- Abstract
The present study aimed to examine the relationship between coach dismissal and team performance, and match physical demands in the top two professional Spanish soccer leagues across both short and long term within a season. Data were collected from all matches played in the First (Liga Santander; n = 2,950) and Second Spanish Division (Liga Smartbank; n = 2,966) over four consecutive seasons (from 2015/2016 to 2018/2019). Team performance (points won), total distance (TD), high-intensity running distance (HIRD, distance 21–24 km⋅h
−1 ), very-high-intensity running distance (VHIRD, >24 km⋅h−1 ) and the number of efforts between 21–24 km⋅h−1 (Sp21) and >24 km⋅h−1 (Sp24) were analysed. All within-season coaching dismissals were examined. Results showed that team performance was significantly higher after coach dismissal across short term (p <.001). Concerning match physical demands, across the short term, only TD was significantly greater after coach dismissal (p <.01); while across long terms, TD, HIRD, VHIRD, Sp21 and Sp24 were significantly greater during post coach dismissal stage (p <.001) than the prior coach dismissal stage. Such findings demonstrate that coach replacement could positively influence on team performance and match physical demands, not only across the short term but also over the season. 6. The effect of COVID-19 lockdown on ball possession performance in Spanish professional soccer leagues.

- Abstract
This research analysed the influence of lockdown and interruption of competitions due to COVID-19 on different ball possession-related variables. Also, match location and the differences between divisions were considering between the pre- and post-lockdown. A total of 144,689 records from 842 soccer matches during the 2019/20 season of the First (n = 380) and Second Spanish Division (n = 462) were collected. Match data were collected using the ChryonHego video-tracking system. Pre-lockdown (1
st –27th matchday in First Division and 1st –31st matchday in Second Spanish Division) and post-lockdown periods (28th –38th matchday in First Division and 32nd –42nd matchday in Second Spanish Division) were compared. The match location contextual-related variable was also considered for the analysis. Linear mixed models showed that the number of outplayed players by ball possession (3.1%), successful passes (17.5%) and total passes (18.9%) increased during the post-lockdown period (p <.001). In addition, match location and league significantly influenced the successful and total passes in all situations (p <.001), increasing in the post-lockdown period. In conclusion, the lockdown period has led changes in the ball possession of the soccer teams. These results help to understand the impact that a particular and unusual situation as the interruption of the league due to COVID-19 lockdown had on the performance of professional soccer teams. International guidelines have been proposed to evaluate them in the clinical setting as a continuous variable, without a clear defined cut-off. However, there are scenarios where the immune infiltration is heterogeneous that some areas of the patient’s tumour have high numbers of TILs while other areas completely lack them. This spontaneous presentation of a heterogeneous immune infiltration could be a great opportunity to study why some tumours present TILs at diagnosis but others do not, while eliminating inter patient’s differences. Methods: In this study, we have identified five TNBC patients that showed great TIL heterogeneity, with areas of low (≤5%) and high (≥50%) numbers of TILs in their surgical specimens. To evaluate immune infiltration heterogeneity, we performed and analyzed bulk RNA-sequencing in three independent triplicates from the high and low TIL areas of each patient. Results: Gene expression was homogeneous within the triplicates in each area but was remarkable different between TILs regions. These differences were not only due to the presence of TILs as there were other non-inflammatory genes and pathways differentially expressed between the two areas. Discussion: This highlights the importance of intratumour heterogeneity driving the immune infiltration, and not patient’s characteristics like the HLA phenotype, germline DNA or immune repertoire. 8. Three-Month Mortality in Nonhaematological Patients with Chronic Pulmonary Aspergillosis: Differences between Subtypes.

- Abstract
Objectives : Chronic pulmonary aspergillosis (CPA) is a fungal lung infection characterised by the slowly progressing destruction of the lung parenchyma and has four main subtypes. The objective of this work was to evaluate the epidemiology of CPA in our area and evaluate the involvement of the different subtypes in mortality. Methods : This was a descriptive longitudinal retrospective study developed in three tertiary hospitals in Spain. Among all patients admitted with a pulmonary aspergillosis diagnosis, we selected those who fulfilled the criteria for chronic aspergillosis according to the criteria of Denning, excluding those with a haematological disorder. Results : Among 409 inpatients recorded as having a pulmonary aspergillosis infection, only 76 (18.5%) fulfilled the criteria for CPA, with an estimated incidence of 0.67 cases/100,000 inhabitants/year. The subtypes detected were subacute invasive aspergillosis (SAIA) in 33 (43.4%) patients, simple aspergilloma (SA) in 25 (32.9%) patients, cavitary chronic aspergillosis (CCPA) in 13 (17.1%) patients, and chronic fibrosis (CFPA) in five (6.5%) patients. The overall three-month mortality rate was 23%, which was higher in SAIA patients. The predictors of early mortality were age > 65 years (OR 3.0 CI 95 1.0-9.5 p = 0.043) and the SAIA subtype vs. other subtypes (OR 3.1 CI 95 1.0-9.5 p = 0.042). Conclusions : The incidence rate estimated was inferior to that previously reported. The three-month mortality in patients with CPA was high, with older age and the SAIA subtype being the variable independent predictors of a worse prognosis.

9. Closing the Sex-Based Differences in Stroke Care: Insights from a Large Telestroke Network on Treatment and Postacute Management.

- Abstract
Background: The evolution of ischemic stroke is different accordin'g to sex and is one of the main causes of death in women. Previous studies have shown that women are less likely to receive acute treatment, and stroke center type is an important predictor of door-to-needle times. We investigated whether women are attended in a similar way to men in the telestroke network with specialized stroke physicians., Methods: A prospective registry of ischemic strokes recorded in the centralized Andalusian telestroke network was analyzed, focusing on sex differences. Demographic data, clinical characteristics, neuroimaging data, treatment intervals, follow-up visits, and clinical outcomes were collected., Results: A total of 3009 suspected stroke patients were attended to in the telestroke network from 2019 to 2023, of which 42.74% were women. Women were older ( p < 0.001) and less independent upon arrival ( p = 0.006) than men. There was no difference in the treatment received or in the treatment time intervals between the groups. Importantly, there was no difference in modified Rankin scale scores at 3 months between sexes. At 3 months post-stroke follow-up, women had fewer imaging tests ( p = 0.018) and fewer outpatient visits ( p < 0.001) than men., Conclusions: No significant difference between men and women has been found in the acute treatment of stroke in a large telestroke network. However, the same is not true for the follow-up and management of patients after the acute phase. 10. Ultrasonographic study of the orbit as an early diagnostic tool in Vogt Koyanagi Harada disease (VKH): A case report.

- Abstract
In recent years, ultrasound has demonstrated its usefulness in the approach to vascular structures and other tissues such as the orbit, facilitating the early diagnosis of various diseases without having to rely on other more invasive or less available tests. In Vogt Koyanagi Harada syndrome, characterised by bilateral acute uveitis, ocular ultrasound is a clear example of the usefulness of ultrasonography in early diagnosis, facilitating the initiation of specific treatment to change the ominous natural history of this disease. - Abstract
Skin and soft tissue infections are a common reason for patients seeking inpatient and outpatient medical care. Surgery is an essential part of managing in many episodes. Careful evaluation of antibiotic therapy could help clinicians in early identification to patients with treatment failure and to consider an alternative approach or a new surgical revision in "focus control". With the arrival of new drugs, there is a need to refine the appropriate drug's decision-making. Drugs with a long half-life (long-acting lipoglycopeptides such as dalbavancin or oritavancin), which allows weekly administration (or even greater), can reduce hospital admission and length of stay with fewer healthcare resources through outpatient management (home hospitalization or day hospitals). New anionic fluoroquinolones (e.g. delafloxacin), highly active in an acidic medium and with the possibility of switch from the intravenous to the oral route, will also make it possible to achieve these new healthcare goals and promote continuity of care.
