Abstract

We study thermalization and thermal transport in single crystals of CsV$_3$Sb$_5$ through the CDW transition by directly measuring thermal diffusivity ($D$), thermal conductivity ($\kappa$), resistivity ($\rho$), and specific heat ($c$). Commensurate with previous reports, we observe a sharp, narrow anomaly in specific heat associated with a first order transition that results in a CDW state below $\sim94$ K. While a corresponding sharp anomaly in thermal diffusivity is also observed, resistivity and thermal conductivity only exhibit small steps at the transition, where the feature is sharp for resistivity and broader for thermal conductivity. Scrutinizing the thermal Einstein relation $\kappa=cD$, we find that this relation is satisfied in the entire temperature range, except in a narrow range around the transition. The Wiedemann-Franz law seems to work outside the critical region as well. Below the transition and persisting below the two-phase regime we find strong resemblance between the resistivity anomaly and the specific heat, which may point to a secondary electronic order parameter that emerges continuously below the transition., Comment: 11 pages, 6 figures

9. Dormant tumors circumvent tumor-specific adaptive immunity by establishing a Treg-dominated niche via DKK3

Abstract

Approximately 30% of breast cancer survivors deemed free of disease will experience locoregional or metastatic recurrence even up to 30 years after initial diagnosis, yet how residual/dormant tumor cells escape immunity elicited by the primary tumor remains unclear. We demonstrate that intrinsically dormant tumor cells are indeed recognized and lysed by antigen-specific T cells in vitro and elicit robust immune responses in vivo. However, despite close proximity to CD8+ killer T cells, dormant tumor cells themselves support early accumulation of protective FoxP3+ T regulatory cells (Tregs), which can be targeted to reduce tumor burden. These intrinsically dormant tumor cells maintain a hybrid epithelial/mesenchymal state that is associated with immune dysfunction, and we find that the tumor-derived, stem cell/basal cell protein Dickkopf WNT signaling pathway inhibitor 3 (DKK3) is critical for Treg inhibition of CD8+ T cells. We also demonstrate that DKK3 promotes immune-mediated progression of proliferative tumors and is significantly associated with poor survival and immunosuppression in human breast cancers. Together, these findings reveal that latent tumors can use fundamental mechanisms of tolerance to alter the T cell microenvironment and subvert immune detection. Thus, targeting these pathways, such as DKK3, may help render dormant tumors susceptible to immunotherapies.

Abstract

Introduction: Coronavirus Disease-2019 (COVID-19) Reporting and Data System (CO-RADS) score in Computed Tomography (CT) scan of the chest is a priority investigation in early identification of the disease. Although, a CT scan gives a clear parenchymal picture of the condition, it has its disadvantages of the cost and specificity in the prognosis of COVID-19. In addition, clinical studies have revealed its decreased utility in assessing the severity of the disease. Aim: To understand the correlation of the CO-RADS score with various inflammatory markers and explore the changes in CORADS score with the severity of COVID-19 infection. Materials and Methods: The present study is a retrospective observational study in a designated COVID-19 centre in Kolhapur, India. The radiological and pathological records of 64 cases for two months (September to October 2020) were reviewed and charted. The present study included all the cases over 18 years. The authors excluded pregnant women, patients with tuberculosis, interstitial lung disease, and pulmonary malignancy with a view of interference with the radiological presentation of COVID19. Along with demographics, biomarkers like Total Leucocyte Count (TLC), C-Reactive Protein (CRP), Lactate Dehydrogenase (LDH), D-Dimer, Interleukin-6 (IL-6), Procalcitonin (PCT), and serum ferritin were retrospectively reviewed and documented. The CO-RADS score, as reported by an expert radiologist, was noted down. Pearson’s correlation coefficient was used to correlate the CO-RADS score with various inflammatory markers. Abstract

Background We previously demonstrated potent antitumor activity against human breast cancer xenografts using photodynamic therapy (PDT) targeting a novel tumor-specific photosensitizer (HS201), which binds heat shock protein 90 (HS201-PDT). However, induction of systemic antitumor immunity by HS201-PDT alone or by the combination strategy with immune checkpoint blockade has yet to be determined.Methods Using unilateral and bilateral implantation models of syngeneic breast tumors (E0771, MM3MG-HER2, and JC-HER3) in mice, we assessed whether HS201-PDT could induce local and systemic antitumor immunity. In an attempt to achieve a stronger abscopal effect for distant tumors, the combination strategy with anti-PD-L1 antibody was tested. Tumor-infiltrating leukocytes were analyzed by single cell RNA-sequencing and receptor-ligand interactome analysis to characterize in more detailed the mechanisms of action of the treatment and key signaling pathways involved.Results HS201-PDT demonstrated greater tumor control and survival in immune competent mice than in immunocompromised mice, suggesting the role of induced antitumor immunity; however, survival was modest and an abscopal effect on distant implanted tumor was weak. A combination of HS201-PDT with anti-PD-L1 antibody demonstrated the greatest antigen-specific immune response, tumor growth suppression, prolonged mouse survival time and abscopal effect. The most significant increase of intratumoral, activated CD8+T cells and decrease of exhausted CD8+T cells occurred following combination treatment compared with HS201-PDT monotherapy. Receptor-ligand interactome analysis showed marked enhancement of several pathways, such as CXCL, GALECTIN, GITRL, PECAM1 and NOTCH, associated with CD8+T cell activation in the combination group. Notably, the expression of the CXCR3 gene signature was the highest in the combination group, possibly explaining the enhanced tumor infiltration by T cells.Conclusions The increased antitumor activity and upregulated CXCR3 gene signature induced by the combination of anti-PD-L1 antibody with HS201-PDT warrants the clinical testing of HS201-PDT combined with PD-1/PD-L1 blockade in patients with breast cancer, and the use of the CXCR3 gene signature as a biomarker.

Background The majority of colorectal carcinomas (CRCs) are insensitive to programmed death protein-1/programmed death-ligand 1 (anti-PD-1/PD-L1) immune checkpoint inhibitor (ICI) antibodies. While there are many causes for ICI insensitivity, recent studies suggest that suppression of innate immune gene expression in tumor cells could be a root cause of this insensitivity and an important factor in the evolution of tumor immunosuppression.Methods We first assessed the reduction of mitochondrial antiviral signaling gene (MAVS) and related RIG-I pathway gene expression in several patient RNA expression datasets. We then engineered MAVS expressing tumor cells and tested their ability to elicit innate and adaptive anti-tumor immunity using both in vitro and in vivo approaches, which we then confirmed using MAVS expressing viral vectors. Finally, we observed that MAVS stimulated PD-L1 expression in multiple cell types and then assessed the combination of PD-L1 ICI antibodies with MAVS tumor expression in vivo.Results MAVS was significantly downregulated in CRCs, but its re-expression could stimulate broad cellular interferon-related responses, in both murine and patient-derived CRCs. In vivo, local MAVS expression elicited significant anti-tumor responses in both immune-sensitive and insensitive CRC models, through the stimulation of an interferon responsive axis that provoked tumor antigen-specific adaptive immunity. Critically, we found that tumor-intrinsic MAVS expression triggered systemic adaptive immune responses that enabled abscopal CD8 +T cell cytotoxicity against distant CRCs. As MAVS also induced PD-L1 expression, we further found synergistic anti-tumor responses in combination with anti-PD-L1 ICIs.Conclusion These data demonstrate that intratumoral MAVS expression results in local and systemic tumor antigen-specific T cell responses, which could be combined with PD-L1 ICI to permit effective anti-tumor immunotherapy in ICI resistant cancers.

22. 'PALLCARE Seva'—A Beacon Amid the Catastrophic COVID-19 Times: A Cross-Sectional Study from a Rural Oncology Institute in Western Maharashtra

Abstract

Introduction Advanced cancer patients often require clinic or hospital follow-up for their symptom control to maintain their quality of life. But it becomes difficult for the patients to attend the same due to financial, commutation, and logistic issues. Objective The aim of this study was to audit the telephonic calls of the service and prospectively collected data to understand the quality of service provided to the patients at follow-up. Materials and Methods An ambispective observational study was conducted on the advanced stage cancer patients referred to the palliative care department at Kolhapur Cancer Center, Kolhapur, Maharashtra. We conducted an audit of the 523 telephonic calls of our service—“PALLCARE Seva” from June 2020 to February 2021. Prospectively, we assessed the quality of service based on 125 telephonic calls (n = 125) for this; we designed a questionnaire consisting of 11 items on the 5-point Likert scale for satisfaction by the patients or their caregivers at the follow-up. After a pilot study, the final format of questionnaire was used to collect the data. Results Of the 523 calls attended, we provided 30.11% patients with dosage change of medications for their symptom management, 16.25% patients have liaised with local general practitioners, and 14.34% of cases had to be referred for emergency management to our hospitals. We provided 23.9% of them with emotional and bereavement support and 6.21% with smartphone-based or video-assisted guidance to the patients and caregivers. Conclusions Liaison of general practitioners was possible in more than one-tenth of cases. The core components of our service were politeness and caring attitude, helpfulness, handling doubts regarding the illness, and an opportunity to share thoughts from the patients or caregivers. More than three-fourth of the callers have rated their experience as satisfactory and would recommend this service to other patients in need.

Today the most important concern in the field of cyber security is finding the serious problems that make loss in secure information. It is mainly due to malicious URLs. Malicious URLs are generated daily. This URLs are having a short life span. Various techniques are used by researchers for detecting such threats in a timely manner. Blacklist method is famous among them. Researchers uses this blacklist method for easily identifying the harmful URLs. They are very simple and easy method. Due to their simplicity they are used as a traditional method for detecting such URLs. But this method suffers from many problems. The lack of ability in detecting newly generated malicious URLs is one of the main drawbacks of Blacklist method. Heuristic approach is also used for identifying some common attacks. It is an advanced technique of Blacklist method. But this method cannot be used for all type of attacks. So this method is used very shortly. For a good experience, the researchers introduce machine learning techniques. Machine Learning techniques go through several phases and detect the malicious URLs in an accurate manner. This method also gives the details about the false positive rate. This review paper studies the different phases such as feature extraction phase and feature representation phase of machine learning techniques for detecting malicious URLs. Different machine learning algorithms used for such detection is also discuss in this paper. And also gives a better understanding about the advantage of using machine learning over other techniques for detecting malicious URLs and problems it suffers. Abstract

The rampant increase in the number of available cyber attack vectors and the frequency of cyber attacks necessitates the implementation of robust cybersecuritysystems. Malicious websites are a significant threat to cybersecurity. Miscreants and hackers use malicious websites for illegal activities such as disrupting the functioning of the systems by implanting malware, gaining unauthorized access to systems, or illegally collecting personal information. We propose and implement an approach for classifying malicious and benign websites given their Uniform Resource Locator(URL) as input. Using the URL provided by the user, we collect Lexical, Host-Based, and Content-Based features for the website. These features are fed into a supervised Machine Learning algorithm as input that classifies the URL as malicious or benign. The models are trained on a dataset consisting of multiple malicious and benign URLs. We have evaluated the accuracy of classification for Random forests, Gradient Boosted Decision Trees and Deep Neural Network classifiers. One loophole in the use of Machine learning for detection is the availability of the same training data to the attackers. This data is exploited by the miscreants to alter the features associated with the Malicious URLs, which will be classified as benign by the supervised learning algorithms. Further, owing to the dynamic nature of the malicious websites, we also propose a paradigm for detecting and countering these manually induced concept drifts. Objectives: Patients with chronic life-limiting or advanced respiratory diseases often suffer from high symptom burden, requiring palliative care to alleviate symptoms, improve quality of life and restore dignity. The present study explored the perception of respiratory physicians and their current practice of integrating palliative care for adult patients with chronic advanced respiratory diseases. Materials and Methods: An exploratory survey method using Google survey forms and SurveyMonkey was emailed to respiratory physicians between December 2020 and May 2021. Results: One hundred and seventy-two respiratory physicians responded to the survey. The majority of respiratory physicians (n = 153; 88.9%) thought that early integration of palliative care early was beneficial. They did not feel referring to palliative care would result in loss of control on patient care (n = 107; 62.21%) and 66 (38.37%) strongly disagreed that the referral would result in a loss of hope in patients. Further exploration into the training needs of respiratory physicians revealed that 121 (70.35%) felt the need for training in end-of-life care. Conclusion: Respiratory physicians in our study had an inclination toward palliative care integration into their routine clinical practice. A majority of them expressed the need to enhance their skills in palliative care. Therefore, concerted efforts at integration and a mutual exchange of knowledge between respiratory physicians and palliative care physicians will ensure that patients with advanced respiratory diseases are provided high-quality palliative care.

Abstract

Megestrol acetate is one of the pharmacological agents used for cancer-associated fatigue. To date, there are no studies on its use in the treatment of post-COVID-19 (coronavirus disease 2019) fatigue. So, we report a case of metastatic carcinoma lung with a partial response with three cycles of palliative chemotherapy. He was contracted with mild COVID-19 infection post three cycles of his chemotherapy. Post this episode, fatigue was his main and most troublesome symptom. After a thorough clinical history, physical examination, and investigations, type 2 post-COVID-19 syndrome was diagnosed. After explaining the risks and benefits, we started the patient on low-dose megestrol acetate (160 mg/d per oral) with low to moderate benefits. However, upon increasing the dose to 480 mg/d, the benefit on the subjective quality of life was significant. Studies with a larger sample and randomized controlled trials have to be conducted to substantiate the hypothesis and actual effect of megestrol acetate in the treatment of post-COVID-19 fatigue.
