2. HBV, HCV and HIV among inmates in Latin America and the Caribbean: A systematic review and meta‐analysis.

Objectives Methods Results Conclusions Hepatitis B virus (HBV), hepatitis C virus (HCV) and human immunodeficiency virus (HIV) affect 340 million people worldwide and have a high impact on vulnerable populations. The aim of this systematic review and meta‐analysis was to estimate the prevalence of these infections among inmates in Latin America and the Caribbean.Searches were conducted in Medline, Embase, LILACS and Web of Science databases on 17 May 2024, without time or language restriction, according to PRISMA guidelines. The Joanna Briggs Institute Critical Appraisal Checklist was used to assess the bias risk in the selected studies. Meta‐analysis was performed by using the random‐effects model and heterogeneity between studies was assessed with I2 statistic. Subgroup analyses and meta‐regression were performed to investigate possible sources of heterogeneity.Seventy‐nine studies were included, encompassing over 230,000 inmates. The estimated prevalences of HBV, HCV and HIV were 1.0% (95% CI: 0.0–1.0), 7.0% (95% CI: 6.0–8.0) and 4.0% (95% CI: 3.0–4.0), respectively. Subgroup analysis showed the following prevalences: HBV among females was 1.0% (95% CI: 0.0–3.0) and among males was 10.0% (95% CI: 3.0–17.0); HCV among females and males was 12.0% (95% CI: 7.0%–17.0%); and HIV among females was 10.0% (95% CI: 5.0–15.0) and among males was 8.0% (95% CI: 6.0–10.0).A substantial proportion of inmates in Latin America and the Caribbean are infected with HBV, HCV or HIV, underscoring the need for viral infection testing, primary care for inmates and increased efforts to promote public health policies for the prison system. The fulfilment of sexual and biological needs for inmates with valid life partners who are undergoing sentences in correctional institutions is a fundamental human right. Human rights, as a legal and normative concept, highlight that individuals possess inherent rights simply because they are human beings. Human rights apply to everyone, anytime and anywhere, are universal, and fundamentally cannot be revoked (non-derogable rights). Therefore, addressing the right to fulfil sexual biological needs is not only a matter that should be considered but also a responsibility of the State to find solutions. As of now, there are no specific regulations governing this matter. Hence, in line with the primary goal of correctional institutions, which conceptualise themselves as places of punishment and facilities where inmates receive benefits for their future lives, it is crucial to treat inmates humanely. This aligns with the concept of a legal paradigm where the ultimate goal of the law is to promote human well-being. Inmates' food refusal is a large-scale phenomenon raising clinical, ethical, and professional responsibility issues. Obtaining a clinical balance of the right to refuse food with the right to protect the inmate's health can be a challenging process. Several reasons may support inmates' choice of refusing food, including political or protest reasons, as well as psychiatric disorders. The World Medical Association defines a hunger striker as a mentally competent person who has indicated his/her decision to go on a hunger strike by refusing to take any food and/or liquid for a significant amount of time. Force-feeding of mentally capable people is not allowed by a legal standpoint, nor medical treatment can be carried out without patients' explicit consent. According to these premises, both the clinical assessment of possible concurrent psychiatric disorders as well as the assessment of inmates' mental capacity to make decisions about refusing food might help in understanding this behavioural issue. This systematic review aims to explore the condition of Transgender Inmates (TGI) in correctional facilities (CF), focusing on their mental health, their access to Gender-affirming Treatments (GAT), and their placement within the CF. Medline, Embase, Scopus, PsychInfo, and CINAHL were searched for studies on TGI in CF. Pooled Odds Ratios (ORs) with 95% confidence interval (95%CI) were estimated through inverse variance models with random-effects. A narrative synthesis of each study's findings was also performed since only a few studies included a control group. Thirteen studies were selected and four considered in the meta-analysis, corresponding to 1,255 TGI and 174,314 controls. Although limited by the small number of studies, the meta-analysis showed that TGI have a higher risk of depression, post-traumatic stress disorder, and suicide attempts during detention than the controls (OR = 3.07, 95% CI = 1.33–7.06; OR = 2.23, 95% CI = 1.46–3.43; OR = 2.25, 95% CI = 1.46–3.49, respectively). Limited access to GAT and housing based on sex assigned at birth rather than gender identity were additional challenges for TGI, threatening their safety and self-affirmation. Our findings highlight the critical role of incarceration as a social determinant of health for TGI. A comprehensive policy reform and staff education should be promoted to foster inclusive and supportive correctional environments and improve safety and health of TGI. [ABSTRACT FROM AUTHOR]

6. Interoceptive sensibility and alcohol craving in Polish prisoners: the role of alexithymia and emotional dysregulation.

Introduction: Alcohol craving, characterized by a strong desire or compulsion to consume alcohol, is a prominent symptom of substance dependence syndrome. Research indicates that alcohol craving is a significant factor leading to the termination of abstinence. The mechanisms underlying the activation of alcohol craving remain not fully understood. The urge to reach for alcohol may be stimulated by emotions, memories, thoughts, or bodily sensations, as well as external factors. It has been postulated that individuals with high levels of interoceptive sensibility tend to exhibit a high degree of alexithymia and emotion dysregulation in the context of alcohol craving. Deficits in identifying and verbalizing emotions, along with an operational thinking style, facilitate alcohol consumption by impeding accurate insight into one's mental state, thereby hindering the comprehension of bodily states, emotions, and the regulation of self. Method: This study involved 160 inmates incarcerated in a prison in Poland, awaiting participation in therapy for individuals with substance dependence following psychiatric diagnosis. Four questionnaires were used in the study: multidimensional Assessment of Interoceptive Sensibility (MAIA) for interoceptive sensibility, Toronto Alexithymia Scale (TAS-20) for alexithymia, Difficulties in Emotion Regulation Scale (DERS) for emotional dysregulation, and the Penn Alcohol Craving Scale (PACS) for alcohol craving assessment. Results: The results of the study are as follows: the study findings indicated that alexithymia and emotional dysregulation significantly mediates the relationship between interoceptive sensibility and alcohol craving. The indirect effect for both factors was found to be significant, similar to the indirect effect observed for alexithymia as an mediator. However, in the case of emotional dysregulation, no significant indirect effect was observed. Conclusion: Our study provides insights into the potential contribution of interoceptive sensibility to the heightened risk of alcohol dependence. Specifically, impaired interoceptive sensibility may be associated with the development of alexithymia and emotional dysregulation, potentially rendering individuals more susceptible to alcohol craving. Interoceptive sensibility could serve as a prerequisite for the cultivation of positive emotional processing skills. [ABSTRACT FROM AUTHOR]

Purpose Methods Results Conclusions The reaction time‐based Concealed Information Test (RT‐CIT) can be used to reveal crime‐related memories based on an analysis of response latencies in a computerized task. While laboratory research shows high validity of the RT‐CIT in student samples, only very few studies have been conducted with forensic target groups.We investigated the applicability and validity of the RT‐CIT in prison inmates and examined the influence of a response deadline (RD) on RT‐CIT validity. In a within‐subjects design, participants memorized a criminal activity and subsequently underwent two identical RT‐CITs differing only in the length of the RD (2000 ms vs. 1000 ms). In the first experiment, the 2000 ms condition was always presented first and in the second experiment the order was switched.Results revealed large effect sizes and classification accuracies with both RDs and strong order effects with larger effects in the respective first RD conditions.In sum, the RT‐CIT has the potential for detecting concealed knowledge in forensic settings and even a relatively demanding RD of 1000 ms seems feasible for such samples. Further research should extend this investigation of the RT‐CIT in forensic settings, preferably investigating the effects of faking attempts and increasing the ecological validity of the research design. Suicidal ideation assessment can help reduce the suicide rate among people in prison. Studies for the assessment of suicidal ideation and risk factors for suicide among inmates have been done in Western countries, but in India, data is lacking regarding the assessment of suicidal ideation among the people in prison.1. To assess the prevalence of suicidal ideationin people in prison.2. To identify aggravating factors for suicidal behavior.It was a cross Sectional Observational Study at a district jail, Lajpore, Surat, Gujarat. After permission from the jail authority, people in prison with or without conviction, who were willing to give consent for participation, were enrolled in the study. Semi-structured questionnaires are used to assess socio-demographic profiles. Modified Suicide Ideation scale (MSSI) and General Health Questionnaire (GHQ) and PatientHealth Questionnaire (PHQ) scales were used as materials to assess suicidal ideation and psychiatric comorbidity. Frequency analysis and Pearson correlation among various factors with the use of appropriate statistical methods. Descriptive and qualitative analysis of data was done on 140 incarcerated persons in prison. Analysis showed that 10 (7.1%) people in prison had moderate suicidal ideation in the last 48 hours while 58 (41.4%) people in prison had suicidal ideation at any point in their life. The prevalence of depressive symptoms was high among people in prison. 57.9% had minimal depressive symptoms, while 9.3% had moderate to severe symptoms. Non-convicted inmates from urban areas were having significantly more psychological distress and depressive symptoms than other inmates. Advanced ageof the inmates, negative impact of imprisonment on life, depressive symptoms, and psychosocial distress emerged as risk factors correlated with suicidal ideation. During suicide risk assessment, inmate’s age, the impact of imprisonment, psychosocial distress, and depressive symptoms should be considered as risk factors among people in prison. Despite consistent literature supporting recidivism, data-based studies that explore the societal factor-recidivism nexus are essentially of Western sources, so only little empirical evidence linking the two variables exists in Nigeria. A cross-section of 38 recidivists were recruited using purposive and snowballing techniques. Thematic method was used to analyze the interview data. Different predictive variables were found, showing their areas of convergence and divergence, in recidivism causation. While lower class, poverty, sociocultural exclusion, inequality, discrimination, and stigmatization accounted for the leading causes, unemployment was not acknowledged as a main and/or direct predictor. The problem has severely impacted prison reforms, criminal justice administration, and norm validation that are aimed at (re)socialization of people within and outside the correctional institutions. Conducting in-depth oral interviews with offenders presents diverse practical pathways toward understanding the dynamics of offending and offenders, as well as missing links in the existing policy, research, and practice with a view to strengthening the evidence base and establishing what works in the real world. Gaining firsthand knowledge and insider information from the recidivists themselves will add to the global call for synergy and mutual reinforcement in both State and community-based policing, justice, and corrections for better performance outcomes. This study aims to analyze the relationship of social support provided by correctional officers to the subjective well-being of inmates in the Class IIA Kotabumi Correctional Institution. The social support received by the inmates, whether in the form of emotional, instrumental, informational, or reward support, is thought to have a significant effect on their subjective feelings of well-being. This study uses a quantitative approach with survey and questionnaire methods as data collection instruments. The sample of this study is inmates in the Class IIA Kotabumi Prison who are randomly selected. Data analysis was carried out using the Pearson correlation test to test the relationship between social support variables and subjective well-being. The results of the study are expected to provide insight into the importance of the role of correctional officers in improving the psychological welfare of inmates and provide recommendations for more humane correctional institution management policies. Abstract The study by Mwatenga et al. found a tuberculosis (TB) prevalence of 10.2% and an HIV prevalence of 19.1% among inmates, with all TB cases co-infected with HIV. Education level, smoking history, and substance use are key predictors of coinfections. Although informative, its single-centred design and reliance on sputum samples may be insufficient for some patient groups, potentially compromising diagnostic accuracy. Expanding the study to include several jails and employing more diagnostic procedures may increase reliability and the ability to generalize. The report also notes the lack of debate on broader socioeconomic variables and structural barriers to healthcare in jails, which are crucial to understanding inmates' health challenges. Overcrowding, malnutrition, and a poor healthcare system are examples of environmental factors that probably contribute to the spread of tuberculosis and make managing HIV more difficult. Additionally, not enough is known about the dietary habits of prisoners and other health conditions like diabetes or mental health, which may have an impact on the course of HIV and TB. Future studies should take these extra characteristics into account to create more comprehensive approaches to controlling HIV coinfections and TB in prison populations. This will help build more effective therapies.

Introduction: The high rate of incarceration, now exceeding 11.5 million people worldwide, has raised concerns about the conditions within penal institutions, such as the consequences of incarceration on the person. This retrospective study aims to investigate the issue of death in custody, exploring the relationship between incarceration, health vulnerabilities, and death from the forensic pathologist's point of view. Methods: We analyzed, from a forensic, clinical and toxicological perspective, 86 cases of deaths in detention facilities in North-Western Italy from 1999 to 2022. Results: The analysis has shown that suicide, mainly committed by hanging, plastic bag suffocation and butane intoxication, represents the leading type of violent death (52%), followed by accidental deaths (16%). On the other hand, cardiovascular diseases are the leading cause of natural deaths (42%), followed by infectious diseases (especially HIV-related). Discussion: The present study identifies the most frequent and critical situations and risk factors related to death in custody and the profile of the inmate who is at a higher risk of death, allowing to highlight the issues to be addressed from a public health point of view. On the whole, it calls for comprehensive reforms, aligned with international human rights standards, addressing mental and physical care gaps, improving correctional officers' education, and training, and focusing on rehabilitation with well-being and dignity. 15. Oral health status of prisoners in India: A systematic review and meta‐analysis.

Aims: Given the high prevalence of oral health problems among prisoners, the goal of this systematic review is to provide a better knowledge of the scope of this problem. Methods: Electronic searches of PubMed/MEDLINE, Embase, Scopus, and Google Scholar were performed. Studies that investigated inmates aged 18 or older with oral health problems were eligible. Variables reported in four or less studies were described narratively. Conversely, for variables reported in more than four studies, a meta‐analysis was performed using random effect model. Furthermore, meta‐regression and sensitivity analysis is also performed to evaluate moderator effect on outcome. Doi and LFT index is applied to assess publication bias. Results: Out of 494 results, 12 studies were included. The pooled prevalence of caries among prisoners is 78.42% (59.48%–92.58%). On meta‐regression, the prevalence of caries appears to be lower in studies with a higher male percentage; however, non‐significant (p =.079) due to small sample size. Community periodontal index (CPI) scores revealed periodontal disease, with scores of 3 and 4. Moreover, a significant need for oral hygiene instruction, prosthesis, extraction, and tooth ache, periodontal disease, oral mucosal lesions, leucoplakia, attrition, abrasion, bruxism, and smoking behaviors were also reported. Conclusion: Poor oral health status in the incarcerated population highlights the urgent need for comprehensive oral health intervention in prisons. [ABSTRACT FROM AUTHOR]

16. Prison healthcare service use and associated factors: a cross sectional study in Northwestern Ethiopia.

Introduction: Studies on inmates' Health Care Service (HCS) utilization are scarce globally, infrequent in Ethiopia while findings about the factors associated with HCS utilization are inconsistent. The present study, therefore, examined inmates' HCS utilization and associated socio-demographic and imprisonment related factors in Northwestern Ethiopia. Methods: The study employed institution-based cross sectional research design and data was collected using questionnaire from 422 inmates in three prisons. The questionnaire collected data about prisoners' demographic characters, imprisonment related information and HCS utilization. Descriptive statistical techniques as well as bi-variate and multiple logistic regressions were used to analyse the data. Results and discussions: The study found that 72.5%, 66.1% and 13.3% of the inmates, respectively, used medical services, guidance and counseling services, and psychiatric services. Inmates with primary education, with secondary education, and who know the availability of the services were more likely to use medical services. Inmates with accused status were less likely to use medical services than inmates with convict status. Divorced marital status and knowledge of the service availability were associated with high guidance and counseling service use. An increase in the length of stay in the prison was associated with a decrease in psychiatry service use while knowledge of service availability was associated with higher odds of psychiatry service use. There are high medical care service utilization while low mental health care service utilization among inmates in Northwestern Ethiopia. Results of the study implied that there is a critical need for immediate health care service promotion and education measures. Besides, there are also needs for large scale, longitudinal and potentially cross-cultural studies to better understand additional factors that influence inmates' HCS utilization. Introduction: The behavioral addiction model posits that repetitive suicidal behaviors can serve as maladaptive strategies for managing stress and negative emotional states, akin to substance addiction. Both behaviors involve negative emotions, offer temporary psychological relief, and persist, indicating shared neurobiological mechanisms. This study explored psychometric differences among major repeaters, occasional attempters, and non-suicidal prisoners. Methods: A multi-centre cross-sectional survey of 363 inmates across four prisons assessed depression, cognitive-emotional regulation, impulsivity, perceived stress, lifetime non-suicidal self-injury and suicide attempts. Results: Mild depression, moderate suicidal ideation, and moderate impulsivity were common, with nearly half of the participants having attempted suicide at least once. Hierarchical multiple regression analyses revealed that repeated suicidal behavior in the past increases susceptibility to future suicidal thoughts, with suicide attempts serving as a maladaptive emotion regulation strategy among repeated attempters. Discussion: The results reveal differences in emotional dysregulation, impulsivity, and stress coping strategies among the studied groups, reinforcing the idea of suicidality as a form of behavioral addiction. The addiction approach helps explain the sensitivity to later suicidal thoughts in former attempters and self-harmers, offering valuable insights for tailored interventions within correctional settings. The current study aims to explore the impact of imprisonment of one of the parents on children, by analyzing the psychological, economic, and social effects they face, in addition to evaluating the role of family and community support in mitigating these effects. The importance of the research lies in understanding how imprisonment affects children's lives, and highlighting the need for effective strategies to support them and provide the necessary assistance to improve their conditions. The problem of the study is the major challenges that children face as a result of the imprisonment of one of the parents, which range from deep psychological effects, such as anxiety and depression, to the negative impact on the economic situation of the family and the social relationships of children. It is necessary to address these problems by providing appropriate support and the necessary assistance to improve the quality of life of affected children. The study reached a set of conclusions and recommendations aimed at improving the conditions of children affected by the imprisonment of one of the parents. The conclusions include the importance of providing psychological and educational support, the need to enhance financial assistance, and the importance of community awareness to reduce social stigma. The main recommendations include developing psychological support programs, improving access to educational resources, and enhancing cooperation between local institutions to provide comprehensive support for children. In general, the study highlights the need for concerted efforts to improve the lives of children affected by the imprisonment of a parent, through effective family and community support, which contributes to enhancing their ability to adapt and deal with the challenges they face. The objective of this study was to analyze the epidemiological links of the human immunodeficiency virus (HIV), hepatitis C virus (HCV) and HIV-HCV coinfections to less studied types of transmission in certain populations. We performed an observational, prospective study on 903 patients aged between 15–87 years who took part in the Open Test Project. They were divided in two subgroups: general population vs. individuals from prisons who were questioned about multiple risk factors. A chi-square independence test was used to establish correlations between risk factors and results of screening tests. Logistic regression was used to calculate the probability of a reactive screening test based on each independent risk factor and age. HIV was very strongly associated with unprotected sexual intercourse with HIV-positive partners (the strongest association), unprotected sexual intercourse with sex workers, newly diagnosed sexually transmitted diseases (STDs), intravenous drug users (IDUs) and sharing injecting materials. In the case of HCV reactive tests, very strong associations have been established with IDUs (the strongest association), unprotected sex with IDUs and sharing injecting materials. Our study indicates the need for implementing targeted public health programs, tailored to the local epidemiology that can ultimately lead to micro-elimination of hepatitis and HIV infections in this area. The present study investigates the impact of the panoptic system on the reduction of violence and the rehabilitation of inmates. Conceived by Jeremy Bentham, the system promotes constant surveillance to instill discipline. The research aims to understand the control mechanisms in the prison system and their relationship with recidivism. The objectives include evaluating the system's effects on reducing violence, analyzing crime prevention strategies, investigating the psychological effects of surveillance, and improving professional training programs for inmates. The aim is to understand the interconnection between four key elements: the panoptic system, inmate monitoring, the sense of constant surveillance, and professional training. The system, based on constant surveillance, is effective in controlling and disciplining prisoners. Electronic monitoring helps prevent crimes and reduce overcrowding. Organizational culture affects inmate rehabilitation and prison efficiency. Training is crucial for inmate reintegration and prison management improvement. These elements highlight the need for innovative approaches to address prison system challenges. Society will benefit from a deeper understanding of the use of the panoptic system in reducing prison violence and rehabilitating inmates. The adopted methodology is systematic literature review, using various sources for in-depth analysis and pattern identification. 22. Drivers of adult educational engagement in Ecuadorian prisons: A gender perspective

This research paper reports an investigation on the determinants of educational engagement among the whole Ecuadorian prison population as collected in the penitentiary census conducted in 2022. By employing a probit regression model, the analysis reveals distinct patterns: while age and ethnicity significantly influence participation among male inmates, environmental factors like overcrowding play a pivotal role for female prisoners. Moreover, higher levels of formal education negatively correlate with educational engagement across both genders. These findings underscore the need for targeted policies that address both individual and environmental barriers to educational participation within Ecuadorian prisons. By understanding these dynamics, policymakers can develop effective interventions to improve prison conditions and enhance rehabilitation outcomes, contributing to broader societal goals of justice and sustainable development.

AbstractThe number of male prisoners is several times that of female prisoners worldwide, yet paradoxically, women illicit drug users outnumber men. Even though the use of illicit drugs is prohibited within prisons, the substances continue to permeate prisons and the behaviour thrives therein. The channels through which illicit drugs make their way into female correctional centres in South Africa otherwise referred to as female prisons in other parts of the world, is one of the findings of this study. Based on the interviews that were conducted with seven formerly incarcerated women in South Africa, the other findings of this qualitative study include the availability of illicit drugs in the correctional centres for females; the attitudes of wardens to illicit drug use within female correctional centres; and the variety of illicit drugs used by the female offenders. This paper analyses the use of illicit drugs amongst females in South African correctional centres thematically. The paper recommends the initiation of recovery support programmes for offenders with drug use problems; and a continuation of such programmes after incarceration, with specific attention to the histories of sexual, physical, and emotional abuse that is experienced by a majority of women offenders, increased scrutiny of everyone who visits the centres, and a regular sensitization of the prison wardens on the need to be more vigilant and interested in drug use within the centres.

24. Volunteering across contexts: comparing attitudes toward volunteering with prisoners and people with mental illness

IntroductionVolunteering represents an opportunity for social transformation and social cohesion. Portugal is one of the European countries with fewer volunteering initiatives. Generally, society distances itself from individuals with mental illness and prison inmates, therefore, stigma becomes one of the barriers to social reintegration. However, volunteering can be a beneficial intervention helping individuals in their reintegration.ObjectivesThis study aims to compare the differences and similarities in the attitudes of volunteers toward volunteering with people with mental illness and prisoners.MethodsA supplementary qualitative secondary analysis was conducted using transcripts from 39 semi-structured individual interviews with volunteers regarding support of inmates in prison and two focus groups with volunteers regarding support of people with mental illness. Data analysis was conducted through an inductive thematic analysis.ResultsFour themes emerged from the analysis: ‘Volunteer motivation and characteristics’, ‘Volunteer’s role’, ‘Volunteering relationship and its impact’, and ‘Challenges faced by volunteers’. There were several similarities between the perspectives toward volunteering in prisons and in mental health care, including the need for specific training in the area and the positive attitudes and behaviors of both groups of volunteers toward volunteering with the individuals supported. The differences were related to the characteristics necessary to be a volunteer, the activities carried out with the individuals supported and the difficulties faced by volunteers.ConclusionThese findings show overall positive attitudes toward volunteering in mental health and in prisons.

25. Interoceptive sensibility and alcohol craving in Polish prisoners: the role of alexithymia and emotional dysregulation

IntroductionAlcohol craving, characterized by a strong desire or compulsion to consume alcohol, is a prominent symptom of substance dependence syndrome. Research indicates that alcohol craving is a significant factor leading to the termination of abstinence. The mechanisms underlying the activation of alcohol craving remain not fully understood. The urge to reach for alcohol may be stimulated by emotions, memories, thoughts, or bodily sensations, as well as external factors. It has been postulated that individuals with high levels of interoceptive sensibility tend to exhibit a high degree of alexithymia and emotion dysregulation in the context of alcohol craving. Deficits in identifying and verbalizing emotions, along with an operational thinking style, facilitate alcohol consumption by impeding accurate insight into one’s mental state, thereby hindering the comprehension of bodily states, emotions, and the regulation of self.MethodThis study involved 160 inmates incarcerated in a prison in Poland, awaiting participation in therapy for individuals with substance dependence following psychiatric diagnosis. Four questionnaires were used in the study: multidimensional Assessment of Interoceptive Sensibility (MAIA) for interoceptive sensibility, Toronto Alexithymia Scale (TAS-20) for alexithymia, Difficulties in Emotion Regulation Scale (DERS) for emotional dysregulation, and the Penn Alcohol Craving Scale (PACS) for alcohol craving assessment.ResultsThe results of the study are as follows: the study findings indicated that alexithymia and emotional dysregulation significantly mediates the relationship between interoceptive sensibility and alcohol craving. The indirect effect for both factors was found to be significant, similar to the indirect effect observed for alexithymia as an mediator. However, in the case of emotional dysregulation, no significant indirect effect was observed.ConclusionOur study provides insights into the potential contribution of interoceptive sensibility to the heightened risk of alcohol dependence. Specifically, impaired interoceptive sensibility may be associated with the development of alexithymia and emotional dysregulation, potentially rendering individuals more susceptible to alcohol craving. Interoceptive sensibility could serve as a prerequisite for the cultivation of positive emotional processing skills.

The study by Mwatenga et al. found a tuberculosis (TB) prevalence of 10.2% and an HIV prevalence of 19.1% among inmates, with all TB cases co-infected with HIV. Education level, smoking history, and substance use are key predictors of coinfections. Although informative, its single-centred design and reliance on sputum samples may be insufficient for some patient groups, potentially compromising diagnostic accuracy. Expanding the study to include several jails and employing more diagnostic procedures may increase reliability and the ability to generalize. The report also notes the lack of debate on broader socioeconomic variables and structural barriers to healthcare in jails, which are crucial to understanding inmates' health challenges. Overcrowding, malnutrition, and a poor healthcare system are examples of environmental factors that probably contribute to the spread of tuberculosis and make managing HIV more difficult. Additionally, not enough is known about the dietary habits of prisoners and other health conditions like diabetes or mental health, which may have an impact on the course of HIV and TB. Future studies should take these extra characteristics into account to create more comprehensive approaches to controlling HIV coinfections and TB in prison populations. 29. Coping Strategies and Depression among Male Inmates of a Nigerian Correctional Facility: A Cross-sectional Study

Background: Researchers have consistently found higher rates of mental illnesses, especially depression, in prison populations compared to the general population. Even though specific coping strategies have been implicated as a factor influencing the mental well-being of inmates, little is known about this interplay in resource-poor settings. This study aimed to investigate the relationship between coping strategies and depression among inmates of a prison in Nigeria. Subjects and Methods: This was a two-stage cross-sectional study involving 269 inmates of Calabar Prison, Nigeria. Participants were administered a sociodemographic questionnaire, the Coping Strategies Inventory-Short Form, and the General Health Questionnaire-12 (GHQ-12). Respondents who scored ≥3 on the GHQ-12 together with 10% of low scorers proceeded to the second stage for an interview using the Mini-International Neuropsychiatric Interview. Results: The prevalence of depression among study participants was 30.5%. Emotion-focused disengagement coping strategies (odds ratio [OR]: 1.12, 95% confidence interval [CI]: 1.05–1.20) and a history of mental illness before imprisonment (OR: 3.4, 95% CI: 1.34–8.57) significantly predicted depression, whereas problem-focused engagement was protective (OR: 1.92, 95% CI: 0.86–0.98). Conclusion: Compared to the general population, depression is more common among prison inmates and is associated with using specific coping strategies. Coping strategies deserve increasing consideration when planning mental health intervention programs for this population.

30. Emotionality and psychosexual maturity of male prison inmates

Introduction. The aim of the presented analyzes is to determine the relationship between emotional intelligence and psychosexual maturity of men incarcerated in prisons. Materials and methods. The study involved 70 men incarcerated in prison, aged 21 to 64 (M=35.62), varied in terms of the length of the sentence and the possibility of applying for parole. The following research tools were used in the study: the Emotional Understanding Test (TRE, Matczak, Piekarska, 2011), the original Psychosexual Maturity Questionnaire (KPDP) in the experimental version and the Social Approval Questionnaire (KAS; Drwal, Wilczyńska, 1980) as a control scale. Results and conclusions. The results of the research indicate that the level of emotional intelligence of prisoners is usually average, and with its increase, the level of psychosexual maturity also increases. Statistical analyzes showed that there are no significant differences in emotional intelligence and psychosexual maturity between the group of first-time inmates and repeat offenders. The ability to apply for parole does not differentiate the level of emotional intelligence or psychosexual maturity. The obtained results also suggest a deficit of knowledge and competence in the field of psychosexual maturity from childhood. It is noticeable both in the case of first-time inmates, repeat offenders, men who can apply for parole and those who do not have such a possibility. Implications. Continuing research with prisoners and analyzing their psychosexual maturity, as well as various variables that may explain its level, may have many benefits, such as finding a way in which prisoners could fulfill their sexual needs or supplementing existing competences in the field of particular periods in the case of people who present average or high results.

The condition of the italian prisons is tragic and worrying, despite, during the pandemic period, the situation was little better. Statistic data show which are the worst problems of the italian places of detention: overcrowding, excessive use (rectius abuse) of the pre-trial detention, criminalisation of the weaker people, very high and dramatic number of suicides. The inmates are forced to live in a idleness condition rather than in the prospects of social reintegration. To rethink execution in the sense of humanising and resocialising punishment a strong political will is needed, even if, today, it’s hard to imagine. Maybe, a feeble hope can be seen in the aims pursued by the Cartabia reform. For now, however, the prison continues to be a non-place.

Background: Prisoners are often associated with mental health and substance use disorders. Coercive measures are widely used in prison settings. The objective of this study was to compare inmates' perceptions and satisfaction with telepsychiatry versus face-to-face consultation and the effects of telepsychiatry on the use of coercive measures. The sample consisted of 100 male inmates from various backgrounds who had experienced both approaches of services (face to face and telepsychiatry). Method: The data were obtained through an interview where the individuals completed a Demographic Data Questionnaire, a Participant Satisfaction Questionnaire to assess satisfaction with face-to-face psychiatric services, and a Participant Satisfaction Questionnaire to assess their satisfaction with services offered via telepsychiatry. Additionally, calculations of time spent waiting for a face-to-face psychiatric evaluation and time spent in handcuffs and in confined spaces were made before and after the introduction of telepsychiatry. Results: Statistically significant improvements (all p-values < 0.001) were noted in waiting times, support for relapse prevention, follow up, quality of mental health care, quality of care in the management of psychiatric problems and related medication, behavior of psychiatrists, duration of the assessment, sense of comfort, and confidentiality. Telepsychiatry led to the elimination of time spent in handcuffs and in confined spaces (transport vehicles). Conclusion: According to the results of this study, telepsychiatry is an acceptable method of service delivery in correctional facilities and was associated with a reduction of coercive practices. Mindfulness-based interventions (MBIs) in prison environments have revealed positive benefits for prisoners' physical and psychological health. This study aimed to verify the efficacy of an MBI program in decreasing depressive symptoms, anxiety, stress, negative effects, and increasing positive affects, self-esteem, and mindfulness state and capacity in prisoners. The sample comprised 44 Portuguese male prisoners, who were divided into two groups: the mindfulness training group (n = 22) and the control group (n = 22). The mindfulness training group demonstrated increased self-esteem and mindfulness capacities. Qualitative analysis showed the usefulness of the training for inmates, not only in their daily prison life, but also post-release and the importance of breathing in coping with anxious and stressful situations. These findings suggest the benefits of MBI in prison settings and propose that these interventions may hold the potential to improve prisoners' reintegration into society. [ABSTRACT FROM AUTHOR]

Background and Aims: The environment within prisons, such as overcrowding, lack of access to portable water, poor sanitation, and hygiene predisposes inmates to infections, including intestinal parasitic infections (IPIs). This study therefore determined the prevalence and associated factors of IPIs among prison inmates in Southern Ghana. Methods: A cross‐sectional study using the stratified sampling technique was employed. The study recruited 461 prison inmates across three notable prisons, with 50 inmates from Ho Central, 357 inmates from Nsawam Medium Security, and 54 inmates from Sekondi Central Prisons all in Southern Ghana. A structured closed‐ended questionnaire was administered to collect data on sociodemographics, lifestyle/behavioral characteristics, and signs and symptoms of IPIs. In addition, stool samples were collected and analyzed for the presence of various stages of intestinal parasites (trophozoites, cysts, ova, and larva) using formol‐ether concentration, and the modified Ziehl–Neelsen techniques. Results: The prevalence of IPIs among inmates of the three selected prisons was 38.2% (95% confidence interval [CI]: 33.72%–42.79%). The prevalence of IPIs among inmates of the three prisons, the Ho Central, Nsawam Medium Security, and Sekondi Central were 46% (95% CI: 31.82%–60.68%), 37.5% (95% CI: 32.49%–42.79%), and 35.2% (95% CI: 22.68%–49.38%), respectively. After adjusting for confounders, the following factors; handwashing after defecation (adjusted odds ratio [AOR]: 0.05 [0.00–0.67]; p = 0.024), trimmed fingernails (AOR: 0.32 [0.13–0.76]; p = 0.011), itchy skin (AOR [95% CI]: 5.99 [3.43–10.43]; p < 0.001), anal itching (AOR [95% CI]: 0.35 [0.19–0.62]; p < 0.001), nausea (AOR [95% CI]: 5.57 [3.22–9.65]; p < 0.001), and worm expulsion (AOR [95% CI]: 3.80 [1.42–10.18]; p = 0.008) were found to be associated with intestinal parasitic infections. Conclusion: The study revealed that the prevalence of intestinal parasitic infections among prisons in Southern Ghana is high and is therefore an important public health concern. The Public Health Department of Ghana Health Service should therefore embark on health promotion and deworming exercise in all prisons in Ghana. [ABSTRACT FROM AUTHOR]

Background: Researchers have consistently found higher rates of mental illnesses, especially depression, in prison populations compared to the general population. Even though specific coping strategies have been implicated as a factor influencing the mental well-being of inmates, little is known about this interplay in resource-poor settings. This study aimed to investigate the relationship between coping strategies and depression among inmates of a prison in Nigeria. Subjects and Methods: This was a two-stage cross-sectional study involving 269 inmates of Calabar Prison, Nigeria. Participants were administered a sociodemographic questionnaire, the Coping Strategies Inventory-Short Form, and the General Health Questionnaire-12 (GHQ-12). Respondents who scored =3 on the GHQ-12 together with 10% of low scorers proceeded to the second stage for an interview using the Mini-International Neuropsychiatric Interview. Results: The prevalence of depression among study participants was 30.5%. Emotion-focused disengagement coping strategies (odds ratio [OR]: 1.12, 95% confidence interval [CI]: 1.05-1.20) and a history of mental illness before imprisonment (OR: 3.4, 95% CI: 1.34-8.57) significantly predicted depression, whereas problem-focused engagement was protective (OR: 1.92, 95% CI: 0.86-0.98). Conclusion: Compared to the general population, depression is more common among prison inmates and is associated with using specific coping strategies. Only about 21% of adult inmates in Jamaica participate in formal rehabilitation programmes.1 As such, the government has taken steps to make in-prison rehabilitation mandatory. By drawing on interviews with 73 formerly incarcerated persons, this article explores the viability and practicability of mandatory in-prison rehabilitation. The evidence suggests that if made mandatory, in-prison rehabilitation may not support the effective reintegration of inmates who value agency. Coercion could also amplify change-resistant behaviours, and limited administrative capacity might also challenge the effective enforcement of the mandate. Towards achieving the effective reintegration of newly released persons and lowering the recidivism rate, the article proposes some correctional reforms which the Jamaican state can pursue in the absence of adequate capacity to implement mandatory in-prison rehabilitation. IntroductionThe high rate of incarceration, now exceeding 11.5 million people worldwide, has raised concerns about the conditions within penal institutions, such as the consequences of incarceration on the person. This retrospective study aims to investigate the issue of death in custody, exploring the relationship between incarceration, health vulnerabilities, and death from the forensic pathologist’s point of view.MethodsWe analyzed, from a forensic, clinical and toxicological perspective, 86 cases of deaths in detention facilities in North-Western Italy from 1999 to 2022.ResultsThe analysis has shown that suicide, mainly committed by hanging, plastic bag suffocation and butane intoxication, represents the leading type of violent death (52%), followed by accidental deaths (16%). On the other hand, cardiovascular diseases are the leading cause of natural deaths (42%), followed by infectious diseases (especially HIV-related).DiscussionThe present study identifies the most frequent and critical situations and risk factors related to death in custody and the profile of the inmate who is at a higher risk of death, allowing to highlight the issues to be addressed from a public health point of view. On the whole, it calls for comprehensive reforms, aligned with international human rights standards, addressing mental and physical care gaps, improving correctional officers' education, and training, and focusing on rehabilitation with well-being and dignity.

IntroductionStudies on inmates’ Health Care Service (HCS) utilization are scarce globally, infrequent in Ethiopia while findings about the factors associated with HCS utilization are inconsistent. The present study, therefore, examined inmates’ HCS utilization and associated socio-demographic and imprisonment related factors in Northwestern Ethiopia.MethodsThe study employed institution-based cross sectional research design and data was collected using questionnaire from 422 inmates in three prisons. The questionnaire collected data about prisoners’ demographic characters, imprisonment related information and HCS utilization. Descriptive statistical techniques as well as bi-variate and multiple logistic regressions were used to analyse the data.Results and discussionsThe study found that 72.5%, 66.1% and 13.3% of the inmates, respectively, used medical services, guidance and counseling services, and psychiatric services. Inmates with primary education, with secondary education, and who know the availability of the services were more likely to use medical services. Inmates with accused status were less likely to use medical services than inmates with convict status. Divorced marital status and knowledge of the service availability were associated with high guidance and counseling service use. An increase in the length of stay in the prison was associated with a decrease in psychiatry service use while knowledge of service availability was associated with higher odds of psychiatry service use. There are high medical care service utilization while low mental health care service utilization among inmates in Northwestern Ethiopia. Results of the study implied that there is a critical need for immediate health care service promotion and education measures. Besides, there are also needs for large scale, longitudinal and potentially cross-cultural studies to better understand additional factors that influence inmates’ HCS utilization.

IntroductionThe behavioral addiction model posits that repetitive suicidal behaviors can serve as maladaptive strategies for managing stress and negative emotional states, akin to substance addiction. Both behaviors involve negative emotions, offer temporary psychological relief, and persist, indicating shared neurobiological mechanisms. This study explored psychometric differences among major repeaters, occasional attempters, and non-suicidal prisoners.MethodsA multi-centre cross-sectional survey of 363 inmates across four prisons assessed depression, cognitive-emotional regulation, impulsivity, perceived stress, lifetime non-suicidal self-injury and suicide attempts.ResultsMild depression, moderate suicidal ideation, and moderate impulsivity were common, with nearly half of the participants having attempted suicide at least once. Hierarchical multiple regression analyses revealed that repeated suicidal behavior in the past increases susceptibility to future suicidal thoughts, with suicide attempts serving as a maladaptive emotion regulation strategy among repeated attempters.DiscussionThe results reveal differences in emotional dysregulation, impulsivity, and stress coping strategies among the studied groups, reinforcing the idea of suicidality as a form of behavioral addiction. The addiction approach helps explain the sensitivity to later suicidal thoughts in former attempters and self-harmers, offering valuable insights for tailored interventions within correctional settings.

Introduction: The aim of the research was to determine the relationship between the fear of COVID-19 and inmates’ the sense of loneliness.Method: The research was carried out using the diagnostic survey method. Two standardized research tools, adapted to Polish conditions, were used: the COVID-19 fear scale of D.K. Ahorsu et al. (Adaptation - A. Lewicka-Zelent, W. Czerski) and the Sense of Loneliness Scale de Jong Gierveld. (adaptation - P. Grygiel, G. Humenny, S. Rębisz, P. Świtaj, J. Sikorska). The group of respondents consisted of 429 people staying in six penitentiary units - 6 prisons and 2 pre-trial detention centers, randomly selected by the Central Board of the Prison Service.Results: The results of own research show that inmates do not feel a strong fear of Covid-19, and their sense of loneliness is of moderate intensity. The correlation between them is weak and positive. This means that as the fear of the coronavirus intensifies, the sense of loneliness increases in people imprisoned in solitary confinement.Implications: The obtained research results indicate the need to continue the research undertaken in a situation where sanitary restrictions related to minimizing the risk of falling ill with COVID-19 are reintroduced. Inmates should take additional measures to prevent negative consequences resulting from the high level of fear of COVID-19 and inmates’ the sense of loneliness. It is not even about wide-ranging impacts, but rather targeted at individual people who experience increased fear of the coronavirus and loneliness, despite being with other inmates in prison. It means the necessity to make a detailed diagnosis in this regard among persons deprived of liberty.

Background: Modern penitentiary systems attach great importance, at least in the area of formal and codified arrangements, to providing inmates with access to health care and rehabilitation. The aim of our study was to analyze the Web of Science (WoS) and SCOPUS medical databases in order to search for and evaluate the available literature discussing the oral status and dental treatment needs of adult male prisoners. Methods: The following terms were used: prisoners or inmates; oral health, oral status; periodontal status, periodontal disease; oral hygiene; caries; mucosa; and saliva. The studies were screened based on their title and abstract according to the PICO (population, intervention, control, and outcome) criteria. The research protocol was prepared on the basis of the 2020 PRISMA guidelines and was not registered. The available literature discussing the oral status and dental treatment needs of adult imprisoned patients was analyzed. The inclusion criteria were as follows: articles published in English between 1 January 2012 and 2022; articles discussing the oral cavity status of adult inmates over 18 years old (hard tissues, periodontal status, saliva, mucosa condition, or oral hygiene); articles with a full text available; and articles that were assessed as satisfactory according to the Newcastle–Ottawa Scale. Results: A total of 934 articles were identified, out of which 9 were included in the systematic review. Two articles discussed the oral condition of prisoners in Europe (Russia and Finland), four examined prisoners in Asia (three in India and one in Saudi Arabia), two examined prisoners in Africa (Nigeria), and one examined prisoners in the Americas (Brazil). Conclusions: The oral status of prisoners has been widely discussed in the available literature. Among inmates, a higher frequency of both caries and periodontal disease along with poorer oral hygiene were observed. This study evaluates the transition from an older to a new prison facility in Italy to help researchers understand the health-enabling features within prisons from incarcerated persons' point of view. A total of 216 inmates completed a questionnaire that measured the prison's environmental quality and quality-of-life related constructs. Bivariate correlations show that as inmates' environmental perceptions improve, so does their place evaluation. When the older and newer prisons were compared, the results revealed the newer prison was more positively evaluated in terms of care of significant spaces, presence of light, beauty, safety, sociability, concentration, and satisfaction. 47. Nonconsensual Sexual Experience Histories of Incarcerated Men: A Mixed Methods Approach.

Nonconsensual sexual experiences (NSEs) may contribute to mental health concerns among incarcerated individuals, yet NSEs are understudied in this population. This study takes a novel approach in examining the prevalence of NSEs among incarcerated males by utilizing both quantitative and qualitative measures. The sample consisted of 189 men from three provincial maximum-security prisons in Ontario, Canada. Based on quantitative findings, 44.2% of the sample experienced NSEs before the age of 18, and 41.7% of the sample endorsed an experience that fit the legal definition of a NSEs as adults. Participants also responded to a qualitative open-ended question about their history of NSEs. Based on qualitative findings, a total of 23% of the men reported at least one incident of a NSE (e.g., child and adult). Based Findings highlight the high prevalence of NSEs among incarcerated men with quantitative responses demonstrating how the use of a behavioral questionnaire may, to some extent, correct for underreporting of NSEs. Qualitative responses illustrate the lived experience of incarcerated men and provide a deeper understanding of their NSEs. Responses also speak to the lack of resources and support available to these men. Findings underscore the need for proactive approaches in meeting mental health needs of incarcerated men in general and with regard to NSEs in particular. Results may inform the development of future correctional procedures (i.e., intake protocols that account for men with NSEs) and resources to support incarcerated men in navigating the psychological impact of non-consensual sexual experiences. The worldwide incarceration rate per 100,000 people varies from 30 in India, 580 in Europe, to 750 in United States. The health of prisoners is of great concern. Research in many countries has shown poor oral health conditions among prisoners, particularly reflected in the high number of lost and untreated decayed teeth. The aim of our study was to evaluate the quality and range in dental procedures conducted on male prisoners, based on a retrospective analysis of medical history gathered at the Academic Center of Dentistry and Specialistic Medicine of Medical University of Silesia in Bytom for the period 2018–2021, and its correlation with the results obtained from the analysis of the Web of Science (WoS) and SCOPUS medical databases. Our research was carried out on the examined group, 86 men (mean age 31 years old), and a control group, 106 men (mean age 32 years old). The retrospective analysis of the medical history was performed. Results of our own research showed the values for decayed and missing teeth were significantly higher in the examined group while the values for the filled teeth component were significantly higher in the control group. The chance for the occurrence of the radices was 2.5 times higher in the examined group than in the control. The examined group was characterized by 3.6 times higher chance of no root canal treatment than the control group. The number of the endodontically treated teeth was significantly higher in the control group. The examined group was characterized by 4.2 higher probability for periapical lesion occurrence in teeth that were not endodontically treated. The number of teeth qualified for the endodontic treatment was significantly higher in the examined group, while the number of teeth qualified for the reendow treatment was significantly higher in the control group. The probability for the occurrence of both vertical and horizontal atrophy in the alveolar process was twice and three times higher in the examined than in the control group. In conclusion, the oral status of inmates is worse when compared to those who live in freedom, which is why there is a need to prepare a scheme to improve the condition of the stomatognathic system in prisoners.
