Abstract

Aims: Approximately 5.7% of potential subcutaneous implantable cardioverter-defibrillator (S-ICD) recipients are ineligible by virtue of their vector morphology, with higher rates of ineligibility observed in some at-risk groups. Mathematical vector rotation is a novel technique that can generate a personalized sensing vector, one with maximal R:T ratio, using electrocardiogram (ECG) signal recorded from the present S-ICD location.Methods and Results: A cohort of S-ICD ineligible patients were identified through ECG screening of ICD patients with no ventricular pacing requirement and their personalized vectors were generated using ECG signal from a Holter monitor. Subcutaneous ICD eligibility in this cohort was then recalculated. In a separate cohort, episodes of arrhythmia were recorded in patients undergoing arrhythmia induction, and arrhythmia detection in standard S-ICD vectors was compared to rotated vectors using an S-ICD simulator. Ninety-two participants (mean age 64.9 ± 2.7 years) underwent screening and 5.4% were found to be S-ICD ineligible. Personalized vector generation increased the R:T ratio in these vectors from 2.21 to 7.21 (4.54-9.88, P < 0.001) increasing the cohort eligibility from 94.6% to 100%. Rotated S-ICD vectors also showed high ventricular fibrillation (VF) detection sensitivity (97.8%), low time to VF detection (6.1 s), and excellent tachycardia discrimination (sensitivity 96%, specificity 88%), with no significant differences between rotated and standard vectors.Conclusion: In S-ICD ineligible patients, mathematical vector rotation can generate a personalized vector that is associated with a significant increase in R:T ratio, resulting in universal device eligibility in our cohort. 9. Subcutaneous implantable cardiac defibrillator : a personalised approach

Abstract

Sudden cardiac death (SCD) remains one of the leading causes of death in the modern world with most of these deaths being attributed to ventricular arrhythmias. Implantable cardiac defibrillators (ICDs) are well established treatment and are recommended by the international guidelines for prevention of sudden cardiac death triggered by ventricular arrhythmias in high-risk populations. But they are not risk-free, and traditional transvenous ICDs are associated with long-term complications with potentially fatal consequences. The subcutaneous ICD (S-ICD) was designed utilising a totally extra-thoracic approach avoiding the complications which have been associated with transvenous ICDs (TV-ICD). The results of clinical trials demonstrated the efficacy of the S-ICD systems in recognising and treating ventricular arrythmias with fewer lead-related complications when compared to the TV-ICDs. However, the downside of the S-ICDs is that unlike TV-ICDs, they are unable to provide bradycardia pacing or Anti-tachycardia pacing (ATP) therapy to terminate ventricular arrythmias painlessly without the need to deliver a shock. Also, they exhibit a relatively higher rate of inappropriate shocks when compared with TV-ICDs. Most of these shocks can be attributed to Twave oversensing (TWO), an inherent risk to the sensing mechanism of the S-ICD. Not all patients are eligible for S-ICDs and mandatory screening of all potential candidates following device manufacturer guidelines helps identify eligible patients based on their underlying ECG morphology. Variable rates of screening success and S-ICD eligibility are reported in the literature. In this thesis I will start by reporting a retrospective analysis of S-ICD eligibility using current recommended screening practices at a tertiary centre for cardiac devices (University Hospital of Southampton). I will then proceed to suggest adopting a different approach towards screening of S-ICD candidates which considers the dynamicity of the ECG signal. I will explore the role of applying prolonged screening using Holter monitors in a wide range of patients' cohorts and prove there is variation in the S-ICD eligibility overtime which can explain oversensing and inappropriate shocks in S-ICDs despite current screening practices. I will also introduce and explain a novel technique utilising artificial intelligence and deep learning methods which has the potential to be applied to clinical practice to help identify S-ICD eligible patients as well as guide vector selection in S-ICD recipients. I will justify choosing R:T ratio as the main determinant of S-ICD eligibility and validate the novel deep learning methodology used in my studies by comparing the outcomes to those produced by the "gold standard" S-ICD simulator. I will then proceed to propose targeting less strict R:T ratios in S-ICD screening in vectors that prove to be stable with prolonged screening. I will then compare the eligibility rates for S-ICD using different R:T ratios, proposing that it is reasonable to revisit the S-ICD Screening thresholds if we adopt prolonged screening approaches. Afterwards, I will address the inability of S-ICDs to provide pacing therapy and the need to coimplant pacemaker devices to cover pacing therapy when clinically indicated. I will highlight that the effect of pacing on the S-ICD sensing has not been well studied before. I will proceed by first introducing a simple radiological method to define the LP position, then demonstrate that there is no effect of LP position on short- and long-term LPs performances. Afterwards I will show that pacing regardless of the pacing site has a significant effect on the R:T ratio, one of the main determinants of S-ICD eligibility and increases the risk of S-ICD oversensing. However, I will conclude that with adoption of personalised approach towards device therapy, it is theoretically feasible to utilise concomitant device therapies in most patients, without increasing the risk of adverse clinical events. In summary, I believe that extravascular cardiac devices are soon to establish themselves as the new standard of care to provide defibrillation protection and pacing therapy. Current issues with S-ICDs can be overcome by incorporating efficient artificial intelligence methods to help with more accurate and yet efficient screening for better patient selection. Together with adopting a personalised approach, higher S-ICD eligibility can be achieved, and the risk of inappropriate shocks can be mitigated.

10. Complexity and synchronisation analysis of electroencephalogram signals for early prediction of neurodevelopmental disorders

Abstract

Neurodevelopmental disorders (NDDs) are a group of neurological disorders emerging during early development and impacting higher-level brain function. Early identification of NDDs raises the possibility of improving outcomes. However, NDDs are still clinically detected through a subjective evaluation approach that lacks biological evidence and requires an extended period to identify the disorder. This thesis aims to identify biomarkers from the electroencephalogram (EEG) for abnormal brain dynamics and utilise these biomarkers for NDDs prediction. Nonlinear time series analysis methods, mainly complexity (entropies) and synchronisation (node degree of the phase-lag index) features are investigated for this purpose due to their ability to reflect brain dynamics. A machine learning framework that combines advanced nonlinear EEG processing methods for NDDs identification has been developed and evaluated. The proposed framework was first used with a dataset of children with autism spectrum disorder (ASD) and controls to validate its classification efficacy. This exploration resulted in high performance, suggesting complexity features as biomarkers for ASD prediction. The framework was then used to explore at-birth EEG characteristics of infants with Hypoxic-Ischemic Encephalopathy (HIE) who later developed cerebral palsy (CP). High performance has been reached, and the proposed features were reported as potential biomarkers for early CP prediction. A regression model has also been developed to explore the correlation between the EEG characteristics of the HIE infants and their two-year cognitive scores. The results suggest the proposed features as biomarkers for early cognitive function prediction. Finally, the framework was used with a dataset of individuals with a major depressive disorder to validate its ability to predict their depression severity. Compared to the state-of-the-art research, an acceptable regression performance has been reached, and the proposed features have been suggested as biomarkers for depression severity prediction. This work lays the foundation for evidence-based decision-making applications for early prediction of CP and cognitive outcomes of infants with HIE, paving the way for establishing tailored intervention programs at an appropriate point during development to improve the outcomes.

11. EEG-based brain connectivity analysis for identifying neurodevelopmental disorders

Abstract

This dissertation aims to identify the neurological biomarkers that could assist in providing reliable, automated and objective prediction of neurodevelopmental disorders (NDDs) in early infancy. Quantitative electroencephalography analysis (qEEG), mainly phase synchronisation-based functional brain connectivity estimated using phase locking value (PLV) and weighted phase lag index (WPLI), were investigated to deduce whether it can be used for the early prediction of such disorders. The resulting connectivity network was quantitatively characterised using complex graph-theoretical features, namely transitivity, global efficiency, radius, diameter, and characteristic path length. These features were then fed into the machine learning algorithms such as linear discriminant analysis (LDA), support vector machine (SVM), decision tree and k-nearest neighbour to examine their discriminant capability in classifying /predicting NDDs. The proposed framework has gained initial validation in classifying autism spectrum disorders (ASD) from an experimentally obtained EEG data set of 24 children. Then, the framework was utilised to predict the appearance of cerebral palsy (CP) at two years of age. The EEG data were recorded within the first week after birth from a cohort of infants born with hypoxic-ischaemic encephalopathy (HIE). The exploration results revealed that the proposed analytical methodology successfully predicted the infants that would develop CP with a performance of 84.6% accuracy, 83% sensitivity, 85% specificity, 84% balanced accuracy and 0.85 area under the curve (AUC) in the delta band, with a close result also obtained in the theta and alpha bands. The WPLI and graph parameters were then used to predict the cognitive scores of infants born with HIE by developing the regression framework correlating these EEG features and a cognitive profile completed in a follow-up assessment at two years of age. The regression analysis showed that the radius feature yielded the best performance (root mean square error (RMSE)= 16.78, mean absolute error (MAE)= 12.07 and R-squared= 0.24). Although this study has successfully demonstrated that the qEEG features could be considered potential biomarkers for identifying the brain deficits causing the NDDs, it has a certain limitation due to the size of the data set. It needs to be validated on large trials with a statistically significant population.

12. Signal processing of electrocardiogram for arrhythmia prediction and classification

Abstract

The prevalence of Cardiovascular Diseases (CVD) currently poses a significant global burden to the healthcare systems. Arrhythmia is a condition associated with CVD that can lead to death in chronic CVD patients or even to an apparent healthy person. Electrocardiogram (ECG) is the first-point diagnosis tool of arrhythmia. Currently, Implantable Cardioverter Defibrillator (ICD) and its subcutaneous version (S-ICD) are used for treating fatal arrhythmias. However, these devices can only detect arrhythmia once it started and therefore in the case of fatal arrhythmias such as Ventricular Fibrillation they are not very effective in saving lives. Recently, ECG has been applied to predict impending arrhythmias. However, their prediction and classification performance need to be improved to become acceptable in day-to-day clinical use. Also, such predictive methods need to be incorporated within the framework of ICD itself. This dissertation addresses this issue and provide a solution by improving the predictive capability of ECG and then integrating it with S-ICD. We first proposed two automated algorithms for precise delineation of ECG fiducial points based on the time- domain Hierarchical clustering and the time-frequency-domain Discrete Wavelet Transform (DWT). Then we developed a new algorithm based on the Phase Space Reconstruction (PSR) to overcome the T-wave over-sensing problem that typically plagues efficacy of ICDs, more specifically the S- ICD system. After that, we reported a risk index based on PSR and fuzzy c-means clustering to predict an impending fatal Ventricular Arrhythmias (VA) and its classifications (four different types of VA). The main feature of the proposed method is that for the first time it has been shown that it is not only possible to predict an impending arrhythmia sufficiently before its actual occurrence in time but also is possible to classify the type of arrhythmia before it actually occurs (1 min after the prediction time point). We further extended these methods for classifying other types of non-fatal arrhythmias such as, Atrial Fibrillation (AF) which are indicative to the risk of stroke in a patient suffering from chronic CVD condition. The methods proposed here could be used in day-to-day clinical practice after rigorous clinical trial to advance technologies such as ICD and S-ICD that can help to pre-empt the occurrence of fatal ventricular arrhythmia - a main cause of Sudden Cardiac Death (SCD).
