6. Matching-Adjusted Indirect Comparisons Applied for Comparative Efficacy of Haematologic Cancer Drugs within Health Economic Analysis

Abstract

Background Despite clarity in the matching-adjusted indirect comparison (MAIC) methodological guidelines, more research was needed to better understand MAIC applicability in practice, its impact on health economic models, and the outlook of reimbursement agencies on submissions including MAICs. Objective The aim of the current thesis was to evaluate how MAICs are used in practice to assess the comparative efficacy of haematologic cancer drugs for health economic modelling purposes. Methods Literature review studies were used to assess the underlying evidence of current treatment options for several haematologic cancer indications. MAICs were employed to indirectly compare the efficacy and safety of such treatments while accounting for the specificity of the underlying data. The MAIC findings were incorporated into a cost-effectiveness model and extrapolated over a lifetime span. Different extrapolation models were tested and compared to assess the best fit to the underlying survival curves. Results Literature shows that haematologic cancer treatments are mainly assessed in single-arm trials with few- to no head-to-head comparisons. While MAICs could close these evidence gaps, a lack of methodological uniformity in the way they are conducted in practice and their results reported could lead to biased or frail outcomes further being incorporated into health economic models. Extrapolation of such outcomes with suboptimal parametric curves could add to the compounding uncertainty and result into weak health technology assessment submissions. Conclusion The current doctoral thesis is advocating for more standardized methodology and reporting standards through a timely reconciliation of the theoretical guidelines with the practical realities.

Abstract

International audience; L'étude de la contamination de la Loire par les microplastiques (taille < 5 mm) a débutée en 2021. A travers deux projets de recherche (ANR Sedi-Plast et projet Plasti-nium) qui permettent de couvrir un très large continuum spatial - de la Loire amont jusqu'à l'estuaire de Loire - différents travaux ont été entrepris pour quantifier les niveaux de contamination des sédiments par les microplastiques, mais également mieux cerner les relations entre microplastiques et processus hydrosédimentaires. Sur la base des premiers éléments aujourd'hui disponibles, les niveaux de concentrations, la typologie des microplastiques et la nature des polymères seront ici discutés et mis en perspective au regard du continuum Terre-Mer.

Abstract

15e Journée scientifique de l'OSUNA, NANTES, FRANCE, 03-/06/2022 - 03/06/2022; Le projet Plasti-nium - Débris Plastiques dans le continuum terre-mer - ambitionne d'étudier la pollution plastique de la ville jusqu'à la mer et de créer une dynamique transverse et interdisciplinaire sur cette thématique au sein de la Région des Pays de la Loire. La problématique des débris plastiques et plus spécifiquement des microplastiques (taille < 5 mm) est aujourd'hui très médiatisée et trouve une caisse de résonance importante dans notre société civile. Concomitamment, la communauté scientifique démontre les impacts négatifs générés par cette pollution plastique sur les écosystèmes. Il en résulte aujourd'hui que les actions gouvernementales et autres actions locales et régionales se multiplient. La région des Pays de la Loire, dans le cadre de la stratégie « ambition marine » de 2018, vise spécifiquement la pollution plastique, et précise que des initiatives très concrètes de réduction sont déjà opérationnelles ou sur le point de l'être. Nantes Métropole, territoire reconnu « Zéro déchet, zéro gaspillage » soutient également des initiatives expérimentales, innovantes, contribuant à un territoire zéro déchet. Évaluer la mise en oeuvre de ces actions quelle que soit leur forme - de la sensibilisation à la gestion intégrée et concertée au niveau d'un territoire - constitue un enjeu. Le projet Plasti-nium vise à offrir un accompagnement scientifique de ces programmes, et de bien articuler l'acquisition des connaissances, leur transfert et les actions des politiques publiques. Cet accompagnement peut largement contribuer au rayonnement national et international de ces initiatives. Dans cette démarche, la finalité du projet Plasti-nium est de fournir un cadre de réflexion et d'expertise pour alimenter l'élaboration et la mise en oeuvre du plan régional de prévention et de gestion des déchets avec lequel la Région ambitionne d'être un territoire leader en termes de gestion des déchets.

18. The role of cardiac acoustic biomarkers in monitoring patients with heart failure: A systematic literature review.

Abstract

Heart failure (HF) creates a considerable clinical, humanistic and economic burden on patients and caregivers as well as on healthcare systems. To attenuate the significant burden of HF, there is a need for enhanced management of patients with HF. The use of digital tools for remote non-invasive monitoring of heart parameters is gaining traction, and cardiac acoustic biomarkers (CABs) have been proposed as a complementary set of measures to assess heart function alongside traditional methods such as electrocardiogram and echocardiography. We conducted a systematic literature review to evaluate associations between CABs and HF outcomes. Embase and MEDLINE databases were searched for recent studies published between 2013 and 2023 that evaluated CABs in patients with HF. Additional grey literature (i.e., conference, congress and pre-print publications from January 2021 to May 2023) searches were included. Two reviewers independently examined all articles; a third resolved conflicts. Data were extracted from articles meeting inclusion criteria. Extracted studies underwent quality and bias assessments using the Joanna Briggs Institute (JBI) critical appraisal tools. In total, 3074 records were screened, 73 full-text articles were assessed for eligibility and 27 publications were included. Third heart sound (S3) and electromechanical activation time (EMAT) were the CABs most often reported in the literature for monitoring HF. Fifteen publications discussed changes in S3 characteristics and its role in HF detection or outcomes: six studies highlighted S3 assessment among various groups of patients with HF; four studies evaluated the strength or amplitude of S3 with clinical outcomes; five studies assessed the relationship between S3 presence and clinical outcomes; and one study assessed both S3 presence and amplitude in relation to HF clinical outcomes. Eleven publications reported on EMAT and its derivatives: five studies on the relationship between EMAT and HF and six studies on the association of EMAT and HF clinical outcomes. Studies reporting the first and fourth heart sound, left ventricular ejection time and systolic dysfunction index were limited. Published literature supported S3 and EMAT as robust CAB measures in HF that may have value in remote clinical monitoring and management of patients with HF. Abstract

Aim: To evaluate adherence, healthcare resource utilization (HRU) and costs for glatiramer acetate (GA; injectable), dimethyl fumarate (oral) and teriflunomide (oral) in relapsing multiple sclerosis. Patients & methods: Retrospective analyses of a claims database. Results: Teriflunomide patients were older with more co-morbidities and fewer relapses versus GA and dimethyl fumarate. GA patients were mostly disease-modifying therapies (DMTs)-treatment naive. Treatment adherence was 61-70%. All DMTs reduced HRU versus pre-index. Costs were comparable across cohorts. High adherence reduced hospitalizations and several costs versus low adherers. Conclusion: Adherence rates were high and comparable with all DMTs. Similar (and high) reductions in HRU and costs occurred with all DMTs. High adherence improved economic outcomes versus low adherence. Thus, investing in adherence improvement is beneficial to improve outcomes in relapsing multiple sclerosis.
