Early cancer diagnosis through characterizing light propagation and nanotechnology increases the survival rate. The present research is aimed at evaluating the consequence of using natural nanoparticles in cancer therapy and diagnosis. Colon cancer cells were differentiated from the normal cells via investigating light diffusion combined with the fluorescence effect of the Ashwagandha chitosan nanoparticles (Ash C NPs). Ionic gelation technique synthesized the Ash C NPs. High-resolution transmission electron microscope, dynamic light scattering, and zeta potential characterized Ash C NPs. Fourier transform infrared spectroscopy analyzed Ash C NPs, chitosan, and Ashwagandha root water extract. Moreover, the MTT assay evaluated the cytotoxicity of Ash C NPs under the action of near-infrared light (NIR) irradiation. The MTT assay outcomes were statistically analyzed by Bonferroni post hoc multiple two-group comparisons using one-way variance analysis (ANOVA). Based on the Monte-Carlo simulation technique, the spatially resolved steady-state diffusely reflected light from the cancerous and healthy cells is acquired. The diffuse equation reconstructed the optical fluence rate using the finite element technique. The fluorescent effect of the nanoparticles was observed when the cells were irradiated with NIR. The MTT assay revealed a decrease in the cell viability under the action of Ash C NPs with and without laser irradiation. Colon cancer and normal cells were differentiated based on the optical characterization after laser irradiation. The light diffusion equation was successfully resolved for the fluence rate on cells’ surfaces showing different normal and cancer cells values. Abstract Background Without inclusion of diverse research participants, it is challenging to understand how study findings will translate into the real world. Despite this, a lack of inclusion of those from under-served groups in research is a prevailing problem due to multi-faceted barriers acting at multiple levels. Therefore, we rapidly reviewed international published literature, in relation to clinical trials, on barriers relating to inclusion, and evidence of approaches that are effective in overcoming these. Methods A rapid literature review was conducted searching PubMed for peer-reviewed articles that discussed barriers to inclusion or strategies to improve inclusion in clinical trial research published between 2010 and 2021. Grey literature articles were excluded. Results Seventy-two eligible articles were included. The main barriers identified were language and communication, lack of trust, access to trials, eligibility criteria, attitudes and beliefs, lack of knowledge around clinical trials, and logistical and practical issues. In relation to evidence-based strategies and enablers, two key themes arose: [1] a multi-faceted approach is essential [2]; no single strategy was universally effective either within or between trials. The key evidence-based strategies identified were cultural competency training, community partnerships, personalised approach, multilingual materials and staff, communication-specific strategies, increasing understanding and trust, and tackling logistical barriers. Conclusions Many of the barriers relating to inclusion are the same as those that impact trial design and healthcare delivery generally. However, the presentation of these barriers among different under-served groups may be unique to each population’s particular circumstances, background, and needs. Based on the literature, we make 15 recommendations that, if implemented, may help improve inclusion within clinical trials and clinical research more generally. The three main recommendations include improving cultural competency and sensitivity of all clinical trial staff through training and ongoing personal development, the need to establish a diverse community advisory panel for ongoing input into the research process, and increasing recruitment of staff from under-served groups. Implementation of these recommendations may help improve representation of under-served groups in clinical trials which would improve the external validity of associated findings.

Background: Sotalol is a class III antiarrhythmic drug used for the management of patients with atrial fibrillation to maintain sinus rhythm. Sotalol-induced QT interval prolongation can be proarrhythmic and is conventionally initiated in an inpatient setting where routine electrocardiographic (ECG) monitoring is available while sotalol reaches the steady state. The emergence of cellular-compatible home ECG devices, such as AliveCor's Kardia Mobile 6L, which offers 6-lead ECG, has made it possible to accurately measure QT intervals outside the hospital., Objectives: This study aimed to evaluate the safety, feasibility, and patient adherence to protocol-driven, pharmacist-led outpatient sotalol initiation using the Kardia Mobile 6L for remote ECG monitoring and to compare these outcomes with patients completing inpatient sotalol protocol for initiation., Methods: Patients who underwent outpatient sotalol initiation through an antiarrhythmic clinic for the treatment of both atrial and ventricular arrhythmias over a period of approximately 3 years, from September 1, 2020, to June 16, 2023, were retrospectively reviewed and compared with a cohort of inpatients initiated on sotalol. The outpatient cohort had a minimum longitudinal follow-up of 90 days., Results: A total of 263 patients using the outpatient sotalol initiation protocol were compared with 28 age- and sex-matched inpatients who underwent inpatient sotalol initiation. The outpatient cohort included 179 men (68%), with an age of 68.8 ± 10.1 years, CHA 2 DS 2 -VASc score of 3.48 ± 1.43, baseline 12-lead QTc interval of 440.77 ± 33.42 ms, and a left ventricular ejection fraction of 57.4% ± 9.23%. Outpatients were started on a median (minimum-maximum) dose of 120 (80-160) mg of sotalol twice a day (120-mg dose; n = 227; 86.3%) and ended at a median (minimum-maximum) dose of 120 (60-160) mg twice a day (120-mg dose; n = 217; 82.5%). The proportions of patients prescribed low-, moderate-, or high-dose sotalol twice a day (60 mg, 80 mg, 120 mg, and 160 mg) at the end of initiation were similar between the inpatient and outpatient groups (P = 0.5). The majority (98.9%; 260 of 263) of patients completed the 3-day outpatient initiation. Outpatient adherence was high during the 3-day initiation period among patients with varied age and socioeconomic background; 258 patients (98.1%) completed the ECG on day 1, and 240 (91.3%) completed the ECG on day 3. No significant QTc interval prolongation was observed during the outpatient postinitiation study period. There were similar sotalol discontinuation rates within 30 days of initiation in the inpatient vs outpatient cohorts (7% vs 8%; P > 0.90). A total of 17 (6.5%) patients stopped sotalol because of symptomatic bradycardia, and 1 death was observed over a 90-day follow-up period in the outpatient cohort., Conclusions: Outpatient initiation of sotalol through a protocol-driven, pharmacist-led antiarrhythmic clinic is feasible and safe, with high adherence rates in a diverse range of patients using personal remote ECG., Competing Interests: Funding Support and Author Disclosures Dr Labreck is a consultant for Boston Scientific and consultant for Atricure. Background: Between 25 and 75% of people with persistent post-acute sequelae of SARS-CoV-2 infection (PASC) experience cognitive difficulties, compromising functional ability, quality of life, and activities of daily living, including work. Despite this significant morbidity, there is a paucity of interventions for this disorder that have undergone evaluation within a formal trial setting. Therefore, we have developed a cognitive rehabilitation programme, specifically designed to address the cognitive symptoms of PASC, notably impaired attention and processing speed, while also accounting for other PASC symptoms (fatigue, post-exertional malaise) that may aggravate the cognitive impairment. This study protocol outlines a randomised controlled trial (RCT) designed to evaluate the effectiveness of this programme compared to standard clinical care., Methods: This is a multi-centre, parallel-group, individually randomised controlled trial, comparing standard clinical care with and without cognitive rehabilitation. We will recruit 120 non-hospitalised adults (aged 30-60 years) from three NHS sites in England with a history of COVID-19 infection and cognitive impairment persisting more than 3 months after the acute infection. Participants will be randomised (1:1) to the intervention or control groups, with the latter represented as a provision of standard clinical care without cognitive rehabilitation. The cognitive rehabilitation programme consists of ten 1-hour sessions, delivered weekly. Outcomes will be collected at baseline, 3, and 6 months, with participant-defined goal-attainment scores, relating to functional goals, at 3 months as the primary outcome measure. Secondary outcomes will be cognitive function, measures of quality of life, social functioning, mental health, fatigue, sleep, post-exertional malaise, and social and health care service use. We will also evaluate the health-economic benefits of cognitive rehabilitation in this population., Discussion: Cognitive impairment in PASC is a major cause of functional disability with no effective treatment. Accordingly, we will undertake an RCT of cognitive rehabilitation, the protocol of which is published here. Solving the Liouville-von-Neumann equation using a density operator provides a more complete picture of dynamical quantum phenomena than by using a wavepacket and solving the Schrödinger equation. As density operators are not restricted to the description of pure states, they can treat both thermalized and open systems. In practice, however, they are rarely used to study molecular systems as the computational resources required are even more prohibitive than those needed for wavepacket dynamics. In this paper, we demonstrate the potential utility of a scheme based on the powerful multi-layer multi-configurational time-dependent Hartree algorithm for propagating multi-dimensional density operators. Studies of two systems using this method are presented at a range of temperatures and including up to 13 degrees of freedom. The first case is single proton transfer in salicylaldimine, while the second is double proton transfer in porphycene. Non-aerated compost tea (CT) was prepared from compost derived from rockweed (Ascophyllum nodosum) and fish (cod, common ling, haddock, saithe) residues that fermented in water. Electrical conductivity, pH, concentrations of dry matter, ash, C, macronutrients (N, P, K, Ca, and Mg), and micronutrients (Cu, Fe, Mn, Mo, and Zn) of CT prepared under different fermentation conditions were measured. The effects of process factors, i.e., water/compost mass ratio (4.2–9.8 g/g) and fermentation time (4.2–9.8 days = 100–236 h), on the physicochemical properties of CT were quantified using quadratic polynomial models. CT obtained at optimal levels of process factors (4.2 g/g and 5.6 days = 134 h) was tested for lettuce seed germination and seedling growth. Diluted CT (25% CT + 75% ultrapure water) improved seedling growth while achieving a high germination percentage (97%). [ABSTRACT FROM AUTHOR]

The present study aimed to use geopolymer materials synthesized from different fly ashes, which are promising for the adsorption of copper ions from aqueous solutions. The characterization of fly ashes and prepared adsorbents was performed by energy-dispersive X-ray spectroscopy (EDS) analysis, Brunauer–Emmett–Teller (BET) surface area analysis, and Scanning Electron Microscopy (SEM). Taguchi and ANOVA methods were used to predict the effect of different working parameters on copper ion removal by prepared geopolymers. Based on data obtained by the Taguchi method, it was found that the factor most influencing the adsorption process is the type of adsorbent used, followed by the solution pH, the reaction time, the adsorbent dose, and the initial copper ion concentration. The ANOVA results agree with the Taguchi method. The optimal conditions of the adsorption process were: fly ash C modified by direct activation with 2 M NaOH, at 70 °C for 4 h, solution pH of 5, initial pollutant concentration of 300 mg/L, 40 g/L adsorbent dose, and 120 min of reaction time. Copper ion removal efficiency was determined experimentally under optimal conditions, achieving a value of 99.71%. [ABSTRACT FROM AUTHOR]

This article presents a comprehensive study on the development and characterization of a novel lightweight filling material, pond ash geomaterial (PAGM), composed of pond ash, expanded polystyrene (EPS) beads, cement, and water. The mechanical properties of the PAGM were systematically investigated through a series of laboratory tests, including density, unconfined compressive strength, stress–strain behavior, initial tangent modulus, energy absorption, and a newly introduced density/strength ratio. The effects of various mix ratios, such as EPS/PA (EPS beads to pond ash), C/PA (cement to pond ash), and W/PA (water to pond ash), were studied. The study showed that PAGM density depended on the EPS/PA ratio and ash gradation. Higher cement content (C/PA) enhanced unconfined compressive strength. Increased EPS/PA ratios reduced strength but enhanced ductility. The initial tangent modulus correlated with strength, while energy absorption related to the EPS/PA, C/PA, and W/PA ratios. The density/strength ratio emerged as a novel parameter to tailor PAGM properties, enabling the material to cater to specific engineering needs. The developed PAGM exhibited promising engineering properties and proved to be a strong and sustainable alternative to traditional lightweight construction materials. This study contributes valuable insights for the optimization and utilization of PAGM in various geotechnical applications. Practical Applications: Pond ash geomaterial (PAGM) has a wide range of uses in geotechnical engineering, making it an essential solution for a variety of construction projects. The flexibility and on-site customization enable adjusted properties to meet their particular needs. PAGM can be used effectively in controlled backfilling, abutment construction, pipeline trenching, embankment widening, and slope stabilization. Its lightweight design reduces pressure on lateral and subsurface parts, ensuring stability. A cost-effective solution to the problems that the automotive industry is facing nowadays regarding regulations on emissions and fuel efficiency is to achieve weight reduction of automobile parts. Glass fiber-reinforced polymers are regularly used to manufacture various components, and some parts may also contain thermoplastic elastomers for toughness improvement. This work aimed to investigate the effect of styrene-(ethylene-co-butylene)-styrene triblock copolymer (E) and treated fly ash (C) on the morphological, thermal, and mechanical properties of long glass fiber (G)-reinforced polypropylene (PP). Results showed that the composites obtained through melt processing methods presented similar thermal stability and improved (nano)mechanical properties compared to 25–30 wt.% G-reinforced PP composites (PP-25G/PP-30G). Specifically, the impact strength and surface hardness were greatly improved. The addition of 20 wt.% E led to a 25–39% increase in impact strength and surface elasticity, while the addition of 6.5 wt.% C led to a 16% increase in surface hardness. The composite based on 25 wt.% G, 6.5 wt.% C, and 20 wt.% E presented the best-balanced properties (8–17% increase in impact strength, 38–41% increase in axial strain, and 35% increase in surface hardness) compared with PP-30G/PP-25G. Structural and morphological analysis confirmed the presence of a strong interaction between the components that make the composites. Rice husk ash (RHA) is a commonly found pozzolanic material that has been used as a partial substitute for cement to reduce greenhouse gas emissions. Concrete-rice husk ash (C-RHA) piles can become an eco-friendly and cost-efficient slope reinforcement alternative. Therefore, a series of centrifuge modeling tests were conducted to understand the stabilities of precast and cast-on-site C-RHA pile-reinforced slopes when subjected to gravity loading. The results show that the precast construction method more substantially improves the slope stability immediately after construction because the precast pile strength peaks before the cast-on-site concrete is fully cured. Moreover, the cast-on-site C-RHA pile-reinforced slope is much more stable after the mixture is fully cured because the grout material creates better bonding between the pile and the surrounding soil as it seeps into the surrounding soil, improving the slope strength and stability.
