Abstract

In our study, it was aimed to compare the volume of accelerated partial breast irradiation in patients diagnosed with early stage breast cancer in CyberKnife target and volume targeted virtual plans (APBI). A virtual plan was created in CyberKnife system for 5 patients who received radiotherapy in our unit, 6 Gy per day for Iris, fixed and multi-leaf collimator (MLC) and 30 Gy in total. Dosimetric parameters for three different collimators were analyzed according to the constraints of the NSABP-39/RTOG 0413 protocol. In the plans, it was ensured that critical organs were protected and PTV received 95% of the prescribed dose. The prescribed dose was defined by a minimum 80% isodose curve. Homogeneity index (HI), conformity index (CI), treatment time (min), monitor unit (MU) and doses received by critical organs were compared. The mean HI values for iris, fixed collimator, and MLC were 1.2- 1.2- 1.19, mean CI values were 1,08-1,06-1,07, mean treatment time was 47,6- 52,2- 25,4 (min) and MU values were found to be 19736,2-2026,.3- 7161,3 on average. Mean MU and duration of treatment were found in the lowest MLC. No significant difference was found for HI, CI. As a result of the comparison of the plans, a significant difference was found for MU and duration of treatment (p< 0,05).ipsilateral breast30 and V15 were found at the lowest MLC.for lung and heartDmax was no significant difference between. As a result, the target volume received the desired dose in each collimator. The contralateral breast and contra-lateral lung doses were the lowest in Iris. Fixed collimator was found to be more suitable for cardiac doses. But these values did not make a significant difference. In the fixed collimator (52,2 min), it may cause difficulties in clinical applications due to the long treatment time. Planların karşılaştırılması sonucunda MU, tedavi süresi için anlamlı fark bulunmuştur (p< 0,05). Ortalama MU ve tedavi süresi, en düşük ÇYK’da bulunmuştur Hİ, Kİ için anlamlı fark bulunmamıştır. Aynı taraf memenin V30 ve V15 değerleri en düşük ÇYK’da çıkmıştır. Akciğer ve kalp için Dmax değerleri arasında anlamlı bir fark bulunmadı. Sonuç olarak, hedef hacim her kolimatörde istenen dozu almıştır. Karşı meme ve karşı akciğer dozları en düşük Iris’te bulundu. Kalp dozları için sabit kolimatörün daha uygun olduğu bulundu. Fakat bu değerler anlamlı olarak fark yaratmadı. Sabit kolimatörde (52,2 dk), tedavi süresi uzun olduğu için klinik uygulamalarda zorluk çıkarabilir. CyberKnife ile meme SBRT uygulamalarında kolimatör seçimi, tümör boyutu, kritik organlara yakınlık durumu ve tümör lokalizasyonuna bağlı olarak değişebilir.

Abstract

The aim of this study was to examine the prognostic factors and treatment outcomes of cervical esophageal carcinoma (CEC) patients who underwent definitive chemoradiotherapy (CRT). The clinical data of 175 biopsyconfirmed CEC patients treated with definitive CRT between April 2005 and September 2021 were retrospectively analyzed. The prognostic factors predicting overall survival (OS), progression-free survival (PFS), and local recurrence-free survival (LRFS) were assessed in uni- and multivariable analyses. The median age of the entire cohort was 56 years (range: 26-87 years). All patients received definitive radiotherapy with a median total dose of 60 Gy, and 52% of the patients received cisplatin-based concurrent chemotherapy. The 2-year OS, PFS, and LRFS rates were 58.8%, 46.9%, and 52.4%, respectively, with a median follow-up duration of 41.6 months. Patients' performance status, clinical nodal stage, tumor size, and treatment response were significant prognostic factors for OS, PFS, and LRFS in univariate analysis. Non-complete treatment response was an independent predictor for poor OS (HR = 4.41, 95% CI, 2.78-7.00, p < 0.001) and PFS (HR = 4.28, 95% CI, 2.79-6.58, p < 0.001), whereas poor performance score was a predictor for worse LRFS (HR = 1.83, 95% CI, 1.12-2.98, p = 0.02) in multivariable analysis. Fifty-two patients (29.7%) experienced grade II or higher toxicity. In this multicenter study, we demonstrated that definitive CRT is a safe and effective treatment for patients with CEC. Higher radiation doses were found to have no effect on treatment outcomes, but a better response to treatment and a better patient performance status did. [ABSTRACT FROM AUTHOR]

Abstract

OBJECTIVE: The objective of the study was to assess nutritional status among radiation oncology outpatients. METHODS: A total of 394 consecutive oncology outpatients who were screened for nutritional status through nutritional risk screening (NRS) 2002 during their admission to 12 radiation oncology centers across Turkey in October 2018 were included in this cross-sectional screening study. Data on cancer type, time of diagnosis (former and newly diagnosed), and NRS 2002 scores were recorded. Patients with NRS 2002 scores =3 were considered to be at risk of malnutrition necessitating the provision of nutritional intervention. NRS 2002 scores were evaluated in the overall study population as well as according to cancer types and time of diagnosis. RESULTS: NRS 2002 assessment (scores ≤3) revealed 133 (33.8%) patients to be at risk for malnutrition. The highest rates for malnutrition risk were noted for patients with lung cancer (43.8%), head-and-neck cancer (43.5%), and gastrointestinal tumors (42.7%). Poor nutritional status was evident in 36.0% and 25.3% of newly diagnosed and former cancer patients, respectively (p=0.067). CONCLUSION: This screening study revealed malnutrition risk and need for nutritional intervention in 33.8% of cancer patients, including 36.0% of newly diagnosed patients. A need for nutritional intervention was evident in two out of every five patients with newly diagnosed cancer, emphasizing the importance of screening for nutritional risk in every cancer patient at the time of initial diagnosis given the role of appropriate multimodal nutritional intervention before anti-cancer therapy in the long-term success. 15. Economical and easy methods of smoke stain removal from maxillofacial prostheses.

Abstract

Aim: Service life of a maxillofacial prosthesis is too short especially because of color instability. This study aimed to investigate the effects of hand soap and chlorhexidine gluconate mouthwash on the color of maxillofacial prosthesis after exposure of cigarette smoke. Materials and Methods: Sixty specimens were fabricated from a maxillofacial silicone material. Two groups (n=20) were pigmented intrinsically mimicking dark and fair tone skin and one group was not pigmented for control. All the specimens were exposed to cigarette smoke in a closed apparatus designed for the study. Color measurements of the specimens were performed before and after smoke exposition by using a spectrophotometer. Half of all the specimens (n=30) consisting of dark, fair and non-pigmented ones was rubbed with liquid hand soap and the other half was immersed in chlorhexidine gluconate mouthwash (0.12% chlorhexidine digluconate). Color measurement was repeated after cleaning process. Color differences were calculated both CIELab (∆E*ab) and CIEDE2000 (∆E00) color measurement formulas. Results: Color groups had significantly different color change values in terms of ∆E00 and ∆E*ab after exposition to smoke and cleaning methods (p < 0.001). The effect of soap and chlorhexidine on color change values did not differ from each other (p=0.284 for ∆E00, p=0.312 for ∆E*ab). The interaction of color groups with cleaning methods was statistically insignificant (p=0.962 for ∆E00, p=0.550 for ∆E*ab). ∆E00 and ∆E*ab values showing color alteration between initial color and after cleaning were significantly different in groups (p < 0.001). Conclusion: Cleaning process using hand soap and chlorhexidine gluconate mouthwash succeed to return the color of stained silicone to the initial color. [ABSTRACT FROM AUTHOR]

Abstract

Bu araştırmada tüm vücut ışınlaması (TVI) ve tüm kemik iliği ışınlaması (TKI) için, Volümetrik Ark Tedavi (VMAT) tekniği kullanılarak, hedef hacim ve kritik organ dozlarının karşılaştırılması amaçlanmıştır. Çalışmada, insan doku ve organ yoğunluklarına sahip, rando fantom kullanıldı. VMAT planları günlük fraksiyon dozu 6 Gy olmak üzere toplam doz 12 Gy olacak şekilde, oluşturuldu. Hedef hacimler ve risk altındaki organların (RAO) aldığı doz değerleri karşılaştırıldı. VMAT-TVI için ortalama akciğer dozu, 7.6 Gy ve lens dozu 4.4 Gy iken, VMAT-TKI için, RAO’nun dozları 2.7 ila 7.15 Gy aralığındadır. Ayrıca her iki plan için tedavi öncesi kalite kontrolü Octavius 4D fantom kullanılarak, planlanan ve verilen dozlar arasında doz doğruluğu, her 3 düzlemde 3mm ve %3 kriterleri ile gama indeks analizi, her iki tedavi planı için ≤1 olduğu saptanmıştır. Planlanan ve uygulanan tedavi arasında oldukça hassas doz dağılımı olduğu gözlemlendi. Her iki tedavi tekniğinde de hedef volüm istenilen dozu aldığı, fakat VMAT-TVI tekniğinde, doz homojenliği ve tedavi uygulama tekniklerinin iyileştirilmesi gerektiği sonucuna varıldı. TKI ile akciğer dışındaki diğer RAO'ların da daha iyi korunabildiği saptandı. Çalışmamızın sonucunda, VMAT'ın gelecekte geleneksel tekniklere göre hem TVI hem de TKI uygulaması için uygun bir teknik olabileceği görüşüne varıldı. It is aimed to compare target volume and critical organ doses using Volumeric arc therapy (VMAT) technique for Whole body irradiation (TVI) and Whole bone marrow irradiation (TKI). In the study, a random phantom with human tissue and organ densities was used.. VMAT plans were created with a daily fraction dose of 6 Gy and a total dose of 12 Gy. Target volumes and dose values received by critical organs were compared. Treatment plans The mean lung dose for VMAT-TVI is 7.6 Gy and the lens dose is 4.4 Gy, while organs at risk (OAR) doses for VMAT-TKI range from 2.7 to 7.15 Gy. In addition, pre-treatment quality control for both plans, dose accuracy between planned and administered doses using the Octavius 4D phantom, gamma index analysis with 3mm and 3% criteria in all 3 planes, we found that ≤1 for both treatment plans. We observed a very precise dose distribution between the planned and administered treatment. It was concluded that in both treatment techniques, the target volume received the desired dose, but in the VMAT-TBI technique, dose homogeneity and treatment application techniques should be improved. It was determined that other RAOs other than the lung were also better protected with TMI. As a result of our study, it was concluded that VMAT may be a suitable technique for both TBI and TMI application compared to traditional techniques in the future.
