The North Atlantic Oscillation (NAO) is a leading mode of atmospheric variability, affecting the North Atlantic Ocean on sub-seasonal to multi-decadal timescales. The NAO changes the atmospheric forcing at the ocean's surface, including winds and surface buoyancy fluxes, both of which are known to impact large-scale gyre circulation. However, the relative role of other physical processes (such as mesoscale eddies and topography) in influencing gyre circulation under NAO variability is not fully understood. Here, we analyze a series of ocean--sea ice simulations using a barotropic vorticity budget to understand long-term response of the North Atlantic gyre circulation to NAO forcing. We find that for each standard deviation increase in the NAO index, the subtropical and subpolar gyres intensify by 0.90 Sv and 3.41 Sv (1 Sv = 10$^6$ m$^3$ s$^{-1}$) respectively. The NAO-induced wind stress anomalies drive approximately 90% of the change in the subtropical gyre's interior flow. However, in the subpolar gyre's interior, in addition to wind stress, flow-topography interactions, stratification (influenced by surface heat fluxes), and non-linear advection significantly influence the circulation. Along the western boundary the bottom pressure torque plays a key role in steering the flow, and the vorticity input by the bottom pressure torque is partly redistributed by non-linear advection. Our study highlights the importance of both atmospheric forcing and oceanic dynamical processes in driving long-term gyre circulation responses to the NAO.

Proton therapy offers significant advantages due to its unique physical and biological properties, particularly the Bragg peak, enabling precise dose delivery to tumors while sparing healthy tissues. However, the clinical implementation is challenged by the oversimplification of the relative biological effectiveness (RBE) as a fixed value of 1.1, which does not account for the complex interplay between dose, linear energy transfer (LET), and biological endpoints. Lack of heterogeneity control or the understanding of the complex interplay may result in unexpected adverse events and suboptimal patient outcomes. On the other hand, expanding our knowledge of variable tumor RBE and LET optimization may provide a better management strategy for radioresistant tumors. This review examines recent advancements in LET calculation methods, including analytical models and Monte Carlo simulations. The integration of LET into plan evaluation is assessed to enhance plan quality control. LET-guided robust optimization demonstrates promise in minimizing high-LET exposure to organs at risk, thereby reducing the risk of adverse events. Dosimetric seed spot analysis is discussed to show its importance in revealing the true LET-related effect upon the adverse event initialization by finding the lesion origins and eliminating the confounding factors from the biological processes. Dose-LET volume histograms (DLVH) are discussed as effective tools for correlating physical dose and LET with clinical outcomes, enabling the derivation of clinically relevant dose-LET volume constraints without reliance on uncertain RBE models. Based on DLVH, the dose-LET volume constraints (DLVC)-guided robust optimization is introduced to upgrade conventional dose-volume constraints-based robust optimization, which optimizes the joint distribution of dose and LET simultaneously.

In-basket message interactions play a crucial role in physician-patient communication, occurring during all phases (pre-, during, and post) of a patient's care journey. However, responding to these patients' inquiries has become a significant burden on healthcare workflows, consuming considerable time for clinical care teams. To address this, we introduce RadOnc-GPT, a specialized Large Language Model (LLM) powered by GPT-4 that has been designed with a focus on radiotherapeutic treatment of prostate cancer with advanced prompt engineering, and specifically designed to assist in generating responses. We integrated RadOnc-GPT with patient electronic health records (EHR) from both the hospital-wide EHR database and an internal, radiation-oncology-specific database. RadOnc-GPT was evaluated on 158 previously recorded in-basket message interactions. Quantitative natural language processing (NLP) analysis and two grading studies with clinicians and nurses were used to assess RadOnc-GPT's responses. Our findings indicate that RadOnc-GPT slightly outperformed the clinical care team in "Clarity" and "Empathy," while achieving comparable scores in "Completeness" and "Correctness." RadOnc-GPT is estimated to save 5.2 minutes per message for nurses and 2.4 minutes for clinicians, from reading the inquiry to sending the response. Employing RadOnc-GPT for in-basket message draft generation has the potential to alleviate the workload of clinical care teams and reduce healthcare costs by producing high-quality, timely responses.

This paper presents the results of a nearly two year long campaign to detect and analyze meteor persistent trains (PTs) - self-emitting phenomena which can linger up to an hour after their parent meteor. The modern understanding of PTs has been primarily developed from the Leonid storms at the turn of the century; our goal was to assess the validity of these conclusions using a diverse sample of meteors with a wide range of velocities and magnitudes. To this end, year-round observations were recorded by the Widefield Persistent Train camera, 2nd edition (WiPT2) and were passed through a pipeline to filter out airplanes and flag potential meteors. These were classified by visual inspection based on the presence and duration of trains. Observed meteors were cross-referenced with the Global Meteor Network (GMN) database, which independently detects and calculates meteor parameters, enabling statistical analysis of PT-leaving meteors. There were 4726 meteors codetected by the GMN, with 636 of these leaving trains. Among these were a large population of slow, dim meteors that left PTs; these slower meteors had a greater train production rate relative to their faster counterparts. Unlike prior research, we did not find a clear magnitude cutoff or a strong association with fast meteor showers. Additionally, we note several interesting trends not previously reported, which include PT eligibility being primarily determined by a meteor's terminal height and an apparent dynamical origin dependence that likely reflects physical meteoroid properties., Comment: Accepted for publication in JGR Space Physics

In radiotherapy, 2D orthogonally projected kV images are used for patient alignment when 3D-on-board imaging(OBI) unavailable. But tumor visibility is constrained due to the projection of patient's anatomy onto a 2D plane, potentially leading to substantial setup errors. In treatment room with 3D-OBI such as cone beam CT(CBCT), the field of view(FOV) of CBCT is limited with unnecessarily high imaging dose, thus unfavorable for pediatric patients. A solution to this dilemma is to reconstruct 3D CT from kV images obtained at the treatment position. Here, we propose a dual-models framework built with hierarchical ViT blocks. Unlike a proof-of-concept approach, our framework considers kV images as the solo input and can synthesize accurate, full-size 3D CT in real time(within milliseconds). We demonstrate the feasibility of the proposed approach on 10 patients with head and neck (H&N) cancer using image quality(MAE: <45HU), dosimetrical accuracy(Gamma passing rate (2%/2mm/10%)>97%) and patient position uncertainty(shift error: <0.4mm). The proposed framework can generate accurate 3D CT faithfully mirroring real-time patient position, thus significantly improving patient setup accuracy, keeping imaging dose minimum, and maintaining treatment veracity., Comment: 17 pages, 8 figures and tables

The North Atlantic Ocean circulation, fueled by winds and surface buoyancy fluxes, carries 1.25 PettaWatts of heat poleward in the subtropics, and helps in regulating global weather and climate patterns. Here, we assess the impacts of changes in winds and surface heat fluxes on the Atlantic Ocean circulation and heat transport using ocean simulations. We decompose the circulation and heat transport into warm and cold cells (resembling a subtropical gyre and the dense overturning circulation respectively), and a mixed cell capturing waters transitioning between warm and cold regions. Warm and mixed cells transport more heat poleward as wind stress increases; however, these anomalies are compensated by reductions in the cold cell's heat transport. Warm and cold cells transport more heat poleward when we increase meridional heat flux gradients. Purpose: Prior AI-based dose prediction studies in photon and proton therapy often neglect underlying physics, limiting their generalizability to handle outlier clinical cases, especially for pencil beam scanning proton therapy (PBSPT). Our aim is to design a physics-aware and generalizable AI-based PBSPT dose prediction method that has the underlying physics considered to achieve high generalizability to properly handle the outlier clinical cases. Methods and Materials: This study analyzed PBSPT plans of 103 prostate and 78 lung cancer patients from our institution,with each case comprising CT images, structure sets, and plan doses from our Monte-Carlo dose engine (serving as the ground truth). Three methods were evaluated in the ablation study: the ROI-based method, the beam mask and sliding window method, and the noisy probing dose method. Twelve cases with uncommon beam angles or prescription doses tested the methods' generalizability to rare treatment planning scenarios. Performance evaluation used DVH indices, 3D Gamma passing rates (3%/2mm/10%), and dice coefficients for dose agreement. Results: The noisy probing dose method showed improved agreement of DVH indices, 3D Gamma passing rates, and dice coefficients compared to the conventional methods for the testing cases. The noisy probing dose method showed better generalizability in the 6 outlier cases than the ROI-based and beam mask-based methods with 3D Gamma passing rates (for prostate cancer, targets: 89.32%$\pm$1.45% vs. 93.48%$\pm$1.51% vs. 96.79%$\pm$0.83%, OARs: 85.87%$\pm$1.73% vs. 91.15%$\pm$1.13% vs. 94.29%$\pm$1.01%). The dose predictions were completed within 0.3 seconds. Conclusions: We've devised a novel noisy probing dose method for PBSPT dose prediction in prostate and lung cancer patients. With more physics included, it enhances the generalizability of dose prediction in handling outlier clinical cases.

Purpose: To enhance an in-house graphic-processing-unit (GPU) accelerated virtual particle (VP)-based Monte Carlo (MC) proton dose engine (VPMC) to model aperture blocks in both dose calculation and optimization for pencil beam scanning proton therapy (PBSPT)-based stereotactic radiosurgery (SRS). Methods and Materials: A block aperture module was integrated into VPMC. VPMC was validated by an opensource code, MCsquare, in eight water phantom simulations with 3cm thick brass apertures: four were with aperture openings of 1, 2, 3, and 4cm without a range shifter, while the other four were with same aperture opening configurations with a range shifter of 45mm water equivalent thickness. VPMC was benchmarked with MCsquare and RayStation MC for 10 patients with small targets (average volume 8.4 cc). Finally, 3 patients were selected for robust optimization with aperture blocks using VPMC. Results: In the water phantoms, 3D gamma passing rate (2%/2mm/10%) between VPMC and MCsquare were 99.71$\pm$0.23%. In the patient geometries, 3D gamma passing rates (3%/2mm/10%) between VPMC/MCsquare and RayStation MC were 97.79$\pm$2.21%/97.78$\pm$1.97%, respectively. The calculation time was greatly decreased from 112.45$\pm$114.08 seconds (MCsquare) to 8.20$\pm$6.42 seconds (VPMC), both having statistical uncertainties of about 0.5%. The robustly optimized plans met all the dose-volume-constraints (DVCs) for the targets and OARs per our institutional protocols. The mean calculation time for 13 influence matrices in robust optimization by VPMC was 41.6 seconds. Purpose: To develop a DL-based PBSPT dose prediction workflow with high accuracy and balanced complexity to support on-line adaptive proton therapy clinical decision and subsequent replanning. Methods: PBSPT plans of 103 prostate cancer patients and 83 lung cancer patients previously treated at our institution were included in the study, each with CTs, structure sets, and plan doses calculated by the in-house developed Monte-Carlo dose engine. For the ablation study, we designed three experiments corresponding to the following three methods: 1) Experiment 1, the conventional region of interest (ROI) method. 2) Experiment 2, the beam mask (generated by raytracing of proton beams) method to improve proton dose prediction. 3) Experiment 3, the sliding window method for the model to focus on local details to further improve proton dose prediction. A fully connected 3D-Unet was adopted as the backbone. Dose volume histogram (DVH) indices, 3D Gamma passing rates, and dice coefficients for the structures enclosed by the iso-dose lines between the predicted and the ground truth doses were used as the evaluation metrics. The calculation time for each proton dose prediction was recorded to evaluate the method's efficiency. Results: Compared to the conventional ROI method, the beam mask method improved the agreement of DVH indices for both targets and OARs and the sliding window method further improved the agreement of the DVH indices. For the 3D Gamma passing rates in the target, OARs, and BODY (outside target and OARs), the beam mask method can improve the passing rates in these regions and the sliding window method further improved them. A similar trend was also observed for the dice coefficients. In fact, this trend was especially remarkable for relatively low prescription isodose lines. The dose predictions for all the testing cases were completed within 0.25s.

Many complex systems--from social and communication networks to biological networks and the Internet--are thought to exhibit scale-free structure. However, prevailing explanations rely on the constant addition of new nodes, an assumption that fails dramatically in some real-world settings. Here, we propose a model in which nodes are allowed to die, and their connections rearrange under a mixture of preferential and random attachment. With these simple dynamics, we show that networks self-organize towards scale-free structure, with a power-law exponent $\gamma = 1 + \frac{1}{p}$ that depends only on the proportion $p$ of preferential (rather than random) attachment. Applying our model to several real networks, we infer $p$ directly from data, and predict the relationship between network size and degree heterogeneity. Together, these results establish that realistic scale-free structure can emerge naturally in networks of constant size and density, with broad implications for the structure and function of complex systems., Comment: 24 pages, 5 figures

This article aims at developing a simple, automated test bench procedure to measure polarization rotation of an electromagnetic wave upon propagation through a nonreciprocal medium. A comprehensive measurement approach is developed from the scattering matrix. Living systems are fundamentally irreversible, breaking detailed balance and establishing an arrow of time. But how does the evident arrow of time for a whole system arise from the interactions among its multiple elements? We show that the local evidence for the arrow of time, which is the entropy production for thermodynamic systems, can be decomposed. First, it can be split into two components: an independent term reflecting the dynamics of individual elements and an interaction term driven by the dependencies among elements. Adapting tools from non--equilibrium physics, we further decompose the interaction term into contributions from pairs of elements, triplets, and higher--order terms. We illustrate our methods on models of cellular sensing and logical computations, as well as on patterns of neural activity in the retina as it responds to visual inputs. We find that neural activity can define the arrow of time even when the visual inputs do not, and that the dominant contribution to this breaking of detailed balance comes from interactions among pairs of neurons., Comment: 16 pages, 10 figures
