4. Coping and well-being in university students: sex and cultural differences

For the psychological and personal well-being of university students, it is considered essential to study the coping strategies they use when faced with conflictive situations in the academic context and the resources that the institution offers to help them overcome these challenges. The objective of this work is to evaluate the effect of sex and culture on the different coping strategies that higher education students use in the face of the difficulties they face in the academic environment. For this purpose, the questionnaire “Coping Strategies Inventory (CSI)” was applied to a sample of 1,281 university students. The results indicate that there are significant differences in the problem-solving strategies used depending on gender and culture, finding interaction between these variables, with European women being the ones who use active strategies the most. On the contrary, men of Berber origin, are the ones who use less coping strategies, both active (emotional expression and social support) and passive (desiderative thinking), to resolve conflicts.

Background: There are evidence indicating that some metabolites of arachidonic acid produced by cytochromes P450 (CYP) and epoxide hydroxylase (EPHX2), such as hydroxyeicosatetraenoic acids (HETEs), epoxyeicosatrienoic acids (EETs) or dihydroxyeicosatrienoic acids (DHETEs), play an important role in blood pressure regulation and they could contribute to the development of hypertension (HT) and kidney damage. Therefore, the main aim of the study was to evaluate whether the genetic polymorphisms of CYP2C8, CYP2C9, CYP2J2, CYP4F2, CYP4F11 and EPHX2, responsible for the formation of HETEs, EETs and DHETEs, are related to the progression of impaired renal function in a group of patients with hypertension. Methods: 151HT patients from a hospital nephrology service were included in the study. Additionally, a group of 87 normotensive subjects were involved in the study as control group. For HT patients, a general biochemistry analysis, estimated glomerular filtration rate and genotyping for different CYPs and EPHX2 variant alleles was performed. Results: CYP4A11 rs3890011, rs9332982 and EPHX2 rs41507953 polymorphisms, according to the dominant model, presented a high risk of impaired kidney function, with odds ratios (OR) of 2.07 (1.00–4.32; P = 0.049) 3.02 (1.11–8.23; P = 0.030) and 3.59 (1.37–9.41; P = 0.009), respectively, and the EPHX2 rs1042032 polymorphism a greater risk according to the recessive model (OR = 6.23; 95% CI = 1.50−25.95; P = 0.007). However, no significant differences in allele frequencies between HT patients and in normotensive subjects for any of the SNP analysed. In addition, the patients with diagnosis of dyslipidemia (n = 90) presented higher frequencies of EPHX2 K55R (rs41507953) and *35A>G (rs1042032) variants than patients without dyslipidemia, 4% vs. 14% (P = 0.005) and 16 vs. 27% (P =0.02), respectively. Conclusions: In this study has been found higher odds of impaired renal function progression associated with rs3890011 and rs9332982 (CYP4A11) and rs41507953 and rs1042032 (EPHX2) polymorphisms, which may serve as biomarkers for improve clinical interventions aimed at avoiding or delaying, in chronic kidney disease patients, progress to end-stage kidney disease needing dialysis or kidney transplant. Resumen: Antecedentes: Existen evidencias que muestran que algunos metabolitos del ácido araquidónico producidos por los citocromos P450 (CYP) y por la enzima epóxido hidroxilasa (EPHX2), tales como los ácidos hidroxieicosatetraenoicos (HETEs), ácidos epoxieicosatrienoicos (EETs) o ácidos dihidroxieicosatrienoicos (DHETEs), juegan un papel importante en la regulación de la presión arterial y, por tanto, podrían contribuir al desarrollo de hipertensión y daño renal. Por ello, el objetivo del estudio fue evaluar si los polimorfismos de los genes CYP2C8, CYP2C9, CYP2J2, CYP4F2, CYP4F11 y EPHX2, responsables de la formación de HETEs, EETs y DHETEs, están relacionados con la progresión de la función renal en un grupo de pacientes con hipertensión arterial (HT). Métodos: Se incluyeron en el estudio 151 pacientes de un servicio hospitalario de nefrología con diagnóstico de HT. Además, un grupo de 87 sujetos normotensos participó en el estudio como grupo de control. A los pacientes con HT se les realizó un análisis de bioquímica general, se calculó la tasa estimada de filtración glomerular y se analizaron los genotipos de los diferentes genes CYPs y EPHX2 incluidos en el estudio. Resultados: Los polimorfismos rs3890011, rs9332982 del gen CYP4A11 y rs41507953 del gen EPHX2 presentaron un alto riesgo de alteración de la función renal con unos odds ratios (OR) de 2,07 (1,00–4,32; P = 0,049) 3,02 (1,11−8,23; P = 0,030) y 3,59 (1,37−9,41; P = 0,009), respectivamente, así como el polimorfismo rs1042032 del gen EPHX2 presentó también un mayor riesgo (OR = 6,23; IC 95% = 1,50−25,95; P = 0,007). Sin embargo, no se observaron diferencias significativas en las frecuencias alélicas entre pacientes con HT y sujetos normotensos para ninguno de los polimorfismos analizados. Además, los pacientes con dislipidemia (n = 90) presentaron una mayor frecuencia de las variantes EPHX2 K55R (rs41507953) y *35A > G (rs1042032) que los pacientes sin dislipidemia, 4% vs. 14% (P = 0,005) y 16 vs. 27% (P = 0.02), respectivamente. Conclusiones: En este estudio se han encontrado mayores probabilidades de progresión de la función renal asociadas a los polimorfismos rs3890011 y rs9332982 del gen CYP4A11 y rs41507953 y rs1042032 del gen EPHX2, que podrían servir como biomarcadores para mejorar las intervenciones clínicas destinadas a evitar o retrasar, en pacientes con enfermedad renal crónica, el avance de la enfermedad renal hacia la etapa terminal en la que necesitarían diálisis o trasplante de riñón.

Onychomycosis is a common fungal infection that affects the nails and accounts for approximately 50% of all nail diseases. The main pathogens involved include dermatophytes, such as Trichophyton rubrum, members of the T. mentagrophytes complex, and emerging pathogens in this infection, T. schoenleinii and T. tonsurans. Tea tree (Melaleuca alternifolia Cheel) essential oil (EO) has been proposed as a promising natural alternative to traditional treatments due to its antimicrobial properties. Among its more than 100 compounds, terpinen-4-ol is one of the main contributors to the antifungal action of this EO. To determine the antifungal activity of tea tree EO against dermatophytes, we designed an in vitro study using EUCAST-AFST protocols to obtain the values of MIC (minimum inhibitory concentration) and MFC (minimum fungicidal concentration) of several commercial M. alternifolia Cheel EOs against three species of dermatophytes isolated from clinical samples with suspected toenail onychomycosis. The results showed that the microorganism most sensitive to the action of the EO was T. rubrum, which had an MIC value more than 13 times lower than the value obtained for T. schoenleinii (0.4% v/v), the most resistant isolate. No differences in antifungal activity were observed by the analysed EOs or between the MIC and MFC values. These in vitro results suggest that tea tree EO is a viable option for the alternative treatment of onychomycosis, although clinical studies are needed to confirm the long-term antifungal activity, safety and efficacy of the oils studied in a clinical context.

Human papillomavirus is the ethological agent of various tumors, including plantar warts as one of the most frequent clinical presentations. Diagnosis of these warts continues to be mainly clinical, and a significant incidence of misdiagnosis leads to inadequate treatment. The aim of this study is to implement and validate a multiplex PCR detection method in the clinical setting to detect HPV in samples and to study genotype distribution in Spain to improve future molecular diagnostics. Viral DNA was extracted from 128 samples of clinically suspected plantar warts from various locations in Spain. A multiplex PCR was run alongside internal controls, and amplicons were processed for sequencing and HPV genotyping. The method was validated by assessing both inter- and intra-run repeatability. The PCR detection method returned 81.2 % (n = 104) positive results in the samples tested. Inter- and intra-run repeatability tests showed excellent intra-run agreement (κ = 1.00, p

18. Implementation of the Flipped Classroom and Its Longitudinal Impact on Improving Academic Performance

- Abstract
The objective has been to know the impact of the flipped classroom methodology on the academic performance of students during their training process in relation to the traditional methodology over time, in order to establish baselines in the academic grades in both models. The research is of a quasi-experimental type of non-equivalent groups, with a longitudinal trend design in the data collection process. The entire available population has been selected, with 1.236 students participating, exploring the grades as an analytical resource, from the 2010/2011 to the 2019/2020 academic years. The results show statistically significant differences in the improvement of academic performance with the flipped classroom methodology. Furthermore, the results reinforce that the flipped teaching model effectively promotes students' interest, their capacity for autonomous learning and personal and cooperative relationships.

Background: Breast cancer is the most common malignancy and the second leading cause of cancer-related mortality in Spanish women. Ribociclib in combination with endocrine therapy (ET) has shown superiority in prolonging survival in patients with hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2−) advanced breast cancer (ABC) vs. ET alone. Methods: CompLEEment-1 is a single-arm, open-label phase 3b trial evaluating ribociclib plus letrozole in a broad population of patients with HR+, HER2– ABC. The primary endpoints were safety and tolerability. Here we report data for Spanish patients enrolled in CompLEEment-1. Results: A total of 526 patients were evaluated (median follow-up: 26.97 months). Baseline characteristics showed a diverse population with a median age of 54 years. At study entry, 56.5% of patients had visceral metastases and 8.7% had received prior chemotherapy for advanced disease. Rates of all-grade and Grade ≥3 adverse events (AEs) were 99.0% and 76.2%, respectively; 21.3% of patients experienced a serious AE, and 15.8% of AEs led to treatment discontinuation. AEs of special interest of neutropenia, increased alanine aminotransferase, increased aspartate aminotransferase and QTcF prolongation occurred in 77.8%, 14.8%, 11.4% and 4.0% of patients, respectively. Patients aged >70 years experienced increased rates of all-grade and Grade ≥3 neutropenia and anemia. Efficacy results were consistent with the global study. Conclusions: Results from Spanish patients enrolled in CompLEEment-1 are consistent with global data showing efficacy and a manageable safety profile for ribociclib plus letrozole treatment in patients with HR+, HER2− ABC, including populations of interest (NCT02941926). IntroductionIntestinal colonization by Multi-Drug Resistant Organisms (MDROs) can pose a threat on the health of critically ill patients. The extent of colonization by these organisms is related to previous antibiotic treatments and their ability to cause infections among adult patients. The aim of this study is to determine the relationship between the intestinal Relative Loads (RLs) of selected antibiotic resistance genes, antibiotic consumption and extra-intestinal spread among critically ill pediatric patients.MethodsRLs of blaCTX-M-1-Family, blaOXA-1, blaOXA-48 and blaVIM were determined in 382 rectal swabs obtained from 90 pediatric critically ill patients using qPCRs. The RLs were compared to the patients’ demographics, antibiotic consumption, and detection of MDROs from extra-intestinal sites. 16SrDNA metagenomic sequencing was performed for 40 samples and clonality analyses were done for representative isolates.Results and discussion76 (74.45%) patients from which 340 (89.01%) rectal swabs were collected had at least one swab that was positive for one of the tested genes. Routine cultures did not identify carbapenemases in 32 (45.1%) and 78 (58.2%) swabs that were positive by PCR for blaOXA-48 and blaVIM, respectively. RLs of above 6.5% were associated with extra-intestinal spread of blaOXA-48-harboring MDROs. Bordetella pertussis not expressing pertactin has increased in countries using acellular pertussis vaccines (ACV). The deficiency is mostly caused by pertactin gene disruption by IS481. To assess the effect of the transition from whole-cell vaccine to ACV on the emergence of B. pertussis not expressing pertactin in Spain, we studied 342 isolates collected during 1986–2018. We identified 93 pertactin-deficient isolates. All were detected after introduction of ACV and represented 38% of isolates collected during the ACV period; 58.1% belonged to a genetic cluster of isolates carrying the unusual prn::del(–292, 1340) mutation. Pertactin inactivation by IS481 insertion was identified in 23.7% of pertactin-deficient isolates, arising independently multiple times and in different phylogenetic branches. Our findings support the emergence and dissemination of a cluster of B. pertussis with an infrequent mechanism of pertactin disruption in Spain, probably resulting from introduction of ACV.

- Abstract
For the psychological and personal well-being of university students, it is considered essential to study the coping strategies they use when faced with conflictive situations in the academic context and the resources that the institution offers to help them overcome these challenges. The objective of this work is to evaluate the effect of sex and culture on the different coping strategies that higher education students use in the face of the difficulties they face in the academic environment. For this purpose, the questionnaire "Coping Strategies Inventory (CSI)" was applied to a sample of 1,281 university students. The results indicate that there are significant differences in the problem-solving strategies used depending on gender and culture, finding interaction between these variables, with European women being the ones who use active strategies the most. On the contrary, men of Berber origin, are the ones who use less coping strategies, both active (emotional expression and social support) and passive (desiderative thinking), to resolve conflicts. Onychomycosis is a common fungal infection that affects the nails and accounts for approximately 50% of all nail diseases. The main pathogens involved include dermatophytes, such as Trichophyton rubrum, members of the T. mentagrophytes complex, and emerging pathogens in this infection, T. schoenleinii and T. tonsurans. Tea tree (Melaleuca alternifolia Cheel) essential oil (EO) has been proposed as a promising natural alternative to traditional treatments due to its antimicrobial properties. Among its more than 100 compounds, terpinen-4-ol is one of the main contributors to the antifungal action of this EO. To determine the antifungal activity of tea tree EO against dermatophytes, we designed an in vitro study using EUCAST-AFST protocols to obtain the values of MIC (minimum inhibitory concentration) and MFC (minimum fungicidal concentration) of several commercial M. alternifolia Cheel EOs against three species of dermatophytes isolated from clinical samples with suspected toenail onychomycosis. The results showed that the microorganism most sensitive to the action of the EO was T. rubrum, which had an MIC value more than 13 times lower than the value obtained for T. schoenleinii (0.4% v/v), the most resistant isolate. No differences in antifungal activity were observed by the analysed EOs or between the MIC and MFC values. These in vitro results suggest that tea tree EO is a viable option for the alternative treatment of onychomycosis, although clinical studies are needed to confirm the long-term antifungal activity, safety and efficacy of the oils studied in a clinical context. Introduction: Coarctation of the aorta (CoA) is one of the more common congenital heart defects that usually manifests in adults as poorly controlled hypertension. When technically possible, the treatment of choice for adult CoA is an endovascular approach with covered stent placement. A case is presented with atypical clinical onset, treated endovascularly with a double layer stent technique. Report: A 41 year old previously asymptomatic woman with an unremarkable past medical history presented with sudden dyspnoea, unstable blood pressure and pulse, and a radial femoral systolic pressure difference of 53 mmHg. A computed tomography scan showed coral reef aorta: aortic stenosis from a highly calcified lesion located distal to the origin of the left subclavian artery, compatible with CoA. Within a few hours, the patient went rapidly into cardiogenic shock with multiple organ failure requiring urgent intervention. Using a dual left iliac conduit and right brachial artery access, the lesion was pre-dilated with an 8 × 60 mm balloon. A double layer technique was then applied by coaxially deploying a BeGraft aortic stent (expanded to 18 mm) followed by a Conformable GORE® TAG® thoracic stent graft (26 × 26 × 100 mm). The patient's symptoms improved and the radial femoral systolic gradient decreased to 12 mmHg. Discussion: Sudden onset CoA is a rare condition in adults that can lead to refractory cardiogenic shock and multiple organ failure. In anatomically complex cases, a double layer technique may be beneficial because it has high radial force and good wall apposition with lower risk of stent collapse than single stent deployment.

Resumen: Introducción: la parada cardiorrespiratoria es una de las principales causas de muerte en los países industrializados. La reanimación cardiopulmonar (RCP) por testigos aumenta la supervivencia. En España el nivel de conocimientos al respecto es bajo, suponiendo un exceso de mortalidad. Es urgente formar a la población en general en RCP. Material y métodos: analizamos el impacto de la implementación de un programa de formación en soporte vital para los profesores y los estudiantes de educación secundaria, realizando un estudio observacional, descriptivo y transversal, con una muestra de 649 estudiantes, a los que se aleatorizó en 2 grupos para ser formados por un profesor de educación secundaria previamente instruido en soporte vital o por un sanitario. Posteriormente se analizó la adquisición de competencias y la diferencia según la edad y el sexo de los estudiantes. Resultados: objetivamos una adecuada adquisición de competencias a nivel global, y una diferencia estadísticamente significativa a favor de los estudiantes formados por su profesor en comparación con los que fueron formados por sanitarios. Conclusión: la adquisición de competencias en RCP por parte de los profesores y los estudiantes de educación secundaria presenta un nivel elevado y adecuado como para garantizar la participación de los mismos en los programas formativos, y la aplicabilidad de lo aprendido en situaciones reales. Consideramos que debe sistematizarse la formación en soporte vital a nivel de la educación secundaria obligatoria, por parte de los profesores. Abstract: Introduction: Cardiorespiratory arrest is one of the leading causes of death in industrialized countries. Bystander cardiopulmonary resuscitation (CPR) increases survival. However, there is a low level of general knowledge about this issue in Spain which entails a greater mortality rate in CA cases. It is urgent to train the general population in CPR. Material and methods: The impact of implementing a life support training program for secondary school teachers and students was analyzed. An observational, descriptive, and cross-sectional study was carried out, with a sample of 649 students, who were randomized into two groups to be trained by a secondary school teacher previously instructed in life support or by a health worker. Subsequently, the acquisition of skills and the difference according to the age and sex of the students were analyzed. Results: An adequate acquisition of skills at a global level was observed, and a statistically significant difference in favor of the students trained by their teacher compared to those who were trained by health professionals. Conclusion: Skills acquisition in basic CPR by secondary school students and teachers presents a high and adequate level to guarantee the participation in training programs and the applicability of what is learned in real situations, both for teachers and students. Life support training should be systematized at secondary education level by secondary school teachers.

ABSTRACT Pediatric liver transplantation (PLTx) is commonly associated with extensive antibiotic treatments that can produce gut microbiome alterations and open the way to dominance by multidrug-resistant organisms (MDROs). In this study, the relationship between intestinal Relative Loads (RLs) of β-lactamase genes, antibiotic consumption, microbiome disruption, and the extraintestinal dissemination of MDROs among PLTx patients is investigated. 28 PLTx patients were included, from whom 169 rectal swabs were collected. Total DNA was extracted and blaCTX-M-1-Family, blaOXA-1, blaOXA-48, and blaVIM were quantified via quantitative polymerase chain reaction (qPCR) and normalized to the total bacterial load (16SrRNA) through LogΔΔCt to determine the RLs. 16SrRNA sequencing was performed for 18 samples, and metagenomic sequencing was performed for 2. Patients’ clinical data were retrieved from the hospital’s database. At least one of the genes tested were detected in all of the patients. The RLs for blaCTX-M-1-Family, blaOXA-1, blaOXA-48, and blaVIM were higher than 1% of the total bacterial population in 67 (80.73%), 56 (78.87%), 57 (77.03%) and 39 (61.9%) samples, respectively. High RLs for blaCTX-M-1-Family, blaOXA-1, and/or blaOXA-48, were positively associated with the consumption of carbapenems with trimethoprim-sulfamethoxazole and coincided with low diversity in the gut microbiome. Low RLs were associated with the consumption of noncarbapenem β-lactams with aminoglycosides (P < 0.05). Extraintestinal isolates harboring the same gene(s) as those detected intraintestinally were found in 18 samples, and the RLs of the respective swabs were high. We demonstrated a relationship between the consumption of carbapenems with trimethoprim-sulfamethoxazole, intestinal dominance by MDROs and extraintestinal spread of these organisms among PLTx patients. IMPORTANCE In this study, we track the relative intestinal loads of antibiotic resistance genes among pediatric liver transplant patients and determine the relationship between this load, antibiotic consumption, and infections caused by antibiotic-resistant organisms. We demonstrate that the consumption of broad spectrum antibiotics increase this load and decrease the gut microbial diversity among these patients. Moreover, the high loads of resistance genes were related to the extraintestinal spread of multidrug-resistant organisms. Together, our data show that the tracking of the relative intestinal loads of antibiotic resistance genes can be used as a biomarker that has the potential to stop the extraintestinal spread of antibiotic-resistant bacteria via the measurement of the intestinal dominance of these organisms, thereby allowing for the application of preventive measures.

The clinical study aim was to investigate whether a tannin-based dietary supplementation could improve the efficacy of standard-of-care treatment of hospitalized COVID-19 patients by restoring gut microbiota function. Adverse events and immunomodulation post-tannin supplementation were also investigated. A total of 124 patients receiving standard-of-care treatment were randomized to oral tannin-based supplement or placebo for a total of 14 days. Longitudinal blood and stool samples were collected for cytokine and 16S rDNA microbiome profiling, and results were compared with 53 healthy controls. Although oral tannin supplementation did not result in clinical improvement or significant gut microbiome shifts after 14-days, a reduction in the inflammatory state was evident and significantly correlated with microbiota modulation. Among cytokines measured, MIP-1α was significantly decreased with tannin treatment (p = 0.03) where it correlated positively with IL-1β and TNF- α, and negatively with stool Bifidobacterium abundance.

- Abstract
In the current strategic context, there is a growing perception of nuclear, radiological, biological or chemical (CBRN) threat means by non-state actors, who will not hesitate to use weapons of mass destruction (WMD) or CBRN agents, if they have access to it and have the appropriate knowledge and means. On the other hand, the recent wave of attacks carried out by lone wolves in Europe and the United States, highlights the danger posed by this type of terrorism. For several decades, the terrorist strategy and tactics of individuals have emerged with particular relevance in the extreme right and Islamic fundamentalism, but with limited success choosing the use of CBRN agents, in general, and chemicals, in particular. The present paper may be useful to evaluate the possibility of this type of terrorism decides to use aggressive chemicals, whose lethality can pose a greater challenge for law enforcement agencies., En el contexto estratégico actual, hay una creciente percepción de la amenaza con medios nucleares, radiológicos, biológicos o químicos (NRBQ) por parte de actores no estatales, que no dudarán en emplear armas de destrucción masiva (ADM) o agentes NRBQ, si logran acceder a ellos y cuentan con los conocimientos y medios adecuados. Por otro lado, la reciente ola de ataques perpetrados por actores solitarios en Europa y Estados Unidos, pone de manifiesto el peligro que representa esta tipología de terrorismo. Desde hace unas décadas, la estrategia y táctica terroristas de los actores solitarios ha ido emergiendo con especial relevancia en la extrema derecha y en el fundamentalismo islámico, pero con escaso éxito a la hora de seleccionar el uso de agentes NRBQ, en general, y químicos, en particular. El presente trabajo resulta de utilidad para evaluar la posibilidad de que este tipo de terrorismo decida usar agresivos químicos, cuya letalidad puede suponer un mayor desafío para las Fuerzas y Cuerpos de Seguridad.

The influence of potassium content (at neutral pH and millimolar Mg(2+)) on the size distribution of FtsZ polymers formed in the presence of constantly replenished GTP under steady-state conditions was studied by a combination of biophysical methods. The size of the GTP-FtsZ polymers decreased with lower potassium concentration, in contrast with the increase in the mass of the GDP-FtsZ oligomers, whereas no effect was observed on FtsZ GTPase activity and critical concentration of polymerization. Remarkably, the concerted formation of a narrow size distribution of GTP-FtsZ polymers previously observed at high salt concentration was maintained in all KCl concentrations tested. Polymers induced with guanosine 5'-(¿,ß-methylene)triphosphate, a slowly hydrolyzable analog of GTP, became larger and polydisperse as the potassium concentration was decreased. Our results suggest that the potassium dependence of the GTP-FtsZ polymer size may be related to changes in the subunit turnover rate that are independent of the GTP hydrolysis rate. Actualmente la Telemedicina (TM) es una realidad en el trabajo diario de muchas especialidades médicas. El desarrollo y el aumento de la disponibilidad de las nuevas tecnologías de información y comunicación, junto con la flexibilidad de conectividad de los equipamientos médicos hace posible la asistencia médica de pacientes que tienen dificultades de acceso a un centro hospitalario. Una de las aplicaciones más importantes de la TM es la asistencia en Medicina de Urgencias. La experiencia acumulada por distintas especialidades médicas ha permitido un gran aumento de las posibilidades de atención urgente en las que la TM puede ser aplicable tanto en el entorno hospitalario como lejos del hospital. Además, las características propias de la atención sanitaria de urgencia, como son la necesidad de rapidez y de fiabilidad en el diagnóstico, hace que cada vez se requieran más y mejores equipamientos y soluciones. El incremento de la comunicación entre especialistas de urgencias con otras especialidades puede permitir mejores diagnósticos con la consiguiente disminución de la morbi-mortalidad en los pacientes que se atienden de urgencia.
