Abstract

This article reports the development of a versatile high-speed reciprocating drive system (HRDS) with interchangeable probe heads to characterize the edge plasma region of ADITYA-U tokamak. This reciprocating probe drive system consisting of Langmuir and magnetic probe heads, is designed, fabricated, installed, and operated for studying the extent of fuel/impurity gas propagation and its influence on plasma dynamics in the far-edge region inside the last closed magnetic flux surface (LCFS). The HRDS is driven by a highly accurate, easy-to-control, dynamic, brushless, permanently excited synchronous servo motor operated by a PXI-commanded controller. The system is remotely operated and allows for precise control of the speed, acceleration, and distance traveled of the probe head on a shot-to-shot basis, facilitating seamless control of operations according to experimental requirements. Using this system, consisting of a linear array of Langmuir probes, measurements of plasma density, temperature, potential, and their fluctuations revealed that the fuel gas-puff impact these mean and fluctuating parameters up to three to four cm inside the LCFS. Attaching an array of magnetic probes to this system led to measurements of magnetic fluctuations inside the LCFS. The HRDS system is fully operational and serves as an important diagnostic tool for ADITYA-U tokamak.

Abstract

In this paper, we report the excitation of coherent density and potential fluctuations induced by magnetohydrodynamic (MHD) activity in the edge plasma region of ADITYA-U Tokamak. When the amplitude of the MHD mode, mainly the m/n = 2/1, increases beyond a threshold value of 0.3-0.4 %, coherent oscillations in the density and potential fluctuations are observed having the same frequency as that of the MHD mode. The mode numbers of these MHD induced density and potential fluctuations are obtained by Langmuir probes placed at different radial, poloidal, and toroidal locations in the edge plasma region. Detailed analyses of these Langmuir probe measurements reveal that the coherent mode in edge potential fluctuation has a mode structure of m/n = 2/1 whereas the edge density fluctuation has an m/n = 1/1 structure. It is further observed that beyond the threshold, the coupled power fraction scales almost linearly with the magnitude of magnetic fluctuations. Furthermore, the rise rates of the coupled power fraction for coherent modes in density and potential fluctuations are also found to be dependent on the growth rate of magnetic fluctuations. The disparate mode structures of the excited modes in density and plasma potential fluctuations suggest that the underlying mechanism for their existence is most likely due to the excitation of the global high-frequency branch of zonal flows occurring through the coupling of even harmonics of potential to the odd harmonics of pressure due to 1/R dependence of the toroidal magnetic field.

Medeniyetler gelişimlerini sahip oldukları geleneklere borçludurlar. Bu geleneklerin sağlamlığı o medeniyetin devamının garantisi ile doğrudan ilgilidir. Mûsikî böyle gelenekli bir sanat alanı olarak her medeniyetin mensupları için Allah vergisi bir meşgale alanıdır. Mûsikîdeki kadim geleneğin sağ-lamlığı bazen yeniliklere karşı direnç oluşturmuştur. Bu gerçeklere karşın zaman zaman deha diye nitelendirilen şahsiyetler tebarüz ederek bu kadim geleneğe yenilik katmıştır. Çoğu zaman bu katkı-lar kendi dönemlerinde sancılı olmuştur. Bazen katkı sahibi şahıslar dışlanmış veya ötekileştirilmiş-tir. Fakat anlaşılmaktadır ki, o katkı sahiplerinin karşısında muhalefet eden kişiler çoğunlukla bir müddet sonra hayranlık içinde yapılan katkıları kabullenmiş ve hatta bazen en güçlü savunucuları haline gelmiştir. Joseph A. Schumpeter ortaya attığı “yaratıcı yıkıcılık” tanımı ile bu şahsiyetlerin faaliyetlerinin de izah edilebilinmesine olanak vermiştir. Bekir Şahin Baloğlu da “yaratıcı yıkıcılık” tanımı üzerinden hareketle Tanbûrî Cemil Bey ve Niyazi Sayın’ın gelenekteki yerine değinerek bu çalışmanın ortaya çıkmasına kaynaklık etmiştir. Tanbûrî Cemil Bey ve Niyazi Sayın'ın çağdaşlarına nazaran ortaya koydukları icra ile uygulamış oldukları "yaratıcı yıkıcılık" kadim geleneği bozmamış bilakis ihya etmiştir. Bu makalede Tanbûrî Cemil ve Niyazi Sayın arasındaki güçlü bağ icra ettikleri tâhir-bûselik peşrevi üzerinden tartışılmış ve sonuç olarak Niyazi Sayın’ın icrasında her ne kadar Cemil Bey’in tavrından izler taşıdığı bölümler olsa da icranın bütününde kendine has icra özellikle-riyle eseri özgün bir şekilde yorumladığı görülmüştür.

5. ANTIBIOTIC LOCK THERAPY FOR PORT CATHETER-RELATED INFECTIONS OF CHILDREN WITH ACUTE LEUKEMIA

Abstract

Background: Port catheters facilitate the administration of chemotherapy, antibiotics, blood products, fluid and parenteral nutritional support to pediatric patients with hematological malignancies. However, as its use has become widespread, local, and systemic catheter-related infections have emerged as important causes of morbidity and mortality. In our study, we aimed to evaluate the success of antibiotic lock therapy in port catheter- related infections of pediatric patients followed up with acute leukemia. Methods: Port catheter cultures taken from a total of 182 pediatric patients with acute lymphoblastic/myeloblastic leukemia who were followed up at Ankara City Hospital Pediatric Hematology Clinic between August 2019 and August 2023 were evaluated retrospectively. Results: Bacterial growth was identified in 739 port catheter culture specimens of 182 patients. Closure or removal of the port was required in 91, and removal of the port catheters in 49 patients due to port catheter-related infections. Antibiotic lock therapy was started in 56 patients with bacterial growth in the port catheter. With antibiotic lock therapy, port catheter-related infections of 42 patients were eradicated, and their catheters began to be used again. As a result, the port catheter-related infections of 42 of 56 (75%) patients whose ports were closed and also received systemic antibiotherapy were eradicated, and no recurrence of infection was observed. Conclusion: Adding antibiotic lock therapy to systemic antibiotics in pediatric patients may be beneficial in terms of catheter salvage.

Abstract

Bu araştırma ev okuma ortamındaki aile içi etkileşimin çocukların okuma davranışları ve becerilerini nasıl değiştirdiğini ortaya çıkarmak amacıyla yürütülmüştür. Bu amaç doğrultusunda etkileşimli kitap okuma etkinlikleri öncesi, sırası, sonrası ve bu sürecin çocuklar ve ailelerin okuma davranışlarına, yaklaşımlarına ve çocukların okuma becerilerine etkisi gözlemlenerek meydana gelen değişim betimlenmeye çalışılmıştır. Araştırma çoklu durum çalışması yöntemiyle, ilkokul birinci sınıftan ikinci sınıfa geçen üç çocuk, bu çocukların aileleri ve sınıf öğretmenleri ile yürütülmüştür. Araştırmada veri toplama aracı olarak okuma metinleri, yapılandırılmamış ve yarı yapılandırılmış görüşmeler, katılımcı olmayan yapılandırılmamış gözlemler, video kayıtları ve araştırmacı günlüğü kullanılmıştır. Araştırmadan elde edilen veriler tümevarım analiz tekniği kullanılarak üç aşamada analiz edilmiş, analizler sonucunda; ailenin okumaya yaklaşımı, çocuğun okumaya yaklaşımı ve değişim olmak üzere üç tema; aile tipolojisi, aile tipolojisine kaynaklık eden unsurlar, çocuk tipolojisi, çocuğun okuma beceri ve davranışlarındaki değişim ve aile-çocuk tipolojisine kaynaklık eden unsurlarda gözlenen değişim şeklinde beş alt temaya ulaşılmıştır. Araştırma, ev okuma ortamının ve aile-çocuk etkileşiminin çocukların okuma becerileri üzerinde etkili olduğunu kanıtlar nitelikte bulgular sunmuştur. Süreçte çocukların okuma becerilerinde, kelime tanıma ve okuduğunu anlama düzeyinde, okumanın bilişsel ve duyuşsal boyutlarında bir dizi değişim gözlemlenmiştir. Etkileşimli kitap okuma eğitimi ve süreçte yapılan rehberlikle birlikte ailelerin okumaya karşı yaklaşım ve tutumlarının değiştiği, çocuklarının iyi bir okur olmasında kendilerine ait görev ve sorumluluklarına ilişkin söylem ve bakış açılarının değiştiği, bu değişimin çocuklarla birlikte okuma deneyimlerine yansıdığı görülmüştür. Çocukların okuma becerilerine yönelik değişimde ise kelime tanıma becerisinde görülen olumlu değişim okuduğunu anlama becerisinde basit anlama sorularındaki gelişim ile sınırlı kalmıştır. Etkileşimli okuma öncesi, sırası ve sonrasına yönelik betimlemeler ile durum açıklanmaya çalışılmış, bu süreçteki değişimin ailelerin okumanın okul temelli bir beceri olduğuna yönelik algıları ile birlikte yaklaşımlarını dolayısıyla ev okuma ortamını, ev okuma ortamındaki değişimin ise çocuktaki değişimi etkilediği görülerek çocuklardaki ve ailedeki değişimin nedenleri tartışılmıştır.

Abstract

Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.

Abstract

Background:Heart failure (HF) is a common condition that affects 1-3% of the general population. Its prevalence exhibits notable international and intranational disparities, partly explained by socioeconomic status, religion, ethnic diversity, and geographic factors. A comprehensive understanding of the epidemiological symptoms of HF in different regions of Türkiye has yet to be revealed.Aims:To examine epidemiological data from 2016 to 2022, focusing on crucial patient characteristics and geographical regions, to determine the incidence and prevalence of HF in Türkiye across seven diverse geographical regions.Study Design:A nationwide population-based retrospective cohort study.Methods:The comprehensive National Electronic Database of the Turkish Ministry of Health was used in this study to obtain data that covers the whole Turkish population from January 1, 2016, to December 31, 2022. The International Classification of Diseases-10 (ICD-10) codes were used to identify adults with HF (n = 2,701,099) and associated comorbidities. Türkiye is divided into seven geographically distinct regions. Epidemiological characteristics and survival data of these regions were analyzed separately. All-cause mortality was set as the primary outcome.Results:In , the total estimated prevalence of adult patients with HF is 2.939%, ranging from 2.442% in Southeastern Anatolia to 4.382% in the Black Sea Region. Except for the Eastern Anatolia Region, the three most often reported comorbidities were hypertension, dyslipidemia, and anxiety disorders. The rates of prescribing guideline-directed medical therapy (GDMT) for HF and other medications varied significantly. GDMT prescription rates were lowest in the Eastern Anatolia Region (82.6% for beta-blockers, 48.7% for RASi, 31.8% for mineralocorticoid receptor antagonists, and 9.4% for SGLT2i). The Mediterranean and Aegean regions had the highest median N-terminal brain natriuretic peptide (NT-proBNP) levels of 1,990,0 pg/ml (518.0-6,636,0) and 1,441,0 pg/ml (363.0-5,000,0), respectively. From 2016 to 2022, 915,897 (33.9%) of 2,701,099 patients died. The Eastern Anatolia Region had the lowest all-cause mortality rate of 26.5%, whereas the Black Sea Region had the highest all-cause mortality rate of 35.3%.Conclusion:Our real-world analysis revealed geographic disparities in HF characteristics, such as decreased mortality in socioeconomically challenged regions. Higher stress susceptibility in developed regions may increase the likelihood of adverse outcomes.

Abstract

Aim:The aim of our study was to compare clinical severity scores and classic spirometry with impulse oscillometry (IOS) results and thoracic high resolution computed tomography (HRCT) scores in children with cystic fibrosis (CF) in order to determine the utility of the latter approach in patient follow-up.Materials and Methods:CF patients over 6 years of age were included. Shwachman-Kulczycki score, underclassical spirometry and IOS were performed when not in acute exacerbation. Thoracic HRCT images obtained within the previous 6 months were evaluated using the Bhalla scoring system.Results:The mean age of the children studied (n=30) was 12.1±4.2 years and 40% were female. Pseudomonas aeruginosa (P. aeroginosa) was isolated from sputum cultures of 40% of the patients. Patients with forced expiratory volume in one second (FEV1) below 80% exhibited significantly higher (resistance) R5, R10 values and significantly lower (reactance) X5 values on IOS (p=0.03, 0.027, 0.006, respectively). Patients with P. aeruginosa had significantly lower FEV1, forced vital capacity, and forced expiratory flow (25-75) values in classic spirometry when compared with patients without P. aeruginosa (p=0.002, p=0.002, and p=0.005, respectively). P. aeruginosa-positive patients showed significantly higher R5 and lower X5 values (p=0.047, 0.046, respectively). 19. Incidental Learning of the Target Language System through a Semi-artificial Language

Abstract

In recent years, there have been studies on incidental learning utilizing semi-artificial languages at the sentence level focusing on specific grammatical features. This study investigates whether extended exposure to texts in a semi-artificial language would result in incidental learning of the target language system. A pretest/posttest experimental design was adopted. Learners in the experimental group were exposed to texts both in English and a semi-artificial language system while learners in the control group were only exposed to texts in English. A grammaticality judgment test, error analysis, and semi-structured interviews were used to evaluate learning and learners’ perceptions of their own learning. The experimental group scored significantly higher in the post-test than the pre-test in the grammaticality judgment test while the control group did not. Error analysis did not result in a significant difference across two writing tasks in either group in the number of errors, though the experimental group demonstrated a decrease in four error categories as opposed to one by the control group. The interview data showed an increased awareness of the structural difference between the two languages and also the target language system. Results may indicate that using semi-artificial languages facilitates awareness and learning of the L2 input.

Bulgular: İlgili literatür taramasının analizlerinde son kilometre koli teslimatı kavramıyla ilgili yayın sayısında önemli bir artış görülmektedir. 23. Antifungal Combination Therapy for Invasive Fungal Infections in Pediatric Leukemia Patients: An Observational Cohort Study

Abstract

Objective: The role of combination regimens in the treatment of invasive fungal infections (IFIs) in hematologic malignancies remains unclear. We aimed to demonstrate data about combined antifungal therapy (CAT) in pediatric leukemia patients with IFI. Method: Between January 2014 and December 2018, a total of 33 IFI episodes in 28 leukemia patients were analyzed retrospectively. Results: The study patients had acute lymphoblastic leukemia (n=19), acute myeloblastic leukemia (n=9), leukemia relapse (n=21; 75%) and remission (n=7; 25%). The patients were classified as having possible (n=26; 78.8%), probable (n=5; 15.1%) and proven IFI (n=2; 6.1%). Liposomal amphotericin B (LamB) was the most preferred agent (50%) in monotherapy. Mean duration of monotherapy was 12.84+-4.28 (5-24) days. LamB plus voriconazole (54.5%) was the most commonly preferred CAT. Mean duration of CAT was 42.36+-36.4 days, and this combination regimen was not changed throughout the treatment period (p=0.571). Total and IFI-related mortality rates were 60.7% vs 46.4%, respectively. Mortality rates were significantly higher in patients with relapse (p=0.006). Complete response was obtained in 81.8% of surviving patients. Side effects of CAT were observed at quite a low level. Conclusion: CAT has been found to be safe in the treatment of IFI episodes of pediatric leukemia. Uncontrolled underlying disease is the most important factor affecting the mortality rates in IFI.

Abstract

Objective: To investigate the clinical and epidemiological features and outcome of Chryseobacterium and Elizabethkingia spp. infections in children, together with antimicrobial susceptibilities. Methods: This retrospective study was conducted at a tertiary pediatric hospital in Turkey. All patients infected with Chryseobacterium/Elizabethkingia spp. among those presenting to Ankara City Hospital between March 2014 and March 2022 were included. Results: A total of 49 cases were included and 29 cases were identified as Elizabethkingia. The median age was 14 (0.2-185.0) months. The majority (89.8%) of these patients had an underlying disease, including malignancy (42.9%). Bacteremia (46.9%) and central line-associated bloodstream infection (28.6%) were the most common infections. The thirty-day all-cause mortality rate was 12.2%. The most commonly used antibiotics were ciprofloxacin and trimethoprim-sulfamethoxazole (TMP-SMX). Forty-five (91.8%) isolates were susceptible to ciprofloxacin, 44 (91.6%) to TMP-SMX, and 21 (87.5%) to levofloxacin. Conclusions: Chryseobacterium and Elizabethkingia spp. are emergent, nosocomial pathogens and the majority of cases were older than the neonatal period. They were mainly seen in patients with long hospital stays, indwelling devices, and those who have received antibiotics within the last month, especially carbapenems. In addition, they were associated with bloodstream infection and malignancy. The most commonly useful antibiotics according to the resistance patterns were ciprofloxacin and TMP-SMX.

Abstract

In this study, the slope stability problem, which occurred when the projected excavation works of a treatment plant has been started, has been examined. The aim of this study is to determine the conditions causing the slope failure and to make the slope stable again. Then, to ensure the stability of the whole slope after all the excavations work in the project area have been completed. For this purpose, firstly, the topographic features of the slope where the failure took place were determined and a model of the failure condition was created. Field and laboratory studies were used to obtain the data to be used in slope modeling. Models reflecting the current situation were analyzed considering static and dynamic conditions and a safe slope design was created. The analyzes are carried out by Slide V.6.0. software which is based on limit equilibrium methods and Bishop Method was preferred. As a result, it was determined that the reason of the failure occurred was the change of the soil’s physical and mechanical parameters due to precipitation. In the slope stability problem considered, the benching method has been proposed as an improvement method, and additionally, suggestions have been made for the disposal of surface waters by drainage methods.

Abstract

Fibromyalgia is a musculoskeletal pain syndrome characterized with the presence of sensitive points and widespread chronic pain and restricts one's daily life activities and decreases the quality of life. The etiology of fibromyalgia is unclear, but there are numerous hypotheses regarding the emergence and progression of the disease. Among these, the biopsychosocial model offers a holistic framework in which biological, psychological and social mechanisms play a role in the development of fibromyalgia. Since the etiology of the disease is not yet understood, effective methods for its treatment have not been found, thus, interventions aim to reduce the effect of fibromyalgia and increase psychological and physiological functionality. This article aims to examine the psyhcological intervention areas and methods for fibromyalgia patients. In the literature, it is concluded that pain avoidance beliefs and behaviors, self-efficacy, physical activity, sleep quality, self-compassion, emotional skills, coping strategies, personality, comorbid psychopathology are the factors related with the emergence of the disease, severity of pain, and adherence to treatment in fibromyalgia patients. In this context, Cognitive-Behavioral Therapy (CBT), physical exercises, sleep management Acceptance and Commitment Therapy (ACT), compassion focused psychotherapies, mindfulness based psychotherapies, emotion expression and emotion regulation techniques, and biofeedback are recommended as effective methods which can be included in the treatment plans of fibromyalgia patients. Moreover, considering the psychosocial factors in the assessment processes was essential to establish individualized treatment plans. In addition, the importance of multidisciplinary approaches in the treatment processes of fibromyalgia has been discussed within the framework of the biopsychosocial model.

Abstract

Hematopoietic stem cell transplantation is a curative therapy for pediatric patients with malignancies and blood or immune system disorders. However, recipients are at risk of opportunistic infections, including cytomegalovirus infection, due to immunosuppression from conditioning regimens. This retrospective study analyzed 439 pediatric hematopoietic stem cell transplantation patients to identify risk factors for cytomegalovirus infection and assess the impact of conditioning regimens. Significant factors included graft source, graft versus host disease, and pre‐transplant clinically significant cytomegalovirus infection. Myeloablative conditioning regimens and the use of anti‐thymocyte globulin increased the risk of clinically significant cytomegalovirus infection. Additionally, the risk of cytomegalovirus infection in patients was found to be statistically significant in the myeloablative subgroup analyses who received anti‐thymocyte globulin alone and those who received both anti‐thymocyte globulin and total body irradiation. In our study, total body irradiation did not significantly affect the risk of clinically significant cytomegalovirus infection. This study covers a 10‐year period and a large pediatric patient population from a single center. Our findings highlight the importance of understanding risk factors for clinically significant cytomegalovirus infection to guide preventive strategies in pediatric HSCT recipients. Abstract

Bu araştırmada Türkçe dersi sınavlarında yer alan soruların üst düzey düşünme becerileri açısından sahip olduğu görünümün betimlenmesi amaçlanmıştır. Araştırma doküman incelemesi yöntemiyle yürütülmüş, veri kaynağının belirlenmesinde amaçlı örnekleme yöntemlerinden ölçüt örnekleme kullanılmıştır. Araştırmanın veri kaynağını 8. sınıf düzeyindeki sınav kâğıtları oluşturmuştur. Araştırmaya LGS başarıları temelinde belirlenen birinci düzey okullar dâhil edilmiştir. Verilerin analizinde betimsel analiz gerçekleştirilmiştir. Araştırmadan elde edilen bulgular, sınavlarda güncellenmiş Bloom taksonomisinin bilişsel alan basamaklarına göre üst düzey düşünme becerilerine yönelik sadece yaratma basamağında soru hazırlandığını, çözümleme ve değerlendirme basamaklarına yönelik sorulara ise yer verilmediğini ortaya çıkarmıştır. PISA okuma becerileri yeterlik düzeylerine göre ise üst düzey okuma becerileri olarak kabul edilen 5 ve 6. düzeyde hiçbir soruya yer verilmediği tespit edilmiştir. Ayrıca incelenen sınavlarda en çok parçada anlam ve dil bilgisi sorularına en az ise sözel mantık, tablo/grafik inceleme ve görsel yorumlama sorularına yer verildiği tespit edilmiştir. Bu durum yazılı sınavlar ile uluslararası ortak sınavlar arasında uyumsuzluk olduğunu göstermektedir. Elde edilen bu sonuçlar alanyazın kapsamında tartışılmış ve öneri niteliğinde çözümleme, değerlendirme ve yaratma basamaklarına yönelik üst düzey soru örnekleri yazılmıştır.

Data generated by social media platforms such as Twitter provide a unique opportunity to examine large-scale public communication produced spontaneously and in real-time, and consequently, contribute to comprehensively understanding the complex set of attitudes towards refugees. The present study utilises a qualitative approach to explore host members’ attitudes towards refugees on social media during the global crisis. In particular, we examine perceptions of a broader population, Twitter users, and analyse public reactions towards Afghan refugees expressed within tweets during the pandemic in Turkey as a response to the 2021 Afghan refugee influx. We analysed and coded 2,686 tweets using qualitative content analysis based on four main categories: 1) Representations of Afghan refugees and immigrants, 2) main causes for rejecting them, 3) taking action against them and their supporters, and 4) positive attitudes towards them. Results highlighted the prevalence of negative perceptions about, dehumanisation of, and racism towards Afghan refugees in Turkey mainly due to perceived realistic, symbolic, demographic, security, moral, and health threats from them. However, the findings also showed there were positive, albeit very few, attitudes towards Afghan refugees in the form of solidarity, help and empathy. We discuss these findings in relation to the existing literature on host society members’ attitudes towards refugees and immigrants and why we need to qualitatively examine these attitudes on social media.

Abstract

Current data on fosfomycin usage in children are limited. We present data on the clinical use of intravenous (IV) fosfomycin in children. Hospitalized patients who received ≥3 days of IV fosfomycin between April 2021 and March 2023 were analyzed retrospectively. Forty-three episodes of infection in 39 patients were evaluated. The mean age of the patients was 5.35 (10 days to 17.5 years) years, and 54% were male. Infections were hospital-acquired in 79% of the episodes. Indications for fosfomycin were urinary tract infection (35%), bacteremia (32.6%), catheter-related bloodstream infection (16.3%), soft tissue infection (4.7%), sepsis (4.7%), surgical site infection (2.3%), burn infection (2.3%), and pneumonia (2.3%). Klebsiella pneumoniae was identified in 46.5% of the episodes, and a pan-drug or extensive drug resistance was detected in 75% of them. Carbapenem was used before fosfomycin at significantly higher rates in K. pneumoniae episodes (P  = .006). Most (88.5%) patients received fosfomycin as a combination therapy. Culture negativity was achieved in 80% of episodes within a median treatment period of 3 (2–22) days, which was significantly shorter in K. pneumoniae episodes (P  < .001). Treatment-related side effects were seen in 9.3% of the episodes. Side effects were significant after 3 weeks of treatment (P  = .013). The unresponsivity rate to fosfomycin was 23.3%. Nine (21%) of the patients who were followed up in the intensive care units mainly died because of sepsis (56%). IV fosfomycin is an effective agent in treating severe pediatric infections caused by resistant microorganisms. Fosfomycin can be used in various indications and is generally safe for children. Abstract

ChatGPT has surged interest to cause people to look for its use in different tasks. However, before allowing it to replace humans, its capabilities should be investigated. As ChatGPT has potential for use in testing and assessment, this study aims to investigate the questions generated by ChatGPT by comparing them to those written by a course instructor. To investigate this issue, this study involved 36 junior students who took a practice test including 20 multiple-choice items generated by ChatGPT and 20 others by the course instructor, resulting in a 40-item test. Results indicate that there was an acceptable degree of consistency between the ChatGPT and the course instructor. Post-hoc analyses point to consistency between the instructor and the chatbot in item difficulty, yet the chatbot's results were weaker in item discrimination power and distractor analysis. Abstract

This study aimed to retrospectively evaluate the baseline and follow‐up viral loads and viral clearance times in cases followed for asymptomatic and symptomatic congenital cytomegalovirus (cCMV) infection between August 2010 and August 2022. Among 93 cases, they had asymptomatic (n: 55) and symptomatic (n: 38). The median baseline blood viral load detected in the symptomatic cCMV (ScCMV) infection (13 054 IU/mL) was significantly higher than that of asymptomatic cCMV (AcCMV) infection (4636 IU/mL) (p < 0.013). There was no difference in median viral clearance times (75 and 90 days, respectively) in baseline viremic cases in the ScCMV and AcCMV infection groups. There were no differences in median baseline blood viral load (6930 IU/mL and 14 268 IU/mL, respectively) and median viral clearance times (75 and 85 days, respectively) between the 6‐week and 6‐month antiviral treatment group. No correlation was found between baseline blood viral load, clinical severity, and the number of systems involved. However, in initial viremic cases, the viral load threshold for a symptomatic case was 8856 IU/mL, with 85.7% sensitivity and 54.5% specificity. [ABSTRACT FROM AUTHOR]
