Background: Onychomycosis is the commonly encountered problem especially in adults. Appears as a simple cosmetic problem infected nails, but have significant effect on patients emotional, occupational and social aspects, also serve as reservoir of mycotic infection and can give rise to repeated skin infections. Dermatophytes are the principal pathogens. Objectives: Isolation and identification of fungal agents causing onychomycosis and to study the predisposing factors with their clinical presentations. Methods: Appropriate samples from 50 patients with a diagnosis of onychomycosis were included in study for a duration of one year, from July 2012 to August-2013. Nails were collected in sterile black craft paper envelopes of 5x5 cms after disinfection and nail borings from nail bed. These were subjected to 20 to 40% KOH mount, routine Culture on SDA with antibiotics and cycloheximide. Isolates were identified based on colony morphology and biochemical reactions. Results: In a sample size of 50, dermatophytes were predominant agents 70.58% compared to non-dermatophytes 29.4%, 78% were males and 22% were females. In these cases distal and lateral sububgual onychomycosis (60%) was the most common clinical presentation followed by proximal subungual onychomycosis (20%). Total positive culture cases were 17 (34%), among which Trichophyton rubrum 41.17% was predominant followed by T.mentagrophytes 17.6%. Conclusion: Not all nail dystrophies are fungal in origin, an accurate laboratory diagnosis of onychomycosis is must, as been shows in this study. Dermatophytes were predominant etiological agents among which T.rubrum was most common Changing. Fungal infections represent a relevant issue in agri-food and biomedical fields because they could compromise quality of food and humans' health. Natural extracts represent a safe alternative to synthetic fungicides and in the green chemistry and circular economy scenario, agro-industrial wastes and by-products offer an eco-friendly source of bioactive natural compounds. In this paper, phenolic-rich extracts from Olea europaea L. de-oiled pomace, Castanea sativa Mill. wood, Punica granatum L. peel, and Vitis vinifera L. pomace and seeds were characterized by HPLC-MS-DAD analysis. Finally, these extracts were tested as antimicrobial agents against pathogenic filamentous fungi and dermatophytes such as Aspergillus brasiliensis, Alternaria sp., Rhizopus stolonifer, and Trichophyton interdigitale. The experimental results evidenced that all extracts exhibited a significant growth inhibition for Trichophyton interdigitale. Punica granatum L., Castanea sativa Mill., and Vitis vinifera L. extracts showed a high activity against Alternaria sp. and Rhizopus stolonifer. These data are promising for the potential applications of some of these extracts as antifungal agents in the food and biomedical fields. [ABSTRACT FROM AUTHOR]

Dermatophytosis, an infectious disease caused by several fungi, can affect the hair, nails, and/or superficial layers of the skin and is of global significance. The most common dermatophytes in cats and dogs are Microsporum canis and Trichophyton mentagrophytes. Wood's lamp examination, microscopic identification, and fungal culture are the conventional clinical diagnostic methods, while PCR (Polymerase Chain Reaction) and qPCR (Quantitative PCR) are playing an increasingly important role in the identification of dermatophytes. However, none of these methods could be applied to point-of-care testing (POCT). The recent development of the CRISPR (Clustered Regularly Interspaced Short Palindromic Repeats) based diagnostic platform promises a rapid, accurate, and portable diagnostic tool. In this paper, we present a Cas12a-fluorescence assay to detect and differentiate the main dermatophytes in clinical samples with high specificity and sensitivity. The Cas12a-based assay was performed with a combination of recombinase polymerase amplification (RPA). The results could be directly visualized by naked eyes under blue light, and all tested samples were consistent with fungal culture and sequencing results. Compared with traditional methods, the RPA-Cas12a-fluorescence assay requires less time (about 30 min) and less complicated equipment, and the visual changes can be clearly observed with naked eyes, which is suitable for on-site clinical diagnosis. Tinea capitis is a common dermatophyte infection associated with pupils of primary school age group. Dermatophytosis is relatively common in tropical and sub-tropical regions where warm and humid climates provide a favorable environment for the fungal to thrive. Prevalence in many of these regions are unknown and shift in etiology has frequently been observed. Hence the objective of this paper was to carry out a survey of Tinea capitis among primary school pupils in Sapele and Ethiope West Local Government Areas in Delta State, Nigeria using standard procedures and estimating its prevalence on the scalp of primary school pupil. Pupils (5,876) aged 4-13 years in government-owned primary schools located in urban and rural areas of Sapele and Ethiope West Local Government Areas, were physically screened for hair-scalp fungal infection. Mycological examination revealed Microsporum audouinii (24%) as the most implicated dermatophyte in these local government areas. Boys; 190 (65%) were more infected than girls 103 (35%) and prevalence was among children between 4-7 years old (51%). Background: Dermatophytosis is commonly encountered in the dermatological clinics. The main aetiological agents in dermatophytosis of skin and nails in humans are Trichophyton (T.) rubrum, T. mentagrophytes and T. interdigitale (former T. mentagrophytes-complex). Terbinafine therapy is usually effective in eradicating infections due to these species by inhibiting their squalene epoxidase (SQLE) enzyme, but increasing numbers of clinically resistant cases and mutations in the SQLE gene have been documented recently. Resistance to antimycotics is phenotypically determined by antifungal susceptibility testing (AFST). However, AFST is not routinely performed for dermatophytes and no breakpoints classifying isolates as susceptible or resistant are available, making it difficult to interpret the clinical impact of a minimal inhibitory concentration (MIC). Summary: PubMed was systematically searched for terbinafine susceptibility testing of dermatophytes on October 20, 2020, by two individual researchers. The inclusion criteria were in vitro terbinafine susceptibility testing of Trichophyton (T.) rubrum, T. mentagrophytes and T. interdigitale with the broth microdilution technique. The exclusion criteria were non-English written papers. Outcomes were reported as MIC range, geometric mean, modal MIC and MIC50 and MIC90 in which 50 or 90% of isolates were inhibited, respectively. The reported MICs ranged from <0.001 to >64 mg/L. The huge variation in MIC is partly explained by the heterogeneity of the Trichophyton isolates, where some originated from routine specimens (wild types) whereas others came from non-responding patients with a known SQLE gene mutation. Another reason for the great variation in MIC is the use of different AFST methods where MIC values are not directly comparable. High MICs were reported particularly in isolates with SQLE gene mutation. The following SQLE alterations were reported: F397L, L393F, L393S, H440Y, F393I, F393V, F415I, F415S, F415V, S443P, A448T, L335F/A448T, S395P/A448T, L393S/A448T, Q408L/A448T, F397L/A448T, I121M/V237I and H440Y/F484Y in terbinafine-resistant isolates. We found a total of 35 monotherapeutic regimens and compared their effect in terms of cure, where "cure" is the absence of dermatophytes. The possible involvement of saprobe fungi in dermatomycoses, as well as the incidence of dermatophytes in dogs and cats with skin lesions was studied. During a period of one year 54 dogs and 47 cats were included in this study. Of the 101 samples, 34 resulted in positive cultures for dermatophytes. Dermatophyte fungi pure cultures were obtained from 12 samples. A simultaneous growth of dermatophytes plus saprobe fungi was observed in 22 of the samples. Of the remaining 67 samples, no fungal growth was observed in 15 samples and others samples only the saprobe fungi was presented. M. canis as the main dermatophyte detected in this study. The following genera of saprobe fungi were also isolated: Alternaria sp (0.99%), Rhizopus sp (2.97%), Trichoderma.

This research was carried out in the Imam Al-Hussein Center, Karbala Governorate, Iraq for Researcher Care and Manuscript Restoration to assess the effects of active ingredient extracted from the Centaurea cineraria methanolic on inhibiting the growth of the cutaneous fungus Trichophyton rubrum and Microsporum canis and determining its appropriate concentration. The plant was chemically tested in the experiment to diagnose active chemicals utilizing qualitative analysis of compounds in the plant sample using the GC-MS technique. Different treatments (concentrations) of 10, 20, 30, 40, 50 and 60% of the methanolic extract were prepared, and the results were obtained as a function of colony diameter on SDA medium along with the colony weight on SD broth medium after 14 days of incubation, and also observing the shape of fungal colonies on the culture medium. The concentrations of the methanolic extract affected the studied fungi, where T60% on Trichophyton exhibited to be the most effective, with a diameter of 9.67 mm, while T10% on Microsporum displayed the lowest effect with a diameter of 77.67 mm. As for the dry weight of the fungal colony, T60% was more effective on Trichophyton, where the average dry weight was 0.106 g, while the T10% on Microsporum exhibited the lowest effect with a dry weight rate of 0.346 g. Caffeine affords several beneficial effects on human health, acting as an antioxidant, anti-inflammatory agent, and analgesic. Caffeine is widely used in cosmetics, but its antimicrobial activity has been scarcely explored, namely against skin infection agents. Dermatophytes are the most common fungal agents of human infection, mainly of skin infections. This work describes the in vitro effect of caffeine during keratinocyte infection by Trichophyton mentagrophytes, one of the most common dermatophytes. The results show that caffeine was endowed with antidermatophytic activity with a MIC, determined following the EUCAST standards, of 8 mM. Caffeine triggered a modification of the levels of two major components of the fungal cell wall, β-(1,3)-glucan and chitin. Caffeine also disturbed the ultrastructure of the fungal cells, particularly the cell wall surface and mitochondria, and autophagic-like structures were observed. During dermatophyte–human keratinocyte interactions, caffeine prevented the loss of viability of keratinocytes and delayed spore germination. Overall, this indicates that caffeine can act as a therapeutic and prophylactic agent for dermatophytosis. [ABSTRACT FROM AUTHOR]

Skin disorders among individuals receiving palliative care may be associated with the primary condition or underlying comorbidities and patients may experience pruritus, discomfort or pain. Common conditions include xerosis, pressure ulcers, intertrigo, superficial fungal infections, telogen effluvium, pruritus, herpes zoster, eczematous disorders and edema. During end-of-life care, there is reduced skin perfusion and metabolism hence leading to susceptibility to infection, pressure and injury. Other factors affecting the skin include limited mobility, nutritional deficits and immunosuppression. Although treatment strategies for each skin condition are usually aligned with standard protocols, considerations among these patients include limited life-expectancies, potential treatment burden, drug-drug interactions as well as comfort-directed rather than cure-directed therapy. For patients with xerosis cutis, the regular use of moisturisers is recommended. The management and prevention of pressure ulcers include the strategies of skin assessment and care, pressure redistribution, nutrition and hydration and ulcer care. Superficial fungal infections require treatment with appropriate topical and/or systemic antifungals while antivirals and adjunctive treatment can be prescribed for herpes zoster. Onychomycoses are nail infections that require prolonged therapy and have high recurrence rates. Dermatophytes are the main etiological agents of these infections, followed by yeasts and non-dermatophyte filamentous fungi. The limited antifungal arsenal used to treat onychomycosis and the change in the susceptibility profile of these agents contribute to the chronicity and recalcitrant profile of infections. The present study aimed to determine the antifungal activity of extracts obtained from pure and mixed cultures of Candida parapsilosis, Trichophyton mentagrophytes, and Trichophyton rubrum. Additionally, in vivo toxicity tests with Galleria mellonella and time-kill assays were carried out. The susceptibility profiles of dermatophytes were determined using a microdilution technique with minimum inhibitory concentrations (MICs) between 250 and 8000 µg/mL. The time-kill assay, compared to growth control, resulted in the death of dermatophytes within 48 h. No toxicity of the extracts was detected in experiments with Galleria mellonella larvae under the test conditions. The extracts of pure and mixed cultures of Candida parapsilosis and dermatophytes present antifungal activity against T. mentagrophytes and T. rubrum. Background: Tinea capitis (T. capitis), commonly known as scalp ringworm, is a fungal infection affecting the scalp and hair. Among the causative agents, Microsporum canis (M. canis) stands out, often transmitted from cats to humans (zoonotic disease). In this study, we investigated the efficacy of Carica papaya (C. papaya), fruit extract against dermatophytes, particularly M. canis, both in vitro and in vivo. Additionally, we aimed to identify the active compounds responsible for suppressing fungal growth and assess the toxicity of C. papaya on human cells. Methodology: It conducted in two parts. First, In Vitro Study include the preparation of C. papaya fruit extract using methanol as the solvent, Phytochemical analysis of the plant extract including Gas chromatography-mass spectrometry (GC-MS) and Fourier-transform infrared spectroscopy (FTIR) was conducted, Cytotoxicity assays were performed using HUH-7 cells, employing the MTT assay (1-(3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide), Antimicrobial activity against M. canis was evaluated, including: Zone of inhibition (ZI), Minimum inhibitory concentration (MIC), Minimum fungicidal concentration (MFC), M. canis cell alterations were observed using scanning electron microscopy (SEM) and transmission electron microscopy (TEM). Second, In Vivo, Albino Wistar male rats were included. Results: The phytochemical analysis of the methanolic extract from papaya revealed several functional groups, including hydroxyl, ammonia, alkane, carbonate, and alcohol. Additionally, the GC-MS analysis identified 15 compounds, with xanthosine and decanoic acid being the predominant components. The methanolic extract of papaya fruits demonstrated potent antifungal activity: ZI = 37 mm, MIC = 1,000 μg/mL, MFC = 1900 μg/mL, MTT results indicated lower cytotoxicity of the fruit extract at concentrations of 20 μg/mL, 50 μg/mL, 100 μg/mL, 150 μg/mL, and 200 μg/mL, The IC50 revealed a significant decrease in cell viability with increasing extract concentration. Notably, papaya extract induced considerable alterations in the morphology of M. canis hyphae and spores. In animal tissue, improvements were observed among the group of rats which treated with Papaya extract. This study highlights the potential of C. papaya fruits as a natural antifungal agent, warranting further exploration for clinical applications. [ABSTRACT FROM AUTHOR]

Dermatophytes are the most common cause of fungal infections worldwide, affecting millions of people annually. The emergence of resistance among dermatophytes along with the availability of antifungal susceptibility procedures suitable for testing antifungal agents against this group of fungi make the combinatorial approach particularly interesting to be investigated. Therefore, we reviewed the scientific literature concerning the antifungal combinations against dermatophytes. A literature search on the subject performed in PubMed yielded 68 publications: 37 articles referring to in vitro studies and 31 articles referring to case reports or clinical studies. In vitro studies involved over 400 clinical isolates of dermatophytes (69% Trichophyton spp., 29% Microsporum spp., and 2% Epidermophyton floccosum). Combinations included two antifungal agents or an antifungal agent plus another chemical compound including plant extracts or essential oils, calcineurin inhibitors, peptides, disinfectant agents, and others. In general, drug combinations yielded variable results spanning from synergism to indifference. Antagonism was rarely seen. In over 700 patients with documented dermatophyte infections, an antifungal combination approach could be evaluated. The most frequent combination included a systemic antifungal agent administered orally (i.e., terbinafine, griseofulvin, or azole—mainly itraconazole) plus a topical medication (i.e., azole, terbinafine, ciclopirox, amorolfine) for several weeks. Clinical results indicate that association of antifungal agents is effective, and it might be useful to accelerate the clinical and microbiological healing of a superficial infection. Antifungal combinations in dermatophytes have gained considerable scientific interest over the years and, in consideration of the interesting results available so far, it is desirable to continue the research in this field. Atopic dermatitis, or eczema, is the most common chronic skin disorder, characterized by red and pruritic lesions. Its etiology is multifaceted, involving an interplay of factors, such as the allergic immune response, skin barrier dysfunction, and dysbiosis of the skin microbiota. Recent studies have explored the role of extracellular vesicles (EVs), which are lipid bilayer-delimitated particles released by all cells, in atopic dermatitis. Examination of the available literature identified that most studies investigated EVs released by Staphylococcus aureus, which were found to impact the skin barrier and promote the release of cytokines that contribute to atopic dermatitis development. In addition, EVs released by the skin fungus, Malassezia sympodialis, were found to contain allergens, suggesting a potential contribution to allergic sensitization via the skin. The final major finding was the role of EVs released by mast cells, which were capable of activating various immune cells and attenuating the allergic response. While research in this area is still in its infancy, the studies examined in this review provide encouraging insights into how EVs released from a variety of cells play a role in both contributing to and protecting against atopic dermatitis. Purpose of Review: Onychomycosis is commonly reported in Africa, but data on its prevalence and spectrum of incriminated pathogens is fragmented in the literature. Understanding its epidemiology and profiling of the incriminated fungal pathogens would be very useful in managing patients as they are often difficult to treat, especially with the global emergence of antifungal resistance. Recent Findings: We identified a total of 44 well-documented studies in 13 (24.1%) of the 54 African countries amounting to a total of 6773 cases of nail fungal infections: 4609 (68.0%) from North Africa, 1338 (19.6%) from West Africa, 524 (7.7%) from East Africa, 243 (3.6%) from Central Africa, and 59 (0.9%) from Southern Africa, with a pooled prevalence of 19.6% (6773/34604). Identification of fungal pathogens was mainly by conventional methods: microscopy (n = 43, 97.7%), culture (n = 42, 95.5%), and periodic acid-Schiff staining (n = 1, 2.3%). Advanced diagnostics with improved sensitivity were also deployed in some studies: PCR (n = 2, 4.5%), sequencing (n = 2, 4.5%), and matrix-assisted laser desorption/ionization time of flight mass spectrometry (n = 1, 2.3%). The most frequent fungal pathogens identified in North and Central Africa were Trichophyton rubrum (80.4% and 49.3%), followed by Candida albicans (12.2% and 27.4%), respectively, while in the West and East African regions, it was Candida albicans (63% and 47.3%) followed by Trichophyton rubrum (19.1% and 18.9%), respectively. Antifungal susceptibility testing was performed in only five studies and showed varied outcomes with high resistance of dermatophytes to ketoconazole, itraconazole, and fluconazole and non-dermatophyte moulds to caspofungin and 5-flucytosine, respectively. Summary: Onychomycoses are frequent but of less cognizance as data was found from only 13 of the 54 African countries. The emergence of drug-resistant onychomycosis is of concern and informs the need to prioritize research in this area to improve diagnostics and the availability of antifungal susceptibility testing. Dermatophytosis is a contagious superficial mycosis caused by a group related fungi in the Genera Trichophyton, Microsporum and Epidermophyton. Early and accurate identification of the causative agent of dermatophytosis is essential for effective treatment and prevention of the disease. Genotypic techniques are more rapid, sensitive and specific than the conventional methods. However, there is a paucity of information on molecular characterization of dermatophytes from domesticated ruminants, particularly in northern Nigeria. The aim of this work was to carry out molecular identification of dermatophytes isolated from sheep and goats in Zaria. Ninety-four goats and 63 sheep skin scrapings were aseptically collected from suspected cases of deramtophytosis. Each specimen was processed for direct microscopic examination and, cultural isolation and phenotypic identification of etiologic agents. The identified dermatophyte isolates were subjected to DNA extraction followed by amplification of the internal transcribed spacer (ITS) regions of their ribosomal DNA. The dermatophytes were identified by comparing the sizes of the amplified ITS regions of each isolate with ITS regions sizes of dermatophytes reported in the literature. Out of the 34 dermatophyte isolates from both sheep and goats, 23 were phenotypically identified as Trichophyton mentagrophytes while 11 were T. verrucosum. The PCR based technique identified 23 of the isolates as T. mentagrophytes while 11 were identified as T. verrucosum in complete agreement with the culture method. The sizes of the ITS regions of all the T. mentagrophytes and T. verrucosum were approximately 600bp except one isolate of T. mentagrophytes which had ITS region size of approximately 550bp. Trichophyton verrucosum and T. mentagrophytes isolated from sheep and goats were successfully identified by PCR method targeting the ITS regions of their ribosomal DNA. Nowadays, dermatophyte infections are relatively easy to cure, especially since the introduction of orally administered antifungals such as terbinafine and itraconazole. However, these drugs may cause side effects due to liver damage or their interactions with other therapeutics. Hence, the search for new effective chemotherapeutics showing antidermatophyte activity seems to be the urge of the moment. Potassium salts of N-acylhydrazinecarbodithioates are used commonly as precursors for the synthesis of biologically active compounds. Keeping that in mind, the activity of a series of five potassium N-acylhydrazinecarbodithioates (1a–e) and their aminotriazole-thione derivatives (2a–e) was evaluated against a set of pathogenic, keratinolytic fungi, such as Trichophyton ssp., Microsporum ssp. and Chrysosporium keratinophilum, but also against some Gram-positive and Gram-negative bacteria. All tested compounds were found non-toxic for L-929 and HeLa cells, with the IC30 and IC50 values assessed in the MTT assay above 128 mg/L. The compound 5-amino-3-(naphtalene-1-yl)-4,5-dihydro-1H-1,2,4-triazole-5-thione (2d) was found active against all fungal strains tested. Scanning Electron Microscopy (SEM) revealed inhibition of mycelium development of Trichophyton rubrum cultivated on nail fragments and treated with 2d 24 h after infection with fungal spores. Transmission Electron Microscopy (TEM) observation of mycelium treated with 2d showed ultrastructural changes in the morphology of germinated spores. Finally, the RNA-seq analysis indicated that a broad spectrum of genes responded to stress induced by the 2d compound. In conclusion, the results confirm the potential of N-acylhydrazinecarbodithioate derivatives for future use as promising leads for new antidermatophyte agents development. [ABSTRACT FROM AUTHOR]

Stable microbial colonization of the skin depends on tight control by the host immune system. The lipid-dependent yeast Malassezia typically colonizes skin as a harmless commensal and is subject to host type 17 immunosurveillance, but this fungus has also been associated with diverse skin pathologies in both humans and animals. Using a murine model of Malassezia exposure, we show that Vγ4+ dermal γδ T cells expand rapidly and are the major source of IL-17A mediating fungal control in colonized skin. A pool of memory-like Malassezia-responsive Vγ4+ T cells persisted in the skin, were enriched in draining lymph nodes even after fungal clearance, and were protective upon fungal re-exposure up to several weeks later. Induction of γδT17 immunity depended on IL-23 and IL-1 family cytokine signalling, whereas Toll-like and C-type lectin receptors were dispensable. Furthermore, Vγ4+ T cells from Malassezia-exposed hosts were able to respond directly and selectively to Malassezia-derived ligands, independently of antigen-presenting host cells. The fungal moieties detected were shared across diverse species of the Malassezia genus, but not conserved in other Basidiomycota or Ascomycota. These data provide novel mechanistic insight into the induction and maintenance of type 17 immunosurveillance of skin commensal colonization that has significant implications for cutaneous health. Author summary: Malassezia is the most abundant fungus living on our skin and is usually harmless, but this microbe has also been shown to play a role in pathological conditions such as eczema and dermatitis. Here, we investigated how a population of Vγ4+ γδ T cells protect mouse skin against fungal overgrowth. While generally considered part of the innate immune system, we found that γδ T cells were maintained long after Malassezia was cleared from the skin of experimentally-infected animals. These cells displayed memory-like features and were highly efficient at fighting the fungus after a secondary challenge. We observed that classic fungal pattern recognition receptors were not involved, and that antigen-presenting cells were not required to generate Malassezia-protective γδ T cells. Finally, we confirmed that murine γδ T cells can directly recognise Malassezia and that the Malassezia-derived structure that triggers these responses was not conserved among other fungi. Our results highlight for the first time an important role for γδ T cells in preventing uncontrolled growth of the most abundant skin fungus. Dermatophytes are a group of fungi that cause dermatophytosis, a type of skin infection affecting both humans and animals. While dermatophytosis is typically self-limiting, it can pose significant challenges for individuals with weakened immune systems. Additionally, current treatment options for dermatophytosis have their limitations, including issues of toxicity, inefficacy, and the emergence of drugresistant strains. Recently, nanotechnology-based approaches have emerged as a promising strategy to overcome the drawbacks of conventional antifungal medications. Gold Nanoparticles (GNPs) have demonstrated substantial antifungal activity against various fungal species, including dermatophytes. Among these, Gold Nanobipyramids (GNBPs) exhibit unique physicochemical properties that enhance their effectiveness against dermatophytes. GNBPs were synthesized using the Seed-Mediated Growth Method (SMGM) due to its simplicity. Optically, GNBPs displayed plasmonic peaks at 1.548 (wavelength 561 nm) and 2.372 (wavelength 807 nm). Structurally, intensity peaks at 38.15° (plane 111) and 44.49° (plane 200) were identified. GNBPs have a surface density of ~70.227±0.530. In the dermatophyte treatment process, GNBPs exhibited varying inhibitory effects. At 40% concentration, fungal growth persisted, with a surface density similar to the no control area. At 60%, inhibition occurred, though less efficiently. The most effective inhibition was observed at 80%, and complete inhibition was achieved at 100% concentration. GNBPs proved successful in inhibiting fungal growth, attributed to their curvature and sharp edges interacting with microbial cell membranes, leading to membrane damage and cellular content leakage. This study used to be conducted in the Graduate Studies Laboratory at the College of Science, Kirkuk University in Iraq from 11/7/2021 to 25/5/2022 with the aim of investigating the fungi causing motive pores and skin ailments, i.e., dermatomycosis in the human physique (tinea corporis) and the incidence of Trichophyton mentagrophytes. A total of 120 samples were collected from sufferers referred to the Dermatology Consultant at Azadi Teaching Hospital and some non-public clinics in Kirkuk, age groups ranging from 1 to 60 years and both genders. All samples were examined and recognized using traditional methods and using culture media for the fungi. The direct microscopic examination of the fungi showed positive results, with an infection rate of 66.66%, while the laboratory culture showed positive results, with 49.16%. The results of the phenotypic examinations of the isolated dermatophytes showed that they belong to the two genera, Trichophyton and Microsporum. The results showed that Trichophyton was the most diagnosed specimen compared to Microsporum. The least patient specimen was diagnosed, and 5 species of Trichophyton were isolated, T. mentagrophytes exhibited the highest prevalence (9.2%), while the lowest belonged to T. interdigitale (2.5%). In the case of Microsporum, two species were isolated, so that, the infection rate of males (33.33%) was higher than that of females (15.83%). The result showed the highest infection rate of T. mentagrophytes in the age group 21-30 years, while the lowest among the group 50-61. After final identification of T. mentagrophytes by PCR, the consequences of the PCR assay were sent to the Genebank website to verify the kind of fungus through comparing it with the fungi registered at this site. Dermatophytosis is a prevalent infection in tropical and subtropical countries, including India. This study aims to investigate the epidemiology and clinical aspects of chronic and recurrent dermatophytosis, identify the clinical isolates, and assess the effectiveness of different microscopy and staining methods for diagnosis. The objective of the study is to study the epidemiology and clinical aspects of chronic and recurrent dermatophytosis and to identify the clinical isolates. Attempts to improve the diagnostic outcome by implementing different microscopy and staining methods have also been performed. Adult patients with chronic and recurrent cases and positive direct microscopy were included, and clinical details were recorded. The mycological culture was performed. Fifteen isolates were selected randomly and subjected to 0.9% NaCl, Chicago sky blue staining, Calcofluor white staining, and Congo red staining in addition to the standard lactophenol cotton blue (LPCB) preparation. Among the 178 patients in the study, females (56.7%) and patients aged 25-45 (50%) were more frequently affected. Tinea corporis was the most common clinical type (89.32%). Recurrent cases (56.1%) were more prevalent than chronic cases (43.9%). Culture positivity was seen in 60.1%, with Trichophyton mentagrophytes being the most common isolate (86%). Among the staining methods used on 15 selected isolates, Lactophenol Cotton Blue (LPCB) was scored as the most effective, scoring 2.6 out of 3. The study revealed several clinical and epidemiological findings related to dermatophytosis in India, including a high degree of communicability, inadvertent therapies including topical steroid misuse, and a persistent nature requiring an extended duration of therapy. Trichophyton mentagrophytes was the most frequently isolated pathogen. Of the staining methods evaluated, Lactophenol Cotton Blue (LPCB) was found to be the most effective. Phytochemical and antifungal properties of anara leaf on dermatophytes were investigated. Anara leaves were purchased from Orie Obibi market, Obibiezena, Owerri-West, L.G.A, Imo State while samples from clippers were collected using swab sticks from barbing shops. Standard microbiological methods were adopted in the isolation of dermatophytes from the samples. Anara leaves were dried, ground, and extracted using ethanol, cold and hot water. Qualitative phytochemical screening was adopted in the determination of the phytochemical constituents of the leaf extracts. Agar well diffusion technique was adopted in the determination of the antifungal properties of the extracts. Tube dilution technique was adopted in the determination of the minimum inhibitory concentration of the extracts. Dermatophytes isolated from the clippers were Trichophyton and Epidermophyton species. The presence of saponins, tannins, phenols, alkaloids and anthraquinones were detected. Antifungal activities of the extracts showed zones of inhibition 12mm to 14m m with hot water extract, 16mm to 20mm with ethanol extract and none with cold water extract. Minimum inhibitory concentration was recorded at 250mg/ml and 500mg/ml with ethanol extract. Minimum fungicidal concentration was recorded at 500mg/ml with ethanol extract against Epidermophyton species. Cold and hot water extracts showed no fungicidal effect against the dermatophytes. The antifungal properties of the extract could be attributed to the phytochemical constituents of the leaf. For the successful treatment of dermatophytoses, especially tinea capitis, there is a need for accurate and rapid diagnostic methods. A lot of recent literature has focused on the detection of dermatophytes directly on sample material such as nails, hair and skin scrapings. Molecular tools offer the ability to rapidly diagnose dermatophytosis within 48 h. This study aimed to compare the results of a commercial real-time PCR (real-time PCR) assay DermaGenius®(DG) 2.0 complete multiplex kit with those of conventional diagnostic methods (direct microscopy and culture). A total of 129 hair samples were collected in Dakar (Senegal) from patients suspected of dermatophytosis. DG was applied for the molecular detection of Candida albicans, Trichophyton rubrum/soudanense, T. interdigitale, T. tonsurans, T. mentagrophytes, T. violaceum, Microsporum canis, M. audouinii, Epidermophyton floccosum, T. benhamiae and T. verrucosum. Dermatophytes species and C. albicans were differentiated by melting curve analysis. The sensitivity and specificity of the PCR assay were 89.3% and 75.3%, respectively. DG PCR was significantly more sensitive than culture (p < 0.001). DG PCR is fast and robust to contamination. In this paper, the main questions discussed were the replacement of culture by a broad-spectrum fungal real-time PCR and the implementation of DG PCR into a routine laboratory in Senegal. Dermatophytoses are superficial infections of human and animal keratinized tissues caused by filamentous fungi named dermatophytes. Because of a high and increasing incidence, as well as the emergence of antifungal resistance, a better understanding of mechanisms involved in adhesion and invasion by dermatophytes is required for the further development of new therapeutic strategies. In the last years, several in vitro and in vivo models have emerged to study dermatophytosis pathogenesis. However, the procedures used for the growth of fungi are quite different, leading to a highly variable composition of inoculum for these models (microconidia, arthroconidia, hyphae), thus rendering difficult the global interpretation of observations. We hereby optimized growth conditions, including medium, temperature, atmosphere, and duration of culture, to improve the sporulation and viability and to favour the production of arthroconidia of several dermatophyte species, including Trichophyton rubrum and Trichophyton benhamiae. The resulting suspensions were then used as inoculum to infect reconstructed human epidermis in order to validate their ability to adhere to and to invade host tissues. By this way, this paper provides recommendations for dermatophytes culture and paves the way towards a standardized procedure for the production of infective spores usable in in vitro and in vivo experimental models. Tinea capitis is a common dermatophyte infection associated with pupils of primary school age group. Dermatophytosis is relatively common in tropical and sub-tropical regions where warm and humid climates provide a favorable environment for the fungal to thrive. Prevalence in many of these regions are unknown and shift in etiology has frequently been observed. Hence the objective of this paper was to carry out a survey of Tinea capitis among primary school pupils in Sapele and Ethiope West Local Government Areas in Delta State, Nigeria using standard procedures and estimating its prevalence on the scalp of primary school pupil. Pupils (5,876) aged 4-13 years in government-owned primary schools located in urban and rural areas of Sapele and Ethiope West Local Government Areas, were physically screened for hair-scalp fungal infection. Mycological examination revealed Microsporum audouinii (24%) as the most implicated dermatophyte in these local government areas. Boys; 190 (65%) were more infected than girls 103 (35%) and prevalence was among children between 4-7 years old (51%). Thus male pupil are affected by Microsporum audouinii in these local government areas under study.

There is an epidemic emergence of increased resistance in dermatophytes with to antifungal drugs with ergosterol1 (Erg1) and Erg11 mutations to terbinafine and azoles. Apart from mutations, mechanisms that predict clinical failure include efflux pumps, cellular kinases, heat shock proteins (Hsp), and biofilms. Apart from itraconazole and SUBATM (Super-Bioavailable) itraconazole, measures that can be used in terbinafine failure include efflux-pump inhibitors, Hsp inhibitors and judicious use of antifungal drugs (topical + systemic) combinations. A PubMed search was done for the relevant studies and reviews published in the last 22 years using keywords dermatophytes OR Trichophyton, anti-fungal, resistance, mechanism and fungal AND resistance mechanisms. Our aim was to look for literature on prevalent species and we specifically researched studies on Trichophyton genus. We have analyzed varied antifungal drug mechanisms and detailed varied experimental and approved drugs to treat recalcitrant dermatophytosis. Apart from administering drugs with low minimum inhibitory concentration, combinations of oral and topical antifungals (based on synergy data) and new formulations of existing drugs are useful in recalcitrant cases. There is a need for research into resistance mechanism of the existent Trichophyton strains in therapeutic failures in tinea corporis & cruris instead of data derived from laboratory strains which may not mirror clinical failures. Dermatophytes are group of filamentous fungi which have adapted to living on the skin of humans and other animals. In the last decade, reports have emerged from Asia of new dermatophyte strains showing resistance to the commonly used antifungal agent terbinafine and others. The spread of these resistant strains has been noted in many other countries globally. Little is known about the mechanisms or management of this emerging problem. Cutaneous fungal infection of the skin and nails poses a significant global public health challenge. Dermatophyte infection, mainly caused by Trichophyton spp., is the primary pathogenic agent responsible for skin, hair, and nail infections worldwide. The epidemiology of these infections varies depending on the geographic location and specific population. However, epidemiological pattern changes have occurred over the past decade. The widespread availability of antimicrobials has led to an increased risk of promoting resistant strains through inappropriate treatment. The escalating prevalence of resistant Trichophyton spp. infections in the past decade has raised serious healthcare concerns on a global scale. Non-dermatophyte infections, on the other hand, present even greater challenges in terms of treatment due to the high failure rate of antifungal therapy. These organisms primarily target the nails, feet, and hands. The diagnosis of cutaneous fungal infections relies on clinical presentation, laboratory investigations, and other ancillary tools available in an outpatient care setting. This review aims to present an updated and comprehensive analysis of the epidemiology, clinical manifestations, and diagnostic testing methods for cutaneous fungal infections caused by dermatophytes and non-dermatophytes. Onychomycosis is commonly studied in Africa but not in patients with diabetics despite having a significant number of her population living with diabetes mellitus (DM). Our review highlights a total of 15 studies with only two from Africa over the past two decades; 8 (53.3%) from Asia, 4 (26.7%) from Europe, 2 (13.3%) from Africa and 1 (6.7%) from North America. A total number of 4321 participants were involved with onychomycosis prevalence of 35.3% (1527/4321). Seven studies documented preponderance of onychomycosis in males, one showed preponderance in females, one showed no statistically significant difference in gender, while correlation with gender was unclear in the remainder. The risk factors identified were duration of diabetes, increasing age, occupation (agriculture), subclinical atherosclerosis, metabolic syndrome, obesity, triglyceride levels, and glycosylated haemoglobin. Three case control studies showed a statistically significant correlation between onychomycosis and individuals with DM. Diagnosis was mainly by microscopy and culture with Trichophyton (T) rubrum as the predominant isolate. Fungal nail infections are grossly underdiagnosed and/or underreported in Africa and hence the need for improved awareness and diagnosis especially in patients with DM. Although focused on Africa, this study also revealed paucity of data on onychomycosis in diabetic patients living in the Americas despite evidence from the literature showing a significant number of individuals from that region are living with diabetes. Infections affecting the superficial keratinized layer of the skin, nails, and hair are referred to as dermatophytosis and dermatomycoses, which constitute the most common type of fungal infection that affects people. This clinical ailment has a prevalence of between 30 and 60% and is more common in India's hot, muggy, tropical climate. Examining the prevalence of superficial mycoses (SM), their clinical symptoms, and the fungal species that were identified as the disease-causing agents were the main objectives of the current study. This study comprised 250 clinically confirmed patients with SM who visited our dermatology department over the course of a year. Skin scrapings, nail clippings, and hair samples were gathered, mounted, and cultured using KOH. Macroscopic examination of culture, tease mount, and phenotypic tests were used to identify the species. The age group of 11–20 years (29%) had the highest prevalence of SM out of the 250 clinically verified cases of the condition that were included in our study, followed by 21–30 years (20%) and 31–40 years (18%). Candida albicans, dermatophytes, and non-dermatophytic moulds were the three most prevalent fungal isolates. The most typical dermatophyte isolate was T. rubrum, which was primarily found in Tinea corporis (TCo), Tinea cruris (TCr), and Tinea faciei (TFa). T. mentagrophytes was the second most frequent isolate. Ganoderma lucidum is an extensively famous medicinal mushroom distributed worldwide. Despite being widely grown in Moroccan forests, there are no studies on its nutritional, nutraceutical and pharmaceutical values. Herein, the objective of this study was to investigate the chemical characterization and antimicrobial properties of G. lucidum methanolic extract. Total phenolic, flavonoid, tannin, ascorbic acid and carotenoid contents were determined by spectrophotometry. The results revealed that the most prevalent bioactive compounds were phenolics and flavonoids, with total values of 154.60 mg GAE/g of dry methanolic extract (dme) and 60.55 mg CE/mg of dme, respectively. A GC–MS analysis identified 80 biologically active molecules, which were mainly divided into the following major groups: sugars (49.49%), organic acids (8.89%), fatty acids (7.75%), amino acids (7.44%), steroids (7.32%), polyphenols (5.92%), and others (13.16%). Additionally, 22 individual phenolic compounds were identified and quantified using HPLC–MS, with emphasis on kaempferol (1714 µg/g of dry weight (dw)), apigenin (1955 µg/g dw) and quercetin (947.2 µg/g dw). The methanolic extract of G. lucidum indicated strong antioxidant capacity by means of the following: DPPH radical-scavenging activity (53.7 µg/mL), β-carotene/linoleate assay (43.75 µg/mL), and reducing power assay (76.62 µg/mL). Furthermore, the extract exhibited potent antimicrobial properties against seven human pathogenic microorganisms, including two bacteria and five fungal strains, at concentrations ranging from 1 to 16 mg/mL. The most sensitive pathogen was Epidermophyton floccosum (MIC = MFC = 1 mg/mL), while Aspergillus fumigatus was the most resistant one (MIC = 16 mg/mL and MFC ≥ 16 mg/mL). Overall, our findings demonstrated valuable nutritional and bioactive compound attributes, and potent antioxidant and antimicrobial properties, of G. lucidum growing in Moroccan forests. Background: Tinea capitis, atopic dermatitis and allergic rhinitis are the most common disorders endured by prepubescent children. Dermatophyte infections have been linked to allergic disorders, such as increased sensitivity to dermatophytes in patients with atopic dermatitis. Objectives: To explore the correlation between tinea capitis and allergic diseases in children and to analyse their risk factors. Methods: This study monitored epidemiological changes in childhood tinea capitis and risk factors for whom with allergic disease in a single centre in three consecutive five‐year intervals by reviewing clinical data and multivariate logistic data analysis. Results: Between 2007 and 2022, there were 127 children patients with tinea capitis, the mean age was 4.83 years, and the male‐to‐female ratio was 1.76:1. Zoophilic Microsporum canis and Trichophyton mentagrophytes were the most prevalent pathogens, and the proportions remained relatively constant every 5 years. There were 34 (26.8%) children with tinea capitis complicated with allergic disease, among them 14 children with atopic dermatitis/eczema, 13 with allergic rhinitis, 8 urticaria, 6 food allergies and 1 allergic asthma. Male, kerion, zoophilic species infections and animal contact history were prevalent features in allergic disease combined with tinea capitis. Patients with tinea capitis plus allergic disease mostly had a family history with similar complications. Conclusion: M. canis and T. mentagrophytes were the most prevalent pathogens of tinea capitis in the last 15 years; atopic dermatitis/eczema and allergic rhinitis were the most frequently associated allergic diseases. This study aims to identify and assess the antimicrobial activity of endophytic fungi found in the endemic plant Abies numidica. Among all isolates, the ANT13 isolate demonstrated significant antimicrobial activity in the preliminary screening, particularly Staphylococcus aureus ATCC 25923 and Candida albicans ATCC 1024, with inhibition zones of 22 and 21.5 mm, respectively. Based on its morphological and molecular features, this isolate was identified as Penicillium brevicompactum. The maximum activity was observed in the ethyl acetate extract, followed by the dichloromethane extract; however, the n-hexane extract exhibited no activity. The ethyl acetate extract demonstrated very significant activity against the five strains of multidrug-resistant Staphylococcus aureus used, with average zones of inhibition ranging from 21 to 26 mm, in contrast to more resistant Enterococcus faecalis ATCC 49452 and Bacillus cereus ATCC 10876. The ethyl acetate extract was also very active against dermatophytes, where the zones of inhibition for Candida albicans, Microsporum canis, Trichophyton mentagrophytes, Trichophyton rubrum, and Epidermophyton floccosum were 23.5, 31, 43, 47, and 53.5 mm, respectively. The MIC values for dermatophytes ranged between 100 and 3200 µg/mL. The wild isolate of Penicillium brevicompactum ANT13 discovered as an endophyte in Abies numidica may be a distinctive source of novel compounds and drug discovery to trait dermatophytes and multidrug-resistant Staphylococcus aureus infections.
