Abstract

BackgroundThe objective of this 3-arm randomized controlled trial was to evaluate the efficacy of computerized cognitive training (CCT) in improving primary outcomes of delayed-recall memory and serum brain-derived neurotrophic factor (BDNF) levels; and the secondary outcomes were working memory, instrumental activities of daily living (IADLs) and health-related quality of life (HRQL) in patients with heart failure (HF).Methods and resultsPatients (n = 256) were randomly assigned to 8 weeks of CCT using BrainHQ, computerized crossword puzzles active control intervention, and usual care. All patients received weekly nurse-enhancement interventions. Data were collected at enrollment and baseline visits and at 10 weeks and 4 and 8 months. In mixed effects models, there were no statistically significant group or group-by-time differences in outcomes. There were statistically significant differences over time in all outcomes in all groups. Patients improved over time on measures of delayed-recall memory, working memory, IADLs, and HRQL and had decreased serum BDNF.ConclusionsCCT did not improve outcomes compared with the active control intervention and usual care. Nurse-enhancement interventions may have led to improved outcomes over time. Future studies are needed to test nurse-enhancement interventions in combination with other cognitive interventions to improve memory in persons with HF.

Abstract

Strain differences have been reported for motor behaviors, and only a subset of spinal cord injury (SCI) patients develop neuropathic pain, implicating genetic or genomic contribution to this condition. Here, we evaluated neuropsychiatric behaviors in A/J, BALB/c, and C57BL/6 male mice and tested genetic or genomic alterations following SCI. A/J and BALB/c naive mice showed significantly less locomotor activity and greater anxiety-like behavior than C57BL/6 mice. Although SCI elicited locomotor dysfunction, C57BL/6 and A/J mice showed the best and the worst post-traumatic recovery, respectively. Mild (m)-SCI mice showed deficits in gait dynamics. All moderate/severe SCI mice exhibited similar degrees of anxiety/depression. mSCI in BALB/c and A/J mice resulted in depression, whereas C57BL/6 mice did not exhibit depression. mSCI mice had significantly lower mechanical thresholds than their controls, indicating high cutaneous hypersensitivity. C57BL/6, but not A/J and BLAB/c mice, showed significantly lower heat thresholds than their controls. C57BL/6 mice exhibited spontaneous pain. RNAseq showed that genes in immune responses and wound healing were upregulated, although A/J mice showed the largest increase. The cell cycle and the truncated isoform of trkB genes were robustly elevated in SCI mice. Thus, different genomics are associated with post-traumatic recovery, underscoring the likely importance of genetic factors in SCI. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Using software for self‐management interventions can improve health outcomes for individuals with low back pain, but there is a dearth of research to confirm its effectiveness. Additionally, no known research has evaluated the effective elements of software‐based interventions for low back pain self‐management components. This study aimed to synthesize the effectiveness of software‐based interventions to promote self‐management health outcomes among individuals with low back pain. Design: A systematic review and meta‐analysis was conducted. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses Statement, relevant studies up to July 2022 were searched via four electronic databases: PubMed, Embase, Cumulative Index to Nursing and Allied Health Literature, and Web of Science. Results: 4908 adults with low back pain who participated in 23 studies were included. Software‐based interventions were effective in reducing fear avoidance (mean difference [MD] = −0.95, 95% CI: −1.45 to −0.44), pain catastrophizing (MD = −1.31, 95% CI: −1.84 to −0.78), disability (MD = −8.21, 95% CI: −13.02 to −3.39), and pain intensity (MD = −0.86, 95% CI: −1.17 to −0.55). Specifically, interventions that included an exercise component were more effective in reducing pain and disability. Additionally, cognitive behavioral therapy (CBT) intervention significantly reduced fear avoidance and pain catastrophizing but had no noticeable impact on disability and pain compared to standard treatment. The certainty of the evidence in this review varied from very low to high across outcomes. The heterogeneity of the study results was significant, suggesting that future studies in this area could optimize the design, time points, measures, and outcomes to strengthen the evidence. Conclusions: Low back pain self‐management interventions delivered through software‐based programs effectively reduce pain intensity, disability, fear avoidance, and pain catastrophizing. Clinical Relevance: Low back pain is among the most common reasons for seeking healthcare visits. Combining exercise and counseling through soft‐based programs may effectively address this issue and its associated suffering and disability. [ABSTRACT FROM AUTHOR]

Abstract

Cancer-related fatigue is a prevalent, debilitating, and persistent condition. Mitochondrial dysfunction is a putative contributor to cancer-related fatigue, but relationships between mitochondrial function and cancer-related fatigue are not well understood.We investigated the relationships between mitochondrial DNA (mtDNA) gene expression and cancer-related fatigue, as well as the effects of fish and soybean oil supplementation on these relationships.A secondary analysis was performed on data from a randomized controlled trial of breast cancer survivors 4-36 months posttreatment with moderate-severe cancer-related fatigue. Participants were randomized to take 6 g fish oil, 6 g soybean oil, or 3 g each daily for 6 weeks. At pre- and postintervention, participants completed the Functional Assessment of Chronic Illness Therapy-Fatigue questionnaire and provided whole blood for assessment of mtDNA gene expression. The expression of 12 protein-encoding genes was reduced to a single dimension using principal component analysis for use in regression analysis. Relationships between mtDNA expression and cancer-related fatigue were assessed using linear regression.Among 68 participants, cancer-related fatigue improved and expression of all mtDNA genes decreased over 6 weeks with no effect of treatment group on either outcome. Participants with lower baseline mtDNA gene expression had greater improvements in cancer-related fatigue. No significant associations were observed between mtDNA gene expression and cancer-related fatigue at baseline or changes in mtDNA gene expression and changes in cancer-related fatigue.Data from this exploratory study add to the growing literature that mitochondrial dysfunction may contribute to the etiology and pathophysiology of cancer-related fatigue.

43. ¿Qué tal si? Una conversación sobre arte contemporáneo y ciencias, interpelando los espacios museales hoy.

Abstract

What if museums could be community spaces that elevate local stories, histories and futures? What if contemporary art could be used in museums to encourage the co-creation of knowledge through dialogue and experimentation while centering equity? What if there was a recognition that we all have knowledge to contribute, share and shape with while decentering the idea of ‘the expert’? This work will showcase the dialogue between two researchers interested on a new reality for museums and exhibition spaces, from the perspective of contemporary art and science. The above three stimulations will be explored through biomimicry, contemporary art and sustainability, as a frame to work with rural schools. We come from the premise that, in order to conceptualize equitable education, we must explore and implement reciprocity, collaboration and commitment, just as decolonial pedagogy has suggested. We believe these concepts offer alternative ideas as to what a museum can be; a space where the community holds the decision-making power in managing their cultural heritage and its place in history. Our work will demonstrate how community museums and contemporary arts can build spaces that offer more than exhibitions by representing local voices through co-creation. In doing so, we aim to provide the answers to the overarching provocation: What if museum spaces worked for a bigger social equity in their territories?, ¿Qué tal si los museos fuesen espacios comunitarios que elevaran narrativas locales, historias y futuros? ¿Qué tal si el arte contemporáneo pudiera usarse para alentar la coproducción de conocimiento a través del diálogo y la experimentación, centrándose a la vez en la equidad social? ¿Qué tal si hubiese un reconocimiento al hecho de que todos tenemos conocimiento con el cual contribuir, compartir y formar y así descentrar la idea de “un experto”? Este trabajo pretende compartir un diálogo entre dos investigadorxs interesadxs en una nueva realidad para los espacios museales y exhibitivos, desde la perspectiva del arte contemporáneo y la ciencia. Nos hemos hecho tres preguntas que, desde la biomimética, el arte contemporáneo y la sustentabilidad, serán respondidas como una eventual metodología de trabajo con escuelas rurales. Tenemos como premisa que para poder pensar una educación equitativa, debemos recurrir a los conceptos de reciprocidad, colaboración y compromiso que la educación decolonial ha sugerido. Creemos que estos conceptos ofrecen ideas alternativas a los que un museo puede ser; un espacio donde la comunidad mantiene el poder de decisión en la administración de su patrimonio y su lugar en la historia. Nuestro objetivo es demostrar cómo los museos comunitarios y el arte contemporáneo pueden construir espacios que ofrecen más exposiciones, representando las voces locales a través de la co-creación. Intentaremos en este diálogo proponer respuestas a ¿qué sucedería si los espacios museales trabajaran para una mayor equidad social en sus territorios?

Abstract

Introduction: Chemotherapy-induced peripheral neurotoxicity (CIPN) remains a significant toxicity in cancer survivors without preventative strategies or rehabilitation. Exercise and physical activity-based interventions have demonstrated promise in reducing existing CIPN symptoms and potentially preventing toxicity, however there is a significant gap in evidence due to the lack of quality clinical trials and appropriate outcome measures. Areas Covered: We systematically reviewed outcome measures in CIPN exercise and physical rehabilitation studies with expert panel consensus via the Peripheral Nerve Society Toxic Neuropathy Consortium to provide recommendations for future trials. Across 26 studies, 75 outcome measures were identified and grouped into 16 domains within three core areas–measures of manifestations of CIPN (e.g. symptoms/signs), measures of the impact of CIPN and other outcome measures. Expert Opinion: This article provides a conceptual framework for CIPN outcome measures and highlights the need for definition of a core outcome measures set. The authors provide recommendations for CIPN exercise and physical rehabilitation trial design and outcome measure selection. The development of a core outcome measure set will be critical in the search for neuroprotective and treatment approaches to support cancer survivors and to address the gap in the identification of effective rehabilitation and treatment options for CIPN.

Abstract

Chronic low back pain is one of the most common, costly, and debilitating pain conditions worldwide. Increased mechanistic understanding of the transition from acute to chronic low back and identification of predictive biomarkers could enhance the clinical assessment performed by health care providers and enable the development of targeted treatment to prevent and/or better manage chronic low back pain. This research protocol was designed to identify the neurological and transcriptomic biomarkers predictive of chronic low back pain at low back pain onset. This is a prospective descriptive longitudinal inception cohort study that will follow 340 individuals with acute low back pain and 40 healthy controls over 2 years. To analyze the neurophysiological and transcriptomic biomarkers of low back pain, the protocol includes psychological and pain-related survey data that will be collected beginning within six weeks of low back pain onset (baseline, 6, 12, 24, 52 weeks, 2 years) and remotely at five additional time points (8, 10, 16, 20 weeks, and 18 months). Quantitative sensory testing and collection of blood samples for RNA sequencing will occur during the six in-person visits. The study results will describe variations in the neurophysiological and transcriptomic profiles of healthy pain-free controls and individuals with low back pain who either recover to pain-free status or develop chronic low back pain.
