Abstract Background Electroconvulsive therapy (ECT) is widely recognized as one of the most effective treatments for various psychiatric disorders and is generally considered safe. However, a few reports have mentioned that multiple ECT sessions could induce electroencephalography (EEG) abnormalities and epileptic seizures, a serious side effect of ECT. We experienced a case with EEG abnormalities after multiple ECT sessions and aimed to share our insights on conducting ECT safely. Case Presentation We present the case of a 73‐year‐old female diagnosed with major depressive disorder. She underwent regular ECT sessions to alleviate her psychiatric symptoms. However, after more than 80 sessions, previously undetected EEG abnormalities were observed. Since the patient did not have clinical seizures, we were able to continue ECT at longer intervals without the use of antiepileptic drugs. Conclusion Our case suggests the importance of routine EEG testing in patients undergoing prolonged ECT. While careful monitoring is necessary, continuing ECT without antiepileptic medication in patients with EEG abnormalities could be permissible.

BackgroundThe current biomarker-supported diagnosis of Alzheimer’s disease (AD) is hindered by invasiveness and cost issues. This study aimed to address these challenges by utilizing portable electroencephalography (EEG). We propose a novel, non-invasive, and cost-effective method for identifying AD, using a sample of patients with biomarker-verified AD, to facilitate early and accessible disease screening.MethodsThis study included 35 patients with biomarker-verified AD, confirmed via cerebrospinal fluid sampling, and 35 age- and sex-balanced healthy volunteers (HVs). All participants underwent portable EEG recordings, focusing on 2-minute resting-state EEG epochs with closed eyes state. EEG recordings were transformed into scalogram images, which were analyzed using “vision Transformer(ViT),” a cutting-edge deep learning model, to differentiate patients from HVs.ResultsThe application of ViT to the scalogram images derived from portable EEG data demonstrated a significant capability to distinguish between patients with biomarker-verified AD and HVs. The method achieved an accuracy of 73%, with an area under the receiver operating characteristic curve of 0.80, indicating robust performance in identifying AD pathology using neurophysiological measures.ConclusionsOur findings highlight the potential of portable EEG combined with advanced deep learning techniques as a transformative tool for screening of biomarker-verified AD. This study not only contributes to the neurophysiological understanding of AD but also opens new avenues for the development of accessible and non-invasive diagnostic methods. The proposed approach paves the way for future clinical applications, offering a promising solution to the limitations of advanced diagnostic practices for dementia.

Abstract Associations between delirium and postoperative adverse events in cardiovascular surgery have been reported and the preoperative identification of high-risk patients of delirium is needed to implement focused interventions. We aimed to develop and validate machine learning models to predict post-cardiovascular surgery delirium. Patients aged ≥ 40 years who underwent cardiovascular surgery at a single hospital were prospectively enrolled. Preoperative and intraoperative factors were assessed. Each patient was evaluated for postoperative delirium 7 days after surgery. We developed machine learning models using the Bernoulli naive Bayes, Support vector machine, Random forest, Extra-trees, and XGBoost algorithms. Stratified fivefold cross-validation was performed for each developed model. Of the 87 patients, 24 (27.6%) developed postoperative delirium. Late-onset epilepsy, particularly focal impaired awareness seizures, often present without convulsions and can cause memory impairment. This can lead patients to initially seek consultation at memory clinics, potentially delaying referral to epilepsy specialists. We report on three patients, aged 40s to 70s, admitted for cognitive evaluation who were finally diagnosed with epileptic seizures as the underlying cause of their symptoms. Notably, all initially presented to local clinics with symptoms suggesting cognitive impairment. Despite initial diagnostic uncertainty, all patients exhibited epileptic activity on electroencephalography (EEG) and responded positively to antiepileptic drugs, suggesting epileptic mechanisms were involved in their symptoms. Both traditional clinical EEG systems and newly developed, one-minute portable EEG devices were used in their evaluations. The portable device, medically approved in Japan, successfully captured sharp-waves like activities with the same durations, amplitudes, and shapes as traditional devices. This highlights its potential to improve epilepsy diagnosis and future screening due to its portability and ease of use. Implementing portable EEG devices could promote timely and appropriate treatment, preventing misdiagnosis of neurological conditions.

IntroductionPostoperative delirium (POD) is common and life-threatening, however, with intensive interventions, a potentially preventable clinical syndrome. Although electroencephalography (EEG) is a promising biomarker of delirium, standard 20-leads EEG holds difficulties for screening usage in clinical practice.ObjectiveWe aimed to develop an accurate algorithm to predict POD using EEG data obtained from portable device.MethodsWe recruited 128 patients who underwent scheduled cardiovascular surgery. Cognitive function assessments were conducted, and portable EEG recordings were obtained prior to surgery.ResultsAmong the patients, 47 (36.7%) patients with POD were identified and they did not significantly differ from patients without POD in sex ratio, age, cognitive function, or treatment duration of intensive care unit. However, significant differences were observed in the preoperative EEG power spectrum densities at various frequencies, especially gamma activity, between patients with and without POD. POD was successfully predicted using preoperative EEG data with a machine learning algorithm, yielding accuracy of 86% and area under the receiver operating characteristic curve of 0.93.DiscussionThis study provides new insights into the objective and biological vulnerability to delirium. The developed algorithm can be applied in general hospitals without advanced equipment and expertise, thereby enabling the reduction of POD occurrences with intensive interventions for high-risk patients.

Abstract Alzheimer’s disease (AD) is a progressive neuropsychiatric disease affecting many elderly people and is characterized by progressive cognitive impairment of memory, visuospatial, and executive functions. As the elderly population is growing, the number of AD patients is increasing considerably. There is currently growing interest in determining AD’s cognitive dysfunction markers. We used exact low-resolution-brain-electromagnetic-tomography independent-component-analysis (eLORETA-ICA) to assess activities of five electroencephalography resting-state-networks (EEG-RSNs) in 90 drug-free AD patients and 11 drug-free patients with mild-cognitive-impairment due to AD (ADMCI). Compared to 147 healthy subjects, the AD/ADMCI patients showed significantly decreased activities in the memory network and occipital alpha activity, where the age difference between the AD/ADMCI and healthy groups was corrected by linear regression analysis. Furthermore, the age-corrected EEG-RSN activities showed correlations with cognitive function test scores in AD/ADMCI. In particular, decreased memory network activity showed correlations with worse total cognitive scores for both Mini-Mental-State-Examination (MMSE) and Alzheimer’s Disease-Assessment-Scale-cognitive-component-Japanese version (ADAS-J cog) including worse sub-scores for orientation, registration, repetition, word recognition and ideational praxis. Our results indicate that AD affects specific EEG-RSNs and deteriorated network activity causes symptoms. Overall, eLORETA-ICA is a useful, non-invasive tool for assessing EEG-functional-network activities and provides better understanding of the neurophysiological mechanisms underlying the disease.

Background and Aims: Although Helicobacter pylori is the most important bacterial carcinogen in gastric cancer (GC), GC can emerge even after H. pylori eradication. Studies suggest that various constituents of the gastric microbiome may influence GC development, but the role of individual pathogens is unclear. Methods: Human gastric mucosal samples were analyzed by 16SrRNA sequencing to investigate microbiome composition and its association with clinical parameters, including GC risk. Identified bacteria in the stomach were cocultured with gastric epithelial cells or inoculated into mice, and transcriptomic changes, DNA damage, and inflammation were analyzed. Bacterial reads in GC tissues were examined together with transcriptomic and genetic sequencing data in the cancer genome atlas dataset. Results: Patients after Helicobacter pylori eradication formed 3 subgroups based on the microbial composition revealed by 16SrRNA sequencing. One dysbiotic group enriched with Fusobacterium and Neisseria species was associated with a significantly higher GC incidence. These species activated prooncogenic pathways in gastric epithelial cells and promoted inflammation in mouse stomachs. Sugar chains that constitute gastric mucin attenuate host-bacteria interactions. Metabolites from Fusobacterium species were genotoxic, and the presence of the bacteria was associated with an inflammatory signature and a higher tumor mutation burden. Conclusion: Gastric microbiota in the dysbiotic stomach is associated with GC development after H. pylori eradication and plays a pathogenic role through direct host-bacteria interaction.

IntroductionThe current study measured the frontal midline theta rhythm (Fmθ), which appears in the frontal midline region during the attentional focus state, using the sheet-type wearable electroencephalograph (EEG) device HARU-1, and examined the modulation of frontal gamma band activity by cognitive tasks.MethodsWe measured the frontal EEG of 20 healthy subjects using HARU-1 for 2 min during the rest eyes-closed condition and simple mental calculation task condition, respectively. Statistical analyses were conducted using permutation testing based on t-test and cluster analysis to compare the results between the resting state and the task condition.ResultsTwelve of 20 subjects showed Fmθ during the task condition. The 12 subjects with Fmθ showed significantly higher activity of the theta and gamma bands, and significantly low activity of the alpha band during the task condition compared to the resting condition. In the eight subjects without Fmθ were significantly low activity of the alpha and beta bands and no significant activity in the theta and gamma band activity during the task condition compared to the resting condition.DiscussionThese results indicate that it is possible to measure Fmθ using HARU-1. A novel finding was the gamma band activity appearing with Fmθ in the left and right frontal forehead regions, suggesting that it reflects the function of the prefrontal cortex in working memory tasks.

Brain aging causes loss of synaptic spines, neuronal apoptosis, and a reduction in neurotransmitter levels. These aging phenomena disturb cortical electrical activity and its synchronization with connected regions. Previous electroencephalography (EEG) studies reported an age-related decrease in electrical activity in the alpha frequency band at occipital, parietal, and temporal areas as well as a decrease in occipital delta activity. However, there is an ongoing debate about whether there is an increase or decrease of the activity in other frequency bands with aging due to inconsistent study findings. In this study, we aimed to detect age-related changes of cortical electrical activities in all five frequency bands (delta, theta, alpha, beta, and gamma) in a large sample of healthy subjects for the first time. Using eLORETA (exact low-resolution brain electromagnetic tomography) analysis, we applied an eLORETA source estimation method to resting-state EEG data in 147 healthy subjects (median age 55, IQR 26.5–67.0) to obtain cortical electrical activity and assessed age-related changes in this activity using correlation analysis with multiple comparison correction. The combination of the eLORETA source estimation method and correlation analysis implemented in eLORETA software detected age-related changes in specific cortical regions for each frequency band: (1) delta and theta cortical electrical activities decreased at the occipital area with age, (2) alpha cortical electrical activity decreased at the occipitoparietotemporal areas with age, (3) beta cortical electrical activity increased at the insula, sensorimotor area, supplementary motor area, premotor area, and right temporal areas with age (most significant correlation at the right insula), (4) gamma cortical electrical activity increased at the frontoparietal and left temporal areas with age. These findings extend previous EEG study findings and provide valuable information related to mechanisms of healthy aging. Overall, our findings revealed that even healthy aging greatly affects cortical electrical activities in a region-specific way.

We experienced a rare case of Munchausen syndrome with peculiar symmetric ulcers on the whole body. When encountering similar cases with factitious ulceration of the skin, a thorough understanding of the patient's psychiatric condition and a cordial consultation to present illness rather than physical medication might lead to disease stability.

Enhanced de novo lipogenesis mediated by sterol regulatory element–binding proteins (SREBPs) is thought to be involved in nonalcoholic steatohepatitis (NASH) pathogenesis. In this study, we assessed the impact of SREBP inhibition on NASH and liver cancer development in murine models. Unexpectedly, SREBP inhibition via deletion of the SREBP cleavage–activating protein (SCAP) in the liver exacerbated liver injury, fibrosis, and carcinogenesis despite markedly reduced hepatic steatosis. These phenotypes were ameliorated by restoring SREBP function. Transcriptome and lipidome analyses revealed that SCAP/SREBP pathway inhibition altered the fatty acid (FA) composition of phosphatidylcholines due to both impaired FA synthesis and disorganized FA incorporation into phosphatidylcholine via lysophosphatidylcholine acyltransferase 3 (LPCAT3) downregulation, which led to endoplasmic reticulum (ER) stress and hepatocyte injury. Supplementation with phosphatidylcholines significantly improved liver injury and ER stress induced by SCAP deletion. The activity of the SCAP/SREBP/LPCAT3 axis was found to be inversely associated with liver fibrosis severity in human NASH. SREBP inhibition also cooperated with impaired autophagy to trigger liver injury. Thus, excessively strong and broad lipogenesis inhibition was counterproductive for NASH therapy; this will have important clinical implications in NASH treatment.

Physiological studies have shown that self-body images receive unique recognition processing in a wide range of brain areas, from the frontal lobe to the parietal-occipital cortex. Event-related potential (ERP) studies have shown that the self-referential effect on the image of a hand increases P300 components, but such studies do not evaluate brain oscillatory activity. In this study, we aimed to discover the self-specific brain electrophysiological activity in relation to hand images. ERPs on the fronto-parietal midline were elicited by a three-stimulus visual oddball task using hand images: the self-hand, another hand (most similar to the self-hand), and another hand (similar to the self-hand). We analyzed ERP waveform and brain oscillatory activity by simple averaging and time-frequency analysis. The simple averaging analysis found no significant differences between the responses for the three stimulus tasks in all time windows. However, time-frequency analysis showed that self-hand stimuli elicited high gamma ERS in 650–900 ms at the Cz electrode compared to other hand stimuli. Our results show that brain activity specific to the self-referential process to the self-hand image was reflected in the long latency gamma band activity in the mid-central region. This high gamma-band activity at the Cz electrode may be similar to the activity of the mirror neuron system, which is involved in hand motion.

Abstract Background In recent years, cases of dystextia (texting disabilities) and dystypia (typing disabilities) have been reported. However, reports describing isolated dystextia without aphasia or other cognitive impairments are rare, and the detailed pathophysiology is not fully understood. Most Japanese people use the alphabetical spelling system (Romaji) for texting and typing. Herein, we report the case of a man with isolated dystextia and dystypia resulting from Romaji conversion difficulties. Case presentation A 48-year-old, right-handed Japanese man developed texting and typing difficulties. The standard neuropsychological tests showed no signs of aphasia or other cognitive impairments, except for slight executive dysfunction. Thus, isolated dystextia and dystypia were diagnosed. Furthermore, the patient experienced Romaji conversion difficulties. Magnetic resonance imaging revealed a subcortical infarction in the left cerebral hemisphere. Single photon emission tomography revealed hypoperfusion, including in the left dorsolateral frontal cortex. Conclusions The left dorsolateral frontal cortex may be related to Romaji conversion in Japanese individuals. Therefore, diaschisis of the left dorsolateral frontal cortex due to subcortical lesions may have impaired Romaji conversion, leading to dystextia and dystypia, in this patient.

Abstract Background Corticobasal syndrome (CBS) is a neurodegenerative disease diagnosed based on clinical manifestations such as asymmetrical parkinsonism, limb apraxia, and speech and language impairment. The background pathology of CBS is commonly a variety of proteinopathies, but association with cerebrovascular disease has also been reported. Foix-Chavany-Marie syndrome (FCMS) is a rare neurological disorder characterized by facio-pharyngo-glossal diplegia with automatic-voluntary movement dissociation presenting with bilateral paresis of the facial, lingual, pharyngeal and masticatory muscles. FCMS is commonly attributable to stroke. Transactive response DNA binding protein of 43 kD (TDP-43) proteinopathy is also known as the pathological background of FCMS, while the pathological background of the majority of CBS cases consists of diverse tauopathies instead of TDP-43 proteinopathy. In this report, we describe a case mimicking FCMS that was finally diagnosed as CBS with suggested 4-repeat tauopathy. Case presentation A 68-year-old female started experiencing difficulty speaking followed by difficulty writing, and especially texting, several years before her visit. Her impairment had been gradually worsening, and she came to our hospital. On neurological examination, she demonstrated the facial apraxia, frontal lobe dysfunction, and upper motor neuron signs. She presented some characteristics suggestive of FCMS. Her symptoms exhibited rapid progression and myoclonus, parkinsonism, and left-side dominant cortical sensory deficit occurred, resulting in the fulfillment of diagnostic criteria for CBS after 9 months. Tau PET imaging displayed notable ligand uptake in the brainstem, subthalamic nuclei, basal ganglia, and bilateral subcortical frontal lobe, suggesting that her pathological background was 4-repeat tauopathy. As a result of her progressive dysphagia, she became unable to eat and passed away after 12 months. Conclusion We hereby present an atypical case of CBS showing clinical features mimicking FCMS at first presentation. TDP-43 proteinopathy was suspected based on the clinical symptoms in the early stages of the disease; however, the clinical course and imaging findings including tau PET suggested that her pathological background was 4-repeat tauopathy.

Abstract Background This study aimed to assess the long-term effects of tofogliflozin, a sodium-glucose cotransporter 2 (SGLT2) inhibitor, on atherosclerosis progression and major clinical parameters in patients with type 2 diabetes lacking an apparent history of cardiovascular disease. Methods This was a prospective observational 2-year extension study of the “Using TOfogliflozin for Possible better Intervention against Atherosclerosis for type 2 diabetes patients (UTOPIA)” trial, a 2-year randomized intervention study. The primary endpoints represented changes in the carotid intima-media thickness (IMT). Secondary endpoints included brachial-ankle pulse wave velocity (baPWV) and biomarkers for glucose metabolism, lipid metabolism, renal function, and cardiovascular risks. Introduction: Pediatric cystic nephroma (PCN) is a rare benign renal neoplasm. The current treatment is a nephrectomy. We report a case of successful enucleation for a giant PCN in an infant. Case presentation: A 10-month-old female was admitted to our department due to a left upper quadrant bulge. Physical examination was unremarkable. Urinalysis showed moderate white blood cells in the sediment. Tumor markers and other general blood tests showed no abnormalities. Ultrasonography showed an enlarged left kidney with a 6 × 5 cm multiseptated cystic mass. Computed tomography revealed a multiloculated homogeneous cystic mass in the left kidney without calcifications or solid components. Magnetic resonance imaging showed a hyperintense multicystic mass. She was diagnosed with pediatric cystic nephroma based on the imaging and clinical features. The initial decision was to follow her up and only intervene if symptoms appeared. Three months later she developed abdominal distention and loss of appetite, so we decided to remove the mass. Due to its benign nature, we opted for an enucleation rather than a nephrectomy. The mass size was 90 × 85 × 70 mm. We encountered significant bleeding during the dissection but were eventually able to save the normal renal parenchyma safely. The pathology confirmed a PCN. At two years of follow up there has been no recurrence and the left kidney remains normal. Conclusion: PCN is a benign cystic lesion of the kidney that can potentially be managed by enucleation rather than nephrectomy.

This study aimed to investigate the effects of administration of xanthan gum-based fluid thickener on the regulation of postprandial blood glucose level, gene expression in the gastrointestinal tract, and gut microbiome. Six-week-old Sprague-Dawley rats were divided into two groups: with (Th group) and without (Co group) the administration of a xanthan gum-based fluid thickener for 5 wks. Blood glucose levels at 60 and 90 min after glucose administration were substantially decreased in the Th group. Glp1 and Glp1r expression in the ileum was significantly upregulated after continuous administration of thickener. RNA-seq analysis revealed that the cholesterol homeostasis, fatty acid metabolism, and glycolysis gene sets were enriched in the ileum of Th group. Microbial composition in the gut was altered after administrating the fluid thickener, and relative abundance in Erysipelotrichales and Christensenellaceae showed positive correlation with Glp1 and Glp1r expression in the ileum. Xanthan gum-based fluid thickener may ameliorate glucose/lipid metabolism.

Preventing adverse pregnancy outcomes is crucial for maternal and child health. Periodontal disease is a risk factor for many systemic diseases including adverse pregnancy outcomes, such as preterm birth and low birth weight. In addition, the administration of the periodontopathic bacterium Porphyromonas gingivalis exacerbates obesity, glucose tolerance, and hepatic steatosis and alters endocrine function in the brown adipose tissue (BAT). However, the effects of having periodontal disease during pregnancy remain unclear. Thus, this study investigates the effect of P. gingivalis administration on obesity, liver, and BAT during pregnancy. Sonicated P. gingivalis (Pg) or saline (Co) was injected intravenously and administered orally to pregnant C57BL/6J mice three times per week. Maternal body weight and fetal body weight on embryonic day (ED) 18 were evaluated. Microarray analysis and qPCR in the liver and BAT and hepatic and plasma triglyceride quantification were performed on dams at ED 18. The body weight of Pg dams was heavier than that of Co dams; however, the fetal body weight was decreased in the offspring of Pg dams. Microarray analysis revealed 254 and 53 differentially expressed genes in the liver and BAT, respectively. Gene set enrichment analysis exhibited the downregulation of fatty acid metabolism gene set in the liver and estrogen response early/late gene sets in the BAT, whereas inflammatory response and IL6/JAK/STAT3 signaling gene sets were upregulated both in the liver and BAT. The downregulation of expression levels of Lpin1, Lpin2, and Lxra in the liver, which are associated with triglyceride synthesis, and a decreasing trend in hepatic triglyceride of Pg dams were observed. P. gingivalis administration may alter lipid metabolism in the liver. Overall, the intravenous and oral administration of sonicated P. gingivalis-induced obesity and modified gene expression in the liver and BAT in pregnant mice and caused fetuses to be underweight.
