The low-lying structure of 15C has been investigated via the neutron-removal 16C(d,t) reaction. Along with the known bound neutron sd-shell states, unbound p-shell hole states have been observed. The excitation energies and the deduced spectroscopic factors of the cross-shell states are an important measure of the [(p)−1(sd)2] neutron configurations in 15C. Our results show a very good agreement with shell-model calculations using the SFO-tls interaction for 15C. However, this same interaction predicted energies that were too low for the corresponding hole states in the N=9 isotone 17O and adjustment of the p-sd and sd-sd monopole terms was required to match the 17O energies. In addition, the excitation energies and spectroscopic factors have been compared to the first calculations of 15C with the ab initio self-consistent Green's function method employing the NNLOsat interaction. The results show the sensitivity to the size of the N=8 shell gap and highlight the need to go beyond the current truncation scheme.

Proton-emission branches of the 10+ isomer in the Tz=−1 nucleus 54Ni have been imaged with the active target and time projection chamber (ACTAR TPC) in an experiment conducted at the Grand Accélérateur National d'Ions Lourds (GANIL). The completed decay scheme allows derivation of the reduced transition strengths, B(E2;10+→8+) and B(E4;10+→6+), for the two competing γ-ray transitions. By means of a comparison with their well-known ‘mirror transitions’ in Tz=+1 54Fe, and aided by a variety of shell-model calculations in the fp model space, effective charges for E4 transitions near N=Z 56Ni can be deduced: επ≈1.40 and εν≈0.30. Mirror-energy differences are explored with various shell-model interactions and isospin-symmetry breaking terms.

The isovector and isoscalar components of neutron-proton pairing are investigated in the N=Z unstable nuclei of the fp-shell through the two-nucleon transfer reaction (p,3He) in inverse kinematics. The combination of particle and gamma-ray detection with radioactive beams of 56Ni and 52Fe, produced by fragmentation at the GANIL/LISE facility, made it possible to carry out this study for the first time in a closed and an open-shell nucleus in the fp-shell. The transfer cross-sections for ground-state to ground-state (J=0+, T=1) and to the first (J=1+, T=0) state were extracted for both cases together with the transfer cross-section ratios σ(0+,T=1)/σ(1+,T=0). They are compared with second-order distorted-wave born approximation (DWBA) calculations. The enhancement of the ground-state to ground-state pair transfer cross-section close to mid-shell, in 52Fe, points towards a superfluid phase in the isovector channel. For the “deuteron-like” transfer, very low cross-sections to the first (J=1+, T=0) state were observed both for 56Ni(p,3He) and 52Fe(p,3He) and are related to a strong hindrance of this channel due to spin-orbit effect. No evidence for an isoscalar deuteron-like condensate is observed.

During the last 12 years using the GABRIELA (Gamma Alpha Beta Recoil Invetsigations with the ELectromagnetic Analyser) detector set up and kinematic recoil separators VASSILISSA / SHELS the experiments aimed to the gamma and electron spectroscopy of the Fm-Db isotopes, formed at the complete fusion reactions48Ca +207,208Pb--255,256 No* ,48Ca +209Bi--257Lr* ,22Ne+238U--260No*,50Ti +208Pb--258Rf* and50Ti +209Bi--259Db* , were performed at FLNR JINR.

Neutron dark decays have been suggested as a solution to the discrepancy between bottle and beam experiments, providing a dark matter candidate that can be searched for in halo nuclei. The free neutron in the final state following the decay of ^{6}He into ^{4}He+n+χ provides an exceptionally clean detection signature when combined with a high efficiency neutron detector. Using a high-intensity ^{6}He^{+} beam at Grand Accélérateur National d'Ions Lourds, a search for a coincident neutron signal resulted in an upper limit on a dark decay branching ratio of Br_{χ}≤4.0×10^{-10} (95% C.L.). Using the dark neutron decay model proposed originally by Fornal and Grinstein, we translate this into an upper bound on a dark neutron branching ratio of O(10^{-5}), improving over global constraints by one to several orders of magnitude depending on m_{χ}.

The observation of a weak proton-emission branch in the decay of the 3174-keV 53m Co isomeric state marked the discovery of proton radioactivity in atomic nuclei in 1970. Here we show, based on the partial half-lives and the decay energies of the possible proton-emission branches, that the exceptionally high angular momentum barriers, [Formula: see text] and [Formula: see text], play a key role in hindering the proton radioactivity from 53m Co, making them very challenging to observe and calculate. Indeed, experiments had to wait decades for significant advances in accelerator facilities and multi-faceted state-of-the-art decay stations to gain full access to all observables. Combining data taken with the TASISpec decay station at the Accelerator Laboratory of the University of Jyväskylä, Finland, and the ACTAR TPC device on LISE3 at GANIL, France, we measured their branching ratios as b p1 = 1.3(1)% and b p2 = 0.025(4)%. These results were compared to cutting-edge shell-model and barrier penetration calculations. Background: Extubation strategy in extracorporeal life support patients remains unclear, and literature only reports studies with significant biases., Objectives: To explore the prognostic impact of an early ventilator-weaning strategy in assisted patients after controlling for confounding factors., Methods: A 10-year retrospective study included 241 patients receiving extracorporeal life support for at least 48 h, corresponding to a total of 977 days spent on assistance. The a priori probability of extubation for each day of assistance was calculated according to daily biological examinations, drug doses, clinical observations, and admission data to pair each day containing an extubation with one on which the patient was not extubated. The primary outcome was survival at day 28. The secondary outcomes were survival at day 7, respiratory infections, and safety criteria., Results: Two similar cohorts of 61 patients were generated. Survival at day 28 was better in patients extubated under assistance in univariate and multivariate (HR = 0.37 [0.2-0.68], p-value = 0.002) analyses. Patients who underwent failed early extubation did not have a different prognosis from those without early extubation. Successful early extubation was associated with a better outcome than a failed or no attempt at early extubation. Survival at day 7 and the rate of respiratory infections were better in early-extubated patients. Safety data did not differ between the two groups., Conclusions: Early extubation during assistance was associated with a superior outcome in our propensity-matched cohort study. Background: Although short- and long-term survival in critically ill patients with cancer has been described, data on their quality of life (QoL) after an intensive care unit (ICU) stay are scarce. This study aimed to determine the impact of an ICU stay on QoL assessed at 3 months in patients with solid malignancies., Methods: A prospective case-control study was conducted in three French ICUs between February 2020 and February 2021. Adult patients with lung, colorectal, or head and neck cancer who were admitted in the ICU were matched in a 1:2 ratio with patients who were not admitted in the ICU regarding their type of cancer, curative or palliative anticancer treatment, and treatment line. The primary endpoint was the QoL assessed at 3 months from inclusion using the mental and physical components of the Short Form 36 (SF-36) Health Survey. The use of anticancer therapies at 3 months was also evaluated., Results: In total, 23 surviving ICU cancer patients were matched with 46 non-ICU cancer patients. Four patients in the ICU group did not respond to the questionnaire. The mental component score of the SF-36 was higher in ICU patients than in non-ICU patients: median of 54 (interquartile range: 42-57) vs. 47 (37-52), respectively (p = 0.01). The physical component score of the SF-36 did not differ between groups: 35 (31-47) vs. 42 (34-47) (p = 0.24). In multivariate analysis, no association was found between patient QoL and an ICU stay. A good performance status and a non-metastatic cancer at baseline were independently associated with a higher physical component score. The use of anticancer therapies at 3 months was comparable between the two groups., Conclusion: In patients with solid malignancies, an ICU stay had no negative impact on QoL at 3 months after discharge when compared with matched non-ICU patients., Competing Interests: The authors have declared that no competing interests exist., (Copyright: © 2023 Toffart et al. Background: High preoperative fibrinogen levels are associated with reduced bleeding rates after cardiac surgery. Fibrinogen is directly involved in inflammatory processes and is a cardiovascular risk factors. Whether high fibrinogen levels before cardiac surgery are a risk factor for mortality or morbidity remains unclear., Aims: This study aimed to examine the association between preoperative fibrinogen levels and mortality and morbidity rates after cardiac surgery., Settings and Design: This is a single-center retrospective study., Material and Methods: Patients (n = 1628) were divided into high (HFGr) and normal (NFGr) fibrinogen level groups, based on the cutoff value of 3.3 g/L, derived from the receiver operating characteristic (ROC) curve analysis. The primary outcome was the 30-day mortality rate. The rates of postoperative complications, including postoperative bleeding and transfusion rates, were examined., Statistical Analysis: Between-group comparisons were performed with the Mann-Whitney U test and Chi-squared test, as suitable. Model discriminative power was examined with the area under the ROC curve., Results: The HFGr and NFGr included 1103 and 525 patients, respectively. Mortality rate was higher in the HFGr than in the NFGr (2.7% vs. 1.1%, P = 0.04). The 12-h bleeding volume (280 mL [195-400] vs. 305 mL [225-435], P = 0.0003) and 24-h bleeding volume values (400 mL [300-550] vs. 450 mL [340-620], P < 0.0001) were lower in the HFGr than in the NFGr. However, the rate of red blood cell transfusion during hospitalization was higher in the HFGr than in the NFGr (21.7% vs. 5.9%, P = 0.0103). Major complications were more frequent in the HFGr than in the NFGr., Conclusion: High fibrinogen levels were associated with reduced postoperative bleeding volume and increased mortality and morbidity rates.
