Abstract: International audience; BackgroundThe present article is an English-language version of the French National Diagnostic and Care Protocol, a pragmatic tool to optimize and harmonize the diagnosis, care pathway, management and follow-up of lymphangioleiomyomatosis in France.MethodsPractical recommendations were developed in accordance with the method for developing a National Diagnosis and Care Protocol for rare diseases of the Haute Autorité de Santé and following international guidelines and literature on lymphangioleiomyomatosis. It was developed by a multidisciplinary group, with the help of patient representatives and of RespiFIL, the rare disease network on respiratory diseases.ResultsLymphangioleiomyomatosis is a rare lung disease characterised by a proliferation of smooth muscle cells that leads to the formation of multiple lung cysts. It occurs sporadically or as part of a genetic disease called tuberous sclerosis complex (TSC). The document addresses multiple aspects of the disease, to guide the clinicians regarding when to suspect a diagnosis of lymphangioleiomyomatosis, what to do in case of recurrent pneumothorax or angiomyolipomas, what investigations are needed to make the diagnosis of lymphangioleiomyomatosis, what the diagnostic criteria are for lymphangioleiomyomatosis, what the principles of management are, and how follow-up can be organised. Recommendations are made regarding the use of pharmaceutical specialties and treatment other than medications.ConclusionThese recommendations are intended to guide the diagnosis and practical management of pulmonary lymphangioleiomyomatosis.

Abstract: International audience; AIMS: Although prebiotics, probiotics, and fecal transplantation can alter the sensation of hunger and/or feeding behavior, the role of the constitutive gut microbiota in the short-term regulation of food intake during normal physiology is still unclear. RESULTS: An antibiotic-induced microbiota depletion study was designed to compare feeding behavior in conventional and microbiota-depleted mice. Tissues were sampled to characterize the time profile of microbiota-derived signals in mice during consumption of either standard or high-fat food for 1 hour. Pharmacological and genetic tools were used to evaluate the contribution of postprandial endotoxemia and inflammatory responses in the short-term regulation of food intake. We observed constitutive microbial and macronutrient-dependent control of food intake at the time scale of a meal, i.e., within 1 hour of food introduction. Specifically, microbiota depletion increased food intake and the microbiota-derived anorectic effect became significant during the consumption of high-fat but not standard food. This anorectic effect correlated with a specific postprandial microbial metabolic signature and did not require postprandial endotoxemia or an NLRP3 (NOD-, LRR- and Pyrin domain-containing protein 3)-inflammasome mediated inflammatory response. Innovation and Conclusion: These findings show that the gut microbiota controls host appetite at the time scale of a meal under normal physiology. Interestingly, a microbiota-derived anorectic effect develops specifically with a high-fat meal, indicating that gut microbiota activity is involved in the satietogenic properties of foods.

Abstract: International audience; We present the phenotypes of seven previously unreported patients with Marbach-Schaaf neurodevelopmental syndrome, all carrying the same recurrent heterozygous missense variant c.1003C>T (p.Arg335Trp) in PRKAR1B. Clinical features of this cohort include global developmental delay and reduced sensitivity to pain, as well as behavioral anomalies. Only one of the seven patients reported here was formally diagnosed with autism spectrum disorder (ASD), while ASD-like features were described in others, overall indicating a lower prevalence of ASD in Marbach-Schaaf neurodevelopmental syndrome than previously assumed. The clinical spectrum of the current cohort is similar to that reported in the initial publication, delineating a complex developmental disorder with behavioral and neurologic features. PRKAR1B encodes the regulatory subunit R1beta of the protein kinase A complex (PKA), and is expressed in the adult and embryonal central nervous system in humans. PKA is crucial to a plethora of cellular signaling pathways, and its composition of different regulatory and catalytic subunits is cell-type specific. We discuss potential molecular disease mechanisms underlying the patients' phenotypes with respect to the different known functions of PKA in neurons, and the phenotypes of existing R1beta-deficient animal models.

Abstract: International audience; This study aims to identify the potential bias of the relative age effect (RAE) in French alpine skiers and to propose a mathematical correction adjustment for such a bias. All performances and birthdates of skiers on the national and international circuit were collected from the 2004 up to 2019. A goodness-of-fit chi-square test and the residuals were used to study the distribution of birth trimesters in youth competitors. A linear relationship between the distribution of performances and the months obtains a calibration coefficient allowing to rebalance the performance by considering the effect of RAE. Individuals born at the beginning of the year are over-represented in the elite young selections in all disciplines for both genders. A coefficient based on the relationship between month of birth and performance adjusts individual performance and cancels out the effect of RAE. The results show that RAE is present in French alpine skiing. We present a method allowing to consider the effect of the RAE in the performances realized in each gender and discipline. This method allows coaches to have a more objective opinion on performance and to reduce selection bias.

Abstract: Background Non-cancer mortality in cancer patients may be higher than overall mortality in the general population due to a combination of factors, such as long-term adverse effects of treatments, and genetic, environmental or lifestyle-related factors. If so, conventional indicators may underestimate net survival and cure fraction. Our aim was to propose and evaluate a mixture cure survival model that takes into account the increased risk of non-cancer death for cancer patients. Methods We assessed the performance of a corrected mixture cure survival model derived from a conventional mixture cure model to estimate the cure fraction, the survival of uncured patients, and the increased risk of non-cancer death in two settings of net survival estimation, grouped life-table data and individual patients’ data. We measured the model’s performance in terms of bias, standard deviation of the estimates and coverage rate, using an extensive simulation study. This study included reliability assessments through violation of some of the model’s assumptions. We also applied the models to colon cancer data from the FRANCIM network. Results When the assumptions were satisfied, the corrected cure model provided unbiased estimates of parameters expressing the increased risk of non-cancer death, the cure fraction, and net survival in uncured patients. No major difference was found when the model was applied to individual or grouped data. The absolute bias was Conclusions The present analysis supports the use of the corrected mixture cure model, with the inclusion of increased risk of non-cancer death for cancer patients to provide better estimates of indicators based on cancer survival. These are important to public health decision-making; they improve patients’ awareness and facilitate their return to normal life.

Abstract: Oxidoreductases are major enzymes of xenobiotic metabolism. Consequently, they are essential in the chemoprotection of the human body. Many xenobiotic metabolism enzymes have been shown to be involved in chemosensory tissue protection. Among them, some were additionally shown to be involved in chemosensory perception, acting in signal termination as well as in the generation of metabolites that change the activation pattern of chemosensory receptors. Oxidoreductases, especially aldehyde dehydrogenases and aldo–keto reductases, are the first barrier against aldehyde compounds, which include numerous odorants. Using a mass spectrometry approach, we characterized the most highly expressed members of these families in the human nasal mucus sampled in the olfactory vicinity. Their expression was also demonstrated using immunohistochemistry in human epitheliums sampled in the olfactory vicinity. Recombinant enzymes corresponding to three highly expressed human oxidoreductases (ALDH1A1, ALDH3A1, AKR1B10) were used to demonstrate the high enzymatic activity of these enzymes toward aldehyde odorants. The structure‒function relationship set based on the enzymatic parameters characterization of a series of aldehyde odorant compounds was supported by the X-ray structure resolution of human ALDH3A1 in complex with octanal.

Abstract: International audience; Evaluating lipid profiles in human tissues and biofluids is critical in identifying lipid metabolites in dysregulated metabolic pathways. Due to various chemical characteristics, single-run lipid analysis has not yet been documented. Such approach is essential for analysing pathology-related lipid metabolites. Age-Related Macular Degeneration (AMD), the leading cause of vision loss in western countries, is emblematic of this limitation. Several studies have identified alterations in individual lipids, but the majority are based on targeted approaches. In this study, we analyzed and identified approximately 500 lipid species in human biofluids (plasma and erythrocytes) and ocular tissues (retina and retinal pigment epithelium) using the complementarity of hydrophilic interaction liquid chromatography (HILIC) and reversed-phase chromatography (RPC), coupled to high-resolution mass spectrometry (HRMS). For that, lipids were extracted from human eyeglobes and blood from 10 subjects and lipidomic analysis was carried out through analysis in HILIC and RPC, alternately. Furthermore, we illustrate the advantages and disadvantages of both techniques for lipid characterization. RPC showed greater sensitivity in hydrophobicity-based lipid separation, detecting diglycerides (DAG), triglycerides (TAG), cholesterol (Chol), and cholesterol esters (CEs), whereas no signal of these molecules was obtained in HILIC. However, due to coelution, RPC was less effective in separating polar lipids like phospholipids, which were separated effectively in HILIC in both ionization modes. The complementary nature of these analytical approaches was essential for the detection and identification of lipid classes/subclasses, which can then provide distinct insights into lipid metabolism, a determinant of the pathophysiology of several diseases involving lipids, notably AMD.

Abstract: Purpose: Multi-omics offer worthwhile and increasingly accessible technologies to diagnostic laboratories seeking potential second-tier strategies to help patients with unresolved rare diseases, especially patients clinically diagnosed with a rare OMIM (Online Mendelian Inheritance in Man) disease. However, no consensus exists regarding the optimal diagnostic care pathway to adopt after negative results with standard approaches.Methods: In 15 unsolved individuals clinically diagnosed with recognizable OMIM diseases but with negative or inconclusive first-line genetic results, we explored the utility of a multi-step approach using several novel omics technologies to establish a molecular diagnosis. Inclusion criteria included a clinical autosomal recessive disease diagnosis and single heterozygous pathogenic variant in the gene of interest identified by first-line analysis (60%–9/15) or a clinical diagnosis of an X-linked recessive or autosomal dominant disease with no causative variant identified (40%–6/15). We performed a multi-step analysis involving short-read genome sequencing (srGS) and complementary approaches such as mRNA sequencing (mRNA-seq), long-read genome sequencing (lrG), or optical genome mapping (oGM) selected according to the outcome of the GS analysis.Results: SrGS alone or in combination with additional genomic and/or transcriptomic technologies allowed us to resolve 87% of individuals by identifying single nucleotide variants/indels missed by first-line targeted tests, identifying variants affecting transcription, or structural variants sometimes requiring lrGS or oGM for their characterization.Conclusion: Hypothesis-driven implementation of combined omics technologies is particularly effective in identifying molecular etiologies. In this study, we detail our experience of the implementation of genomics and transcriptomics technologies in a pilot cohort of previously investigated patients with a typical clinical diagnosis without molecular etiology.

Abstract: Yeast co-inoculations in winemaking are often studied in the framework of modulating the aromatic profiles of wines. Our study aimed to investigate the impact of three cocultures and corresponding pure cultures of Saccharomyces cerevisiae on the chemical composition and the sensory profile of Chardonnay wine. Coculture makes it possible to obtain completely new aromatic expressions that do not exist in the original pure cultures attributed to yeast interactions. Esters, fatty acids and phenol families were identified as affected. The sensory profiles and metabolome of the cocultures, corresponding pure cultures and associated wine blends from both pure cultures were found to be different. The coculture did not turn out to be the addition of the two pure culture wines, indicating the impact of interaction. High resolution mass spectrometry revealed thousands of cocultures biomarkers. The metabolic pathways involved in these wine composition changes were highlighted, most of them belonging to nitrogen metabolism.

Abstract: International audience; The evidence that most agricultural landscapes are failing to deliver on biodiversity conservation and ecosystem services provision suggests that future landscapes will need to be more explicitly designed. Although recent research has produced a number of ecological and social principles that should form the basis of agricultural landscape design process, implementation is still in its infancy. One difficulty is the context-dependency of ecological responses and the resulting limiting capacity to predict the benefits of landscape transformation for the targeted organisms or services. In addition, there is a poor understanding of the obstacles to and levers for the implementation of collective management at the landscape scale. In this paper, we argue that Landscape Monitoring Networks (LMN), i.e. long-term and standardized monitoring of ecological and managerial processes within a set of replicated regional landscapes, can contribute to tackling these issues. We first present the current challenges in designing agroecological landscapes before outlining the principles of LMN and how these research facilities could help deliver ecological and social understanding along a gradient from place-based to generic knowledge. We then discuss critical issues that need to be solved to ensure that LMN delivers relevant knowledge for landscape design. We illustrate this through the experience of an ongoing LMN that was created in France in 2014 to address biodiversity and pest control services in agricultural landscapes.

Abstract: International audience; BACKGROUND: The excess mortality observed in Acute Myeloblastic Leukaemia (AML) patients, partly attributed to unequal access to curative treatments, could be linked to care pathways. METHODS: We included 1039 AML incident cases diagnosed between 2012-2016 from the 3 French blood cancer registries (3,625,400 inhabitants). We describe patients according to age, the medical entry unit and access to the specialised haematology unit (SHU) during follow-up. Multivariate logistic regression model was done to determine the association between covariables and access to SHU. A total of 713 patients (69%) had access to SHU during care. RESULTS: The most common care pathway concerned referral from the general practitioner to SHU, n = 459(44%). The univariate analysis observed a downward trend for the most deprived patients. Abstract: Background Faecalibacterium prausnitzii, a promising next-generation probiotic, is one of the most abundant acetate-consuming, butyrate-producing bacteria in the healthy human gut. However, little is known about the gene expression strategies used by this bacterium to adapt to the availability of acetate in the human gut. Result We first established a growth model of F. duncaniae A2-165 (previously known as F. prausnitzii A2-165) using batch cultures, under high- and low-acetate conditions. Over late exponential and early stationary phases, the bacteria displayed a growth deficiency and a low butyrate production in low- compared with high-acetate conditions. Using RNA-seq, we compared expression patterns between early stationary and late exponential phases in high- and low-acetate conditions and between high- and low-acetate conditions in the early stationary phase. Functional classification of the low-acetate transcriptome revealed the specific activation of a general stress response, including upregulation of chaperones, toxin-antitoxin type II systems and downregulation of numerous protein synthesis genes. We observed two distinct import system transcriptomes, under low and high-acetate conditions, suggesting major adaptation responses to nutrient-deprived conditions of the early-stationary growth phase. Specifically, in high-acetate conditions, the feoAABC operon encoding one FeoB ferrous iron transporter was strongly activated but not the feoAB gene encoding the second FeoB transporter of F. duncaniae A2-165. This strong activation in early-stationary phase under high-acetate conditions suggested that (i) the level of extracellular ferrous iron is low (ii) FeoAABC system is required for ferrous iron uptake, in this iron-poor environment. Using RT-PCR, we demonstrated that excess ferrous iron represses feoB expression (i.e feoAABC operon) but not feoAB expression in high-acetate conditions. Finally, we conducted an integrated analysis of a healthy human fecal metaproteome, in which we were able to detect FeoB peptides from both genus Faecalibacterium and strain A2-165. Conclusion We characterized two early-stationary lifestyles of F. duncaniae A2-165 at the transcriptional level, related to acetate consumption and butyrate production. We characterized the regulation of feoAABC operon encoding an high-affinity ferrous iron transporter that may play a major role in iron homeostasis and butyrate synthesis in iron-poor and acetate-rich conditions in F. duncaniae A2-165.

Abstract: International audience; Glioblastoma (GBM) is the most deadly type of malignant brain tumor, despite extensive molecular analyses of GBM cells. In recent years, the tumor microenvironment (TME) has been recognized as an important player and therapeutic target in GBM. However, there is a need for a full and integrated understanding of the different cellular and molecular components involved in the GBM TME and their interactions for the development of more efficient therapies. In this review, we provide a comprehensive report of the GBM TME, which assembles the contributions of physicians and translational researchers working on brain tumor pathology and therapy in France. We propose a holistic view of the subject by delineating the specific features of the GBM TME at the cellular, molecular, and therapeutic levels.

Abstract: We report a huge organic diversity in the Tissint Mars meteorite and the sampling of several mineralogical lithologies, which revealed that the organic molecules were nonuniformly distributed in functionality and abundance. The range of organics in Tissint meteorite were abundant C 3-7 aliphatic branched carboxylic acids and aldehydes, olefins, and polyaromatics with and without heteroatoms in a homologous oxidation structural continuum. Organomagnesium compounds were extremely abundant in olivine macrocrystals and in the melt veins, reflecting specific organo-synsthesis processes in close interaction with the magnesium silicates and temperature stresses, as previously observed. The diverse chemistry and abundance in complex molecules reveal heterogeneity in organic speciation within the minerals grown in the martian mantle and crust that may have evolved over geological time.

Abstract: International audience; Multiblock component methods aim to study the relationships between several sets of variables. Regularized Generalized Canonical Correlation Analysis (RGCCA) is a unified and flexible framework that gathers fifty years of multiblock component methods. RGCCA offers a unified implementation strategy for all these methods. This implementation is made available within the RGCCA package. In addition, the RGCCA package produces graphical outputs and statistics to assess the robustness/significance of the analysis. The usefulness of the RGCCA package is illustrated in this paper on two real datasets. Abstract: Public school food procurement has been identified as a key lever in the transition towards sustainable food systems. In this study, we assess the nutritional quality and the carbon footprint of 2020 school menus served in 101 municipalities in the inner suburbs of Paris. In this sample, school canteens menus meet an average 8.2/15 (min = 4, max = 14) adequacy score to the regulatory nutritional quality frequency criteria and their carbon footprint averages at 1.9 (min = 1.2, max = 2.6) kgCO2e/day. The nutritional and environmental qualities of canteen menus were not correlated with each other. In-house canteens have a significantly higher nutritional quality – 0.7 more points – and so do larger canteens. The carbon footprint significantly decreases with an increasing education level of the population and, for in-house canteens, it also decreases by 0.16 kgCO2e/day with a ten-fold increase in canteen size and by 0.0035 kgCO2e/day per percent of left-wing vote, breaking even with delegated canteens above 3500 enrolled children and 53% of left-wing vote respectively. The frequency of certified food (mean = 18%, min = 0%, max = 51%), a cornerstone of the 2018 national law aiming at more sustainable institutional catering, has no impact on our indicators of nutritional quality and carbon footprint. The substantial variations between canteens in both nutritional and environmental qualities suggests that there is room for improvement on both ends.

Abstract: International audience; Crop losses from pests threaten global food security and safety. In the last six decades, pest control using chemical pesticides has resulted in important yield gains per unit area, worldwide. However, the long-term sustainability of chemical pest control has been increasingly thrown into doubt due to the negative impact on human health, biodiversity, and the environment. Consequently, there is an urgent need to improve the science of crop protection in order to tackle the five key challenges of 21st century agriculture holistically: (i) maintaining or improving agricultural productivity, (ii) producing healthy food, (iii) reducing the negative impacts of agriculture on ecosystem and human health, (iv) ensuring the economic viability of farms, and (v) adapting agriculture to climate change. Agroecological Crop Protection (ACP) can be a powerful approach to address these challenges, as we demonstrate in this paper. ACP is the application of the principles of agroecology to crop protection in order to promote virtuous and sustainable changes in agriculture and food systems. ACP combines multiple approaches and disciplines including ecology, agroecology, and Integrated Pest Management. It promotes a crop protection system compatible with healthy agricultural and food systems, agroecological principles and the “one health” approach. We predict that ACP will meet the challenge of pesticide-free agriculture in the future. In this paper, we will first present the scientific, agricultural and social components of ACP. We will then analyze the research approaches, questions, methods and tools needed to adopt ACP. Finally, we suggest key mechanisms to facilitate the transition to ACP, which will ultimately provide sustainable food, feed, and fuel in a context of major global change.

Abstract: International audience; Although fluorescent proteins are widely used as biomarkers (Yin), no study focuses on their influence on the microbial stress response. Here, the Green Fluorescent Protein (GFP) was fused to two proteins of interest in Saccharomyces cerevisiae. Pab1p and Sur7p, respectively involved in stress granules structure and in Can1 membrane domains. These were chosen since questions remain regarding the understanding of the behavior of S. cerevisiae facing different heat kinetics or oxidative stresses. The main results showed that Pab1p-GFP fluorescent mutant displayed a higher resistance than that of the wild type under a heat shock. Moreover, fluorescent mutants exposed to oxidative stresses displayed changes in the cultivability compared to the wild type strain. In silico approaches showed that the presence of the GFP did not influence the structure and so the functionality of the tagged proteins meaning that changes in yeast resistance were certainly related to GFP ROS-scavenging ability (Yang).

Abstract: Séance : Santé; International audience; In this communication we carried an analysis of the activity of 3 veterinarians during 3 annual health visits in organic dairy farms. This visit is defined here as a joint activity between two professionals who aim at improving the herd’ sanitary condition. Our qualitative study covers 3 cases of these visits. An analysis grid was designed to highlight the objectives of veterinarians, the dilemmas they face in this peculiar situation and the decisions they make based on their guiding principles. The dilemmas encountered by the veterinarians during the health visit are 1. the choice of topics to be addressed in a limited time; 2. the consideration of the perspective of the breeder; 3. the discrepancy between advices on preventive zootechnical measures and prudent drug uses and the vets’ income being based on the sale of medicines. These dilemmas are solved on the basis of assumptions that differ from one veterinarian to another, for example a partnership posture or a training posture. The health visit is also an opportunity to exchange knowledge far beyond the strict Framework of the farm that improves the technical, sectoral and strategic knowledge search of the veterinarian, especially on organic farming. Finally, the vets does mentor the health strategy of the farmer, which the stakeholders are not fully aware.The results of this study show many opportunities to improve the herd health thanks to the health visit. They also show how the veterinarian can learn from the exchanges with the breeder, during the visit. Finally, the methodology belongs to a process based on the activity analysis and aimed at redesigning the veterinary education. It indeed offers new perspectives for veterinary training and skills to be used during the health visit.; Cette communication présente l’analyse de l’activité de 3 vétérinaires lors de 3 visites de bilan sanitaire d’élevage (BSE) au sein d’élevages bovins laitiers biologiques. Cette visite est ici définie comme une activité conjointe entre deux professionnels dont l’objectif est l’amélioration de l’état sanitaire de l’élevage. Notre étude qualitative porte sur 3 cas de visites de BSE. Une grille d’analyse a été construite afin de mettre en lumière les objectifs des vétérinaires, les dilemmes auxquels ils font face en situation et les décisions qu’ils prennent en fonction de leurs principes directeurs. Le contexte de la visite met le vétérinaire face à des dilemmes 1. dans le choix des thèmes à aborder par rapport à la contrainte temporelle ; 2. dans la prise en compte des divergences de vue de l’éleveur ; 3.dans le conseil sur les mesures zootechniques préventives et la réduction des intrants médicamenteux alors que son revenu se base sur la vente de médicaments. Ces dilemmes sont tranchés au regard de postulats qui diffèrent selon les vétérinaires, par exemple une posture de partenariat ou une posture de formateur. La visite de BSE est également un moment d’échanges de connaissances dépassant le cadre strict de l’élevage qui alimentent la veille technique, sectorielle et stratégique du vétérinaire, en particulier sur l’agriculture biologique. Enfin, elle joue un rôle de mentoring sur la stratégie sanitaire de l’éleveur dont les parties prenantes ne sont pas entièrement conscientes. Les résultats de cette étude montrent les perspectives que le travail du vétérinaire lors de la visite du BSE offre à l’éleveur pour l’amélioration de la santé des animaux dans les élevages dans le cadre du BSE. Ils montrent également comment, lors de la visite, le vétérinaire peut s’enrichir à partir de ses échanges avec l’éleveur. Enfin, la méthodologie utilisée pour obtenir ces résultats s’inscrit dans une démarche qui a pour but de repenser la formation à la lumière de l’activité analysée. Elle ouvre de nouvelles perspectives pour la formation vétérinaire sur le BSE et sa mise en pratique sur le terrain.

24. iPSCs derived from infertile men carrying complex genetic abnormalities can generate primordial germ-like cells

Abstract: Despite increasing insight into the genetics of infertility, the developmental disease processes remain unclear due to the lack of adequate experimental models. The advent of induced pluripotent stem cell (iPSC) technology has provided a unique tool for in vitro disease modeling enabling major advances in our understanding of developmental disease processes. We report the full characterization of complex genetic abnormalities in two infertile patients with either azoospermia or XX male syndrome and we identify genes of potential interest implicated in their infertility. Using the erythroblasts of both patients, we generated primed iPSCs and converted them into a naive-like pluripotent state. Naive-iPSCs were then differentiated into primordial germ-like cells (PGC-LCs). The expression of early PGC marker genes SOX17, CD-38, NANOS3, c-KIT, TFAP2C, and D2-40, confirmed progression towards the early germline stage. Our results demonstrate that iPSCs from two infertile patients with significant genetic abnormalities are capable of efficient production of PGCs. Such in vitro model of infertility will certainly help identifying causative factors leading to early germ cells development failure and provide a valuable tool to explore novel therapeutic strategies.

Abstract: International audience; IntroductionLes lomboradiculalgies postopératoires (LRPO), comme d’autres types de douleur, sont considérées comme une expérience sensorielle et émotionnelle désagréable influencée par des facteurs biologiques, psychologiques et sociaux [1, 2, 3]. Malgré l’innovation constante de la technologie numérique et de l’intelligence artificielle, la douleur est toujours évaluée par des « outils de référence » tels que l’échelle visuelle analogique de la douleur (EVA) [4]. La principale limite de cet outil est qu’il ne tient pas compte des nombreuses dimensions de la douleur, comme les incapacités fonctionnelles, la détresse psychologique ou de la qualité de vie [3]. En pratique, les évaluations de ces dimensions sont considérées indépendamment, même si l’expérience clinique souligne l’imbrication de ces dimensions les unes avec les autres. Notre objectif était de développer un nouvel indice multidimensionnel de réponse clinique (MCRI), incluant l’intensité de la douleur, la capacité fonctionnelle, l’anxiété/dépression, la qualité de vie et la cartographie de la douleur, l’objectif étant une évaluation multidimensionnelle des douleurs.Matériel et méthodesC’est une étude observationnelle, prospective, multicentrique (PREDIBACK) incluant 200 patients avec des LRPO avec un suivi de 12 mois. L’intensité de la douleur, la surface de la douleur corporelle (en cm2 via une interface tactile), l’incapacité fonctionnelle (Oswestry Disability Index), l’anxiété/dépression (Hospital Anxiety and Depression scale), la qualité de vie (EuroQol-5 dimensions). Ces résultats ont été combinés à l’aide d’une analyse factorielle pour créer l’indice composite MCRI. Le MCRI a été comparé aux autres mesures par des tests de corrélations. La capacité du MCRI à détecter la satisfaction du patient après une thérapie a également été analysée.RésultatsLe MCRI représentait de façon plus pertinente toutes les dimensions de la douleur comparée aux autres scores classiques (corrélation avec les autres critères>0,603). De plus, comparé aux autres critères, le MCRI a montré la plus grande sensibilité (77,4 %) et spécificité (79,8 %) pour détecter de la satisfaction du patient par rapport aux autres scores.Discussion/conclusionCe nouvel indice composite MCRI pourrait contribuer à affiner l’évaluation de l’état de santé général du patient douloureux chronique sur la base de paramètres objectifs. Ce nouvel indice MCRI doit être éprouvé en milieu clinique afin de déterminer l’efficacité des thérapies.

Abstract: International audience; Ferroptosis, an iron-dependent regulated cell death triggered by high lipid peroxide levels, has been implicated in several neurodegenerative diseases, including Parkinson's disease (PD). Brain regions such as the striatum are highly rich in both peroxidation susceptible PUFAs and iron, which accumulate at a greater rate than age in PD. The exact molecular pathways and patho-physiological conditions promoting cell death in the dopaminergic neurons that are particularly susceptible in PD remain elusive. In the current work, we show that modifying the PUFA composition in membranes of dopaminergic neurons using arachidonic acid (AA) can determine ferroptosis susceptibility. Furthermore, cotreatment with iron (Fe), increases AA-containing phospholipid association and synergistically promotes high lipid peroxidation to facilitate ferroptosis. Ex vivo analysis with organotypic brain slices, confirm that AA + Fe induces cell death in the nigrostriatal pathway and can be rescued by the anti-ferroptotic drug Ferrostatin-1. Prevention of ferroptotic AA + Fe induced cell death through inhibition of ACSL4, ALOX15 or ALOX15B provides mechanistic support of this lipid peroxidation pathway being involved in dopaminergic neuronal death and novel potential pharmacological targets for neuroprotective strategies in PD.

Abstract: International audience; There is a continued unmet need for treatments that can slow Parkinson's disease progression due to the lack of understanding behind the molecular mechanisms underlying neurodegeneration. Since its discovery, ferroptosis has been implicated in several diseases and represents a therapeutic target in Parkinson's disease. Here, we use two highly relevant human dopaminergic neuronal models to show that endogenous levels of α-synuclein can determine the sensitivity of dopaminergic neurons to ferroptosis. We show that reducing α-synuclein expression in dopaminergic neurons leads to ferroptosis evasion, while elevated α-synuclein expression in patients' small-molecule-derived neuronal precursor cells with SNCA triplication causes an increased vulnerability to lipid peroxidation and ferroptosis. Lipid profiling reveals that ferroptosis resistance is due to a reduction in ether-linked phospholipids, required for ferroptosis, in neurons depleted of α-synuclein (α-syn). These results provide a molecular mechanism linking α-syn levels to the sensitivity of dopaminergic neurons to ferroptosis, suggesting potential therapeutic relevance.

Abstract: International audience; Background Apathy is a common behavioral syndrome that occurs across neurological and psychiatric disorders. An influential theoretical framework defined apathy as the quantitative reduction of self-generated voluntary and purposeful behaviors. There is evidence in the literature of the multidimensional nature of apathy with cognitive, behavioral, and emotional dimensions. To date, apathy has been assessed using various scales and questionnaires. Alternative objective and ecological measurements of apathy are needed. New method We used the ECOCAPTURE protocol and an ethological approach to investigate behavior in bvFTD patients under ecological conditions (a waiting room) while they freely explored a novel environment. Data were collected by behavioral coding from 7-minute video using an ethogram and transformed into behavior time series data. We present an approach considering behavioral kinetics to assess behavior. We aimed to construct a new behavior analysis method, called ECOCAPTURE kinetics, using temporal classification for behavior time series data analysis. To develop our classifier, we retained a nonelastic Euclidian metric, combined with a convolutional approach. Results We applied the ECOCAPTURE kinetics method to a cohort of 20 bvFTD patients and 18 healthy controls. We showed that bvFTD patients can be classified according to their behavioral kinetics into three groups. Each subgroup was characterized by specific behavior disorders and neuropsychological profile. Abstract: Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disorder resulting from biallelic alterations of the SMN1 gene: deletion, gene conversion or, in rare cases, intragenic variants. The disease severity is mainly influenced by the copy number of SMN2, a nearly identical gene, which produces only low amounts of full-length (FL) mRNA. Here we describe the first example of retrotransposon insertion as a pathogenic SMN1 mutational event. The 50-year-old patient is clinically affected by SMA type III with a diagnostic odyssey spanning nearly 30 years. Despite a mild disease course, he carries a single SMN2 copy. Using Exome Sequencing and Sanger sequencing, we characterized a SVA-F retrotransposon inserted in SMN1 intron 7. Using RT-PCR and RNASeq experiments on lymphoblastoid cell lines, we documented the dramatic decrease of FL transcript production in the patient compared to subjects with the same SMN1 and SMN2 copy number, thus validating the pathogenicity of this SVA insertion. We characterized the mutant FL-SMN1-SVA transcript and showed that it is subject to degradation by nonsense-mediated mRNA decay. The stability of the SMN-SVA protein may explain the mild course of the disease. This observation exemplifies the role of retrotransposons in human genetic disorders.

Abstract: International audience; Objective: To provide recommendations for enhanced recovery after cardiac surgery (ERACS) based on a multimodal perioperative medicine approach in adult cardiac surgery patients with the aim of improving patient satisfaction, reducing postoperative mortality and morbidity, and reducing the length of hospital stay. Design: A consensus committee of 20 experts from the French Society of Anaesthesia and Intensive Care Medicine (Societe francaise d'anesthesie et de reanimation, SFAR) and the French Society of Thoracic and Cardiovascular Surgery (Societe francaise de chirurgie thoracique et cardio-vasculaire, SFCTCV) was convened. A formal conflict-of-interest policy was developed at the onset of the process and enforced throughout. The entire guideline process was conducted independently of any industry funding. The authors were advised to follow the principles of the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system to guide the assessment of the quality of evidence. Methods: Six fields were defined: (1) selection of the patient pathway and its information; (2) preoperative management and rehabilitation; (3) anaesthesia and analgesia for cardiac surgery; (4) surgical strategy for cardiac surgery and bypass management; (5) patient blood management; and (6) postoperative enhanced recovery. For each field, the objective of the recommendations was to answer questions formulated according to the PICO model (Population, Intervention, Comparison, Outcome). Based on these questions, an extensive bibliographic search was carried out and analyses were performed using the GRADE approach. The recommendations were formulated according to the GRADE methodology and then voted on by all the experts according to the GRADE grid method. Results: The SFAR/SFCTCV guideline panel provided 33 recommendations on the management of patients undergoing cardiac surgery under cardiopulmonary bypass or off-pump. After three rounds of voting and several amendments, a strong agreement was reached for the 33 recommendations. Of these recommendations, 10 have a high level of evidence (7 GRADE 1+ and 3 GRADE 1-); 19 have a moderate level of evidence (15 GRADE 2+ and 4 GRADE 2-); and 4 are expert opinions. Finally, no recommendations were provided for 3 questions. Conclusions: Strong agreement existed among the experts to provide recommendations to optimise the complete perioperative management of patients undergoing cardiac surgery. (C) 2022 The Authors. Abstract: International audience; The expanding knowledge on the systemic influence of the human microbiome suggests that fecal samples are underexploited sources of new beneficial strains for extra-intestinal health. We have recently shown that acetate, a main circulating microbiota-derived molecule, reduces the deleterious effects of pulmonary Streptococcus pneumoniae and enteric Salmonella enterica serovar Typhimurium bacterial post-influenza superinfections. Considering the beneficial and broad effects of acetate, we intended to isolate a commensal strain, producing acetate and potentially exploitable in the context of respiratory infections. We designed successive steps to select intestinal commensals that are extremely oxygen-sensitive, cultivable after a freezing process, without a proinflammatory effect on IL-8 induction, and producing acetate. We have identified the Blautia faecis DSM33383 strain, which decreased the TNFα-induced production of IL-8 by the intestinal epithelial cell line HT-29. The beneficial effect of this bacterial strain was further studied in two preclinical models of post-influenza Streptococcus pneumoniae (S.p) and Salmonella enterica serovar Typhimurium (S.t) superinfection. The intragastrical administration of Blautia faecis DSM33383 led to protection in influenza-infected mice suffering from an S.p. and, to a lesser extent, from an S.t secondary infection. Altogether, this study showed that Blautia faecis DSM33383 could be a promising candidate for preventive management of respiratory infectious diseases.

Abstract: Introduction: Like other countries, France has invested in a national medical genomics program. Among the four pilot research studies, the DEFIDIAG project focuses on the use of whole genome sequencing (WGS) for patients with intellectual disability (ID), a neurodevelopmental condition affecting 1–3% of the general population but due to a plethora of genes. However, the access to genomic analyses has many potential individual and societal issues in addition to the technical challenges. In order to help decision-makers optimally introduce genomic testing in France, there is a need to identify the socio-economic obstacles and leverages associated with the implementation of WGS.Methods and Analysis: This humanities and social sciences analysis is part of the DEFIDIAG study. The main goal of DEFIDIAG is to compare the percentage of causal genetic diagnoses obtained by trio WGS (including the patient and both parents) (WGST) to the percentage obtained using the minimal reference strategy currently used in France (Fragile-X testing, chromosomal microarray analysis, and gene panel strategy including 44 ID genes) for patients with ID having their first clinical genetics consultation. Additionally, four complementary studies will be conducted. First, a cost-effectiveness analysis will be undertaken in a subsample of 196 patients consulting for the first time for a genetic evaluation; in a blinded fashion, WGST and solo (index case, only) genomic analysis (WGSS) will be compared to the reference strategy. In addition, quantitative studies will be conducted: the first will estimate the cost of the diagnostic odyssey that could potentially be avoidable with first-line WGST in all patients previously investigated in the DEFIDIAG study; the second will estimate changes in follow-up of the patients in the year after the return of the WGST analysis compared to the period before inclusion. Finally, through semi-directive interviews, we will explore the expectations of 60 parents regarding genomic analyses.Discussion: Humanities and social sciences studies can be used to demonstrate the efficiency of WGS and assess the value that families associate with sequencing. These studies are thus expected to clarify trade-offs and to help optimize the implementation of genomic sequencing in France.Ethics Statement: The protocol was approved by the Ethics Committee Sud Méditerranée I (June 2019)—identification number: 2018-A00680-55 and the French data privacy commission (CNIL, authorization 919361).Clinical Trial Registration: (ClinicalTrials.gov), identifier (NCT04154891).

Abstract: International audience; BRD4 is part of a multiprotein complex involved in loading the cohesin complex onto DNA, a fundamental process required for cohesin-mediated loop extrusion and formation of Topologically Associating Domains. Pathogenic variations in this complex have been associated with a growing number of syndromes, collectively known as cohesinopathies, the most classic being Cornelia de Lange syndrome. However, no cohort study has been conducted to delineate the clinical and molecular spectrum of BRD4-related disorder. We formed an international collaborative study, and collected 14 new patients, including two fetuses. We performed phenotype and genotype analysis, integrated prenatal findings from fetopathological examinations, phenotypes of pediatric patients and adults. We report the first cohort of patients with BRD4-related disorder and delineate the dysmorphic features at different ages. This work extends the phenotypic spectrum of cohesinopathies and characterize a new clinically relevant and recognizable pattern, distinguishable from the other cohesinopathies.

Abstract: International audience; Objective: The prognostication of metastatic pancreatic adenocarcinoma (mPDAC) patients remains uncertain, mainly based on carbohydrate antigen 19-9 (CA19-9), with limited utility. Circulating tumour DNA (ctDNA) has been suggested as a prognostic factor, but its added value has been poorly explored. The objective was to determine whether ctDNA is an independent factor for the prognostication of mPDAC.Design: Translational study based on two prospective collections of plasma samples of mPDAC patients naïve for chemotherapy. One used as a test series and the other as validation series coming from two randomised trials (Prodige 35 and Prodige 37). CtDNA was assessed by digital droplet PCR targeting two methylated markers (HOXD8 and POU4F1) according to a newly developed and validated method. Univariate and multivariate analyses were performed according to ctDNA status.Results: Of 372 plasma samples available, 354 patients were analyzed for survival. In the validation series, 145 of 255 patients were found ctDNA positive (56.8%), Median PFS and OS were 5.3 and 8.2 months in ctDNA-positive and 6.2 and 12.6 months in ctDNA-negative patients, respectively. ctDNA positivity was more often associated with young age, high CA19-9 level and neutrophils lymphocytes ratio. In multivariate analysis including these previous markers, ctDNA was confirmed as an independent prognostic marker for PFS (adjusted hazard ratio (HR) 1.5, CI 95% [1.03-2.18], p = 0.034) and OS (HR 1.62, CI 95% [1.05-2.5], p = 0.029).Conclusions: In this first ctDNA assessment in a large series of mPDAC derived from clinical trials, ctDNA was detectable in 56.8% of patients and confirmed as an independent prognostic marker.

This dormant form of bacteria, made of several layers of barriers which provide extreme resistance to many abiotic stresses (radiation, temperature, pressure, etc.), are difficult to investigate in situ. To better understand the biological and chemical mechanisms involved and specific to spores resistance, the acquisition of environmental parameters is necessary. For that purpose, our research has been focused on the detection and analysis of a unique spore component, dipicolinic acid (DPA), used as the main in situ metabolite for sporulating bacteria detection. In its native form, DPA is only weakly fluorescent but after Ultraviolet irradiation at the wavelength of 254 nm (UVc), DPA photoproducts (DPAp) exhibit a remarkable fluorescence signal. These photoproducts are rarely identified and part of this study gives new insights offered by mass spectrometry (MS) in the determination of DPA photoproducts. Thanks to DPA assay techniques and fluorescence spectrometry, we highlighted the instability of photoproducts and introduced new assumptions on the effects of UVc on DPA. Studies in spectroscopy and microscopy allowed us to better understand these native probes in bacterial spores and will allow the implementation of a new method for studying the physico-chemical parameters of spore resistance.

Abstract: International audience; Mid-infrared spectroscopy was explored in order to evaluate its ability to classify geographical origin and bitter content of Gentiana lutea roots sourced from wild and cultivated growing conditions. Wild and cultivated Gentiana lutea roots from the four French mountains Massif Central, Jura, Alpes and Pyrénées were analyzed by Infrared spectroscopy and liquid chromatography. Unsupervised analyses assessed heterogeneity of Gentiana lutea roots in the different sampling sites due to their evolutive composition along plant growth. Predictive models using partial least squares discriminant analysis and probabilistic artificial neural network were discussed according to FTIR spectral regions and gave 100 % accuracy in authentication of geographical origin and growing conditions. Nevertheless, classification of gentian roots according to their bitter content, based on FTIR spectral signatures, was challenging due to their biological heterogeneity. We propose an unprecedented classification of Gentiana lutea roots according to their analyzed bitter content: Low, Medium and High, comprised in the range [6–8] %, [8–10] % and [10–12] % in dry weight, respectively. FTIR coupled with chemometrics applied directly on gentian roots allowed for a decent level of predictability by PLS-DA (Q2Cum = 0.41) and Artificial Neural Network (89.1 % accuracy), when using the (650–1800 cm−1) infrared region.

Abstract: International audience; In this study, stationary and time-resolvedfluorescence signatures, were statistically and chemometrically analyzed among three typologies of Chardonnay wines (A, B and C) with the objectives to evaluate their sensitivity to acidic and polyphenolic changes. For that purpose, a dataset was built using Excitation Emission Matrices of fluorescence (N = 103) decomposed by a Parallel Factor Analysis (PARAFAC), andfluorescence decays (N = 22), mathematically fitted, using the conventional exponential modeling and the phasor plot representation. Wine PARAFAC component C4 coupledwith its phasor plot g and s values enable the description of malolactic fermentation (MLF) occurrence in Chardonnay wines. Such proxies reflect wine concentration modifications in total acidity, malic/lactic and phenol acids.Lower g values among fresh MLF + wines compared to MLF- wines are explained by a quenching effect on wine fluorophores by both organic and phenolic acids.The combination of multispectral fluorescence parametersopens a novel routinely implementable methodology to diagnose fermentative processes.

A partir des données des recensements de la population de 2007 et 2017, une classification des villes moyennes met en évidence les dynamiques contrastées qu'elles connaissent depuis les années 2000. La typologie ainsi obtenue est ensuite analysée au prisme des transformations sociodémographiques de 1968 à 2017 des différentes classes qui la composent. Nous montrons que le déclin urbain concerne prioritairement les villes moyennes situées dans le Centre et le Nord-Est de la France, caractérisées par une spécialisation ancienne dans la production industrielle. Nous analysons, ensuite, certaines conséquences de ces dynamiques contrastées. Si celles-ci procèdent initialement des différences de spécialisation productive, elles tendent de plus en plus à distinguer les villes selon le degré auquel leurs populations se paupérisent.

Abstract: International audience; Despite an increasing acknowledgement of fathers’ involvement in and impact on children’s lives, fathers remain underrepresented in child feeding and childhood obesity prevention research, interventions, and actions. Built on our own experiences with conducting research with fathers and recent evidence on this topic, this Research in Practice article has three aims. It will first substantiate the importance of including and studying fathers in the field of child feeding and childhood obesity prevention based on recent study results. Secondly, the article will present and discuss barriers to fathers' inclusion and participation (why isn't it happening?), among other issues, by drawing on sociological and gender ideological insights. Finally, it will provide recommendations and suggestions related to recruitment, focus and methods that can facilitate fathers’ involvement in future research, interventions, and practice. Taken together, this article aims to provide tools for giving fathers a voice in the field of child nutrition and by doing so, to decrease maternal-only “burden” of care. We hope our experiences and theoretical reflections will inspire and support researchers and practitioners to be as successful as possible in the realm of family care.

Abstract: Background: With the development of next generation sequencing technologies in France, exome sequencing (ES) has recently emerged as an opportunity to improve the diagnosis rate of patients presenting an intellectual disability (ID). To help French policy makers determine an adequate tariff for ES, we aimed to assess the unit cost per ES diagnostic test for ID from the preparation of the pre-analytical step until the report writing step and to identify its main cost drivers.Methods: A micro-costing bottom-up approach was conducted for the year 2018 in a French setting as part of the DISSEQ study, a cost-effectiveness study funded by the Ministry of Health and performed in collaboration with the GAD (Génétique des Anomalies du Développement), a genetic team from the Dijon University Hospital, and a public sequencing platform, the Centre National de Recherche en Génomique Humaine (CNRGH). The analysis was conducted from the point of view of these two ES stakeholders. All of the resources (labor, equipment, disposables and reagents, reusable material) required to analyze blood samples were identified, collected and valued. Several sensitivity analyses were performed.Results: The unit nominal cost per ES diagnostic test for ID was estimated to be €2,019.39. Labor represented 50.7% of the total cost. The analytical step (from the preparation of libraries to the analysis of sequences) represented 88% of the total cost. Sensitivity analyses suggested that a simultaneous price decrease of 20% for the capture kit and 50% for the sequencing support kit led to an estimation of €1,769 per ES diagnostic test for ID.Conclusion: This is the first estimation of ES cost to be done in the French setting of ID diagnosis. The estimation is especially influenced by the price of equipment kits, but more generally by the organization of the centers involved in the different steps of the analysis and the time period in which the study was conducted. Abstract: This article is part of the themed collection: Food & Function HOT Articles 2023.; International audience; Understanding the mechanisms of food digestion is of paramount importance to determine the effect foods have on human health. Significant knowledge on the fate of food during digestion has been generated in healthy adults due to the development of physiologically-relevant in vitro digestion models. However, it appears that the performance of the oro-gastrointestinal tract is affected by ageing and that a model simulating the digestive conditions found in a younger adult (65 years). The objectives of the present paper were: (1) to conduct an exhaustive literature search to find data on the physiological parameters of the older adult oro-gastrointestinal tract, (2) to define the parameters of an in vitro digestion model adapted to the older adult. International experts have discussed all the parameters during a dedicated workshop organized within the INFOGEST network. Data on food bolus properties collected in the older adult were gathered, including food particle size found in older adult boluses. In the stomach and small intestine, data suggest that significant physiological changes are observed between younger and older adults. In the latter, the rate of gastric emptying is slowed down, the pH of the stomach content is higher, the amount of secretions and thus the hydrolytic activities of gastric and intestinal digestive enzymes are reduced and the concentration of bile salts lower. The consensus in vitro digestion model of the older adult proposed here will allow significant progress to be made in understanding the fate of food in this specific population, facilitating the development of foods adapted to their nutritional needs. Abstract: International audience; Interactions among plants have been long recognized as a major force driving plant community dynamics and crop yield. Surprisingly, our knowledge of the ecological genetics associated with variation of plant-plant interactions remains limited. In this opinion article by scientists from complementary disciplines, the international PLANTCOM network identified four timely questions to foster a better understanding of the mechanisms mediating plant assemblages. We propose that by identifying the key relationships among phenotypic traits involved in plant-plant interactions and the underlying adaptive genetic and molecular pathways, while considering environmental fluctuations at diverse spatial and time scales, we can improve predictions of genotype-by-genotype-by-environment interactions and modeling of productive and stable plant assemblages in wild habitats and crop fields.

Abstract: International audience; The healthcare burden of cardiovascular diseases remains a major issue worldwide. Understanding the underlying mechanisms and improving identification of people with a higher risk profile of systemic vascular disease through noninvasive examinations is crucial. In ophthalmology, retinal vascular network imaging is simple and noninvasive and can provide in vivo information of the microstructure and vascular health. For more than 10 years, different research teams have been working on developing software to enable automatic analysis of the retinal vascular network from different imaging techniques (retinal fundus photographs, OCT angiography, adaptive optics, etc.) and to provide a description of the geometric characteristics of its arterial and venous components. Thus, the structure of retinal vessels could be considered a witness of the systemic vascular status. A new approach called "oculomics" using retinal image datasets and artificial intelligence algorithms recently increased the interest in retinal microvascular biomarkers. Despite the large volume of associated research, the role of retinal biomarkers in the screening, monitoring, or prediction of systemic vascular disease remains uncertain. A PubMed search was conducted until August 2022 and yielded relevant peer-reviewed articles based on a set of inclusion criteria. This literature review is intended to summarize the state of the art in oculomics and cardiovascular disease research.

Abstract: Ecological farming, such as organic and low-input farming, is gaining popularity in the public discourse. One question is how this type of farming may impact farm labour from a socio- economic point of view. The article first discusses how low-input farming practices (i.e. with lower reliance on inputs derived from fossil fuels) may affect the economic returns to labour, measured as the farm’s revenue per hour of labour input, on data from the Farm Accountancy Data Network (FADN) in 2004-2015 for four European countries. Returns to labour appear to be highest at the two extremes – very low-input farms and highly intensive farms. Farms in the low-input end of the spectrum are in the minority, while the overwhelming majority of farms are intensive and have internal economic incentives to intensify further. The article also analyses how working conditions differ between organic and conventional dairy farms in two European countries based on interviews with farmers in 2019. Results show that all dimensions of working conditions are affected by being an organic farm or not, but this is not the only factor. There are many influences on working conditions, such as the production context and workforce composition.

Abstract: Background and purpose: Standard treatment of squamous cell carcinoma of the anus (SCCA)is 5-fluorouracil (5FU) and mitomycin C (MMC) based chemoradiotherapy (CRT). This phase II study (EudraCT: 2011-005436-26) assessed the tolerance and complete response (CR) rate at 8 weeks of panitumumab (Pmab) combined with MMC-5FU-based CRT.Methods: Patients with locally advanced tumors without metastases (T2 > 3 cm, T3-T4, or N + whatever T stage) were treated with IMRT up to 65 Gy and concomitant CT according to the doses defined by a previous phase I study (MMC: 10 mg/m2; 5FU: 400 mg/m2; Pmab: 3 mg/kg). The expected CR rate was 80%.Results: Forty-five patients (male: 9, female: 36; median age: 60.1 [41.5-81]) were enrolled in 15 French centers. The most common related grade 3-4 toxicities observed were digestive (51.1%), hematologic (lymphopenia: 73.4%; neutropenia: 11.1%), radiation dermatitis (13.3%), and asthenia (11.1%) with RT interruption in 14 patients. One patient died because of mesenteric ischemia during the CRT, possibly related to treatment. In ITT analysis, the CR rate at 8 weeks after CRT was 66.7% [90%CI: 53.4-78.2]. Median follow-up was 43.6 months [IC 95%: 38.61-47.01]. Overall survival, recurrence-free and colostomy-free survival at 3 years were 80% [95%CI: 65.1-89], 62.2% [IC95%: 46.5-74.6] and 68.8 % [IC95%: 53.1-80.2] respectively.Conclusion: Panitumumab in combination with CRT for locally advanced SCCA failed to meet the expected CR rate and exhibited a poor tolerance. Abstract: International audience; Background: Following European guidelines, patients with aggressive metastatic or locally advanced, non-resectable, duodeno-pancreatic (DP) neuroendocrine tumours (NETs) should receive systemic combination chemotherapy until progression. Aggressive disease is defined as progressive and/or symptomatic metastases with or without significant hepatic invasion (>30–50%), and/or bone metastases. Methods: This academic randomised, double-blind, placebo-controlled phase II study aims to evaluate lanreotide autogel 120 mg (LAN) as maintenance treatment after at least 2 months of first-line treatment (L1) in aggressive G1-G2 DP-NET. Patients were randomly assigned in a 1:1 ratio to receive LAN or placebo (PBO), every 28 days, until progression or toxicity. The primary end-point was progression-free survival (PFS) at 6 months. Results: Among the 118 planned patients, 53 were included. Of these, 81.1% had a G2 tumour, and 90.6% had metastatic disease. L1 therapy consisted of chemotherapy (96.8%). Median duration of L1 was 4.6 months (range: 2.0–7.7). At the time of randomisation, 81.1% of patients had stable disease. Median follow-up was 27.0 months (95% CI: 19.5; 31.2). PFS at 6 months was 73.1% (90% CI: 55.3; 86.6) in LAN versus 54.2% (90% CI: 35.8; 71.8) in PBO. Median PFS was 19.4 months (95% CI: 7.6; 32.6) and 7.6 months (95% CI: 3.0; 9.0), respectively. Median overall survival was 41.9 months in PBO and was not reached in LAN. The toxicity profile was mainly grade 1–2 expected toxicities. Conclusions: The encouraging results of lanreotide autogel 120 mg as a maintenance treatment after L1 in aggressive G1/2 DP-NET should be confirmed. Abstract: International audience; Climate change is driving the need to investigate responses to water limitation of morphological traits involved in competition for light, the main resource for which crops and weeds compete in conventional temperature and tropical cropping systems, to better understand field crop-weed dynamics. Our objective was to develop an innovative approach to quantify weed species responses to water limitation, using three species. This approach combined (1) key morphological traits involved in competition for light (taken from a mechanistic crop-weed model) as criteria to analyse responses to water limitation and (2) a pot/greenhouse platform allowing automated precision-watering and daily quantification of soil water availability in each pot. For all species and growth stages, increased plant height per unit of aboveground biomass and production of smaller/thicker leaves were the most noteable responses. Plants with a strong increase in plant height per unit of aboveground biomass in response to water limitation maintained high levels of specific leaf area, even at low soil water availability. Increases in biomass allocation to roots (vs. aboveground parts) and leaves (vs. stems and reproductive organs) were also observed, but not for all species and growth stages. Overall, these effects of water limitation on morphological traits indicate strong interactions between competition for light and water.

Abstract: International audience; Payments for Environmental or Ecosystem Services (PES) schemes have become a popular tool to addressenvironmental degradation and to promote sustainable management of ecosystem services. We use metaregressionanalysis on a sample of 110 individual studies to investigate the determinants of the environmentaleffectiveness, defined as the probability to increase environmental services (ES) provision, of about 149 PESschemesimplemented worldwide. We find that increased effectiveness of PES schemes is strongly associatedwith periodical third-party monitoring, generic reference design and to a lesser extent results-based payments.We further study the determinants of PES additionality, defined as direct changes in ES provision induced by thePES scheme, compared to a baseline without PES, on a smaller sample of 41 studies from which we could obtainthe necessary data. The results confirm the role of certain design variables, such as monitoring type, and raise apotential trade-off between enrolment and additionality in the assessment of PES effectiveness.
