O presente estudo investiga a relação entre as práticas de controle gerencial, comportamento individual e performance organizacional, no âmbito do setor da Saúde no Brasil. Analisar relações que abarcam as características do processo orçamentário (nível organizacional), bem como os aspectos comportamentais e atitudinais dos indivíduos e o desdobramento na performance orçamentária, situando estes gestores clínicos, que atuam neste processo, em termos de formação, posicionamento na estrutura organizacional, perfil, status e engajamento organizacional, constituem-se uma oportunidade de pesquisa, buscando-se preencher esta lacuna de pesquisa, no campo de investigação da Contabilidade Gerencial, de forma mais específica do fenômeno do orçamento, como prática de controle gerencial nas organizações hospitalares no Brasil. O estudo foi desenvolvido em duas partes, sendo que na primeira, um artigo desenvolvido sob a estratégia de métodos mistos abarcará as relações das variáveis sob o nível individual (motivação - comprometimento orçamentário) com as organizacionais (feedback orçamentário), baseadas no cenário do tipo de hospital (dado pela fonte de financiamento predominante), buscando evidências de como estas variáveis podem afetar o comportamento individual e explicar o atingimento da performance orçamentária. Utilizou-se análise estatística do questionário de acordo com a Modelagem de Equações Estruturais e análise de entrevistas com os clinical managers e gestores administrativos, cujos resultados mostraram que a participação orçamentária tem efeito direto no comprometimento com a meta orçamentária, porém este não afeta diretamente a performance. Quando testada a relação de intermediação do feedback orçamentário, esta também não foi suportada, mas apresentou um efeito parcial. A moderação do tipo de hospital sob a relação feedback e performance orçamentária foi confirmada e, quando o tipo de hospital é atribuído ao setor público, este afeta a performance, confirmando-se então o modelo proposto no cenário dos hospitais públicos. Na segunda parte, um artigo qualitativo traçou o perfil deste gestor clínico no contexto brasileiro dos hospitais públicos e privados, a partir da análise das entrevistas conduzidas com os clinical managers, elucidando questões atreladas à sua atuação no desenvolvimento das atividades gerenciais. Os resultados apontaram que, apesar de serem considerados profissionais híbridos, carecem de preparação inicial quando assumem a posição gerencial, sendo esta adquirida ao longo de sua trajetória, motivando assim a reflexão em relação a aspectos quanto à identidade profissional, formação educacional e recursos humanos, além de questões de cunho social e antropológico. A presente tese contribuiu para o entendimento do fenômeno do processo orçamentário, clarificando como se dá a participação orçamentária dos clinical managers, cujo alvo é a performance orçamentária no contexto dos hospitais públicos e privados no Brasil. The present study investigates the relationship between managerial control practices, individual behavior, and organizational performance within the Health sector in Brazil. To analyze relations that encompass the characteristics of the budgeting process (organizational level), as well as the behavioral and attitudinal aspects of individuals and the unfolding in the budgeting performance, placing these clinical managers, who act in this process, in terms of education, position in the organizational structure, profile, status and organizational engagement, constitute a research opportunity, seeking to fill this research gap, in the field of Management Accounting research, more specifically in the phenomenon of the budget, as a practice of management control in hospital organizations in Brazil. The study was developed in two parts, and in the first one, an article developed under the mixed methods strategy will cover the relations of variables under the individual level (motivation - budget commitment) with the organizational ones (budget feedback), based on the scenario of the type of hospital (given by the predominant financing source), seeking evidence of how these variables can affect the individual behavior and explain the achievement of the budget performance. We used statistical analysis of the questionnaire according to Structural Equation Modeling and analysis of interviews with clinical managers and administrative managers, whose results showed that budget participation has a direct effect on the commitment to the budget target, but this does not directly affect performance. When tested for the mediating relationship of budget feedback, this was also not supported, but showed a partial effect. The moderation of the type of hospital under the feedback and budget performance relationship was confirmed, and when the type of hospital is assigned to the public sector, it affects performance, thus confirming the proposed model in the public hospital setting. In the second part, a qualitative article outlined the profile of this clinical manager in the Brazilian context of public and private hospitals, based on the analysis of interviews conducted with clinical managers, elucidating issues related to their performance in the development of managerial activities. The results pointed out that, although they are considered hybrid professionals, they lack initial preparation when they assume the managerial position, which is acquired along their trajectory, thus motivating reflection regarding aspects related to professional identity, educational background and human resources, besides issues of social and anthropological nature. The present thesis contributed to the understanding of the phenomenon of the budget process, clarifying how the budgetary participation of clinical managers takes place, whose target is the budget performance in the context of public and private hospitals in Brazil.
