Objective: We evaluated the extent to which frailty mediated the association between age, poor functional outcomes, and mortality after acute ischemic stroke when patients were treated with brain reperfusion (thrombolytic therapy and/or thrombectomy). Materials and methods: This retrospective cohort study included patients diagnosed with ischemic stroke who had undergone intravenous cerebral reperfusion therapy and/or mechanical thrombectomy. We created a mediation model by analyzing the direct natural effect of an mRS score > 2 and death on age-mediated frailty according to the Frailty Index. Results: We enrolled 292 patients with acute ischemic stroke who underwent brain reperfusion. Their mean age was 67.7 ± 13.1 years. Ninety days after the stroke ictus, 54 (18.5%) participants died, and 83 (28.4%) lived with moderate to severe disability (2 < mRS < 6). In the mediation analysis of the composite outcome of disability (mRS score > 2) or death, frailty accounted for 28% of the total effect of age. The models used to test for the interaction between age and frailty did not show statistically significant interactions for either outcome, and the addition of the interaction did not significantly change the direct or indirect effects, nor did it improve model fit. Conclusion: Frailty mediated almost one-third of the effect of age on the composite outcome of disability or death after acute ischemic stroke. [ABSTRACT FROM AUTHOR]

Introduction: Ankylosing Spondylitis (AS) is a type of chronic, autoimmune arthritis that primarily involves the spinal and sacroiliac joints. In more severe cases, this pathology can lead to a condition that results in loss of flexibility and fusion of the spine. Objective: To recognize viable and effective non-pharmacological treatment methods for the disease in question, given the great importance of the association with pharmacological treatment. Methodology: This is a descriptive research of the integrative literature review type, which sought to research the non-drug treatment of Spondylitis. The research was conducted through online access in the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Google Scholar, Virtual Health Library (VHL) and EBSCO Information Services databases, in the months of September and October 2022. Results and discussion: Physical activities, whether aquatic or terrestrial, are of great value when related to non-drug treatment of diseases that course with bone and muscle pain, by delaying the progression of the pathology and improving the patient's quality of life. In addition, home exercises indicated by physical therapists can also help with the aforementioned benefits. Furthermore, it is important to associate this with drug or even surgical treatment, especially in cases that are already advanced. Conclusion: The least invasive forms of treatment for ankylosing spondylitis are those that refer to physical activity that, associated with early diagnosis and pharmacological treatment, contribute to therapeutic success, lower risk of sequelae and, therefore, to a better quality of life for the patient. Autism Spectrum Disorder (ASD) represents a diverse group of conditions related to neurodevelopment, affecting one in every 100 children in the world, being prevalent in males. The main characteristics are the difficulty with social interaction and communication, in addition to repetitive and restrictive behaviors. The diagnosis can be made between 18 and 24 months of age, when characteristic symptoms can be distinguished from the child's typical development. It is noted that serotonin becomes important in ASD studies, which indicate that approximately 45% of patients have high serum levels of 5-HT and, consequently, less social interest. Furthermore, hypovitaminosis, also known as vitamin D deficiency, is related to ASD as well as melatonin deficiency. Therefore, the imbalance of serotonin, vitamin D and melatonin levels in patients diagnosed with ASD are of great relevance in terms of well-being and global health of the individual, due, respectively, to inference in social relationships, disorders neural pathways, as well as significant changes in the circadian cycle. Furthermore, it is feasible to observe that these particularities of patients with Autism Spectrum Disorder are of paramount importance in terms of early diagnosis, as well as effective treatment without external pharmacological measures. However, this is not yet a current reality.

As escalas revelaram boa consistência interna. Futuros estudos que incidam sobre os mesmos construtos devem estar protegidos com critérios de inclusão mais rigorosos., Cyberbullying is defined as an aggressive and repetitive act with the goal of causing discomfort to the person it is aimed at, usually envolving an imbalance of power between the bully and the bullied, representing one of the main issues associated with the wrongful use of the internet. This study intends to validate, for the Portuguese population, a scale developed by Lee et al. (2017) that studies the perpetration and victimization related to cyberbullying. The sample was composed of 475 participants, aged between 18 and 59 years old. Results showed that the scales have internal consistency. The confirmatory analysis indicates low levels of adjustment, and the factorial analysis shows some exceptions compared to the originally proposed structure. Convergent validity was established between the utilized scales. The results of this study were as expected. The aggressiveness and empathy constructs are indeed seemingly related, respectively, with the perpetration and victimization of cyberbullying. Future studies on these constructs would benefit from stricter inclusion criteria.

Introduction: although the study of human anatomy is a fundamental subject in the medical course, there are some difficulties in learning it, and this is related to the scarcity of corpses, inadequate anatomical parts, lack of attention and motivation on the part of students, little student familiarity with anatomical terminologies and lack of use of active technologies and methodologies in teaching. Objective: to discuss the main challenges of teaching Human Anatomy in medical schools. Methodology: this is a narrative literature review. The research was carried out through access to the National Library of Medicine, Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO Information Services. Results and discussion: among the main challenges, we highlight the obstacles caused by bureaucracies related to the availability of corpses, the investment in technologies related to the high cost, as well as the difficulty in memorizing the large number of atypical names by the students, added the passive methodology applied by most faculties, with a reduced specific workload for the study of anatomy. Final considerations: it is necessary to adapt the significant portion of medical courses that have not yet updated the teaching methodology, to one that encourages the active search associated with studies of the anatomy of the human body, associated with clinical practice, thus increasing the workload of this discipline, in order to attract the interest of students in a more resolute way, given the viable opportunities available today.

Introduction: adolescence has been seen in the Brazilian and worldwide scenario as a period of intense and abrupt physical, emotional and mainly behavioral changes. With the advent of AIDS and the emergence of sexually transmitted infections (STIs) – important causes of morbidity and mortality in Brazil – the health system expanded the view of risk factors associated with transmissible diseases and brought to the discussion the issue of vulnerability and sexuality. Objective: to explain about changes in sexual behavior among young people, even in the face of greater debate on the subject, and the incidence of STIs, which remains high. Methodology: this is a narrative literature review. The search was carried out through online access in the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO databases Information Services, October 2021. Results: there is a significant change in the behavior of young people regarding sexual health and the way they relate to their partners. In addition, it is noted that the current generation is more exposed, given that, due to the advent of the internet and, later, relationship applications, there was a revolution in the forms of interpersonal interaction, bringing many facilities, especially for the LGBTQIA+ population, who found in these virtual instruments a safe space for affective and sexual contact. Final considerations: it is important that young people are guided in relation to their sexual practices. In addition to the study of anatomy, sex education provides access to information about different aspects of sexuality. From it, people become able to exercise their sexual and reproductive desires, being informed about their health, given that sexuality is linked by different dimensions, such as biological, social and cultural.
Introduction: Inflammatory response in AD is characterized by the presence of activated microglia (the resident immunocompetent cells of the brain) in close association with neuritic plaques. Current evidence suggests that microglia are primarily involved in phagocytic activity and may be responsible for inducing additional neuronal damage by generating oxygen species and proteolytic enzymes. If anti-inflammatory drugs protect against the neurodegeneration seen in the brain of AD patients, then patients with a history of anti-inflammatory use should have a reduction in pathological changes in the brain and brain inflammation. Objective: to explain about the use of non-steroidal anti-inflammatory drugs as drug therapy for Alzheimer's disease. Results: Brain inflammation is believed to contribute to the pathological features of Alzheimer's disease (AD), and it has been postulated that anti-inflammatories protect against this tissue damage. However, one of the controversial factors regarding the use of non-steroidal anti-inflammatory drugs to reduce the risk of developing Alzheimer's disease is the toxicity associated with these drugs. Methodology: This is a systematic literature review with works searched in the following databases: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Library Health Virtual (BVS) and EBSCO Information Services. Relevant sources inherent to the theme were analyzed, using as one of the main criteria the choice of current, original and international articles. A total of 12 scientific articles were available for review. Final considerations: Anti-inflammatory drugs have been suggested as a possible treatment for Alzheimer's disease (AD). The association of immune proteins and immunocompetent microglial cells with senile plaques (SP) in AD and normal aging suggests that these drugs may be able to modify the course of AD, either interfering with SP formation or suppressing inflammation.

To understand the current practices in stroke evaluation, the main clinical decision support system and artificial intelligence (AI) technologies need to be understood to assist the therapist in obtaining better insights about impairments and level of activity and participation in persons with stroke during rehabilitation. This scoping review maps the use of AI for the functional evaluation of persons with stroke; the context involves any setting of rehabilitation. Data were extracted from CENTRAL, MEDLINE, EMBASE, LILACS, CINAHL, PEDRO Web of Science, IEEE Xplore, AAAI Publications, ACM Digital Library, MathSciNet, and arXiv up to January 2021. The data obtained from the literature review were summarized in a single dataset in which each reference paper was considered as an instance, and the study characteristics were considered as attributes. The attributes used for the multiple correspondence analysis were publication year, study type, sample size, age, stroke phase, stroke type, functional status, AI type, and AI function. Forty-four studies were included. The analysis showed that spasticity analysis based on ML techniques was used for the cases of stroke with moderate functional status. The techniques of deep learning and pressure sensors were used for gait analysis. Machine learning techniques and algorithms were used for upper limb and reaching analyses. The inertial measurement unit technique was applied in studies where the functional status was between mild and severe. The fuzzy logic technique was used for activity classifiers. The prevailing research themes demonstrated the growing utility of AI algorithms for stroke evaluation. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: We compared fixed and articulated ankle-foot orthoses (AFOs) in home-based mobility tasks to assess short-term mobility, dynamic balance, quality of life, anxiety/depression, disability level, stroke severity, autonomy, human functioning, and patient satisfaction., Methods: This was a two-arm, parallel-group, randomized controlled trial with concealed allocation, assessor blinding, and a complete case analysis involving patients with chronic stroke. The participants were randomized into two groups: fixed ( n = 24) and articulated ( n = 23) AFOs. The AFOs were custom-fabricated, and both groups performed four-week home-based mobility tasks five days weekly. Primary outcome measures included changes in balance and mobility assessed using the Tinetti Performance-Oriented Mobility Assessment (POMA), Timed Up and Go (TUG) test, and Functional Ambulation Category (FAC). Secondary outcomes included quality of life, anxiety/depression, disability, stroke severity, autonomy, human functioning, and patient satisfaction., Results: In a between-group comparison, after adjusting for age, sex, stroke severity, and thrombolysis, the articulated AFO group showed better performance in the TUG test ( p = 0.020; d = 0.93), POMA-Gait ( p = 0.001; d = 0.53), POMA-Total ( p = 0.048; d = 0.98), and FAC ( p = 0.003; d = 1.03) than the fixed AFO group. Moreover, significant difference was noted in human functioning (moving around using equipment)between the groups ( p = 0.047; d = 92)., Conclusion: A program involving home-based mobility tasks and articulated AFOs improved functional mobility after stroke.

- Abstract
Objective: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic., Methods: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI)., Results: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001)., Conclusion: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.
