5. Learning Health Systems Research Competencies: A Survey of the Rehabilitation Research Community

Objective. The Learning Health Systems Rehabilitation Research Network (LeaRRn), an NIH-funded rehabilitation research resource center, aims to advance the research capacity of learning health systems (LHSs) within the rehabilitation community. A needs assessment survey was administered to inform development of educational resources. Methods. The online survey included 55 items addressing interest in and knowledge of 33 LHS research core competencies in 7 domains and additional items on respondent characteristics. Recruitment targeting rehabilitation researchers and health system collaborators was conducted by LeaRRn, LeaRRn health system partners, rehabilitation professional organizations, and research university program directors using email, listservs, and social media announcements. Results. Of the 650 people who initiated the survey, 410 respondents constituted the study sample. Respondents indicated interest in LHS research and responded to at least 1 competency item and/or demographic question. Two-thirds of the study sample had doctoral research degrees, and one-third reported research as their profession. The most common clinical disciplines were physical therapy (38%), communication sciences and disorders (22%), and occupational therapy (10%). Across all 55 competency items, 95% of respondents expressed "a lot" or "some" interest in learning more, but only 19% reported "a lot" of knowledge. Respondents reported "a lot" of interest in a range of topics, including selecting outcome measures that are meaningful to patients (78%) and implementing research evidence in health systems (75%). "None" or "some" knowledge was reported most often in Systems Science areas such as understanding the interrelationships between financing, organization, delivery, and rehabilitation outcomes (93%) and assessing the extent to which research activities will improve the equity of health systems (93%). Conclusion. Results from this large survey of the rehabilitation research community indicate strong interest in LHS research competencies and opportunities to advance skills and training. Impact. Competencies where respondents indicated high interest and limited knowledge can inform development of LHS educational content that is most needed. - Abstract
Objective: Idiopathic subglottic stenosis is a rare disease, and time to diagnosis is often prolonged. In the United States, some estimate it takes an average of 9 years for patients with similar rare disease to be diagnosed. Patient experience during this period is termed the diagnostic odyssey. The aim of this study is to use qualitative methods grounded in behavioral‐ecological conceptual frameworks to identify drivers of diagnostic odyssey length that can help inform efforts to improve health care for iSGS patients. Methods: Qualitative study using semi‐structured interviews. Setting consisted of participants who were recruited from those enrolled in a large, prospective multicenter trial. We use directed content analysis to analyze qualitative semi‐structured interviews with iSGS patients focusing on their pathways to diagnosis. Results: Overall, 30 patients with iSGS underwent semi‐structured interviews. The patient‐reported median time to diagnosis was 21 months. On average, the participants visited four different health care providers. Specialists were most likely to make an appropriate referral to otolaryngology that ended in diagnosis. However, when primary care providers referred to otolaryngology, patients experienced a shorter diagnostic odyssey. The most important behavioral‐ecological factors in accelerating diagnosis were strong social support for the patient and providers' willingness to refer. Conclusion: Several factors affected time to diagnosis for iSGS patients. Patient social capital was a catalyst in decreasing time to diagnosis. Patient‐reported medical paternalism and gatekeeping limited specialty care referrals extended diagnostic odysseys. BACKGROUND: Employers are increasingly seeking a competitive advantage through targeted hiring of people with disabilities. We conducted several case studies to learn more about companies that led in creating their own disability diversity initiatives. OBJECTIVE: In this article, we share insights emerging from case studies conducted across seven companies. We illustrate the motives, processes, and outcomes of these initiatives. METHODS: This study is built on the previously published case studies conducted across seven companies. We applied elements of consensual qualitative research (CQR) for the data collection and analyses before performing an in-depth qualitative content analysis using the data coded for each company, looking for commonalities and differences. RESULTS: Although practices differed, all companies experienced noted benefits. Committed leadership and complementary company values facilitated successful outcomes for initiatives. The strength or salience of disability-inclusive actions and practices appeared to moderate outcomes related to company performance, employee perceptions of the company, and cohesiveness. CONCLUSION: Company disability initiatives can yield positive impacts on company performance and culture. The practices we identified and their positive outcomes serve as beacons to other organizations that recognize disability as a valued part of company diversity. [ABSTRACT FROM AUTHOR]

- Abstract
Objectives: Serial intralesional steroid injection (SILSI) has been increasingly used to treat idiopathic subglottic stenosis (iSGS). Prior studies have shown effectiveness, but not in all patients. This multi‐institutional study evaluates the effect of SILSI on time to recurrent operation, peak expiratory flow (PEF), and dyspnea. Methods: Post‐hoc secondary analysis of the North American Airway Collaborative data were performed to evaluate the outcomes of iSGS patients undergoing and not undergoing SILSI. The primary outcome was time to recurrent operation, evaluated using Kaplan–Meier curves and Cox regression analysis. Secondary outcomes were change in PEF and clinical chronic obstructive pulmonary disease questionnaire (CCQ) score. Within patients undergoing SILSI, demographics, time from last procedure, and PEF at initiation of SILSI were evaluated to determine the effect on recurrence. Results: Two hundred and ninety patients were included, 238 undergoing endoscopic dilation alone and 52 undergoing dilation and SILSI. No differences in baseline characteristics were observed. There was no difference in time to recurrence (hazard ratio: 0.64; p = 0.183). There were no differences in PEF or CCQ across the 2.5‐year study period. Among 52 patients undergoing SILSI, PEF at the time of starting SILSI did not affect recurrence (χ2 = 0.09, p = 0.77). Conclusion: Patients undergoing and not undergoing SILSI had similar times to recurrence, PEF, and CCQ. Factors predicting recurrence among patients undergoing SILSI were not identified. These results support a randomized controlled trial with a uniform SILSI protocol to quantify the effects of SILSI on objective and subjective outcomes and help determine which iSGS patients benefit most. Objectives: To investigate how the symptoms of mucus and cough impact adults living with laryngotracheal stenosis, and to use this information to guide future research and treatment plans. Methods: A survey was developed with the support of patient advisors and distributed to people suffering with laryngotracheal stenosis. The survey comprised 15 closed and open questions relating to mucus and cough and included the Leicester Cough Questionnaire (LCQ). Descriptive statistics, X2 and thematic analyses were completed. Results: In total, 641 participants completed the survey, with 83.62% (n = 536) reporting problems with mucus; 79% having daily issues of varying severity that led to difficulties with cough (46.18%) and breathing (20.90%). Mucus affected voice and swallowing to a lesser degree. Respondents described a range of triggers; they identified smoky air as the worst environmental trigger. Strategies to manage mucus varied widely with drinking water (72.26%), increasing liquid intake in general (49.35%) and avoiding or reducing dairy (45.32%) the most common approaches to control symptoms. The LCQ showed a median total score of 14 (interquartile range 11-17) indicative of cough negatively affecting quality of life. Thematic analysis of free text responses identified 4 key themes--the Mucus Cycle, Social impact, Psychological impact, and Physical impact. Conclusion: This study shows the relevance of research focusing on mucus and cough and its negative impact on quality of life, among adults with laryngotracheal stenosis. It demonstrates the inconsistent advice and management strategies provided by clinicians for this issue. Further research is required to identify clearer treatment options and pathways. Career pathways are critical in the vocational rehabilitation (VR) of individuals with disabilities, linking education, training, and employment to improve career outcomes.This report explores the establishment of the Career Pathways Learning Community, the development of the Career Pathways Toolkit, and the transition to the Implementation Group, highlighting how these initiatives support effective career development frameworks for individuals with disabilities within VR systems.A nationwide recruitment process brought together 61 stakeholders, including counselors and administrators from various state VR agencies, to form a Learning Community focused on six key elements of Career Pathways. This effort facilitated the design of a customizable Toolkit and established an Implementation Group to apply it.The Toolkit consists of four sections: an introduction to the Career Pathways model, an exploration of its advantages, a comprehensive implementation guide, and evaluation strategies. It provides valuable resources for enhancing career development for individuals with disabilities, while the Implementation Group fosters systems change within VR practices.The development and implementation of Career Pathways within VR systems are crucial for enabling individuals with disabilities to achieve sustainable employment. The Career Pathways Toolkit, created by the Learning Community facilitated by VRTAC-QE, serves as a practical resource for fostering systems change. Background/Aims: Laryngotracheal stenosis is a rare but devastating proximal airway fibrosis that restricts a patient's ability to breathe. Treatment is primarily surgical and to date, there has never been a multi-institutional, randomized, prospective, and interventional clinical trial for a medical therapy to treat laryngotracheal stenosis. Therefore, we aimed to obtain patient feedback to guide successful trial design, recruitment, retention, and for identifying potential barriers to study participation. Methods: Over 1000 members of an international laryngotracheal stenosis online support community (the Living with Idiopathic Subglottic Stenosis Facebook group) were sent two questionnaires for a proposed interventional double-blinded, randomized, placebo-controlled clinical trial. Results: A total of 317 and 558 participants responded to the first and second surveys, respectively. The majority of participants (77%) were willing to consider enrollment, regardless of having a 50% chance of receiving placebo versus treatment (78%). The majority (84%) of participants were willing to travel 200 miles to participate for up to six in-person visits over 50 days. Specific side effects, including anemia/thrombocytopenia (72%) or risk of infection (69.3%) had the greatest impact on clinical trial participation with other side effects (peripheral edema (53%), oral ulcers (51%), and gastrointestinal side effects (41%)) having less impact. Conclusion: Patients with laryngotracheal stenosis possess nuanced insight into their disease and treatment options. As a group, they are extremely motivated for better therapies. Future laryngotracheal stenosis clinical trials should focus on providing excellent side effect -related education and utilizing feedback from online advocacy groups to optimize recruitment and retention. Staphylococcus aureus is a pathogen with a high prevalence of antibiotic resistance and is responsible for a majority of skin and soft tissue infections (SSTIs) in humans. Here, we aimed to determine the role of Ap4A as a second messenger pathway involved in the S. aureus respiratory stress response during infection. Ap4A is synthesized from AMP and ATP when aminoacyl tRNA synthetases lack tRNA substrate. Ap4A is subsequently degraded into two ADP by the enzyme YqeK in S. aureus. Ap4A exists in both prokaryotes and eukaryotes, but it is unclear whether it is primarily a damage signal in bacteria when exposed to stress or whether it also plays an important signaling role. As the mechanisms of Ap4A are yet to be fully understood, more research is needed to clarify its effects in S. aureus and its potential as a drug target. Here, we characterize the impact of yqeK deletion and overexpression on S. aureus during anaerobic and nitric oxide (NO ) induced respiratory stress. While deletion ofyqeK had a significant impact on S. aureus fitness during stress, overexpression of yqeK did not impact fitness. Subsequent phenotypic and transcriptional analysis will provide new insights into the role Ap4A plays as a potential secondary messenger important for responding to respiratory stress in S. aureus. The pathogen Staphylococcus aureus is responsible for the majority of skin and soft tissue infections (SSTIs) in humans, and can lead to sepsis and death. The immune response to infection includes induction of nitric oxide (NO), a membrane permeable radical which binds to metals in the electron transport chain and can inhibit respiration. Nitric oxide stress in bacteria also leads to degradation of lipid biosynthesis, damage to chromosomal integrity, and disrupted metabolic enzymes. S. aureus, however, continues to replicate in the presence of NO- and respiratory stress. We hypothesize that second messenger signaling pathways may be involved in the unique response of S. aureus to NO- stress. In the presence of oxygen, elevated levels of the second messenger cyclic di-adenylate monophosphate (c-di-AMP) correlate with greater reactive oxygen species (ROS) generation. Because ROS are potentiated by NO-stress and are one mechanism of NO- toxicity, we hypothesized that c-di-AMP signaling is integrated with the NO- stress response as well. C-di-AMP is synthesized from two ATP molecules by an adenylate cyclase (DacA) and is degraded by phosphodiesterase GdpP; both enzymes are essential in the growth and fitness of S. aureus. Here, we show that overexpression of dacA, which increased c-di-AMP levels,inhibited growth of S. aureus and was toxic during NO- stress. Interestingly, overexpression of GdpP failed to decrease c-di-AMP levels as expected, perhaps due to a feedback inhibition mechanism. Based on these preliminary data, it is clear that c-di-AMP plays a major role during stress in S. aureus. Ongoing experiments aim to elucidate the specific regulatory role of c-di-AMP and design an alternate strategy for decreasing c-di-AMP levels via DacA knockdown. Ultimately, determining the essential processes utilized by S. aureus to survive in sub-optimal conditions will lay a foundation for the development of drugs to target these mechanisms and attenuate virulence. Underserved populations face many barriers to vocational rehabilitation (VR) services. However, outreach can build bridges with underserved populations.This study compares Rehabilitation Services Administration (RSA) client data with US Census data to identify which demographic groups are underserved at the national level. This data analysis strategy can also be applied by agencies and providers to identify underserved populations at the local level.RSA 911 data from 77 agencies in the United States and its territories between the first quarter of 2021 through the third quarter of 2023 was compared to US Census data. RSA 911 data was analyzed, quantifying VR populations by demographic type. Diachronic analysis was utilized to identify service changes over time.This study found that several historically underserved demographic groups—including African Americans and Native Americans—were being served by VR at or above those groups’ representation among the general population. However, Hispanic/Latino, Asian, and female populations were underrepresented in VR applications.VR agencies and providers have made significant progress with many traditionally underserved groups. However, Hispanic/Latino, Asian, and female populations are groups that require enhanced outreach efforts. 43. Relevance of learning health systems to physiatrists and its synergy with implementation science: A commentary.

- Abstract
As health care attempts to bridge the gap between evidence and practice, the concept of the learning health system (LHS) is becoming increasingly relevant. LHS integrates evidence with health systems data, driving health care quality and outcomes through updates in policy, practice, and care delivery. In addition, LHS research is becoming critically important as there are several initiatives underway to increase research capacity, expertise, and implementation, including attempts to stimulate increasing numbers of LHS researchers. Physical Medicine & Rehabilitation (PM&R) physicians (physiatrists), nurses, therapists (physical therapists, occupational therapists, speech therapists, clinical psychologists), and scientists are affiliated with LHSs. As LHS research expands in health care systems, better awareness and understanding of LHSs and LHS research competencies are key for rehabilitation professionals including physiatrists. To address this need, the Agency of Healthcare Research and Quality (AHRQ) identified 33 core competencies, grouped into eight domains, for training LHS researchers. The domains are: (1) Systems Science; (2) Research Questions and Standards of Scientific Evidence; (3) Research Methods; (4) Informatics; (5) Ethics of Research and Implementation in Health Systems; (6) Improvement and Implementation Science; (7) Engagement, Leadership, and Research Management; and the recently added (8) Health and Healthcare Equity and Justice. The purpose of this commentary is to define LHS and its relevance to physiatrists, present the role of implementation science (IS) in LHSs and application of IS principles to design LHSs, illustrate current LHS research in rehabilitation, and discuss potential solutions to improve awareness and to stimulate interest in LHS research and IS among physiatrists in LHSs., (© 2024 The Authors.
