Objetivo: Avaliação de dados de encaminhamento de adolescentes portadores de doença falciforme (DF) para o ambulatório de adultos em um serviço único da cidade de São Paulo. Métodos: Foi realizada avaliação do prontuário eletrônico para coleta de dados de pacientes que completaram 18 anos entre os anos de 2021 a 2024. Em 2023, foi implementado o grupo de WhatsApp para informações sobre encaminhamentos de pacientes entre as equipes da Hematologia Pediátrica e de Adulto. Os dados agrupados em 2 grupos: Grupo 1 (G1): anos de 2021 e 2022 e Grupo 2 (G2): anos de 2023 e 2024. Dados coletados: ano de nascimento, genótipos, data da última consulta na pediatria e data da consulta no ambulatório de adultos. Resultados: Foram avaliados prontuários de 97 pacientes, 56 (58%) do G1 e 41 (42%) do G2, correspondendo a 2,3 pacientes/mês para ambos os grupos; destes 62 (64%) tinham anemia falciforme, 26 (27%) doença SC e 9 (9%) S-beta talassemia. No grupo total foram incluídos pacientes de outros serviços pediátricos (PCTE) (encaminhados via CROSS-Central de Regulação de Ofertas de Serviços de Saúde). No G1, observou-se 10 PCTE, com relação PCTE /internos (PCTI) de 1:5 e no G2 foram 4 PCTE com relação de 1:9. O tempo médio entre encaminhamento e consulta no ambulatório de adultos foi de 3 meses no G1 e no G2. Para PCTE, a mediana de tempo foi de 18 meses (IQ: 6;24) no G1 e 9 meses (IQ: 5;9,5) no G2. Houve diferença significante no tempo de encaminhamento (PCTI x PCTE) apenas no G1 (p < 0,0001). Sete pacientes do G1 abandonaram o tratamento (5 PCTI e 2 PCTE) e um do G2 (PCTI). Discussão: Segundo o Ministério da Saúde, o maior número óbitos em doentes com DF ocorreu na faixa etária de 20-29 anos. Estudos demonstram que o ambulatório de transição previne o abandono de tratamento desses adolescentes. A implementação do grupo de WhatsApp visou uma melhor comunicação entre hematologistas pediátricos e de adultos, mas não houve diferença no tempo de encaminhamento quando comparados G1 e G2, embora no G1 só tenham sido contabilizados pacientes que efetivamente chegaram ao ambulatório de adultos. Por isso, o abandono de tratamento observado no G1 pode ser ainda maior. No G2, devido à implantação do WhatsApp, foi possível busca ativa dos pacientes, diminuindo o abandono. Nos PCTE observou-se maior intervalo entre encaminhamento e consulta, e a pandemia de COVID pode ter contribuído nos achados do G1. Ressalta-se a diferença no número de PCTI e de PCTE: G1: 46 e 10; G2: 37 e 4, respectivamente. Na cidade de São Paulo existem 3 serviços pediátricos que não têm retaguarda para encaminhamento de seus pacientes e é sabido que a oferta de vagas pelos serviços que atendem adultos é restrita, o que nos leva a supor que muitos pacientes se encontram desassistidos. Conclusão: A implantação de um serviço de transição adequado para cada instituição é imprescindível para a diminuição do abandono do tratamento dos adolescentes com DF e, consequentemente, para maior sobrevida. No entanto, são necessárias políticas públicas que garantam que todos os adolescentes terão seu tratamento em serviços especializados em DF.

Objetivos: Identificar possíveis fatores de risco para a hipovitaminose D em crianças e adolescentes com doença falciforme em ambulatório na cidade de São Paulo. Material e métodos: Estudo transversal com crianças e adolescentes de três a dezoito anos com doença falciforme. Os pacientes foram agrupados em dois grupos quanto à deficiência de vitamina D (25-OH-D < 20 ng/mL) e foram avaliados diversos fatores de risco. As variáveis consideradas foram: idade, gênero, raça, gravidade da hemoglobinopatia, uso de hidroxureia, presença de danos crônicos associados à doença falciforme, consumo de vitamina D nos primeiros dois anos de vida, ingestão diária de fontes alimentares de vitamina D, dias de exposição solar, estação do ano da coleta de vitamina D, complicações agudas da doença falciforme (síndrome torácica aguda, crise vaso-oclusiva, infecção), hospitalização e atendimento de emergência. Resultados: Foram avaliados 60 pacientes, com média de idade de 10,80+4,21 anos. A prevalência de deficiência de vitamina D foi de 46,7% (21,02 ± 8,47 ng/mL). Na comparação entre os grupos com e sem deficiência de vitamina D, idade (p = 0,002) e estação do ano da coleta da 25-OH-D (p = 0,005) tiveram significância estatística. Idade apresentou OR 1,23 (IC95%:1,07;1,41 / p = 0,004) e estação do ano da coleta da 25-OH-D encontrou OR 5,21 (IC95%:1,58;17,14 / p = 0,007) para outono/inverno. Após regressão linear, notou-se associação para idade (β = -0,80/ IC95%:-1,29;-0,320/ p = 0,002), dias de exposição solar (β = 0,83/ IC95%:0,07;1,58/ p = 0,032) e coletas de vitamina D em outono/inverno (β = -7,94/ IC95%:-12,02;-3,85/ p = 0,032). Não houve significância estatística para variáveis de complicações agudas da doença falciforme, bem como para achados laboratoriais. Na regressão linear, a vitamina D apresentou associação significativa com hemoglobina (β = 0,025/ IC95%:0,012;0,037/ p = 0,041). Discussão: A redução do tempo de exposição solar durante a pandemia por COVID-19, referida por 51,9% dos pacientes, pode ter levado ao aumento da prevalência de deficiência de vitamina D nesta população. A prevalência de deficiência de vitamina D em países mais setentrionais, por sua vez, é maior quando comparados à países tropicais, resultado da influência da insolação durante as estações do ano. Uma absorção menor de vitamina D por exposição solar com a idade, somado à alta prevalência de suplementação nos primeiros dois anos de idade (78,3%), podem justificar a idade como fator de risco para hipovitaminose D. O estudo não encontrou nenhuma associação significativa das repercussões clínicas da doença falciforme com a deficiência de vitamina D. O valor de hemoglobina significativo na regressão linear sugere associação entre anemia e deficiência de vitamina D. Conclusão: A prevalência de hipovitaminose D nesta população é alta. Idade, estação do ano da coleta da 25-OH-D e dias de exposição solar apresentaram-se como fatores de risco para deficiência. Não houve significância estatística na avaliação das repercussões clínicas na hipovitaminose D.

Objetivos: realizar um levantamento da literatura para investigar as possíveis repercussões clínicas e laboratoriais da deficiência de vitamina D em crianças e adolescentes com doença falciforme. Material e métodos: Foi realizada uma revisão sistemática seguindo as recomendações do PRISMA. Estudos foram selecionados a partir de cinco bases de dados online (MEDLINE, LILACS, Cochrane, EMBASE, SCIELO). Os critérios de elegibilidade consideraram artigos observacionais publicados em inglês, português, espanhol e francês até 30 de novembro de 2023. A busca foi realizada entre setembro e novembro de 2023. Para a estratégia de busca, foram incluídos descritores utilizando Medical Subject Headings (MeSH) “vitamin D” E “sickle cell, anemia”. Para a seleção dos estudos, foram excluídas publicações não observacionais (revisões ou estudos de intervenção) ou aquelas que avaliavam suplementação de vitamina. Estudos duplicados nas plataformas de pesquisa acessadas também foram excluídos após serem consolidados em uma única lista. Resultados: Dos 497 artigos encontrados, 12 atenderam aos critérios de inclusão. Alguns desfechos clínicos e laboratoriais foram identificados, com a crise vaso-oclusiva sendo o desfecho primário presente em 25% deles. A hospitalização também foi um desfecho em um quarto dos artigos selecionados. Entre os achados laboratoriais, anemia e hemólise foram os desfechos mais prevalentes. Discussão: Como observado, um dos desfechos primários mais frequentes foi a crise vaso-oclusiva. A fisiopatologia da doença falciforme envolve diferentes mecanismos e vias inflamatórias, com um aumento em diversas citocinas, como por exemplo TNF-α e IL-1. Esses mediadores inflamatórios, combinados com as células falciformes, induzem a ativação endotelial, na qual um endotélio danificado e estimulado recruta leucócitos para aderirem à parede do vaso. Após uma segunda onda de sinais de ativação inflamatória, os neutrófilos aderidos ao endotélio eventualmente capturam e aderem às células falciformes, culminando no mecanismo de vaso-oclusão. A hospitalização também foi um desfecho em um quarto dos artigos selecionados, o que pode ocorrer devido a crises vaso-oclusivas, anemia, hemólise ou infecções, dado o risco infeccioso aumentado observado nesta população. A hospitalização na doença falciforme é negativamente correlacionada com a qualidade de vida. A suplementação de vitamina D em pacientes com doença falciforme tem mostrado melhora na qualidade de vida relacionada à saúde e melhor desempenho físico tanto em indivíduos saudáveis quanto naqueles com a hemoglobinopatia em questão. Embora a suplementação de vitamina D nesta população pareça ter alguns benefícios, ainda não há evidências fortes até o momento para a adoção de novas práticas. Conclusão: Crises vaso-oclusivas, hospitalização e hemólise são desfechos descritos nos estudos. Recomendações formais para a suplementação de vitamina D ainda não são diretas, mas alguns estudos sugerem uma redução desses desfechos após a suplementação a longo prazo.

Objetivos: Realizar um levantamento da literatura para encontrar a prevalência de deficiência de vitamina D em pacientes pediátricos com doença falciforme. Material e Métodos: Uma revisão narrativa foi realizada analisando artigos da base de dados Medline do National Institute of Medicine entre abril de 2023 e novembro de 2023. Os critérios de inclusão foram estudos observacionais em populações pediátricas de até 20 anos, publicados em inglês. Estudos de intervenção, ensaios clínicos, resumos e revisões foram excluídos da análise. Resultados: Considerando os critérios de inclusão e exclusão listados, o número final de artigos selecionados foi 18, a partir de 85 listados. A frequência geral de deficiência de vitamina D na faixa etária pediátrica com doença falciforme é de 50,49% (569/1127), considerando 20 ng/mL como ponto de corte para deficiência. Ao excluir dois artigos que provavelmente envolvem a mesma amostra, a prevalência global é de 58,17% (516/887). Considerando o ponto de corte para deficiência de vitamina D como valores abaixo de 12 ng/mL, observamos uma prevalência maior de 61,19% (41/67) em hipovitaminose D. Discussão: Todos os estudos desenvolvidos em pacientes pediátricos com doença falciforme evidenciam que a prevalência de deficiência de vitamina D nesta população parece ser maior que na população geral. Estudos globais com crianças e adolescentes saudáveis resultam em uma frequência de 18% a 36% de hipovitaminose D, considerando como deficiência valores inferiores a 20 ng/mL. Estes valores de prevalência são menores quando comparados aos 50,49% dos 12 estudos avaliados em crianças com doença falciforme e utilizando o mesmo valor de referência. Quanto à avaliação para o Brasil, estudos prévios em uma amostra de São Paulo evidenciaram na década passada prevalência entre 34,9% a 36,1% dos pacientes com valores deficientes de vitamina D, porém com análises anteriores ao período de pandemia por SARS-CoV-2. Outros fatores associados a diferença de valores nos estudos internacionais são o período de coleta do ano e a latitude em que o levantamento foi realizado, evidenciando a forte influência da exposição solar nos achados. Conclusão: A deficiência de vitamina D é altamente prevalente na população pediátrica com doença falciforme. Também se entende que estudos futuros devem avaliar a prevalência atual da hipovitaminose D nessa população, especialmente após a pandemia de COVID-19.

Abstract This research was conducted on industrial agriculture in Indonesia. Risk analysis was carried out based on previous research. One source of risk was obtained, namely raw materials that did not meet specifications, which was then proposed to be mitigated by evaluating supplier performance. This activity involves a lot of data, requiring efficient and effective data storage and access. The level in the simulation layout includes analysing system needs, using problem diagrams, compiling activity diagrams, deciding subprocesses, and filtering information. The analysis is carried out by comparing the use of supply chains with Blockchain and without Blockchain, which is then obtained to determine whether there is an increase. A sequentially stored data scenario describes a situation when the transaction process is in progress and is stored sequentially according to the process that occurs. Storing data in groups explains a problem when a transaction has been completed and stored in groups with similar data, making it easier to track specific data. In this regard, a simulation will be carried out using a website, namely a blockchain demo. The design stage starts with identifying system requirements, creating use case diagrams, compiling activity diagrams, determining subprocesses, and selecting information. The simulation results obtained will be analysed to determine the feasibility of Blockchain as a means of supporting risk mitigation related to data using aspects, including security, trust, traceability, sustainability, and costs.

As customers increasingly rely on online reviews for making consumption decisions, the dangers arising from misinformation and fakery have become an acute source of concern for consumers, firms, and society at large. Many online review platforms claim a role as guardians of trust in the information exchange process. Yet, little is known about the practices firms can utilize to design platforms that build and safeguard consumer trust. The authors draw on governance and identity disclosure literature streams to propose five practices that mitigate fakery and build trust in the platform: monitoring, exposure, community building, status endowment, and identity disclosure. Five studies (1) establish the individual and joint effects these practices have on platform trust, (2) identify the mediating processes by which the practices build trust, (3) verify the ecological value of the conceptual framework, (4) provide support of the salience of the practices, and (5) show how the practices build trust above and beyond review content–level characteristics. Interaction effects suggest that exposure, community building, and status endowment obviate the need for identity disclosure in building platform trust, a finding that mitigates related privacy concerns. Collectively, the findings can improve platform design, enabling review platforms to fulfill their promise of mitigating fakery and increasing trust. [ABSTRACT FROM AUTHOR]

The study aims to answer how and what kind of the social integration mechanisms may influence the relationship between the two dimensions of absorptive capacity which is, potential absorptive capacity (PACAP) and realized absorptive capacity (RACAP). The social integration mechanism has been neglected from the discussion on how firm assimilate and transform external knowledge to advance their absorptive capacity. In doing so, we answer recent call for research about a more thorough systematic literature review of the absorptive capacity and social integration mechanisms and its definitions. We surveyed literatures published during the years 2010 to 2020 and only found 8 articles discussing moderating variable between PACAP and RACAP out of 277 articles on absorptive capacity. The aim of this systematic review is to evaluate what social integration mechanisms influence the relationship between PACAP and RACAP and how they help overcome barriers to knowledge sharing. The results showed social integration mechanism can be in terms of high commitment works system, knowledge ambiguity, organizational size, relational learning, and cultural balance. Therefore, more research underlines the importance of social integration mechanism should be encouraged.

In an effort to circumvent the high cost of standard countermeasures against side-channel attacks in post-quantum cryptography, some works have developed low-cost detection-based countermeasures. These countermeasures try to detect maliciously generated input ciphertexts and react to them by discarding the ciphertext or secret key. In this work, we take a look at two previously proposed low-cost countermeasures: the ciphertext sanity check and the decapsulation failure check, and demonstrate successful attacks on these schemes. We show that the first countermeasure can be broken with little to no overhead, while the second countermeasure requires a more elaborate attack strategy that relies on valid chosen ciphertexts. Thus, in this work, we propose the first chosen-ciphertext based side-channel attack that only relies on valid ciphertexts for key recovery. As part of this attack, a third contribution of our paper is an improved solver that retrieves the secret key from linear inequalities constructed using side-channel leakage from the decryption procedure. Our solver is an improvement over the state-of-the-art Belief Propagation solvers by Pessl and Prokop, and later Delvaux. Our method is simpler, easier to understand and has lower computational complexity, while needing less than half the inequalities compared to previous methods.

Abstract Background New Zealand recently introduced law permitting terminally ill people to request and receive assisted dying (AD) in specified circumstances. Given the nature and complexity of this new health service, research is vital to determine how AD is operating in practice. Objective To identify research priorities regarding the implementation and delivery of AD in New Zealand. Methods Using an adapted research prioritisation methodology, the researchers identified 15 potential AD research topics. A mixed-methods survey of health professionals was undertaken where respondents were asked to rate the 15 topics according to the relative importance for research to be conducted on each issue. Respondents could also suggest additional research areas, and were invited to participate in a follow-up interview. Results One hundred and nineteen respondents completed the survey. 31% had some experience with AD. The highest rated research topic was the ‘effectiveness of safeguards in the Act to protect people’; the lowest rated topic was research into the ‘experiences of non-provider (e.g., administrative, cleaning) staff where assisted dying is being provided’. Respondents suggested 49 other research topics. Twenty-six interviews were conducted. Thematic analysis of interview data and open-ended survey questions was undertaken. Six research themes were identified: general factors related to the wider health system; the experiences of health care providers at the bedside; medico-legal issues; the impact of AD; experiences on the day of dying; and the overall effectiveness of the AD system. Key issues for stakeholders included safety of the AD service; ensuring access to AD; achieving equity for ‘structurally disadvantaged’ groups; and ensuring the well-being of patients, families/whānau, providers and non-providers. Conclusions Based on early experiences of the implementation of the AD service, health professionals provide important insights into what research should be prioritised post-legalisation of AD. These findings can be used to shape the research agenda so that research may inform law, policy and best practice.

The models made by 3D printing technology provide surgeons with an accurate analysis of complex anatomical structures, allowing the planning, training, and surgery simulation. In orthopedic surgery, this technique is especially applied in oncological surgeries, bone, and joint reconstructions, and orthopedic trauma surgeries. In these cases, it is possible to prototype anatomical models for surgical planning, simulating, and training, besides printing of instruments and implants.Purpose: The purpose of this paper is to describe the acquisition and processing from computed tomography images for 3D printing, to describe modeling and the 3D printing process of the biomodels in real size. This paper highlights 3D printing with the applicability of the 3D biomodels in orthopedic surgeries and shows some examples of surgical planning in orthopedic trauma surgery.Patients and Methods: Four examples were selected to demonstrate the workflow and rationale throughout the process of planning and printing 3D models to be used in a variety of situations in orthopedic trauma surgeries. In all cases, the use of 3D modeling has impacted and improved the final treatment strategy.Conclusion: The use of the virtual anatomical model and the 3D printed anatomical model with the additive manufacturing technology proved to be effective and useful in planning and performing the surgical treatment of complex articular fractures, allowing surgical planning both virtual and with the 3D printed anatomical model, besides being useful during the surgical time as a navigation instrument.Graphical Abstract: Keywords: 3D printing, 3D biomodel, virtual surgical planning

Aims: To evaluate the expression of neutrophilic cathelicidin pro-LL-37 (hCAP18) and the expression of CAMP gene in patients with chronic primary neutropenia (CPN), and its correlation with the etiology of the disease. Methods: Prospective study, carried out from 2021 to 2023, including children and adult patients treated at a neutropenia reference service. We analyzed 4 patients with severe congenital neutropenia (SCN), 2 with cyclic neutropenia (CN), 3 with autoimmune neutropenia (AIN), 3 with chronic idiopathic neutropenia (CIN) and 2 patients with glycogen storage disease 1B (DDG1B). The hCAP18 protein expression was evaluated in the plasma of patients with different types of neutropenia and healthy control individuals, using the Western-Blot technique. The CAMP gene expression assay was performed using the real-time PCR (qPCR) technique, using RNA extracted from whole blood. Results: The expression of hCAP18 was not observed in patients with SCN, even after increasing the neutrophil count using filgrastim. In individuals with CN, the expression of hCAP18 was also not observed during the neutropenic phase, but the protein was detected in the phase of normalization of the neutrophil count, showing a positive correlation of the protein expression with the number of neutrophils in this etiology, different from that of SCN. Expression of hCAP18 was also detected in patients with AIN, CIN and DDG1B, despite pronounced neutropenia in some patients, and in the control group. The qPCR showed a low expression of CAMP in patients with SCN, while the other etiologies showed variations in gene expression, being positive in some individuals and negative in others with the same etiology. However, using this method, it was possible to notice a positive correlation (r = 0.732 and p < 0.05 – Spearman test) between the expression of the CAMP gene and the expression of hCAP18. Discussion: hCAP18, encoded by the CAMP gene, is an important component of the secondary granules of neutrophils, with immunological and bactericidal activity. The results of this work corroborate the data by Karlsson et al., who showed absence or significant decrease in the expression of hCAP18 in individuals with SCN and its expression in individuals with CIN and AIN. In addition, we also demonstrated a positive correlation between the expression of the CAMP gene and hCAP18 and a decrease in the expression of the CAMP gene in patients with SCN. Therefore, plasma hCAP18 assessment is suggested as a differential CPN screening method, aiding in the diagnosis of SCN due to low or undetectable hCAP18 levels. Conclusion: There is a correlation between CAMP gene expression and hCAP18 expression with CPN. In addition, we also demonstrated that hCAP18 is not expressed in patients with SCN and in the neutropenic phase of CN. Thus, the use of this technique is suggested as a new screening routine for the investigation of patients with CPN, especially in patients with SCN and CN, differentiating them from the others and avoiding more invasive tests.

What explains ethnic minorities' attachment to the state? Our current understanding has primarily considered structural factors, particularly access to political power and modernization. Diverging from existing explanations, I theorize that mundane experiences with the state, in street-level bureaucracy, can inform ethnic minorities' attitudes toward the state. What they see and experience in street-level bureaucracy signals to ethnic minorities what their prospects might be in a country that is politically dominated by another ethnic group. Leveraging extensive fieldwork in Myanmar, I show that ethnic minorities who have had positive encounters with street-level bureaucrats express stronger attachment to the state. This is the case even when an ethnic group is in direct conflict with the state. I also find that service experiences are more relevant in explaining ethnic minorities' attachment to the state compared to factors highlighted in existing research. [ABSTRACT FROM AUTHOR]
