Objective: This preliminary study aims to use "dynamic thiol/disulfide homeostasis" with the new Erel method to evaluate the effect of oxidative stress in patients with endometrioma. Material and Method: The study group consisted of 40 cases with histologically confirmed endometrioma, and 40 women with no laparoscopically confirmed endometriosis were taken as the control group. Plasma thiol, total thiol (TT), and disulfide (SS) levels were measured with the new, fully automatic method described by Erel and Neselioglu. Serum Ca-125, sedimentation (Sed), C-reactive Protein (CRP), and thiol/disulfide levels were measured. The two groups' plasma thiol, total thiol, and disulfide levels were compared, and the relation between thiol/disulfide homeostasis and stage of the endometriosis, Ca-125, Sed, and CRP was evaluated. Results: In cases with endometrioma, disulfide/native thiol (3,12±2,02, 2,05±1,21, p=0,005) and disulfide/total thiol ratios (3,50±2,52, 2,22±1,36, p=0,006) were significantly increased, native thiol (469,30±126,52, 571,72±125,32 p=0,00) total thiol levels (505,17 ± 133,88, 603,0±134,22 p=0,02) were significantly decreased when compared with the control group. There was a positive correlation between native thiol level and revised American Society for Reproductive Medicine Classification (r-ASRM) (p=0,041). Conclusion: As expected, "dynamic thiol/disulfide homeostasis" with the new Erel method, the significant decrease in total thiol and native thiol levels, which are used as oxidative stress markers, and the increase in disulfide values demonstrated by this study support the hypothesis that oxidative stress plays a role in endometriosis and these markers can be used in the management of endometriosis

Aim of the study is to evaluate the pregnancy outcome and recurrence of the disease in patients with endometrioma. All patients who underwent laparoscopic surgery for endometrioma between January 2017- 2020 in Etlik Zubeyde Hanim Women Health Training and Research Hospital, had a stage 3-4 disease, and met the inclusion criteria were evaluated. The exclusion criteria were; being menopausal, having laparotomy for endometrioma surgery, or presence of concomitant leiomyoma. The post-operative reproductive outcome and the recurrence rate were evaluated. Out of the 97 patients recruited, 57 (58.8%) were followed up for infertility, while the remaining forty had no desire for future pregnancy. Sixty-seven patients had elective surgery, and 30 (30.9%) underwent emergency surgery. Pregnancy was achieved in 18 (31.5%) within 3-18 months (8.94±4.54) after surgery, of which seven (38.8%) were spontaneous pregnancies while 11 had pregnancy through assisted reproductive techniques. The live birth rate was 88.8%, thirteen term delivery. Out of 97 patients, only four (4,1%) had recurrent endometrioma. The total pregnancy rate was high in the secondary infertile group (44.8%-17.8%), and the IVF pregnancy rate (60%) was high in the primary infertile group. Endometrioma surgery can be recommended for selected patients while preserving ovarian tissue.

T/Y-shaped dysmorphic uterus is frequently associated with subfertility, abortion, preterm delivery, and recurrent implantation failure. In this study, the impact of hysteroscopic metroplasty for a T/Y-shaped uterus on obstetric outcomes was investigated in patients with infertility and recurrent pregnancy loss (RPL) and compared the uterine anomaly subgroups.Cases with infertility and/or RPL diagnosed with T/Y-shaped uterus by hysterosalpingography (HSG) and 3D transvaginal ultrasonography (3D TVUSG) with no other male or female pathology have recruited the study. Medical history and the obstetric outcome after two years of follow-up were recorded.Out of the 92 cases recruited, T-shaped uteri (Group-1) were observed in 30, while 62 (67.3%) patients had Y-shaped uteri. Pregnancy was achieved in 50 (54.3%) of the cases, 28 (56%) of the pregnancies occurred spontaneously, and in 22 (44%) pregnancy was achieved through assisted reproductive techniques (ART). Term delivery occurred in 29 (76.3%) of the pregnancies while 9 (23.6%) had preterm deliveries and a total of 38 (76%) resulted in a live birth. Out of the 31 patients with previous RPL history, 13 (41.9%) achieved pregnancy and 11 out of 13 (84.6%) had a term pregnancy followed by live birth.Spontaneous pregnancy, and term pregnancy rates after surgery were found to be promising in these uterine anomalies. The pregnancy outcome of the primary infertile patients was found the same in two groups (50%-46.1%), but the results of the RPL patients were found to be better in the T-shaped group than in the Y-shaped group (70%-28.5%).

Objective: To investigate the predictive value of systemic inflammatory markers for the success of labor induction with foley catheter.Material and Method: Low-risk pregnancies with a single, live, vertex presentation at ≥37 weeks of gestation who underwent labor induction with a foley catheter were included in this retrospective cohort study. Delivery by cesarean section after labor induction was considered as induction failure. The study population was divided into two groups as those who delivered vaginally after labor induction and those who delivered by cesarean section. Demographic and clinical characteristics, prepartum complete blood count values, neutrophil/lymphocyte ratio and platelet/lymphocyte ratio of the two groups were compared.Results: A total of 308 pregnant women were included in the study. Vaginal delivery was performed in 159 (51.6%) of the cases after labor induction, while pregnancy was terminated by cesarean section in 149 (48.4%) cases. The rate of nulliparity was found to be higher in cases who delivered by cesarean section (68.5%) than those who delivered vaginally (54.1%) (p = 0.01). There was no statistically significant difference between leukocyte counts (9804.9 ± 3283.4 h/mm3 vs. 10437.1 ± 4439.8 h/mm3), neutrophil/lymphocyte ratio (4.7 ± 4.1 vs. 4.4 ± 3.9) and platelet/lymphocyte ratio (149.3 ± 67.0 vs. 139.3 ± 50.1) of cases who delivered vaginally and required cesarean delivery (p>0.05).Conclusion: There was no significant relationship between systemic inflammation markers measured before labor induction and labor induction success. However, nulliparity was found to be a risk factor for the failure of labor induction with foley catheter., Amaç: Sistemik inflamasyon belirteçlerinin foley kateter ile doğum indüksiyonu başarısını öngörmedeki değerini araştırmak.Gereç ve Yöntem: Retrospektif kohort çalışmaya foley kateter ile doğum indüksiyonu uygulanan ≥ 37 hafta, tek, canlı, verteks geliş olan düşük riskli gebeler dahil edildi. Doğum indüksiyonu sonrası sezaryen ile doğumun gerçekleşmesi indüksiyon başarısızlığı olarak kabul edildi. Çalışma popülasyonu doğum indüksiyonu sonrası vajinal doğum yapanlar ve sezaryen doğum yapanlar olarak iki gruba ayrıldı. İki grubun demografik ve klinik özellikleri, prepartum tam kan sayımı değerleri, nötrofil/lenfosit oranı ve platelet/lenfosit oranı karşılaştırıldı.Bulgular: Çalışmaya toplam 308 gebe dahil edildi. Olguların 159’unda (%51,6) doğum indüksiyonu sonrası vajinal yolla doğum gerçekleşirken, 149’unda (%48,4) sezaryen doğum ile gebelik sonlandı. Nulliparite oranı sezaryen ile doğum yapan olgularda (%68,5) vajinal yolla doğum yapanlara (%54,1) göre daha yüksek bulundu (p=0,01). Vajinal yolla doğum yapan ve sezaryen doğum gereksinimi olan olguların lökosit sayıları (9804,9 ± 3283,4 h/mm3’e karşı 10437,1 ± 4439,8 h/mm3), nötrofil/lenfosit oranı (4,7 ± 4,1’e karşı 4,4 ± 3,9) ve trombosit/lenfosit (149,3 ± 67,0’e karşı 139,3 ± 50,1) oranı arasında istatistiksel olarak anlamlı bir fark saptanmadı (p>0,05). Sonuç: Doğum indüksiyonu öncesi bakılan sistemik inflamasyon belirteçleri ile doğum indüksiyon başarısı arasında anlamlı ilişki bulunmadı. Bununla birlikte nulliparitenin foley kateter ile doğum indüksiyon başarısızlığı için bir risk faktörü olduğu saptandı.
