In the view of the German government, the One Health approach is a pioneering compass for inter- and transdisciplinary thinking, networking, and action. To protect the health of humans, animals, plants, and ecosystems, it should always receive attention at all its interfaces and activities. The One Health approach has gained political importance in recent years and is being taken into account in several strategies.This article reports on the current strategies using a One Health approach. These include the German Antibiotic Resistance Strategy, the German Strategy for Adaptation to Climate Change, the global initiative Nature for Health, and the international pandemic agreement, which is currently being drafted and in which prevention also plays an important role. The issues of biodiversity loss and climate protection must be placed in a common context that takes into account the interdependencies of the health status of humans, animals, plants, and ecosystems. By involving relevant disciplines at different levels as a matter of course, we can succeed in making a joint contribution to sustainable development, as required by the United Nations' Agenda 2030. This perspective guides Germany's global engagement in global health policy toward greater stability, freedom, diversity, solidarity, and respect for human rights. Thus, a holistic approach such as One Health can contribute to achieving sustainability and strengthening democratic principles., (© 2023. The Author(s).)

Abstract: The urocortin 1 (UCN1)-expressing centrally projecting Edinger-Westphal (EWcp) nucleus is influenced by circadian rhythms, hormones, stress, and pain, all known migraine triggers. Our study investigated EWcp's potential involvement in migraine. Using RNAscope in situ hybridization and immunostaining, we examined the expression of calcitonin gene-related peptide (CGRP) receptor components in both mouse and human EWcp and dorsal raphe nucleus (DRN). Tracing study examined connection between EWcp and the spinal trigeminal nucleus (STN). The intraperitoneal CGRP injection model of migraine was applied and validated by light-dark box, and von Frey assays in mice, in situ hybridization combined with immunostaining, were used to assess the functional-morphological changes. The functional connectivity matrix of EW was examined using functional magnetic resonance imaging in control humans and interictal migraineurs. We proved the expression of CGRP receptor components in both murine and human DRN and EWcp. We identified a direct urocortinergic projection from EWcp to the STN. Photophobic behavior, periorbital hyperalgesia, increased c-fos gene-encoded protein immunoreactivity in the lateral periaqueductal gray matter and trigeminal ganglia, and phosphorylated c-AMP-responsive element binding protein in the STN supported the efficacy of CGRP-induced migraine-like state. Calcitonin gene-related peptide administration also increased c-fos gene-encoded protein expression, Ucn1 mRNA, and peptide content in EWcp/UCN1 neurons while reducing serotonin and tryptophan hydroxylase-2 levels in the DRN. Targeted ablation of EWcp/UCN1 neurons induced hyperalgesia. A positive functional connectivity between EW and STN as well as DRN has been identified by functional magnetic resonance imaging. Very short chemical exchange saturation transfer (CEST) pulses are beneficial in cardiac continuous wave (cw) CEST MRI, especially in small animals because of their rapid heartbeat; however, they result in signal modulations caused by Rabi oscillations. Therefore, we implemented two different filter techniques, DOwnsampling by SEparation of CEST spectrum into two parts (DOSE) and time domain (TD)-based filtering, to correct for these signal corruptions, allowing a reliable quantification of glucose-weighted CEST (glucoCEST) MRI contrast. In our study, cw CEST measurements were performed on a 9.4-T small animal BioSpec system using CEST pulses in the range of 10 to 200 ms. Experimental dependencies of Rabi oscillations on key MRI parameters were validated by Bloch-McConnell (BM) simulations. Filter efficiency was explored in a glucose concentration series as well as in the myocardium of healthy mice (n = 8), and glucoCEST contrast was subsequently quantified. The experimental results showed that the impact of Rabi oscillations on CEST spectra increased with decreasing CEST pulse length, optimized B 0 homogeneity, and shorter T 2 relaxation time, in accordance with results from BM simulations. Both investigated filter techniques reduced these signal modulations significantly, with DOSE filtering preserving the amplitude and TD filtering the spectral information of CEST data more accurately. Upon filter application, a significant decrease in glucoCEST contrast in the myocardium of healthy mice was observed after glucose infusion (p TD  = 0.0079, p DOSE  = 0.0044). Electro-osmotic flow (EOF) is a phenomenon where fluid motion occurs in porous materials or micro/nano-channels when an external electric field is applied. In the particular example of single-molecule electrophoresis using single nanopores, the role of EOF on the translocation velocity of the analyte molecule through the nanopore is not fully understood. The complexity arises from a combination of effects from hydrodynamics in restricted environments, electrostatics emanating from charge decorations and geometry of the pores. We address this fundamental issue using the Poisson–Nernst–Planck and Navier–Stokes (PNP–NS) equations for cylindrical solid-state nanopores and three representative protein nanopores (α-hemolysin, MspA, and CsgG). We present the velocity profiles inside the nanopores as a function of charge decoration and geometry of the pore and applied electric field. We report several unexpected results: (a) The apparent charges of the protein nanopores are different from their net charge and the surface charge of the whole protein geometry, and the net charge of inner surface is consistent with the apparent charge. (b) The fluid velocity depends non-monotonically on voltage. The three protein nanopores exhibit unique EOF and velocity–voltage relations, which cannot be simply deduced from their net charge. Furthermore, effective point mutations can significantly change both the direction and the magnitude of EOF. The present computational analysis offers an opportunity to further understand the origins of the speed of transport of charged macromolecules in restricted space and to design desirable nanopores for tuning the speed of macromolecules through nanopores. Background: Positron emission tomography (PET) with 15O-gas for quantifying cerebral blood flow (CBF) and oxygen metabolism is the gold standard for assessing hemodynamics in ischemic cerebrovascular disease. However, conventional 15O-gas PET methods do not provide information on regional arrival timing, a hemodynamic parameter typically measured using computed tomography (CT) perfusion with contrast media. This study demonstrated that 15O-gas PET with a state-of-the-art clinical PET scanner and optimized analysis can generate arrival time maps. In this retrospective study of ten patients with unilateral stenosis or occlusion of the major arteries, we compared PET-derived arrival time maps with CT perfusion Tmax maps. Results: In PET with short inhalation of [15O]-CO2 gases, dynamic images were reconstructed with 2-sec temporal resolution, followed by weighted least-squares fitting of one-tissue compartment models, with or without the contributions from vascular components. PET arrival time maps were visually comparable to CT perfusion Tmax maps regarding the spatial extent of delayed brain regions, with less noise and higher image quality when using the model without the vascular components. Region-of-interest analyses showed good correlations between the two modalities: correlation coefficients of 0.834 for absolute values and 0.718 for ipsilateral-to-contralateral differences, respectively, indicating that 15O-gas PET can quantitatively measure the arrival time with reasonable accuracy. Conclusions: The present method generates arrival-time maps with 15O-gas PET by applying optimized kinetic analysis to dynamic [15O]-CO2 images acquired using a state-of-the-art, high-sensitivity clinical PET scanner. Additional arrival time information for conventional PET parameters of CBF and oxygen metabolism may facilitate a more comprehensive understanding of the hemodynamic status in cerebrovascular steno-occlusive diseases. Purpose: This study aimed to evaluate the perfomance of Siemens Healthineers' StrokeSegApp performance in automatically segmenting diffusion and perfusion lesions in patients with acute ischemic stroke and to assess its clinical utility in guiding mechanical thrombectomy decisions. Methods: This retrospective study used MRI data of acute ischemic stroke patients from the prospective observational single-center 1000Plus study, acquired between September 2008 and June 2013 (clinicaltrials.org ; NCT00715533) and manually segmented by radiologists as the ground truth. The performance of the StrokeSegApp was compared against this ground truth using the dice similarity coefficient (DSC) and Bland–Altman plots. The study also evaluated the application's ability to recommend mechanical thrombectomy based on DEFUSE 2 and 3 trial criteria. Results: The StrokeSegApp demonstrated a mean DSC of 0.60 (95% CI: 0.57–0.63; n = 241) for diffusion deficit segmentation and 0.80 (95% CI: 0.76–0.85; n = 56) for perfusion deficit segmentation. The mean volume deviation was 0.49 mL for diffusion lesions and −7.69 mL for perfusion lesions. Out of 56 subjects meeting DEFUSE 2/3 criteria in the cohort, it correctly identified mechanical thrombectomy candidates with a sensitivity of 82.1% (95% CI: 63.1–93.9%) and a specificity of 96.4% (95% CI: 81.7–99.9%). Conclusion: The Siemens Healthineers' StrokeSegApp provides accurate automated segmentation of ischemic stroke lesions, comparable to human experts as well as similar commercial software, and shows potential as a reliable tool in clinical decision-making for stroke treatment. We employed high-resolution fMRI to distinguish the impacts of anisometropia and strabismus amblyopia on the evoked ocular dominance (OD) response. Sixteen amblyopic participants (eight females) plus eight individuals with normal vision (one female), participated in this study for whom we measured the difference between the response to stimulation of the two eyes, across areas V1-V4. In controls, the evoked OD response formed the expected striped pattern within V1. Compared to controls, the OD response in amblyopic participants formed larger fused patches that extended into downstream visual areas. Moreover, both anisometropic and strabismic participants showed elevated OD responses across V1-V4. Beyond these common effects, and despite similar densities of amblyopia between the two groups, strabismus, and anisometropia had differential impacts on the OD bias, binocular response, and correlation between V1 depth levels. Specifically, we found a greater increase in the size of the V1 portion that responded preferentially to fellow eye stimulation in anisometropic compared with strabismic individuals. We also found a greater difference between the amplitudes of the response to binocular stimulation, in those regions that responded preferentially to the fellow versus amblyopic eye, in anisometropic compared with strabismic participants. In contrast, strabismic participants demonstrated an increased correlation between the OD responses evoked within V1 superficial and deep depths, whereas anisometropic individuals did not. These results provide the primary direct functional evidence for distinct impacts of strabismus and anisometropia on the mesoscale functional organization of the human visual system, thus extending what was inferred previously about amblyopia from animal models. Background/Objectives: Alzheimer's disease (AD) is characterized by progressive brain atrophy marked by cognitive decline and memory loss, which significantly affect patients' quality of life. Transcranial ultrasound stimulation (TUS) is a potential physical treatment for AD patients. However, the specific brain regions stimulated by TUS and its therapeutic effects remain unclear. Methods: In this study, magnetic resonance imaging (MRI) and FreeSurfer segmentation were employed to assess alterations in the brain volume of AD patients after TUS. Results: Our findings revealed significant volume increases in the corpus callosum (CC) and lateral orbitofrontal cortex (lOFC) in the TUS group. Moreover, the volumetric changes in the CC were strongly correlated with improvements in the Mini-Mental State Examination score, which is a widely used measure of cognitive function of AD patients. Conclusions: TUS has the potential to alleviate disease progression and offers a non-invasive therapeutic approach to the improvement of cognitive function in AD patients. [ABSTRACT FROM AUTHOR]

In surface waters, photodegradation is a major abiotic removal pathway of the neurotoxin monomethylmercury (MMHg), acting as a key control on the amounts of MMHg available for biological uptake. Different environmental factors can alter the rate of MMHg photodegradation. However, our understanding of how MMHg photodegradation pathways in complex matrixes along the land-to-ocean aquatic continuum respond to changes in salinity, dissolved organic carbon (DOC) concentration and dissolved organic matter (DOM) composition is incomplete. In a set of laboratory experiments combining several artificial and natural waters, we demonstrate that the interplay of DOC concentration, DOM composition, and salinity affects the photodegradation rate of MMHg. The presence of DOM was found to facilitate MMHg photodegradation, but degradation rates were not altered by varying DOC concentrations over two orders of magnitude. We found DOM composition to have a stronger effect on MMHg photodegradation rates than DOC concentration. However, at high DOC levels, where most UV radiation was lost within the first cm of the reaction vessels, lower MMHg photodegradation rates were observed. When moving from terrestrially influenced waters, characterized by a high degree of humification, towards marine conditions with a protein-rich DOM pool, MMHg photodegradation rates increased. In contrast, salinity had a stabilizing effect on MMHg. Hence, especially in systems with low salt and DOC concentrations, changes in either salinity or DOC concentration can impact the photodegradation rates of MMHg. [ABSTRACT FROM AUTHOR]

From sequences of discrete events, humans build mental models of their world. Referred to as graph learning, the process produces a model encoding the graph of event-to-event transition probabilities. Recent evidence suggests that some networks are easier to learn than others, but the neural underpinnings of this effect remain unknown. Here we use fMRI to show that even over short timescales the network structure of a temporal sequence of stimuli determines the fidelity of event representations as well as the dimensionality of the space in which those representations are encoded: when the graph was modular as opposed to lattice-like, BOLD representations in visual areas better predicted trial identity and displayed higher intrinsic dimensionality. Broadly, our study shows that network context influences the strength of learned neural representations, motivating future work in the design, optimization, and adaptation of network contexts for distinct types of learning. Objective: To investigate differences in the microstructure of the spinothalamic tract (STT) white matter in people with chronic neck and shoulder pain (CNSP) using diffusion tensor imaging, and to assess its correlation with pain intensity and duration of the pain. Materials and methods: A 3.0T MRI scanner was used to perform diffusion tensor imaging scans on 31 people with CNSP and 24 healthy controls (HCs), employing the Automatic Fiber Segmentation and Quantification (AFQ) method to extract the STT and quantitatively analyze the fractional anisotropy (FA) and mean diffusivity (MD), reflecting the microstructural integrity of nerve fibers. Correlations of these differences with duration of pain and visual analog scale (VAS) scores were analyzed. Results: No significant differences in the mean FA or MD values of the bilateral STT were observed between people with CNSP and HCs (p > 0.05), as indicated by the two-sample t test. Further point-by-point comparison along 100 equidistant nodes within the STT pathway revealed significant reductions in FA values in the left (segments 12–18, 81–89) and right (segments 9–19, 76–80) STT in the CNSP group compared to HCs; significant increases in MD values were observed in the left (segments 1–13, 26–30, 71–91) and right (segments 8–17, 76–91) STT (p < 0.05, FWE corrected). Partial correlation analysis indicates that in people with CNSP, the FA values of the STT in regions with damaged white matter structure show a negative correlation with VAS scores and duration of pain, whereas MD values show a positive correlation with VAS scores and duration of pain. Conclusion: This study found that people with CNSP exhibit white matter microstructural abnormalities in the specific segments of STT. These abnormalities are associated with the patient's pain intensity and disease duration. The findings offer a new neuroimaging perspective on the pathophysiological basis of chronic pain in the ascending conduction process and its potential role in developing targeted intervention strategies. However, due to the limited sample size and the lack of statistical significance when analyzing the entire spinothalamic tract, these conclusions should be interpreted with caution. Objective: This study aims to evaluate the clinical significance of ultrasound-based measurement of optic nerve sheath diameter (ONSD) in predicting intracerebral hemorrhage (ICH) complicated by cerebral-cardiac syndrome (CCS). Methods: Patients with ICH and who were treated in the intensive care unit (ICU) at Shijiazhuang People's Hospital between October 2021 and November 2022 were included in this study. Participants were divided into two groups: those with CCS and those without. Various clinical parameters, including sex, age, electrocardiogram (ECG) findings, myocardial markers, B-type natriuretic peptide (BNP) levels, Glasgow Coma Scale (GCS) score, ONSD, hematoma volume, and midline shift, were assessed. A binary logistic regression model and receiver operating characteristic (ROC) curve analysis were employed to determine the predictive value of each risk factor for ICH complicated by CCS. Results: ONSD measurements differed significantly between males and females, with males exhibiting larger ONSD values. Additionally, significant differences were observed in ONSD, hematoma volume, midline shift, and GCS scores between the CCS and non-CCS groups. A direct correlation was identified between ONSD and both hematoma volume and midline shift. Multiple regression analysis demonstrated that ONSD, hematoma volume, and GCS score are independent risk factors for predicting ICH complicated by CCS. ROC curve analysis for ONSD in predicting ICH with CCS revealed an area under the curve (AUC) of 0.80, with an optimal cutoff value of 5.88 cm, yielding a sensitivity of 83% and a specificity of 79%. When ONSD, hematoma volume, and GCS score were combined, the predictive accuracy improved, with an AUC of 0.880. Conclusion: Males tend to have larger ONSD measurements compared to females. Ultrasound is a valuable tool for measuring ONSD, comparable to computed tomography, and is useful in detecting intracranial hypertension and mass effect. DNA computing represents a subfield of molecular computing with the potential to become a significant area of next-generation computation due to the high programmability inherent in the sequence-dependent molecular behaviour of DNA. Recent studies in DNA computing have extended from mathematical informatics to biomedical applications, with a particular focus on diagnostics that exploit the biocompatibility of DNA molecules. The output of DNA computing devices is encoded in nucleic acid molecules, which must then be decoded into human-recognizable signals for practical applications. Nanopore technology, which utilizes an electrical and label-free decoding approach, provides a unique platform to bridge DNA and electronic computing for practical use. In this tutorial review, we summarise the fundamental knowledge, technologies, and methodologies of DNA computing (logic gates, circuits, neural networks, and non-DNA input circuity). We then focus on nanopore-based decoding, and highlight recent advances in medical diagnostics targeting microRNAs as biomarkers. Finally, we conclude with the potential and challenges for the practical implementation of these techniques. We hope that this tutorial will provide a comprehensive insight and enable the general reader to grasp the fundamental principles and diverse applications of DNA computing and nanopore decoding, and will inspire a wide range of scientists to explore and push the boundaries of these technologies. BACKGROUND: Maximal safe resection is the objective of most neuro-oncological operations. Intraoperative magnetic resonance imaging (iMRI) may guide the surgeon to improve the extent of safe resection. There is limited evidence comparing the impact of iMRI on the rates of further resection between tumour types. AIM: To investigate the impact of iMRI on the rate of further resection following visualisation of residual tumour. METHODS: A retrospective cohort study identified all intracranial tumour operations performed in the 1.5 T iMRI machine of a single centre (2007–2023). Patients were identified using SurgiNet and were grouped according to their histopathological diagnosis in accordance with the WHO 2021 classification. The primary outcome was the rate of reoperation due to iMRI visualisation of residual tumours. RESULTS: A total of 574 cases were identified, including 152 low-grade gliomas (LGG), 108 high-grade gliomas (HGG), 194 pituitary neuroendocrine tumours (PitNETs), 15 metastases, and 6 meningiomas. Further resection following iMRI visualisation occurred in 45% of LGG cases, 47% of HGG cases, 29% of PitNET cases, and no meningioma or metastasis cases. Chi-square analysis showed that the rate of further resection after iMRI use across 2018–2023 was significantly higher than that across 2007–2012 (46% versus 33%, p = 0.036). CONCLUSION: Intraoperative MRI for guiding further resection was most useful in cases of LGG and HGG, possibly reflecting the difficulty of differentiating these tumour types from normal brain tissue. Optical coherence tomography angiography (OCTA), as a new generation of non-invasive and efficient fundus imaging technology, can provide non-invasive assessment of vascular lesions in the retina and choroid. In terms of anatomy and development, the retina is referred to as an extension of the central nervous system (CNS). CNS diseases are closely related to changes in fundus structure and blood vessels, and direct visualization of fundus structure and blood vessels provides an effective "window" for CNS research. This has important practical significance for identifying the characteristic changes of various CNS diseases on OCTA in the future, and plays a key role in promoting early screening, diagnosis, and monitoring of disease progression in CNS diseases. This article reviews relevant fundus studies by comparing and summarizing the unique advantages and existing limitations of OCTA in various CNS disease patients, in order to demonstrate the clinical significance of OCTA in the diagnosis and treatment of CNS diseases. Objective: Hemifacial spasm (HFS) is treated by a surgical procedure called microvascular decompression (MVD). However, HFS re-appearing phenomenon after surgery, presenting as early recurrence, is experienced by some patients after MVD. Dynamic susceptibility contrast (DSC) perfusion magnetic resonance imaging (MRI) and two analytical methods : receiver operating characteristic (ROC) curve and machine learning, were used to predict early recurrence in this study. Methods: This study enrolled 60 patients who underwent MVD for HFS. They were divided into two groups : group A consisted of 32 patients who had early recurrence and group B consisted of 28 patients who had no early recurrence of HFS. DSC perfusion MRI was undergone by all patients before the surgery to obtain the several parameters. ROC curve and machine learning methods were used to predict early recurrence using these parameters. Results: Group A had significantly lower relative cerebral blood flow than group B in most of the selected brain regions, as shown by the region-of-interest-based analysis. By combining three extraction fraction (EF) values at middle temporal gyrus, posterior cingulate, and brainstem, with age, using naive Bayes machine learning method, the best prediction model for early recurrence was obtained. This model had an area under the curve value of 0.845. Conclusion: By combining EF values with age or sex using machine learning methods, DSC perfusion MRI can be used to predict early recurrence before MVD surgery. This may help neurosurgeons to identify patients who are at risk of HFS recurrence and provide appropriate postoperative care. [ABSTRACT FROM AUTHOR]

Purpose: Our study aimed to evaluate the prognostic value of NT-proBNP in predicting adverse outcomes among patients with anterior circulation infarction (ACI) and posterior circulation infarction (PCI), specifically in those without pre-existing cardiac comorbidities. Patients and Methods: This single-center, prospective observational study enrolled patients with acute ischemic stroke (AIS) within 7 days of symptom onset. We aimed to elucidate predictive role of NT-proBNP levels in determining adverse outcomes in AIS patients. Additionally, the study sought to explore the relationship between NT-proBNP levels and the risk of poor functional outcomes in both ACI and PCI patients without underlying cardiac comorbidities. Results: A total of 821 patients were included in our study. Both univariate and multivariate logistic analyses indicated that higher NT-proBNP was an independent risk factor for adverse outcomes of ischemic stroke patients at 90 days. In noncardiogenic patients, the risks of adverse outcomes during follow-up were significantly elevated in the medium and high NT-proBNP groups (medium group: OR 1.75, 95% CI 1.03– 2.98, P=0.039; high group: OR 2.46, 95% CI 1.30– 4.67, P=0.006), with a dose-dependent trend. The association was similarly observed in patients with isolated ACI (medium group: OR 2.02, 95% CI 1.06– 3.83, P=0.031; high group: OR 2.70, 95% CI 1.25– 5.79, P=0.011). High NT-proBNP levels were independently associated with END in patients without underlying cardiac comorbidities (high group: OR 2.14, 95% CI 1.06– 4.31, P=0.033) and this association was also observed in ACI patients (high group: OR 5.39, 95% CI 1.70– 17.04, P=0.004). Moreover, when incorporated into the clinical prediction model, NT-proBNP exhibited excellent sensitivity and specificity for predicting stroke-related functional outcomes. Conclusion: NT-proBNP demonstrates potential as a valuable biomarker in the clinical predictive model for functional outcomes specifically in ACI patients suggesting that elevated NT-proBNP levels in these patients should prompt closer monitoring and more comprehensive patient management. Patients with unilateral spatial neglect (USN) are unable to explore or to report stimuli presented in the left personal and extra-personal space. USN is usually caused by lesion of the right parietal lobe: nowadays, it is also clear the key role of structural connections (the second and the third branch of the right Superior Longitudinal Fasciculus, respectively, SLF II and III) and functional networks (Dorsal and Ventral Attention Network, respectively, DAN and VAN) in USN. In this multimodal case report, we have merged those structural and functional information derived from a patient with a right parietal lobe tumour and USN before surgery. Functional, structural and neuropsychological data were also collected 6 months after surgery, when the USN was spontaneously recovered. Diffusion metrics and Functional Connectivity (FC) of the right SLF and DAN, before and after surgery, were compared with the same data of a patient with a tumour in a similar location, but without USN, and with a control sample. Results indicate an impairment in the right SLF III and a reduction of FC of the right DAN in patients with USN before surgery compared to controls; after surgery, when USN was recovered, patient's diffusion metrics and FC showed no differences compared to the controls. The aim of this study is to develop a novel phantom for the evaluation of clinical CEST imaging settings, e.g., B 0 and B 1 field inhomogeneities, CEST contrast, and post-processing. We made a phantom composed of two slice sections: a grid section for local offset frequency evaluation and a sample section for CEST effect evaluation using different concentrations of an egg white albumin solution. On a 3 Tesla MR scanner, a phantom study was performed using CEST imaging; the mean B 1 amplitudes were set at 1.2 and 1.9 µT, and CEST images with and without B 0 corrections were acquired. Next, region of interest (ROI) analysis was performed for each slice. Then, CEST images with and without B 0 corrections were compared at each B 1 amplitude. The B 0 corrected Z-spectrums at each local region in the grid section showed a shifting of the curve bottom to 0 ppm. Z-spectrum at B 1  = 1.9 µT showed a broader curve shape than that at 1.2 µT. Moreover, MTR asym values at 3.5 ppm for each albumin sample at B 1  = 1.9 µT were about two times higher than those at 1.2 µT. The intricate development of the human amygdala involves a complex interplay of diverse processes, varying in speed and duration. In humans, transient cytoarchitectural structures deliquesce, leading to the formation of functionally distinct nuclei as a result of multiple interdependent developmental events. This study compares the amygdala's cytoarchitectural development in conjunction with specific antibody reactivity for neuronal, glial, neuropil, and radial glial fibers, synaptic, extracellular matrix, and myelin components in 39 fetal human brains. We recognized that the early fetal period, as a continuation of the embryonic period, is still dominated by relatively uniform histogenetic processes. The typical appearance of ovoid cell clusters in the lateral nucleus during midfetal period is most likely associated with the cell migration and axonal growth processes in the developing human brain. Notably, synaptic markers are firstly detected in the corticomedial group of nuclei, while immunoreactivity for the panaxonal neurofilament marker SMI 312 is found dorsally. The late fetal period is characterized by a protracted migration process evidenced by the presence of doublecortin and SOX-2 immunoreactivity ventrally, in the prospective paralaminar nucleus, reinforced by vimentin immunoreactivity in the last remaining radial glial fibers. Nearing the term period, SMI 99 immunoreactivity indicates that perinatal myelination becomes prominent primarily along major axonal pathways, laying the foundation for more pronounced functional maturation. Polyelectrolytes can electrophoretically be driven through nanopores in order to be detected. The respective translocation events are often very fast and the process needs to be controlled to promote efficient detection. To this end, we attempt to control the translocation dynamics by coating the inner surface of a nanopore. For this, different charge distributions are chosen that result in substantial variations of the pore–polymer interactions. In addition and in view of the existing detection modalities, experimental settings, and nanopore materials, different types of sensors inside the nanopore have been considered to probe the translocation process and its temporal spread. The respective transport of polyelectrolytes through the coated nanopores is modeled through a multi-physics computational scheme that incorporates a mesoscopic/electrokinetic description for the solvent and particle-based scheme for the polymer. This investigation could underline the interplay between sensing modality, nanopore material, and detection accuracy. The electro-osmotic flow and electrophoretic motion in a pore are analyzed together with the polymeric temporal and spatial fluctuations unraveling their correlations and pathways to optimize the translocation speed and dynamics. Accordingly, this work sketches pathways in order to tune the pore–polymer interactions in order to control the translocation dynamics and, in the long run, errors in their measurements. This paper proposes a statistical physics-based data assimilation model for the mobility of water-bound hydrogen nuclear spins in the brain in the context of diffusion weighted magnetic resonance imaging (DWI or DW-MRI). Point of departure is a statistical hopping model that emulates molecular motion in the presence of static and stationary microscale obstacles, statistically reflected in the apparent inhomogeneous anisotropic DWI signal profiles. Subsequently, we propose a Riemann–Finsler geometric interpretation in terms of a metric transform that simulates this molecular process as free diffusion on a vacuous manifold with all diffusion obstacles absorbed in its geometry. The geometrization procedure supports the reconstruction of neural tracts (geodesic tractography) and their quantitative characterization (tractometry). The Riemann-DTI model for geodesic tractography based on diffusion tensor imaging (DTI) arises as a limiting case. The genuine Finslerian case is a geometric representation of high-angular resolution DTI , i.e., a generalized rank-two DTI framework without the quadratic restriction implied by a simplifying Gaussianity assumption on local diffusion or a second-order harmonic approximation of local orientation distributions. Introduction: Disorders of consciousness (DoC) from severe brain injuries have significant impacts. However, further research on nuanced biomarkers is needed to fully understand the condition. This study employed resting-state functional MRI (rs-fMRI) and the amplitude of low-frequency fluctuation (ALFF) to investigate differential brain activity in patients with DoC following spinal cord stimulation (SCS) therapy. It also assessed the predictive value of rs-fMRI and ALFF in determining the consciousness levels at 3 months post-therapy. Methods: We analyzed rs-fMRI data from 31 patients with traumatic brain injury (TBI) and 22 with non-traumatic brain injury (non-TBI) diagnosed with DoC. ALFF was measured before SCS therapy, and clinical outcomes were assessed 3 months later using the Coma Recovery Scale-Revised. Results: Patients with TBI showed increased ALFF in the thalamus and anterior cingulate cortex, whereas the middle occipital lobe showed decreased ALFF. In the non-TBI group, a higher ALFF was noted in the precuneus, with a reduced ALFF in the occipital and temporal lobes. Patients with improved consciousness post-SCS exhibited distinct ALFF patterns compared with those with unchanged consciousness, particularly in the posterior cingulate and occipital regions. Conclusion: The application of ALFF in rs-fMRI may be a predictive tool for post-treatment outcomes in patients with DoC of varying etiologies. Differential ALFF in specific brain regions could indicate the likelihood of improvement in consciousness following SCS therapy. The human brain is organized as a hierarchical global network. Functional connectivity research reveals that sensory cortices are connected to corresponding association cortices via a series of intermediate nodes linked by synchronous neural activity. These sensory pathways and relay stations converge onto central cortical hubs such as the default‐mode network (DMN). The DMN regions are believed to be critical for representing concepts and, hence, language acquisition and use. Although prior research has established that major senses are placed at a similar distance from the DMN—five to six connective steps—it is still unknown how the olfactory system functionally connects to the large‐scale cortical hubs of the human brain. In this study, we investigated the connective distance from olfactory seed areas to the DMN. The connective distance involves a series of three to four intermediate steps. Furthermore, we parcellated the olfactory cortical subregions and found evidence of two distinct olfactory pathways. One emerges from the anterior olfactory nucleus and olfactory tubercle; it involves early access to the orbitofrontal cortex, known for processing reward and multisensory signals. The other emerges from the frontal and temporal regions of the piriform cortex, involving the anterior insula, intermediate frontal sulcus, and parietal operculum. In glioma surgery, maximizing the extent of resection while preserving cognitive functions requires an understanding of the unique architecture of the white matter (WM) pathways of the single patient and of their spatial relationship with the tumor. Tractography enables the reconstruction of WM pathways, and bundle segmentation allows the identification of critical connections for functional preservation. This study evaluates the effectiveness of a streamline-based approach for bundle segmentation on a clinical dataset as compared to the traditional ROI-based approach. We performed bundle segmentation of the arcuate fasciculus, of its indirect anterior and posterior segments, and of the inferior fronto-occipital fasciculus in the healthy hemisphere of 25 high-grade glioma patients using both ROI- and streamline-based approaches. ROI-based segmentation involved manually delineating ROIs on MR anatomical images in Trackvis (V0.6.2.1). Streamline-based segmentations were performed in Tractome, which integrates clustering algorithms with the visual inspection and manipulation of streamlines. Shape analysis was conducted on each bundle. A paired t-test was performed on the irregularity measurement to compare segmentations achieved with the two approaches. Qualitative differences were evaluated through visual inspection. Streamline-based segmentation consistently yielded significantly lower irregularity scores (p < 0.001) compared to ROI-based segmentation for all the examined bundles, indicating more compact and accurate bundle reconstructions. Qualitative assessment identified common biases in ROI-based segmentations, such as the inclusion of anatomically implausible streamlines or streamlines with undesired trajectories. Streamline-based bundle segmentation with Tractome provides reliable and more accurate reconstructions compared to the ROI-based approach. By directly manipulating streamlines rather than relying on voxel-based ROI delineations, Tractome allows us to discern and discard implausible or undesired streamlines and to identify the course of WM bundles even when the anatomy is distorted by the lesion. Background: Tractography allows the in vivo study of subcortical white matter, and it is a potential tool for providing predictive indices on post-operative outcomes. We aim at establishing whether there is a relation between cognitive outcome and the status of the inferior fronto-occipital fasciculus's (IFOF's) microstructure. Methods: The longitudinal neuropsychological data of thirty young (median age: 35 years) patients operated on for DLGG in the left temporo-insular cortex along with pre-surgery tractography data were processed. Results: A degraded integrity of the left (vs. right) IFOF (lower fractional anisotropy and length, p < 0.001; higher mean and axial diffusivity, p < 0.01) was found, with lower microstructural variables in the infiltration (vs. dislocation) group. Significant decreases immediately post-surgery vs. pre-surgery mainly occurred in lexico-semantics (p < 0.001), with significant improvements at follow-up in all the tests (p < 0.01 to p < 0.001), despite values in the range of 44% to 47.82% of patients with below cut-off scores regarding naming verbs and making visual lexical decisions. The status of left and right IFOFs is predictive of a decrease in immediate post-surgery performance for several tests (p < 0.05); similarly, it is predictive of better recovery in the follow-up performance for naming nouns, naming verbs, making phonological fluency lexical decisions, and the token test (p < −0.05). For the ROC analysis, a significant result was obtained for the verb-naming test, with a cut-off of 79%. Conclusions: This study supports the role of the predictive value of pre-operative tractography for assessing the immediate post-operative result and at follow-up the risk of developing a cognitive deficit. This comprehensive review aims to explore imaging outcome measures targeting compartmentalized inflammation in Phase 2 clinical trials for multiple sclerosis (MS). The traditional primary imaging outcomes used in Phase 2 MS trials, new or enhancing white matter lesions on MRI, target the effects of peripheral inflammation, but the widespread inflammation behind a mostly closed blood-brain barrier is not captured. This review discusses several emerging imaging technologies that could be used as surrogate markers of compartmentalized inflammation, targeting chronic active lesions, meningeal inflammation, and innate immune activation within the normal-appearing white matter and gray matter. The integration of specific imaging outcomes into Phase 2 trials can provide a more accurate assessment of treatment efficacy, ultimately contributing to the development of more effective therapies for MS. Background: 11C-PBR28 positron emission tomography (PET), targeting the translocator protein, and paramagnetic rim lesions (PRL) have emerged as promising imaging markers of MS chronic inflammation. No consensus on which is the optimal marker exists. Objectives: To investigate the ability of 11C-PBR28 PET and PRL assessment to identify chronic inflammation in white matter (WM) MS lesions and their relation to neurological impairment. Methods: Based on 11C-PBR28 uptake, brain WM lesions from 30 MS patients were classified as PET active or inactive. The PRL presence was assessed on 7T phase reconstructions, T1/T2 ratio was calculated to measure WM microstructural integrity. Results: Less than half (44%) of non-PRL WM lesions were active on 11C-PBR28 imaging either throughout the lesion (whole active) or at its periphery. PET peripherally active lesions and PRL did not differ in T1/T2 ratio and 11C-PBR28 uptake. A positive correlation was observed between PRL and active PET lesion count. Whole active PET lesion volume was the strongest predictor (β = 0.97, p < 0.001) of increased Expanded Disability Status Scale scores. Conclusion: 11C-PBR28 imaging reveals more active WM lesions than 7T PRL assessment. Although PRL and PET active lesion counts are related, neurological disability is better explained by PET whole active lesion volume. [ABSTRACT FROM AUTHOR]

Introduction: Impulsivity is one of the overlapping symptoms common to borderline personality disorder (BPD) and attention deficit hyperactivity disorder (ADHD), but the neurobiological basis of these disorders remains uncertain. This systematic review aims to identify abnormalities in the gray and white matter associated with impulsivity in BPD and ADHD. Methods: We conducted a systematic search of the PubMed, Embase, and SCOPUS databases, adhering to PRISMA guidelines. Studies that investigated gray and white matter alterations in BPD or ADHD populations and their relationship with impulsivity were included. We reviewed information from 23 studies involving 992 participants, which included findings from structural MRI and DTI. Results: The review identified various nonhomogeneous changes associated with impulsivity in BPD and ADHD. BPD was mainly associated with abnormalities in the prefrontal cortex (PFC) and limbic areas, which correlated negatively with impulsivity. In contrast, impulsivity associated with ADHD was associated with structural changes in the caudate nucleus and frontal–striatal pathways. Despite the overlapping symptoms of impulsivity, the neurobiological mechanisms appeared to differ between the two disorders. Conclusions: These findings emphasize the distinct neurostructural correlates of impulsivity in BPD and ADHD. While both disorders show impulsivity as one of their main symptoms, the fundamental brain structures associated with this trait are different. BPD is primarily associated with abnormalities in the prefrontal cortex and limbic system, whereas the alterations seen in ADHD tend to focus on the caudate nucleus and frontostriatal pathways. With the fast‐growing and evolving omics data, the demand for streamlined and adaptable tools to handle bioinformatics analysis continues to grow. In response to this need, Automated Bioinformatics Analysis (AutoBA) is introduced, an autonomous AI agent designed explicitly for fully automated multi‐omic analyses based on large language models (LLMs). AutoBA simplifies the analytical process by requiring minimal user input while delivering detailed step‐by‐step plans for various bioinformatics tasks. AutoBA's unique capacity to self‐design analysis processes based on input data variations further underscores its versatility. Compared with online bioinformatic services, AutoBA offers multiple LLM backends, with options for both online and local usage, prioritizing data security and user privacy. In comparison to ChatGPT and open‐source LLMs, an automated code repair (ACR) mechanism in AutoBA is designed to improve its stability in automated end‐to‐end bioinformatics analysis tasks. Moreover, different from the predefined pipeline, AutoBA has adaptability in sync with emerging bioinformatics tools. This functional magnetic resonance imaging (fMRI) study examined resting-state (RS) connectivity in adolescent and adult patients with anorexia nervosa (AN) using symptom provocation paradigms. Differential food reward mechanisms were investigated through separate assessments of responses to food images and low-caloric/high-caloric food consumption. Thirteen young (≤ 21 years) and seventeen adult (> 21 years) patients with AN and age-matched controls underwent two stimulus-driven fMRI sessions involving RS scans before and after the presentation of food-related stimuli and food consumption. Graph theory and machine learning were used for analyzing the fMRI and clinical data. Healthy controls (HCs) showed widespread developmental changes, while young participants with AN exhibited cerebellum differences for high-calorie food. Young individuals with AN displayed increased connectivity during the consumption of potato chips compared to zucchini, with no differences in adults with AN. Multiparametric machine learning accurately distinguished young individuals with AN from healthy controls based on RS connectivity following food visual stimulation ("anticipatory") and consumption ("consummatory"). This study highlights the differential food reward mechanisms and minimal developmental changes in RS connectivity from youth to adulthood in individuals with AN compared to healthy controls. Young individuals with AN demonstrated heightened reactivity to high-caloric foods, while adults showed decreased responsiveness, potentially due to desensitization. Background: Iron accumulates in the brain during aging and is the focus of intensive research as an abnormal load, particularly in Deep Gray Matter (DGM), is related to neurodegeneration. Magnetic Resonance Imaging (MRI) metrics such as Quantitative Susceptibility Mapping (QSM) and apparent transverse relaxation rate R 2 * can be used to follow up iron in vivo. While the influence of age and sex on iron levels has already been reported, a careful consideration of neuronal risk factors, as well as for an enhanced sensitivity, is needed to define the normal evolution. Methods: QSM and R 2 * at ultra-high field MRI are used to study iron in DGM using a carefully-characterized cohort of the healthy aging brain (SENIOR). Seventy-seven cognitively healthy elders (from 54 to 78 y/o) with clinical, biology, genetics, and cardiovascular risk factors careful evaluation. Differences linked with age, sex, cardiovascular risk factors and weight are studied. Results: Age and sex have an influence on the brain iron deposition measured by QSM and R 2 * in a context of normal aging, without appearance of a pathological neurodegenerative process. Iron deposition shows higher values in the caudate and the putamen in older participants. Female participants present a higher level of iron in the amygdala, and males in the thalamus. Female participants also present differences in the accumbens, caudate and hippocampus when evaluating the joint age and sex effect. Participants with higher cardiovascular risk factors showed higher values of the iron, even without any impairment in their cognitive capability. An overweight is related with a higher iron load in the putamen for QSM and R 2 * in female participants. We controlled that these modifications of iron deposition are not related to a specific profile in the genotype of ApoE loci. Conclusions: Establishing baseline values of QSM and R 2 * as iron probes in the context of aging is essential to determine differences in the process of neurodegeneration. Age and sex of participants are important factors that affect brain iron normal values. On the other hand, the presence of cardiovascular risk factors, which can be associated with age related diseases, can also potentially be linked with the iron deposition in the brain. Previous studies demonstrated the existence of hand and tool areas in lateral and ventral occipitotemporal cortex (OTC), as well as an overlap between them. We reinvestigated this organization using 7T fMRI, benefiting from a higher signal-to-noise ratio than 3T. This enabled us to include a wider array of categories to achieve a more holistic perspective, and to omit certain spatial preprocessing steps. Despite these improvements, univariate analysis confirmed the existence of hand-tool overlap across OTC, which is striking given the omission of the spatial preprocessing steps that can influence overlap. There was significantly more overlap between hands and tools, compared to other overlap types in the left hemisphere of OTC. The overlap was also larger in the left lateral OTC as compared to the right lateral OTC. We found in all hand areas a differentiation between tools and other types of objects, although they still responded more to bodies than to tools. Regarding the tool areas, we observed a differentiation between hands and other categories such as faces and non-tool objects. Left hemisphere tool areas also differentiated between hands and bodies. When excluding the overlapping voxels from the hand and tool areas, they still showed a significant response to tools or hands (compared to objects or faces) respectively. Multi-voxel pattern analysis indicated that neural representations in the hand areas showed greater similarity between hands and tools than between hands and other objects. In the tool areas, the neural representations between tools and hands and between tools and other type of objects were all equally similar. To summarize, capitalizing on the benefits of 7T fMRI, we further substantiate the evidence in favor of hand-tool overlap in several regions of occipitotemporal cortex. [ABSTRACT FROM AUTHOR]

Introduction: Strokes are traditionally attributed to risk factors like aging, hypertension, diabetes, and atherosclerosis. Chagas disease has emerged as an important risk factor for stroke in Latin American. Our study aims at describing the largest cohort of patients with Chagas disease and ischemic stroke and determining variables associated with stroke recurrence and cardioembolic cause. Methods: This study is the result of a national multicenter cohort study conducted in Brazil. The study spanned from January 2009 to December 2016 and involved a comprehensive retrospective analysis of medical records of patients with both Chagas disease and stroke. This cohort comprised 499 individuals from diverse Brazilian regions, focusing on vascular risk factors and the epidemiological variables associated with Chagas disease and stroke. Results: Our findings underscore the significant prevalence of traditional vascular risk factors among Chagas disease patients who had stroke. 81% of patients had hypertension, 56% dyslipidemia, and 25% diabetes. We observed a 29.7% recurrence rate, especially within the cardioembolic subgroup. 56% of the patients had embolic stroke of undetermined source (ESUS). Specific electrocardiography abnormalities were associated with an increased risk of cardioembolic etiology (with three altered results increasing 81 fold the chance of the stroke being of cardioembolic nature). Age emerged as a protective factor (OR: 0.98, 95% confidence interval [CI]: 0.970–0.997) against cardioembolic etiology. Anticoagulation therapy was associated with reduced risk (OR: 0.221, 95% CI 0.104–0.472), highlighting the importance of accurate etiological classification. Conversely, female gender (OR: 1.83, 95% CI: 1.039–3.249) emerged as a significant risk factor for stroke recurrence. Conclusion: This study significantly advances our epidemiological understanding of the intersection between Chagas disease and stroke. It emphasizes the critical need for extensive epidemiological investigations, a deeper comprehension of stroke recurrence determinants, and accurate etiological classification to reduce the ESUS population. Our findings have substantial clinical implications, suggesting the need of control of vascular risk factors and comorbidities and hold promise for improving patient care and reducing the burden of Chagas disease and stroke worldwide. Despite the identification of several dozen genetic loci associated with ischemic stroke (IS), the genetic bases of this disease remain largely unexplored. In this research we present the results of genome-wide association studies (GWAS) based on classical statistical testing and machine learning algorithms (logistic regression, gradient boosting on decision trees, and tabular deep learning model TabNet). To build a consensus on the results obtained by different techniques, the Pareto-Optimal solution was proposed and applied. These methods were applied to real genotypic data of sick and healthy individuals of European ancestry obtained from the Database of Genotypes and Phenotypes (5,581 individuals, 883,749 single nucleotide polymorphisms). Finally, 131 genes were identified as candidates for association with the onset of IS. UBQLN1, TRPS1, and MUSK were previously described as associated with the course of IS in model animals. ACOT11 taking part in metabolism of fatty acids was shown for the first time to be associated with IS. The identified genes were compared with genes from the Illuminating Druggable Genome project. The product of GPR26 representing the G-coupled protein receptor can be considered as a therapeutic target for stroke prevention. Prior work has shown Americans have higher levels of memory specificity than East Asians. Neuroimaging studies have not investigated mechanisms that account for cultural differences at retrieval. In this study, we use fMRI to assess whether mnemonic discrimination, distinguishing novel from previously encountered stimuli, accounts for cultural differences in memory. Fifty-five American and 55 Taiwanese young adults completed an object recognition paradigm testing discrimination of old targets, similar lures and novel foils. Mnemonic discrimination was tested by comparing discrimination of similar lures from studied targets, and results showed the relationship between activity in right fusiform gyrus and behavioural discrimination between target and lure objects differed across cultural groups. Parametric modulation analyses of activity during lure correct rejections also indicated that groups differed in left superior parietal cortex response to variations in lure similarity. Additional analyses of old vs. new activity indicated that Americans and Taiwanese differ in the neural activity supporting general object recognition in the hippocampus, left inferior frontal gyrus and middle frontal gyrus. Results are juxtaposed against comparisons of the regions activated in common across the two cultures. Background: Moderate-severe traumatic brain injury (msTBI) stands as a prominent etiology of adult disability, with increased risk for cognitive impairment and dementia. Although some recovery often occurs within the first year postinjury, predicting long-term cognitive outcomes remains challenging, partly due to the significant pathophysiological heterogeneity of TBI, including acute cerebrovascular injury. The primary aim of our recently funded study, cerebral autoregulation, brain perfusion, and neurocognitive outcomes after traumatic brain injury (CAPCOG-TBI), is to determine if acute cerebrovascular dysfunction after msTBI measured using multimodal non-invasive neuromonitoring is associated with cognitive outcome at 1-year post-injury. Methods: This longitudinal observational study will be conducted at two Level 1 trauma centers in Texas, USA, and will include adult patients with msTBI, and/or mild TBI with neuroimaging abnormalities. Multimodal cerebral vascular assessment using transcranial Doppler and cerebral near-infrared spectroscopy (NIRS) will be conducted within 7-days of onset of TBI. Longitudinal outcomes, including cognitive/functional assessments (Glasgow Outcome Scale and Patient-Reported Outcomes Measurement Information System), cerebral vascular assessment, and imaging will be performed at follow-ups 3-, 6-, and 12-months post-injury. We aim to recruit 100 subjects with msTBI along with 30 orthopedic trauma controls (OTC). This study is funded by National Institute of Neurological Disease and Stroke (NINDS) and is registered on Clinicaltrial.org (NCT06480838). Expected results: We anticipate that msTBI patients will exhibit impaired cerebrovascular function in the acute phase compared to the OTC group. The severity of cerebrovascular dysfunction during this stage is expected to inversely correlate with cognitive and functional outcomes at 1-year postinjury. Additionally, recovery from cerebrovascular dysfunction is expected to be linked to cognitive recovery. Conclusion: The results of this study could help to understand the contribution of cerebrovascular dysfunction to cognitive outcomes after TBI and pave the way for innovative vascular-focused interventions aimed at enhancing cognitive recovery and mitigating neurodegeneration following msTB. Objective: We measured cerebral venous oxygenation after aneurysmal subarachnoid hemorrhage (aSAH) using quantitative susceptibility mapping (QSM) to explore its relationship with cognitive function. Methods: Twenty participants, including 10 patients with aSAH and 10 healthy volunteers as the control group, were included. Patients with aSAH were evaluated at 2 days, 3 weeks, and 6 months after aSAH. Each participant underwent magnetic resonance imaging and completed the Montreal Cognitive Assessment (MoCA) at baseline, midpoint, and endpoint. QSM was used to determine the magnetic susceptibility of the cerebral veins. Furthermore, the relationship between MoCA and oxygen saturation in the cerebral veins was examined. Results: The first scans of the cerebral veins and straight sinus susceptibility were considerably more significant in the aSAH group than in the healthy control group. At the 6-month follow-up, the mean oxygen saturation steadily increased in the aSAH group. Cerebral venous oxygen saturation was moderately correlated with MoCA (r = 0.5319, P =.0025). Conclusion: QSM can be used to measure changes in cerebral venous oxygenation levels in patients with aSAH. During the acute phase of aSAH, there is a reduction in the oxygen saturation in the cerebral veins, and the shift in oxygen saturation levels may correlate with cognitive outcomes in patients with aSAH. Currently, the oncogenic mechanism of endoplasmic reticulum stress‐CAF (ERS‐CAF) subpopulation in chordoma remains unknown. Here, single‐cell RNA sequencing, spatial transcriptomics, GeoMx Digital Spatial Profiler, data‐independent acquisition proteomics, bulk RNA‐seq, and multiplexed quantitative immunofluorescence are used to unveil the precise molecular mechanism of how ERS‐CAF affected chordoma progression. Results show that hypoxic microenvironment reprograms CAFs into ERS‐CAF subtype. Mechanistically, this occurrs via hypoxia‐mediated transcriptional upregulation of IER2. Overexpression of IER2 in CAFs promotes chordoma progression, which can be impeded by IER2 knockdown or use of ERS inhibitors. IER2 also induces expression of ERS‐CAF marker genes and results in production of a pro‐tumorigenic paracrine GMFG signaling, which exert its biological function via directly binding to ITGB1 on tumor cells. ITGB1 inhibition attenuates tumor malignant progression, which can be partially reversed by exogenous GMFG intervention. Further analyses reveal a positive correlation between ITGB1high tumor cell counts and SPP1+ macrophage density, as well as the spatial proximity of these two cell types. Clinically, a significant correlation of high IER2/ITGB1 expression with tumor aggressive phenotype and poor patient survival is observed. Collectively, the findings suggest that ERS‐CAF regulates SPP1+ macrophage to aggravate chordoma progression via the IER2/GMFG/ITGB1 axis, which may be targeted therapeutically in future.
