Abstract

Scientific evidence regularly guides policy decisions1, with behavioural science increasingly part of this process2. In April 2020, an influential paper3 proposed 19 policy recommendations (claims) detailing how evidence from behavioural science could contribute to efforts to reduce impacts and end the COVID-19 pandemic. Here we assess 747 pandemic-related research articles that empirically investigated those claims. We report the scale of evidence and whether evidence supports them to indicate applicability for policymaking. Two independent teams, involving 72 reviewers, found evidence for 18 of 19 claims, with both teams finding evidence supporting 16 (89%) of those 18 claims. The strongest evidence supported claims that anticipated culture, polarization and misinformation would be associated with policy effectiveness. Claims suggesting trusted leaders and positive social norms increased adherence to behavioural interventions also had strong empirical support, as did appealing to social consensus or bipartisan agreement. Targeted language in messaging yielded mixed effects and there were no effects for highlighting individual benefits or protecting others. No available evidence existed to assess any distinct differences in effects between using the terms physical distancing and social distancing. Analysis of 463 papers containing data showed generally large samples; 418 involved human participants with a mean of 16,848 (median of 1,699). That statistical power underscored improved suitability of behavioural science research for informing policy decisions. Furthermore, by implementing a standardized approach to evidence selection and synthesis, we amplify broader implications for advancing scientific evidence in policy formulation and prioritization.

Abstract

Renewable sources of electricity generation currently provide 47 % of the UK's total installed capacity. Offshore wind accounts for 13 % of the total and is set to increase significantly over the next decade, driven by the UK Government's target to reach net zero by the year 2050. An increasing number of offshore wind farms will use voltage source converter (VSC) high voltage direct current (HVDC) transmission schemes to transfer power to the onshore electricity grid. VSC-HVDC transmission is also being used to support the integration of renewables into future electricity grids. Given the rapid rollout of VSC-HVDC links, a thorough understanding of the technology is required. At present, much of the research into VSC-HVDC in the public domain fails to capture the complexities of real-world control hardware, leading to inaccuracies in simulation models. Limited research has been carried out into the effect of controller implementation in software upon real-world processing delays in modular multilevel converters (MMC). The research presented in this thesis addresses these shortcomings by providing a detailed analysis of the delays in the capacitor balancing control (CBC) loop of an MMC. Several sorting algorithms for use in the CBC loop have been implemented across a range of industrially representative control hardware and software platforms. The processing resource usage and execution delay have been measured to guide the algorithm selection process. A link between execution delay, choice of CBC method, and controller performance has been identified and is discussed. A simple method for incorporating this execution delay into a PSCAD/EMTDC simulation is then presented as a means of improving simulation model fidelity. A full suite of control software has been developed for a reduced-scale converter hardware prototype (CHP) MMC for future research into control loop delay and synchronisation. The implementation techniques developed are applicable to MMCs with a distributed control architecture in academia and industry.

Abstract

18F-GP1 is a novel highly specific radiotracer that binds to activated platelets and thrombus. We aimed to establish the observer repeatability of coronary, carotid and cerebral 18F-GP1 uptake in patients presenting with acute myocardial infarction or ischaemic stroke. Forty-three patients presenting with acute myocardial infarction or ischaemic stroke underwent hybrid positron emission tomography (PET) and computed tomography (CT) angiography. Qualitative and quantitative assessment of 18F-GP1 uptake was performed on coronary arteries, carotid arteries and brain parenchyma. Qualitative uptake of 18F-GP1 had excellent intraobserver and interobserver agreement, with complete agreement for the presence or absence of visual 18F-GP1 uptake. For quantitative analysis, there were excellent intraclass correlation coefficients for intraobserver repeatability for coronary artery, carotid artery and brain parenchymal SUVmax and TBRmax measurements (all ≥ 0.92). Coronary artery and brain parenchymal analyses showed the strongest agreement in SUVmax values with mean biases of − 0.04 (limits of agreement − 0.21 to 0.20) and 0.02 (limits of agreement − 0.29 to 0.32) respectively. There was good interclass correlation coefficients for interobserver repeatability for coronary artery, carotid artery and brain parenchymal SUVmax and TBRmax measurements (all ≥ 0.89). The strongest interobserver agreement was seen with brain parenchymal SUVmax (mean SUVmax 1.95 ± 0.94) and TBRmax (mean TBRmax 9.55 ± 6.56) with mean biases of − 0.05 (limits of agreement − 0.37 to 0.27) and 0.04 (limits of agreement − 0.59 to 0.52) respectively. Visual qualitative and quantitative 18F-GP1 PET-CT image analyses provide robust and repeatable measurements of activated platelets and thrombi within the coronary arteries, carotid arteries and brain parenchyma. Abstract

Simple Summary: In our paper, we examine various facets of prostate cancer lung metastasis. We explore their clinical manifestations, such as respiratory symptoms and potential complications. Diagnostic modalities, including imaging techniques and biomarker analysis, are scrutinized for accurate identification. Treatment strategies, encompassing surgery, radiation therapy, chemotherapy, and targeted therapies, are discussed for their efficacy. Survival outcomes are assessed to gauge the effectiveness of different interventions. Through comprehensive analysis, we aim to provide insights into managing prostate cancer lung metastasis and enhancing patient care and outcomes. Prostate cancer lung metastasis represents a clinical conundrum due to its implications for advanced disease progression and the complexities it introduces in treatment planning. As the disease progresses to distant sites such as the lung, the clinical management becomes increasingly intricate, requiring tailored therapeutic strategies to address the unique characteristics of metastatic lesions. This review seeks to synthesize the current state of knowledge surrounding prostate cancer metastasis to the lung, shedding light on the diverse array of clinical presentations encountered, ranging from subtle radiological findings to overt symptomatic manifestations. By examining the diagnostic modalities utilized in identifying this metastasis, including advanced imaging techniques and histopathological analyses, this review aims to provide insights into the diagnostic landscape and the challenges associated with accurately characterizing lung metastatic lesions in prostate cancer patients. Moreover, this review delves into the nuances of therapeutic interventions employed in managing prostate cancer lung metastasis, encompassing systemic treatments such as hormonal therapies and chemotherapy, as well as metastasis-directed therapies including surgery and radiotherapy. Abstract

This article presents a case series of 25 prostate cancer patients who developed malignant ascites, a rare and aggressive disease progression. The study found that malignant ascites due to prostate cancer is associated with advanced metastatic castration-resistant disease, particularly involving the liver, and poor clinical outcomes despite standard systemic therapy. The median overall survival for the whole cohort was 4 months, with no significant difference between patients who received chemotherapy and those who received supportive care. The article emphasizes the need for better systemic therapy and highlights the importance of providing the best supportive care to improve the quality of life for these patients.
