Abstract

Purpose of Review Review work to create and evaluate educational materials that could serve as a primary prevention strategy to help both providers and patients in Panama, Colombia, and the USA reduce disease burden of Toxoplasma infections. Recent Findings Educational programs had not been evaluated for efficacy in Panama, USA, or Colombia. Summary Educational programs for high school students, pregnant women, medical students and professionals, scientists, and lay personnel were created. In most settings, short-term effects were evaluated. In Panama, Colombia, and USA, all materials showed short-term utility in transmitting information to learners. These educational materials can serve as a component of larger public health programs to lower disease burden from congenital toxoplasmosis. Future priorities include conducting robust longitudinal studies of whether education correlates with reduced adverse disease outcomes, modifying educational materials as new information regarding region-specific risk factors is discovered, and ensuring materials are widely accessible.

2. Robustness of reported postacute health outcomes in children with SARS-CoV-2 infection : a systematic review

Abstract

ObjectiveTo systematically assess the robustness of reported postacute SARS-CoV-2 infection health outcomes in children.MethodsA search on PubMed and Web of Science was conducted to identify studies published up to 22 January 2022 that reported on postacute SARS-CoV-2 infection health outcomes in children (Results21 studies including 81 896 children reported up to 97 symptoms with follow-up periods of 2.0–11.5 months. Fifteen studies had no control group. The reported proportion of children with post-COVID syndrome was between 0% and 66.5% in children with SARS-CoV-2 infection (n=16 986) and between 2.0% and 53.3% in children without SARS-CoV-2 infection (n=64 910). Only two studies made a clear causal interpretation of an association between SARS-CoV-2 infection and the main outcome of ‘post-COVID syndrome’ and provided recommendations regarding prevention measures. The robustness of all 21 studies was seriously limited due to an overall critical risk of bias.ConclusionsThe robustness of reported postacute SARS-CoV-2 infection health outcomes in children is seriously limited, at least in all the published articles we could identify. None of the studies provided evidence with reasonable certainty on whether SARS-CoV-2 infection has an impact on postacute health outcomes, let alone to what extent. Children and their families urgently need much more reliable and methodologically robust evidence to address their concerns and improve care.

3. P-value and Bayesian analysis in randomized-controlled trials in child health research published in 2007 and 2017: a methodological review

Abstract

Background Reliance on P-value of significance in clinical trials is a source of debate because of misconceptions and misinterpretations associated with it. Bayesian methods are suggested as an alternative approach. As randomized-controlled trials (RCTs) are essential in generating research evidence, we investigated the change in the use of P-values and Bayesian analysis and the clustering of P-values at key significance levels in child health RCTs published in 2007 and 2017. Methods We searched Cochrane Central Register of Controlled Trials to identify random samples of child health RCTs published in 2007 (n = 300) and 2017 (n = 300). Data on trial characteristics and analytic approaches were extracted. We analyzed the 600 RCTs using the frequentist and Bayesian methods. The change in the proportion of trials reporting P-values and Bayesian analyses was assessed using Pearson/Fisher Exact tests and non-informative Dirichlet priors. Results Of 600 RCTs, 535 (89%) used frequentist methods only versus 65 (11%) that included some Bayesian methods. Only 2 of the 65 trials used Bayesian inferential statistics. The use of frequentist methods decreased from (273, 91% to 262, 87%) while the inclusion of Bayesian analysis slightly increased from (27, 9% to 38, 13%) between 2007 and 2017. Although most RCTs were from Europe (172, 29%) and North America (133, 22%), the increase in proportion of trials by continent was most in Asia (mean difference (MD) = 0.14, 95% credible interval (CI) 0.08–0.20) with posterior probability (PP) of 1.00. Parallel (487, 81.2%) and cluster (58, 9.7%) RCTs were the most common RCT types but the increase in cluster RCT (0.06, 95%CI 0.02–0.11, PP = 0.99) was more than any other RCT types over 10 years. We found clustering of P-values at the significance level of 0.05 (437, 72.8%), which increased between 2007 (209, 69.7%) and 2017 (228, 76%). The smallest P-value reported in this review was 0.0001 (1, 0.2%). Conclusions The statistical framework in child health RCTs has not changed from the frequentist methods that is based on P-values with an unexplained clustering at the significance level of 0.05. Bayesian methods may increase the confidence in interpretation of results of RCTs

Abstract

Purpose of Review Review international efforts to build a global public health initiative focused on toxoplasmosis with spillover benefits to save lives, sight, cognition and motor function benefiting maternal and child health. Recent Findings Multiple countries’ efforts to eliminate toxoplasmosis demonstrate progress and context for this review and new work. Summary Problems with potential solutions proposed include accessibility of accurate, inexpensive diagnostic testing, pre-natal screening and facilitating tools, missed and delayed neonatal diagnosis, restricted access, high costs, delays in obtaining medicines emergently, delayed insurance pre-approvals and high medicare copays taking considerable physician time and effort, harmful shortcuts being taken in methods to prepare medicines in settings where access is restricted, reluctance to perform ventriculoperitoneal shunts promptly when needed without recognition of potential benefit, access to resources for care, especially for marginalized populations, and limited use of recent advances in management of neurologic and retinal disease which can lead to good outcomes.

Purpose of ReviewReview building of programs to eliminateToxoplasmainfections.Recent FindingsMorbidity and mortality from toxoplasmosis led to programs in USA, Panama, and Colombia to facilitate understanding, treatment, prevention, and regional resources, incorporating student work.SummaryStudies foundational for building recent, regional approaches/programs are reviewed. Introduction provides an overview/review of programs in Panamá, the United States, and other countries. High prevalence/risk of exposure led to laws mandating testing in gestation, reporting, and development of broad-based teaching materials aboutToxoplasma.These were tested for efficacy as learning tools for high-school students, pregnant women, medical students, physicians, scientists, public health officials and general public. Digitized, free, smart phone application effectively taught pregnant women about toxoplasmosis prevention. Perinatal infection care programs, identifying true regional risk factors, and point-of-care gestational screening facilitate prevention and care. When implemented fully across all demographics, such programs present opportunities to save lives, sight, and cognition with considerable spillover benefits for individuals and societies.
