In 2009, through Court Order 004, the Colombian Constitutional Court declared that thirty‐four separate groups of Indigenous peoples were at risk of extermination due to armed conflict, and it ordered the government to protect them. In 2013, the court then ordered the government to prove its compliance with Order 004. The government did so through a complex documentation process. The production of documents in Colombia not only records compliance but also constitutes compliance even if the actions are limited. In this article, I examine the role of documentation in mediating legal protection in a violent context. How does a governmental report obscure a persisting threat that is mostly ignored? Using participant observation at the Colombian Interior Ministry on the government's response, I illustrate how this documentation converts ordinary governmental actions into restitution of rights and manufactures compliance via the display—and concealment—of numbers through a focus on aesthetics. This generates the effect of sufficient state response while obfuscating the failure to achieve any intended changes: a phenomenon that I term complying incompliantly. Transforming negligence into a proper response through a paper trail outsmarts auditing and maintains the state's legitimacy by normalizing the government's shortcomings. The amount of scientific papers is growing together with the development of science itself; but, although there is an unprecedented availability of large citation indexes, some daily activities of researchers remain time‐consuming and poorly supported. In this paper, we present Visual Bibliographies (VisualBib), a real‐time visual platform, designed using a zz‐structure‐based model for linking metadata and a narrative, visual approach for showing bibliographies. VisualBib represents a usable, advanced, and visual tool, which simplifies the management of bibliographies, supports a core set of bibliographic tasks, and helps researchers during complex analyses on scientific bibliographies. We present the variety of metadata formats and visualization methods, proposing two use case scenarios. The maturity of the system implementation allowed us two studies, for evaluating both the effectiveness of VisualBib in providing answers to specific data analysis tasks and to support experienced users during real‐life uses. Background: Pelvic pain has cognitive, behavioral, sexual, and emotional consequences. Nurses involved in pelvic floor rehabilitation clinics have contacts with patients reporting chronic pain and should know the most appropriate service for patient referral, to submit the problem to professionals capable of correctly assessing and managing the condition. Furthermore, in some countries nurses can use conservative methods to treat the painful symptoms inside a multidisciplinary team such as breathing retraining, biofeedback, and noninvasive neuromodulation. This paper aims to provide an overview of the literature regarding the role of rehabilitation nurses in dealing with patients suffering from chronic urogenital pelvic pain or urogenital painful syndromes, inside a multidisciplinary team. Methods: Scoping review on Pubmed, CINAHL, Embase, Scopus, Web of Science including trials, reviews, case studies or series, and other descriptive studies regarding the role of nurses inside the multidisciplinary team in the management of males and females presenting chronic pelvic pain (CPP) or chronic pelvic pain syndrome (CPPS). Results: The 36 papers included in this review allowed answering research questions in four areas of nursing: collecting basic information, referring the person to appropriate services, evidence‐based nursing interventions for CPP and CPPS, and proper documentation. Clinical history and assessment of breathing pattern, Muscular assessment and research of trigger points are the main points of data collection. Techniques for muscular relaxation and breathing retraining are important aspects of treatment, as well as biofeedback and noninvasive neuromodulation where the law allows nurses to practice such techniques. The McGill pain questionnaire and the pain inventory of the International Pain Society allow systematic data collection and handover. Conclusion: Rehabilitation nurses work inside multidisciplinary teams when dealing with persons suffering from pelvic pain; further research is needed as our comprehension of the underlying pathophysiological mechanisms of CPP and CPPS evolve. Background: A gap in vancomycin therapeutic drug monitoring (TDM) electronic documentation, management, and related clinical pharmacist activities was identified by our clinical pharmacy team. In response, an electronic TDM (eTDM) template was planned, designed, and implemented by the pharmacy department team in collaboration with the electronic medical records team (EMR) and documentation committee. Aim: To assess the impact of an electronic therapeutic drug monitoring (eTDM) template to measure the impact of adherence to vancomycin guidelines. Guideline adherence indicators include the number of vancomycin levels within the therapeutic range and the number of appropriate concentrations taken. Method: An interventional, single‐centre study of vancomycin therapy in adult patients was performed from 2019–2021. Data were extracted from the electronic medical records and TDM paper‐based forms completed by clinical pharmacists. The number of concentrations within the therapeutic range and the number of appropriately taken levels were analysed by the chi‐square test. This study received an exemption from the Peninsula Health Human Research Ethics Commitee (QA/60523/PH‐2019/197291). Results: There was a total of 198 concentrations collected in the 'before' period and 125 concentrations collected in the 'after' period. The number of concentrations in therapeutic range increased from 34.8% to 43.2% (p = 0.132), not statistically significant. The number of concentrations taken appropriately increased from 33.8% to 55.2% (p < 0.001). The proportion of patients with pharmacist involvement increased from 43.0% to 57.0% (p = 0.868). Conclusion: The study shows that implementation of an electronic vancomycin monitoring template improved the proportion of appropriate levels taken and is the preferred method of documentation by clinical pharmacists. Future projects may benefit from analysing the cost associated with unnecessary vancomycin serum levels ordered or inclusion of pharmacists in the TDM of other narrow therapeutic medications. This study presents and explains the phenomenon of indexjacking, which involves the systematic infiltration of hijacked journals into international indexing databases, with Scopus being one of the most infiltrated among these databases. Through an analysis of known lists of hijacked journals, the study identified at least 67 hijacked journals that have penetrated Scopus since 2013. Of these, 33 journals indexed unauthorized content in Scopus and 23 compromised the homepage link in the journal's profile, while 11 did both. As of September 2023, 41 hijacked journals are still compromising the data of legitimate journals in Scopus. The presence of hijacked journals in Scopus is a challenge for scientific integrity due to the legitimization of unreliable papers that have not undergone peer review and compromises the quality of the Scopus database. The presence of hijacked journals in Scopus has far‐reaching effects. Papers published in these journals may be cited, and unauthorized content from these journals in Scopus is thus imported into other databases, including ORCID and the WHO COVID‐19 Research Database. This poses a particular challenge for research evaluation in those countries, where cloned versions of approved journals may be used to acquire publications and verifying their authenticity can be difficult. The paper reviews the evolution of Interactive Management, later referred to as Structured Democratic Dialogue, starting from the early 1970s up to this date. The authors propose a generational classification scheme consisting of five periods based primarily on whether some or all stages of the process were implemented synchronously or asynchronously and whether the participants' presence was physical, virtual or hybrid. Other aspects such as modifications in the steps of the process; the evolution of the software; domains of applications; file management; methods of collecting or recording contributions, votes, clarifications and preparation of reports; and key players are also considered and reported within the context of the primary scheme. The paper considers key advances achieved at each generational stage in terms of process or software, discusses associated challenges and concludes with a view towards the future of the emerging fifth generation. Background Study: The research discusses the need for a paradigm shift in engineering education current practices to accommodate the digital native students. The paper emphasizes the importance of integrating disruptive technologies, namely Virtual Reality (VR) through Head Mounted Displays VR (HMD VR) and Desktop Based VR (DB VR) and comparing it with the traditional teaching methods. The research focuses on second‐year undergraduate students enrolled in a concrete structures course. Objectives: The objective of the study is to analyse the impact of DB VR and HMD VR on students' motivation, performance and workload in comparison with traditional 2D drawings. The study also aims to investigate how simulation sickness and system usability could impact students' motivation and performance when interpreting concrete structures. Methods: The study divided second‐year civil engineering students into three groups, 2D, DB VR and HMD VR. The performance and motivation levels of students were assessed based on the number of mistakes identified and an intrinsic motivation survey. Furthermore, data on system usability and simulation sickness were collected. Spearman rank correlation was conducted to analyse the relationships between the variables tested. Results: The results have shown that students' motivation and performance were much higher when using VR in general and HMD VR specifically, students' efficiency and accuracy were also observed. On the other hand, usability was found to be moderately correlated with motivation, with students rating HMD VR as the most usable approach. A moderate inverse correlation was observed between simulation sickness and motivation, indicating that higher simulation sickness led to lower motivation levels. Conclusion: The study concludes that integrating VR in general, DB VR, or HMD VR into a concrete structures course for undergraduate students can significantly improve students' performance and motivation. Furthermore, enhancing usability of any of the methods used can also improve the performance and motivation. On the other hand, simulation sickness inversely affects motivation and performance, therefore, VR developers and educators should not neglect it. This paper discusses the evolution of documentary culture in early medieval Tuscia by quantitatively examining the Latin spelling of charter scribes in relation to the following factors: time, the distinction between the formulaic and non‐formulaic parts of the document, the scribe's domicile, the scribe's professional status, and the document type. The paper asks what the spelling of charters tells us about administrative and socio‐cultural changes in charter production and in scribal education. This paper tests the applicability of an unmanned aerial system (UAS) in obtaining spatial data from archaeological sites in forested areas. Our case study discusses the remains of the anti‐Turkish fortifications called 'Turške Šance'—the defensive ditches that are preserved in the area of the Preški vrh (Ravne na Koroškem, Slovenia). Up to 10 fortifications of various shapes and sizes were built in the last quarter of the 15th century. Today, they represent a unique archaeological site. A detailed geodetic survey of the site was performed and compared with results of photogrammetry using the DJI Phantom 4 PRO UAS drone and DJI Phantom camera. Furthermore, official Slovenian LiDAR data of the area were obtained, and archaeologic field surveys, in situ inspections and metal detector scanning were made. The points obtained from the total station were used for 3D modelling and taken as a reference. Drone photogrammetry was performed once per week to compare each point cloud with another using CloudCompare and to the reference one depending on the volume of the tree leaves which was estimated with green leaf index (GLI) and leaf area index (LAI). The increase of the tree leaves volume deteriorates the point cloud obtained with a UAS. The paper analyses how significant this influence is. How do visual methods, particularly the practice of photography, help us to visualise and understand absence in deathscapes? In this paper I will argue that photographs, with their ability to freeze moments, are able to capture what I term points of praxis – moments in which practices by individuals inscribe meaning onto deathscapes, and in that intense and captured presence, evoke the feeling of absent individuals. Such points of praxis can exist in two ways – the praxis of the body and the praxis of objects – both of which I will illuminate with the use of visual ethnographic methods. To support my argument, I present work taken from a three‐year visual ethnographic study of Bukit Brown Cemetery, specifically drawing on the social and cultural documentation of the Hungry Ghost Festival. Background: Phonetic transcription is recognized in regulatory standards as an essential skill for Speech and Language Therapists (SLTs) in the assessment, diagnosis and management of clients with speech difficulties. Previous research has identified that approaches to phonetic transcription vary, and that SLTs often lack confidence in transcribing. However, SLTs' views and working practices have not been investigated in detail, particularly in terms of whole service approaches and following the recent increase in telehealth. Aims: To investigate SLTs' views about phonetic transcription, their working practices at both individual and service levels, and the factors that influence these. Methods & Procedures: A total of 19 SLTs from the UK were recruited to online focus groups via social media and local networks. Participants discussed their views of, and practices in, phonetic transcription. Themes were identified using reflexive thematic analysis. Outcomes & Results: Three broad themes were generated division and unity; one small part of a big job; and fit for purpose. SLTs were uniformly proud of their ability to phonetically transcribe and viewed this as a unique skill, but clear differences existed between different groups of SLTs in their views and practices. Investing in phonetic transcription was not always a priority for SLTs or services, and although many felt under‐confident in their skills they considered these to be adequate for the populations they usually encounter. SLTs make an early judgement about possible therapy targets, which influences the level of detail used in their phonetic transcription. Practical barriers are often not addressed at service level, and assessment via telehealth poses some specific challenges. Conclusions & Implications: SLTs and services would benefit from increased investment in phonetic transcription in terms of time, opportunities for continuing professional development (CPD) and initiatives such as electronic patient records (EPRs) which support the use of phonetic symbols. Identifying target sounds at an early stage raises questions about the implications of disregarding other features of speech, and the selection of appropriate intervention approaches. Further research is needed to analyse actual rather than reported practices, and to consider the relationship between phonetic transcription and intervention approaches. Future studies could also identify precise CPD requirements and evaluate the effectiveness of CPD. What this paper adds: What is already known on the subject: Previous research has demonstrated that SLTs often lack confidence in phonetic transcription and that practices are varied, with relatively little use of narrow transcription. SLTs are interested in opportunities to maintain and develop transcription skills but do not often undertake CPD for transcription. What this paper adds to existing knowledge: By using focus groups as a forum for discussions, this study provides a rich and detailed insight into SLTs' views about clinical transcription and their working practices, with previously unreported details about the reasons for these practices in a clinical context and at a service‐wide level. What are the potential or actual clinical implications of this work?: Transcription is often de‐prioritized in non‐specialist contexts, with practical barriers and a lack of clear and consistent protocols at a whole‐service level. There is an opportunity for service managers to address the systemic difficulties in using transcription effectively by raising the profile and value of transcription amongst clinicians, and promoting CPD opportunities, using the findings of this study as a rationale for funding this. Introduction: People living with dementia experience communication difficulties. Personal information documents, or healthcare passports, enable communication of information essential for the care of a person with dementia. Despite the potential for providing person‐centred care, personal information documents are not ubiquitously used. The Capability Opportunity Motivation—Behaviour (COM‐B) model can be used to understand factors determining individuals' behaviours. Objectives: This study aimed to identify the barriers to and facilitators of the use of healthcare passports for people living with dementia through a systematic review methodology. Methods: A systematic search of six electronic databases was undertaken. Grey literature was searched using three databases. All study types reporting barriers to or facilitators of the use of personal information documents in the care of adults living with dementia in high‐income countries were included. Study quality was assessed using the NICE Quality Appraisal Checklist. Thematic synthesis was used to develop descriptive themes, which were subsequently mapped to the COM‐B framework. Results: Nineteen papers were included. Themes included training, awareness, embedding the process in norms and appreciating the value of the personal information documents. A broad range of barriers and facilitators was identified within each COM‐B domain. Conclusion: This framework provides a starting point for evidence‐informed initiatives to improve the use of personal information documents in the care of people with dementia. Review results will guide evaluation of a local personal information document, which will be designed with input from the Dementia Champions Network (includes carers and other stakeholders). Background: An innovative and interactive real‐world environment can be presented with augmented reality (AR) that comprises digital visual elements, audio, or other sensory information delivered via technology to enhance one's experience. AR has numerous potential applications in various everyday fields. The education sector is one such arena where AR has been implemented prominently globally. AR in education can impact active students' learning to memorize effectively and can persuade them to engage in a meaningful process of learning. Objectives: This article presents a bibliometric analysis of some recent applications of AR, emphasizing the potential effects, challenges, and trends of AR in the education sector for the past 15 years. Methods: A total of 3909 documents published during 2006–2020 were retrieved from the Scopus database for analysis. The bibliometric analysis included document types, subject categorization, document growth, as well as top contributing sources, countries, authors, and funding sponsors. It also analyses keywords, abstracts, titles, and characteristics of the most cited documents. Additionally, it provides a brief account of the challenges for AR adaptation in education. Results and conclusions: The findings uncover an increased use of AR to promote enhanced learning in the education field. This work will provide the survey, limitations, and drawbacks, as well as open usability and technical issues that may represent new challenges for future research in this field. In the European Union Trademark law (EUTM), clear and precise subject‐matter of a trade mark is a prerequisite of proper filing and determines the conditions and scope of protection. Registration rules have evolved together with the meaning of this standard. The paper examines the legal status of position marks, which lacked their own registration specifics within initial EUTM, and thus were usually filed through other categories (figurative, colour, three‐dimensional), and contrasts this practice with the registration scheme of the new law. The analysis looks into the effects of Court of Justice's guidance when retroactively applied to trade marks registered upon earlier law. The exemplary case of a blade of grass inside a bottle is used to map the meaning of 'unaligned' description and/or indication vis‐à‐vis the prime relevance of (graphic) representation. The final part explores how identifying the subject‐matter of a position mark has an impact on assessing its distinctiveness, engaging within functionality objections and proving genuine use. The paper advocates for refraining from applying a fixed understanding of the requirement of clear and precise subject‐matter, especially with regard to old registrations, and, a fortiori, to flexible subject‐matter, such as position signs. To a great extent, the effectiveness of a government may be traced to its constituting documents which serve as a guide or map for how it is supposed to function. From a systems perspective, we may expect a more systemic Constitution to inform the operation of a government that is more systemic and so more effective. This paper uses Integrative Propositional Analysis (IPA) to conduct a preliminary evaluation of the systemic structure of the US Constitution. The analysis finds that the US Constitution has a vanishingly low level of structure. This previously unrecognized lack of structure provides a new explanation for why the nation is increasingly misgoverned (and/or more difficult to govern) and why divisions are deepening between partisan groups, leading to argument and conflict. A path forward is suggested to develop a more systemic Constitution and more effective governance; one supporting a more just, prosperous, and sustainable society. [ABSTRACT FROM AUTHOR]

Background: Current methods of antimicrobial usage surveillance have limited efficacy in changing practice due to delayed reporting to clinicians and the inability to stratify by medical specialty. This study was undertaken in a tertiary teaching hospital with a well established antimicrobial stewardship (AMS) programme and electronic medicines management (eMM) system in Sydney, Australia. Aims: To describe and analyse the implementation of a novel AMS audit and feedback method, in the context of an eMM system. Methods: The AMS team conducted the audit weekly, and the study design was a prospective, observational study. All acute, adult inpatients were included in this intervention. All active systemic antimicrobial prescriptions on the day of the rounds were included. Results: The prevalence of patients on antimicrobial therapy was 37%. The median time taken per round was 44 min for eMM compared to 58 min for paper. All key performance indicators improved over the study period. Appropriateness compared to guidelines increased from 55% to 71%, and documentation of an indication increased from 75% to 98%. There were 1413 recommendations made, with the most common being to cease an antimicrobial agent. The recommendation uptake rate was 47% at 24 h post-round. Conclusions: AMS rounds are an effective tool for auditing and providing feedback on antimicrobial use and should include all antimicrobials rather than solely 'restricted' agents. These rounds had a high uptake rate, improvements in the appropriateness of antimicrobial use, and a planned duration or review date. This article draws on 2 years' worth of ethnographic observation of team meetings to explore decision‐making in an NHS clinical genomics service. The focus of discussions was on ambiguous genomic results known as VUS or Variants of Uncertain Significance, which may be pathogenic but which also may turn out to be benign. In examining decision‐making around such results, we note how, in contrast to much policy and promotional material in this area, clinicians in these meetings (clinical geneticists and genetic counsellors) place great emphasis on parental phenotypes and whether the parents of a patient share the symptoms and signs of the suspected condition. This information is then combined with the result of genomic tests to decide whether the variant a patient has is responsible for their condition. This article explores the way in which clinicians attempt to flexibly enrol parents into genomic explanations through informal diagnosis of their possible phenotypes and the way in which actually meeting parents allows some clinicians to trump explanations based on documentary or photographic data. The paper sheds light on the way that earlier scholarly understandings of such decisions (around, say dysmorphology) remain relevant and explores claims that laboratory tests overrule clinical decision‐making. Introduction: Deaths in care homes and "at home" are anticipated to account for a third of UK deaths by 2040. Currently, palliative and end of life care are not part of statutory training in care homes. Reflective practice is a tool that can facilitate practice‐based learning and support. Following a feasibly study to test "online" supportive conversations and reflection sessions (OSCaRS) to support care home staff in relation to death/dying during the first months of the COVID pandemic, a one‐year practice development follow‐up project was undertaken with the aim to create a team of NHS/specialist palliative care (SPC)‐based facilitators to lead and support OSCaRS provision in up to 50 care homes in one region in Scotland—the focus of this paper. Methods: Forty care home managers attended an on‐line session explaining the project, with a similar session held for 19 NHS/SPC‐based nurses external to care homes. Those interested in facilitating OSCaRS then attended three education sessions. Data collected: records of all activities; reflective notes on OSCaRS organised/delivered; a summary of each OSCaRS reflection/learning points; final interviews with NHS/SPC trainee facilitators. Results: A total of 19 NHS/SPC facilitators delivered one or more OSCaRS in 22 participating care homes. However, as of January 2022 only six trained facilitators remained active. Out of the 158 OSCaRS arranged, 96 took place with a total of 262 staff attending. There were three important aspects that emerged: the role, remit, and resources of NHS/SPC supporting OSCaRS; requirements within care homes for establishing OSCaRS; and, the practice‐based learning topics discussed at each OSCaRS. Conclusion: Attempts to establish a team of NHS/SPC facilitators to lead OSCaRS highlights that end of life care education in care homes does not clearly fall within the contractual remit of either group or risks being missed due to more pressing priorities. Aim: The aim of the paper is to compare the quality of nursing documentation in the Children's Hospital before and after the NANDA‐I nursing diagnoses training. Methods: Research employed the interventional study design, and pre–post study design. Before and after the NANDA‐I nursing diagnoses training, 50 nursing records were analyzed in the interventional pre–post study, using D‐Catch instrument. Results: The most often documented problem‐centered nursing diagnosis before training was anxiety and after the training, hyperthermia. The most common risk diagnoses before and after the training was risk of infection. Before the training, one health promotion diagnosis was determined in the nursing records, and after the training the number increased to four. The highest value was given to readability of the nursing documentation both before and after the training. The lowest score before the training was given to the quality determiners of the accurate nursing diagnoses and after the training given to the determiners of the results' quantity. The sum score of documenting the nursing interventions was the most inconsistent before the training and after the training. The most consistent was the readability of the nursing records before and after the training. Statistically significant differences in the improvement of quality were revealed in all areas except for the readability of the nursing documentation and the quantity of nursing assessment. Conclusions: The results of the study revealed that following the training, the quality of nursing documentation improved, the wording of the nursing diagnoses improved, and the number of accurate nursing diagnoses had increased. Implications for Nursing Practice: Results of the research provide an overview of the importance of the training in improving the quality of nursing documentation and aid the educators in planning the trainings, focusing more on the challenges in the documentation. Background: A lack of standardisation of documentation accompanying older people when transferring from residential to acute care is common and this may result in gaps in information and in care for older people. In Ireland, this lack of standardisation prompted the development of an evidence based national transfer document. Objectives: To pilot a new national transfer document for use when transferring older people from residential to acute care and obtain the perceptions of its use from staff in residential and acute care settings. Methods: This was a pre‐ and post‐study design using purposive sampling following the STROBE guidelines. The pilot was conducted in 26 sites providing residential care and three university hospitals providing acute care. Pre‐pilot questionnaires focused on current documentation and were distributed to staff in residential care (n = 875). A pilot of the new paper‐based transfer document was then conducted over three months and post‐pilot questionnaires distributed to staff from both residential and acute care settings (n = 1085). The findings of the pilot study were discussed with multidisciplinary expert advisory and stakeholder groups who recommended some revisions. This consensus informed the development of the final design of the new revised transfer document. Results: Pre‐pilot: 23% response rate; 83% (n = 168) participants agreed/strongly agreed that existing documentation was straightforward to complete but could be more person‐centred. Post‐pilot: 11% response rate; 75% (n = 93) of participants agreed/strongly agreed that the new transfer document promoted person‐centred care but recommended revisions to the new document regarding layout and time to complete. Conclusions: This study highlighted some of the challenges of providing safe, effective and relevant transfer information that is feasible and usable in everyday practice. Implications for practice: Standardisation and being person‐centred are important determining factors in the provision of relevant up to date information on the resident being transferred. Records are persistent representations of activities created by partakers, observers, or their authorized proxies. People are generally willing to trust vital records such as birth, death, and marriage certificates. However, conspiracy theories and other misinformation may negatively impact perceptions of such documents, particularly when they are associated with a significant person or event. This paper explores the relationship between archival records and trustworthiness by reporting results of a survey that asked genealogists about their perceptions of 44th U.S. President Barack Obama's birth certificate, which was then at the center of the "birtherism" conspiracy. We found that although most participants perceived the birth certificate as trustworthy, others engaged in a biased review, considering it not trustworthy because of the news and politics surrounding it. These findings suggest that a conspiracy theory can act as a moderating variable that undermines the efficacy of normal or recommended practices and procedures for evaluating online information such as birth certificates. We provide recommendations and propose strategies for archivists to disseminate correct information to counteract the spread of misinformation about the authenticity of vital records, and we discuss future directions for research. Midwives and nurses play a critical role in safeguarding the lives of women in resource‐constrained African countries. Working within the context of scarce resources may undermine their moral agency and hinder their ability to care. The purpose of this paper is to understand the influence of resource scarcity on midwifery and nursing care and practice. A critical ethnography was conducted in the obstetric department of three tertiary‐level facilities in Ghana. Purposive sampling was used to recruit 30 midwives and nurses and semistructured interviews, field notes and documentary materials were used to generate in‐depth understanding. Ethical approval was granted from Canada and Ghana and written, and ongoing informed consent was obtained from the participants. Five conceptual themes depicting the impact of scarce resources on midwifery and nursing care were discovered: compromised care, constrained care, dehumanized care, missed care and disengaged care. Improving the maternal health of women and averting avoidable maternal morbidity and mortality require governments and institutions to invest in health infrastructure that will support the delivery of ethical and safe midwifery care for women in their most vulnerable period. Background: Person‐centred care is a growing imperative in healthcare, but the documentation of person‐centred care is challenging. According to the Gothenburg Framework of Person‐centred Care, care should be documented in continuously revised care plans and based on patients' personally formulated goals and resources to secure a continuous partnership. Objectives: This study aimed to examine care plans produced within a randomised controlled trial that tested a person‐centred care intervention in older people with acute coronary syndrome. Nurses with training in the theory and practice of person‐centred care had written the care plans. Methods: We conducted a secondary analysis of care plans developed in a randomised controlled trial for assessing person‐centred care in patients with acute coronary syndrome (Myocardial Infarct [MI] or unstable angina pectoris). The study sample included 84 patients, with three care plans for each patient from inpatient (T1), outpatient (T2) and primary care (T3), that is, a total of 252 care plans. We conducted a descriptive quantitative content analysis of the care plans to examine the reported patients' life‐world and medical/health resources and goals. Results: The analysis illustrates the differences and overlaps between life‐world and medical/health goals and resources. The documented goals and resources change over time: life‐world goals and resources decreased with time as medical/health goals and resources documentation increased. Conclusions: This paper illustrates that in the setting of a randomised controlled trial, nurses with training in person‐centred care recorded fewer life‐world and more medical/health goals over time. Placing life‐world goals at the top of the goal hierarchy enables alignment with medical/health goals. Further research should explore whether the goals and resources documented in care plans accurately reflect patients' wishes as they transition along the care chain. This paper presents a contribution to the study of bibliographic corpora through science mapping. From a graph representation of documents and their textual dimension, stochastic block models can provide a simultaneous clustering of documents and words that we call a domain‐topic model. Previous work investigated the resulting topics, or word clusters, while ours focuses on the study of the document clusters we call domains. To enable the description and interactive navigation of domains, we introduce measures and interfaces that consider the structure of the model to relate both types of clusters. We then present a procedure that extends the block model to cluster metadata attributes of documents, which we call a domain‐chained model, noting that our measures and interfaces transpose to metadata clusters. We provide an example application to a corpus relevant to current science, technology and society (STS) research and an interesting case for our approach: the abstracts presented between 1995 and 2017 at the American Society of Clinical Oncology Annual Meeting, the major oncology research conference. Through a sequence of domain‐topic and domain‐chained models, we identify and describe a group of domains that have notably grown through the last decades and which we relate to the establishment of "oncopolicy" as a major concern in oncology. Mental health nursing is a skilled profession, well positioned to support patients towards recovery with evidence‐based therapeutic interventions. However, the profession continues to be challenged by tensions surrounding the delivery of restrictive interventions and concerns over tendencies towards defensive practices. This paper examines the ambiguity this creates within the mental health nursing role. Organizational cultures that overvalue metrics and administrative tasks create barriers for therapeutic engagement while contributing to role confusion and stress within nursing. We need to address such structural constraints on nurses as mental health nurses' well‐being is crucial to service delivery and the realization of therapeutic goals. From the UK perspective, authors argue that there is a need to examine service structures that foster compassionate and transformational leadership to enable mental health nurses to exercise the agency to practice therapeutically. Background: Conversion from paper‐based to electronic medical records (EMR) may affect the quality and timeliness of the completion of Goals‐of‐Care (GOC) documents during hospital admissions and this may have been further impacted by the COVID‐19 pandemic. Aims: To determine the impact of EMR and COVID‐19 on the proper completion of GOC forms and the factors associated with inpatient changes in GOC. Methods: We conducted a cross‐sectional study of adult general medicine admissions (August 2018–September 2020) at Dandenong Hospital (Victoria, Australia). We used interrupted time series to model the changes in the rates of proper GOC completion (adequate documented discussion, completed ≤2 days) after the introduction of EMR and the arrival of COVID‐19. Results: We included a total of 5147 patients. The pre‐EMR GOC proper completion rate was 27.7% (overall completion, 86.5%). There was a decrease in the proper completion rate by 2.21% per month (95% confidence interval (CI): −2.83 to −1.58) after EMR implementation despite an increase in overall completion rates (91.2%). The main reason for the negative trend was a decline in adequate documentation despite improvements in timeliness. COVID‐19 arrival saw a reversal of this negative trend, with proper completion rates increasing by 2.25% per month (95% CI: 1.35 to 3.15) compared with the EMR period, but also resulted in a higher proportion of GOC changes within 2 days of admission. Conclusions: EMR improved the timeliness and overall completion rates of GOC at the cost of a lower quality of documented discussion. Background: Previous eBook studies were primarily cross‐sectional surveys or experimental studies providing a snapshot of the impact of eBook reading on children's emergent literacy skills. Scholars have yet to characterize more fine grain developmental progressions in the use of eReaders and similar devices – that is, how children's eBook‐related practices evolve over time and what might be changing behaviourally over the course of repeated readings for any age group. Objectives: This is the first study to characterize young children's (2 to 3 years) use of eReaders over the span of several months under semi‐naturalistic conditions. Methods: Twenty five mothers and their 2.5‐ to 3.5‐year‐old child engaged in a series of documentation exercises and interviews around use of a novel eReader device. This study is exploratory and deductive in nature, and hypotheses were not set forth a priori. Results and Conclusions: The results of the present study suggest that children develop increasing mastery of the affordances of eReaders as their device exposure increases. Over time, their ability to independently engage with devices rapidly increases. They transition from needing technical support from parents and high levels of encouragement to attend to literacy content over hotspots, to sometimes using the device independently, to considering the device a mainstay in their home – no longer a novelty. Implications for practice and/or policy: The developmental usability patterns noted in this study extend our naturalistic understanding of young children's dexterity with digital devices and point to methodological approaches that might yield more ecologically valid findings in future studies. Transracial adoption was legalized in South Africa through legislative reform as South Africa moved into a post‐apartheid era of non‐racialism. Transracial adoption offers one option of placement for Black children in need of alternative care. However, adoption continues to face obstacles to implementation, including cultural obstacles which impact on placement of Black African children. This paper reports on adoption statistics on national adoption in the period 2013–2021 and a documentary analysis of the current legal and policy framework governing transracial adoption. We cover international instruments, local laws, including the Children's Act of 2005, and relevant policy documents designed to facilitate the implementation of adoption. We show that the law is supportive of transracial adoption, but that policy documents offer a somewhat contradictory stance to the practice. The lack of success of transracial adoption as a child protection strategy does not lie within the legal framework but may lie in the difficulties in interpreting and implementing the range of different policies by those implementing these policies. More research into the implementation practice of these bureaucrats is required to understand the underutilization of transracial adoption as a placement option for Black children in South Africa. Purpose: The purpose of this paper is to describe the implementation of evidence‐based, individualized coping plans and to compare the rates of child anxiety, child cooperation, and parent anxiety before and after implementation of these coping plans for children receiving recurring BoNT‐A injections emphasizing the role of Certified Child Life Specialists (CCLSs) within the interdisciplinary team approach. Design and Methods: A retrospective chart review was conducted for children receiving BoNT‐A injections at a Physical Medicine clinic pre‐ and post‐implementation of the coping plan. Descriptive statistics were used to evaluate care plan implementation, child cooperation, child anxiety, and parent anxiety. Results: Post‐implementation of coping care plans, children experienced improved cooperation during BoNT‐A injections. Parental anxiety decreased once coping plans were implemented. Documentation improved after the implementation of coping plans specific to areas involving cooperation, child's anxiety and distress, and parental anxiety and distress. It was also noted that there was an increase in child anxiety assessments within documentation. Practice Implications: CCLS were able to utilize their expertise to collaboratively create individualized coping care plans to increase child's cooperation and decrease parental anxiety during BoNT‐A injections. Background: Anaphylaxis is a life‐threatening allergic reaction with significant risk for children with allergies. Access to potentially life‐saving medication is critical for these children. This review aims to describe state laws and policies guiding stock epinephrine in schools for allergic or anaphylactic events and detail recommendations for best practices. Methods: This paper is a comprehensive review of the 50 states and the District of Columbia's (DC) laws and policies for stock epinephrine at schools for children in pre‐kindergarten through 12th grade. Results: All 50 states and DC allow undesignated epinephrine in schools to use for allergic or anaphylactic reactions. A key difference arises in whether states allow (N = 37) or mandate (N = 14) that schools stock epinephrine. States exhibit differences in the body responsible for developing stock epinephrine policy (N = 48), along with stakeholders responsible for developing procedures (N = 30) and implementing them (N = 19). Differences also exist in state epinephrine procurement, administration, training, and liability. Conclusions: This review of stock epinephrine laws and policies highlights the significant variation in state legislation despite widespread adoption. Only one‐quarter of states mandate that schools have stock epinephrine available for emergency use, underscoring need for a more unified approach with consistent guidelines, comprehensive training, and possible funding for implementation. One important cause of retinal haemorrhages (RHs) in children is abusive head trauma (AHT). Our programme of research was conducted on children admitted to hospital over a seven‐year period who had RHs as part of their clinical presentation. This paper summarises important findings from previous publications. As a prerequisite, two basic studies were necessary: (i) a method of reporting the precise location of the RH in the retina; and (ii) defining a simple clinical classification of RHs based on the appearance of the retinal layer involved. Because raised intracranial pressure (RICP) is a frequent secondary brain insult following serious adult head injury, causing hypoxic‐ischaemic injury with a resultant poor outcome, it was considered whether such RICP might also cause RHs in children. Detailed, state‐of‐the‐art monitoring methods were used to investigate this relationship along with retinal imaging. Methodical documentation of the type, area and lifespan of RHs meant that it was possible to address the question of predicting AHT from RH numbers and characteristics, and to describe not only the duration of different layer haemorrhages, but also a previously unreported observation that some RHs transiently enlarge over two to three days before resolving normally. Background: A resuscitation plan is a medically authorised order to use or withhold resuscitation interventions. Absence of appropriate resuscitation orders exposes patients to the risk of invasive medical interventions that may be of questionable benefit depending on individual circumstances. Aims: To describe among junior doctors: (i) self‐reported confidence discussing and completing resuscitation plans; (ii) knowledge of resuscitation policy including whether resuscitation plans are legally enforceable and key triggers for completion; and (iii) the factors associated with higher knowledge of triggers for completing resuscitation plans. Methods: A cross‐sectional survey was conducted at five hospitals. Junior doctors on clinical rotation were approached at scheduled training sessions, before or after ward rounds or at change of rotation orientation days and provided with a pen‐and‐paper survey. Results: A total of 118 junior doctors participated. Most felt confident discussing (79%; n = 92) and documenting (87%; n = 102) resuscitation plans with patients. However, only 45% (n = 52) of doctors correctly identified that resuscitation plans are legally enforceable medical orders. On average, doctors correctly identified 6.8 (standard deviation = 1.8) out of 10 triggers for completing a resuscitation plan. Doctors aged >30 years were four times more likely to have high knowledge of triggers for completing resuscitation plans (odds ratio 4.28 (95% confidence interval 1.54–11.89); P = 0.0053). Conclusion: Most junior doctors feel confident discussing and documenting resuscitation plans. There is a need to improve knowledge about legal obligations to follow completed resuscitation plans, and about when resuscitation plans should be completed to ensure they are completed with patients who are most at risk. Purpose: The purpose of this study was to assess the extent to which academic promotion and tenure (APT) criteria and guidelines in schools of nursing recognize predatory publishing. This assessment included an analysis of APT documents looking specifically for guidance about predatory publications by faculty in schools of nursing. Design: This study used a cross‐sectional, descriptive design and was conducted in 2020. Methods: A mixed methods approach was used to collect data from two sources. Data were extracted from APT documents for 92 research‐intensive universities found online and specifically focused on documents for universities and for schools of nursing in the United States. Interviews were conducted with a subsample of academic administrators (n = 10) from selected schools. Findings: The majority (57%; n = 50) of APT documents reviewed addressed quality of the journals in which faculty publish. However, very nonspecific terms, such as "high quality" or "peer reviewed" were used. None of the documents reviewed (n = 88) included any reference to predatory journals. Deans who were interviewed validated the analysis of the APT documents. While most deans reported faculty were aware of predatory journals and the risks of publishing in them, formal guidelines for consequences for publishing in predatory journals were not developed or available. Conclusion: This study examined how schools of nursing in research‐intensive universities address the issue of predatory journals. APT criteria do not provide guidance to faculty and promotion and tenure committees about issues related to predatory publications as low‐quality publication outlets. Recommendations for APT committees, mentors, and faculty are provided. Clinical relevance: Clinicians rely on researchers, many of whom are faculty, to publish rigorous studies that produce evidence they can translate into practice. One measure of the quality of a study's findings is where the paper is published and reflects the level of peer review it has been through. Faculty who publish in predatory journals may not have had their work reviewed by experts; evidence produced may or may not be adequate for translation to guide nursing practice. Although nursing terminologies and classifications represent nursing knowledge across diverse clinical areas, end‐of‐life care seems under represented in many aspects of these instruments. NANDA‐ I is an international nursing diagnostic classification widely used in nursing education and research. This taxonomy is based on seven axes, including the axis of time. In this commentary we bring discussion to the need to update nursing terminology by including the term end‐of‐life in the time axis of NANDA‐I. After describing the epidemiologic aspects of end‐of‐life care and discussing the relevant nursing role, we discuss patients' and family's human responses towards the end‐of‐life time and circumstance, which are central to defining nursing diagnoses. End‐of‐life care is one priority in health care, and nursing diagnoses should represent that situation as well. This paper focuses on a specific and international nursing diagnosis classification, NANDA‐I, which lacks an end‐of‐life component to its time axis for defining labels of nursing diagnoses. Attending to the importance of classifications in clinical reasoning, nursing diagnoses could better represent responses towards this health condition, opening new opportunities for increasing nursing roles in clinical practice, and also for new studies aiming to validate nursing diagnoses, and promoting an evidence‐based practice by including end‐of‐life in the axis time. Objective: Perioperative initiation of opioids continues to be a major contributor to chronic use, misuse and diversion in regional areas. There is considerable effort to mitigate harm through avoiding excessive prescribing and reducing the risk of persistent postoperative opioid use. Improving perioperative documentation practices has been reported to ensure appropriate opioid initiation and de‐escalation. It has not been established whether these strategies are utilised in regional hospitals. Methods: A retrospective observational study of perioperative opioid prescribing and documentation practices in Goulburn Base Hospital, a regional centre in the Southern New South Wales (NSW) Local Health District. Data were collected from 110 records and validated for adult patients undergoing elective total knee replacement (TKR) or total hip replacement (THR) from 12 January 2020 to 13 January 2021. Outcome Measures: To observe perioperative opioid prescribing and utilisation of harm reduction strategies in a regional hospital. Results: 65% of patients were opioid naïve (ON). Preoperative pain assessments and patient education were completed in 23% and 15% of records, respectively. Postoperative opioids were prescribed for 99% of patients, with 74% prescribed a slow‐release (SR) formulation. 50% of patients were discharged with an SR prescription. Inadequate postoperative pain control was reported in 21% of patients. Of the 103 patients prescribed opioids on discharge, only 20% included a de‐escalation plan and only 35% of discharge summaries included dose and quantity of opioids supplies. Conclusions: This study has identified underutilisation of perioperative harm reduction strategies despite the potential to improve appropriate initiation and de‐escalation of opioids. This article examines land acquisition for the construction of a metro train line in Lahore, Pakistan, to argue that the role and temporality of bureaucratic documentary practices are key to enabling urban informality and associated regimes of substantive citizenship. I examine the changing role of bureaucratic documents along the axes of (a) state and citizens, as documents are transferred between various actors who attribute different meanings to them; and (b) time, as documents assume varying meanings across their life cycles. First, I argue that documents constitute the disaggregated state and facilitate inconsistent practices that produce urban informality by exempting such settlements from some regulations while subjecting them to others. Second, a study of the time effects of these contingent practices and associated documentation shows that while original settlers may manipulate the disparate institutions of the state to secure resources, tolerance of such settlements across generations prompts a reformulation of local conceptions of landownership. Contemporary residents, then, position themselves as propertied citizens in possession of ownership documents. In this case of land acquisition, such efforts secured limited recognition from the state: residents were cast as illegal encroachers but were eventually offered some compensation as humanitarian aid, thereby reproducing socioeconomic inequalities. The EU database of processing factors for pesticide residues has been set up in 2018, first updated in 2022 and now updated for a second time. The extended version contains 2,651 studies, 17,937 individual and 4,229 median processing factors, all evaluated according to up‐to‐date residue definitions and storage stability information. The studies were either provided to EFSA in the MRL setting procedures, during the renewal of approval of active substances or were available to Member States in the framework of other regulatory procedures. All studies have been thoroughly (re‐)evaluated with respect to the well‐proven set of quality criteria already applied in the EU database. Processing factors were derived, and their reliability judged. Further representative processes such as baking of apples, production of apple flakes, canning of sweet corn, production of semolina and some further cooking processes have been described. To systematically search and synthesise available literature on barriers and enablers to evidence‐based care for patients with laparotomy wounds reported by acute care nurses. Specifically, we focused on wound assessment, infection control techniques, wound products used, escalation of care, dressing application, documentation and holistic care. The Preferred Reporting Items for Systematic review and Meta‐Analyses extension for Scoping Reviews Checklist and explanation documents directed the review. The methodology framework created by Arksey and O'Malley, updated by Levac et al., and the Joanna Briggs Institute were utilised to assist the scoping review process. Data synthesis was guided by the Theoretical Domains Framework. Six qualitative and mixed methods studies were selected for the review. Most reported barriers and enablers were mapped to knowledge, skills, beliefs about consequences, environmental context and resources and beliefs about capability domains. The main barriers were limited access to and utilisation of wound assessment tools and clinical practice guidelines for wound management and suboptimal time management skills. Inconsistent management of laparotomy wounds was related to ward culture and nurses' lack of knowledge and skills in surgical wound assessment and aseptic technique during wound encounters. The reported enablers were knowledge of multi‐factorial risk factors for surgical wound recovery, valuing education and reflective practice and believing that protocols should be utilised alongside comprehensive wound assessments. Holistic wound care included patient education on the role of mobilisation and nutrition in wound healing. Acute care nurses do not routinely incorporate comprehensive, evidence‐based care recommendations for laparotomy wound management. Further research on evidence‐based care behaviours in managing laparotomy wounds is required. With the boom in urban living has come a boom in urban gardening. In particular, urban community gardening is an increasingly popular form of horticultural production, community involvement and connection to nature. Through the establishment and management of community gardens, biodiversity can flourish, with community gardens as 'hotspots' of flora and fauna within the urban matrix. Gardeners can deeply connect with the natural elements of gardens and thus learn about and gain appreciation for the natural world. Such interactions can combat the loss of nature experiences in cities. Despite their benefits for nature and for people, community gardens are threatened ecosystems as often temporary fixtures in city landscapes due to lack of land tenure and policy protection.In this perspective, we recognize community gardens as an important ecosystem in urban conservation and argue for the defence of urban community gardens by city policy. We formalize this activity and the value of these ecosystems with scientific evidence from ecological and social‐ecological research in 39 community gardens in Berlin and Munich, Germany.Although our data reveal that these gardens support large amounts of biodiversity and catalyse human‐nature connections, a lack of comprehensive documentation of social‐ecological benefits at the city level can make community gardens vulnerable to urban planning threats; we have seen losses of multiple research sites in the last 4 years of biodiversity research.Policy implications: To protect community gardens now and for future urban generations, we call for systematic and comprehensive data collection on community gardening activities and policy support for these urban ecosystems. This article deals with a new study document, Churches and Moral Discernment: Facilitating Dialogue to Build Koinonia, adopted by the Commission on Faith and Order of the World Council of Churches in 2021. This document addresses basic issues of moral discernment in ecumenical relations and seeks to spell out how churches can deal constructively with disagreement on ethical issues. Such disagreement can lead to tensions that threaten division within churches and can become a burden in ecumenical cooperation between churches. Underlying the specific ethical questions and the complexity of church relations is often the question of the place and self‐understanding of churches in their specific society at a specific time. Thus, it is a question about dealing with the historicity of the public witness of the churches and their reference and relationship to the world. This article introduces micro‐prosody as the study of the duration and timing of speech events. We present a descriptive framework, formalising micro‐prosody in terms of gestural landmarks and coordination relations between them, and we use the framework to illustrate different patterns of micro‐prosody across languages. We show that potential ambiguity between coordination relations can be resolved by considering how they structure natural variation in speech. The framework presented here is intended to offer effective tools for phonetic documentation. We end with some considerations for a theory of micro‐prosody, including how micro‐prosody relates to other levels of phonological structure, and a brief discussion of different data types that can be used to infer aspects of micro‐prosody. Background: Many individuals worldwide continue to take benzodiazepine receptor agonists (BZRAs) long term (≥3 months). The aim of this study was to conduct a content analysis of the views and experiences of discontinuing long‐term BZRA use as documented in the free‐text responses of respondents to an online questionnaire examining mediators of behaviour change relating to the discontinuation of long‐term BZRA use. Design: The questionnaire was disseminated via online BZRA support groups to community‐based adults with either current or previous experience of long‐term BZRA use. The four free‐text questions focused on (1) barriers and (2) facilitators to discontinuing BZRA use; (3) additional supports required to discontinue BZRA use; and (4) additional comments regarding BZRA use. Response data were analysed using summative content analysis. Results: The most commonly reported barrier to BZRA discontinuation related to the consequences of stopping the medication, including withdrawal symptoms and the possibility of return of the original symptoms. The most common facilitator that respondents reported would help them in discontinuing BZRA use was support, primarily from medical professionals. Many respondents reported having been harmed or negatively affected in some way because of BZRA use. Several respondents expressed regret over ever taking BZRAs and/or reported that, with the benefit of hindsight, they should never have taken BZRAs in the first instance. Conclusion: The findings highlight the range of barriers faced by those attempting BZRA discontinuation and the importance of additional supports. Holistic and person‐centred approaches are needed to support discontinuation of long‐term BZRA use that considers an individual's personal circumstances and wider social context. The prevention of pressure ulcer (PU) or pressure injury (PI) wounds is of public health importance in developed countries, including Hungary. The study aimed to assess the PU/PI prevention and care practices of Hungarian public hospitals and identify organizational and management factors. In 2022, a national, questionnaire‐based survey of inpatient institutions relevant to PU/PI care was conducted, providing a picture of the practices of 86 hospitals for the year 2019. The questionnaire was processed using descriptive statistics and regression analysis. The survey results show that good practices in Hungary are isolated, the reporting system is inhomogeneous, and documentation is not uniform across our institutional system. Of the 86 institutions, 71.0% operate a PU prevention team, 64.0% use prophylactic dressings, and 88.3% use an anti‐decubitus mattress, with an average ratio of 26.1% to the number of beds. Less than half of the institutions reported the incidence of hospital acquired pressure injuries (HAPIs). In this sample, we found no significant association between hospital type and hospital size with the incidence of full‐thickness HAPIs (stage III and IV wounds). Developing a comprehensive PU/PI reporting system and updating the national PU/PI prevention and care guidelines are essential in Hungary. [ABSTRACT FROM AUTHOR]

Aim: This study aimed to undertake a bibliometric analysis to provide comprehensive information in demonstrating the current status and outline the overall trends in the area of readiness for hospital discharge (RHD). Design: A bibliometric and visual analysis of RHD literature was undertaken. Methods: Articles were retrieved from the Web of Science network from 2002 to 2021. VOSviewer was used to identify the co‐authorship network of countries/institutions, co‐authorship and co‐citation analysis of authors, and co‐citation analysis of references and citation analysis of documents. CiteSpace was used to identify the keyword co‐occurrence network and perform cluster analysis, detecting the keywords with citation bursts and speculated frontiers in this research field. Results: A total of 512 articles were included in the final analysis. Key findings are: (1) There has been a continuous but somewhat fluctuating rise in the number of publications. (2) 56.05% of publications come from the USA with Marquette University making the highest contribution. (3) Most publications (17, 3.32%) in RHD research were from the Journal of Clinical Nursing, and Anaesthesia and Analgesia had the highest number of citations (584 citations). (4) Weiss ME (25, 4.88%) was the most productive author whose articles have been the most highly cited (646 citations). (5) Cited references from Weiss ME (2007) also made the largest contribution to co‐citations, and the most cited reference was from Jack BW (2009) (1022 citations). (6) The 20 most frequent keywords and keywords with the strongest citation bursts were retrieved. There were seven research hotspots, and three emerging research frontiers were explored. Conclusions: The bibliometric analysis of material published in the last 20 years indicates that there have been statistically significant gains in comprehensive information on RHD, including the knowledge mapping of the countries, institutions, authors, references and keywords. The hotspots and frontiers, which have been explored can give guidance to researchers as to new angles and directions to take. Implications for Nursing Management: The effects of the implementation of ERAS on RHD‐related risk for adverse post‐discharge outcomes in surgical patients will be of increasing concern for healthcare professionals.It is important for patients and their relatives to be confident that on discharge they are sufficiently ready for hospital discharge to enable them to safely and smoothly make the transition to home.Improving the level of RHD in discharged patients can help nurse managers and researchers measure the effectiveness of discharge planning services. Background: Although nurses are expected to address the social determinants of health (SDH) in clinical settings, the perspectives of front‐line nurses on the integration of SDH into their clinical practice remain unclear. Understanding the dynamism of this integration and its outcomes can yield crucial insights into effective nursing care. This study aims to elucidate the integration and adoption of tool‐based SDH assessment nursing programs and their impacts on daily nursing care. Methods: We conducted qualitative research at a small community‐based hospital in Japan, where a tool‐based program characterized by social background interviews and documentation was implemented. Nurses at the hospital were recruited via purposive and snowball sampling. After hypothesis generation, semi‐constructed in‐depth online interviews were conducted. Each interview lasted between 30 and 50 min. The data were analyzed via thematic analysis using the framework approach. Results: A total of 16 nurses participated. Participants' incorporation of the novel SDH assessment program was bolstered by prior learning and their recognition of its practical value. Institutional support and collaborative teamwork further facilitated the adoption of this innovation. Enhanced knowledge about the social contexts of their patients contributed to increased respect, empathy, and self‐affirmation among participants, consequently enhancing the quality of nursing care. Conclusion: Through team‐based learning, reflection, and support, nurses can integrate a tool‐based SDH assessment program into their daily nursing practice. This program has the potential to empower nurses to deliver more holistic care and redefine their professional identity. Further research is warranted to assess patient‐reported outcomes. Aim: To explore social and healthcare professionals' experiences of end‐of‐life (EOL) care planning and documentation in palliative care. Design: A qualitative study with narrative methodology. Methods: A narrative method with interviews was used. Data were collected from purposively selected registered nurses (n = 18), practical nurses (n = 5), social workers (n = 5) and physicians (n = 5) working in palliative care unit in five hospitals in three hospital districts. Content analysis within narrative methodologies was undertaken. Results: Two main categories – patient‐oriented EOL care planning and multi‐professional EOL care planning documentation– were formed. Patient‐oriented EOL care planning included treatment goals planning, disease treatment planning and EOL care setting planning. Multi‐professional EOL care planning documentation included healthcare professionals' and social professionals' perspectives. Healthcare professionals' perspectives on EOL care planning documentation included benefits of structured documentation and poor support of electronic health record (EHR) for documentation. Social professionals' perspective on EOL care planning documentation included usefulness of multi‐professional documentation and externality of social professionals in multi‐professional documentation. Conclusion: The results of this interdisciplinary study demonstrated a gap between what healthcare professionals consider important in Advance Care Planning (ACP), that is, proactive, patient‐oriented and multi‐professional EOL care planning and the ability to access and document this in a useful and accessible way in the EHR. Relevance to Clinical Practice: Knowledge of the patient‐centered EOL care planning and multi‐professional documentation processes and their challenges are prerequisites for documentation to be supported by technology. Aim: Develop and test a data collection tool—Neurological End‐Of‐Life Care Assessment Tool (NEOLCAT)—for extracting data from patient health records (PHRs) on end‐of‐life care of neurological patients in an acute hospital ward. Design: Instrument development and inter‐rater reliability (IRR) assessment. Method: NEOLCAT was constructed from patient care items obtained from clinical guidelines and literature on end‐of‐life care. Expert clinicians reviewed the items. Using percentage agreement and Fleiss' kappa we calculated IRR on 32 nominal items, out of 76 items. Results: IRR of NEOLCAT showed 89% (range 83%–95%) overall categorical percentage agreement. The Fleiss' kappa categorical coefficient was 0.84 (range 0.71–0.91). There was fair or moderate agreement on six items, and moderate or almost perfect agreement on 26 items. Conclusion: The NEOLCAT shows promising psychometric properties for studying clinical components of care of neurological patients at the end‐of‐life on an acute hospital ward but could be further developed in future studies. Aim: The aim of this study was to gain insight into nurses' perspectives on the shift‐to‐shift handover in relation to providing Person‐centred care (PCC) in nursing homes. Background: PCC is perceived as the gold standard for nursing home care. To preserve the continuity of PCC, an adequate handover during the nurses' shift change is essential. There is, however, little empirical evidence for what constitutes best shift‐to‐shift nursing handover practices in nursing homes. Design: An exploratory qualitative descriptive study. Methods: Nine nurses were selected purposively and through snowball sampling from five Dutch nursing homes. Semi‐structured face‐to‐face and telephone interviews were conducted. Analysis relied on Braun and Clarke's thematic analysis. Results: Four main themes were identified related to enabling PCC informed handovers: (1) knowing the resident to be enable to provide PCC was key, (2) the actual handover, (3) additional ways of information transfer and (4) nurses' knowledge of the resident prior to start shift. Conclusion: The shift‐to‐shift handover is one way that nurses become informed about residents. Knowing the resident is essential to enable PCC. The fundamental underlying question is to what extent nurses have to know the resident in order to enable PCC. Once that level of detail has been established, in‐depth research is needed to determine the best method for conveying this information to all nurses. Automated text categorization methods are of broad relevance for domain experts since they free researchers and practitioners from manual labeling, save their resources (e.g., time, labor), and enrich the data with information helpful to study substantive questions. Despite a variety of newly developed categorization methods that require substantial amounts of annotated data, little is known about how to build models when (a) labeling texts with categories requires substantial domain expertise and/or in‐depth reading, (b) only a few annotated documents are available for model training, and (c) no relevant computational resources, such as pretrained models, are available. In a collaboration with environmental scientists who study the socio‐ecological impact of funded biodiversity conservation projects, we develop a method that integrates deep domain expertise with computational models to automatically categorize project reports based on a small sample of 93 annotated documents. Our results suggest that domain expertise can improve automated categorization and that the magnitude of these improvements is influenced by the experts' understanding of categories and their confidence in their annotation, as well as data sparsity and additional category characteristics such as the portion of exclusive keywords that can identify a category.
