Background and Objectives: Hypertensive disorders, particularly pre-eclampsia, pose significant risks during pregnancy, affecting both maternal and neonatal health. The study aims to analyze short- and long-term health implications for mothers and their children, comparing those with pre-eclampsia to those without, to improve understanding of risk factors, diagnostic markers, and outcomes. Materials and Methods: This retrospective observational study involved 235 patients, 98 with pre-eclampsia and 137 without, monitored from 2015 to 2018 at the Obstetrics and Gynecology Department of the "Pius Brînzeu" Emergency County Clinical Hospital in Timișoara, Romania. Results: Women with pre-eclampsia were older, had higher BMIs, and more frequently had a family history of pre-eclampsia, hypertension, and diabetes. They also had lower educational and socioeconomic levels and fewer prenatal visits. Biochemical markers such as higher proteinuria, elevated sFlt-1, and lower PlGF were significant in diagnosing pre-eclampsia. Short-term maternal complications like eclampsia, HELLP syndrome, and acute kidney injury were more prevalent in the pre-eclampsia group. Neonatal outcomes included higher rates of preterm birth, low birth weight, and NICU admissions. Long-term mothers with a history of pre-eclampsia had higher incidences of chronic hypertension, cardiovascular disease, kidney problems, diabetes, and mental health disorders. Their children faced increased risks of neuropsychological delays, chronic respiratory issues, behavioral disorders, learning difficulties, and frequent infections. Conclusions: The study highlights the significant short- and long-term health impacts of pre-eclampsia on both mothers and their children. Early monitoring, intervention, and comprehensive management are crucial in mitigating these risks. Background: Preeclampsia (PE) and eclampsia (E) are severe pregnancy complications with significant maternal and neonatal health impacts. This study explores the association of the rs5707 polymorphism in the renin-angiotensin system (RAS) with PE/E and related neonatal outcomes. Materials and Methods: We conducted a cross-sectional study involving 400 mother–newborn dyads at the "Pius Brinzeu" Emergency Clinical Hospital Timisoara. Participants were divided into a control group (254 normotensive women) and a PE/E group (146 women with PE/E). Genotyping for the rs5707 polymorphism was performed using real-time PCR, and statistical analyses assessed associations with maternal body mass index (BMI) and neonatal outcomes. Results: The AA genotype of rs5707 was significantly associated with a reduced risk of PE/E and more favorable neonatal outcomes, including higher Apgar scores, greater birth weights, and longer gestational ages. Conversely, the AC genotype correlated with increased maternal BMI and adverse neonatal outcomes. Odds ratios highlighted the protective effect of the AA genotype against PE/E and the increased risk associated with the AC genotype. Conclusions: This study revealed the critical role of the rs5707 polymorphism in PE/E development and neonatal health. Genetic screening for rs5707 could enhance early identification and personalized intervention strategies, improving outcomes for both mothers and neonates. Preeclampsia (PE) is a hypertensive disorder in pregnancy associated with significant fetal and maternal complications. Antiphospholipid syndrome (APS) is an acquired form of thrombophilia characterized by recurrent venous or arterial thrombosis and obstetric complications that significantly increases morbidity and mortality rates. While preeclampsia may not be the most prevalent obstetric complication in APS, it significantly impacts the long-term health of both mother and child. The treatment of preeclampsia in antiphospholipid syndrome is different from the treatment of preeclampsia as an independent disease. Despite current treatments involving anticoagulants, antiplatelet agents, and antihypertensive drugs, obstetric complications may persist, underscoring the need for cohesive management and effective treatments. The objective of our review is to briefly present knowledge about the physiopathology of preeclampsia and the role of antiphospholipid antibodies in this process. Based on the existing literature, our review aims to identify future directions in molecular pathology toward the discovery of biomarkers and targeted treatments. The application of multidisciplinary approaches and prognostic models, including new biomarkers, could be beneficial in the prediction of PE. [ABSTRACT FROM AUTHOR]

Background: The cardiac biomarker NT-proBNP is released by the ventricles in response to increased cardiac wall tension, showing cardiac activity in heart failure. The primary objective of this comparative study was to analyze the variations of NT-proBNP levels among pregnant patients and to determine the potential influence of SARS-CoV-2 infection on these values. Secondly, the study focused on NT-proBNP levels and their influence on the type of birth. Methods: Blood samples were taken from 160 pregnant mothers in order to determine, through the solid-phase enzyme-linked immunosorbent assay (ELISA) method, the NT-proBNP concentrations from the plasma. The cohort was separated into two distinct groups based on SARS-CoV-2 diagnostic results: negative to the infection, and positive to the infection. Results: The SARS-CoV-2-positive group of patients presented with higher levels of NT-proBNP and had higher rates of cesarean sections. (4) Conclusions: Our research highlights the crucial relationship between elevated NT-proBNP values and the mode of giving birth, natural delivery or cesarean section, and also the influence of SARS-CoV-2 viral infection and this biomarker. Preeclampsia is a leading cause of maternal and fetal morbidity and mortality worldwide. The role of vitamin D supplementation during early pregnancy in the prevention of preeclampsia remains unclear. Our objective was to synthesize and critically appraise the available evidence from observational and interventional studies to determine the effects of early pregnancy vitamin D supplementation on the risk of preeclampsia. A systematic review was conducted in March 2023 using PubMed, Web of Science, Cochrane, and Scopus databases, including literature published up to February 2023. In adherence to PRISMA guidelines, a structured and systematic search strategy was employed. A total of five studies were included in the review, encompassing 1474 patients. Overall, vitamin D supplementation during early pregnancy was associated with a reduced incidence of preeclampsia in all studies (ORs ranging from 0.26 to 0.31), while others showed an increased risk of preeclampsia with low vitamin D levels during the first trimester (ORs of 4.60, 1.94, and 2.52). However, other studies found no significant protective effect but good overall safety for various vitamin D dosages administered during the first trimester. Nevertheless, variations in vitamin D dosage, the timing of supplementation, and definitions of vitamin D insufficiency may have contributed to the inconsistencies in the observed outcomes. Some studies reported significant secondary outcomes, such as a reduction in blood pressure, preterm labor, and improved neonatal outcomes, such as birth weight. The evidence from this systematic review suggests that early pregnancy vitamin D supplementation may have a role in reducing the risk of preeclampsia. However, inconsistencies in the timing of supplementation, dosages, and methodological differences between studies highlight the need for further research to determine the optimal supplementation strategy and to clarify the relationship between vitamin D and preeclampsia risk. Introduction. Adenomyosis is defined as the presence of endometrial tissue deep within the myometrium. It can be present diffusely throughout the myometrium or focally, forming a circumscribed collection called adenomyoma. Fibromatosis is a disease characterized by the appearance of fibroids (fibromatous tumors) or fibrosis (growth of fibers in a tissue). Uterine fibroma is a benign tumor that is irrigated from vessels branched from the uterine arteries, developed from the muscular elements of the uterus. The purpose of this study was to highlight the main differences between these two pathologies, as well as the approach from the point of view of case management. Materials and method. Between January 2021 and June 2023, we conducted a study at the ObstetricsGynecology Clinical Department I of the “Pius Brânzeu” County Emergency Clinical Hospital, Timişoara. We included patients whose ultrasonographic examination revealed the presence of uterine formations and whose anatomopathological findings later confirmed the following diagnoses. We kept records of information about the patients’ traits, clinical symptoms, additional pathological features made clear by ultrasonography, our response to each case, and how it developed. Results and conclusions. Abnormal vaginal bleeding was the main symptom of both illnesses. The evaluation of the patients revealed the ultrasonography findings that could be indicative of the illnesses under study. In contrast to fibromatosis cases, adenomyosis cases required a distinct approach to the case management. Depending on the specifics of the case, a customized strategy was necessary for adenomyosis instances. Adenomyosis cases were frequently challenging to manage and required a doctor with extensive experience. The distinctiveness of the cases, the surgeon’s experience and the accompanying pathologies had an impact on the progression and prognosis. [ABSTRACT FROM AUTHOR]

Introduction. There are several causes of fluid collection in the uterine cavity. Depending on the fluid type, we talk about hydrometra, hematometra and pyometra. With the help of ultrasonography, these aspects can be evident. The interpretation and management of these cases will consider the patient’s hormonal status. The purpose of this study was to determine whether endometrial fluid found during a transvaginal ultrasonography examination was a sign of pathological endometrial abnormalities, by presenting the management and evolution of several cases encountered in medical practice. Materials and method. We conducted a study between January 2021 and December 2022, within the Obstetrics-Gynecology Clinical Department I of the “Pius Brânzeu” County Emergency Clinical Hospital, Timişoara, in which we included patients in whom the ultrasonographic examination highlighted the presence of a fluid content at the level of the uterine cavity. We recorded data related to the characteristics of the patients, clinical symptoms, other pathological aspects highlighted by ultrasound, the approach to each case and their evolution. Results and conclusions. Fluid accumulation in the uterine cavity can be accompanied by painful symptoms or pathological vaginal discharge. In some cases, patients may be asymptomatic, and endometrial fluid collection is identified only during a routine ultrasonographic examination. This aspect is described in certain physiological situations and does not require medical intervention. In other cases, it is present in the context of benign or malignant conditions of the genital organs that require additional investigations and specialized treatment. In managing cases with endometrial fluid in postmenopausal women, it is recommended to consider endometrial thickness. Endometrial fluid is not a reason for further invasive testing of the endometrial cavity if the endometrial thickness is 4 mm or less. Introduction. The patient with uterine pathology currently benefits from minimally invasive treatment, which brings essential advantages compared to open surgery. Laparoscopic hysterectomy is the ideal solution for patients who have indication for this type of approach. This paper aims to highlight the incidence and evolution of the cases treated in Bega Clinic, Timișoara. Materials and method. A retrospective study was conducted between 2017 and 2019 in the obstetrics-gynecology clinics of the “Pius Brînzeu” County Emergency Clinical Hospital from Timișoara. The inclusion criteria were: the patient who underwent a total laparoscopic hysterectomy. In addition, the pathologies for which the treatment was applied, other associated pathologies if identified, the used surgical therapy, the evolution and the complications were evaluated. Results. Most indications for laparoscopic hysterectomy were the benign pathology, which was confirmed by histopathological examination of the removed specimens. Preoperative evaluation of the case is critical to identify potential risk factors for laparoscopic surgery on the patient. The evolution of the circumstances and the complications identified both intraoperative and postoperative were noted. The importance of team cooperation was highlighted to obtain better communication and coordination, so it would be possible to improve the results in these cases. Conclusions. When it comes to performing a hysterectomy, laparoscopic surgery is a preferred method by both physicians and patients, due to multiple benefits. [ABSTRACT FROM AUTHOR]
