Scale scores (adjusted common odds ratio, 1.63 [95% CI, 1.04–2.57]; P=0.033) if adjusted for early intubation. CONCLUSIONS: In this post hoc analysis of the neutral BASICS trial, early intubation was linked to unfavorable outcomes, which might mitigate a potential benefit from EVT by indirect effects due to an increased risk of early intubation. This relationship may be considered when assessing the efficacy of EVT in patients with basilar artery occlusion

Abstract

Abstract Background The differences in clinical bioethics between the Mediterranean and Latin American cultures have not been analyzed. The objective of the study is to compare the ethical conflicts that internists in Spain, Mexico and Argentina have. Methods Cross-sectional observational study through a survey directed at internists from Spain, Argentina and Mexico. The survey was administered to affiliated members of the National Societies of Internal Medicine across three countries via an online platform. Results 762 internists participated, 261 from Spain, 154 from Argentina and 347 from Mexico. The main ethical conflicts that internists in Spain, Argentina and Mexico have are related (in order) to the end of life, to the clinical relationship and to the patient’s autonomy. Withholding and withdrawing life-sustaining treatment is the most frequent conflict in Spain and Argentina and the second in Mexico. Conclusions Internists from Spain and Argentina identify very similar ethical conflicts. Furthermore, they consider them more frequent and difficult than in Mexico. In Argentina they are less satisfied with the way they are resolved. To explain these differences, socio-cultural factors are postulated, among others: paternalism, individualism, masculinity, organization of the health system, formal training in bioethics and assessment of death.
