Purpose: To assess any effects of a state-wide sodium reduction intervention on sodium intake, sources of dietary sodium and discretionary salt use at a population level. Methods: Data (24-h urinary sodium excretion, self-report survey, a 24-h dietary recall) were collected cross-sectionally at baseline (2016/2017) and follow-up (2020) from adults in Victoria, Australia. Intervention activities included consumer awareness advertising campaign, public debate generation via mass media, strengthening existing policy initiatives and supporting food innovation with industry. Results: There were 339 participants at baseline and 211 at follow-up, with 144 and 90 of participants completing a 24-h dietary recall, respectively. There was no difference in adjusted 24-h urinary sodium excretion between baseline and follow-up (134 vs 131 mmol/24 h; p = 0.260). There were no differences in the percentage of participants adding salt during cooking (63% vs 68%; p = 0.244), adding salt at the table (34% vs 37%; p = 0.400) or regularly taking action to control salt/sodium intake (22% vs 21%; p = 0.793). There were large differences in the quantity of dietary sodium sourced from retail stores (57% vs 77%, p < 0.001), and less sodium was sourced from foods at fresh food markets (13% vs 2%; p ≤ 0.001) at follow-up. No large differences were apparent for foods with different levels of processing or for food groups. Conclusion: There was no clear population-level effect of the 4-year multi-component Victorian Salt Reduction Intervention on sodium intake with Victorian adults continuing to consume sodium above recommended levels. The findings indicate that more intensive and sustained efforts aiming at the retail and food industry with national level support are likely to be required to achieve a measurable improvement in sodium intake at a state level. Objectives: Non-contrast computed tomography of the brain (NCCTB) is commonly used to detect intracranial pathology but is subject to interpretation errors. Machine learning can augment clinical decision-making and improve NCCTB scan interpretation. This retrospective detection accuracy study assessed the performance of radiologists assisted by a deep learning model and compared the standalone performance of the model with that of unassisted radiologists. Methods: A deep learning model was trained on 212,484 NCCTB scans drawn from a private radiology group in Australia. Scans from inpatient, outpatient, and emergency settings were included. Scan inclusion criteria were age ≥ 18 years and series slice thickness ≤ 1.5 mm. Thirty-two radiologists reviewed 2848 scans with and without the assistance of the deep learning system and rated their confidence in the presence of each finding using a 7-point scale. Differences in AUC and Matthews correlation coefficient (MCC) were calculated using a ground-truth gold standard. Results: The model demonstrated an average area under the receiver operating characteristic curve (AUC) of 0.93 across 144 NCCTB findings and significantly improved radiologist interpretation performance. Assisted and unassisted radiologists demonstrated an average AUC of 0.79 and 0.73 across 22 grouped parent findings and 0.72 and 0.68 across 189 child findings, respectively. When assisted by the model, radiologist AUC was significantly improved for 91 findings (158 findings were non-inferior), and reading time was significantly reduced. Conclusions: The assistance of a comprehensive deep learning model significantly improved radiologist detection accuracy across a wide range of clinical findings and demonstrated the potential to improve NCCTB interpretation. Clinical relevance statement: This study evaluated a comprehensive CT brain deep learning model, which performed strongly, improved the performance of radiologists, and reduced interpretation time. The model may reduce errors, improve efficiency, facilitate triage, and better enable the delivery of timely patient care. The use of a fallow phase is an important tool for maximizing crop yield potential in moisture limited agricultural environments, with a focus on removing weeds to optimize fallow efficiency. Repeated whole field herbicide treatments to control low-density weed populations is expensive and wasteful. Site-specific herbicide applications to low-density fallow weed populations is currently facilitated by proprietary, sensor-based spray booms. The use of image analysis for fallow weed detection is an opportunity to develop a system with potential for in-crop weed recognition. Here we present OpenWeedLocator (OWL), an open-source, low-cost and image-based device for fallow weed detection that improves accessibility to this technology for the weed control community. A comprehensive GitHub repository was developed, promoting community engagement with site-specific weed control methods. Validation of OWL as a low-cost tool was achieved using four, existing colour-based algorithms over seven fallow fields in New South Wales, Australia. The four algorithms were similarly effective in detecting weeds with average precision of 79% and recall of 52%. In individual transects up to 92% precision and 74% recall indicate the performance potential of OWL in fallow fields. The literature on subjective memory concerns (SMC) as a predictor for future cognitive decline is varied. Furthermore, recent research has pointed to additional complexity arising from variability in the experience of SMC themselves (i.e. whether they are remitting or sustained over time). We investigated the associations between SMC and objectively measured cognition in an Australian population-based cohort. Four waves (4-year intervals between waves) of data from 1236 participants (aged 62.4 ± 1.5 years, 53% male) were used. We categorized participants as experiencing SMC, when they indicated that their memory problems might interfere with their day-to-day life and/or they had seen a doctor about their memory. SMC was categorized as "no" reported SMC, "remitting", "new-onset" or "sustained" SMC. Cognitive assessment of immediate and delayed recall, working memory, psychomotor speed, attention and processing speed were assessed using a neuropsychological battery. Eighteen percent of participants were characterised as having SMC: 6% (77) "remitting", 6% (77) "new-onset" and 6% (69) "sustained" SMC. There was no consistent evidence for an association between SMC and subsequent decline in cognition. However, SMC was associated with poorer performance on contemporaneous tasks of attention and processing speed compared to "no" SMC. Asking about SMC may indicate a current decline in cognitive function but, in this sample at least, did not indicate an increased risk of future decline. [ABSTRACT FROM AUTHOR]

This study investigated the application of a recurrent neural network for optimising pharmacological treatment for depression. A clinical dataset of 458 participants from specialist and community psychiatric services in Australia, New Zealand and Japan were extracted from an existing custom-built, web-based tool called Psynary. This data, which included baseline and self-completed reviews, was used to train and refine a novel algorithm which was a fully connected network feature extractor and long short-term memory algorithm was firstly trained in isolation and then integrated and annealed using slow learning rates due to the low dimensionality of the data. The accuracy of predicting depression remission before processing patient review data was 49.8%. After processing only 2 reviews, the accuracy was 76.5%. When considering a change in medication, the precision of changing medications was 97.4% and the recall was 71.4%. The medications with predicted best results were antipsychotics (88%) and selective serotonin reuptake inhibitors (87.9%). This is the first study that has created an all-in-one algorithm for optimising treatments for all subtypes of depression. Reducing treatment optimisation time for patients suffering with depression may lead to earlier remission and hence reduce the high levels of disability associated with the condition. Furthermore, in a setting where mental health conditions are increasing strain on mental health services, the utilisation of web-based tools for remote monitoring and machine/deep learning algorithms may assist clinicians in both specialist and primary care in extending specialist mental healthcare to a larger patient community. Summary: In this study of 695 Australian older adults (aged ≥50 years), we found that men and women had a similar trajectory of health-related quality of life (HRQoL) recovery following fragility fracture at any skeletal site. These results provide us with critical knowledge that improves our understanding of health outcomes post-fracture. Introduction: Mortality is higher in men than that in women following a fragility fracture, but it is unclear whether recovery of patient-reported outcomes such as health-related quality of life (HRQoL) differs between sexes. This study aimed to identify sex differences in HRQoL recovery 12 months post-fracture. Methods: Data were from the Australian arm of the International Costs and Utilities Related to Osteoporotic Fractures Study (AusICUROS). Participants recruited to AusICUROS were adults aged ≥50 years who sustained a fragility fracture. HRQoL was measured using the EQ-5D-3L at three time-points post-fracture: within 2 weeks (including pre-fracture recall) and at 4 and 12 months. Multivariate logistic regression analyses were undertaken, adjusting for confounders including age, education, income, and healthcare utilization post-fracture. Results: Overall, 695 AusICUROS participants (536 women, 77.1%) were eligible for analysis with fractures at the hip (n = 150), distal forearm (n = 261), vertebrae (n = 61), humerus (n = 52), and other skeletal sites (n = 171). At the time of fracture, men were younger, reported a higher income, and were more likely to be employed, compared with women. For all fracture sites combined, there were no differences between men and women in recovery to pre-fracture HRQoL at 12-month follow-up (adjusted OR = 1.09; 95% CI: 0.75–1.61). When stratified by fracture site, no significant sex differences were seen for hip (OR = 1.02; 95% CI: 0.42–2.52), distal forearm (OR = 1.60; 95% CI: 0.68–3.78), vertebral (OR = 2.28; 95% CI: 0.61–8.48), humeral (OR = 1.62; 95% CI: 0.16–9.99), and other fractures (OR = 1.00; 95% CI: 0.44–2.26). Although visual arts pedagogies are considered central within early childhood education programs, teacher self-efficacy has a direct impact on the quality and delivery of visual arts curricula. Until recently, the visual arts self-efficacy, pedagogical knowledge, and practice of in-service early childhood teachers have remained largely unexplored. The authors of this paper present a qualitative, iterative re-analysis of their three PhD studies which broadly focussed on early childhood visual arts praxis and specifically examined the visual arts beliefs and pedagogy of early childhood teachers in Australia and New Zealand. A thematic analysis of the intersecting self-efficacy findings raised in the three studies identifies the powerful influence of self-efficacy on teaching practice in the visual arts domain and offers new understandings about visual arts self-efficacy amongst early childhood teachers. The combined findings reveal several factors that restrain or enhance teachers' visual arts self-efficacy across time, including the impact of childhood experiences, pre-service training and epistemological beliefs. The paper also identifies several enabling conditions that appear to support teachers to develop and maintain positive visual arts self-efficacy beliefs, including practical engagement with materials, sustained professional learning, relational trust and intentional leadership. These enabling conditions offer practical strategies and research recommendations in service of positive visual arts self-efficacy to enhance quality visual arts teaching in early childhood contexts. Commensal relationships between wild plants and their dispersers play a key ecological and evolutionary role in community structure and function. While non-human dispersers are often considered critical to plant recruitment, human dispersers have received much less attention, especially when it comes to non-domesticated plants. Australia, as a continent historically characterized by economies reliant on non-domesticated plants, is thus a key system for exploring the ecological role of people as seed dispersers in the absence of agriculture. Here, we utilize a controlled observation research design, employing ecological surveys and ethnographic observations to examine how seed dispersal and landscape burning by Martu Aboriginal people affects the distribution of three preferred plants and one (edible, but non-preferred) control species. Using an information theoretic approach, we find that the three preferred plants show evidence of human dispersal, with the strongest evidence supporting anthropogenic dispersal for the wild bush tomato, Solanum diversiflorum. Human dispersal of wild plants has received limited attention. Despite efforts to foster a more equitable gender representation, Australia's higher education sector and workforce continue to be highly segregated. This article focusses on the gendered experiences of first-in-family (FIF) students—many who are from low-socioeconomic communities—transitioning to Australian universities. In terms of the gendered nature of widening participation, we know students who are FIF will often study disciplines which align with traditional gender norms. Drawing on the First-in-Family Project (n = 48), we present the analysis of our findings in two parts. First, we provide an overview of the cohort where we analyse the gendering of degree choice of FIF students. Second, underpinned by theoretical work focussed on student gendered and classed subjectivities, we address how FIF young people negotiated classed and gendered norms during the transition to university. Our data suggest that young people engage in 'gender work' and 'class work' which informs their identities and how they make choices about their futures. Scholars in critical masculinities studies argue that we need men involved and engaged in gender equity movements for gender justice to be realised. Yet we need to know more about how different groups of men are understanding gender equity and what the barriers might be. Amidst significant media interest in elite private boys' schooling and its possible (re)production of sexist cultures, this paper explores how 13 men who attended such schools in Australia between the 1970s and the 2000s understand gender justice, revealing a diversity of positions and practices across the different generational groups. We argue the men's engagements with gender justice are shaped by a broad 'curriculum of privilege' including school and non-school based experiences that mediates their lives. Artificial intelligence (AI) tools, notably ChatGPT, are increasingly recognised for their transformative potential in higher education. This study employs a detailed case study approach complemented by a survey, delving into ChatGPT's impact on pedagogical practices, student engagement, and academic performance. It involved 74 undergraduate and postgraduate students enrolled in data analytics courses in Australia. The quantitative analysis highlights ChatGPT's role in providing personalised and on-demand support, which is highly valued among users for its flexibility and responsiveness, meeting a critical demand in educational settings. Notably, the study identifies a medium effect size ( η 2 = 0.173 ) in perceived benefits, indicating that ChatGPT accounts for approximately 17.3% of the variance in improved academic outcomes. However, challenges such as ChatGPT's limited understanding of complex queries and the lack of human interactions are primary concerns, with a medium effect size ( η 2 = 0.289 ) suggesting significant areas for improvement. Furthermore, statistical analyses reveal a clear relationship between the frequency of ChatGPT usage and the perception of its benefits, underscoring the transformative potential for users who have integrated it into their academic practices. Despite these challenges, the differential impact on users versus non-users highlights the potential for ChatGPT to foster more engaging and effective educational practices. The findings advocate for targeted strategies to epitomise ChatGPT's integration into educational settings, emphasising the need for ongoing research and the development of comprehensive guidelines to navigate its complexities and maximise its educational benefits. Research on gender issues and mathematics education is often conducted in classroom settings and/or with teachers, students, and parents. However, perspectives about mathematics from adults beyond teachers and parents can have an impact on students. Thus, we conducted research in Australia and Canada about the general public's views of gender and mathematics. Participants (n = 405) were surveyed using a questionnaire in which all questions were worded in a non-binary manner. In this article, we focus on participants' views about gender and mathematics ability, and the importance of studying mathematics by gender. We report both overall trends and trends by demographic group (country, gender, age, and education level). The majority of participants indicated that there was no relationship between gender and mathematics ability or between gender and the importance of studying mathematics. Participants with gendered views typically felt that boys/men/males are better at mathematics and that it was more important for girls/women/females to study mathematics. Although the findings were generally encouraging, the existence of sexist, stereotyped views highlights the need for additional work to probe people's views of gender and mathematics. In recognition memory, the variance of the target distribution is almost universally found to be greater than that of the lure distribution. However, these estimates commonly come from long-term memory paradigms where words are used as stimuli. Two exceptions to this rule have found evidence for greater lure variability: a short-term memory task (Yotsumoto et al., Memory & Cognition,36, 282–294 2008) and in an eyewitness memory paradigm (Wixted et al., Cognitive Psychology,105, 81–114 2018). In the present work, we conducted a series of recognition memory experiments using different stimulus (faces vs. words) along with different paradigms (long-term vs. short-term paradigms) to evaluate whether either of these conditions would result in greater variability in lure items. Greater target variability was observed across stimulus types and memory paradigms. The introduction of statistical concepts into school curricula in Australia and New Zealand in the early 1990s initiated an ongoing research program into the learning and teaching of statistics and probability in both countries. This paper reviews the contribution of Australian and New Zealand researchers to building statistical literacy at school, alongside international developments. From recognising how students develop understanding of specific statistical and probabilistic concepts, through teacher knowledge and beliefs for teaching statistics, to intervention studies and targeted teaching, the field of statistics education has grown and changed. Statistics and probability are now well established as part of the mathematics curriculum. The importance of linking statistical literacy and statistical understanding across the curriculum, as well as in STEM, has also begun to receive attention as other subjects have recognised the importance of data in their fields. Following a comprehensive review of the field in Australia and New Zealand, this paper then considers emerging areas of interest, such as new approaches to data visualisation, and suggests future research. The teaching and learning of statistical thinking begins at a young age in Australia, with a focus on data representation and interpretation from Foundation Year (age 5), and the collection, sorting and categorising of items from the natural environment starting even earlier. The intangible concept of data, as part of statistical literacy, can be complex for children to grasp, especially when applying the notion of data to the everyday world or when data are explored in isolation to an investigation process. Authentic data modelling experiences present meaningful opportunities to apply statistical thinking although expert STEM knowledge is not always accessible to primary classroom teachers, nor is it always obvious how to implement such authentic problems within a classroom context. In this exploratory case study, we present data from a Year 4 classroom (age 9) statistical investigation addressing, 'How big is a leaf?' linking data to the real-life STEM context they represented. The authors were interested in how the teacher's communication processes supported her students' emerging understandings about data. Wit's (2018) cognitive tuning framework offered a way to capture how the communication processes in a group build to a commonly shared frame of reference. Findings revealed a pattern of communication between the teacher and students, supporting students' changing conceptions of data and related statistical thinking processes, throughout the investigation. The reciprocal relationship between psychiatric and substance use disorders is well-known, yet it remains largely unknown whether mental health morbidity causally leads to addictive behaviours. This paper utilises a fixed effects instrumental variables model, which is identified by time-varying sources of plausibly exogenous variations in mental health, and a nationally representative panel dataset from Australia to present robust evidence on the causal impact of mental distress on cigarette smoking and alcohol drinking behaviours. We find that mental distress significantly increases the prevalence and intensity of either cigarette or alcohol consumption. Further analysis reveals that mental distress also substantially increases household monetary expenditures on either tobacco or alcohol. The impact is greater for lower educated individuals or children of smokers, and is slightly higher for males. Our findings highlight the importance of mental health screening and treatment programs, especially among lower educated individuals or children of smokers, to assist in the prevention of addictive activities. The transformation of cities into smarter and more efficient environments relies on proactive and timely detection and maintenance of city-wide infrastructure, including roadside infrastructure such as road signs and the cleaning of illegally dumped rubbish. Currently, these maintenance tasks rely predominantly on citizen reports or on-site checks by council staff. However, this approach has been shown to be time-consuming and highly costly, resulting in significant delays that negatively impact communities. This paper presents AIoT-CitySense, an AI and IoT-driven city-scale sensing framework, developed and piloted in collaboration with a local government in Australia. AIoT-CitySense has been designed to address the unique requirements of roadside infrastructure maintenance within the local government municipality. A tailored solution of AIoT-CitySense has been deployed on existing waste service trucks that cover a road network of approximately 100 kms in the municipality. Our analysis shows that proactive detection for roadside infrastructure maintenance using our solution reached an impressive 85%, surpassing the timeframes associated with manual reporting processes. AIoT-CitySense can potentially transform various domains, such as efficient detection of potholes and precise line marking for pedestrians. The COVID-19 virus was unveiled to the world as a health crisis and later also as an economic crisis. For some organisations, it has become an ethical crisis. This is certainly the case for large organisations in Australia, where the way many enterprises handled a government wage subsidy called JobKeeper led to a public backlash, media pressure, and a variety of responses ranging from 'We acted legally' to the full return of the subsidy. Some organisations later reported profits, and the public response indicated concern about this behaviour, many considering it immoral despite it being legally compliant. It is, we contend, a question to which stakeholder theory can be applied, examining how organisations view and respond to the public. We use content analysis of mainstream media to provide information about public reactions and information from official sources to confirm corporate action. We show that there is a significant ethical component in the public response to the behaviour of organisations as they respond to the crisis. COVID has been an ethical, health, and financial crisis for these organisations. Public pressure, exerted in and through the media, made the general public a definite stakeholder. Understanding the drivers of student dropout from higher education has been a policy concern for several decades. However, the contributing role of certain factors—including student mental health—remains poorly understood. Furthermore, existing studies linking student mental health and university dropout are limited in both methodology and scope—for example, they often rely on small and/or non-representative samples or subjective measures, and focus almost exclusively on main effects. This paper overcomes many of these shortcomings by leveraging unique linked administrative data on the full population of domestic students commencing undergraduate studies at Australian universities between 2012 and 2015 (n = 652,139). Using these data, we document that approximately 15% of students drop out of university within their first academic year. Critically, students receiving treatment for mental health problems are 4.3 (adjusted) to 8.3 (unadjusted) percentage points more likely to drop out of higher education. This association remains in the presence of an encompassing set of potential confounds, and is remarkably uniform across segments of the student population determined by individual, family, and programme characteristics. Rockburst can cause significant damage to infrastructure and equipment, and pose a substantial risk to the safety of mine workers. Effective prediction of short-term rockburst damage can greatly assist in the implementation of preventive measures and risk mitigation strategies. Therefore, the development of accurate and reliable models for rockburst damage prediction is of great importance. This study has developed a novel hybrid model, based on eXtreme Gradient Boosting (XGBoost) and Sand Cat Swarm Optimization (SCSO), for predicting the scale of short-term rockburst damage. The model, which leveraged a data set of 254 rockburst damage cases collected from rockburst event reports across multiple mines in Australia and Canada, was comprehensively evaluated via non-parametric statistical tests and compared with other prevalent models. The results demonstrate that the SCSO–XGBoost model achieved a test accuracy of 88.46% for the rockburst damage scale, presenting the best comprehensive performance among the models in this study. Furthermore, the SCSO showed significant advantages in reducing overfitting, greatly enhancing the generalization capability of the XGBoost model. Lastly, various model explanation methods were employed to analyze the contribution of input variables. It was found that Peak Particle Velocity (PPV), Geological Structure (GS), Ground Support System Capacity (GSSC), and Stress Condition Factor (SCF) provide the most significant contributions to predicting rockburst damage. This study explains the decision-making rationale behind the SCSO–XGBoost model's identification of different damage scales from both a holistic and sample perspective. In addition, a graphical user interface has been developed and demonstrated for testing purposes. In conclusion, as a study integrating a new SCSO metaheuristic method with the XGBoost ensemble model for rockburst damage identification, the established SCSO–XGBoost model effectively predicts the types of short-term rockburst damage, offering superior predictive performance and explainability compared to previous works. Highlights: This study presents a novel hybrid ensemble model for short-term rockburst damage prediction, combining a new metaheuristic method with the XGBoost model. It provides a comprehensive and detailed discussion on the optimization process and performance evaluation of the hybrid model. Floods are among the most destructive extreme events that exist, being the main cause of people affected by natural disasters. In the near future, estimated flood intensity and frequency are projected to increase. In this context, automatic and accurate satellite-derived flood maps are key for fast emergency response and damage assessment. However, current approaches for operational flood mapping present limitations due to cloud coverage on acquired satellite images, the accuracy of flood detection, and the generalization of methods across different geographies. In this work, a machine learning framework for operational flood mapping from optical satellite images addressing these problems is presented. It is based on a clouds-aware segmentation model trained in an extended version of the WorldFloods dataset. The model produces accurate and fast water segmentation masks even in areas covered by semitransparent clouds, increasing the coverage for emergency response scenarios. The proposed approach can be applied to both Sentinel-2 and Landsat 8/9 data, which enables a much higher revisit of the damaged region, also key for operational purposes. Detection accuracy and generalization of proposed model is carefully evaluated in a novel global dataset composed of manually labeled flood maps. We provide evidence of better performance than current operational methods based on thresholding spectral indices. Moreover, we demonstrate the applicability of our pipeline to map recent large flood events that occurred in Pakistan, between June and September 2022, and in Australia, between February and April 2022. Finally, the high-resolution (10-30m) flood extent maps are intersected with other high-resolution layers of cropland, building delineations, and population density. Using this workflow, we estimated that approximately 10 million people were affected and 700k buildings and 25,000 km 2 of cropland were flooded in 2022 Pakistan floods. [ABSTRACT FROM AUTHOR]

Background: The COVID-19 a pandemic changed the world. Public health directives to socially distance with stay-at-home orders altered injury risk factor exposure, resulting injury patterns and conducting injury prevention (IP). The objective of this study was to determine the impact the COVID-19 pandemic on injury and IP at North American trauma centers (TC). Results: Sixty-two responses were received from pediatric (44%), adult (11%), and combined (31%) TC, from 22 American states, 5 Canadian provinces and Australia. The majority (91%) of programs targeted age groups from birth to 15 years old. Nearly one-third reported IP to be less of an institutional priority with funding redistributed in 15% of centers [median (IQR) − 25% (− 43, 1)], and resultant staffing changes at 38% of centers. A decrease in IP efforts was reported at 64% of TC. Overall, the majority of respondents reviewed injury data, with the top reported increased mechanisms mainly intentional: Firearm-related (75%), assaults (72%), and abuse (71%). Leading increased unintentional injuries were injuries occurring in the home such as falls (70%), followed by ATV (62%), and cycling (57%). Sites pivoted by presenting (74%) or participating (73%) in IP education virtually, social media posts (61%) and the addition of technology (29%). Top barriers were redeployment of partners (45%) and staff (31%), as well as lack of technology (40%) in the target population. Facilitators were technology at TC (74%), support of trauma program (63%), and having IP funding maintained (55%). Conclusions: Nearly two-thirds of TC decreased IP efforts during the pandemic due to staffing and funding reductions. The leading reported increased injuries were intentional, indicating that violence prevention is needed, along with support for mental health. Heavy rainfall and precipitation play a massive role in shaping the socio-agricultural landscape of a country. Being one of the key indicators of climate change, natural disasters, and of the general topology of a region, rainfall prediction is a gift of estimation that can be used for multiple beneficial causes. Machine learning has an impressive repertoire in aiding prediction and estimation of rainfall. This paper aims to find the effect of ensemble learning, a subset of machine learning, on a rainfall prediction dataset, to increase the predictability of the models used. The classification models used in this paper were tested once individually, and then with applied ensemble techniques like bagging and boosting, on a rainfall dataset based in Australia. The objective of this paper is to demonstrate a reduction in bias and variance via ensemble learning techniques while also analyzing the increase or decrease in the aforementioned metrics. The study shows an overall reduction in bias by an average of 6% using boosting, and an average reduction in variance by 13.6%. Model performance was observed to become more generalized by lowering the false negative rate by an average of more than 20%. We explore the relationship between heterosexual partners' relative income and the incidence of both domestic violence and emotional abuse. Using Australian data drawn from society-wide surveys, we find women who earn more than their male partners are subject to a 33% increase in partner violence and a 20% increase in emotional abuse compared to mean levels. We show the relationship between relative spouse income and female partner abuse is best modelled by a binary variable that captures "female breadwinning." This finding differs from those of some earlier studies that considered only serious abuse and found a continuous negative relationship between female partners' relative income and abuse. Instead, our findings suggest a mechanism related to gender norms generating domestic violence. We find no link between relative income and abuse of male partners. [ABSTRACT FROM AUTHOR]

Background: In Australia, prescription melatonin became a 'Pharmacist Only Medicine' for people over 55 with insomnia from June 2021. However, little is known about pharmacists' views on melatonin down-scheduling and perceived impacts on practice. Aim: To explore Australian community pharmacists' views on and attitudes towards the down-scheduling of melatonin. Method: A convenience sample of community pharmacists and pharmacy interns were recruited. Participants completed a survey capturing demographic and professional practice details, and rated their knowledge, beliefs and attitudes towards melatonin. This was followed by an online semi-structured interview. Interviews were guided by a schedule of questions developed using the Theoretical Domains Framework and explored the perceived role of melatonin, preparation/response to down-scheduling, practice changes and patient interactions. Interviews continued until data saturation and were digitally recorded, transcribed verbatim and analysed using the Framework Approach. Results: Twenty-four interviews were conducted with community pharmacists (n = 19) and intern pharmacists (n = 5), all practicing in metropolitan areas. Pharmacists/intern pharmacists welcomed the increased accessibility of melatonin for patients. However, pharmacists perceived a disconnect between the guidelines, supply protocols and pack sizes with practice, making it difficult to monitor patient use of melatonin. The miscommunication of eligibility also contributed to patient-pharmacist tension when supply was denied. Importantly, most participants indicated their interest in upskilling their knowledge in melatonin use in sleep, specifically formulation differences and dosage titration. Conclusion: While pharmacists welcomed the down-scheduling of melatonin, several challenges were noted, contributing to pharmacist-patient tensions in practice. Findings highlight the need to refine and unify melatonin supply protocols and amend pack sizes to reflect guideline recommendations as well as better educating the public about the risk-benefits of melatonin. Worldwide, women are over-represented in precarious and insecure employment arrangements. Importantly, the high unpaid labour demands women experience over the life course compromise paid labour force participation for women. This study explores the way different trajectories of time spent in unpaid labour throughout women's prime working and child-rearing years (from baseline age of 25–35 yrs to 42–52 yrs) are associated with indicators of precarious employment and labour force detachment later in life. We applied group-based trajectory modelling to 17 waves (2002–2018) of data from the Household Income and Labour Dynamics in Australia survey to identify trajectories in unpaid labour. We then examined associations between these estimated trajectories and employment outcomes in wave 19 (2019). Our study shows that chronic exposure to high amounts of unpaid labour over prime working-age years (compared to lower exposure levels) increases women's probability of precarious employment and labour force detachment later in prime working life. This provides evidence that ongoing inequity in the division of unpaid labour has considerable long-term implications for gender inequality in the paid labour force, and underscores the importance of urgently addressing how men and women share and prioritise time across both paid and unpaid labour domains. Aim: This study compares the pattern of physical activity and sleep between shift and non-shift workers using a novel physical activity–sleep index. By drawing from a diverse occupational population, this research aims to reduce any occupational specific biases which are prevalent in shift-work research. Subject and methods: Current data included 7607 workers (shift workers n = 832) from the Household Income and Labour Dynamics of Australia cohort study. The combined physical activity–sleep index comprised three physical activity components and three sleep health components: achieving moderate (1pt) or high (2pts) IPAQ classification; accruing ≥30% of physical activity as vigorous intensity (1pt); meeting sleep duration recommendations on a work night (1pt); and non-work night (1pt); and reporting no insomnia symptoms (1pt) (higher score = healthy behaviour, max. 6). Generalised linear modelling was used to compare behaviours of shift and non-shift workers. Results: Findings showed shift workers reported significantly lower activity–sleep scores (3.59 vs 3.73, p < 0.001), lower sleep behaviour sub-score (2.01 vs. 2.22, p < 0.001) and were more likely to report insomnia symptoms (p < 0.001) compared to non-shift workers. No difference was reported for overall physical activity (shift = 1.58 vs. non-shift = 1.51, p = 0.383). Conclusion: When viewed in conjunction using the combined activity–sleep index, shift workers displayed significantly poorer combined behaviours when compared to non-shift workers. Summary: We estimated the short-term impact of fragility fractures on community-dwelling women in five countries. Women with fragility fractures reported significantly more difficulties performing activities of daily living and significantly higher levels of lost productivity and caregiver support than those without fractures; results highlight the multi-country indirect burden of fragility fractures. Introduction: To estimate the impact of fragility fractures on activities of daily living (ADL), productivity loss and caregiver support in women with a recent fragility fracture. Methods: This multi-centre cross-sectional study enrolled community-dwelling women aged ≥ 50 years in South Korea, Spain, Germany, Australia and the United States. The fragility fracture cohort consisted of women with an index fragility fracture in the past 12 months; the fracture free cohort consisted of women with no fracture in the 18 months prior to study enrolment. Study participants completed three validated questionnaires: Lawton Instrumental ADL (IADL), Physical Self-Maintenance Scale (PSMS) and iMTA Productivity Cost Questionnaire (iPCQ). Results: In total, 1,253 participants from 41 sites across the five countries were included. Compared with the fracture free cohorts, fragility fracture cohorts had significantly lower function and were more dependent on support (p < 0.05 in all countries for Lawton IADL, and in South Korea, Spain, Australia and the United States for PSMS), significantly higher hours of paid absenteeism (p < 0.05, Spain, Germany, Australia), significantly higher unpaid lost productivity (p < 0.05, South Korea, Spain, Germany), significantly more days of paid help received in the home (p < 0.05 South Korea, Spain and the United States), and significantly more days of unpaid help from family members or friends (p < 0.05, all countries). Conclusion: In this multi-national study, fragility fractures in community-dwelling ≥ 50 years women were associated with several outcomes indicating higher indirect burden and lower quality of life, including more difficulties performing ADL and higher levels of lost productivity and caregiver support. Epidemiological evidence suggests that regular physical activity (PA) positively impacts individuals' mental health (MH). The PA-MH relationship may be critical among immigrants owing to psycho-social-cultural influences. This scoping review of 61 studies employed a holistic bio-psycho-socio-cultural framework to thoroughly investigate the complex relationship between PA (across life domains) and immigrants' MH in Western countries. A systematic search of five electronic databases (Medline, PubMed, Embase, PsycINFO and Anthropology Plus) was conducted to locate relevant articles. No limitations were applied to study design, age, gender, home country, MH condition or PA type. A bio-psycho-socio-cultural-informed conceptual model guided the analysis of the multi-domain PA-MH relationship. Immigrant PA-MH studies were conducted and reported most commonly in the USA (38%), Australia (18%), and Canada (11%). Overall, PA was positively related to MH. Each domain-specific PA appeared to be associated with unique MH-promoting pathways/mechanisms. Leisure-related PA may support MH by enhancing self-agency and minimizing risky behaviors, whilst travel- and domestic-related PA may promote self-accomplishment and physical engagement. Ethnic sports appeared to enhance resilience. Occupational-related PA was associated with either positive or negative MH, depending on the type of occupation. A bio-psycho-socio-cultural-informed model is required to gain an encompassing and integrated understanding of immigrants' health. The first iteration of such a model is presented here, along with an illustration of how the model may be used to deepen analysis and understanding of the multi-domain PA-MH relationship among immigrants and inform public health planners and practitioners. Purpose: Corporal punishment (CP) and physical abuse (PA) in childhood are associated with increased risk of child-to-parent violence (CPV). Without context of discipline (i.e., the intention of behavior change, and use of reasonable force), both CP and PA represent the use of physical force against a child. It is still unclear if their associations with CPV are similar when they co-occur, or when they occur in isolation. The current study examined the differential and cumulative association of different types of physical force in childhood with rates of CPV. Method: The sample consisted of 1,132 participants, between 18 to 87-years-old (M = 50.95, SD = 14.24) and included 59.5% female and 39.2% male participants who completed an online survey measuring CP, PA and CPV. Participants formed three groups: low CP or PA (group low), high CP only (group HCP), or high PA and CP (group PA + CP). Results: Two one-way ANOVAs with planned contrasts were conducted separately for CPV against mothers and fathers. The group HCP reported significantly higher CPV against both the mother and the father than group low and there was no significant difference between group HCP and group PA + CP. Conclusions: Higher rates of CP are associated with higher rates of CPV; however, this rate does not increase further when there is concurrent PA. This suggests that there may be a low sensitivity for retaliation from a young person, or coercive training (through high parent–child conflict), in environments where there is physical force from a parent. Purpose: Idiopathic pulmonary fibrosis (IPF) is a type of interstitial lung disease found mostly in elderly persons, characterized by a high symptom burden and frequent encounters with health services. This study aimed to quantify the economic burden of IPF in Australia with a focus on resource utilization and associated direct costs. Methods: Participants were recruited from the Australian IPF Registry (AIPFR) between August 2018 and December 2019. Data on resource utilization and costs were collected via cost diaries and linked administrative data. Clinical data were collected from the AIPFR. A "bottom up" costing methodology was utilized, and the costing was performed from a partial societal perspective focusing primarily on direct medical and non-medical costs. Costs were standardized to 2021 Australian dollars ($). Results: The average annual total direct costs per person with IPF was $31,655 (95% confidence interval (95% CI): $27,723–$35,757). Extrapolating costs based on prevalence estimates, the total annual costs in Australia are projected to be $299 million (95% CI: $262 million–$338 million). Costs were mainly driven by antifibrotic medication, hospital admissions and medications for comorbidities. Disease severity, comorbidities and antifibrotic medication all had varying impacts on resource utilization and costs. Conclusion: This cost-of-illness study provides the first comprehensive assessment of IPF-related direct costs in Australia, identifies the key cost drivers and provides a framework for future health economic analyses. Additionally, it provided insight into the major cost drivers which include antifibrotic medication, hospital admissions and medications related to comorbidities. Purpose: Australia has a school-based human papillomavirus (HPV) vaccination scheme that was implemented for girls in 2007 and for boys in 2013. HPV vaccination status is important for many studies into HPV infection. Here we wanted to estimate the validity of self-reported HPV vaccination status by comparing self-report to data from the national vaccination register. Methods: Australian residents aged 18–70 years were recruited for the Oral Diversity Study from October 2020 to November 2021. Participants were asked to provide consent for record linkage to the Australian Immunisation Register (AIR). They were also asked to fill out a questionnaire about HPV vaccination, lifestyle, and sexual behavior. Results: 1,023 participants were recruited, permission was received from 911 participants for HPV vaccination record linkage, and 850 self-reported vaccination and were part of the validity analysis. Of those 233 (26%) were confirmed to be HPV vaccinated. Ninety-one percent of the vaccinated were females (n = 212), 19 males and two non-binary. The highest HPV vaccine uptake was seen in the youngest age group (18–29 years; 80%), followed by 66% in 30–39 year olds, 2% in 40–49 year olds and then dropped significantly to 0.7% for people 50–70 years old. The sensitivity of self-report was 99.0%, and the specificity 94.5%, and the positive predictive value was 85.7% and the negative predictive value 99.7%. The introduction of HIV pre-exposure prophylaxis (PrEP) has the potential to impact the attitudes gay and bisexual men (GBM) who consequently choose to take PrEP have towards treatment as prevention (TasP), and the extent to which they are willing to have condomless anal intercourse (CLAI) with an HIV-positive sexual partner who has an undetectable viral load (UVL). Using a cross-sectional sample from an observational cohort study conducted from August 2018 to March 2020, we examined the extent to which PrEP-experienced GBM are willing to have CLAI with a partner who has a UVL. Simple and multiple logistic regression models were used to identify associated variables. Of the 1386 participants included in the analyses, 79.0% believed in the effectiveness of TasP, and 55.3% were willing to have CLAI with a partner who has a UVL. Wiling participants were less worried about getting HIV when taking PrEP and more likely to believe in TasP. Further research is needed to better understand the gap between belief in TasP and willingness to have CLAI with a partner who has a UVL among PrEP-experienced GBM. [ABSTRACT FROM AUTHOR]

Background: Globally, drowning is a leading cause of unintentional injury and death among children. Teaching aquatic competencies (swimming skills and water safety knowledge) to children has been proposed as a prevention strategy. In Australia, however, many children are not meeting standard aquatic competency benchmarks. Exploration of the connection between demographic and background factors and aquatic competencies could provide insight into why differences in acquisition of aquatic knowledge and skills occur. Main body: A systematic literature review guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses was performed to identify studies that reported on the association between demographic and background factors and aquatic competencies. Nine databases were searched for English language peer-reviewed studies published since 2000. Fourteen studies fulfilled all inclusion criteria. Studies were quasi-experimental or cross-sectional in design, which is considered quality level III-2 or IV, respectively, on the National Health and Medical Research Council Evidence Hierarchy. Study quality was moderate, and risk of bias was high. While aquatic competencies can be taught, this review found that factors including age, gender, geographic residence, medical conditions/disabilities, socioeconomic status, and swimming frequency were significantly associated with the demonstration and/or acquisition of aquatic competencies. Conclusion: This review provides insight into demographic and background factors that are significantly associated with the development of aquatic competence. Whilst further investigation is required to increase the evidence base, these findings may assist in tailoring swimming and water safety programs to accommodate those at-risk of not achieving age-appropriate aquatic competencies. A range of studies have found that immigrants generally start out using different travel modes but over time they 'assimilate' toward adopting similar travel modes to the general population. These studies tend to focus on 'when' and 'if' travel assimilation occurs, with some studies using socioeconomic factors to explain 'why' this occurs. But few studies have explored the role of culture, attitudes and other 'soft' factors in shaping the process of travel assimilation among immigrants. In Australia, South Asians have been the largest and fastest growing immigrant group, and as skilled migrants they face few 'hard' barriers to car use. The aim of this paper is to explore the interaction between cultural influences, attitudes and initial travel experiences upon arrival in Australia on long-term travel assimilation amongst South Asian immigrants. Qualitative interviews with 20 South Asian immigrants were used to identify a range of cultural and psychosocial factors, such as perceptions towards travel modes and gender-based cultural norms. Attitudes and behaviours evolve during their early years in Australia, beginning with a 'honeymoon period' – a phase where all travel modes are seen as positive – before car use begins to dominate. The findings have implications for how we understand the interactions between attitudes, cultural practice and travel behaviour and how they evolve over time. They also imply that policymakers have only a narrow window of time to encourage sustainable transport among South Asian immigrants before the travel 'honeymoon period' wears off. Background: Within the quality use of medicines (QUM)—which entails timely access to, and the rational use of, medicines—medicine safety is a global health priority. In multicultural countries, such as Australia, national medicines policies are focused on achieving QUM, although this is more challenging among their Culturally and Linguistically Diverse (CALD) patients (i.e., those from ethnic minority groups). Aim: This review aimed to identify and explore the specific challenges to achieving QUM, as experienced by CALD patients living in Australia. Method: A systematic literature search was conducted using Web of Science, Scopus, Academic search complete, CINHAL, PubMed and Medline. Qualitative studies describing any aspects of QUM among CALD patients in Australia were included. Results: Major challenges in facilitating QUM among CALD patients in Australia were identified, particularly in relation to the following medicines management pathway steps: difficulties around participation in treatment decision-making alongside deficiencies in information provision about medicines. Furthermore, medication non-adherence was commonly observed and reported. When mapped against the bio-psycho-socio-systems model, the main contributors to the medicine management challenges identified related to "social" and "system" factors, reflecting the current health-system's lack of capacity and resourcing to respond to patients' low health literacy levels, communication and language barriers, and cultural and religious perceptions about medicines. Conclusion: QUM challenges were different among different ethnic groups. This review suggests a need to engage with CALD patients in co-designing culturally appropriate resources and/or interventions to enable the health-system to address the identified barriers to QUM. Starting from fresh day-to-day real-time big data, the study aims to develop a new data analytics model, adopting the design science research methodology, which can provide invaluable options and techniques to make prediction easier from immediate past datasets. This study introduces an innovative design artifact as a big data solution for hospitality managers to utilize analytics for predictive strategic decision-making in post-COVID situation. The artifact can also be generalized for other sectors with tailoring aspects which are subject to further studies. The proposed artifact is then compared with other design artifacts related to big data solutions where it outperforms them in terms of comprehensiveness. Background: Patient-reported outcome measures (PROMs) provide a measure of self-perceived health status or health-related quality of life. They have been used to support provider-patient decisions, healthcare delivery, and value-based healthcare models. A barrier to routine collection of PROMs is the perception that PROMs lack clinical utility. As such, it is important to establish clinicians' acceptability of the PROM prior to large-scale implementation. This study explored the acceptability of the routine use and collection of a generic PROM in healthcare services from the perspective of healthcare staff. Methods: Individual semi-structured interviews were completed from September 2020 to March 2021 with 26 staff from two multi-disciplinary community healthcare services in Melbourne, Australia. Interviews explored their experiences of using the EQ-5D-5L in their routine care. Interviews were recorded and transcribed verbatim. Data were analysed according to a framework approach, using inductive and deductive techniques. Results: Participants discussed the acceptability of the EQ-5D-5L with reference to four themes: practicalities of use; holistic nature; influence on client care; and influence on health service. Participants found the EQ-5D-5L quick and easy to administer, and appreciated that it measured multiple domains of health that were relevant to their clients' care. They believed the EQ-5D-5L helped to identify client problems, and inform service delivery. They also reported features that were less acceptable, including a lack of item specificity to some healthcare disciplines. Participants reported the challenge of managing conflict between their assessment of the client's health and the client's perspective of their own health, leading some to question whether the client could provide an accurate reflection of their own health. Conclusions: The EQ-5D-5L has several features that healthcare staff viewed as acceptable for routine collection and use in healthcare. Training on the validity of the patient reported perspective and broadening the scope of PROMs collection beyond the EQ-5D-5L should be considered to facilitate large-scale implementation. This study examined whether parent-reported atypical development in their child's first year was associated with age of diagnosis and age when parents first needed to consult a specialist about their child's development. It involved 423 children who participated in the Australian Autism Biobank. Most parents retrospectively identified ≥ 1 domain of atypical child development. Atypical development in most domains was associated with an earlier age when parents felt specialist consultation was needed. Atypical development in the "gaze abnormalities", "lack of response to social stimuli", and "no social communication" subdomains within the social domain was associated with an earlier age of diagnosis, as was atypical development in the "hypo/hypersensitivity" and "preoccupation with parts of objects" subdomains within the stereotyped/restricted behavior domain. Purpose: This study builds on knowledge of intimate partner violence (IPV) perpetration by examining theoretically important aspects of this crime within an Australian community sample. More specifically, the current study examines the relationship between IPV offenders' levels of self-control and offense specialization and determines whether this relationship differs by gender. Method: An online survey advertised on social media yielded an analysis sample of 155 females and 170 males. Self-control was measured using the Brief Self-Control scale and specialization was measured with the Offense Specialization Coefficient (OSC) and a multilevel item response theory measurement approach. Results: Findings indicate that IPV offenders with low levels of self-control are more versatile in their offending and that this relationship was more evident and pronounced for females. Conclusions: Findings are mostly supportive of Gottfredson and Hirschi's General Theory of Crime and highlight important differences within IPV offender populations that should be considered when designing targeted prevention and intervention methods. Background: The prevalence of anxiety and depression symptoms in semi-elite Australian footballers is unknown. The primary objective of this study was to determine the prevalence of generalised anxiety disorder (GAD) and depressive symptoms in semi-elite Australian Football players. Our secondary objective was to explore the association between demographic and football-specific factors with GAD and depressive symptoms. A cross-sectional epidemiological study including 369 semi-elite Western Australian Football League (WAFL) players from the Men and Women's 2022 season (n = 337 men, 91%) was conducted. Symptoms of depression were measured using the Patient Health Questionnaire-9 scale (PHQ-9) and symptoms of GAD with the GAD-7 scale. Results: Our response rate was 82.9%. Thirteen players had missing data. The prevalence of GAD symptoms was 8.5% in men and 28.6% in women (10% overall). The prevalence of depressive symptoms was 20% in men and 57% in women (23% overall). Being a woman (gender) was associated with a sevenfold increased risk of GAD and/or depression symptoms [odds ratio (OR): 7.33, 95% confidence interval (CI): 3.18–16.92; p < 0.001]. Players of 'Aboriginal or Torres Strait Islander' ethnicity were two times more likely to report GAD and/or depression symptoms in comparison to players of 'Australian' ethnicity (OR: 2.13; 95% CI: 1.01–4.49; p = 0.048). Concussion history was not a significant risk factor for GAD or depression symptoms. Conclusion: This study demonstrated that approximately 1 in 10 WAFL players met the diagnostic cut-off criteria for probable GAD, and 1 in 5 for probable depression. The prevalence for depression symptoms in this study were far higher than the national average in the comparative age bracket. WAFL women's players also reported a substantially higher prevalence of GAD and depressive symptoms than men, and should be further investigated as a priority by the WAFL. Introduction: COVID-19 related lockdowns have impacted the sexual activity of gay and bisexual men (GBM). We investigated trends in sexual behaviors and the COVID-19 context in which they occurred (COVID-notification rates and jurisdictional restrictions) to understand changes in the duration and severity of periods of lockdown on the sexual behavior of Australian GBM. Methods: In an online, prospective observational study of 831 GBM from May 2020 to May 2021, we investigated associations between changes in sexual behavior among Australian GBM, lockdowns, and COVID-19 notification rates through weekly surveys from May 2020 to May 2021. Results: The mean age was 45.71 years (SD: 13.93). Most identified as gay (89.0%) and 10.2% were living with HIV. There was an overall increase in the mean weekly number of non-committed relationship partners (0.53–0.90, p < 0.001). The state of Victoria experienced a significant extended COVID-19 outbreak, accompanied by severe lockdown restrictions. In response, Victorian men's partner numbers shifted three times, while elsewhere there was an overall gradually increasing trend. Conclusions: Less severe outbreaks with shorter lockdown periods, involving fewer and geographically contained, COVID-19 notifications were accompanied by non-significant changes in sex with non-relationship partners than more severe outbreaks over extended periods and larger geographical areas. Objectives: To estimate capability wellbeing lost from the general adult populations in the UK, Australia and the Netherlands in the first year of the COVID-19 pandemic and the associated social restrictions, including lockdowns. Design: Cross-sectional with recalled timepoints. Setting: Online panels in the UK, Australia and the Netherlands conducted in February 2021 (data collected 26 January–2 March 2021). Participants: Representative general adult (≥ 18 years old) population samples in the UK (n = 1,017), Australia (n = 1,011) and the Netherlands (n = 1,017) Main outcome measure: Participants completed the ICECAP-A capability wellbeing measure in February 2021, and for two recalled timepoints during the initial lockdowns in April 2020 and in February 2020 (prior to COVID-19 restrictions in all three countries). ICECAP-A scores on a 0–1 no capability–full capability scale were calculated for each timepoint. Societal willingness to pay estimates for a year of full capability (YFC) was used to place a monetary value associated with change in capability per person and per country. Paired t tests were used to compare changes in ICECAP-A and YFC from pre- to post-COVID-19-related restrictions in each country. Results: Mean (standard deviation) loss of capability wellbeing during the initial lockdown was 0.100 (0.17) in the UK, 0.074 (0.17) in Australia and 0.049 (0.12) in the Netherlands. In February 2021, losses compared to pre-lockdown were 0.043 (0.14) in the UK, 0.022 (0.13) in Australia and 0.006 (0.11) in the Netherlands. In monetary terms, these losses were equivalent to £14.8 billion, AUD$8.6 billion and €2.1 billion lost per month in April 2020 and £6.4 billion, A$2.6 billion and €260 million per month in February 2021 for the UK, Australia and the Netherlands, respectively. Conclusions: There were substantial losses in capability wellbeing in the first year of the COVID-19 pandemic. Introduction: Substantial difference in mortality following severe traumatic brain injury (TBI) across international trauma centers has previously been demonstrated. This could be partly attributed to variability in the severity coding of the injuries. This study evaluated the inter-rater and intra-rater reliability of Abbreviated Injury Scale (AIS) scores of patients with severe TBI across three international level I trauma centers. Methods: A total 150 patients (50 per center) were randomly selected from each respective trauma registry: University Medical Center Utrecht (UMCU), the Netherlands; John Hunter Hospital (JHH), Australia; and Harborview Medical Center (HMC), the United States. Reliability between coders and trauma centers was measured with the intraclass correlation coefficient (ICC). Results: The reliability between the coders and the original trauma registry scores was 0.50, 0.50, and 0.41 in, respectively, UMCU, JHH, and HMC. The AIS coders at UMCU scored the most AIS codes of ≥ 4. Reliability within the trauma centers was substantial in UMCU (ICC = 0.62) and HMC (ICC = 0.78) and almost perfect in JHH (ICC = 0.85). Reliability between trauma centers was 0.70 between UMCU and JHH, 0.70 between JHH and HMC, and 0.59 between UMCU and HMC. Conclusion: The results of this study demonstrated a substantial and almost perfect reliability of the AIS coders within the same trauma center, but variability across trauma centers. This indicates a need to improve inter-rater reliability in AIS coders and quality assessments of trauma registry data, specifically for patients with head injuries.
