Individuals with minor ischaemic stroke and intracranial occlusion are at increased risk of poor outcomes. Intravenous thrombolysis with tenecteplase might improve outcomes in this population. We aimed to test the superiority of intravenous tenecteplase over non-thrombolytic standard of care in patients with minor ischaemic stroke and intracranial occlusion or focal perfusion abnormality.

12. Heritability and Principal Component Analysis of Phytochemical Traits in Guava Under Indian Subtropics

- Abstract
Guava is rich in antioxidant compounds as it contains a high level of ascorbic acid, carotenoids and phenolic compounds. Genotypic variance, heritability and genetic advance study were carried out to assess the magnitude of variation among the different guava genotypes. High phenotypic and genotypic coefficients of variation were recorded for flavonoids (23.56 and 23.55, respectively) followed by antioxidant activity (18.97 and 18.88, respectively). Heritability and genetic advance (1.00 and 39.51, respectively) were recorded to be highest for flavonoids. Phenotypic coefficient of variance was slightly higher than the genotypic coefficient of variance for all the traits under study, indicating the existence of diversity in guava cultivars. Cluster analysis differentiated guava genotypes into distinct clusters based on flesh and skin colour as well as phytochemical properties. Fruit weight was negatively correlated with antioxidant activity, anthocyanins, lycopene and carotenoid content, which clearly indicates that white-fleshed guavas are larger in size but have lower amounts of these compounds. Correlation studies also revealed that antioxidant activity of fruits was strongly and positively regulated by total phenols, carotenoids and flavonoids. Further, it was worth noting that the ascorbic acid content was also negatively correlated with these phytochemical compounds, which might be due to the fact that the presence of anthocyanins has a negative impact on the presence of ascorbic acid. Thus, the results of the study can be further utilized in guava breeding programmes by understanding the inherent association of important phytochemical traits.

14. Racial Differences in Medications for Opioid Use Disorder Initiation in a Carceral Setting.

Background: The opioid overdose crisis significantly affects marginalized communities, with people of color experiencing higher rates of overdose and barriers to treatment. The syndemic of opioid use disorder and mass incarceration exacerbates racial health disparities. Some carceral facilities offer medication for addiction treatment, though no significant research explores differences in type of treatment uptake by race in these settings. This study focuses on the racial differences in medications for opioid use disorder (MOUD) preferences among incarcerated individuals., Methods: A retrospective cohort study was conducted at the Rhode Island Department of Corrections (RIDOC), examining MOUD-type preferences (buprenorphine or methadone) among incarcerated individuals. The study utilized RIDOC electronic medical records from January 1, 2017 to December 31, 2022, involving 3533 unique incarceration events. Participants were categorized by race (White vs non-White) and MOUD status (new initiation vs community continuation), with logistic regression models., Results: The study found no direct racial disparity in preferences for MOUD type. However, an interaction between race and MOUD initiation status significantly influenced MOUD-type preference. Among those initiating MOUD during incarceration, non-White individuals were more likely to choose buprenorphine compared to their White counterparts., Conclusions: This research provides new insights into the intersection of race, incarceration, and MOUD preferences. While direct racial disparities in MOUD type were not observed, the analysis uncovered a notable interaction effect: race influences the relationship between MOUD initiation status and the selected MOUD treatment during incarceration. Specifically, data demonstrate that the likelihood of choosing buprenorphine varies significantly based on both racial background and whether the treatment was initiated during incarceration or in the community. Contractile myosin and cell adhesion work together to induce tissue shape changes, but how they are patterned to achieve diverse morphogenetic outcomes remains unclear. Epithelial folding occurs via apical constriction, mediated by apical contractile myosin engaged with adherens junctions, as in Drosophila ventral furrow formation. While it has been shown that a multicellular gradient of myosin contractility determines folding shape, the impact of multicellular patterning of adherens junction levels on tissue folding is unknown. We identified a novel Drosophila gene moat essential for differential apical constriction and folding behaviors across the ventral epithelium which contains both folding ventral furrow and nonfolding ectodermal anterior midgut (ectoAMG). We show that Moat functions to downregulate polarity-dependent adherens junctions through inhibiting cortical clustering of Bazooka/Par3 proteins. Such downregulation of polarity-dependent junctions is critical for establishing a myosin-dependent pattern of adherens junctions, which in turn mediates differential apical constriction in the ventral epithelium. In moat mutants, abnormally high levels of polarity-dependent junctions promote ectopic apical constriction in cells with low-level contractile myosin, resulting in expansion of infolding from ventral furrow to ectoAMG, and flattening of ventral furrow constriction gradient. 17. Atrial Septal Aneurysm Leading to Ischemic Stroke: A Case Report and Literature Review.

- Abstract
Stroke is the most common cause of death and disability in the world, and ischemic etiology plays a major role. Atrial septal aneurysm (ASA) is a localized saccular deformity of the atrial septum, associated with ischemic stroke independently or in association with other atrial septal defects (ASD). There is a higher incidence of stroke in the population with ASD. In these patients, the presence of ASA is an important predictor of recurrent stroke. This is a case of ischemic stroke in a 44-year-old who presented with sudden-onset right-sided body weakness, expressive aphasia, and non-specific confusion for one hour, with an initial National Institutes of Health Stroke Scale (NIHSS) score of 7. CT angiogram revealed occlusion of the M3 branch of the left middle cerebral artery in the left lateral frontal lobe. Code stroke was called, and the patient was given tenecteplase (TNK), after which her right-side weakness and aphasia resolved. Trans-thoracic echo with bubble study showed ASA with positive bubble study. Lone ASA or ASA with concomitant ASD poses a higher risk of recurrent stroke in younger patients, especially those without significant risk factors for strokes. Patients with ASA and concomitant ASD are at high risk for recurrent ischemic stroke and should be kept under surveillance with continued medical therapy. 18. Investigation of magnetic exchange interactions in Cr-dopedCa0.5Sr0.5RuO3.

- Abstract
The value of Curie temperature ( T
C ) and resistivity forCa0.5Sr0.5Ru1-xCrxO3are found to increase with increase in x from 0 to 0.1. The TC increases from 38 K to 107 K. In this work, we investigate the increment using spin polarised density functional theory. The compounds exhibit Griffiths phase. Our results show that to understand the behaviour of the increase in T C, it is important to consider not only the competing super exchange and super-super exchange interactions between Ru/Cr-Ru/Cr ion pairs but also the increment in the value of the magnetic moment due to the localized character of the dopant (Cr) ion. Our results suggest that the increased resistivity with Cr doping could be due to increased scattering rate and strong on-site coulomb repulsion due to Cr doping. It is also important to note that the behaviour of T Cwith the Ru-O-Ru bond angle is not in line with the functional form reported for ferrites, chromites and some ferromagnetic oxides. 19. Structural and physical properties of Ni1-xV x alloys around and away from quantum critical point.

We investigate the room temperature structure (global and local), temperature dependent magnetic and transport behaviour of Ni1-xV
x (0⩽x⩽0.13) alloys. Our Energy Dispersive Analysis of x-rays results show that the prepared compositions are stoichiometric. With increase in V doping, the compounds exhibit a quantum phase transition around xc = 0.12, where the ferromagnetic phase is suppressed. Our results show that all the compounds stabilize in face centred cubic structure at RT and the lattice parameter shows unusual behaviour close to xc . The magnetic and heat capacity studies show signature of Griffiths phase on either side of xc . From 25 K to the lowest collected temperature, we observe a linear T dependence of resistivity at x = 0.1 and around xc , which is separated by a Fermi-liquid region around x = 0.106. This suggests that the origin of the transport behaviour is different around the quantum critical point and away from it. Our Ni K-edge x-ray Absorption Spectroscopy results show that there is a significant reduction in the first coordination number around Ni central atom on doping. Further, with doping, there is distortion in the first coordination shell around Ni. This suggests, with V doping, the local structure around Ni is different from the global structure as obtained from the x-ray Diffraction results. Interestingly, with doping, we observe a direct connection between the extent of distortion at RT and the magnetic disorder obtained at 2 K. We believe our results will motivate the scientific community to further study the interplay between the structural disorder and quantum fluctuations with temperature at the local level., (© 2024 IOP Publishing Ltd.)- Published

Cytomegalovirus (CMV) infection is often asymptomatic. However, in certain individuals, it can cause non-specific signs and symptoms that maybe hard to recognise. The condition may therefore be overlooked or misdiagnosed, leading to prolonged illness and serious sequelae. 21. Predictive modeling of thermal inactivation of non-O157 Shiga toxin-producing Escherichia coli in ground beef with varying fat contents.

A mathematical model to predict the thermal inactivation of non-O157 Shiga toxin-producing Escherichia coli (STEC) in ground beef was developed, with temperature and fat content of ground beef as controlling factors. Survival curves for a cocktail of non-O157 STEC strains in ground beef at four temperatures (55, 60, 65, and 68 °C) and six fat levels (5, 10, 15, 20, 25, and 30%) were generated. Nine primary models-log-linear, log-linear with tail, biphasic, sigmoidal, four-factor sigmoidal, Baranyi, Weibull, mixed Weibull, and Gompertz-were tested for fitting the survival curves. Primary modeling analysis showed the Weibull model had the highest accuracy factor and Akaike's weight, making it the best-fitting model. The parameters of the Weibull model were estimated using a nonlinear mixed, and response surface modeling was used to develop a second-order polynomial regression to estimate the impact of fat in ground beef and cooking temperature on the heat resistance of non-O157 STEC strains. The secondary model was successfully validated by comparing predicted lethality (log
10 CFU/g) with the observed values for ground beef containing 10 and 27% fat at 58 and 62 °C. Process lethality obtained from experimental data was within the prediction interval of the predictive model. 22. Pseudogap in BaPb x Bi 1‒ x O 3 ( x = 0.7, 0.75 and 1.0).

In this work, we have investigated the crystal and electronic structure of the orthorhombic phase of BaPb
x Bi1-xO3 (BPBO) for x = 0.7 (BPBO70), 0.75 (BPBO75) and 1.0 (BPO), using temperature dependent x-ray diffraction measurements, photoemission spectroscopy, and electronic structure calculations. Our results show the importance of particle size and strain in governing superconductivity. Interestingly, the temperature evolution of the structural parameters in the case of BPBO70 is similar to that of BPBO75 but the magnitude of the change is diminished. The BPBO75 and BPO compounds exhibit metallic nature, which is corroborated by the core level studies. The electronic structure calculations in conjunction with the core level studies suggest that oxygen vacancies play an important role for metallicity observed in the end compound. The exponent to the spectral line shape close to the Fermi level suggests the origin of pseudogap to be due to other contributions in addition to disorder in the case of BPBO70 and BPO. The core level studies also show that as one goes from x = 0.70 to 1.0, there occurs chemical potential shift towards the valence band suggesting hole doping. Our results open the venue to further study these compounds as a function of particle size, nature of carriers for its transport behaviour, electronic structure below TC , composition at the grain boundaries and microscopic origin of pseudogap in the non-superconducting phase. 23. A mentored hands-on training model for scaling up implementation and intervention research in India: "connecting the dots".

Despite the high burden of mental disorders in low- and middle-income countries (LMICs), less than 25% of those in need have access to appropriate services, in part due to a scarcity of locally relevant, evidence-based interventions and models of care. To address this gap, researchers from India and the United States and the Indian Council of Medical Research (ICMR) collaboratively developed a "Grantathon" model to provide mentored research training to 24 new principal investigators (PIs). This included a week-long didactic training, a customized web-based data entry/analysis system and a National Coordination Unit (NCU) to support PIs and track process objectives. Outcome objectives were assessed via scholarly output including publications, awards received and subsequent grants that were leveraged. Multiple mentorship strategies including collaborative problem-solving approaches were used to foster single-centre and multicentre research. Flexible, approachable and engaged support from mentors helped PIs overcome research barriers, and the NCU addressed local policy and day-to-day challenges through informal monthly review meetings. Bi-annual formal review presentations by all PIs continued through the COVID-19 pandemic, enabling interim results reporting and scientific review, also serving to reinforce accountability. To date, more than 33 publications, 47 scientific presentations, 12 awards, two measurement tools, five intervention manuals and eight research grants have been generated in an open-access environment. 3 and Sr0.5 Ca0.5 Ru1-xCrx O3 ( x = 0, 0.05 and 0.1) employing x-ray diffraction, resistivity, magnetic studies and x-ray photoemission spectroscopy. Our results show the compounds undergo a crossover from itinerant ferromagnetism to localized ferromagnetism. The combined studies suggest Ru and Cr be in the 4+ valence state. A Griffith phase and an enhancement in Curie temperature ( Tc ) from 38 K to 107 K are observed with Cr doping. A shift in the chemical potential towards the valence band is observed with Cr doping. In the metallic samples, interestingly, a direct link between the resistivity and orthorhombic strain is observed. We also observe a connection between orthorhombic strain and Tc in all the samples. Detailed studies in this direction will be helpful to choose suitable substrate materials for thin-film/device fabrication and hence manoeuvre its properties. In the non-metallic samples, the resistivity is mainly governed due to disorder, electron-electron correlation effects and a reduction in the number of electrons at the Fermi level. The value of the resistivity for the 5% Cr doped sample suggests semi-metallic behaviour. Understanding its nature in detail using electron spectroscopic techniques could unravel the possibility of its utility in high-mobility transistors at room temperature and its combined property with ferromagnetism will be helpful in making spintronic devices., (© 2023 IOP Publishing Ltd.)- Published

25. The Benefits, Challenges, and Strategies toward Establishing a Community-Engaged Knowledge Hub: An Integrative Review.

- Abstract
Current knowledge creation and mobilization efforts are concentrated in academic institutions. A community-engaged knowledge hub (CEKH) has the potential for transdisciplinary and cross-sectorial collaboration between knowledge producers, mobilizers, and users to develop more relevant and effective research practices as well as to increase community capacity in terms of knowledge production. Objective: To summarize existing original research articles on knowledge hubs or platforms and to identify the benefits, challenges, and ways to address challenges when developing a CEKH. Methods: This study followed a systematic integrative review design. Following a comprehensive search of academic and grey literature databases, we screened 9030 unique articles using predetermined inclusion criteria and identified 20 studies for the final synthesis. We employed thematic analysis to summarize the results. Results: The focus of the majority of these knowledge mobilization hubs was related to health and wellness. Knowledge hubs have a multitude of benefits for the key stakeholders including academics, communities, service providers, and policymakers, including improving dissemination processes, providing more effective community interventions, ensuring informed care, and creating policy assessment tools. Challenges in creating knowledge hubs are generally consistent for all stakeholders, rather than for individual stakeholders, and typically pertain to funding, resources, and conflicting perspectives. As such, strategies to address challenges are also emphasized and should be executed in unison. Conclusions: This study informs the development of a future CEKH through the identification of the benefits, challenges, and strategies to mitigate challenges when developing knowledge hubs. This study addresses a literature gap regarding the comparisons of knowledge hubs and stakeholder experiences.

Children spend many hours in urban parks and playgrounds, where the tree canopy could filter microplastics released from the surrounding urban hotspots. However, the majority of children's playgrounds also contain plastic structures that could potentially release microplastics. To assess if the children's playgrounds pose a higher exposure risk than other places inside the park, we evaluate the extent of microplastic contamination in the sand, soil, and leaf samples from 19 playgrounds inside urban parks in Los Angeles, CA, USA. The average microplastic concentration in sand samples collected inside the playground was 72 p g
-1 , and >50 % of identified plastics were either polyethylene or polypropylene. Microplastic concentrations inside the playgrounds were on average >5 times greater than concentrations outside the playgrounds in the park, indicating that children playing within the playground may be exposed to more microplastics than children playing outside the playground in the same park. By comparing the microplastic composition found inside and outside the playgrounds with the plastic composition of the plastic structures in the playground, we show that plastic structures and other products used inside the playgrounds could contribute to elevated microplastic concentration. The population density was slightly correlated with a microplastic concentration in the park soil but did not correlate with microplastic concentration inside the playgrounds. - Abstract
In this work, we have investigated the precursor effects to superconductivity in BaPb
0.75 Bi0.25 O3 using temperature dependent resistivity, x-ray diffraction technique and photoemission spectroscopy. The present compound exhibits superconductivity around 11 K (TC). The synthesis procedure adopted is much simpler as compared to the procedure available in the literature. In the temperature range (10 K-25 K) i.e. aboveTC, our results show an increase in both the orthorhombic and tetragonal strain. The well screened features observed in Bi and Pb 4f7/2core levels are indicative of the metallic nature of the sample. The compound exhibits finite intensity at the Fermi level at 300 K and this intensity decreases with decrease in temperature and develops into a pseudogap; the energy dependence of the spectral density of states suggests disordered metallic state. Furthermore, our band structure calculations reveal that the structural transition upon Pb doping results in the closing of the band gap at the Fermi level., (© 2022 IOP Publishing Ltd.)- Published

With the growing prevalence of obesity in the pediatric population, reports of its severe complications are increasing. Obesity hypoventilation syndrome is an uncommon disorder in children with altered respiratory mechanics, sleep-disordered breathing, and impaired ventilatory responses leading to persistent hypercapnia. Presentation is varied, and children may remain relatively asymptomatic until challenged with a respiratory infection, when they may present with acute respiratory failure. With increasing use of genetic testing in pediatric patients, our knowledge of potential contributors to hypoventilation syndromes is growing. Although mutations in the paired-like homeobox 2B gene are known to be causative of congenital central hypoventilation syndrome, other genes may also contribute to hypoventilation phenotypes. 29. Transport of microplastics in stormwater treatment systems under freeze-thaw cycles: Critical role of plastic density.

Stormwater treatment systems remove and accumulate microplastics from surface runoff, but some of them can be moved downward to groundwater by natural freeze-thaw cycles. Yet, it is unclear whether or how microplastic properties such as density could affect the extent to which freeze-thaw cycles would move microplastics in the subsurface. To examine the transport and redistribution of microplastics in the subsurface by freeze-thaw cycles, three types of microplastics, with density smaller than (polypropylene or PP), similar to (polystyrene or PS), or greater than (polyethylene terephthalate or PET) water, were first deposited on the top of packed sand-the most common filter media used in infiltration-based stormwater treatment systems. Then the columns were subjected to either 23 h of drying at 22 ⁰C (control) or freeze-thaw treatment (freezing at -20 ⁰C for 6 h and thawing at 22 ⁰C for 17 h) followed by a wetting event. The cycle was repeated 36 times, and the effluents were analyzed for microplastics. Microplastics were observed in effluents from the columns that were contaminated with PET and subjected to freeze-thaw cycles. Comparison of the distribution of microplastics in sand columns at the end of 36 cycles confirmed that freeze-thaw cycles could disproportionally accelerate the downward mobility of denser microplastics. Using a force balance model, we show that smaller microplastics (<50 µm) can be pushed at higher velocity by the ice-water interface, irrespective of the density of microplastics. However, plastic density becomes critical when the size of microplastics is larger than 50 µm. The coupled experimental studies and theoretical framework improved the understanding of why denser microplastics such as PET and PVC may move deeper into the subsurface in the stormwater treatment systems and consequently elevate groundwater pollution risk., (Copyright © 2022. Background: There is enough empirical literature suggesting impairments in neuropsychological functioning in patients with obsessive compulsive disorder (OCD) despite inconsistencies., Aim: The aim of the index study was to study the effect of Selective Serotonin Reuptake Inhibitors (SSRIs) on neuropsychological functions and illness severity in drug-naïve subjects with OCD., Methods: A total of 50 subjects with diagnosis of obsessive-compulsive disorder (DSM-5) in the age range of 18-55 years, drug naïve, without comorbid depression/anxiety disorders were randomized to receive either Sertraline or Fluvoxamine. Neuropsychological functions and severity of illness were evaluated using NIMHANS neuropsychological battery and Y-BOCS at baseline and 12 weeks posttreatment with SSRIs., Results: At baseline, study subjects had moderate severity of OCD and neuropsychological functions were impaired in a substantial number of subjects. More than half of the subjects were having impairment in verbal fluency and category fluency i.e., executive function impairment; the scores for DSST, DVT (assessing mental speed and sustained attention), Verbal N Back 1 error (assessing verbal working memory) WCST total trials (assessing set shifting) were found to be significantly correlated with Y-BOCS score i.e. severity of illness. Significant improvement was observed in both the illness severity and neuropsychological functions at end point. The proportion of adequate performers on various neuropsychological tests increased significantly at 12 weeks., Conclusion: A substantial number of patients with OCD have neuropsychological impairments, however, the pattern of impairments does not follow any sequence. Objective: Early response to selective serotonin reuptake inhibitors (SSRIs) at 4 weeks may predict response at 12 weeks. The objective of this study was to examine if early response to SSRIs can reliably indicate future response to the same drug, while simultaneously comparing 2 medications frequently used to treat obsessive-compulsive disorder (OCD): sertraline and fluvoxamine., Methods: This was a randomized, open-label prospective study of 50 drug-naive patients with OCD (per DSM-5 criteria) conducted from January 2019 to November 2020 in the outpatient department of a tertiary care teaching hospital in North India. Patients were randomly assigned to receive either sertraline or fluvoxamine after assessing the severity of illness with the Yale-Brown Obsessive-Compulsive Scale (YBOCS). Following the induction phase of treatment for 2 weeks, patients were assessed for early response, defined as ≥ 20% reduction in YBOCS score after 4 weeks of reaching the minimum therapeutic dose of each drug. The final YBOCS assessment to evaluate response was conducted at 12 weeks., Results: A total of 60% (n = 30) of patients with OCD showed early response at week 4 with a mean dose of sertraline 95 mg and fluvoxamine 102 mg/d. Both drugs showed significant reduction in YBOCS score at week 12. 32. Microplastics retained in stormwater control measures: Where do they come from and where do they go?

Stormwater control measures (SCM) can remove and accumulate microplastics and may serve as a long-term source of microplastics for groundwater pollution because of their potential for downward mobility in subsurface. Furthermore, the number of microplastics accumulated in SCM may have been underestimated as the calculation typically only accounts for microplastics accumulated via episodic stormwater loading and ignores microplastic accumuation via continuous atmospheric deposition. To evaluate the source pathways of accumulated microplastics and their potential for downward mobility to groundwater, we analyzed spatial distributions of microplastics above ground on the canopy around SCM and below ground in the subsurface in and outside the boundaries of fourteen SCM in Los Angeles. Using an exponential model, we link subsurface retardation of microplastics to the median particle size of soil (D
50 ) and land use. Despite receiving significantly more stormwater, microplastic concentrations in SCM at surface depth or subsurface depth were not significantly different from the concentration at the same depth outside the SCM. Similar concentration in and outside of SCM indicates that stormwater is not the sole source of microplastics accumulated in SCM. The high concentration of microplastics on leaves of vegetation in SCM confirms that the contribution of atmospheric deposition is significant. Within and outside the SCM boundary, microplastics are removed within the top 5 cm of the subsurface, and their concentration decreases exponentially with depth, indicating limited potential for groundwater pollution from the microplastics accumulated in SCM. Outside the SCM boundary, the subsurface retardation coefficient decreases with increases in D50 , indicating straining of microplastics as the dominant removal mechanism. Inside the boundary of SCM, however, the retardation coefficient was independent of D50 , implying that microplastics could have either moved deeper into the filter layer in SCM or that compost, mulch, or organic amendments used in the filter media were pre-contaminated with microplastics. Background: The YouTube platform has great potential of serving as a healthcare resource due to its easy accessibility, navigability and wide audience reach. Breast cancer screening is an important preventative measure that can reduce breast cancer mortality by 40%. Therefore, platforms being used as a healthcare resources, such as YouTube, can and should be used to advocate for essential preventative measures such as breast cancer screening., Methods: In this study, the usefulness of videos related to breast cancer and breast cancer screening were analyzed. Videos were first screened for inclusion and then were categorized into very useful, moderately useful, somewhat useful, and not useful categories according to a 10-point criteria scale developed by medical professionals based on existing breast cancer screening guidelines. Two reviewers independently assessed each video using the scale., Results: 200 videos were identified in the preliminary analysis (100 for the search phrase 'breast cancer' and 100 for the search phrase 'breast cancer screening'). After exclusion of duplicates and non-relevant videos, 162 videos were included in the final analysis. We found the following distribution of videos: 4.3% very useful, 17.9% moderately useful, 39.5% somewhat useful, and 38.3% not useful videos. There was a significant association between each of the following and the video's level of usefulness: video length, the number of likes, and the uploading source. Longer videos were very useful, somewhat useful videos were the most liked, personally produced videos were the most not useful, and advertisements produced the highest ratio of very useful to not useful videos., Conclusion: It is necessary to create more reliable and useful healthcare resources for the general population as well as to monitor health information on easily accessible social platforms such as YouTube.

34. Effect of sertraline and fluvoxamine on quality of life in patients with obsessive-compulsive disorder: A 12-week interventional study.

Background: Obsessive-compulsive disorder (OCD) is associated with poor quality of life (QoL) and functionality. Treatment leads to improvement in QoL and better functioning., Aim: To assess the effect of treatment with SSRIs on QoL and disability in first-episode, drug-naïve patients with OCD., Materials and Methods: Fifty first-episode, drug-naïve patients with a diagnosis of OCD according to the Diagnostic and Statistical Manual 5 were assessed for severity of illness (Y-BOCS), World Health Organization QoL (WHO-QoL Bref), and disability (WHODAS 2.0) at baseline and at 12 weeks after receiving treatment with either sertraline or fluvoxamine., Results: The scores for QoL were low and for disability were high at baseline, and the scores for WHO-QoL-Bref and WHODAS 2.0 improved significantly after 12 weeks of treatment compared to baseline. This improvement correlated with reduction in the illness severity scores on Yale-Brown Obsessive-Compulsive Scale. The responders to treatment had better QoL and lower disability compared to nonresponders., Conclusion: There is an impairment in QoL and disability in first-episode, drug-naïve patients with OCD, and QoL improves and disability decreases with adequate treatment with SSRIs, and this improvement correlates with improvement in the illness severity., Competing Interests: There are no conflicts of interest., (Copyright: © 2022 Industrial Psychiatry Journal.)

Importance: Thrombosis with thrombocytopenia syndrome (TTS) has been reported after vaccination with the SARS-CoV-2 vaccines ChAdOx1 nCov-19 (Oxford-AstraZeneca) and Ad26.COV2.S (Janssen/Johnson & Johnson)., Objective: To describe the clinical characteristics and outcome of patients with cerebral venous sinus thrombosis (CVST) after SARS-CoV-2 vaccination with and without TTS., Design, Setting, and Participants: This cohort study used data from an international registry of consecutive patients with CVST within 28 days of SARS-CoV-2 vaccination included between March 29 and June 18, 2021, from 81 hospitals in 19 countries. For reference, data from patients with CVST between 2015 and 2018 were derived from an existing international registry. Clinical characteristics and mortality rate were described for adults with (1) CVST in the setting of SARS-CoV-2 vaccine-induced immune thrombotic thrombocytopenia, (2) CVST after SARS-CoV-2 vaccination not fulling criteria for TTS, and (3) CVST unrelated to SARS-CoV-2 vaccination., Exposures: Patients were classified as having TTS if they had new-onset thrombocytopenia without recent exposure to heparin, in accordance with the Brighton Collaboration interim criteria., Main Outcomes and Measures: Clinical characteristics and mortality rate., Results: Of 116 patients with postvaccination CVST, 78 (67.2%) had TTS, of whom 76 had been vaccinated with ChAdOx1 nCov-19; 38 (32.8%) had no indication of TTS. The control group included 207 patients with CVST before the COVID-19 pandemic. A total of 63 of 78 (81%), 30 of 38 (79%), and 145 of 207 (70.0%) patients, respectively, were female, and the mean (SD) age was 45 (14), 55 (20), and 42 (16) years, respectively. Concomitant thromboembolism occurred in 25 of 70 patients (36%) in the TTS group, 2 of 35 (6%) in the no TTS group, and 10 of 206 (4.9%) in the control group, and in-hospital mortality rates were 47% (36 of 76; 95% CI, 37-58), 5% (2 of 37; 95% CI, 1-18), and 3.9% (8 of 207; 95% CI, 2.0-7.4), respectively. The mortality rate was 61% (14 of 23) among patients in the TTS group diagnosed before the condition garnered attention in the scientific community and 42% (22 of 53) among patients diagnosed later., Conclusions and Relevance: In this cohort study of patients with CVST, a distinct clinical profile and high mortality rate was observed in patients meeting criteria for TTS after SARS-CoV-2 vaccination.

- Abstract
Purpose: Neck dissection is often performed in patients with oral cancer to both treat and reduce the risk of subsequent neck metastases. Injury to the hypoglossal nerve may result in dysarthria, dysphagia, and profound difficulty with upper airway control. Although surgical landmarks facilitate intra-operative identification of vital structures to be preserved, they should not be an absolute measure, due to anatomical variants. We present a rare case of unilateral aberrant anatomy of the hypoglossal nerve, passing superficial to the internal jugular vein., Methods: A 70-year-old female presented to the emergency department with an indurated and ulcerated floor of mouth lesion, later confirmed to be a squamous cell carcinoma. She was treated with wide local excision, bilateral selective neck dissection of levels I to III, surgical tracheostomy, anterior mandibulectomy and reconstruction with a left composite radial forearm free flap., Results: A nerve-like structure was identified crossing superficially and perpendicular to the internal jugular vein within the left neck, which was later determined to be an anatomical variant of the hypoglossal nerve. This was carefully dissected and preserved, and the remainder of the surgery completed uneventfully. On the right, the hypoglossal nerve followed its normal anatomical course. The patient made a good recovery and suffered no neurological complications., Conclusion: Identification, meticulous dissection and preservation of the hypoglossal nerve is essential in lymphadenectomy involving levels I and II. Detailed knowledge of both normal and variant anatomy is fundamental for surgeons, which will allow for identification and protection of important neurovascular structures, thereby minimising surgical morbidity., (© 2021.
