The antioxidant capacity and the inflammatory potential of diet during pregnancy may represent a prevention opportunity for allergic and respiratory diseases. We aimed to investigate the associations between the antioxidant and the inflammatory potential of maternal diet in the last 3 months of pregnancy with allergic and respiratory diseases in children. Analyses were performed on 9679 mother–child pairs from the ELFE birth cohort. The dietary total antioxidant capacity (DTAC), without coffee, was estimated with the Trolox equivalent antioxidant capacity (TEAC), the total radical trapping antioxidant parameter (TRAP) and the ferric reducing-antioxidant power (FRAP). The inflammatory potential of the maternal diet was assessed by the energy-adjusted dietary inflammatory index (E-DII). Allergic and respiratory diseases in children up to 5·5 years were considered jointly through five allergic and respiratory multimorbidity clusters ('asymptomatic' - reference, 'early wheeze without asthma', 'asthma only', 'allergies without asthma' and 'multi-allergic'). Multinomial logistic regressions were performed and adjusted for main confounders. A diet with a higher antioxidant potential was associated with a lower risk of belonging to the 'early wheeze without asthma' cluster (aOR (95 % CI) = 0·95 (0·90, 0·99) per sd of TEAC score). A higher E-DII was associated with a higher risk of belonging to the 'asthma only' cluster (aOR (95 % CI) = 1·09 (1·00, 1·19) per sd). No association was found with the 'allergies without asthma' or 'multi-allergic' clusters. An antioxidant-rich diet during pregnancy was associated with better respiratory health, while a pro-inflammatory diet was associated with poorer respiratory health in children up to 5·5 years, though the associations were weak. Identification and specific quantification of isomers in a complex biological matrix by mass spectrometry alone is not an easy task due to their identical chemical formula and therefore their same mass‐to‐charge ratio (m/z). Here, the potential of direct introduction combined with ion mobility–mass spectrometry (DI‐IM‐MS) for rapid quantification of isomers as human milk oligosaccharides (HMOs) was investigated. Differences in HMO profiles between various analyzed breast milk samples were highlighted using the single ion mobility monitoring (SIM2) acquisition for high ion mobility resolution detection. Furthermore, the Se+ (secretor) or Se− (non‐secretor) phenotype could be assigned to breast milk samples studied based on their HMO contents, especially on the response of 2′‐fucosyllactose (2'‐FL) and lacto‐N‐fucopentaose I (LNFP I). The possibility of quantifying a specific isomer in breast milk by DI‐IM‐MS was also investigated. The standard addition method allowed the determination of the 2'‐FL despite the presence of other oligosaccharides, including 3‐fucosyllactose (3‐FL) isomer in breast milk. Background: Food allergy (FA) is an inappropriate immunological response to food proteins resulting from an impaired induction of oral tolerance. Various early environmental factors can affect the establishment of intestinal homeostasis, predisposing to FA in early life. In this context, we aimed to assess the effect of chronic perinatal exposure to food‐grade titanium dioxide (fg‐TiO2), a common food additive. Methods: Dams were fed a control versus fg‐TiO2‐enriched diet from preconception to weaning, and their progeny received the same diet at weaning. A comprehensive analysis of baseline intestinal and systemic homeostasis was performed in offspring 1 week after weaning by assessing gut barrier maturation and microbiota composition, and local and systemic immune system and metabolome. The effect of fg‐TiO2 on the susceptibility of progeny to develop oral tolerance versus FA to cow's milk proteins (CMP) was performed starting at the same baseline time‐point, using established models. Sensitization to CMP was investigated by measuring β‐lactoglobulin and casein‐specific IgG1 and IgE antibodies, and elicitation of the allergic reaction by measuring mouse mast cell protease (mMCP1) in plasma collected after an oral food challenge. Results: Perinatal exposure to fg‐TiO2 at realistic human doses led to an increased propensity to develop FA and an impaired induction of oral tolerance only in young males, which could be related to global baseline alterations in intestinal barrier, gut microbiota composition, local and systemic immunity, and metabolism. Conclusions: Long‐term perinatal exposure to fg‐TiO2 alters intestinal homeostasis establishment and predisposes to food allergy, with a clear gender effect. [ABSTRACT FROM AUTHOR]

Mandatory labelling of allergenic food ingredients has helped allergic consumer manage their condition, but unintended allergens and precautionary allergen labels (PAL) continue to cause confusion for allergic consumers and the food industry alike. Identifying doses of food protein that are safe for the majority of allergic consumers and test methods for their analysis are essential for evidence‐based application of allergen labelling. The ThRAll project addressed this by developing a multiplex prototype mass spectrometry (MS)‐based reference method capable of analyzing six allergenic foods (cow's milk, hen's egg, peanut, hazelnut, almond and soybean) with complementary assessment of immunoassay and DNA‐based methods. The MS method was transferable between laboratories and has the sensitivity required to quantify the allergens from egg, milk, almond and hazelnut in chocolate, meeting test method performance requirements identified for these allergenic foods by the recent FAO‐WHO expert consultation. Further refinement is needed to improved sensitivity for peanut and soy. In parallel an approach for harmonizing and integrating oral food challenge data in allergic subjects was developed and data collection piloted using an on‐line database. Data gaps were identified for many allergenic foods and there is an urgent need to confirm the allergenic activity of highly processed food matrices. [ABSTRACT FROM AUTHOR]

We investigated the associations between maternal diet quality and allergic and respiratory diseases in children. Analyses were based on 1316 mother–child pairs from the EDEN mother–child cohort. Maternal diet quality during pregnancy was assessed through a food-based score (the Diet Quality), a nutrient-based score (the PANDiet), and the adherence to guidelines for main food groups. Clusters of allergic and respiratory multimorbidity clusters up to 8 years were identified using Latent Class Analysis. Associations were assessed by adjusted multinomial logistic regressions. Four clusters were identified for children: "asymptomatic" (67%, reference group), "asthma only" (14%), "allergies without asthma" (12%), "multi-allergic" (7%). These clusters were not associated with mother diet quality assessed by both scores. Children from mothers consuming legumes once a month or less were at higher risk of belonging to the "multi-allergic" cluster (odds ratio (OR) (95% confidence interval (95%CI)) = 1.60 (1.01;2.54)). No association was found with other food groups or other clusters. In our study, allergic and respiratory multimorbidity in children was described with four distinct clusters. Our results suggest an interest in legumes consumption in the prevention of allergic diseases but need to be confirmed in larger cohorts and randomized control trials. [ABSTRACT FROM AUTHOR]

Background: The new European regulations require the enrichment of formulas with docosahexaenoic acid (DHA) because of the positive effects of long‐chain polyunsaturated fatty acids (LCPUFAs) on neurodevelopment and visual acuity. In this observational study, we aimed to evaluate whether the consumption of LCPUFA‐enriched formula was associated with the risk of infection and allergy in early childhood. Methods: Analyses involved data from 8389 formula‐fed infants from the ELFE birth cohort. Formula enrichment was identified from the list of ingredients of the formula consumed at 2 months. Infections (gastrointestinal, lower respiratory tract [LRTI], upper respiratory tract) and allergies (wheezing, itchy rash, asthma medication, food allergy) from age 2 months to 5.5 years were reported by parents during follow‐up surveys. Multivariable logistic regression models were used to assess associations between the consumption of LCPUFA‐enriched formula and the risk of infection and allergy. Results: Among formula‐fed infants at 2 months, 36% consumed formula enriched with DHA and arachidonic acid (ARA), and 11% consumed formula additionally enriched with eicosapentaenoic acid (EPA). Enriched formula consumption was not associated with infection or allergy, except for an association between consumption of DHA/ARA/EPA‐enriched formula and lower use of asthma medications. Furthermore, as compared with non‐DHA/ARA/EPA‐enriched formula, consumption of formula with high EPA content (≥3.2 mg/100 kcal) was related to lower risk of LRTI and lower use of asthma medications. Conclusion: This study suggests that consumption of DHA/ARA/EPA‐enriched formula (especially those with high EPA content) is associated with a lower risk of LRTI and lower use of asthma medications. Exposure to environmental chemicals has been associated with higher risk of childhood allergies. This study aimed to examine the association between infant's dietary exposure to mixtures of chemicals and allergic and respiratory multimorbidity in childhood. Dietary exposures were assessed at 8 and 12 months in 724 and 745 children of the EDEN cohort. Allergic and respiratory multimorbidity clusters were identified using latent class analyses. Associations between dietary exposure to mixtures of chemicals and allergic clusters were assessed by adjusted multinomial logistic regressions. At 8 months, higher exposure to a mixture of furans, trace elements, dioxins and PAHs was positively associated with the "asthma only" cluster, while moderate exposure to a mixture of PAHs, pesticides, PCBs and acrylamide was negatively associated with this cluster. A mixture of PCBs and BFRs was positively associated with the "multi-morbidity" cluster. Exposure to a mixture of pesticides and trace elements was positively associated with the "allergy without asthma" cluster. At 12-months, higher exposure to a mixture of trace elements and pesticides was positively associated with "multi-morbidity" cluster.
